Heme oxygenase-1 (HO-1), intercellular adhesion molecule-1 (ICAM-1) and matrix metalloproteinase-1 (MMP-1) are reported to be UVA1/UVB radiation-inducible genes. “On top of that, as part of the photo-aging process we have evidence of the effect of our ingredients on the levels of expression of genes involved in collagen degradation, suggesting a link not only to skin health but also to skin appearance. This study suggests an effect of natural antioxidants on overall skin wellness, which is relevant for men and women in all age groups.” The new study included 65 healthy volunteers aged between 18 and 60. The participants were randomly assigned to randomly consume 20 mg per day of the tomato nutrient complex or placebo for 12 weeks, or 20 mg per day of lutein or placebo for 12 weeks. A two-week washout period separated the placebo and active intervention periods. At the beginning and at the end of each phase the skin was irradiated.Results of the placebo-controlled, double blinded, randomized cross-over study indicated that the tomato nutrient complex (TNC) totally inhibited the upregulation of HO-1, ICAM-1 and MMP1 mRNA by both UVA1 and UVA/B. On the other hand, lutein only completely inhibited gene expression if taken during the first 12 weeks (ie. prior to placebo), while a significantly smaller effect was observed if it was taken during the second 12 week phase (ie. after placebo), compared to TNC. (NEXT) Diallyl trisulfide in garlic induces apoptosis in primary effusion lymphoma Kyoto Pharmaceutical University (Japan), November 7, 2022 Reports from Kyoto Pharmaceutical University stated, “The allyl sulfides, including diallyl sulfide (DAS), diallyl disulfide (DAD), and diallyl trisulfide (DAT), contained in garlic and members of the Allium family, have a variety of pharmacological activities. Therefore, allyl sulfides have been evaluated as potential novel chemotherapeutic agents.” Our news editors obtained a quote from the research, “Here, we found that DAT inhibited nuclear factor-kB (NF-kB) signaling and induced apoptosis in primary effusion lymphoma (PEL), a subtype of non-Hodgkin's B-cell lymphoma caused by Kaposi's sarcoma-associated herpesvirus (KSHV). We examined the cytotoxic effects of DAS, DAD and DAT on PEL cells. DAT significantly reduced the viability of PEL cells compared with uninfected B-lymphoma cells, and induced the apoptosis of PEL cells by activating caspase-9. DAT induced stabilization of IkBa, and suppressed NF-kB transcriptional activity in PEL cells. We examined the mechanism underlying DAT-mediated IkBa stabilization. The results indicated that DAT stabilized IkBa by inhibiting the phosphorylation of IkBa by the IkB kinase (IKK) complex. Furthermore, DAT induced proteasomal degradation of TRAF6, and DAT suppressed IKKb-phosphorylation through downregulation of TRAF6. It is known that activation of NF-kB is essential for survival of PEL cells. In fact, the NF-kB inhibitor BAY11-7082 induced apoptosis in PEL cells. In addition, DAT suppressed the production of progeny virus from PEL cells. The administration of DAT suppressed the development of PEL cells and ascites in SCID mice xenografted with PEL cells.” According to the news editors, the research concluded: “These findings provide evidence that DAT has antitumor activity against PEL cells in-vitro and in-vivo, suggesting it to be a novel therapeutic agent for the treatment of PEL.” (NEXT) PTSD May Speed Up Cellular Aging Boston University, November 13, 2022 From birth to death, a lot