Dr. Sabrina Strickland is board certified in sports medicine and orthopedic surgery at HSS, where she specializes in patellofemoral surgery for instability, cartilage disease, ACL and meniscal surgery and shoulder surgery. She completed her residency and fellowship in sports medicine at HSS after completing medical school at Rush in Chicago. Prior to that, Dr. Strickland received her BA from Cornell University. Currently, Dr. Strickland is an associate professor of orthopedic surgery at Weill Cornell Medical College. She retired from her part-time position as the Chief of Orthopedics at the VA in 2012 after seven years of service. Dr. Strickland is also the co-founder of the HSS Patellofemoral Center. In one case, a healthcare company came up with a strategy and deployed it; and the strategy didn't go as planned. The other one, it did go as planned—it worked great. Of course, I'm coming in on the back end like a Monday morning quarterback here; but the plan that failed, I have to say, I wasn't surprised. Had they asked me ahead of time, I would have told them to save their money because the plan was never gonna work, even though the strategy looked like kind of a straight line from here to there. Nor was I shocked by the success of the other plan, even though this one that triumphed had what looked like five extra steps and was slightly counterintuitive if you looked at it cold, without understanding the way the healthcare industry actually works. Here's my point: It might feel like the healthcare industry is chaos monkey central and impossible to predict actions and reactions—and, for sure, there's always unknowns and intersecting variables—but it's not a complete black box. The trick is, as you know and I know, you gotta understand what other stakeholders are up to. You gotta get a bead on what they're doing and what their incentives are because then you can better predict actions and potentially reactions. So, let me state the obvious (that's why listeners tune in to this show as I just said, and it's what we aim to shine a light on here at Relentless Health Value): the pushes and the pulls and the forces. What's going on outside of the organizations or the silos that we work within day-to-day. Because if you're looking to sell to, partner with, not be obstructed by [insert some stakeholder here], then it's very vital to be keyed in on what they're doing or what their customers are doing or what their customers' vendors are doing. This show should feel like it gives you a measure of control (or at least that's my hope) or a method to find the measure of control. And I hope you succeed. That's why I continue to put out these shows. The RHV tribe members want the same thing I want—to fix the healthcare industry for patients and for members—so, thanks for being here and for making actionable the insights that you might find here. I have been so looking forward to doing a show with Ben Schwartz, MD, MBA, orthopedic surgeon and prolific writer of deeply thoughtful and insightful posts on LinkedIn. In this healthcare podcast, we are talking about bundled payments. And today's your lucky day if you think you know a lot about bundles, because most people who listen to this show at least know enough to be dangerous. So, that's our starting point, which is why I asked Dr. Schwartz to talk to me about what most people find surprising about bundles and bundled payments. There are four surprises that we go through in the show today. Listen to the show or read the transcript to find out exactly what they are. So, no spoiler alert alert. But relative to these surprises, we get into the four types of bundles that may or may not be available. And those four types of bundles are: 1. CMS bundles such as the BPCI (Bundled Payments for Care Improvement) and the CJR (Comprehensive Care for Joint Replacement) bundles, and we talk about the current state of said BPCI bundles, which are being sunsetted probably because so many efficient clinical teams are being penalized for getting too efficient. They become victims of their own success the way the program is currently designed, wherein the goalposts keep shifting. 2. Commercial bundles—ie, a bundle that is offered by a commercial carrier such as a BUCA (ie, Blue Cross Blue Shield/UnitedHealthcare/Cigna/Aetna/Anthem) carrier 3. Direct bundle—a bundle that is paid for directly by a plan sponsor such as a self-insured employer 4. Condition- or diagnosis-specific bundle. These types of bundles do not spiral around a surgical intervention at their core, which most of the current bundles do. This may describe CMS's recently announced “Making Care Primary” initiative, but we'll have to see about that. Speaking about the #3 kind of bundle, the employer-direct bundles, especially for musculoskeletal (MSK), let me share a post by Moby Parsons, MD, that I thought captured the entrepreneurial spirit of some of these orthopedic surgeons who are seeking employers to direct contract with and cut out the middleman, etc (which, by the way, is the main topic of an entire show upcoming with Elizabeth Mitchell from the Purchaser Business Group on Health). But Dr. Parsons wrote: “When our bundle business has sufficient growth to ensure the absolute sustainability of our practice against declining reimbursements … in a fee-for-service system, I am getting this tattoo. Don't tell my wife. [And the tattoo is ‘Free Yourself.']” My guest today, aforementioned, is Dr. Ben Schwartz. He's an orthopedic surgeon in the Boston area still in full-time clinical practice. He's grown very interested in healthcare innovation, healthcare technology, and does some advising and investing. Dr. Schwartz also writes a great Substack called Dem Dry Bones. Dr. Gomoll is a Professor of Orthopedic Surgery, a Fellow of the American Academy of Orthopaedic Surgeons, the American Orthopaedic Society for Sports Medicine, and the International Cartilage Repair Society. He holds leadership positions in these and other US and international professional societies, as