Lydia Poole discusses "The Big Humanitarian Rethink," a digital consultation she launched with Ben Parker to capture diverse perspectives on reforming the humanitarian system during a period of unprecedented funding cuts. The consultation revealed two distinct camps: "reimaginers" who advocate for radical transformation addressing power imbalances, and "improvers" who prefer incremental change while preserving humanitarian principles. Despite these differences, both groups agree on the need to maintain some international crisis response capabilities. As the humanitarian sector navigates this watershed moment, Lydia emphasizes the urgent need to make difficult choices about what to preserve while avoiding a disorderly collapse of vital services. This conversation offers rare insight into a sector at a crossroads, balancing immediate operational demands with the opportunity for meaningful, long-overdue reform.Today's GuestLydia Poole is a humanitarian financing expert with over 15 years of experience. Along with Ben Parker, Lydia co-created "The Big Humanitarian Rethink" citizen consultation, an initiative to collect diverse perspectives on humanitarian system reform following recent funding cuts. The robot's wires are more concealed, indicating a shift towards a production model. Equipped with six cameras, the robot has human-like hands with increased strength, able to carry 25kg. It features improved battery life, a partnership with OpenAI for conversational capabilities, and is being tested by BMW for automotive manufacturing. Why You Need A Second Brain Now (And How AI Can Help You Build It) The concept of a "second brain," popularized by Tiago Forte, helps store and organize information our brains can't retain. This involves capturing, organizing, distilling, and expressing data. AI can simplify this process, especially in organizing and distilling information, using tools like TextCortex, Mem, and Notion. Can AI Even Be Open Source? It's Complicated AI relies heavily on open source, yet major AI vendors hesitate to fully open-source their programs. The complexity lies in defining open-source AI, which is complicated by the intertwining of software and data. The Open Source Initiative is working with tech companies to develop a definition by October 2024. Google Follows Microsoft's Sneaky Route to Swallow the AI Market Big Tech companies like Google are acquiring AI talent by hiring leadership and licensing intellectual property rather than buying startups, avoiding antitrust scrutiny. Google hired Character.ai's co-founders and will license their technology. This trend, seen in Microsoft's and Amazon's actions, consolidates power in AI under major tech firms. 10 Reasons Why AI May Be Overrated While AI garners substantial media attention and investment, skeptics argue its impact is overstated. MIT economist Daron Acemoglu doubts AI's economic revolution, calling it “autocorrect on steroids.” Limitations include overhyped capabilities, environmental impact, and challenges with AI's intelligence and reliability, suggesting AI may enhance rather than replace human work. AI Accurately Turns Thoughts into Images Researchers at Radboud University have advanced AI to reconstruct images from brain activity with unprecedented accuracy. By analyzing fMRI scans and electrode recordings, the AI successfully recreated images viewed by humans and monkeys. This innovation could lead to treatments for vision loss, enhance communication for those with impairments, and revolutionize neuromarketing and entertainment.

