Enam is a University of Buffalo School of Law graduate and currently works as a Legal and Technology Consultant at LawBeta. Enam's story and journey are the epitome of being guided by a unique journey. From Big Law and Wall Street employee for 15 years, to pursuing his passion, in legal technology, a riveting story and one of our longest episodes to date! Enam and I start before his entry into Buffalo School of Law. Inspired by Atticus Finch to go to Law School after reading To Kill a Mocking Bird in 3rd grade, in pursuit of standing up for people and gaining justice. We then moved to Enam's first year of Law School, which he loved and was a time to restart again. It was a lot of work, but it was full of meeting new people and an unbelievably valuable change of scenery. We then moved to Enam's post-graduation experiences, keying in on what he wanted to do with the Law and especially what he did not want to do with the Law in terms of specialization. After this, we spoke about his time in Big Law and working on Wall Street. When first starting, Enam described it as a steep learning curve, but it felt right because it was new and interesting. After the new feeling wore off, Enam would move firms and finally end up at Moody's. He would delve into each experience and what caused each change. After 7 years at Moody, and with the advent of Covid, Enam decided that he needed something new and would move to California. But, Enam would enter into a period of darkness, with his career in the unknown and not wanting to return to Law. Then, out of nowhere, when deciding to go to a conference, he would meet a random person who he still speaks to today, who, after looking over his LinkedIn, told him he was an extremely impressive professional. This story would be, for me, one of the rawest and most emotional segments in this podcast's history, as this one moment, would be a piece of immense inspiration, and lead him to follow his passion. We then came to today, where Enam is a Legal and Technology consultant at LawBeta, consultants at the vibrant intersection of Law and Technology. Enam would speak about his experience thus far, and just how amazing it has been. Finally, Enam gave his technology recommendations, from what kind of LLMs he uses, and the devices he has, and even a shoutout to one of the sponsors of this podcast, Rhetoric. As well, as some spectacular book and podcast recommendations. Enam's journey, not only is one of our longest episodes and one that is full of a wide range of emotions, and amazing career advice from a veteran of the industry. Episode 145: Family Planning for the LGBTQIA+Future Dr. Hoque explains how to assist with family planning for the LGBTQIA+ community. Some principles such as avoiding unintended pregnancies and reducing and early treatment of STIs are discussed.  Written by Ashfi Hoque, MBA, MS4, Ross University School of Medicine. You are listening to Rio Bravo qWeek Podcast, your weekly dose of knowledge brought to you by the Rio Bravo Family Medicine Residency Program from Bakersfield, California, a UCLA-affiliated program sponsored by Clinica Sierra Vista, Let Us Be Your Healthcare Home. This podcast was created for educational purposes only. Visit your primary care provider for additional medical advice.Arreaza: Welcome to episode 145 of the Rio Bravo qWeek podcast. My name is Hector Arreaza, a faculty member of the Rio Bravo Family Medicine Residency Program.Ashfi: Hello everyone, I am Ashfi Hoque a 4th-year medical student at Ross University School of Medicine. I am from Long Beach, California. Patient advocacy and patient-centered care have always been a priority of mine. I've volunteered for years at the LGBT+ center in Weho and Long Beach. Today we will be discussing Family Planning for everyone while learning ways to become LGBTQIA+ inclusive. Arreaza: Yes, family planning is important, and I'm glad you included all types of families. I believe medical care must be offered to everyone, and I also believe in freedom of conscience, that's why I can freely express that I support traditional family for me. Why did you choose this topic?Ashfi: I chose this topic because my partner recently went to get her physical. Her provider had an extensive conversation about family planning and even discussed the anticipated cost of freezing her oocytes. I really loved the way this provider went about the conversation so I started researching ways I can support my community and also teach others to provide Queer inclusive medical care. What is LGBTQIA+?LGBTQIA+ stands for Lesbian, Gay, Bisexual, Trans, Queer, Intersex, Asexual, etc. The community will be referenced as the Queer community, an umbrella term for people who are not heterosexual or not cisgender. There are many inequalities that the community faces and we can do our due diligence to educate ourselves continuously and be aware that terminology and health needs may change. We