may change, but our DNA—the long, double-helix molecule that contains all of a person's genetic code—stays the same. The instructions for reading that code can shift, however, as the chemical tags on and around a DNA sequence change throughout our lives, depending on our age, environment, and behavior. This outside influence on how our genes are read and expressed by cells is called epigenetics—and researchers studying it have discovered clues that may show why some veterans live longer than others. In a new study of military veterans published in Translational Psychiatry, researchers report findings that suggest former service personnel with PTSD are at greater risk of early death. “Our study found that PTSD and comorbid conditions, like substance misuse, are associated with a cellular marker of early death found in DNA methylation patterns,” says Erika Wolf, a professor of psychiatry at the Boston University School of Medicine and senior author of the study. The study included two samples of veterans that had representative levels of trauma and other psychiatric conditions, like substance use and personality disorders. One group included 434 veterans in their early 30s, who had served in post-9/11 conflicts; the other group included 647 middle-age veterans and their trauma-exposed spouses. Both groups were assessed for a range of psychological conditions, and had blood drawn to obtain genetic information and to test for levels of a variety of inflammatory molecules. The results indicate PTSD symptoms were a factor in faster cellular aging—.36 of a year faster. So, for every year that the cells of someone without PTSD age, the cells of someone with more severe PTSD symptoms age a year and a third. (NEXT) Higher sense of purpose in life may be linked to lower mortality risk Boston University, November 14, 2022 Growing research indicates that one's purpose—i.e., the extent to which someone perceives a sense of direction and goals in their life—may be linked to health-protective benefits such as better physical functioning and lower risks of cardiovascular disease or cognitive decline. Now, a new study led by a Boston University School of Public Health (BUSPH) researcher found that people with higher levels of purpose may have a lower risk of death from any cause, and that this association is applicable across race/ethnicity and gender. Published in the journal Preventive Medicine, the study results did suggest that this association is slightly stronger among women than it is among men, but there was no significant difference by race/ethnicity. “In another study I led, we found that the effect of purpose on lowering all-cause mortality may differ by socioeconomic status. In this study, we extended the prior evidence and found that the beneficial effect of purpose persisted regardless of gender and race/ethnicity.” For the study, the team assessed self-reported sense of purpose among more than 13,000 people, based on the “purpose in life” of the Ryff Psychological Well-being Scales, a widely used tool that measures different aspects of well-being and happiness. The researchers also examined mortality risk over an eight-year period beginning between 2006-2008. The results showed that people with the highest sense of purpose indicated the lowest risk of death (15.2 percent mortality risk), compared to people with the lowest sense of purpose (36.5 percent mortality risk). The team also gathered data on additional factors that can influence health, such socioeconomic