This episode was produced by Edmund Hayes and Jouke Heringa. Ep1. Water History and the Pre-Modern Middle East The cities of the medieval Middle East were some of the largest in the world, dwarfing the major cities of western Europe, for example. So how did they support large populations in relatively arid conditions? In this episode we provide an overview of the kinds of hydraulic infrastructure and social institutions that allowed pre-modern Middle Eastern cities to function. Speakers: Maaike van Berkel and Josephine van den Bent. Interviewer: Edmund Hayes. This episode, and this series on water history and the medieval Middle East was produced by Edmund Hayes and Jouke Heringa as part of the project, “Source of Life: Water Management in the Premodern Middle East” at Radboud University. Webb's study of the second-brightest gamma-ray burst ever seen reveals tellurium.A team of scientists has used multiple space and ground-based telescopes, including NASA's James Webb Space Telescope, NASA's Fermi Gamma-ray Space Telescope, and NASA's Neil Gehrels Swift Observatory, to observe an exceptionally bright gamma-ray burst, GRB 230307A, and identify the neutron star merger that generated an explosion that created the burst. Webb also helped scientists detect the chemical element tellurium in the explosion's aftermath.Image: Gamma-Ray Burst 230307AThis image from NASA's James Webb Space Telescope NIRCam (Near-Infrared Camera) instrument highlights Gamma-Ray Burst (GRB) 230307A and its associated kilonova, as well as its former home galaxy, among their local environment of other galaxies and foreground stars. The GRB likely was powered by the merger of two neutron stars. The neutron stars were kicked out of their home galaxy and traveled the distance of about 120,000 light-years, approximately the diameter of the Milky Way galaxy, before finally merging several hundred million years later.Image: NASA, ESA, CSA, STScI, A. Levan (Radboud University and University of Warwick).Other elements near tellurium on the periodic table – like iodine, which is needed for much of life on Earth – are also likely to be present among the kilonova's ejected material. A kilonova is an explosion produced by a neutron star merging with either a black hole or with another neutron star.“Just over 150 years since Dmitri Mendeleev wrote down the periodic table of elements, we are now finally in the position to start filling in those last blanks of understanding where everything was made, thanks to Webb,” said Andrew Levan of Radboud University in the Netherlands and the University of Warwick in the UK, lead author of the study. While neutron star mergers have long been theorized as being the ideal “pressure cookers” to create some of the rarer elements substantially heavier than iron, astronomers have previously encountered a few obstacles in obtaining solid evidence.Long Gamma-Ray BurstKilonovae are extremely rare, making it difficult to observe these events. Short gamma-ray bursts (GRBs), traditionally thought to be those that last less than two seconds, can be byproducts of these infrequent merger episodes. (In contrast, long gamma-ray bursts may last several minutes and are usually associated with the explosive death of a massive star.)The case of GRB 230307A is particularly remarkable. First detected by Fermi in March, it is the second brightest GRB observed in over 50 years of observations, about 1,000 times brighter than a typical gamma-ray burst that Fermi observes. It also lasted for 200 seconds, placing it firmly in the category of long duration gamma-ray bursts, despite its different origin.“This burst is way into the long category. It's not near the border. But it seems to be coming from a merging neutron star,” added Eric Burns, a co-author of the paper and member of the Fermi team at Louisiana State University.Opportunity: Telescope CollaborationThe collaboration of many telescopes on the ground and in space allowed scientists to piece together a wealth of information about this event as soon as the burst was first detected. It is an example of how satellites and telescopes work together to witness changes in the universe as they unfold. After the first detection, an intensive series of observations from the ground and from space, including with Swift, swung into action to pinpoint the source on the sky and track how its brightness changed. These observations in the gamma-ray, X-ray, optical, infrared, and radio showed that the optical/infrared counterpart was faint, evolved quickly, and became very red – the hallmarks of a kilonova.