have another Rio Bravo episode, Caring for LGBTQ+ Patients on Episode 103, that discusses healthcare disparities, but during this episode, we will be diving into an introduction to bridging health gaps, creating health equity, and building trust with the community. A 2023 Global Survey found that the self-identified Queer community represents 9% of the population, while the true estimate may be higher due to safety concerns. While diabetics are 10-13% of the population. These statistics show that as a medical provider, you'll encounter Queer patients more often than you think. One of the healthcare issues that Queer folks face is a lack of family planning. What is Family Planning? The World Health Organization (WHO) defines family planning as “the ability of individuals and couples to anticipate and attain their desired number of children and the spacing and timing of their births. It is achieved through the use of contraceptive methods and the treatment of involuntary infertility.”Family planning serves three critical needs: Avoiding unintended pregnanciesReducing sexually transmitted diseases (STDs)Early treatment of STDs to reduce rates of infertilityWhen discussing family planning for patients, here are some examples of questions you can ask. What name may I use to address you? What are your pronouns? What is your gender? (Only if necessary for care, what is your assigned sex at birth?)Are you sexually active? What is the gender(s) of your partner(s)?Are you concerned about unintended pregnancy? Are you currently using any contraceptive measures? Are you taking any precautions to reduce STI/STD such as physical barriers like condoms, dental dams, or any harm reduction such as PrEP?What kind of STI/STD screening are you requesting?Do you need me to request additional labs such as oral or anal swabs? Those questions must be asked in a natural, non-judgmental way. While STD/STI screening and treatment is part of family planning, the part that we tend to neglect is the desire for Queer folks to build a family. Why is Family Planning Important for the LGBTQIA+ community? The Queer community gained the legal right to marry eight years ago, in 2015. They did not have the nationwide right to adopt until the last state, Mississippi, overturned the unconstitutional restrictions for the Queer community to adopt in 2016. A UCLA study in 2018 titled, “How many same-sex couples in the US are raising children?” reported cis-heterosexual couples: 3% are raising at least one adopted child and 95% are raising biological children while same-sex couples: 21.4% are raising at least one adopted child and 68% have a biological child. When it comes to family planning, there is more than adoption for Queer people. Queer folks are not offered the same pregnancy planning options, such as cis hetero-couples who are experiencing infertility or cis-women planning for advanced maternal age pregnancy. However, the options are quite similar. These options require specific types of planning, and that information can be provided to patients by their primary care doctor. A couple needs to know their options and consider the long-term financial planning necessary for surrogacy, in vitro fertilization (IVF), or donor insemination. The main difference for many Queer couples is the method of conception needed. Depending on sexual orientation and gender identity, patients may have varying reproductive needs as part of their family planning. We cannot make assumptions about how family planning should look and need to remember this journey looks different from person to person and couple to couple. How to approach family planning with the LGBTQIA+ community? Basic tenants of providing medical care for queer patients: Clinics specializing in Queer family planning found patient-centered care leads to better outcomes. The best approach is to be mindful, conscious, and to communicate without assumptions. We have to start with providers building trust, being honest, showing sensitivity assisting with reproductive services, and working towards being more knowledgeable about Queer parenthood. A provider could ask questions such as: Would you like information about family planning? What do you imagine your future family to look like?Would you like to see options and potential costs?Would you need a referral for a specialist?Or it can be as simple as being honest about your scope of knowledge by stating, “I am not well versed in LGBTQIA+ community issues but what ways can I support you?” It is ethically appropriate to transition care to a physician with better knowledge if you feel unable to assist a person from the LGBTQ+ community. Make sure to do it in a polite and respectful way.Gender inclusive: With more people openly identifying as non-binary and trans, there is a need for a gender-neutral approach to discussing a patient's biological and reproductive needs. First, we will avoid assuming gender