status, other demographic characteristics, baseline physical health, and depression, and found that an increase in these factors was also associated with increases in a higher sense of purpose. (NEXT) Hibiscus compound shows anti-Alzheimer disease activity Pohang University of Science and Technology, November 16 2022. A report published in Alzheimer's Research & Therapy revealed that gossypetin, a flavonoid occurring in the calyx of the hibiscus flower, activates a process that reduces brain accumulation of amyloid beta, a protein that clumps to form toxic brain plaques in people with Alzheimer disease. Gossypetin has been reported to have antioxidant, antiatherosclerotic and anticancer effects. Earlier research had suggested a benefit for gossypetin, which is structurally similar to quercetin, against the aggregation of amyloid beta and tau proteins that occurs in Alzheimer disease. However, gossypetin's action in animal models of the disease had not been evaluated. Researchers at Pohang University of Science and Technology administered gossypetin or a control substance to mice that were bred to develop a condition similar to that of Alzheimer disease in humans. After 13 weeks of daily treatment, mice that received the flavonoid had less amyloid beta in the brain's hippocampus (an area involved in memory and learning) and cortex in comparison with the control mice. Gossypetin-treated animals also demonstrated better spatial learning and memory than untreated mice. Rather than affecting the production of amyloid beta, the research team found that gossypetin helped clear it by enhancing the scavenging ability of the brain's immune cells, which are known as microglia. Microglia normally consume amyloid beta but can become exhausted by continual exposure, which leads to a chronic damaging inflammatory reaction. (NEXT) Over a billion young people are potentially at risk of hearing loss from headphones, earbuds, loud music venues Mayo Clinic, November 15, 2022 More than 1 billion teens and young people are potentially at risk of hearing loss because of their use of headphones and earbuds and attendance at loud music venues, concludes a pooled data analysis of the available evidence, published in the open access journal BMJ Global Health. The World Health Organization (WHO) estimates that over 430 million people worldwide currently have disabling hearing loss. Young people are particularly vulnerable because of their use of personal listening devices (PLDs), such as smartphones, headphones and earbuds, and attendance at loud music venues, amid poor regulatory enforcement. Previously published research suggests that PLD users often choose volumes as high as 105 dB while average sound levels at entertainment venues range from 104 to 112 dB, exceeding permissible levels (80 dB for adults; 75 dB for children) even if for very short periods of time. A group of 33 studies, corresponding to data from 35 records and 19,046 participants, was included; 17 records focused on PLD use and 18 focused on loud entertainment venues. The pooled data analysis indicates that the prevalence of unsafe listening practices from PLD use and attendance at loud entertainment venues is common worldwide—24% and 48%, respectively, among teens and young people. Based on these figures, the researchers estimate that the global number of teens and young adults who could potentially be at risk of hearing loss as a result ranges from 0.67 to 1.35 billion.