“This type of explosion is very rapid, with the material in the explosion also expanding swiftly,” said Om Sharan Salafia, a co-author of the study at the INAF – Brera Astronomical Observatory in Italy. “As the whole cloud expands, the material cools off quickly and the peak of its light becomes visible in infrared, and becomes redder on timescales of days to weeks.”Image: Killanova – Webb vs ModelThis graphic presentation compares the spectral data of GRB 230307A's kilonova as observed by NASA's James Webb Space Telescope and a kilonova model. Both show a distinct peak in the region of the spectrum associated with tellurium, with the area shaded in red. The detection of tellurium, which is rarer than platinum on Earth, marks Webb's first direct look at an individual heavy element from a kilonova.Illustration: NASA, ESA, CSA, Joseph Olmsted (STScI).At later times it would have been impossible to study this kilonova from the ground, but these were the perfect conditions for Webb's NIRCam (Near-Infrared Camera) and NIRSpec (Near-Infrared Spectrograph) instruments to observe this tumultuous environment. The spectrum has broad lines that show the material is ejected at high speeds, but one feature is clear: light emitted by tellurium, an element rarer than platinum on Earth. The highly sensitive infrared capabilities of Webb helped scientists identify the home address of the two neutron stars that created the kilonova: a spiral galaxy about 120,000 light-years away from the site of the merger.Prior to their venture, they were once two normal massive stars that formed a binary system in their home spiral galaxy. Since the duo was gravitationally bound, both stars were launched together on two separate occasions: when one among the pair exploded as a supernova and became a neutron star, and when the other star followed suit.In this case, the neutron stars remained as a binary system despite two explosive jolts and were kicked out of their home galaxy. The pair traveled approximately the equivalent of the Milky Way galaxy's diameter before merging several hundred million years later.Scientists expect to find even more kilonovae in the future due to the increasing opportunities to have space and ground-based telescopes work in complementary ways to study changes in the universe. For example, while Webb can peer deeper into space than ever before, the remarkable field of view of NASA's upcoming Nancy Grace Roman Space Telescope will enable astronomers to scout where and how frequently these explosions occur.“Webb provides a phenomenal boost and may find even heavier elements,” said Ben Gompertz, a co-author of the study at the University of Birmingham in the UK. “As we get more frequent observations, the models will improve and the spectrum may evolve more in time. Webb has certainly opened the door to do a lot more, and its abilities will be completely transformative for our understanding of the universe.”These findings have been published in the journal Nature. The James Webb Space Telescope is the world's premier space science observatory. Webb is solving mysteries in our solar system, looking beyond to distant worlds around other stars, and probing the mysterious structures and origins of our universe and our place in it. Webb is an international program led by NASA with its partners, ESA (European Space Agency) and the Canadian Space Agency. This show is part of the Spreaker Prime Network, if you are interested in advertising on this podcast, contact us at https://www.spreaker.com/show/5953955/advertisement

Patient from Hell
Episode 38: Am I making the right decision? with Dr. Glyn Elwyn and Danielle Schubbe

Patient from Hell

Play Episode Listen Later Oct 4, 2023 54:38

In this episode, we talk with Dr. Glyn Elwyn and Danielle Schubbe about the ethical imperative of including patients in the decision-making process. They discuss how patient goals inform treatment decisions and the challenge of making choices for a “future self” with the lived experiences and preferences of today. Glyn and Danielle talk about this process for early stage breast cancer and some of the tools they've created to help both clinicians and patients navigate shared decision-making, including the three-talk model. Key Highlights: The art of combining clinical guidelines and patient preference to make the best treatment decision. The role of patient goals in making decisions and realizing that decisions are being made for a “future self” using “current self” experiences. Exploring the three-talk model of decision making: team talk, option talk and decision talk. About our guests: Glyn Elwyn BA MD MSc PhD FRCGP is a clinician, researcher, and innovator. He is a tenured professor at The Dartmouth Institute for Health Policy and Clinical Practice, USA, and at the Scientific Institute for Quality of Healthcare, Radboud University Nijmegen Medical Center, Netherlands. He has Visiting Professor positions at University College London, UK, and at the University of Lausanne in Switzerland. After reading the humanities he qualified in medicine, completed a Masters in Education, and obtained his doctorate at Radboud University, Nijmegen, Netherlands, with Professor Richard Grol. Glyn Elwyn studies coproduction, shared decision making, and the application of machine learning to digital recordings of clinical encounters. Danielle Schubbe joined the Coproduction Laboratory in September 2017. She is an external PhD student of Health Services Research at Radboudumc in Nijmegen, Netherlands. She has worked on multiple PCORI-funded studies about shared decision making and the implementation of shared decision making in diverse clinical contexts. Key Moments: 19 minutes: On integrating patient preference with clinical guidelines. “I think most discerning clinicians understand how to work within guidelines, and I think most expert clinicians would say I bring in patient preference as well as know what the rules are saying or what the guidelines are saying.” 