identity based on the biological sex of a patient. Episode 14 of Rio Bravo does a great job of breaking down gender diversity and the difference between gender identity and biological sex. For transgender and nonbinary patients, providing care for medical transitioning often includes conversations about family planning before starting HRT. It is common to ask patients about to begin HRT if they would like to freeze their sperm or eggs. Second, we want to avoid assuming anything based on what reproductive organs a patient has. We can ask a patient about their intention to start a family. Avoid asking if a trans patient has received transitional surgery (bottom surgery) unless it is completely necessary for the care we are providing. Instead, it is appropriate to ask the patient if birthing is an option? Have you given birth before? Were there any complications? Is there any current hormonal treatment? This mindful strategy is also useful for patients who may have limitations in: producing oocytes or sperm, the ability to house a fetus in utero, or implantation and fertility. Third, we are going to address our underlying beliefs and assumptions about gendered parenthood. Parenthood is almost always thought of as motherhood and fatherhood, but this can be alienating for transgender patients. There are many possible ways of being a parent, and to be inclusive let's consider the possibility of a masculine woman or transmasculine man being a birthing parent or of a transgender woman being the mother of a child without giving birth to the child. There are many more scenarios we can discuss at another point. In the interest of time, we are going to shift into discussing family planning for lesbian and gay people and couples. Sexuality inclusive:For homosexual cis-gendered people who are single or in relationships, family planning can look similar to couples facing infertility issues. When having family planning conversations with these patients, a provider should ask broad, unassuming questions. If you have established that a queer person or couple wants a child, then you can ask if they have a family plan. If the patient or couple has a plan, follow the couple's lead. If the patient(s) do not have a plan, then you can begin to ask questions like: Do you have someone in mind to be a birth giver? Do you have a sperm donor? Do you have an egg donor? These questions are a great transition into discussing the following options for family planning. What are the options for having a newborn and the financial and ethical cost?Having a child can cost up to $100k, and this does not even include the cost of childcare. Infertility treatment is not covered by regular insurance, so patients need either infertility insurance or private financing to cover the cost of treatment. However, fertility insurance does not cover same-sex couples. There is a large emotional, physical, and ethical cost to deciding which route to choose. Let's discuss options and obstacles.1. Donor Insemination: The most affordable route is having a birth-giving parent who is fertile with a known sperm donor. This method can be as simple as using a syringe to inseminate the uterus-carrying person, but we need to consider necessary attorney fees to terminate the parental rights of the sperm donor. Sperm from a sperm bank requires an extensive workup including STD panel, HIV, and genetic disorder screening. The sperm donor gives up all parental rights during the process. The price of these procedures is constantly changing and depends on location.California Cryobank costs start at $1200 for anonymous donors and $1900 for identification disclosure donor which the child will receive information about the donor at age 18. Selecting a donor can include specifics such as race, talents, education, hobbies, physical attributes, and showing donor baby photos. There are two common insemination processes:Intracervical insemination: semen inside the cervical opening and covers the cervixIntrauterine insemination: semen is inserted through the cervix and placed directly into the cavityThe next option jumps up in cost significantly.2. Freezing Eggs (Oocyte Cryopreservation):Pacific Fertility Center Los Angeles, reports a single cycle of egg freezing can cost $6-10k per freezing cycle and may need multiple cycles without medication. The medications are typically around $3-6k depending on how much your body needs. Storage is an additional cost of $700-$1,000 a year. This is an option for parents planning pregnancy during advanced ages.3. In Vitro Fertilization (IVF): It is a process where an oocyte is collected similarly to freezing eggs but fertilized with a partner's or donor's sperm.Pacific Fertility Center Los Angeles reports it costs $8-13k per cycle of fertilization. It is an option for those who have issues with infertility, previous pelvic inflammatory diseases, surgeries, and issues with implantations.4. Surrogacy: This is the process of hiring a professional birthing surrogate to carry an embryo. This is an alternative option for couples who decline or cannot carry a pregnancy. The surrogate has no legal rights or biological relation to the fetus. Family Tree Surrogacy reports it costs about $45-65k.5. Adoption: Foster care adoption in California can be $1-5k. American Cost of Adoption, reports the cost of adoption for infants in California $40-70k including the medical expenses for the birth-giving person and legal expenses for the process. Versus adopting an infant from another country due lack of resources and poverty may better their lives or cause a higher demand for infants which may be an ethical issue. Also, transcultural adoption where the race of the parents and the children are different, and navigating culture and race with the children. Adoptees have reported having racial identity crises. With all these studies, it is well documented that providers will not be perfect at giving care to the Queer community. These studies do not represent every queer person and do not take the intersectionality of race, class, or gender identity into consideration. It is our job as providers to be supportive of all types of patients in order to increase their access to proper medical care. _______________Conclusion: Now we conclude episode number 145, “Family Planning for the LGBTQIA+.” Future Dr. Hoque explained how queer people can be included in family planning conversations, even before heterosexual couples. She described some options such as donor insemination, freezing eggs, IVF, and adoption. Dr. Arreaza explained that it is important to ask reproductive questions in a natural, non-judgmental way to all your patients, and refer to another professional when needed. This week we thank Hector Arreaza and Ashfi Hoque. A., Houk, C., Sedovy, M. W., Johnstone, S. R., Chappell, J. C. Abstract: The platelet-derived growth factor-BB (PDGF-BB) pathway provides critical regulation of cerebrovascular pericytes, orchestrating their investment and retention within the brain microcirculation. Dysregulated PDGF Receptor-beta (PDGFRb) signaling can lead to pericyte defects that compromise blood-brain barrier (BBB) integrity and cerebral perfusion, impairing neuronal activity and viability, which fuels cognitive and memory deficits. Receptor tyrosine kinases (RTKs) like PDGF-BB and vascular endothelial growth factor-A (VEGF-A) are often modulated by soluble isoforms of cognate receptors that establish signaling activity within a physiological range. Soluble PDGFRb (sPDGFRb) isoforms have been reported to form by enzymatic cleavage from cerebrovascular mural cells, and pericytes in particular, largely under pathological conditions. However, pre-mRNA alternative splicing has not been widely explored as a possible mechanism for generating sPDGFRb variants, and specifically during tissue homeostasis. Here, we found sPDGFRb protein in the murine brain and other tissues under normal, physiological conditions. Utilizing brain samples for follow-on analysis, we identified mRNA sequences corresponding to sPDGFRb isoforms, which facilitated construction of predicted protein structures and related amino acid sequences. Human cell lines yielded comparable sequences and protein model predictions. Retention of ligand binding capacity was confirmed for sPDGFRb by co-immunoprecipitation. Visualizing fluorescently labeled sPDGFRb transcripts revealed a spatial distribution corresponding to murine brain pericytes alongside cerebrovascular endothelium. Soluble PDGFRb protein was detected throughout the brain parenchyma in distinct regions such as along the lateral ventricles, with signals also found more broadly adjacent to cerebral microvessels consistent with pericyte labeling. To better understand how sPDGFRb variants might be regulated, we found elevated transcript and protein levels in the murine brain with age, and acute hypoxia increased sPDGFRb variant transcripts in a cell-based model of intact vessels. Our findings indicate that soluble isoforms of PDGFRb likely arise from pre-mRNA alternative splicing, in addition to enzymatic cleavage mechanisms, and these variants exist under normal physiological conditions. Follow-on studies will be needed to establish potential roles for sPDGFRb in regulating PDGF-BB signaling to maintain pericyte quiescence, BBB integrity, and cerebral perfusion -- critical processes underlying neuronal health and function, and in turn memory and cognition.

