In this episode I discuss failure analysis component or joint failures during product testing or use and manufacturing problems such as supplier quality problems and manufacturing output that doesn't meet specifications. Failure analysis is part of performing a root cause analysis to identify the action, event, or decision that led to a component failing in a product or led to a manufacturing problem. The information from a root cause analysis is used to either fix a problem - in the case of product design and manufacturing - or assign blame - in the case of an insurance claim or litigation. On this episode of The Gas Compression Podcast, host Michael Haning welcomes Jim Detorre, President at Failure Analysis Services, Inc. With over three decades of machining background and over two decades working for Caterpillar dealers, Jim details his experience transitioning to failure analysis services. What is the typical failure analysis procedure and what are the best practices? Jim gives a thorough walkthrough, skimming across the surface of the complexities involved in diagnosing the failure. He provides a multitude of questions to gather as much factual data as possible and interpret those facts and why the failure occurred. Jim estimates that about 75% of failures are due to contamination and lack of cleanliness. Any time the compressor frame or engine is opened up, it risks contamination. The environment for servicing work should be contained and clean if at all possible. Before the pandemic, Failure Analysis Services hosted 30 classes a year and is now offering in-person courses again. Check out their website for more details. Today I have a very special guest on for an unscripted show. Meet my 10-year-old son Mason. He is in 4th grade and his favorite sport is wrestling. He loved wrestling during the first two years because he had great success. In his third year, he got a new coach, learned new moves, and was moved into a new wrestling bracket. He lost all his matches that season and was devastated.  Mason and I are going to go over how he felt when he won and lost, what to do with those feelings, and how to learn from both winning and losing. This will give an example of how to do a failure analysis with your child so that you can instill the growth mindset that will help them triumph through all life's ups and downs. Safety Podcast, Safety Program, Investigations, Human Performance, Safety Differently, Operational Excellence Bob is a degreed Aerospace Engineer, having graduated from Texas A&M University in 1970. He worked at McDonnell Douglas as an Aeronautical Loads Engineer on the F4 Phantom Fighter until 1973, then became employed at Allied Chemical's Nylon plant in Chesterfield, Virginia --an internationally recognized leader in Manufacturing Reliability at the time. Nelms eventually became part of Allied's Corporate Reliability Center, where he concentrated his attention on Failure Analysis until the Center was disbanded in 1986 -- whence he formed Failsafe.   His professional work initially focused on the physical causes of machinery failure. He developed an expertise in experimental stress analysis, including strain gage and photoelasticity studies. He also became involved in probabilistic reliability studies, including Weibull and other statistical analysis methods. But his passion emerged when it became obvious to him that human beings cause all physical failure. Since 1996, Bob has devoted most of his time trying to clarify the sensitive human issues at the root of all failure.