well as being on the editorial boards of several orthopedic journals. He has been awarded several Best Doctors designations, is a Castle Connolly Top Doctor, recipient of the Who is Who Humanitarian award, and was recognized as one of 16 stand-out sports knee surgeons in North America. According to the American Cancer Society, about 14,000 cases of invasive cervical cancer in the United States are said to be diagnosed. This statistic can bring so much fear, which warrants education on screening and preventative modalities. Within the gynecologic realm, another root of wary comes from the terms sexually transmitted disease, STD, or STI. When uttered, the room usually goes quite silent. Behind these words, spells of stigma and prejudice remain, which can result in the avoidance of these topics. However, when pertinent things are unspoken, mayhem usually arises due to the lack of knowledge and necessary precaution. This is why we tackle these polarizing topics today: human papillomavirus, the Gardasil vaccine, herpes, safe sexual practices, reproductive anatomy, cervical cancer, colposcopy, and Pap smears. Dr. Jason Cormier of Lafayette Surgical Specialty Hospital joins Discover Lafayette to share his phenomenal story of success as well as overcoming trials and tribulations. He is not only a renowned neurosurgeon, but he is also a race car driver, accomplished musician, inventor, and author of the newly released "Driven" which he wrote to encourage others on ways to overcome seemingly impossible obstacles. A native of Lafayette, Dr. Cormier is a graduate of St. Thomas More. At LSU, he played point guard alongside Shaquille O'Neal and Chris Jackson under the tutelage of Coach Dale Brown and played pro ball for a brief time in Europe. He went on to graduate from LSU Med School and was a neurosurgery resident at the University of Alabama at Birmingham for six years where he trained in complex spinal surgery.  Dr. Cormier has been named one of America's top Surgeons in the field of neurosurgery by the Consumers Research Council and Guide to America's Top Surgeons and has worked extensively with Traumatic Brain Injuries and assists NCAA Football, the NFL, NASCAR, the Jockey's Guild, various rodeo organizations and other sports representing athletes in high-contact sports.   Since 2014, he has been listed in the “Leading Physicians of the World” for Neurosurgery, and recognized as an international expert in brain injury and concussion. Humble in the recounting of his story, Dr. Cormier says, "No one stressed education more than my mother" who taught science at UL-Lafayette and at Lafayette High for many years. He wanted to play in the NBA but when his career was cut short his mom pushed him to pursue higher education. Always being good in science, medicine was a natural fit. Specializing in complex brain surgery which includes vascular neurosurgery and arteriovenous malformation (a tangle of blood vessels connecting arteries and veins, which disrupts normal blood flow and oxygen circulation), Dr. Cormier says he handles about 95% of the aneurisms that come into Lafayette. He also performs minimally invasive procedures to take out tumors in the brain and treats brain trauma. Dr. Jason Cormier is proud of the team he works with at Lafayette Surgical Specialty Hospital and gives them much credit: "It's a team effort. You can't do it alone in a silo as one human being. You need a team. I tell every single patient I operate on that I have a good team backing me up and that I have the good Lord. I believe that from start to finish when you walk in the door. Buffy Domingue, CEO, and her administrative team are also a big part of that." Dr. Cormier credits Lafayette Surgical Specialty Hospital for being the first to bring in the MiRus IO Expandable Lumbar Interbody technology which allows him to perform less invasive spinal neurosurgery on people suffering from degenerative disc disease. Cleared for use by the FDA in August 2021, the MiRus IO allows the surgeon to enter collapsed disc space and expand it by inserting the device into the disc. This minimally invasive procedure calls for a 26 mm incision versus the traditional 26-inch incision. When operating in a small corridor such as allowed by the MiRus IO, outcomes are dramatically changed for spinal surgery patients. There is less blood loss, the patient can get up and walk on the same day as the procedure, and there is a reduced need for pain medications. The MiRus IO Expandable Lumbar Interbody Technology utilizes MoRe® Rhenium-based Superalloy technology, a noncorrosive metal that is the strongest in the world. It is non-corrosive and far superior to nickel or iron inserts which can cause pain in patients. Dr. Cormier says,

I am a super fan of Dr. Mudgil and we had a deep conversation about this current 'temporary' new normal. This interview is part of our HIMSS18 coverage and was recorded live in the exhibit hall. On this episode we talk with Stuart Goldberg, MD, Chief Scientific Officer at Cota. Dr. Goldberg tells us all about Cota's physician-led journey to organize and classify real-world evidence about every type of cancer. This effort led to something they call the Cota Nodal Address (CNA), and it's enabling health systems to get very specific about the types of patients they're treating, the costs associated with them, and the outcomes being achieved. This apples-to-apples comparison allows Cota to identify variation and address it in ways that are beneficial to the provider, payer and patient. It enables a concept Dr. Goldberg calls "Precision Payment", which in turn enables proper bundled payment design, and precision medicine.  And Cota is not only physician led, but the payers are on board too. Literally. Horizon Blue Cross Blue Shield lead their Series A funding round. Plus, they just closed a $40 Million Series C funding round in February. Cota is clearly a company to watch. This was one of the most intriguing conversations I had at HIMSS '18 and one that I'll be exploring some more.
