This week on HairPod, I sat down with an expert in all things hair loss, Dr. Angie Phipps. She is a hair transplant surgeon with over two decades of experience in the industry, and she was the star of the TLC reality series, Bad Hair Day. She joined us to shed some light on an important issue that affects over 100 million Americans: male and female pattern hair loss. Female vs. Male Pattern Hair Loss Female pattern hair loss is different from male pattern baldness in a few key ways. They are both caused by the impacts of hormones on different receptors throughout the body, and these hormones cause hair loss when those hormones affect our hair follicles in certain ways. In men, this type of hair loss centralizes on certain areas, like the forehead and top of the crown. Women can experience thinning in any zone, and it typically happens in a more diffuse way. For both men and women, the amount of hair loss you'll experience due to this condition is genetic, and science doesn't have a consistent way to shut down the gene or the receptors. How Effective is Finasteride? Male pattern hair loss happens due to one specific hormone, and science has found a way to block that hormone without any negative side effects. That treatment is called finasteride, and it helps many men stop their hair loss. Women, on the other hand, experience hair loss due to a combination of hormones, and there are negative effects to blocking them. This means that hormonal treatments are only really helpful for men at this time. For men who don't experience great results with finasteride, Dr. Phipps typically prescribes dutasteride. In either case, the medications must be continued to block the hormone. If the medication is stopped, hair loss will resume. Finasteride as a Preventative Measure Genetic hair loss can skip generations, but if it's somewhere in your family history, you may very well experience it. Dr. Phipps recommends starting the medication early so that you don't experience extensive hair loss before stopping it. While there are always risks for medication to cause side effects, finasteride has been studied long-term and it has not had major long-term side effects on any internal processes or organs.  Is Finasteride Right for Me? Regular hair shedding is normal, so how do you know if you're experiencing hair loss? It's normal to lose 100-150 hairs per day, so if you see a bit of hair in your comb every day, there's no need to panic. But if you start to see an increasing amount of hair coming out in your brush or in the shower, there's no harm in consulting with a professional. They have the tools and knowledge to determine whether you're experiencing hair loss. If you think you're experiencing hair loss and hair loss runs in your family, Dr. Phipps suggests starting preventative care early. But hair loss can be caused by other factors, and a hair loss specialist can give you an idea of what is causing the hair loss. Visit your primary care provider for additional medical advice.Insights into Cervical Cancer.Chantal: Cervical cancer stands as the most prevalent form of cancer in women globally costing the lives of approximately 350,000 women annually. About 4,000 women die of cervical cancer a year in the US. Cervical cancer is initially asymptomatic, allowing it to advance to a more severe stage if not detected early. The positive news is that cervical cancer is highly preventable through screening for precancerous lesions or the presence of HPV —the primary culprit behind most cases.The role of HPV: Human Papilloma Virus, according to the World Health Organization, caused an estimated 620,000 cancer cases in women and 70,000 cancer cases in men.Cervical cancer is more prevalent in certain regions. In regions with established screening initiatives, the incidence rate and mortality rate of cancer are lower than in resource-limited areas. This highlights that resource-constrained countries continue to bear a burden of this disease. In nations like the United States, access to the HPV vaccine along with routine screenings, like Pap smears and HPV tests has significantly decreased the prevalence of cervical cancer.Screening recommendations from the US Preventive Services Task Force (USPSTF) and the American Cancer Society (ACS).The U.S Preventive Services Task Force advises that women aged 21 to 29 undergo a Pap test every three years while those aged 30 to 65 should opt for co-testing (Pap and HPV tests) every five years. These examinations are usually conducted in outpatient facilities, where a medical professional collects a sample of cervical cells that are later examined under a microscope.A normal result states that the sample was adequate for evaluation, in other words, that endocervical/transformation zone components are present, and that the patient is “Negative for intraepithelial lesion or malignancy.” ACS recommends cervical cancer screening begin at age 25 for women and people with a cervix. Those aged 25 to 65 should