Abstract: Recent findings are challenging the classical hematopoietic model in which long-term hematopoietic stem cells (LT-HSC) are the base of the hematopoietic system. Clonal dynamics analysis of the hematopoietic system indicate that LT-HSC are not the main contributors of normal hemapoiesis in physiological conditions and the hematopoietic system is mainly maintained by multipotent progenitors (MPPs, hereafter HPC) and LT-HSCs are mostly in a non-active state. The first HSCs emerge from the aorta-gonad and mesonephros (AGM) region along with hematopoietic progenitors (HPC) within hematopoietic clusters. Molecular pathways that determine the HSC fate instead of HPC are still unknown, although inflammatory signaling, including NF-KB has been implicated in the development of HSCs. Here, we identify a chromatin binding function for IKB (also known as the inhibitor of NF-KB) that is Polycomb repression complex 2 (PRC2)- dependent and specifically determines dormant vs proliferating HSCs from the onset of their emergence in the AGM. We find a specific reduction of LT-HSCs in the IKB knockout new-born pups. This defect is manifested at the FL stage already, and traceable to the first emerging HSCs in the E11.5 AGM, without affecting the general HPC population. IKB-deficient LT-HSCs express dormancy signature genes, are less proliferative and can robustly respond to activation stimuli such as in vitro culture and serial transplantation. At the molecular level, we find decreased PRC2-dependent H3K27me3 at the promoters of several retinoic acid signaling elements in the IKB- deficient aortic endothelium and E14.5 FL LT-HSCs. Additionally, IKB binding itself is found in the promoters of retinoic acid receptors rar in the AGM, and rar{gamma} in the LT-HSC of FL. Overall, we demonstrate that the retinoic acid pathway is over-activated in the hematopoietic clusters of IKB-deficient AGMs leading to premature dormancy of LT- HSCs that persists in the FL LT-HSCs. Vor ziemlich genau 20 Jahren schickte sich eine kleine Gruppe von Unternehmen an, etwas zu schaffen, von dem es zuvor jahrzehntelang hieß, dass es schlicht unmöglich sei: Immobilienkredite auf einer zentralen Plattform miteinander vergleichbar zu machen. Die Argumente der "Geht nicht"-Fraktion: es gebe Dutzende Variablen bei der Kreditwürdigkeit der Interessenten und dann wiederum Dutzende bei dem Kauf- und Bauvorhaben selbst. Das alles in eine zentrale, vergleichbare Anfrage zu packen sei schlicht unmöglich.  Doch die Gründer der Interhyp, der Hypoport und auch der Plattform Planethome gaben sich mit dem "Geht nicht"-Argument nicht zufrieden, versuchten es doch. Mit durchschlagendem Erfolg: die Interhyp ging 2008 für knapp eine halbe Milliarde an die ING, Hypoport ist an der Börse 3,2 Mrd. Euro Wert, und Planethome ist auch zu einem Unternehmen mit über 700 Mitarbeiterinen und Mitarbeitern und 115 Mio. Umsatz gewachsen. Rund ein Drittel aller Immobilienfinanzierungen laufen über Plattformen.  Gründer und damaliger Vorstand von Planethome war: Ralph Müller. Der ist nun Vorstand bei der IKB Deutsche Industriebank AG, eine Bank mit wechselvoller Geschichte in der Finanzkrise (auch darüber war zu reden), die aber inzwischen im Besitz des Finanzinvestors Lone Star wieder hohe einstellige Eigenkapitalrenditen erwirtschaftet.  Unter Müllers Mitarbeit hat die IKB nun etwas vor, was auch seit 20 Jahren angeblich nicht geht: Mittelstandskredite auf eine zentrale Plattform zu ziehen und vergleichbar zu machen. Zusammengeschlossen hat man sich dazu mit: Hypoport. Also jenem Unternehmen, das schon einmal die Silos aufzubrechen half, die es bei der Kreditvergabe im Immobilienbereich gab. Im Januar soll es losgehen, 50 Banken sind bereits an Bord.  