A recent 12-week randomised controlled trial involving healthy adults aged 55-70 yielded significant positive findings regarding pomegranate extract's effect on blood pressure. While the study might not have shown significant differences compared to placebo across all initially measured markers, it revealed important reductions within the group taking pomegranate extract daily. Specifically, participants supplementing with pomegranate extract saw a statistically significant average decrease in Systolic Blood Pressure (SBP) by 5.22 mmHg and Diastolic Blood Pressure (DBP) by 3.6 mmHg. The researchers highlight the potential clinical relevance of the SBP reduction, noting that a 5 mmHg drop is associated with an estimated 10% decrease in the risk of major cardiovascular events. Thorough history taking and clinical acumen are still key elements in establishing a diagnosis of GCA.2.      The patient's initial response and subsequent decline in vision illustrate the need for ongoing monitoring and readiness to adapt the treatment approach. It also shows the necessity of close monitoring of inflammatory markers and clinical symptoms.3.      Amaurosis fugax warrants urgent referral to an ophthalmologist.4.      High index of suspicion for GCA and prompt referral of GCA suspects will ensure best outcome for patients.5.      Progressive visual loss and elevated inflammatory markers should alert the clinician to glucocorticoid-resistant GCA.6.      The effectiveness of tocilizumab in this case highlights its role as a valuable treatment option for refractory GCA, especially when traditional therapies are not sufficiently effective. Targeted biologic agents may open up new treatment approaches in the future particularly in patients with progressive visual loss despite administration of intravenous methylprednisolone. 7.      Managing complex cases like GCA often requires a collaborative approach involving rheumatologists, ophthalmologists, and other specialists to ensure comprehensive care and optimal outcomes.8.      There is often value in case reports to start the evidence cascade that is required to bring new, life altering treatments to the forefront.

Also check out the video version on my WeitzChiro YouTube page.]  Podcast Highlights 1:48  Dried urine testing or DUTCH testing uses a dried sample for convenience, which is a big advantage over the other leading type of urine hormone testing, which is the 24 hour urine test that requires carrying around a jug of urine that you keep in the refrigerator all day.  This is very inconvenient and you lose the the changes in those hormones levels, esp. cortisol, that vary throughout the day using a 24 hour sample.  The dried urine test you collect samples throughout the day so you can look at the cortisol pattern, which correlates with what is seen on saliva in studies that have been published.  And the urine testing of hormones allows you to see the metabolites of estrogens and androgens that you can't see on blood testing.  And there is a DUTCH complete test that includes markers in urine for melatonin and B12 and B6, etc. that allows us to see a fuller picture of the hormone story.  For example, this complete testing is helpful if you have a patient who has fatigue you can find out if it is related to testosterone or cortisol or B12. 4:57  On the other hand, dried urine testing does not allow you to measure SHBG, which can be measured through blood, such as when a woman is on birth control and this drives up her SHBG, which drives down her free hormones. Urine testing is also not a good way to measure thyroid hormones. Also, if your kidneys are not functioning well, then you are not excreting urine as well as you should and this can impact urine testing. Also, saliva has some advantages for the cortisol awakening response (CAR), so DUTCH plus includes saliva for the first two samples of cortisol, since for the CAR you need an instantaneous sample when you first wake up within the first five minutes.  DUTCH uses a cotton swab that soaks up saliva instead of having to spit into a tube like other labs use and fill it with 3 ml of saliva after waking up without rinsing your mouth and without drinking any water and for most people this takes 15-20 min, which negates the value of the CAR test.                                Mark Newman, MS is a recognized expert and international speaker in the field of hormone testing. Kim:                     The noise, I would say, in the rest of the world, outside of themselves. I think most people forget to listen to what's going on with their own body, and there's so much confusion with what everyone else is doing. Dr. Weitz:            But how do you know what your body's really saying? Kim:                     You got to be mindful, you got to be grounded. And most people are walking around emotionally triggered and they don't even recognize it. I mean our rational mind knows that food is fuel. Our irrational mind thinks food is comfort, joy, entertainment, the enemy, fun. And so, if you think food is anything other than fuel, it's time to check in with what's going on. Take some deep breaths and figure out where the discomfort is in your body or in your life that could be causing your mind to seek some pleasure from something that is outside of yourself. Leaky gut is a controversial concept in the functional medicine world. No, I'm not talking about the controversy in conventional medicine about whether leaky gut exists at all. Most of us understand that there's tons of research substantiating that leaky gut exists and that it's a problem, but there are differences about how to test for leaky gut. There's the old lactulose mannitol intestinal permeability test, but most of us today are either using a serum test for zonulin or a stool zonulin test, or a serum test for zonulin antibodies, and there's a lot of controversy whether any of these are accurate.  