have a primary HPV test every 5 years. (A primary HPV test means the HPV test is done without cytology; follow-up screening can be done with a Papanicolaou (Pap) test if needed.) If primary HPV testing is not available, screening may be done with either a co-test every 5 years, which combines an HPV test with a Papanicolaou (Pap) test, or a Pap test alone every 3 years. How is Cervical Cancer Classified?Two systems categorize lesions: the Cervical Intraepithelial Neoplasia (CIN) scale and the Bethesda system.The CIN scale categorizes lesions based on the degree of involvement of the cervical lining ranging from mild (CIN I) to moderate (CIN II) to severe dysplasia (CIN III).The Bethesda system emphasizes cytological findings organizing results into categories such as atypical squamous cells, low-grade lesions (LSIL), and high-grade lesions (HSIL).ASCUS (Atypical Squamous Cells of Undetermined Significance) is the most common abnormality seen in pap smears. It may or may not indicate a problem, you have to make a decision based on the patient. Cervical cancer is largely linked to high-risk HPV (hrHPV), mostly HPV 16 and 18, and scientists are investigating tests that identify hrHPV DNA or RNA. These tests may provide a more accurate evaluation of cancer risk compared to traditional cytology. Examples include DNA amplification tests like Cobas test and the Xpert HPV test.Obstacles to Screening.Despite the efficacy of cervical cancer screening, many women face many obstacles to testing. In regions with limited resources, fear, embarrassment, lack of awareness, and restricted healthcare access pose challenges to screening.In Nigeria, a study revealed that women often avoid Pap smears due to a lack of awareness. Similarly, healthcare providers in Ecuador highlighted issues like the absence of screening programs and inadequate health promotion efforts. Women in Peru face obstacles such as long waiting times preferences for female healthcare providers and limited access to health facilities. In 2022, 31% of minority women in the US did not undergo Pap smears in the past three years; many of these women were uninsured, unemployed, or low-income. These challenges contribute to higher rates of cervical cancer among women who do not follow recommended screening guidelines.We must mention the cultural obstacle as well. Some cultures do not allow any kind of pelvic exams before marriage. They put a major emphasis on being a “virgin,” and placing a speculum in the vagina may be considered culturally unacceptable. In those cases, the doctor has to use their best persuasion skills to accomplish the goals of care. For example, they may suggest having the mother in the room during the pap smear, using the smallest speculum possible, or other techniques.Self-sampling.In 2020, the World Health Organization (WHO) introduced a global initiative to combat cervical cancer worldwide. The initiative aims to:Vaccinate 90% of girls by age 15.Screen 70% of women by age 35.Treat 90% of women with lesions and invasive cancer by 2030.To achieve these goals, self-sampling for HPV testing has been introduced as a viable option for cervical cancer screening. Self-sampling for HPV testing is seen as an alternative for cervical cancer screening that addresses barriers associated with traditional methods. This approach enables women to take samples themselves using swabs or brushes removing the necessity for a pelvic examination. The option to mail in samples and receive results within two weeks enhances the convenience, privacy, and accessibility of the process giving individuals control over their health.While self-sampling for hrHPV detection is not currently standard practice in the United States, it has been successfully implemented in countries across Europe, Africa, and South America. Pilot studies are ongoing in nations like Canada and New Zealand to assess its effectiveness offering promise for its impact.In May 2024, the Food and Drug Administration (FDA) approved primary HPV self-collection for cervical cancer screening in a health-care setting. That means, the patient still has to go to a clinic to self-collect her sample. How Effective is HPV Self-Sampling?Research supports the accuracy of HPV self-sampling. A study conducted by Polman et al., which involved a randomized controlled trial, demonstrated that HPV tests on self-collected samples were just as precise as those done on samples collected by clinicians in detecting high-grade lesions (CIN II and CIN III). Similarly, a meta-analysis conducted by Arbyn et al. showed no difference in sensitivity or specificity between self-sampled and clinician-sampled tests for detecting CIN grade II or higher.These results indicate that self-sampling could be an adequate screening method for cervical cancer. This reassurance may motivate women to partake in screenings knowing they have a convenient and effective option. Ok, let's say a patient has collected her sample or the sample was collected