Friends & Colleagues, The focus of this episode is of immediate importance to all of us. It’s especially important right now, in the month before the inauguration of President-elect Biden & VP-elect Kamala Harris. (Of note, this interview was recorded in late August, so we were unaware of the election outcomes). The topic is ‘fixing’ healthcare and our guest today is Dr. Vivian Lee, the President of Health Platforms at Verily. Dr. Lee recently published a book entitled -The Long Fix - Solving America’s Healthcare Crisis with Strategies that work for Everyone - in which she explains the fundamental problems in healthcare and provides practical solutions. The book speaks directly to policy makers, providers, payers & patients. In this interview, we’ll hit the high points of the book and get a sense of the priority Dr. Lee assigns to certain problems. The book is remarkable and so is Dr. Lee. She is a Harvard-Radcliffe College graduate who obtained a doctorate in medical engineering from Oxford University as a Rhodes Scholar and then earned an M.D. with honors from Harvard Medical School. She has also earned an Executive MBA at NYU’s Stern School of Business. Dr. Lee is a radiologist and medical researcher who has authored over 200 peer-reviewed journals. Prior to her current role, Dr. Lee was the Dean & CEO of the University of Utah Health - one of the nation’s leading hospital and academic medical centers. During her tenure, she and her colleagues achieved numerous nationally recognized accomplishments in quality, safety, patient experience, growth and entrepreneurial commercialization. In 2019 she was ranked #11 among the ‘Most Influential People in Healthcare’ by Modern Healthcare.  She has been elected to the National Academy of Medicine. In this episode, Dr. Lee and I will touch on:A broad range of issues ranging from the mind boggling complexities of billing and payment, to the lack of price transparency, to the crippling and uncontrolled costs of medications, to the impact of healthcare costs on suppressing employee wages and retirement funds.The one issue Dr. Lee calls out as the core off-the-charts problem in the American healthcare system.Why Dr. Lee believes that it’s critically important for all citizens to understand how healthcare delivery works and what it would take to make it better. Why universal access to healthcare is not only the right thing to do from an ethical perspective, but also from a public health, productivity and economic point of view.Why Dr. Lee believes we must shift to value-based care in order for universal coverage/access to be optimally effective.I have to say that it was a privilege and a pleasure to speak with Dr. Lee.  Given her remarkable accomplishments, her brilliance, and the enormity of her current position, I was struck by how down-to-earth and humble she is. We didn’t have the opportunity to directly discuss the issue of leadership in healthcare; but an unspoken message that came across is that we need a leadership culture that Dr. Lee embodies - data-driven, humanistic, socially aware, and selfless - leadership that is, first and foremost, for the people.  Dr. Lee wrote this book for the American public - to bring some clarity to our complicated, opaque healthcare system. Tips for Students Who Are Interested in Pulmonary & Critical Care Dr. Moseson encourages students who are interested in pulmonary and critical care to research and apply for residency programs that offer this specialty, and that have faculty in this specialty. They should do research into the aspects of this field that interest them, and how this matches with a given residency program. Pulmonary Health & the Economy Physicians should work to “make what is invisible visible.” Issues like forest fires are widely visible, and people can see that it is bad for our lungs. However, issues like terrible air quality in certain areas often go unnoticed. Air pollution — particulate matter — has been shown to cause respiratory issues like heart attacks, asthma, strokes, COPD, and more. Therefore, physicians should help to educate the public. Air quality issues often affect the poorest people the most. For example, a house offers a layer of protection from bad air quality. Therefore, the homeless might be affected two to three times more than a housed person in the same area if there is poor air quality. Furthermore, it is no coincidence that poor air quality itself often occurs in areas inhabited by minority groups. Even if the public is not concerned about the health of affected populations, we can use the economic costs of healthcare to convince them. It is more economically beneficial to have healthy people who earn money, rather than those who have to stay home sick and incur exorbitant costs for things like inhalers. Advocacy as a Medical Student Dr. Moseson sympathizes with students who struggle to participate in advocacy for projects about which they are passionate because they are afraid to offend or to step out of line. Nevertheless, Dr. Moseson encourages medical students to learn the science around the topic that is their passion. The students can be an educated voice of reason in their organizations, and provide a valuable scientific perspective on suggested ideas. She also encourages students to participate in research projects, which is not just important for their medical education, but can inform and enhance their advocacy as well. Does the thought of practicing open science give you sweaty palms? That’s a normal reaction for those of us who weren’t formally trained in the open-science methodology. The sweaty-palm reaction is really not that surprising since most of us have gotten where we are today because we’ve been meticulous in our work and tried to put out the best work we possibly could. In a nutshell, we tend to be perfectionists. But science, like life, is far from perfect. It’s messy. And it often takes unexpected twists and turns. Once we embrace this reality and view research as a conversation starter, we’ll be able to move past the sweaty-palms stage. Part of getting comfortable with open-science practices is your mindset. It’s about valuing doing rigorous science, even when it gets messy. Open science is also about creating an environment where feedback is sought and embraced. It’s about learning along the way so that you can do even better science going forward. A note from Talking Taiwan host Felicia Lin:   September is National Suicide Prevention Month. Unfortunately, suicide is a very real issue for the Asian American community. According to the Office of Minority Health, in 2017 the leading cause of death in young Asian Americans in the US was suicide. My guest on this episode of Talking Taiwan is passionate about promoting emotional wellness and a prevention model for mental health.   