I personally run a stool zonulin as part of a stool test, but quite frankly, I assume that most of my patients with gut problems have leaky gut. There's also controversy about the best strategies for healing leaky gut and whether probiotics, and which ones, prebiotics, which ones, bone broth, et cetera, et cetera. How should we heal leaky gut? I'm hoping that Dr. Jess Armine can give us some clarification and guidance for how to understand leaky gut and how to treat it. Rahbar and Gurevich on SIBO and IBS: Discussion on SIBO and IBS with Dr. Sam Rahbar and Dr. Ilana Gurevich: Rational Wellness Podcast 298.  We also had a presentation by Dr. Mark Pimentel on the latest research that his group has recently published on SIBO and IBS:  SIBO: New Research Findings with Dr. Mark Pimentel: Rational Wellness Podcast 311.   I also recommend that you listen to the interview that Shivan Sarna and Dr. Allison Siebecker conducted with Dr. Ali Rezaie of Dr. Pimentel's group:  Digestive Disease Week 2023 Research Update by Dr. Ali Rezaie (June 2023)    3:42  Irritable bowel syndrome or IBS is the most common gastrointestinal disorder occurring in up to 20% of the US population.  It is a functional digestive disorder marked by the following symptoms: gas and bloating, stomach pain, diarrhea, constipation, or alternating diarrhea and constipation.  IBS is considered a functional disorder because if you do a scope or do a CAT scan, you won't see any structural problems in the intestines.  By contrast, if you have Crohn's or ulcerative colitis and you do a scope or a scan, you'll likely see damage to the lining of the intestines, possibly bleeding, and erosions.  Because it's a functional disorder, IBS was often considered to be a stress-induced disorder.  Dr. Pimentel has now proven that approximately 60% of cases of IBS are caused by Small Intestinal Bacteria Overgrowth or SIBO.  5:35  The large intestine, aka, the colon, is a large and very elastic tube and it contains the trillions of bacteria, archea, and fungi that compose our microbiome. When these bacteria in the colon ferment and produce gas, this is not a problem. But the small intestine is a smaller tube and doesn't stretch as much and doesn't contain anywhere as much bacteria as the large intestine.  The small intestine is where most nutrients are absorbed from our food, so there need to be a smaller amount of bacteria lining its mucosa or this would interfere with this.  If you consume fermentable fiber and there is an overgrowth of bacteria in the small intestine, this can lead to gas and bloating, which can cause pain and a change in bowel habits.  Hydrogen sulfide and hydrogen gas are usually associated with diarrhea, while methane gas is usually associated with constipation.    Dr. Ben Weitz is available for Functional Nutrition consultations specializing in Functional Gastrointestinal Disorders like IBS/SIBO and Reflux and also Cardiometabolic Risk Factors like elevated lipids, high blood sugar, and high blood pressure.  Dr. Weitz has also successfully helped many patients with managing their weight and improving their athletic performance, as well as sports chiropractic work by calling his Santa Monica office 310-395-3111. Also check out the video version on my WeitzChiro YouTube page.]    Podcast Highlights 2:27  Jeffrey Smith has been dedicated to speaking out about the potential dangers of genetically modified organisms since he heard a lecture 27 years ago by a genetic engineer who was blowing the whistle on Monsanto that was about to release its first genetically modified crops into our food supply.  Jeffrey runs the ResponsibleTechnology.org website and if you go to ResponsibleTechnology.org/takeaction before May 21 you can take action and add your voice to the discussion in the U.S. Department of Agriculture about how to regulate genetically modified organisms so we can prevent an apocalypse. Now Jeffrey is also speaking up about the dangers of GMO 2.0, which is the subject of the documentary Don't Let the Gene Out of the Bottle, which is a 16-minute film, which happens to be at the website above. 6:26  GMO Foods.  Traditional GMO foods have a gene from another species transferred into that plant, but we now have have GMO 2.0, which utilizes the new gene editing tools to knock out genes or gut the genome in a place or two and hoping that when the thing gets reattached, it either adds what you want in or leaves out what you want and doesn't bring anything in extra that you don't want.  This involves a lot of wishful thinking and obviously a lot could go wrong.  Jeffrey has a six minute film at responsibletechnology.org/takeaction that discusses some of these dangers: Seven Reasons Why Gene Editing Is Dangerous and Unpredictable.  The biotech industry has been licking its wounds because the movement to limit and ban GMO foods has been successful at informing consumers, and doctors that it is not safe that we have been able to contain it to 12 genetically engineered food crops as opposed to Monsanto's stated goal by this time to have 100% of all commercial seeds genetically engineered and patented.  What's even scarier is that it used to take a long period of time and was expensive to genetically engineer foods, but now you can buy a gene editing kit from Amazon for a few thousand dollars and create your own GMO every day. 9:41  Monsanto convinced the White House that GMOs were safe and that GMOs were going to increase US exports and domination by the US in world food trade and they should be allowed on the market.  Monsanto were able to get a former attorney of theirs--Michael Taylor--into the FDA to create a pro GMO policy.       Jeffrey Smith is a global thought leader on the health dangers of genetically modified organisms for over 27 years. Also check out the video version on my WeitzChiro YouTube page.]    