Anlass für uns, einmal mit Müller in unserem Podcast zu sprechen: was genau haben die IKB und die Hypoport da vor? Hasst man die beiden dafür, dass nun auch bei Mittelstandskrediten bald die Margen zerbröseln könnten wie einst in der Immobilienfinanzierung? 嘉宾:赵清(建筑师)主播:nada 剪辑:SJ起因是在建筑展上人挤人。想了想,这背后的原因不是一两句话能说清的。· 为何不同的艺术机构都在某一时刻意识到,该专门策划建筑展了?· 建筑要怎么展?毕竟我们在展馆里看到的都不是实物。· 如果策划一片厉害的建筑群,它究竟是展览,还是对城市空间的再塑造?· 外国建筑师来中国办展的时间,为何与他们在中国接项目的时间如此巧合?为了展开讲讲,于是有了这样一期节目。# 名词解释:· PSA上海当代艺术博物馆,成立于 2012 年,是内地第一家公立当代艺术博物馆。PSA 的建筑由原上海南市发电厂改造而来,最有标志性的是一座大烟囱。选址在黄浦江边,一开始便有意模仿伦敦的泰特美术馆(Tate Modern)。2013 年,PSA 开始“建筑与城市”的展览和研究,举办过展览的建筑师和事务所包括:篠原一男、尤纳· 弗莱德曼、托马斯·赫斯维克、藤本壮介、坂本一成、伯纳德· 屈米、伊东丰雄、巴克里希纳· 多西、坂茂、SuperStudio 超级工作室。其中又以日本建筑师居多。· 石上纯也日本建筑师,毕业于东京艺术大学。石上纯也风格明显,他的建筑常常是造型主导,而非功能,其中的代表作有“住宅与餐厅”、“森林幼儿园”、“儿童乐园”。他认为,建筑早已不再是一个简单的遮蔽物,而是巨大的世界空间里的“风景”。除了建筑,石上纯也设计桌、椅,造型同样是非常规的。· OCAT华侨城当代艺术中心,一家民营美术馆,由华侨城集团赞助。在深圳、上海、北京、西安都设有分馆。OCAT 的学术委员会主席是艺术史学者、策展人、芝加哥大学教授巫鸿。· 克莱因伊夫·克莱因(Yves Klein),法国 1950 至 1960 年代前卫艺术的代表人物,因单色画创作而出名,其中以 IKB 191为最,并出现了“克莱因蓝”一词。克莱因还创作了《单音-沉默交响曲》(The Monotone-Silence Symphony),乐团将单一音符持续演奏 10 分钟,再进入 10 分钟的沉默。· 谷之教堂石上纯也在山东日照白鹭湾设计的一座教堂。与传统教堂造型大相径庭,如弯曲折叠的纸片,高耸轻薄,缝隙狭长。据称 2020 年将完工。· 光之教堂日本建筑师安藤忠雄的代表作之一,1989 年建成,位于日本大阪。设计的重点是在教堂的一面墙上开了一个十字形的洞,营造出特殊的神圣感。· 朗香教堂位于巴黎东南面的朗香,柯布西耶设计,1954 年建成。它的风格难以归类,不规则的形状和曲面完全不同于当时流行的“白色盒子”式的现代建筑风格,也是非常不“柯布西耶”的建筑作品。· Superstudio超级工作室,源自 1960 年代的意大利激进建筑小组 Superstudio,被认为影响了诸多建筑师,但至今没有一件真正的建筑作品。他们提出的“超建筑”,主张重新审视建筑,把它作为一种批判政治、社会的文化工具。· 王大闳台湾重要的现代主义建筑师,和贝聿铭是哈佛建筑系的同窗。王大闳在战后的台湾设计了许多经典建筑,最出名的是国父纪念馆,但王大闳本人一直认为这是个“妥协”的作品,并不满意。后来,王大闳的传记作者倪安宇写道,从许多官建筑设计案中,“王大闳悟出了建筑为政治工具的道理”。 · 冯纪忠、王大闳建筑展指 2014 年在 OCAT 上海分馆举办的《久违的现代:冯纪忠、王大闳建筑文献展》。推荐阅读《史建谈“久违的现代:冯纪忠、王大闳建筑文献展”》,http://artforum.com.cn/words/7033· 埃莱娜·比奈建筑摄影师。埃莱娜擅长从建筑的细部入手,对建筑提出个性化的诗意解读,而非强调建筑的整体结构和通常的空间感。除了现代建筑,埃莱娜还拍摄了苏州园林。· 上海、威尼斯、深港双年展,和建筑的关系上海双年展从 2000 年第三届开始,加入了建筑的部分;威尼斯建筑双年展从1980 年开始举办,此前,建筑是艺术双年展的一部分。深港双年展的全称是“深港城市建筑双城双年展”,是全球少有的以城市或城市化为固定主题的双年展,特别突出建筑师的角色。除了城市更新,也谈论城中村、边缘地带和公民意识。· 菲利普·约翰逊美国现代主义建筑师,普利兹克奖的第一位得主。早年在哈佛大学主修历史和哲学,未能结业。后成为“包豪斯”代表人物密斯·凡·德罗的追随者。1930年,菲利普·约翰逊在 MoMA 成立了建筑与设计部门,对包豪斯和现代主义风格在美国的流行有很大影响。· 现代建筑国际展览会1932 年在 MoMA 举办的一场重要展览“国际样式:1922年以来的建筑”。策展人为菲利普·约翰逊和亨利-罗素·希区柯克,展出的重要建筑师包括柯布西耶、格罗皮乌斯和密斯。但也因未能重视美国国内重要的建筑师弗兰克·赖特,而引发争议。· 密斯“包豪斯”代表人物密斯·凡·德罗,主张极简风格,名言是“少即是多”。1929 年设计的巴塞罗那国际博览会的德国馆是其代表作。1937 年移民美国,并在芝加哥长期生活。推荐阅读:西班牙插画师、作家 Agustín Ferrer Casas 创作的漫画《密斯》。· Mies in BerlinMoMA 在 2001 年推出的展览· MoMA 的建筑改造MoMA 在 1997 年花费 8.58 亿美元改造,2006 年建成后展览及项目空间多了一倍。2017 年,MoMA 又计划耗资 4 亿美元扩建,今年 10 月重新开幕。至此,MoMA 已经翻新 6 次。一些人相信扩建翻新能让博物馆获得新生,但反对者认为,这不过是地产故事。· Steven Holl美国建筑师,以设计美术馆和文化教育类的公共建筑著称。在中国的项目包括成都来福士和北京当代 MOMA,皆为大型商业综合体。Steven Holl 认为,他在中国遇到了理想的甲方,而在美国,1980 年代开始建筑业的衰落,让建筑师失去尊严。· 西扎阿尔瓦罗·西扎,葡萄牙建筑师,普利茨克奖得主,颁奖词是 “西扎借由光来塑形他的设计,有一种似是而非的朴实感,并坦率地展现在世人眼前。”在中国美术学院设计的中国国际设计博物馆在 2018 年完工。· 四方当代艺术湖区位于南京佛手湖附近,最早地产商陆军打算建造精品住宅项目。2003 年,其子陆寻邀请 12 位中国建筑师、 12 位国际建筑师,计划推出建筑群。· 长城下的公社坐落在长城脚下的建筑群,2002 年初完成,由地产公司 SOHO 的张欣和潘石屹投资,邀请 12 名亚洲知名建筑师设计,定位是“私人收藏的当代建筑艺术作品”。曾受邀参加威尼斯双年展。现在是由凯宾斯基管理的高档酒店。· 金华建筑艺术公园2002 年开始的计划。公园中的 17 个小型公共建筑由 17 组、21 位建筑师、艺术家设计。公园于 2007 年开放,但 2012 年被报道“荒废”。· Heatherwick托马斯·赫斯维克(Thomas Heatherwick),英国建筑师,设计夺人眼球,常因合理性引发争议。因为 2010 年世博会的英国馆(“种子教堂”)设计,变得更出名。最近备受关注的是纽约哈德逊广场上的 Vessel。· B.I.G.正当红的丹麦建筑师 Barke Ingels 创办的事务所,他的设计理念是既要可持续发展,又要有趣好玩。· 皮亚诺伦佐·皮亚诺(Renzo Piano),意大利建筑师,普利茨克奖得主。最著名的作品是巴黎的蓬皮杜艺术中心。他在中国的第一个项目是江南布衣新总部,位于杭州。· 乡建指乡村建筑。包括民宿,也包括政府主导的农民住房的改造,有时两者会有模糊地带。