Dr. Eunice Yuen is, a child and adult psychiatrist who specializes in Asian American mental health. She sat down to speak to me about the stigma of mental health and the reluctance of Asian Americans to seek help. Dr. Yuen noted that prevention through mindfulness of emotional wellness are especially important for teenagers and young adults, which is the age at which mental illness often first manifests.   Dr. Yuen is working on an innovative tool called CHATogether that helps to resolve conflicts between Asian American parents and children. Welcome back to the Neuroscience Meets Social and Emotional Learning podcast, episode #83. This is PART 2 of our last episode on “How Looking at Your Brain Can Change Your Life” with a deeper dive into what we learned from getting a SPECT Image Brain Scan at Dr. Amen’s Clinic in Costa Mesa, CA.For those who are new here, my name is Andrea Samadi,  I’m a former educator who created this podcast to bring awareness, ideas and strategies to our most pressing issues facing educators in the workplace, or parents working from home or in the corporate space, to keep us all working at our highest levels of productivity. The goal is to bring the most current brain research and practical neuroscience, connected to our social and emotional skills, to take our results to the next level.Last episode we did a case study of my friend Doug Sutton, whose whole life changed when he made the decision to get a SPECT image brain scan from Dr. Amen’s Clinics. While Doug was having some noticeable issues with his health, like brain fog and his energy levels, I made the decision to get the scan just to improve and optimize what we are already doing, as well as to see if there is anything they notice that we should be aware of now.  When I was interviewing Dr. Shane Creado, episode 72 on “Sleep Strategies that Will Guarantee a Competitive  Advantage” I wanted to know specific strategies that I could use to optimize my performance and results, and he suggested “why don’t you just go and get your brain scanned. This way, we can look at the results, and see exactly what type of brain you have, and he could make more targeted suggestions from there. Does the test reveal a snapshot of your brain as it is today, or in general?” All of these are great questions, and the reason I figured I would break up the experience into parts.If you have considered getting a brain scan, and you are either like Doug, you’ve been noticing some symptoms (brain fog, maybe memory problems, low energy) and you want to look at the organ that controls pretty much everything that you do, and everything that you are, then this podcast episode series is for you. Or maybe you are like me, you don’t have any symptoms, but you just want to get an accurate snapshot of what is happening in your brain, so you can optimize your results, and at least know what’s going on with the most important organ in your body.So, here’s what I would recommend you do if you are thinking about it. Start with doing as much research as you can about Dr. Amen,[i] Amen Clinics[ii] and The Brain Warrior’s Way Podcast that he and his wife host, to hear from the many guests he interviews, showcasing different ways to put your brain health first.[iii] Dr. Amen, one of the most famous psychiatrists in the country, found that looking at the brain could tell you a lot about someone when treating them. Adding SPECT Imaging Brain Scans to psychiatry, took his practice to the next level.  On his website, you will find all of the locations he has offices (east coast, west coast, and central locations like their Chicago office where Dr. Creado works out of).  There’s a lot of free resources you can access, as you learn more about what issues you might be facing, thinking of solutions that he recommends. Once you have trust in his work, the decision to call and learn more about moving ahead with a brain scan is much easier. He has a wonderful overview of the process on how to read the scans called “Brain SPECT Imaging Made Ridiculously Simple”[iv] on his blog. In this overview you will learn:Why Dr. Amen started doing SPECT brain imaging in the first place.What a SPECT Scan (Single, Photon, Emission, Computed Tomography) looks like.What a healthy brain looks like (full, even, symmetrical activity) from even blood flow. (AMEN CLINICS photo of a healthy brain-even blood flow)What an unhealthy brain looks like, spotting signs of Alzheimer’s Disease ahead of time, so you can work on optimizing the brain.Alzheimer’s can be seen in the brain years before symptoms occur. Both sides of the brain are affected with decreases in the Parietal Lobe (top back part of your brain) and Temporal Lobes.What drugs or alcohol do to the brain? They damage the brain.Photo of a healthy brain, next to an addict's brain.6. What even a mild traumatic brain injury can do to the brain.7. How SPECT scans show evidence of trauma, toxins like mold or chemotherapy, or infections like Lyme disease. My friend Doug Sutton spoke about mold exposure that gave him brain fog, and he also mentioned that BulletProof Radio Dave Asprey also had mold exposure, and it was Dr. Amen who inspired him to create his BulletProof line of products.8. How SPECT scans can help people with mental health disorders and addictions.This drug education poster hangs in 100,000 schools.  It’s a more realistic image of what drugs do to your brain than the image that I remember growing up of an egg cracking into a pan to represent a brain on drugs. It just doesn’t have the same impact as this poster does, or real brains that were damaged by drugs/alcohol use.What the scan tells us? Good activity, (blood flow) Too little activity, Too much activity. Then Dr. Amen and his team can balance the brain with supplements, and strategies.The difference between a surface view of the brain, and active view (inside) the brain.If you want to see a more humorous overview of the SPECT scan, actress and comedian Laura Clery covers her SPECT scan from start to finish.