31 minutes: On the challenge of making a decision for your future self. “Your decision today about that future is gonna be misinformed because you've never experienced that future yourself.... We cannot predict you in three years' time. How will you feel? We can only ask you to do your best to predict how you feel in the future… and you will be wrong.” 49 minutes: Using the three-talk method of decision making. “Then there's the option talk. That's when you present if there is more than one option for the patient to consider for their treatment. That's when you go over all the nitty gritty of the pros and cons of the two surgical treatment options, in our case for early stage breast cancer,  in a way that is hopefully not really overwhelming for the patient.” This episode was supported by the Patient Centered Outcomes Research Institute (PCORI) and features these PCORI studies by Dr. Glyn Elwyn & Danielle Schubbe - Study 1 and Study 2. There is a growing resentment toward Western aid in recipient countries. There are many possible reasons why: a history of double agendas from donors, creating dependencies; a strong desire for self-determination; an increasingly multipolar world – the list goes on.   It's about time to discuss the unexpected outcomes of aid that can lead to the backlash or undermining of the social contract in recipient countries. Are religions the primary source of violent conflicts of human history? Can a person be spiritual but not religious? Is secularism devoid of close-mindedness and irrationality? These are some of the questions addressed in this episode by theolgian, Jonas Atlas. Jonas is a Belgian scholar of religion who writes and lectures on religion, politics and mysticism. Growing up Christian in predominately secular Europe he found himself having to defend himself for being religious. Additionally, his marriage to a Muslim from Turkey influenced his interest in deconstructing the way people think about religions. Jonas currently teaches classes on ethics, spirituality, and religion at the KDG University of Applied Sciences and Arts in Antwerp, Belgium. He is also an independent researcher at the Radboud University, as a member of the Race, Religion, and Secularism network. His latest book, Religion: Reality Behind the Myths, unravels the most common misunderstandings about religion. Here are some of the things we talk about: • What are the seven myths about religion? • What is religion? • Religions, like languages, evolve • When we pretend to live without structures, we become blind to all the structures we are really in • Can you be spiritual but not religious? • The characteristics and behaviors that people think are bad about religions are in all organizations because violence, control, oppression is in our consciousness • Everything that can be said of a religion can also be said of the secular world (i.e. they both do dumb things) • “Us versus them” mentality keeps us from reconfiguring our consciousness • The spirituality of the big bang Jonas Atlas links: Website: https://jonasatlas.net/ Book: https://www.amazon.com/Religion-Reality-Behind-Jonas-Atlas/dp/1803411821 Podcast: https://podcasters.spotify.com/pod/show/re-visioning-religion (where you can hear the conversations with Jeffrey Kripal and William Cavanaugh) Interfaith conversations with Muslim scholars, activists and artists: https://halalmonk.com/ © the spiritual forum 2023

“What I find worth remarking upon is the fact that the vast majority of people are so alienated from the Bluefin's life world that they don't know what an extraordinary creature she is—and instead just widely see her as a foodstuff, trafficked on the global market. It's imperative for that worldview to change.” In this episode, we welcome Jennifer E. Telesca, Associate Professor of Environmental Governance in the Department of Geography, Planning, and Environment at the Nijmegen School of Management, Radboud University, the Netherlands. Her work takes a critical approach to ocean studies, spanning the interests of environmental diplomacy, ethnographies of international law in society, the human–animal relationship, political economy, the politics of extinction, and science and technology in policymaking. She conducts fieldwork at the United Nations and in treaty bodies, diplomatic missions, and other sites scaled supranationally. Red Gold: The Managed Extinction of the Giant Bluefin Tuna (University of Minnesota Press, 2020) is Telesca's first single-authored book. Its on-the-ground, first-person research has shown just how damned the lives of fishes are in the very world entrusted to care for them in ocean governance. Her second book on hydrothermal vents, tentatively titled, The Midnight Zone, invites readers to honor creatures in all their mysterious and seemingly impossible forms at sites in the deep dark sea—open to regulatory oversight—where scientists believe life on Earth began. Some of the topics we explore in this conversation include how the Giant Bluefin tuna went from being food for the poor to becoming a global delicacy symbolic of luxury, how fish have long been "an object through which global empires have been mediated," Jen's concerns with the scams and blue-washing of eco certifications in seafood, and more. Any athlete who holds a professional license and is a PTO Professional member is eligible. The leading athletes share out an end of season bonus pool of $2 million split equally between women and men, with a cool $100,000 going to the top ranked athletes.   “I'm really overwhelmed to be on top of the PTO World Rankings after the challenging Covid years. Although I didn't have the one outstanding race I would have wished for, I was able to be very consistent in my performance throughout the season, thanks to my amazing team behind me. They kept me fit and healthy,” said Haug, who's three counting scores in 2022 included Challenge Roth (131.41 points), Ironman Hawaii (110.20 pts) and the Ironman St. George World Championships (109.56 pts).   “This year we saw some absolutely outstanding performances, for example Daniela at the Ironman Worlds in Utah, Chelsea in Kona or Taylor at the 70.3 Worlds. That inspires me, shows how fantastic our sport is and that if you don't go up a level each year, you'll fall behind.”   “The new PTO Open races give us a unique opportunity to race the best athletes from all distances, which improves the quality of every race and adds the spice everyone needs to perform. The team format of the Collins Cup has also pushed racing to a new level, strengthening camaraderie, team spirit, fair play and tactics.”   Blummenfelt has had an outstanding 12 months, not just in winning the full Ironman World Championships in May and the 70.3 version in October, but also breaking 7 hours in the Sub7 Challenge in June.   His three counting scores included: Iroman Hawaii (128.26 pts), the Ironman St. George World Championships (114.98 pts) and the Ironman 70.3 World Championships (113.14 pts).   “It was a strong year for me winning the Ironman World Championships and 70.3 World Championships in St George among other performances. Finishing top of the 2022 PTO World Rankings is something I'm very proud of and I'll be trying to do the same again in 2023.”   The PTO and an athlete committee has been working on an updated version of the PTO World Rankings system for 2023, that will be announced next month.   New study says Strava kudos motivate you to run more When you are looking for motivation to get out for a run, is Strava getting you out the door? Well, a study out of Radboud University in Nijmegen, Netherlands, says that Strava, the largest online social network for athletes, spurs exercise and gives runners motivation to record an activity. The study, published in the Journal of Social Networks, analyzed Strava profiles to see how runners in the same virtual Strava club influenced each other's running behaviour. Researchers focused on five different Strava clubs in the Netherlands that functioned as an online extension of real-life running clubs, totalling 329 members.   In a pre-study survey, they asked the subjects prime what their motive for using Strava was: 83 per cent expressed it was to log activities; 78 per cent said it was to track their progression; 31 per cent said it was for social interaction with other runners, while 21 per cent was to receive validation for their activity.   Researchers predicted that runners use their peers' behaviour as a yardstick and may intentionally try to imitate peers' behaviours to promote their relative standing or to avoid rejection.   Figure. 3. Development of the mean of running attributes (based on running volume, frequency, and gender) They investigated the recorded data for a year, constructing a longitudinal dataset of networks and behaviour, and discovered that those who receive more kudos were inclined to run more. While running variables differed over time within clubs in Figure 3, no consistent seasonal pattern was found.   Researchers initially thought that athletes who ran more would be the most influential on the app, but it was the opposite; athletes who ran less were more likely to follow the running behaviour of their friends who ran more. According to the study, “Those who received more kudos for their activities, ran more.”   The study also found that many runners change their running behaviour to do more kudos-worthy activities, (i.e., run farther, run longer, chase segments). Athletes who had kudos turned off ran less than those who had kudos enabled.   What's New in the 303:   Specialized buys former Pearl Izumi building LOUISVILLE, Colo. (BRAIN) — Specialized Bicycle Components bought the award-winning office building here previously owned by Pearl Izumi, for $14.9 million.   The company said Thursday that it would use the space for an innovation center.   "We have known for quite some time that we have outgrown our current innovation center in Boulder. This is our best economical option in the area. Our commitment to bring radically innovative bikes, equipment and experiences to riders requires investing in functional, inspiring, and collaborative workspaces for our teammates. We are greatly looking forward to the future of this new innovation center," the company said in a statement to BRAIN.   Pearl Izumi moved into the 55,000-square-foot building in 2013. The custom-built building features locally sourced materials and sustainable features. In 2014 it was honored by the Colorado and Denver chapters of The American Institute of Architects.   Pearl Izumi was owned by Shimano from 2008 until last May, when it was purchased by United Sports Brands, the parent of Nathan, Cutters, and Shock Doctor, among other brands.   In September USB said it was reducing staff and vacating the building.   