On July 16, 1965, a massive alien spacecraft from the Zeta Reticuli star system landed at the Nevada test site north of Las Vegas. Following a plan set in motion by President Kennedy in 1962, the alien visitors known as the Ebens welcomed 12 astronaut-trained military personnel aboard their craft for the 10-month journey to their home planet, Serpo, 39 light-years away. In November 2005, former and current members of the Defense Intelligence Agency--directed by Kennedy to organize the Serpo exchange program--came forward to reveal the operation, including details from the 3,000-page debriefing of the 7 members of the Serpo team who returned after 13 years on the planet.   Working with the DIA originators of the Serpo project and the diary kept by the expedition's commanding officer, Len Kasten chronicles the complete journey of these cosmic pioneers, including their remarkable stories of life on an alien planet, superluminal space travel, and advanced knowledge of alien technologies. He reveals how the Ebens presented the U.S. with “The Yellow Book”--a complete history of the universe recorded holographically, allowing the reader to view actual scenes from pre-history to the present. Melvyn Bragg and guests discuss the British phase of a movement that spread across Europe in the mid-19th and early 20th centuries. Influenced by Charles Baudelaire and by Walter Pater, these Decadents rejected the mainstream Victorian view that art needed a moral purpose, and valued instead the intense sensations art provoked, celebrating art for art's sake. Oscar Wilde was at its heart, Aubrey Beardsley adorned it with his illustrations and they, with others, provoked moral panic with their supposed degeneracy. After burning brightly, the movement was soon lost its energy in Britain yet it has proved influential. En partenariat avec la Tate Britain, à Londres, le musée d'Orsay montrera pour la première fois en France une rétrospective de son oeuvre dans son ensemble, première monographie de l'artiste en Europe depuis l'exposition du Victoria & Albert museum de Londres en 1966.Les dessins vifs et virtuoses de cette figure originale de la scène londonienne fin-de-siècle mettent en scène un univers étrange, érotique et anticonformiste. This is a subscriber only episode, so come on and sign up at tonywalker.substack.com (tonywalker.substack.com) John Buchan:  John Buchan, officially 1st Baron Tweedsmuir was born in 1875 in Perth, Scotland and died in 1940 aged only 64 in Montreal, Canada. He was Governor General of Canada until he died in office. His father was a minister of the Church of Scotland. Many of our ghost story writers are children of clergymen.  He studied Classics at the University of Glasgow and then moved to Oxford University. After that he went to South Africa where he was private secretary to the High Commissioner of South Africa. Like Kipling, who we read last week, Buchan was a conservative son of the British Empire. He was a bit of a softie for a conservative though because later when MP for Peebles just south of Edinburgh, he supported votes for women, national health insurance for the poor and curtailing the power of the House of Lords. Though a Scot, he was not a Scottish Nationalist, and in common with many Scots of his class and time, he though Scotland was best off within the British Empire. When he returned from South Africa, he was called to the English Bar as a barrister (an advocate in Scotland). He was also editor of the still existing conservative magazine: The Spectator. In 1916, Buchan went to the Western Front, attached to the Intelligence Corps. Just before this he had just published his famous spy story The 39 Steps.  In 1935, Buchan went as Governor General to Canada. He had always liked Canada, written about it as a journalist and fought alongside Canadian troops in the First World War. He encouraged a distinct Canadian identity and nationality. He hosted King George VI on the king's tour of Canada.  He suffered a stroke at Rideau Hall and then a head injury as he fell. He was treated by the famous neurologist Wilder Penfield.  A Journey of Little Profit was published in 1896 in The Yellow Book, so he was just twenty-one at the time. It's a well-regarded story about a bargain with the Devil. Because of its Scottish setting it has echoes with James Hogg's The Private Memoirs and Confessions of a Justified Sinner, and of course with the Faust Legend. The great use of dialogue and language with lots of Scottish dialect words sets this story apart but also the fact that Duncan Stewart becomes fond of the Devil and escapes with his soul. We see him first the older wiser man, now regretful of his wild youth. It's a great story, I hope you enjoyed it too. Robert W. Chambers Robert William Chambers was born in Brooklyn, New York in 1865 and died in New York in 1933, aged 68. He was born to a well-off family; his father was a corporate lawyer with his own firm which did very well.  