Dr. Aaron Zelikovich discusses the utility of neurofilament light chain as a serum biomarker in peripheral neuropathy. Show citation: Karam C. Muscle Nerve. 2026;73(1):86-92. doi:10.1002/mus.70073 Show transcript: Dr. Aaron Zelikovich: Welcome to today's neurology minute. My name is Aaron Zelikovich, a neuromuscular specialist at Lenox Hill Hospital in New York City. Today, we will discuss a recent article on the utility of neurofilament light chain as a serum biomarker in peripheral neuropathy. It has been studied in other neurological diseases like ALS and multiple sclerosis, as in the 2024 study by Robert Fox et al, which highlighted the limitations of serum neurofilament light chain in patients with multiple sclerosis, since the elevation was inconsistent and tended to occur weeks after MRI changes, and was really only found to be helpful in certain clinical situations. The study we highlight today is a single-center retrospective study that highlights the opportunities and limitations of using serum neurofilament light chain as a biomarker to monitor treatment response and peripheral neuropathy. Serum neurofilament light chain has been shown as an indicator of neuronal injury in both central and peripheral nervous system disease that has been associated with axonal injury or degeneration. It is now commercially available. The authors in this study provide a real-world single-center retrospective study that looked at various forms of peripheral neuropathy over 12 months. Patients had to be evaluated and meet criteria for peripheral neuropathy with either genetic testing, nerve conduction studies, and/or clinical exams. Neuropathies included TTR amyloid, vasculitis, CMT, CIDP, GBS, and anti-MAG neuropathy. Patients with TTR amyloid who were treatment naive and had elevated serum neurofilament light chain showed a reduction in neurofilament light chain levels with treatment. Additionally, patients with CIDP who were treatment naive with elevated serum neurofilament light chain also showed a reduction in neurofilament light chain levels with treatment. All patients with idiopathic peripheral neuropathy had normal serum neurofilament light chain levels. However, serum neurofilament light chain can vary in patients based on age, if they have diabetes, renal dysfunction, and body weight. And this makes it really challenging to interpret it in an isolated setting. Serum neurofilament light chain is a new biomarker for peripheral neuropathies. It can be a supplemental tool in the appropriate clinical context. Future studies are needed to identify its potential to be used as a treatment response biomarker in neuropathies like CADP, GBS, and TTR amyloid. Thank you so much, and have a wonderful day.

Bio: David William Thomas was born May 20, 1949, in St. Catharines, Ontario. He is the eldest son of British parents, Moreen Duff Muir (May 4, 1928 – May 18, 2022), a church organist for thirty years originally from Glasgow, Scotland and composer of church music, and John E. Thomas (1926–1996), a medical ethicist from Merthyr Tydfil, Wales who was head of the Philosophy Department at McMaster University, and the author of several books. Dave's younger brother, Ian Thomas, is a Canadian singer-songwriter. The family moved temporarily to Durham, North Carolina, where his father attended Duke University and earned a PhD in philosophy. The family moved back to Dundas, Ontario, in 1961, where Dave attended Dundas District high school, and later graduated with an honours Bachelor of Arts degree in English literature from McMaster University in Hamilton, Ontario.[1] Starting his career as a copywriter at ad agency McCann Erickson in 1974, Thomas became the head writer of the Coca-Cola account in Canada within a year. After watching a Second City stage show in Toronto, and while suffering from self-described "boredom" in his advertising work, he auditioned for the Second City troupe and was chosen as a performer.[2] He was a cast member of the Toronto production of Godspell, along with Victor Garber, Martin Short, Eugene Levy, Gilda Radner, and Andrea Martin.[3] Paul Shaffer was the musical director.[3 He first achieved fame as a cast member of the Canadian TV comedy series SCTV, joining Godspell castmates Levy, Martin and later Short, plus Rick Moranis, John Candy, Harold Ramis, Catherine O'Hara and others. Notable characters on the show include Doug McKenzie of beer-swilling brothers Bob and Doug McKenzie, editorialist Bill Needle, Scottish scone-chef/bluesman Angus Crock, motor-mouthed TV ad announcer Harvey K-Tel, Lowery organist/curio pitchman Tex Boil and the "Cruising Gourmet". Thomas's first film role was in Home to Stay, directed by Delbert Mann, in which Thomas played in a scene with Hollywood legend Henry Fonda. He then wrote, co-directed, and starred in the Bob & Doug McKenzie feature film Strange Brew. Soon after, he wrote for and acted in The New Show, produced by Lorne Michaels during his hiatus from Saturday Night Live. Short-lived, this show featured a powerhouse writing staff including Thomas along with Buck Henry, George Meyer, Jack Handey, Al Franken, Tom Davis, Valri Bromfield and Steve Martin. Thomas tried his hand at network television hour-long