Join us for an evening with authors from Kaya Press, the group of dedicated writers, artists, readers, and lovers of books working together to publish the most challenging, thoughtful, and provocative literature being produced throughout the Asian and Pacific Island diasporas, with special guest Abeer Hoque. The Secret Room In Kazim Ali's wildly inventive novel The Secret Room, written as musical score for a string quartet, he asks: How does one create a life of meaning in the face of loneliness and alienation from one’s own family, culture, or even sense of self? During the space of one single day, the lives of four people converge and diverge in ways they themselves may not even measure. Sonia Chang, a violinist prepares for a concert. Rizwan Syed, a yoga teacher who gives so much to others, makes one last panicked attempt at reconciliation with his own family. Jody Merchant tries to balance a difficult and stressful work-life with a dream she abandoned long ago. Pratap Patel trudges through his life trying to ignore the pain he still feels at old losses. Just like the real musical quality of a string quartet, these four characters weave in and out of one another's experiences in a raw, fluid song that mimics the hidden lives that exist within us all. Praise for Kazim Ali “Here are new organizing principles; to allow ourselves to be organized by music; to be scored. This is a text that suggests not to worry about how to read it. Rather, it extends an invitation to allow the text to happen with us (and/or for us to happen with the text), and this is a Revolutionary Hermeneutics: to open to the experiences of pain and awe. Text as ambient drift we can move through (the same space where healing and magic happens). The way a line divines another, a voice divines a voice, and the emergent conversation, and how this conversation is a hidden music, the music we have been waiting for.”-- Selah Saterstrom, author of Slab and Ideal Suggestions: Essays in Divinatory Poetics "Kazim Ali has managed to render into the English language the universal inner voice." -- Lucille Clifton Kazim Ali's books include five volumes of poetry, The Far Mosque, The Fortieth Day, Bright Felon, Sky Ward, and All One’s Blue: New and Selected Poems; three novels, Quinn’s Passage, The Disappearance of Seth and Wind Instrument; a collection of short stories, Uncle Sharif’s Life in Music, and three collections of essays, Orange Alert: Essays on Poetry, Art and the Architecture of Silence, Fasting for Ramadan and Resident Alien: On Border-crossing and the Undocumented Divine. He has translated books by Sohrab Sepehri, Ananda Devi and Marguerite Duras. He is an associate professor of Comparative Literature and the director of the Creative Writing Program at Oberlin College. The Flayed City Hari Alluri is an author who, according to U.S. Poet Laureate Juan Felipe Herrera, “carries a new, quiet brush of multi-currents, of multi-worlds to paint this holographic life-scape.” In The Flayed City, Alluri gives an intimate look into the lives of city dwellers and immigrants, imagining the souls that reside in “broom-filled nights”, “skyscrapers for buoys”, and under an “aluminum rising sun”. The charged poems in The Flayed City sweep together “an archipelago song” scored by memory and landscape, history and mythology, desire and loss. Driven by what is residual—of displacement, of family, of violent yet delicate masculinity, of undervalued yet imperative work—Alluri's lines quiver with the poet's distinctive rendering of praise and lament steeped with “gravity and blood” where “the smell of ants being born surrounds us” and “city lights form constellations // invented to symbolize war.” Praise for Hari Alluri “Hari Alluri is Michaux for our time. Which is to say: he is the poet who is able to find myth in our days of sorrow and displacement, when so many lose homes and identities, Hari Alluri offers a new music. When cities are destroyed by fire, Hari Alluri offers lyric fire that heals the heart, that lets theimagination save us. When there is nothing left to say and the page of our drive to stop the pain is brightly-lit and blank, Hari Alluri brings a few words that sing, brings them by the hand, gives them to us—not just words but images, sparks, from which the fire comes, from which whole villages are alive again. This is the poet to live with."-- Ilya Kaminsky, author of Dancing in Odessa  [Hari Alluri] carries a new, quiet brush of multi-currents, of multi-worlds to paint this holographic life-scape; a most rare set of poems—with jazz beat word lines, long-line wisdom and open space scenes where you can widen your eyes, scrape your hands and rush into colliding worlds. Bravo, many bravos!” Hari Alluri, who immigrated to Vancouver, Coast Salish territories at age twelve, is the author of Carving Ashes (CiCAC, 2013) and The Promise of Rust (Mouthfeel, 2016). An award-winning poet, educator, and teaching artist, his work appears widely in anthologies, journals and online venues, including Chautauqua, Poemeleon and Split This Rock. He is a founding editor at Locked Horn Press, where he has co-edited two anthologies, Gendered & Written: Forums on Poetics andRead America(s): An Anthology. He holds an MFA in Creative Writing from San Diego State University and, along with the Federico Moramarco