Podcast Highlights 2:25  Dr. Brandy explained that his journey started in September 2017 when he and his wife were on a Viking cruise and he read the book, How Not To Die by Dr. Michael Greger.  How Not To Die has lots of scientific references and shows that eating a plant based diet is associated with lower rates of cancer and other chronic diseases, including diabetes, cardiovascular disease, and dementia. Gregor also discusses the Blue Zones where Dan Buettner looked at regions in the world where people lived the longest, including Loma Linda, California, where most people are Seven Day Adventists and they are vegetarian, don't drink alcohol, and exercise a lot. The average woman there lives 10 years longer and the average man lives 14 years longer than the average American.  Dr. Brandy started right then to eat a plant based diet. 6:25  When Dr. Brandy came home the last week of September he was doing a surgery case and he felt a pop in his right collarbone.  This pain then got worse and worse and on November 10th he went to pick up a container of water and his clavicle cracked in half.  He went to the Urgent Care center and got an x-ray. Then he got an MRI and his orthopedic surgeon told him that he either had multiple myeloma or a cancer metastasis. He had a biopsy and lab work, which confirmed IgA multiple myeloma free Kappa chain. Dr. Brandy's oncologist recommended a triple drug regimen of a common medication for myeloma, a corticosteroid and Velcade, a proteasome inhibitor. Dr. Brandy accepted with the first two medications but rejected Velcade, because its common side effects include peripheral neuropathy, which is bad for a surgeon.  Then Dr. Brandy researched every thing he could do from a diet, lifestyle, and nutritional supplement perspective and started employing them.  He started taking various herbs, employed exercise, stress reduction, sleep, intermittent fasting, detoxification, and every month his numbers kept getting better and by six months he was in a complete remission.     Dr. Dominic Brandy is a plastic surgeon and anti-aging expert, but after being diagnosed in 2017 with multiple myeloma, an incurable blood cancer, he plunged himself into cancer research. Also check out the video version on my WeitzChiro YouTube page.]    Podcast Highlights 0:30  Mast cell activation syndrome.  Mast cells are immune cells found throughout the body that contain granules that contain various chemical mediators, including histamine and heparin.  Mast cells are involved in allergies and in various inflammatory and immune functions, including wound healing, angiogenesis, immune tolerance, and defense against pathogens. In mast cell activation syndrome, mast cells are inappropriately and excessively release chemical mediators, resulting in a range of symptoms that are grouped into cardiovascular, dermatological, GI, neurological, and respiratory in nature.  These include flushing, hives, a reddish complexion, itchiness, burning, lightheadedness, dizziness, arrhythmia, diarrhea or constipation, stomach pain, nausea, acid reflux, difficulty swallowing, headaches, fatigue, lack of concentration, and other cognitive problems, congestion, coughing, and even anaphylaxis. 2:47  Dr. Dempsey first started to focus on mast cell activation syndrome (MCAS) when researching trying to understand a patient who had a flare of this condition.  She spoke with Dr. Lawrence Afrin, who was a recognized expert on mast cell activation syndrome, who explained the mechanism for this condition.  She recognized that patients she had treated in the past probably had some elements of this condition as well. 12:05  MCAS is a multisystemic process and the most common symptoms include genital urinary complaints, hormonal complaints like endometriosis, painful periods, and excessive bleeding, migraines, interstitial cystitis, pain syndromes, high or low blood pressure, etc.. 13:36  Diagnosis of MCAS.  One test that Dr. Dempsey will do with patients is to take a tongue depressor and scrape it across the back of the patient.  If there is a skin reaction, that indicates that mast cells are releasing histamine and that the patient may have MCAS.  This is called dermatographism.  The lab testing for MCAS is tricky, since some of the mediators that can be measured are very short lived, so she has a refrigerated centrifuge in her lab to be able to specially process the samples.  There are a number of markers but there is no one marker that is definitive for making a diagnosis. Tryptase, histamine plasma or whole blood, chromagranin A, and C-reactive protein are markers that can be measured.  If the patient has had an endoscopy or colonoscopy, we can biopsy a sample of the intestinal mucosa and see if there are more than 20 mast cells in the field. La dieta del grupo sanguíneo surge de los trabajos del naturópata Peter D’Adamo, que propone sus ideas sobre la dieta y el grupo sanguíneo en su libro Eat Right 4 Your Type. Se basa en seleccionar aquellos alimentos que dicho libro indica que son adecuados para el grupo sanguíneo de cada individuo.- A los individuos del grupo O como “cazadores“, ya que su grupo se creó hace 40.000 años. Las personas de este grupo deben ingerir principalmente carne, pescado, verdura, semillas y fruta (menos kiwis, naranjas, melón y coco). Con moderación pueden tomar cereales. También pueden tomar legumbres, pero nunca en sustitución de la proteína de la carne roja, que es importantísima para este grupo.- A los del grupo A como los “agricultores“, porque el grupo A surgió hace 25.000 – 10.000 años. Es al único grupo sanguíneo al que se le recomienda ser vegetariano, aunque les conviene también comer pescado y marisco, así como legumbres y productos a base de soja.- A los del grupo B como los “nómadas“, ya que surgieron hace 15.000 – 10.000 años.