shows in 1986 when he wrote and co-executive produced Steel Collar Man for CBS. The pilot was produced but did not go to series. He co-wrote Spies Like Us (1985) with Dan Aykroyd. In 1988, Thomas wrote another hour long show for CBS, B Men, which was back ordered, but Thomas took a directing job at Paramount, which caused the network to drop the series. He reportedly introduced John Travolta and Kelly Preston while directing them in the Paramount film The Experts. He wrote for, produced, and starred in The Dave Thomas Comedy Show (1990). In 1991, he starred in the Showtime comedy, Public Enemy #2. In 1992, he tried his hand at reality TV and co-executive produced ABC's America's Funniest People with Vin Di Bona, but left after thirteen weeks to appear in the film Coneheads. In 1993, he co-starred in ABC's Grace Under Fire with Brett Butler and Tom Poston and continued with the show for 5 seasons. In 1995 Thomas starred in the ABC television film Picture Perfect with Mary Page Keller and Richard Karn. In 1995 Thomas produced a pilot of a game show called Family Challenge for ABC. When ABC did not pick up the series, Thomas sold Family Challenge to the Family Channel, where he produced 144 episodes of the show spread over 2 seasons. In 1996, Thomas played the title role in the Fox television film Mr. Foster's Field Trip aka Kidz in the Wood with Julia Duffy. In 1996, he wrote the book SCTV: Behind the Scenes (McClelland & Stewart, publishers). From 1999–2002, he voiced various roles on the animated series Mission Hill. Thomas co-starred in the Paramount feature Rat Race. As of 2001, Thomas has been the Executive Creative Director of Animax Entertainment, an animation studio based officially in Culver City, California. In 2001–2002 Thomas appeared with Eugene Levy and Martin Short on Short's show Primetime Glick as Bob Hope (an impression he had first developed for SCTV with great success). In 2002, he co-starred with Jason Priestley, Dave Foley, and Ewen Bremner in Fancy Dancing. The next year he played a lead role in Beethoven's 5th. In 2003, he directed a hospital comedy feature film entitled Whitecoats, which he also wrote. As of 2004, Thomas was on the official Advisory Committee for the Comedy program at Humber College, the only such diploma program in the world. In 2004, he and Moranis again worked together voicing Rutt and Tuke, two moose based on the McKenzie Brothers, in Disney's animated feature Brother Bear.[4] Thomas has had a long career doing voices for animation including Animaniacs, Duckman, CatDog, The Adventures of Tarzan, Justice League and multiple roles on The Simpsons, King of the Hill and Family Guy. In 2005, he had a guest stint as Charlize Theron's "Uncle Trevor" on Fox's Arrested Development. In 2006, he reprised his voice role in Brother Bear 2 and appeared as himself in the feature film The Aristocrats. He began production on ArnoldSpeaks.com, a video blog, as the voice of Arnold Schwarzenegger; Animax Entertainment won an Emmy for a broadband animated series produced for ESPN, Off Mikes. In 2007, Thomas and Rick Moranis reprised their roles as Bob and Doug McKenzie in a one-hour special, Bob & Doug McKenzie's Two-Four Anniversary, for CBC Television. The show featured cameos from McKenzie celeb fans like Ben Stiller, Dave Foley, Tom Green, Paul Shaffer, Andy Dick, Matt Groening, Barry Pepper, Martin Short, and Geddy Lee. Former Prime Minister of Canada Paul Martin was the host. In 2008, Thomas revived Bob and Doug McKenzie in a new animated series, Bob & Doug. While Thomas reprises the character of Doug in the new series, Moranis chose not to voice the character of Bob, which instead is voiced by Dave Coulier. Moranis is, however, involved in the series as an executive producer.[5] In November 2009, Thomas received an Honorary Doctor of Letters from his alma mater McMaster University and gave the fall convocation speech. In 2010 Animax continued to produce branded entertainment, advertising and digital shorts for corporations like Disney, Warner Brothers, NBC Universal, and Kodak. In 2011, Thomas's company Animax produced another animated show for MTV entitled Big Box along with numerous Internet shorts such as Life With Dad.[6][7] In 2012 and 2013 Thomas guest starred in the dramatic shows Perception and Bones as well as comedy shows Comedy Bang! Bang! and How I Met Your Mother. In addition in 2013 Thomas voiced the recurring role of Jeff Foxworthy's father Jesco in the CMT show Bounty Hunters. Thomas joined the writing staff of the Fox crime drama television series Bones beginning in 2013. Thomas worked for two seasons on Bones, writing several episodes and working on staff as consulting producer for two seasons.[8] In 2015 Thomas joined the writing staff of NBC's The Blacklist as a consulting producer. In 2020 life-sized statues of Thomas and Rick Moranis as their characters Bob and Doug McKenzie were put in place at the ICE District Sports Arena in Edmonton, Alberta.[9] Also in 2020, the Governor-General of Canada announced that Thomas was being appointed to the Order of Canada,[10] Canada's highest civilian award. In 2021, Thomas and Max Allan Collins teamed to write a sci-fi mystery novel, The Many Lives of Jimmy Leighton.

