“Women are getting off birth control amid misinformation explosion.” - a very problematic article in the Washington Post last week. Written by two people with no medical background, the article is highly politicized, filled with anecdotal evidence, and dismissive of women's lived experiences with birth control. Once I saw it, I had to address it here on the podcast because it's disgusting. Birth control is obviously not the ultimate evil, but it's also not the ultimate panacea. It should not be prescribed for problems outside of birth control until the underlying factors have been explored. Anything else is a disservice to our patients and is done for the convenience of the prescribing physician. In this episode, I'm giving you my take and the facts on everything written. How are we at a point where women are terrified of a hormone they've had in their bodies most of their lives? Many of us have heard horrible things about estrogen and its connection to breast cancer. A lot of this public aversion stems from a study that came out in the early 2000s called the Women's Health Initiative (WHI). The WHI was a landmark study designed to define the risks and benefits of interventions, notably hormonal therapy. Unfortunately, when journals and the media reported on the findings, they didn't put them into the appropriate clinical context and did not mention the study's limitations. Ultimately, this led to women ditching their hormones en masse, suffering in silence without an alternative. Today I'm sharing what you need to know about the WHI, what the data shows about the connection between hormone therapy and the risk of disease, why we need to shift our perspective on hormone therapy, and alternatives for women who can't benefit from estrogen therapy. I can't bring myself emotionally, spiritually, psychologically, or physically to use hormone pellets with my patients. Hormone pellets are tiny pellets containing hormones and medications inserted under your skin and replaced every three to six months. In my clinic, many unhappy patients come to me with complications from pellets. Outside of my clinical experience, I also tried pellets myself a few years ago, which was the worst experience of my life. We're back for part two of this special series of interviews with my kids. Last week, you heard from my oldest-  and only daughter - Hannah, and today, I'm talking to my oldest son, Hunter. Hunter is 28, and the only one of my children who decided to go into medicine, and he's currently in medical school. Hunter was born in Oklahoma City during the third year of my residency in 1996. Probably the most adaptable of the four, his mother and I divorced when he was young, and later, in the sixth grade, he came to live with me and his stepmom, Katie. Polycystic Ovary Syndrome (PCOS) is a relatively common condition that affects many women of reproductive age. Despite that, we still lack clear diagnostic criteria, resulting in many women receiving an incorrect diagnosis of PCOS or having PCOS and remaining undiagnosed. The reality of women's health in this country leaves much to be desired, from exclusion from biomedical research to gaslighting by providers and everything in between. It's a controversial topic, and we need to increase awareness because definite improvements need to be made. The thing with research is that new studies tend to get passed around on social media, sensationalized, and spread like wildfire. In many cases, it's probably not something you need to be overly worried about. Today, I'm breaking down a new study that came out earlier this year, reported in the New York Times. It was about menopausal hormone therapy and dementia, and it's been kicking up a stir. I'm talking about this today as an example of how to look at these types of studies and determine what's legit in a world where seemingly contradictory research seems to be constantly popping up. In this episode, I'm highlighting the strengths and limitations of the study and discussing the need for critical analysis when interpreting research studies, the differences between bioidentical hormone replacement and synthetic progestin, how to evaluate the risks and benefits of hormone therapy, why you need to do your research and more. Estrogen has many protective effects, from bone and heart health to thyroid function and sexual motivation. I can't tell you how often I've put a woman on estrogen only to return, saying she felt better for a couple of weeks but decided to stop the medication. Why? Because her friends told her that estrogen causes breast cancer. This narrative is spreading like wildfire, and it's frankly misogynistic. Why should women have to suffer the life-altering adverse effects of low estrogen when they have an alternative? How can you say across the board that no one should use this treatment when it has improved the lives of so many people? We've already done a couple of episodes on estrogen, and today I'm diving in again to get the message across that estrogen should not be feared. Endometriosis is a debilitating condition affecting millions of women worldwide, yet many go undiagnosed or misdiagnosed for