by a clinician, what is next?Management of Cervical Cancer Screening Results.The process of managing cervical cancer screening results involves evaluating a patient's immediate and five-year risk of developing cervical abnormalities (CIN 3+) following guidelines from the American Society for Colposcopy and Cervical Pathology (ASCCP).The ASCCP app is the best investment you can make in primary care. It is only $9.99, but it can save you a lot of time in clinic. Estimating risk is a process that considers factors such as current HPV test results, past screening outcomes, the patients' age, and whether they've had a hysterectomy or not. When Risk is Elevated, Prompt Action.If a patient's immediate risk of developing CIN 3 exceeds 4%, expedited treatment is typically recommended. This treatment may entail one of several procedures aimed at removing abnormal cervical tissue.Loop Electrosurgical Excision Procedure (LEEP): A common method that removes tissue using an electric wire loop. Cold Knife Conization: In this procedure, a scalpel removes a cone-shaped section of the cervix.Laser Cone Biopsy: This technique involves removing a cone-shaped section of tissue using a laser.Alternatively, healthcare providers may opt for treatment methods such, as cryotherapy, thermos-ablation, and laser ablation to eliminate abnormal tissue.And those procedures are typically out of the scope of family medicine, but many family doctors may perform them with the proper training and experience.When the risk is deemed low, Surveillance.Patients with a risk of CIN 3 below 4% are typically advised to undergo surveillance with HPV testing every 1-5 years. If HPV testing is not available cytology alone (Pap test) is considered acceptable.Special considerations for women.For women under 25, a cautious approach is taken. If a low-grade lesion (LSIL) is identified through cytology, it is recommended to repeat the test annually for two years. If two consecutive tests show normal results the patient can resume screening intervals based on age. However, if a high-grade lesion (HSIL) is detected, a colposcopy and biopsy are recommended. It should be noted that expedited treatment is generally not advised for this age group since many high-grade lesions may resolve spontaneously.For women over 25, the presence of low-grade lesions or persistent high-risk HPV often leads to recommendations for colposcopy and cervical biopsy.When a cervical biopsy shows adenocarcinoma in situ it is suggested to perform an excisional procedure to rule out invasive cancer. The next steps depend on the margins of the excised tissue; If the margins show positive results (indicating abnormal tissue remains) further excision is necessary to ensure clear margins. This may be followed by a hysterectomy due to the risk of residual disease. For individuals who have been treated for high-grade lesions there is still a risk of developing cervical cancer. Therefore, long-term surveillance is essential. Women over 25 should undergo HPV testing six months after treatment, then annually until three consecutive negative tests are obtained. Subsequently testing every three years is advised for 25 years. As for women under 25, cervical cytology should be done six months post-treatment. Then at six-month intervals until three consecutive negative results are achieved. Once they reach 25 years old, they should switch to HPV testing.As summary, HPV is the most common cause of cervical cancer, and screening must be implemented no matter what your zip code is because adequate screening can lead to a lower mortality. Remember that self-collection is an alternative for your patients, and it is FDA-approved if it is done in a healthcare setting. The ASCCP guidelines are very useful but difficult to memorize, so you can invest in the ASCCP phone app to provide accurate care for your patients. Thanks!References: 1. World Health Organization. This week, I dive into more ways to support a healthy equine microbiome.In the last episode, I explained what a microbiome is and why it is essential for every aspect of a horse's health. If you have not already done so, I suggest you tune in to that episode to understand why supporting the microbiome is crucial for the hindgut and every organ in a horse's body.Maintaining Healthy Gut BacteriaMaintaining a healthy population of bacteria in a horse's digestive system is essential, especially considering that 75% of their energy comes from fermenting fiber. When the balance gets disrupted due to poor feeding practices, exposure to toxicity or chemicals, or stress, health issues like colic, ulcers, fecal water syndrome, diarrhea, or immunity-related problems like scratches, rain rot, festering infections, and chronic thrush may arise. Creating a Supportive EnvironmentA supportive environment starts with carefully considering the horse's diet, avoiding high-sugar and high-protein intake, managing fat levels, minimizing exposure to chemicals and