Robert’s early love was art and of course this is evident in the story, The Yellow Sign. He first studied art in New York and then aged 21 went to Paris to study at the Ecole des Beaux Arts. When he returned to New York, he sold his illustrations to well-known magazines such as Vogue, but in 1887, while he was in Munich, Germany, he completed his first novel and then he began writing weird tales. At that time, the most prominent art movement was the Art Nouveau with its lines and curls drawn from plants and nature. His best known collection is The King in Yellow, from which this story The Yellow Sign, is drawn. This idea of a book that sends you mad is found in Lovecraft with his Necromicon but also in a story that I will read out one day, about Michael Scott the Scottish Wizard from the 12th Century whose book was kept in Wolsty Castle’s library suspended on an iron spike for fear of the occult knowledge it contained. And of course, books have been banned, notably by the Catholic Church with its famous Index, because of the dangerous information they are thought to contain. Chambers was admired by Lovecraft and perhaps because of that, has not been forgotten like some of his contemporaries. This story: The Yellow Sign, was one of my favourites. In reviews elsewhere they say that Tessie was a prostitute. She was not a prostitute: she was a nude model, which is not the same thing! Though she would have been looked down on by polite society in New York. Though she loves the painter Mr Scott, they can never marry because of her status, but she can be his mistress. I didn’t find Scott an unsympathetic character. I am not sure how much the story needed the love interest in fact, and it was when Chambers was exploring that theme that my interest flagged a little. I thought it was about time I did another American story. I enjoyed doing the accent and I also enjoyed doing the Cockney accent of the bell boy, but the best thing about the story for me was the Yellow Sign. First of all, we have the horrible watcher, and then the gruesome details from the bell boy that confirm his unnaturalness. There is a great comparison with a soft fat coffin worm, that is quite horrid.  The occult sign that Tessie finds and gives to Scott seems destined for him and in fact there is a sense on an inexorable destiny that cannot be avoided. Some evil intelligence has planned all of this — brought the Yellow Sign to Scott and planted the King in Yellow Book in his library. And then, as the end comes, I think the approach of the King in Yellow — if that is who it is — is done very well. Very sensory and paced like a slow drum beat as the thing comes ever closer. So, this is a weird tale and a horror story, but I did say that I would be reading weird tales as well as specifically ghost stories. Let me know what you think! Show Notes: Dr. Steve Dunlop is an emergency physician practicing at Hennepin County Medical Center. He trained at the University of Minnesota in med school, then did his emergency medicine residency at HCMC. Steve has his master's in public health and completed his certificate from the American Society of Tropical Medicine and Hygiene. He's widely published and is an assistant professor of emergency medicine at the University of Minnesota. He's quite the traveling man and is no stranger to sub Saharan Africa and many other parts of the world. Indeed he is an international physician of mystery and we're honored to have him join us to discuss fever in the return traveler. Today's talk will focus primarily on the process of assessing the return traveler and the big, bad uglies that we don't miss in these patients. So, buckle up and enjoy this tropical disease safari with Dr. Dunlop.   CHAPTER 1:     Malaria is a parasitic disease that is transmitted by the anopheles mosquito. Plasmodium falciparum malaria is the most common, and tends to be the most serious. Malaria has an interesting, somewhat complex life cycle and patients present with cyclical fevers, headache, body aches, nausea and other non-specific symptoms. There are over 200 million cases a year and up to 500,000 deaths per year. Up to 2000 cases per year in the United States are reported. After inoculation by an infected mosquito, there is an incubation period of 7-to 30-days.      We'll summarize this first segment with an overview of the general topic of fever in the return traveler as outlined by Dr. Dunlop. As we discussed, there are diagnoses that we in medicine are trained to think about, and while we consider them in our differential diagnosis, they are still considered "zebras" in medicine. These diagnoses happen, but for some reason, causes of febrile illness that would be considered "zebras" seem to be less apt to make it on that differential list, or are basically the, "come on, you're kidding, right?" diagnosis.      In the case of a disease like malaria, especially when it presents in the developed world, patients often are seen 3-times or more before the diagnosis is made.  Different diseases have varying incubation periods and latency periods. These unknowns make the diagnosis, let alone the awareness of these various so-called 'tropical diseases" all the more difficult. We first must ask the relevant questions, which should start with "have you traveled anywhere recently?", "When?", "For how long?", "Where?", and "What did you do there?".      Delusions of parasitosis are not always delusions. As Fred's example of the fish tape worm reveals, one should always perform a careful and detailed exam. With regards to some of the parasite illnesses, the latency periods can be months to years, and a continuous or oscillating fever is not always necessary.      With regard to malaria, for instance, a fever may not be present when the patient shows up with their other non-specific symptoms. Again, the latency period is variable with malaria, not to mention the various causes of malaria.      