Poetry International Teaching Prize, he has received VONA/Voices and Las Dos Brujas fellowships and a National Film Board of Canada grant. Hari currently serves as editor of pacific Review in San Diego, Kumeyaay land. Photo by Cynthia Dewi Oka Olive Witch (Harper 360) In the 1970s, Nigeria is flush with oil money, building new universities, and hanging on to old colonial habits. Abeer Hoque is a Bangladeshi girl growing up in a small sunlit university town where the red clay earth, corporal punishment and running games are facts of life. At thirteen she moves with her family to suburban Pittsburgh and finds herself surrounded by clouded skies and high schoolers who speak in movie quotes and pop culture slang. Finding her place as a young woman in America proves more difficult than she can imagine. Disassociated from her parents and laid low by academic pressure and a spiraling depression, she is committed to a psychiatric ward in Philadelphia. When she moves to Bangladesh on her own, it proves yet another beginning for someone who is only just getting used to being an outsider – wherever she is. Arresting and beautifully written, with poems and weather conditions framing each chapter, Olive Witch is an intimate memoir about taking the long way home. Praise for Abeer Y. Hoque “Told with vivid lyricism yet unflinching in its gaze, Abeer Hoque's memoir is the coming-of-age story of migration on three continents, and about the pain, rupture, and redemptive possibilities of displacement.”
--Tahmima Anam, author of The Bones of Grace "An unflinching yet luminously beautiful take on family, race, sex and the treachery of memory. Don’t be fooled by the frangipani beauty of Abeer Hoque’s prose. Its razor-sharp edges can draw blood."--Sandip Roy, author of Don't Let Him Know Abeer Y. Hoque is a Bangladeshi-American writer and photographer. Her first book of fiction, The Lovers and the Leavers, was published by HarperCollins to critical acclaim. She also has a book of travel photographs and poems, The Long Way Home. Several newly emerged wireless technologies (e.g., Internet-of-Things)---extensively backed by the tech industry---are being widely adopted and have resulted in a proliferation of diverse smart appliances and gadgets (e.g., smart thermostat, wearables, smartphones), which has ensuingly shaped our modern digital life. These technologies utilize several communication protocols that usually have stringent requirements stated in their specifications or standards, which their implementations are expected to comply with. Noncompliance exhibited by an implementation can cause interoperability issues, inconsistent behavior, or even security vulnerabilities.Automatically detecting whether a protocol implementation is noncompliant with a given property is a long-standing and challenging problem. Moreover, lack of robustness in a protocol implementation to malicious attacks---exploiting subtle vulnerabilities in the implementation---mounted by the compromised nodes in an adversarial environment can limit the practical utility of the implementation by impairing the performance of the protocol and can even have detrimental effects on the availability of the network.Given the stake associated with these wireless technologies, the requirement to ensure secure and reliable operations of the protocol implementations calls for pre-deployment measures. In this talk, I will focus on fortifying these emerging technologies along two dimensions. I will first present an automated framework that enables a developer to check whether a protocol implementation violates its desired properties derived from its specifications and standards. Finally, I will present an automated adversarial testing platform to help developers find malicious attacks that impair the performance of their protocol implementations. In her first novel, The Lovers and the Leavers (Fourth Estate, 2015), Abeer Hoque undertakes a literary challenge that I suspect even the most seasoned writer would find daunting: how do you tell the stories of those people, old and young, cosmopolitan and rural, living throughout the world in the South Asian diaspora? She meets this challenge through a series of interconnected stories, in which the links among characters emerge subtly but inextricably, a web of family ties that reaches from Bangladesh and India around the globe. Moreover, she captures these stories not only in prose, but also in poetry and photography, making The Lovers and the Leavers a multimedia, multi-genre experience. It’s an ambitious undertaking, spirited and subtle. Yet for all of Hoque’s impressive artistry, she seeks very recognizable ends: to give us a vivid sense of place as rich as the people who inhabit it and to render the inner lives of those people, to let us feel their passions and their pains–those that mark them and make them sometimes beautiful, sometimes broken, but always always compelling.