preservatives, and regulating pH levels.How Medications and Stress Affect the Microbiome Apart from diet, medications and stress levels also significantly impact the equine microbiome. Medicines can alter bacterial populations, so it is always essential to evaluate how necessary they are and their potential side effects. Emotional or physical stress can elevate acidity levels in a horse's body. That could affect their bacterial populations and cause inflammation. Assessing lifestyle factors, addressing stressors, and making necessary changes will all contribute to maintaining a healthy equine microbiome.Introduction to Prebiotics for Gut HealthProviding your horse with prebiotics is a proactive way to support their gut health. Prebiotics are in the soluble and insoluble fibers of plants. Foraging horses can access their probiotics naturally from the grasses, leaves, and shrubs they eat. In a domestic setting, however, you must give those to your horse in a feed bucket. Beet pulp, chia seeds, cooked barley, cooked oats, and certain herbs are all options that will help in creating an environment conducive to healthy gut bacteria.Understanding Probiotics and Their BenefitsProbiotics are living biological organisms crucial for modifying the microbiota, producing beneficial compounds, and enhancing intestinal immunity. Clifford Stoll, author of The Cuckoo's Egg, an investigation into the hacking of the Lawrence Berkeley National Laboratory that led to the capture of hacker Marcus Hess, famously said: “Treat your password like your toothbrush. Don't let anyone else use it and get a new one every six months.”The paper, How Effective is Multifactor Authentication (MFA) at Deterring Cyberattacks? concludes that implementing MFA leads to a 99.22% reduction in the risk of compromise across the entire population, and a 98.56% reduction even in cases where credentials have been leaked. To be clear, MFAs can be hacked. The conversation with Professor Hanke will provide listeners with an update on the U.S. economy and the impact of inflation and how government policies affect families and the private sector, specifically small and medium size businesses. Will the Fed's interest rate hikes lead to a soft-landing or a major recession in 2023? On America's Roundtable Radio, Professor Hanke will highlight his economic predictions for 2023. His predictions for 2022 on the inflation rate was right on target, a warning he and his colleague John Greenwood relayed through a commentary in The Wall Street Journal a year ago titled "Too Much Money Portends High Inflation — The Fed should pay attention to Milton Friedman's wisdom. The discussion will also examine U.S. and European Union sanctions imposed on Russia. This past week we found out that with energy prices up, Moscow is reaping significant financial gains through the sale of oil and gas. A recent Wall Street Journal report stated that through July 2022, Moscow has totaled $97 billion from oil and gas sales, earning $5.4 billion more per month than last year's monthly average. Russia is actually pumping almost as much oil into the global market as it did before prior to the conflict with Ukraine. US Assistance to Ukraine Rises to $60 Billion — While the European Union's Contribution Amounts to $10 Billion Only The Biden Administration along with Democrat and Republican members of Congress have authorized over $60 billion in US taxpayer funds to support Ukraine's military and provide assistance to Europe's NATO members. However, the EU comprised of 27 member states in Europe has only provided 10 billion euros (USD$10 billion) since the beginning of the war. As a leading world expert on currency boards, measuring and stopping hyperinflation, currency and commodity trading, Dr. Hanke will relay his thoughts on the US and Euro currencies and the currency board that he developed for a number of countries around the world. His recent commentary in The Wall Street Journal titled "Floating Exchange Rates Add to Economic Uncertainty (https://www.wsj.com/articles/exchange-rates-float-beyond-parity-dollar-euro-monetary-system-bretton-woods-agreement-inflation-currencies-11659991140)” raised important points. Hanke has also held senior appointments in the governments of many other countries, including Albania, Kazakhstan, the United Arab Emirates, and Yugoslavia. A well-known currency and commodity trader, Hanke is chairman of the Supervisory Board of Advanced Metallurgical Group N.V. in Amsterdam and chairman emeritus of the Friedberg Mercantile Group, Inc. in Toronto. Employees must take a stand to decide if their presence is really needed in a meeting. Do they need to be there? What is the contribution they are making? What's the impact if they are not there? How Effective is the Survey Method to Judge the Overall Health of an Organization?Most organizations probably don't need a survey because they already know what they are doing. If they are engaging with the employees regularly, they know