· 泰晤士小镇位于上海松江,靠近大学城,是 2001 年政府发起的“一城九镇”项目的一部分,规划面积 1 平方公里,希望效仿英国的农村村庄,设计公司也来自英国。2004 年交付时,很快售罄,价格不菲,但实际常住的人很少,来拍婚纱的人很多。除了泰晤士小镇,其余 8 个郊区小镇分别模仿北欧、意大利、西班牙、加拿大、荷兰和德国的建筑风格。· “孤独图书馆”三联海边公益图书馆,由建筑师董功设计,2015 年在网上走红。所在的阿那亚是一个地产项目,位于北戴河海岸,已开发 6 期,网红建筑也与日俱增。· 王澍建筑师,普利茨克奖得主,中国美院建筑艺术学院院长,工作室起名“业余”。王澍大部分建成项目都坐落在杭州和宁波,他设计了中国美院象山校区;在杭州富阳,王澍同时完成了“文村”的民宅改造,和富阳美术馆、博物馆、档案馆。· 西岸位于上海的徐汇滨江地区,政府希望效仿巴黎左岸、伦敦南岸,打造文化地标。现有龙美术馆(西岸馆)、余德耀美术馆、上海摄影艺术中心、油罐艺术中心。· 衡复微空间位于上海的衡复街道的五岔路口,面积 20 平方米,原为一家轮胎店,在 2017 年由梓耘斋建筑事务所改造为展览空间,周边的菜场、居民楼的立面也被修缮。但该空间现已改为“智能交投与人工交投相结合的资源回收站”。· 冯纪忠建筑师,同济大学建筑与城市规划学院创始人,创办中国第一个城市规划专业及风景园林专业,担任同济建筑系系主任 27 年。1946 年从奥地利维也纳留学回国,曾参与“大上海都市计划”,但计划最终没能实施。冯纪忠反对苏联专家对上海发展“摊大饼”的主张,认为要保留老城区。1978 年,受上海市政府委托,冯纪忠设计了方塔园,“与古为新”,但受到批评。冯纪忠还设计了何陋轩。· 何陋轩位于上海松江的方塔园东南角,一片竹林深处,1986 年建成。建筑师王澍非常钟爱何陋轩,他认为何陋轩的“中国性”不是靠“表面的形式或符号”支撑,而是有强烈意识,并“贯彻到建造的细枝末节”。冯纪忠自己的说法是,何陋轩早十年做不了,因为文革刚过,接受不了不伦不类的东西。晚十年也做不了,中国的经济已经发展起来了,不会让他以茅草和竹子去盖一间房子,这显得太贫穷了。· 延光阁位于苏州艺圃园内,苏州园林中最大的水榭。设备:Blue Yeti(网红产品的扑街,已弃用)片尾:赵十「上层建筑」是一档关于城市的播客节目。从设计出发,谈论城市有关的一切,既谈经济基础,也谈上层建筑。欢迎给我们写信 senttozoo@gmail.com

„Ég held oft að stutt líf geti sagt okkur meira um lífið en langt líf,“ segir Laufey Helgadóttir listfræðingur um franska listamanninn Yves Klein, sem lifði hratt og ákaft, lést 34 ára gamall árið 1962. Hann var júdókappi, sjálflærður listamaður, félagi í Rósakrossreglunni, og gjörsamlega dolfallinn yfir bláa litnum. Þótt listferillinn væri aðeins sjö ár liggur eftir hann aragrúi af verkum og skrifum sem komu honum í bækur listasögunnar sem einum áhrifamesta listamanni Frakklands á 20. öld, ásamt Marcel Duchamp. Hann er þekktastur fyrir bláar einlitamyndir sínar, mónókrómin svokölluðu, en hann þróaði litinn sjálfur og nefnidi International Klein Blue (IKB) og fékk einkaleyfi fyrir. „Það var bindiefni í litnum sem gerði það að verkum að útgeislun þessa bláa litar var akkúrat sú sem hann var að leita að. Hann vildi að áhorfandinn myndi sogast inn í verkið. Að útgeislunin frá verkinu yrði það mikil að það listmagnaði allt umhverfið og áhorfandinn gæti þannig týnt sér í verkinu,“ segir Laufey um bláa IKB-litinn. Blái liturinn var táknrænn fyrir Klein, í litnum fann hann fyrir frelsi. Hann sagði: „Fyrst er ekkert, síðan er alls ekkert, svo kemur djúp blátt.“ Með tímanum færði hann sig frekar inn á svið óefniskennds myndnæmis. Markmiðið var að með tímanum yrði ekki neitt; tóm. Hann seldi til að mynda nokkur óefniskennd verk á ferlinum; sumsé ekkert. Hann tilheyrði hópi nýraunsæismanna en list hans flæðir inn á svið alls konar stefna og strauma; mínímalisma, landslags, ferlis- og líkamslistar, gjörningalistar, og annarra framúrstefnuhreyfinga sjöunda áratugar síðustu aldar. Í þætti dagsins skoðum við merkilegt en stutt lífshlaup Yves Kleins sem og listferil hans með áherslu á bláu einlitamyndirnar. Viðmælandi minn er Laufey Helgadóttir listfræðingur. Hann vildi að áhorfandinn myndi sogast inn í verkið. Að útgeislunin frá verkinu yrði það mikil að það listmagnaði allt umhverfið og áhorfandinn gæti þannig týnt sér í verkinu,“ segir Laufey um bláa IKB-litinn. Blái liturinn var táknrænn fyrir Klein, í litnum fann hann fyrir frelsi. Hann sagði: „Fyrst er ekkert, síðan er alls ekkert, svo kemur djúp blátt.