Defense Secretary Pete Hegseth issued a new edict last week to reduce the time personnel spend on cybersecurity training, among other reforms. The directive came in a Sept. 30 memo to senior Pentagon leadership and DOD agency and field activity directors, ordering the military departments, in coordination with the Pentagon's chief information officer, to “Relax the mandatory frequency for Cybersecurity training.” Hegseth also called for narrowly tailoring records management training to service member roles and allowing flexibility in training delivery, as well as automating information management systems to eliminate training requirements. Additionally, Hegseth directed the military departments and other Pentagon leaders to “relax” the mandatory frequency for controlled unclassified information (CUI) training; remove Privacy Act Training from the Common Military Training (CMT) list; eliminate the mandatory frequency for “Combating Trafficking in Persons” refresher training after appropriate legislation is enacted; consolidate mandatory training topics, “as appropriate”; and develop an integrated CMT program plan. The changes are to be “implemented expeditiously,” per Hegseth's directive. Federal courts are upgrading their cybersecurity on a number of fronts, but multifactor authentication for the system that gives the public access to court data poses “unique challenges,” the Administrative Office of the United States Courts told Sen. Ron Wyden in a letter last week. Wyden, D-Ore., wrote a scathing August letter to the Supreme Court in response to the latest major breach of the federal judiciary's electronic case filing system. The director of the Administrative Office of the United States Courts responded on behalf of the Supreme Court. It is “simply not the case” that the courts have, in the words of Wyden, “ignored” advice from experts on securing the Case Management/Electronic Case Files (CM/ECF) system, wrote Robert Conrad Jr., director of the office. Conrad wrote in the Sept. 30 letter: “Substantial planning for the modernization effort began in 2022, and we are now approaching the development and implementation phase of the project. We expect implementation will begin in the next two years in a modular and iterative manner.” In recent years, the office has been testing technical components on its modernization effort, and is centralizing the operation of data standards to enable security, Conrad said. This Episodes Questions: Brians Questions: I've been making music boxes and trying to replicate the traditional style (see attached photos). I'm struggling to get the curved edge just right. I've tried using a planer, but I keep messing up and have ended up wasting a lot of boards. Router round-overs almost work, but still need planed or sanded after routing to get the shape just right, and also they're pretty dangerous given the small and awkward size of the pieces—I'm working with ¾-inch thick wood that's 2.5 inches high for the box sides. I'd like a repeatable process that provides consistent shapes, but can't seem to find a way to achieve it. Advice? Trish I've heard a few of your podcasts where you speak about making boxes and using a router to make the miter. I'm assuming you're using a bit with a 45° angle. What I don't understand is, how do you get the sides to be the exact same length while using your router table. It would seem to me that having the top of the angle (the point of the angle) against the fence would be almost impossible to get perfectly sized sides. Can you please describe what I am missing here, since a perfect 45° angle using a route a bit seems incredibly simple. Mike G. Guys Questions: I have always tried to select straight grain for legs, aprons, stretchers and the frame of a frame and panel. I am now thinking this may be too much straight grain in a piece of furniture. When are the times that you would not do this and would instead use a more interesting grain pattern in these pieces. Does the wood species affect your choice? For example, it seems like the current trend with walnut is not worrying about getting any straight grain in the pieces at all. Thanks for all your help. Jeff Hi from Melbourne Australia. I've been listening for years, and I like how y'all do things! Thanks for making the best woodworking pod. Like everyone, I have a small shop, that I fit a lot into. Without listing every item… I've got stationary machines covered with a euro style slider (Hammer K3), a 14” bandsaw (N4400) and a combo planer/thicknesser (A3-31). Those 3 cover my needs really well, and I've learned to work within the capacity of my tools and space. It's the secondary/benchtop machines that are causing trouble. Mitre saw was first to go. Don't miss it, don't need it. I'm looking at sanders next. The disc sander stays, couldn't live without it. But the 1632 drum sander and the oscillating bobbin sander are both rarely used and take up space I want back. Could you life without them? If not, what's the essential job they do for you? I'm usually making furniture sized objects. With the finish I get off the helical thicknesser, I find that hand planes/scrapers, ROS, and hand sanding is usually all I need. For bigger flat things, I'm gonna pay a pro shop to put it thru a wide belt sander anyway. Johnny Huys Questions: I was trained on a belt sander, and as Guy has mentioned, there is a learning curve. I'm now very proficient with one and it is a central part of my stock prep: from the planer, I belt sand a rough-sized board with 80 grit, wet it down and let it dry to raise any remaining compression marks from the planer, and then belt sand with 120. I then cut the board to final dimension, random orbital sand with 120, cut the joinery, and then random orbital sand the fitted piece to 180 before final assembly. The process leaves flawless surfaces every time. For panel glue-ups and tabletops, I glue up from the planer using cauls and then make sure the 80 grit belt sanding that follows evens out any discrepancies in height along the joints (hopefully they are minimal). I have zero experience with a drum sander, but I would consider getting one if it could replace some of the above belt sanding, as the belt sander is no light weight hand tool, and it's a killer on my back when I have to sand a full width dining table. I would likely get the PM2244, as I have heard it is the easiest to adjust. You all have mentioned that a drum sander does NOT offer a finish ready surface, as it leaves sanding ridges along the workpiece. This is not a problem in my workflow if it replaces the belt sander. My questions: Are these ridges similar to what I get with the belt sander for a given grit, or are they deeper, requiring by comparison to the belt sander extra time on the following grit? Since I am not seeking to dimension or flatten, but merely prep the surface for the next grit, would one pass in the drum sander for each grit be enough, or am I looking at multiple passes per grit? With a belt sander, there is a lot of back and forth, but with a drum sander I'm wondering if a single pass through will give the same result. How easy and fast is it to change the grit on a drum sander if I want to run 80 grit and then 120 in each sanding session? A dual drum unit is not in the budget. Finally, how reasonable is it to get good results sanding a tabletop that exceeds the width of the sander (i.e., the 22 in the Powermatic) but is within the bounds of the larger number (here, the 44)? With some practice, can I get reliable results, or does this just create more work after sanding, where I'll likely be pulling out the belt sander to even out a center ridge anyway? Keep in mind this could include tabletops from small night stands to full 8' dining tables. As a professional shop, the whole endeavor would be to save me some time and labor. Is it worth getting a drum sander for my workflow, or should I just stick to the belt sander? Michael After listening to your podcast this week I listened with interest the question that was posted by Jose about track saw blade deflection and you guys had a great response. Unfortunately I was hoping that you would address the issue that I'm having with my Makita SP 6000 unit. I bought this unit a couple of years ago and despite great reviews have been a little disappointed. The saw seems to be underpowered. It struggles getting through material whether its 3/4" plywood or thicker hardwood. I started with the stock Makita blade 165x20 48 tooth then with with a CMT 165x20 24 tooth and am currently using a Ridge Carbide TRK16048A 48 tooth blade. I have a Festool extractor and using Makita tracks. The problem is the saw bogs down (I have to creep with the feed rate and still have the issue although not as bad) and burning. Also sometimes the right side of the track edges up where two tracks join causing the saw to catch. I have the good TSO GRC12 track connectors. The Makita ones are junk. I What's up with this? Do I need to move this thing out the door, bite the bullet and buy a Festool? Love the podcast. Tom