what the answers will be to the survey questions. Surveys can create overwhelming data, and companies don't know what to do with it. Marguerite Duane, MD, MHA is a board-certified family physician and adjunct associate professor at Georgetown University. Dr. Berlin conducts research to better understand the neural basis of impulsivity, compulsivity, and emotion with the goal of more targeted treatment. She employs neuroimaging and neuropsychological and psychopharmacological testing of brain lesion and compulsive, impulsive, and personality disorder patients. She is also interested in the neural basis of consciousness, dynamic unconscious processes, the use of psychedelics to treat mental disorders, and in the neural basis of creativity. Impulse control is not just a problem in childhood, it is perhaps the major problem in adults who have significant psychological troubles. This includes the addictions, but also severe mood swings that adults want to change, but do not seem to have the power to change in these moments. When it is hard to control repeated impulses, we talk about compulsions; cigarette smoking can be seen as a compulsion. When repetitive thoughts are hard to regulate, we talk about obsessions. There seems to be a tug of war between those deep midbrain nuclei and the control system which involves the prefrontal cortex. Various areas of the prefrontal cortex have somewhat different effects, but prefrontal is associated with self regulation, while midbrain nuclei have to do with impulses, motivations, emotions, and so on. The prefrontal cortex is sometimes called the “organ of civilization” or as Heather dubs it, “the brake system”.The Big NewsIt is amazing how low level electrical stimulation by microscopic electrodes can profoundly change human mood disorders, like severe depression. The brain areas stimulated include the nucleus accumbens and ventral striatum of the basal ganglia, and local areas in the prefrontal cortex. “Medical science is often an art as well as a science, but Deep Brain Stimulation (DBS) really can have significant effect and impact on people with difficult-to-treat conditions, like OCD (Obsessive Compulsive Disorder), but also people with intractable or untreatable depression,” says Dr. Heather Berlin.DBS can be surprisingly effective. As Dr. Berlin points out, “DBS is a huge success story. 40 to 50% of patients with severe untreatable depression, and about 60 to 70% of patients with severe OCD all have significant improvement in people who’ve tried every first line treatment and nothing has worked. In many cases you can see an immediate effect, as in chronic depression -- suddenly you turn on the electrodes -- you don't tell them when it's on or off, right? And their whole face lights up. And you ask "What do you feel like?" And they say "Oh, it feels wonderful. It feels like I won the lottery! It's so great!" And as they're talking, you turn off the electrodes and you just see their whole affect drop right back down.” Surprising Advances in Psychedelic ResearchIn the 1960s, psychedelics got a mixed reception, because many people had spectacular experiences, but physicians often wondered if there were harmful side effects. Now we are seeing a return to psychedelics as a promising treatment for different neuropsychiatric conditions. The new therapies always combine the psychedelic-assisted treatment with the presence of a specialized psychotherapist. Dr. Berlin gives us a summary of the new discoveries in the use of psychedelics, including MDMA for the treatment of PTSD, psilocybin to treat anxiety and people with end of life issues, ketamine, a dissociative anesthetic, for the treatment of severe depression and particular suicidality, and more recently DMT (or ayahuasca) as a treatment for certain psychiatric illnesses. After 50 years of persistent efforts to find solutions, in the last decade we finally have treatments with dramatic positive effects. Welcome back to Six Minute Sex Ed, the podcast that helps families talk about sex and relationships, hosted by Sex Education teacher, Kim Cavill: https://www.teaandintimacy.com/ This episode is Level Two, which makes it more complex than my level one episodes and especially great for tweens and teens. We’re going to talk about Emergency Contraception. What is it? Emergency contraception is birth control taken up to five days after risky sex in order to prevent pregnancy. There are three kinds: two pills and the copper IUD. How does it work? Levonelle prevents ovulation, Ulipristal blocks the body’s own hormones, and the Copper IUD prevents sperm from reaching the egg. See a copper IUD here: http://teenhealthsource.com/birthcontrol/copper-iud-details/ How Effective is it? Levonelle is 95% effective in the first 12 hours after risky/unprotected sex, then steadily drops in efficacy (85% after 48 hours, 58% after 72 hours). Ullipristal is 95% effective for five days after risky/unprotected sex. The Copper IUD is 99.9% effective