The topic of tropical diseases is important to Dr. Dunlop because of the realization that the world has become a smaller place. One can easily travel to the other side of the world in a day. Minnesota, for instance, has one of the highest immigrant refugee per capita rates in the country, and consequently one of the largest global health programs in the country. Minnesota alone has over 1-million border crossings per year. Therefore, it is all the more important for general practitioners and other front line providers to have a greater awareness of where these patients recently came from and/or have traveled back to lately.      Seeing and evaluating a refugee immigrant versus other immigrants also carries nuance. Refugees in general undergo a very robust pre-departure screening, treatment and vaccinations. Other migrants however, may not have had the same king of screenings and treatments as refugees.      People travel extensively now, and sometimes forget to tell us about some of the quick stop-over trips that are made in the course of a larger holiday. It's, therefore, important to also consider these extra "side-trips" in the course of the bigger trip and access our resources. Dr. Dunlop refers to the CDC Yellow Book. Specifically, the "clinician view" tab should be accessed. This site allows us to see the most updated travel medicine guidelines regarding vaccines and chemoprophylaxis. As Dr. Dunlop states, fresh water exposure history will clue us in to various parasitic illnesses, such as schistosomiasis. Food intake is also taken into consideration, but this can become murky, and of course can lead to a variety of GI illnesses. Vaccine preventable illnesses, such as Hepatitis A, Typhoid and Yellow Fever, depending on where you are traveling, may be advised. Regarding malaria prophylaxis in particular, chemoprophylaxis regimens need to be given before, during and after an abroad trip. This CDC Yellow Book resource advises specific advice for specific regions and cities in all countries. All the vaccines, chemoprophylaxis, precautions and other considerations are provided here.      Public health programs track various diseases, however, the data gathering is largely backed by wealthier governments. The CDC in particular has surveillance posts throughout the world which monitors health and disease phenomena. WHO [the World Health Organization] takes on the responsibility of disseminating this information, but the efforts do tend to be collaborative.        CHAPTER 2:      VFR travelers, or "visiting friends and relatives" comprise of the majority of malaria cases in our country. Many of these people. for reasons unclear, don't always adhere to chemoprophylaxis during their often extended visits to their home countries. As a side note, malaria was common in the United States until the mid-1800's, and was largely eradicated in the ensuing decades due to DDT. Good for the malaria, bad for the eagles, and a lot of other critters. A pesticide that unfortunately killed a lot of other organisms as well.      Dr. Dunlop presents a case, that Fred and I butcher our way through. Bear in mind, folks, this discussion is meant to be broad, and not super-granular in terms of each tropical illness, although our friendly Dr. Robotvoice that you've already met, will chime in periodically to give us some factoids about some of these diseases we should be considering. So, we have a 35-year old male, VFR traveler who goes back to Ghana. He returns with a fever. While home in Ghana, he stayed in his village; and basically while there, he lived like the locals - so to not stick out like a sore "Americanized" thumb amongst his friends and family. However, as Steve points out, and while we should be ever vigilant for malaria and still test for it, and still consider the other tropical diseases in this patient, it's still prudent to do the tests for the common illnesses. Influenza, pneumonia, strep, UTIs, etc.      How do we test for malaria now? Thick and thin blood smears are still the standard in diagnosis of malaria. But, those smears can also show other diseases, such as African Sleeping Sickness. However, the rapid malaria tests are even more commonly performed now, and will give you a quicker turnaround in the diagnosis. The sensitivity and specificity of the rapid malaria tests are good, and if the patient looks non-septic, perhaps improves with conservative care or some IV fluids, the test is sufficient. It's common to repeat the test several times if the patient continues with symptoms. The thick and thin smears are still warranted, though, in these cases as well. CDC can be helpful, however is not diagnostic of malaria.       In this same patient, if the malaria testing is negative, and other screening of common febrile causes is negative, then throw in some bone pain, joint pain, severe headache and a blanching rash - Dengue Fever is to be considered. Getting Dengue again further down the road can be much worse, such as Dengue Hemorrhagic Fever. Chikungunya is making itself known quite well in recent years too. While there is less likelihood of these and other viral diseases to cause severe illness, they are helpful to track from a public health standpoint. Sex tourism is quite common as well, and STIs must also be considered, such as HIV. In fact, some health systems are doing rapid HIV tests on all-comers. There is about a 0.1% positive rate at Hennepin Ed, for instance, and the significance here is that we're identifying the superinfected hosts who are at the highest likelihood of passing it to others. So, again, besides doing our basic work-ups like a chest x-ray, urinalysis, etc., the work-up for malaria is indicated as directed. A CDC and LFTs can also be helpful. Leukopenia, thrombocytopenia and transaminitis, a lot of vector borne illnesses will exhibit these findings. Rickettsial diseases are included in this.      