En este episodio abordo la farmacología en neurorrehabilitación del adulto desde una perspectiva clínica y realista, pensada especialmente para profesionales no médicos que conviven a diario con informes, pautas y nombres de fármacos sin disponer siempre de un marco claro para interpretarlos. Recorremos los principales medicamentos utilizados en patologías neurológicas frecuentes —ictus, lesión medular, esclerosis múltiple, enfermedad de Parkinson, ELA, distonías y traumatismo craneoencefálico— diferenciando entre tratamientos agudos, terapias modificadoras de la enfermedad y manejo farmacológico de secuelas. A lo largo del episodio explico de forma progresiva los mecanismos de acción, la base neurofisiológica y el estado actual de la evidencia, poniendo especial énfasis en qué fármacos realmente cambian el pronóstico y cuáles cumplen un papel fundamentalmente sintomático. El objetivo no es prescribir, sino entender mejor cómo la farmacología condiciona la recuperación, la participación en terapia y la toma de decisiones en neurorrehabilitación, con una mirada crítica y basada en la evidencia disponible. The clearing price of optionality in the market is impacted by a myriad of factors. To be sure, the economy and corporate profit cycle, the stance of monetary policy and geopolitical risk all matter. Flows in the derivatives markets are also of consequence as they impact the environment for supply and demand. And in this context, it was excellent to welcome Benn Eifert, Founder and Managing Partner of QVR Advisors, back to the Alpha Exchange.Benn brings a data-driven lens to today's complex volatility markets, and in this conversation, he discusses the unintended consequences of crowded risk-managed option strategies, such as buffer ETFs and overwriting. We explore how structural flows have drained the volatility risk premium, especially from the front end of the curve where the flow has especially been one way, leading to substantial optionality being sold into the marketplace. Benn states that since 2012, the VRP has largely vanished, leaving the returns to owning short-dated straddles actually positive since then. For risk-managed ETFs, the implications are unfavorable as many of these strategies often reduce beta without providing meaningful downside protection, results that are posted on the QVR website. From Benn's perspective, even for a large and liquid options complex like the S&P 500, the sheer volume of capital chasing the same trade is distorting expected returns.We finish the conversation by exploring the evolution of QVR's trade implementation and risk management practices. Here, Benn shares that the listed options market has become a focal point relative to years ago when OTC products transacted bi-laterally with large banks were of greater importance. Inzwischen hat sie sich in der Gendermedizin spezialisiert und dort ihre Sichtweise auf das Herz vertieft. Zusammen mit der Ärztin Suzann Kirschner-Brouns hat sie nun ein Buch herausgegeben mit dem Titel "Herzsprechstunde – Warum das WEIBLICHE HERZ anders ist und wie es gesund bleibt". Ihre These: "Das Frauenherz ist kein kleines Männerherz". Geschlechterunterschiede in der Medizin, so erfährt man im Buch, wurden über viele Jahre vernachlässigt. Die Gendermedizin ist wichtig geworden, besonders bei Herz-Kreislauf-Erkrankungen, aber auch in anderen medizinischen Bereichen. Im Gespräch mit Niko Härting erläutert Stefan Brink die Rolle und Funktion des Europarats und berichtet über die eklatanten Eingriffe in Persönlichkeitsrechte der Leistungssportler, die im Zeichen des Kampfes für einen „sauberen Sport“ zu deren beruflichem Alltag gehören. All dies auf der Basis – angeblich – „freiwilliger“ Einwilligungen der Sportlerinnen und Sportler. Ab Minute 12:20 : Martin Eifert, Professor für öffentliches Recht und Verwaltungsrecht an der Humboldt-Uni Berlin ist seit Februar 2023 (auf Vorschlag der „Grünen“) Richter des 1. Senats des BVerfG. Im April erschien in der JuristenZeitung ein Aufsatz, den Eifert gemeinsam mit Nora Wienfort verfasst hat (JZ 2023, 270 ff.). Es geht um „Hassrede als Gefährdung der verfassungsrechtlich geschützten offenen Kommunikation unter Freien und Gleichen“. In diesem Aufsatz wird versucht, die Meinungsfreiheit (Art. 5 Abs. 1 GG) durch einen „Schutz der Entschließungsfreiheit zur sozialen Exponierung“ als Ausprägung des Allgemeinen Persönlichkeitsrechts zu ergänzen, um auf diese Weise „Einschüchterungseffekte“ von „Hassrede“ verfassungsrechtlich zu erfassen. Dies läuft auf eine Einschränkung der Meinungsfreiheit im Zeichen des Schutzes des „öffentlichen Diskurses“ hinaus und liegt auf einer Linie mit der Rhetorik, die man seit dem Netzwerkdurchsetzungsgesetz (NetzDG) verwendet, um gegen „lawful but awful speech“ vorzugehen. Tendenziell schränkt man die Meinungsfreiheit lautstarker Minderheiten im Interesse des ungestörten „öffentlichen Diskurses“ der Mehrheit ein. Ab Minute 30:10 : Ferdinand Kirchhof, emeritierter Professor für öffentliches Recht und von 2010 bis 2018 Vizepräsident des BVerfG, hat sich in der NJW unlängst mit dem „Wirkungsbereich des Grundrechts auf Meinungsfreiheit“ befasst (NJW 2023, 1922 ff.). Kirchhof ist ein Konservativer. Anders als der linksliberale Eifert sieht er die Meinungsfreiheit weniger bedroht durch „Hassrede“ als durch „Zensur aus den Reihen der Gesellschaft“, „Political Correctness“, „Cancel Culture“ und das, was Kirchhof unter „Wokeness“ versteht. Ähnlich wie Eifert sieht Kirchhof die Meinungsfreiheit weniger durch den Staat bedroht als durch „durchsetzungsstarke Minderheiten“. Ähnlich wie Eifert beklagt er einen unzureichenden Schutz des „Publikums“ vor lautstarken Minderheiten. Die Ähnlichkeit des Denkens von Eifert und Kirchhoff ist frappierend.

Serious, professional investors and traders lost huge sums of money giving out unsecured loans to crypto hedge funds like 3AC, which then proceeded to incinerate their money. In other words, lots of people, with a range of sophistication, threw out some basics of risk management wholesale. Whether stemming from a recession, a policy shock, a terrorist attack, or a global pandemic, financial markets periodically go through major selloffs. It has always been this way. Euphoria eventually leads to panic, every time. Another way to look at these crashes is through the lens of volatility (and its commensurate rise), which has become a fundamental component of investment decisions. As time goes by, innovations can (and will) suppress volatility and can sometimes give the impression that risk has been eliminated, but that is always a fool's dream. Eventually the piper must be paid. Our guests this week were Benn Eifert (Founder & CIO) and Scott Maidel (Head of Business Development), of QVR Advisors, a boutique asset manager that specializes in strategies seeking to profit from volatility and all its downstream effects – including attempts to suppress it.