for five days after risky/unprotected sex, making it the most effective form of EC available. Side effects: All three interventions are very safe and side effects are very rare, though they include irregular bleeding, upset stomach, and headaches. Some medical conditions make EC unsafe to take, so it’s important to speak to a doctor to check it’s safe: if you can’t reach your doctor, text “PPNOW" to 774636 (PPINFO) text with someone from Planned Parenthood who can help. Side effects of the copper IUD include temporary cramping, spotting, and heavier or more painful periods. Tips: Because levonelle is time sensitive, it’s a good idea to have a dose on hand, before it’s actually needed, just make sure it’s available. You can also use some forms of the birth control pill as emergency contraception, but ONLY if taken according to very specific guidelines and it’s less effective than the other options I’ve talked about, so make sure to speak to a doctor before trying this method. In this first for PT Inquest, the guys review a letter to the editor. No paper is perfect. When looking in hindsight, papers that seemed to be originally accurate reveal themselves to be flawed. How do we steer clear of those mistakes in the present? What is the responsibility of a journal to catch these issues before publishing a paper? Heads 'N Tales Podcast - Talking Sports Injury Rehab, Prevention, Perseverance, Concussions & Athlete Transition

Last Sunday, the Green Bay Packers and New York Giants squared off on wild card weekend.  The Packers were victorious, but did not come out unscathed.  Jordy Nelson, who leads the Packers with 97 catches for 1,257 yards and 14 touchdowns, broke his ribs on a controversial hit made by Giants defensive back, Leon Hall. As you can see in the picture above and the video below, Hall lowered the crown of his helmet into Nelson's body in an attempt to break up a pass.  What are your thoughts on this hit?  Clean or dirty? Comment below!  Look carefully at Leon Hall after contact... Looks a little woozy to me... wouldn't be surprised if he game himself a concussion. The reason I dedicated an entire episode to this hit and described it as "bush league" is because of how unnecessary I felt it was. Just as easily as Leon Hall targeted his facemask into Nelson's ribs, he could have placed his head to either side of the receiver. Had Hall taken that approach, Jordy most likely wouldn't have been injured, and Hall wouldn't have exposed himself to a concussion or spinal cord injury.  For this reason, tackling 101 always teaches athletes to NEVER lower their heads and to "see what you hit."  For NFL players, this is their livelihood!  How would you want to be tackled knowing that your next play, game or season is what is putting food on your family's table? Look out for your brothers on the field.  You may be wearing different colored jerseys, but you all have the same goals at the end of the day.  Don't try to end your opponents career because you just might end yours in the process.(Check out an article I wrote a few years back for Jay Fraga's Knockout Project on defenseless receivers in the NFL : Fixing Concussions with Band-Aid’s: How Effective is the NFL’s Defenseless Receiver Rule? ) Ultimately, Leon Hall was not fined although the NFL rules prohibit "lowering the head and making contact with the crown or 'hairline' parts of the helmet against any part of a defenseless receivers body."  In my opinion, Hall's hit meets this definition.  I feel this hit was made with the intention to injure, not just to make a tackle or break up the pass.  Although I am critical of Leon Hall's hit and I am assuming his intentions, I am also somewhat of a hypocrite because I used to be the same type of player. On every play I tried to run over any defensive back who attempted to tackle me.  When I did, my teammates got fired up, my coaches got fired up and I felt whole.  So trust me, I understand what was going through Leon Hall's head as he was in pursuit of the Packers' leading receiver. But then again, maybe neither of us are to blame.

But then again, maybe neither of us are to blame.  I was the type of running back who fought for every inch and tried to punish as many defenders as I could along the way.  I don't recommend anyone do this and I believe this is partially why my career was shorter than I hoped. Although this may just look like a normal picture of me running the football on a Friday night in 2007, there is actually a lot more going on.  Take a look at my right hand.  My clenched fist is in preparation for attempting to run over the Roxbury High School outside linebacker.  I succeeded, but at a cost.  I separated my shoulder on this play, which led to hurting my opposite shoulder and then using my head as a weapon.  I truely believe if I sat out for this shoulder injury, I never would have injured my brain to the extent I did. In 2009 a journalist by the name Malcolm Gladwell wrote an article called How Different are Football & Dog Fighting?  