There is a high incidence of gram negative sepsis with severe malaria, therefore, a comprehensive sepsis work-up and treatment plan with brad spectrum abx is appropriate; even in the setting of a positive malaria test or smear. IV quinidine is approved for use in the United States, although production of this is now an issue. There is hope that other drugs will go through the FDA process, although this is expensive. Artesunate is the IV med for malaria used elsewhere in the world, and the CDC has an investigative provision to use this drug when and if quinidine is not available. Malarone can be given PO or crushed and placed in an NG tube if IV preparations are temporarily unavailable. Doxycycline and Clindamycin do have some utility in malaria treatment as well. So, in the unlikely event that you are treating the sick malaria patient, these can be used as well in a pinch.        CHAPTER 3:      For respiratory patients who have traveled and you don't know the etiology, think respiratory isolation until you have a better sense of what's going on. Most likely, it is not something serious. However, there are serious respiratory illnesses endemic to certain countries and regions, such as MERS and SARS. Again, the Yellow Book can be helpful here. Personal protection including masks for patient and providers/care givers is recommended.      Diarrheal illnesses are also to be considered such as cholera. In general, abx are not always needed. The extreme cases of fever, abdominal pain and severe diarrhea is a different story, though, and abx can be given here. While some of the diarrheal etiologies for travelers are viral, many are bacterial. There is some resistance to certain antibiotics in campylobacter. Traveler's Diarrhea, for instance, does not require antibiotic therapy. Patients at higher risk are often on antacid meds at baseline. Situational needs may warrant antibiotics, such as the need to avoid a bathroom break next to a pride of lions, or a curious hippopotamus. Imodium is helpful, as is bismuth subsalicylate. Warn your patients about the black stools, though, too. Bismuth may be able to be used as prophylaxis as well. Imodium can be helpful for traveler's diarrhea, but it can lead to obstipation and it should be avoided in severe cases. Contacting the CDC for general questions about a tropical illness is an option. Steve also mentions that in the Hennepin service area, the option of contacting the Emergency Physician on duty is also an option. Again, look into the option of having a rapid malaria test available at your shop.      Dr. Dunlop discusses EMR systems, in this case, EPIC showing an improved ability to screen patients based on their demographics, country of origin, travel history, etc. of potential infectious considerations. For instance, what kind of malaria or other travel illnesses to consider based on what the patient's registration information provides.      Follow-up for most patients who have been diagnosed with a travel illness, especially something complex like malaria, it would be best to refer them to someone steeped in this field. As Dr. Dunlop mentions, it's prudent to look up ASTMH certified providers in your area. This information could be obtained from a travel medicine clinic, and would be a good place to start. Health care providers as patients tend to be some of the worst offenders in terms of compliance with travel medicine, chemoprophylaxis, etc. Utilizing a travel medicine clinic will be most prudent, and besides medications, many other topics can be addressed, including issues one wouldn't think of, like vaccinations - that may be problematic for older patients, insurance coverage for medications, etc.

Alaura: So, it’s really ironic is that my verbal, like, speaking storytelling skills are a little bit off, which is why I like writing. But, I’ll tell you how I started. I’ve actually started in the theater. I was a child actress and, that’s what I thought I was going to do my entire life. I was on the stage, I literally grew up on the stage. Kira: Wow. Alaura: And I went to the Baltimore School for the Arts for high school. I majored in theater in undergrad and got my graduate degree in acting. So, it was kind of like, that was my path; I was going to be a professional actress. I focused on the creation of original works, so I did have that writing element in there. But, life is a lot harder—laugh—than your dreams, right? You know, the reality is most actors are unemployed for the majority of their careers, and I had to find a way to pay back those student loans and pay bills and be an adult. And so I got into sales. I got into business-to-business sales. One of my first jobs was actually on inside sales for a start-up, and I liked that environment a lot, of that small team, that kind of feisty, scrappy team, building and growing that business, and it felt like a good place to be. There are some books that are so fundamental to work in an academic field that practitioners refer to them simply by the author’s last name. Many of us had respectfully