Contributor: Aaron Lessen MD Educational Pearls: Pregnant patients at high risk of cardiac arrest, in cardiac arrest, or in extremis require special care A useful mnemonic to recall the appropriate management of critically ill pregnant patients is TOLDD T: Tilt the patient to the left lateral decubitus position This position relieves pressure exerted from the uterus onto the inferior vena cava, which reduces cardiac preload If the patient is receiving CPR, an assistant should displace the uterus manually from the IVC towards the patient's left side O: Administer high-flow adjunctive oxygen  L: Lines should be placed above the diaphragm Lines below the diaphragm are ineffective due to uterine compression of the IVC May consider humeral interosseous line vs. internal jugular or subclavian central line D: Dates should be estimated > 20 weeks, can consider a resuscitative hysterotomy (previously known as perimortem c-section) to improve chances of survival The uterus is palpable at the umbilicus at 20 weeks and 1 cm superior to the umbilicus for every week thereafter D: Call the labor and delivery unit for additional help References ACOG Practice Bulletin No. 211 Summary: Critical Care in Pregnancy. Trechos do livro “Nama-Yoga”, de Bhausaheb Maharaj. Bhausaheb Maharaj ou Venkatesh Khanderao Deshpande (1843 - 1914) foi o fundador de sua tradição “Inchagiri Sampradaya”, difundida por seu discípulo Siddharameshwar Maharaj, mestre de Nisargadatta Maharaj e de outros grandes professores. Nascido em Umdi na Índia, ainda criança, Bhausaheb desenvolveu uma profunda devoção ao Senhor Hanuman e costumava realizar adoração diariamente em um templo próximo. Aos quatorze anos conheceu seu guru Sri Nimbargi Maharaj. Com um coração bondoso, Bhausaheb dedicou sua vida ao seu Sadguru e a Deus. Em 1875, após seu despertar, ele foi autorizado por Nimbargi a continuar sua linhagem. Ao partir, seu Mestre Nimbargi lhe pediu para espalhar os ensinamentos por toda parte. Assim, Bhausaheb estabeleceu seu Ashram em Inchgiri, uma vila perto de Umadi, e começou a realizar suas atividades espirituais. Logo, Inchgiri tornou-se famosa e um local de atividade espiritual. Os ensinamentos de Bhausaheb Maharaj são chamados de Jnana-Marga e Nama-Yoga. WARNING: THE INFORMATION BELOW MAY BE TRIGGERING1.64 lakhs, a daily average of 450 or 18 per hour…..That is the number of deaths due to suicides in India in 2021The statistics for death by suicide amongst young people is worse. Not surprising then that this is the third leading cause of death amongst young adults and a significant problem amongst college students in India. Mental health can bye impacted due to a myriad of factor amongst young people. The two big issues that stand out are a) Stigma around mental health, it is a taboo subject that individuals and families are reluctant to engage with. 2) An education system that is not knowledge oriented but exam focused - testing students on their ability to ace the system.In a post independence India there has been a rise in social capital attached to educational attainment (because it leads to acceptable jobs, financial and material gains). Indian students face some of the toughest competitive examinations in the world. And just to share an example - with less than 1% acceptance rate the Indian Institute of Technology (IIT) exam is said to be tougher than getting in Harvard. A million students, yes that is right 1 MILLION students apply for the 13000 seats. The pandemic of course has exacerbated the issue. Peer pressure, all consuming access to media, parental pressure, fear of failure, lack of access to opportunities all compound the issue. With prominent Bollywood celebrities and more young people speaking about the issue - there has been some progress. But, by and large the issue remains a taboo and largely undiagnosed.In this episode of The Elephant in the Room podcast I spoke with Dr Lata Dhir Prof of Organisational Behaviour & Leadership and Group Head of Design Thinking at SPJIMR about the deep challenges we face in addressing this hidden crisis. As the cost to bring new therapeutics to the market continues to soar, it is more important than ever for Life Sciences companies to develop robust Market Access strategies. Mindy & Ryan are joined by Vynamic's Raina Dhir and Maggie Gorman to discuss the criticality of Market Access in ensuring successful drug launches and how Life Sciences organizations can ensure they avoid pitfalls and are set up for success. There are many forces disrupting the Life Sciences marketplace that have led to increased complexities around bringing new drugs to market. Payors and Pharmaceutical Benefit Managers have undergone a period of marketplace consolidation, changing the structure of payments and formulary negotiations. We've also seen the emergence of larger ‘payviders' – or integrated provider and payor groups - where the stakeholder is uniformly focused on cost-effectiveness. These payviders have rolled out prohibitive utilization management programs to curb spending on new therapeutics. Compounding this scrutiny on pricing is the increasing cost of specialty drugs as well as cell- and gene-therapies. All these factors clearly spell out the need for Life Sciences organizations to put a greater emphasis on the Market Access function (04:30). Planning needs to happen early and teams need to form robust yet agile strategic approaches to navigate the ever-shifting landscape (12:43). Social media has come to stay as one of the major means of socializing and the current generation of young adults are at the frontline of anything social media-related, from money-making, to stardom, endorsements, learning, entertainment, you name it. But then for most of these young adults whose brains, according to research, are yet to be fully developed, finding social solace on social media can spell doom if not properly checked. According to Idiom.thefreedictionary.com, a person can be described as a blank canvas if they project themselves as having nothing of substance or permanence within or imprinted upon them, such that they can be easily filled with entirely new things. It refers to a painter's canvas that has yet to be painted on. I am not debating for or against social media. A lot of research has highlighted its advantages (Brown and Michinov, 2019; Tefertiller et al., 2020, Twenge and Campbell, 2019) and disadvantages (Dhir et al., 2018;  Tonacci et al., 2019). What I am really saying is for every time you want to visit social media for validation, don't ‘blank' your canvas. Rather, show up with your canvas already painted with a clear definition of who you are, what you stand for and your intention to grow on that path even though you are keeping an open mind to things. Remember that most of what you see on social media has been refined to look that way, which may be far from reality. If you want to be real and authentic, don't ‘blank' your canvas because you want validation, it can lead to another kind of problem for you in the long run.  As the world celebrates this year's International Youth Day and tries to bring more attention to the youth and their peculiar needs, I want to join in this celebration and at the same time challenge every youth out there to understand that the world needs their peculiar brand of originality, their special uniqueness. Language and Learning Foundation (LLF) is an education nonprofit dedicated to addressing the foundational learning crisis that collaborates with the national and state governments in India. Foundational skills such as reading with comprehension, writing independently, and doing simple subtraction are gateway skills that must be acquired and mastered for all future learning in schools. Dr. Dhir Jhingran, the founder of LLF, is a part of a 12-member National Steering Committee which is formed for developing National Curriculum Frameworks as per the perspectives of NEP 2020. We had the pleasure of interviewing Elephant Stone over Zoom video!Montreal-based modern psych-pop band Elephant Stone announced their new EP Le voyage de M. Lonely dans la lune, out now via Elephants On Parade.​​In Elephant Stone, Dhir has woven stories of loss and grief, existential crisis and rebirth, and a world withering away, all set to elegant, intense droning music. What Dhir knows – and something so many of us can relate to over the past two years – is presented with such clarity on Le voyage de M. Lonely dans la lune. The EP is sung entirely in French, a departure for the English-speaking band. The record is what Dhir calls a “love letter to Montreal, Quebec, and all of our French-speaking fans around the world.” While Dhir, a first generation Indian immigrant with very strong cultural ties to the motherland, was born and raised in Montreal, he's always had a strong connection with French-speaking friends and fans. Elephant Stone was born out of a need for refuge for Dhir. After playing in bands such as The Datsons and The High Dials, he picked up the sitar and began writing about personal experiences, leading to a carved out niche in the modern psych-rock genre by infusing it with some pop sensibilities. As a highly-regarded sitar player, Dhir has also collaborated with indie-rock icons like Beck, and legendary cult bands like The Brian Jonestown Massacre, The Black Angels, The Soundtrack of Our Lives, The Dream Syndicate, and many more. Dhir's journey with music, in and out of Elephant Stone, was wrought with trying to find a place that fit him until he decided that what he made was worth sharing in the space that he created for himself. Although the societal narratives of the time placed