This was written in the aftermath of Michael Vick's dog fighting NFL dismissal and jail sentence.  The article talks about concussions and CTE, but also compares football to dogfighting and to be honest, I thought Gladwell was on to something then and I still do now.   "In a fighting dog, the quality prized above all others is the willingness to persevere even in the face of injury and pain.  A dog that will not do that is labeled a 'cur' (worthless) and abandoned.  A dog that keeps charging at it's opponents is said to possess 'gameness' and game dogs are revered."  -  Malcolm Gladwell, How different are Football & Dog Fighting? (2009)   Leon Hall is a 10 year veteran in the NFL so he was undoubtedly brainwashed by the same old school mentality that I was exposed to growing up.  You know what I'm talking about, "no pain no gain," "suck it up," "don't rub it we got ice" and my favorite "you just got your bell rung."  It was the same era of glorified cheap shots, twisting ankles and gouging eyes under the scrum (I never partook in this part, but was on the receiving end plenty of times).  This is all we knew.  A part of me does understand why Leon Hall launched himself into Jordy Nelson with reckless abandon.  But I also understand that is exactly what is wrong with the game of football.  You want to save the game, fix that problem in my opinion.  No new rule changes, or equipment would be required, just a shift in the definition of toughness...or gameness.   Gameness - A dogs desire to please an owner at any expense to itself - Carl Semencic , in “The World of Fighting Dogs” (1984)   As recently as December 1st, 2016, Gladwell brought up the dog fighting connection again in a collaborative article with sports journalist, Bill Simmons, titled Simmons vs. Gladwell: The Future of Football.  This article was written in the wake of Cam Newton's repetitive head shots and immediately after Luke Kuechly was seen sobbing after suffering a concussion on a Thursday night prime-time game against the Saints.  In my opinion, I believe Kuechly was not crying because of pain or an uncontrollable reaction to his concussion. I think, Kuechly was crying because he was silently being labeled as a 'cur' by the media, his coaches, teammates and the running dialog in his head. But you know what, I have been there too and it sucks.   Another interesting point in The Future of Football article was a segment about a recent boxing match where one of the boxers quit in the 8th round because he didn't want to take anymore punishment. This is an example of an athlete admitting he doesn't need to be as game as a dog.  Below, I provided a video of highlights from star NFL running back, Ezekiel Elliott (Zeke).  I put this here as an example for young running backs on how they should run the ball.  In my opinion, Zeke run's the with the perfect combination of brains and braun, which hopefully will lead to a longer career.  In watching the highlights, notice that Zeke only lowers his shoulder to run over a defender when he as at the goal line, or to protect himself.  My advice to running backs is that you are supposed to run away from defenders, not at them if you can avoid it.  If you are too slow like I was, find a new position.  After reading this post, it might be seem like I have a personal vendetta against Leon Hall.  I wholeheartedly do not, and as I mentioned, I'm sure we have a lot in common in terms up our football upbringing.  Also, as all athletes know, you turn a switch on when you step on that field.  You're not that same person on the streets.  Trust me, I get it!  Not many NFL players can say they played in the league for 10 years so Leon obviously is doing something right.  I'd love to talk to him about how he was able to stay healthy for that long in a profession with a 100% injury rate. I will assume Hall has learned to please his coaches, teammates and the media along the way.  You don't spend that much time in the league if you're not a game dog.  That being said, Gladwell points out that organizations who select for gameness have a responsibility not to abuse the trust (ex. between athlete and coach).    "If you have men in your charge who would jump off a cliff for you, you cannot march them to the edge of the cliff—and dogfighting fails this test." - Malcolm Gladwell, How different are Football & Dog Fighting? (2009) Hits and plays like the Hall-Nelson hit happen every weekend all around the country.  However, the NFL and all sports organizations at all levels of play need to exercise their responsibility to protect athletes.  I am simply using this hit as one of many examples of how we can make football safer and to teach people what the real definition of toughness is.  Unfortunately I learned the definition long after my playing days, but my hope is some young buck reads this and the light bulb goes off in their head sooner rather than later.   "'Toughness' is deciding to engage.