At the time, Michael Magerman, USB's president and CEO, said the building had been "mostly vacant" since the acquisition because many employees began working from home during the pandemic. Some of the brand's workforce was already dispersed before the pandemic.   "In an effort to maximize efficiencies, USB will move the remaining functions and people to remote work or relocate them from Colorado to USB's California location. USB has already made significant investments into the brand and we intend to invest in and grow the business for many years to come," Magerman said in a statement to BRAIN.   Some Pearl Izumi employees were given the option to relocate to USB's headquarters in Fountain Valley, California, or to work remotely.   Closing: Thanks again for listening in this week.  Please be sure to follow us @303endurance and of course go to iTunes and give us a rating and a comment.  We'd really appreciate it! How Past Extinctions At The La Brea Tar Pit Can Teach Us About Our Climate Future If you drive through Los Angeles, you'll pass by some of California's most iconic sites—the Hollywood Walk of Fame, Universal Studios, the Santa Monica Pier. But if you don't look for it, you may miss the La Brea tar pits—a place where Ice Age life from around 50 thousand years ago got trapped and preserved in sticky black ooze. Visitors can see megafauna, including skeletons of saber tooth cats and dire wolves, along with a vast collection of specimens, including things as small as beetle wings and rodent dung. La Brea was recently named as one of the world's most important geological heritage sites by the International Union of Geological Sciences. The museum is currently planning an extensive redesign that will seek to connect visitors to research, offering lessons about climate, extinction, and survival. Dr. Lori Bettison-Varga, president and director of the Los Angeles Museum of Natural History, joins Ira to explain the significance of the site, and how a trove of Ice Age specimens can serve as a modern-day climate laboratory.   Across The Country, RSV Is Overwhelming Medical Systems If you have a child—or interact with children on a regular basis—odds are you've heard about a very contagious virus: RSV, or respiratory syncytial virus. This isn't a new illness, but it has been surging across the country. This has left parents and caretakers stressed about how to keep their kids safe. Hospitals across the country are having trouble coping with this year's surge, which has come earlier and stronger than normal. This week, Science Friday is spotlighting two regions affected by the wave: Wisconsin and Washington, D.C. The two regions have their own challenges when it comes to the RSV surge. In Wisconsin, care deserts and a large elderly population make containing this virus important to avoid dangerous consequences. In Washington, D.C., hospitals are feeling the effects of years of shutting down pediatric units to make room for adult beds. Joining Ira to talk about RSV in Wisconsin and Washington D.C. are two journalists who have been following this: Jenny Peek, news editor for Wisconsin Public Radio and Aja Drain, reporter at WAMU public radio.   What You Should Know About This RSV Surge Respiratory syncytial virus, known as RSV, is the number one cause of infant hospitalizations in the United States, and cases are soaring this year. Because young children have spent part—if not most—of their childhoods isolated, masking, or staying home due to the pandemic, many of their immune systems haven't been exposed to RSV until now. It's caused a huge surge in cases, and placed a heavy burden on pediatric clinics and hospitals. What do you need to know about the spike in infections? Ira talks with Dr. Carol Kao, a pediatrician and assistant professor at Washington University in St. Louis, Missouri, who has treated RSV for years. They dig into why this surge is happening now, the basics of the virus, how RSV is treated, and where we stand with an RSV vaccine.   Mapping Brain Connections Reinforces Theories On Human Cognition Brain regions are associated with different functions—the hippocampus is responsible for long-term memory, for example, and the frontal lobe for personality, behavior, and emotions. After decades of research using sophisticated brain imaging, there's a growing consensus among neuroscientists that understanding the connections between brain regions may be even more important than the functions of the regions themselves. When it comes to understanding human cognition, the whole is greater than the sum of its parts. Ira speaks with Dr. Stephanie Forkel, assistant professor at the Donders Centre for Cognitive Neuroimaging at Radboud University in Nijmegen in the Netherlands, who wrote a review article in the journal Science about the importance of brain connectivity, and what it means for the future of neuroscience. Transcripts for each segment will be available the week after the show airs on sciencefriday.com.

"We humans are just specks of dust on a slightly bigger speck of dust in the immeasurable vastness of space. We can't cause stars to explode, we don't set the wheels of galaxies spinning, and it is not we who span the vault of heaven above us. But we can marvel at the universe and ask questions about it. We can have faith, hope, and love in this world-and this makes us stardust of a very special kind." —Dr. Heino Falcke We tend to think that science and spirituality are mutually exclusive.
