The average American spreads their food consumption out over 15 hours a day, which basically means they're eating from the time they wake up until the time they go to bed and fasting only during sleep. Increasing this fasting window by even just a few hours can provide a multitude of health benefits, (1, 2, 3) including: Decreased rates of obesity Improved blood pressure Decreased insulin resistance Better lipid profiles Decreased risk of cardiovascular disease Decreased cancer risk Decreased inflammation These benefits are seen with many different fasting practices. With time-restricted feeding, for instance, an individual eats for only a portion of the day, say 8-12 hours, and fasts the remainder of the day. Periodic prolonged fasting, on the other hand, ranges from as little as 24 hours to up to 3-5 days during which an individual is consuming only water. Keep in mind that fasting isn't for everyone. Those who have a history of disordered eating or diabetes, are pregnant or breastfeeding, or are participating in high-volume training should all consult with a doctor before fasting. However, if fasting is right for you, it can be a small, easy-to-implement change that can have a significant positive impact on your health. “If a woman can understand how she feels on different days her cycle, then she can start dialing and being more personalized with her training. When the hormones are low, this is where women have more power, more speed, recover better, have more “mojo” because they have less central nervous system fatigue. Around ovulation, with that boost of estrogen, some women feel bulletproof and other women feel a little bit flat, and then a couple days later they'll feel bulletproof, so, understanding that as well. And then when you get into the high hormone phase, with estrogen and progesterone being the highest, this is where we want women to deload more, where they're working functional technique and they're not doing too much high intensity, they're not trying to do top-end VO2 work because physiologically you're fighting your body because there's so many different metabolic and thermoregulatory and metabolic shifts that happen with the hormones. It's really hard to do that high, high intensity in the high hormone phase.” - Dr. Stacy Sims In the words of Stacy Sims, Ph.D, women are not small men.  Dr. Sims is an applied researcher, innovator, and entrepreneur in human performance, specifically sex differences in training, nutrition, and environmental conditions.   She is dedicated to educating women so that they can use their natural cycle to their advantage and train with their physiology, not against it. Dr. Sims earned her Ph.D. in Environmental Exercise Physiology and Sports Nutrition from the University of Otago in New Zealand.  From there, she began work as an exercise physiologist and nutrition scientist at Stanford University, where she specialized women's health and performance. During her tenure at Stanford, she had the opportunity to translate earlier research into a science-based layperson's book, ROAR: How to Match Your Food and Fitness to Your Unique Female Physiology for Optimum Performance, Great Health, and a Strong, Lean Body for Life.   In the book, she provides insights into how women can adapt their nutrition, hydration, and training to optimize their performance across the lifespan. Dr. Sims's contributions to the international research environment and the sports nutrition industry have established a new niche in sports nutrition and established her reputation as an expert in sex differences in training, nutrition, and health. She is a regularly featured speaker at professional and academic conferences, including those hosted by US Olympic Committee, High-Performance Sport NZ, and USA Cycling, and is a Senior Research Associate at AUT University. In this episode, we dissect several practices for shift preparation with a common goal of operating at a peak level of performance and experiencing more joy in what we do. Your shoulder is the most complicated joint in your body.  The shoulder bones are just floating in space! Just like your glutes (see prior podcast), our lifestyles spent mostly sitting at desks puts us in a slouched position, with drooping shoulders and poor scapular activation.  In this podcast Dr Mike Mallin and Dr Mark Goodman talk again with our favorite physical therapist, Denny Dragon of Redbird Physical Therapy in Bend, OR.  Denny helped Mike rehab his shoulder after a rotator cuff tear...or two.  What we Learned: 1) Scapular elevation is important to protect the rotator cuff. 2) Chances are good you're doing the bodybuilder shoulder raise and don't even know it.  3) Mark doesn't move his shoulder right. 4) The best way to train your scapula to move is to do lots and lots of shoulder raises, the right way.

Your butt may be more important than you ever realized! Our sedentary lifestyles spent mostly sitting at desks train your glute not to activate.  This leads to imbalances, weakness, and injury. In this podcast Dr. Mike Mallin and Dr. Mark Goodman talk with one of our favorite physical therapists, Denny Dragon of Redbird Physical Therapy in Bend, OR. Denny has and continues to treat world class athletes and knows more about the glutes than anyone we know.    What we Learned: 1) The glute medius is responsible for stabilizing the hip joint and instability leads to knee pain, hip pain, and ankle sprains.  2) The glute maximus is the "powerhouse" and we can optimize its use through some simple drills.  3) Mike has a weak glute medius 4) The best way to activate your glute medius is to do banded lateral walks.

Peter Weimersheimer: Pelvic Ultrasound: If you can get the answer with transabdominal US, you don’t necessarily need to go for the endocavitary probe TEE: Why learn this if I get good TTE views during arrest?  Answer: Because you often can’t.  Also, some tips on how to start a TEE program.