“ Með tímanum færði hann sig frekar inn á svið óefniskennds myndnæmis. Markmiðið var að með tímanum yrði ekki neitt; tóm. Hann seldi til að mynda nokkur óefniskennd verk á ferlinum; sumsé ekkert. Hann tilheyrði hópi nýraunsæismanna en list hans flæðir inn á svið alls konar stefna og strauma; mínímalisma, landslags, ferlis- og líkamslistar, gjörningalistar, og annarra framúrstefnuhreyfinga sjöunda áratugar síðustu aldar. In der vorliegenden Arbeit wurde versucht, Konzepte für die Evaluation der Behandlungskosten von Intensivpatienten zu erstellen. Die so gewonnenen Erkenntnisse fanden Eingang in Ansätze für eine Optimierung der Ausgaben. Abschließend wurden alternative, vom DRG-System unabhängige Modelle für die Erlösberechnung untersucht. Hierfür wurden bei 390 Patienten die Behandlungskosten für ihren Intensivaufenthalt erhoben und mit aus DRG Datensätzen errechneten, durchschnittlichen Behandlungskosten, hier als „Fallwert“ bezeichnet, verglichen. Zusätzlich wurden die Ausgaben für Labordiagnostik, eventuell erwirtschaftete Zusatzentgelte (ZE), Liegedauer, Beatmungsstunden und die erreichte Anzahl an TISS28 und Intensivkomplexbehandlungspunkten (IKB) eines jeden Falles bestimmt. Die summierten Kosten aller betrachteten Fälle beliefen sich auf 5.594.345,89€ und lagen damit 389.541,65€ über dem errechneten Fallwert von 5.204.804,24€. Erst durch die ZE ergab sich eine positive Kostendeckung von 327.044,87€. Die 139 prä MDC Fälle des betrachteten Kollektivs verursachten Kosten von 4.794.728,03€ (4.060.487,28€ abzüglich ZE) und lagen damit 766.822,68€ (51.317,58€) über dem Fallwert. 55% der prä MDC Ausgaben entfielen auf die Personalkosten (PK) für ärztlichen Dienst (ÄD), Pflege (PD) und medizinisch-technischen Dienst (MTD). Sowohl bei den PK ÄD als auch den PK PD wiesen Fälle mit nichtgedeckten PK signifikant höhere Liegezeiten und PK pro Stunde auf. Die Kalkulation der PK basiert auf der aufwandsgewichteten Liegedauer, somit bilden die Kosten pro ungewichteter Stunde indirekt die Komplexität eines Falls ab. Eine lineare Regressionsfunktion zeigte jedoch, dass dieser Faktor im Vergleich zu der Liegedauer nur einen geringen Einfluss auf die Abweichung der Personalkosten vom geschätzten Fallwert hatte. Die Kosten der 251 nicht-prä MDC Fälle lagen bei 799.617,86€ und damit 377.281,02€ unterhalb des errechneten Fallwertes von 1.176.898,88€. Die ZE beliefen sich auf nur 1.081€. Die Personalkosten verursachen in diesem Kollektiv 61% der Gesamtkosten, und auch hier wiesen Fälle mit nicht gedeckten PK signifikant höhere Liegezeiten und Kosten pro Stunde auf, wobei der erstgenannte Faktor wiederum den größten Einfluss auf die Abweichung der Kosten vom Fallwert hatte. Verlässt man die Aufteilung nach MDC, so verursachten Pneumoniepatienten mit median 28.082,43€ die höchsten Kosten, gefolgt von Fällen mit Organersatztherapie mit 25.088,12€. Beide Male führte auch die Berücksichtigung von ZE nicht zu einer Deckung der medianen Ausgaben. Bei den Patienten mit Organersatztherapie lagen jedoch die summierten Kosten nach Abzug der ZE unterhalb des Fallwertes. Bei Aufnahmen aus externen Kliniken waren die mediane Kosten auch mit ZE über dem medianen Fallwert angesiedelt (14.728,89€ vs. 12.415,49€), die summierten Kosten wiesen hier von allen betrachteten Kollektiven die größte Abweichung vom summierten Fallwert auf (1.227.248,42€ bzw. abzüglich ZE 975.405,76€ vs. 789.660,45€). Die medianen Kosten der 30 Nonsurvivor waren selbst mit den ZE nicht gedeckt. Aufgrund von Ausreißern bei den Erlösen aus ZE lagen die summierten Kosten dieses Kollektivs jedoch 233.988,43€ unterhalb des Fallwertes. Somit erzielte diese Gruppe einen positiven Erlös, der über dem alle übrigen Fälle (93.056,44€) lag. Die medianen Ausgaben bei Transplantationspatienten lagen bereits ohne ZE unterhalb des geschätzten medianen Fallwertes (9.501,64€ vs. 18.735,66€). Der in einem nächsten