and affectionately referred to Endymion Wilkinson‘s Chinese History: A Manual, Revised and Enlarged (2000) simply as “Wilkinson” (or, “The Yellow Book,” as opposed to an earlier blue-covered version of the text), and have had well-worn and dog-eared copies of it on hand at all times. I purchased my own copy shortly after beginning my doctoral program, and immediately understood why the encyclopedic guide to research in Chinese history had been so formative and so indispensible for so many people. It was in every way an essential text for anyone studying or practicing the history of China. The recent publication of Wilkinson’s Chinese History: A New Manual (Harvard University Asia Center, 2012) was and remains a major event. The manual quickly sold out (within a month of its publication!), and Wilkinson has already submitted revisions for a second printing. Chinese History: A New Manual is in many ways an entirely new organism that is quite different from its predecessors. It incorporates a million new words of text and substantially new material on everything from Chinese archaeology to environmental history. Its seventy-six chapters range from the basics of the Chinese language to the nuances of historical bibliography, incorporating detailed accounts of topics that are fundamental to understanding China and its culture (geography, literature, food and drink, etc.), as well as chronologically-organized research guides to individual periods of Chinese history. Scattered throughout the text are insets on a wide range of material, from nonverbal salutations to the mariner’s compass, that together comprise a wonderful kind of miscellany. The book is, in every way, absolutely indispensible to work in Chinese history. In the course of our conversation, we talked about many aspects of the genesis of and research strategies that produced Wilkinson’s project. We also talked about the present state and possible futures of Chinese history, and the qualities that might make a work into a lasting contribution to that field. Enjoy! What they say and how they say it. * Then they have to deliver a solid product. * They have a unique sales choreography. * It took over a year to incorporate the sales choreography. It took some buy-in. * Allow your staff to inject their personality into the 27-point presentation. * We all use scripts every day in everything we do, including shaking someone's hand as we walk into a business meeting. Their initial greeting is a script. * About 8 out of 10 are open to having a conversation first. The other 2 realize there are 73 beds and after looking around they open up to answering some questions. * He's very big on lead generation. Most focus on those looking to buy a bed today. He addresses those that wake up late with a sore back and he directs them to get a free guide, “What's Keeping You Awake At Night” so they are priming the pump. * 27 different “out of place” ads such as places as home show expos, chiropractor sections in the yellow pages. (Still got a 7:1 ROI in the Yellow Book with an 800 number vs a local number.) * TV Guide ads - three ways to respond. Get phone, web opt-in and mail the coupon. (He's running 27 currently.)"] * His main prospect is the Baby Boomer to senior citizen so he runs ads that attract them: Charlotte, Lynn, Valorie and Lauralie * The four main personas really identified themselves. All but one are Baby Boomers. * Interviewed past clients. They just asked them. It helps when you sell good stuff and deliver a great experience. If you're afraid to this, that is telling in and of itself. * Some came to the store. Some met for coffee. Some met for lunch. * Their 27-point “Sleep Assessment” is the script behind the scenes. It was 30 for a while but some were redundant and was streamlined. * They recognized they have to be different and have a different process to separate themselves from their competition. * Now they are on a clipboard. * He and Ben are on the floor about 10% of the time and they are working on the business 90% of the time. [Tweet ""Have personas for your business and sell to those personas the way they want to buy" says @GardnersMattres."] * Sales structure and sales process is the key to success. “If you're ready to accept a paycheck from me every two weeks that is loose and off the cuff, I'll let you sell that way.” * In three years they've hired 6 sales people and none have left. * You need a system to manage your prospects, clients and leads. It's expensive to have things fall through the cracks. You really need this if you sell an intangible offering. * Lead generation from an advertising standpoint is the biggest thing you can do. * To learn good lead generation you need to get a coach/mentor, do home study courses, etc, etc. You have to jump in and start learning and doing. * Know your customers, how they process information, what it takes to get them to come in and make the sale. * Jeff has invested at least $200,000 in the last 5-6 years in sales and marketing training and it has paid off. * Always invest in yourself to grow. * The first product Jeff bought was $1,200. It was a little iffy. He was a little scared. But ask yourself “is this thing I'm investing in going to pay off the fastest for me?” * Implement. All of the ideas in the world are worth nothing if you don't implement. Do the work. Do the modules. Pick one part of even the biggest program and implement it. * The compounding effects are worth it.