[v]  I’m usually a pretty serious person, especially when it comes to getting my brain scanned, so this overview of a very serious topic, was hilarious to watch. Actress Laura Clery was having some problems with her memory and wanted to look and see if there was anything they would find out with this scan. She admits that she is a former drug user and hoping that the damage she did in her early years is reversible, which we have heard Dr. Amen say when he says “You’re not stuck with the brain you have.” It is possible to improve brain structure and function. She walks you through the experience from start to finish, not missing a detail, of finding the clinic, to the IV injection of Saratech that maps out your brain, to the test where you have to click the space bar when you see a letter, and not click the bar when you see an “x” which was more difficult than it sounds, (my husband said this test drove him crazy and it looks like Laura had a similar experience). She also takes you through the actual brain scan where she was told to lay still, but she moved, and had to do the entire scan over again.   The Brain Scan ProcessIt’s interesting that everyone asks the brain scan technician “what did you see” after the scan, and he/she will tell you “wait until you speak with the doctor.” Once your scan is complete, you will have the opportunity to discuss the results with a doctor. I chose Dr. Creado from the Chicago offices to do my evaluation, since he was the one who recommended I take the test in the first place, so I won’t be able to record PART 3 of this episode with the results until the end of next week. Brain Scan Predictions Based on My Understanding of the BrainI do have some predictions of what I think we will see, based on studying Dr. Amen’s work for the past year, and applying what I am learning to my own life. You can do the same, especially if you take his Thrive by 25 Online[vi] course. It will be interesting to see if my predictions are correct, but this is what I think we are going to see.With my brain scan, I think that I have what he considers a busy brain, or hyperfrontality where we can be over focused. He suggests that a way to improve this would be to prevent serotonin from going too low, or I will have problems, which I kind of figured out on my own over the years. He suggests to naturally increase serotonin through exercise, high protein foods and supplements. I already know that without exercise, I just don’t work well, because I need this boost in serotonin to work at my best. I also eat a high protein/higher fat diet which also works well for me. He also suggests taking supplements to calm the busy brain like 5 HTP or saffron, and I have been doing this for the past year.For my husband’s brain scan, I think that he will show hypofrontality, or lower frontal lobe activity, which is the opposite of mine. He talks all the time about having a shorter attention span, and wonders if he has ADHD. He did have a mild traumatic brain injury years ago, and he works best with scheduled exercise and a higher protein diet. When our minds become hyper regulated, our body shuts down and goes into survival mode. Today, this can happen all of the time - we are firing off these hormones constantly. I always want to know why something is happening. As I was beginning my health journey, I searched for a practitioner who would spend the time, ask the hard questions to get to the root of my complaints and try to explain why I was not feeling as well as I could. For some of us, sheltering in place with others allows us to turn to one another for emotional and physical comfort. However there are millions of people across the globe who are sheltering in isolation or in unsafe or unsupportive environments. They are missing another persons comforting support and touch. David speaks personally from this experience because he is currently sheltering in place alone - that is, away from his family - so his advice is coming from a place of knowing. Elliott and his colleagues have developed a model for the treatment of early attachment and healing called the 3 pillars model discussed in detail in episode 34. In today's episode we discuss the tenets of the first pillar - the power of the mind to imagine - the essential elements of connection and security needed for coping during this COVID quarantine crisis. Ann & David discuss the empirically studied outcome of the power of imagination to generate learning and neurological growth. Todd Kashdan, PhD, is a professor of psychology at George Mason University, where he’s senior scientist at the Center for the Advancement of Wellbeing. He’s been a leading researcher in  positive psychology from when that area first started to blow up, but often plays the role of someone who challenges established wisdom in that area of research. When looking for the right job, Dr. Kashdan sought the one that would give him the most autonomy in his work. He’s delighted that at George Mason he’s been able to follow his interests. As he gleefully states, he “gets paid to read books, write articles, and study whatever he wants,” which includes but is not limited to gratitude, positive emotions, spirituality, purpose, curiosity, creativity, resilience, and anxiety – just to name a few. Dr. Kashdan doesn’t shy away from controversial, and that’s, at least partly, by design. His mantra is, whatever is the zeitgeist of the moment – be it mindfulness, positivity, etc. – there must be a psychological benefit to the opposite. That’s why you’ll frequently hear him arguing for the very opposite of whatever is the popular message of the day.   In today’s episode, Dr. Kashdan and I dive deep into his controversial side. He shares the story of the impromptu speech for which he was “banished” as a speaker from a well-known positive psychology organization. He shares lessons he’s learned on softening his approach and explains why he’s not about to give up on speaking out. Networking Years ago as the internet was becoming more popular, networking online was easy and convenient. Fast forward to today, we’re bombarded with information everywhere we look. The attributes that make you stand apart are what make you who you are and what make up your personal brand. When you’re looking to make a job switch, you want to make sure you’re noticed and taken seriously. This starts by networking effectively and building your personal brand.