Schritt durchgeführte Versuch, Fallkosten nicht anhand von vordefinierten Fallwerten, sondern alleine über die Kosten pro TISS28, IKB, Beatmungsstunde oder Behandlungsstunde zu evaluieren, erwies sich als wenig erfolgreich. Legte man die aus den DRG Daten errechneten Fallwerte zugrunde, so konnte nur für die Kosten pro TISS28 und nur im Kollektiv der prä MDC Patienten eine suffiziente Vorhersagekraft bezüglich der Kostendeckung festgestellt werden (AUC=0,797). Die Sachkosten Arzneimittel (SK AM) beliefen sich auf insgesamt 1.185.556,65€ und lagen damit 577.937,51€ über dem errechneten Wert. Von den Gesamtkosten entfielen 555.220,06€ auf den Ist-Verbrauch von teuren Antibiotika, Antimykotika und bestimmten Blutprodukten (SK AM mit Einzelkosten). Dies wurde als Ansatzpunkt für eine Überlegung zur Ausgabensenkung gewählt. In der Literatur besteht der Konsens, dass durch zielgerichtete Pharmakotherapie Kosten gesenkt werden können. Hiervon ausgehend wurde überprüft, ob in dem betrachteten Datensatz ein statistischer Zusammenhang zwischen den Ausgaben für Labordiagnostik und Arzneimittel bestand. Eine lineare Regressionsanalyse zeigte einen negativen Einfluss der Laborausgaben auf die SK AM mit Einzelkosten. Eine getrennte Betrachtung der mikrobiologischen Diagnostik und der klinischen Chemie ergab nur für letztere ein negatives Einfließen in die Regressionsfunktion. Ausgehend von den erhobenen Daten scheinen Mehrausgaben für einen bestimmten Bereich der Labordiagnostik somit eine Kostenersparnis bei Arzneimitteln zu ermöglichen. Abschließend wurde die Problematik der Kostendeckung bei hochkomplexen Fällen zum Anlass genommen, alternative, vom DRG-System unabhängige Methoden für die Erlösberechnung zu untersuchen. Es sollte gezeigt werden, dass aus klinischen Daten die tatsächlichen Behandlungskoten suffizient berechnet werden und diese Werte damit auch als Grundlage für die Erlösbestimmung dienen können. Von den vier untersuchten Variablen (TISS28, IKB, Beatmungsdauer und Liegedauer) zeigten der TISS28 und die Liegedauer die höchste Kostenkorrelation (R²=0,964 und R²=0,944). Bei Patienten mit einer Aufenthaltsdauer von unter 48 Stunden korrelierte von allen Variablen jedoch nur noch der TISS28 suffizient mit den Ausgaben (R²=0,884). Nach Randomisierung der betrachteten Fälle auf zwei Gruppen wurde an Kollektiv 1 für jede Variable eine Regressionsfunktion erstellt, welche bei Kollektiv 2 die Behandlungskosten vorhersagen sollte. Zuvor durchgeführte Überlegungen kamen zu dem Schluss, dass eine kubische Funktion das Verhältnis von Kosten zu Variable am besten abbildet. Die mit dem TISS28 geschätzten Kosten wichen bei den prä MDC um median -1,76%, beim Gesamtkollektiv um median -1,78% von den tatsächlichen Kosten ab. Die Schätzung anhand der Liegedauer erreichte Abweichungen von median -6,75% und -1,30%. Die mit der IKB erzielten Werte divergierten dagegen stark von den tatsächlichen Kosten (-9,57% und 16,02%). Die Kosten pro IKB Punkt wiesen jedoch eine hohe Varianz auf, was zusammen mit einer geringen Fallzahl zu einem ungenauen Regressionsmodell und damit einer eingeschränkten Beurteilbarkeit der Ergebnisse führte. Durch den Vergleich mit definierten Fallwerten ist es folglich möglich, die intensivmedizinischen Kosten von verschiedenen Patientenkollektiven zu evaluieren und Ansätze für die Optimierung der Ausgaben zu entwickeln. Auch wäre ein von Fallpauschalen losgelöstes System der Erlösberechnung, welches auf relativ simplen, klinischen Parametern basiert, denkbar.