strict limitations on many young women, Maya, with the blessing of her parents, left home to study engineering in Australia at the age of 15.Armed with a deep sense of empowerment and the courage to step into the unknown, Maya vigorously pursued her dreams. She earned a Masters of Engineering from the University of Pennsylvania and pursued a Doctorate in Computer Engineering from Carnegie Mellon University, before ultimately landing a role as a trader on Wall Street.After getting married and starting a family, Maya's life suddenly took an unexpected turn when her husband was diagnosed with Stage 4 Non-Hodgkin's Lymphoma. A favourable GDP data print for the first quarter of the financial year 2021-22 resulted in a strong positive opening for the benchmark indices, which pushed them to new record high levels. But soon profit-taking ensued, driving both Sensex and Nifty lower for the day. Furthermore, a weak manufacturing PMI print and moderation in GST collections in August also dampened the mood. The GST mop-up in August stood at Rs 1,12,020 crore as against Rs 1,16,393 crore in July according to the data released by the finance ministry. Meanwhile, the factory activities lost momentum in August due to Covid-induced curbs and rising input costs, showed IHS Markit Purchasing Managers' Index (PMI) as the figure fell to 52.3 in the month from 55.3 in July. Amid this backdrop, BSE barometer Sensex cracked 580 points from a record high of 57,919 touched to end at 57,338, down 214 points. The NSE Nifty closed the day at 17,076, down 56 points. The index had touched a new high of 17,226 in the intraday session. In the 30-pack Sensex, 18 stocks closed in the red with M&M, Tata Steel, Bajaj Finserv and TCS as top losers. Asian Paints, Nestle India, Axis Bank and DRL were the best performers. The broader markets outperformed the benchmarks, with BSE Midcap closing near record-high levels, up 0.92 per cent. The BSE Smallcap index added 0.22 per cent. On the sectoral front, Nifty Metal and IT indices bled over 1 per cent each, with the former snapping its three-day winning run. On the other hand, Nifty Realty surged over 5 per cent and was the best performing index. The auto index ended flat amid a mixed bag show in August month and as companies flagged chip shortages. In stock-specific news, shares of AU Small Finance Bank rebounded nearly 7 per cent after a 13 per cent crash on Tuesday as the management clarified on the recent exits in the bank, including on the resignation of internal audit head, Sumit Dhir.  The bank in an exchange filing on August 31, 2021, said Dhir resigned following personal reasons. The scrip recouped gains and ended 2 per cent higher on BSE. Shares of Oberoi Realty surged 11 per cent to Rs 778.05 on the BSE following the announcement of the inclusion of shares in the F&O segment, effective October 1. Likhita Infrastructure rose 4 per cent after the company received orders worth Rs 145.86 crore from Indradhanush Gas Grid for laying & construction of steel gas pipeline and terminals along with associated facilities for sections 5 & 9 of the North-East Gas Grid (NEGG) Project. BHEL added nearly 5 per cent as the company said it has secured the largest ever order worth Rs 10,800 crore from NPCIL. Meanwhile, in the primary market, the IPO by Ami Organics sailed through amid a strong response from retail investors and QIBs. Overall, the issue was subscribed nearly 2 times at the end of Day 1. Vijay Diagnostic IPO however started off on a weak note as the IPO garnered only 30 per cent bids on the first day of issue. Both IPOs close on Friday. Now, going into trade on Thursday, stock-specific activity and global cues will continue to influence market trajectory. Auto stocks will remain in focus amid the release of August sales figures. Oil market companies will also hog the limelight amid the outcome of the Opec+ meet due later today. Investors must also brace for higher volatility amid the weekly F&O expiry.

Formed in Montreal in 2009, Elephant Stone is the brainchild of vocalist, bassist, songwriter, and sitar player, Rishi Dhir. Elephant Stone have toured extensively throughout North America and Europe and are prominent members of the burgeoning psych rock scene. Dhir has recorded, performed, and toured with indie rock icons (Beck), legendary cult bands (The Brian Jonestown Massacre), and contemporary psych titans (The Black Angels and The Horrors).Released in 2009 on Dhir's own Elephants on Parade label, Elephant Stone's debut album, The Seven Seas, was nominated for the 2009 Polaris Music Prize, which honors the best full-length Canadian album. In 2013 Elephant Stone released their second full-length LP, Elephant Stone. The album received critical acclaim from NPR, Brooklyn Vegan, Consequence of Sound, and other important media outlets.