years. Early diagnosis leads to better outcomes and improved quality of life. As part of our Endometriosis Awareness Month series, I'm joined by holistic health coach Khush Sra today. In addition to being an Endometriosis Nutritionist, Khush has personally suffered with endometriosis and was diagnosed around 20 years ago. She knows the ins and outs of endo, not just from a practitioner's perspective and as a woman and a patient. In my practice, I've found the best approach to treating endo is multifaceted and covers multiple angles. Many surgeons who are excellent at removing endometriosis tissue don't have a plan for preventing recurrence beyond prescribing the pill. This is where nutritionists like Khush can come in and provide additional tools and expertise. In this conversation, Khush and I discuss her journey from years of deteriorating health with severe endometriosis to being completely symptom free, her approach to working with endometriosis patients, the importance of understanding the individual when creating a treatment plan, and more. Progesterone is the great calmer. It's the hormone responsible for pregnancy and is probably my second favorite (right after testosterone). With its central role in the menstrual cycle, I've seen progesterone help many women in my practice. Menopause and perimenopause shouldn't be something you just power through, yet that's the way that many women deal with their hormone or energy issues. There's a better way, and it doesn't involve being afraid of estrogen and hormone therapy. Dr. Shawn Tassone answers your questions on cancer, estrogen, hormones, and health, and talks about why the narrative matters in getting to the root cause of a patient's problems. [3:05] Being a man in the arena of women's health gives him a different perspective. As the author of The Hormone Balance Bible, Shawn helps women transform their body and life during perimenopause and menopause. [4:10] Shawn found that if he could share stories of hormonal archetypes with his patients, it connects much better than just raw data. It gives women a starting point to get the information they are looking for and points them in the right direction to learn more. [5:50] Hormone issues can feel very isolating, like you're not being acknowledged. Hearing the stories of other people and how it affected them is affirming. [6:20] If 10,000 men's penises fell off tomorrow, there would be a national outrage and inquiry. But when hundreds of thousands of women are complaining of endometriosis, pelvic pain, or fibroids, they are told that it's just what happens when they get older. [8:10] The more women physicians that experience the menopausal period of life the more natural options and reception that women will get in our culture. [8:45] Speak up and share what's on your mind. There is always one next right step and the answer is never “come back next year and I'll check again.” In the case of perimenopause and menopause, there is always a next right step for being healthier next year than you are now. [10:50] Half of Shawn's patients bring labs from other physicians that say everything is normal. You can't just look at the numbers. You have to look at the person and how they personally feel. The narrative part of your patient's history matters. [12:10] Shawn tells the story of a mother run ragged trying to maintain and support her family and how the narrative of the saboteur helped her understand her situation. Women have a tendency to power through painful sex or exhaustion or stress. [15:40] Shawn's mother passed away from ovarian cancer and that started his own journey to learn how hormones influenced the situation. Oncologist's jobs are to help you live longer. They aren't particularly interested in quality of life. [19:10] When it comes to cancer, it's always important to try to identify the root cause whether it was a hormone disruptor or toxic exposure. With every single patient that Dr. Anna has seen with cancer, the issue was never their own natural production of hormones. [21:10] There are a number of factors when it comes to cancer. If it's overall health, Shawn points them to a macha product or fish oils and magnesium. [28:10] It's important for healthcare providers to communicate with each other. Dr. Anna had a patient that had been treated for breast cancer and had a Vitamin D hydroxy level of 11, where the optimal number is closer to 80. This was a major failure on the part of her healthcare team since Vitamin D is a key hormone player. [31:15] Estrogen is crucial in slowing bone loss while also maintaining brain, cardiovascular, and skin health. If your quality of life is impacted negatively because you're afraid of hormones, you're putting the cart in front of the horse. [33:05] There is a study that is ongoing that looks at the effect of hormones like estrogen on rates of cancer and it looks like there is no increased risk of breast cancer. The media has demonized estrogen to the detriment of both women and men. [34:10] Vitamin D doesn't make you feel any different, but it is helpful for the long term. The silent factors like Vitamin D levels are often the most overlooked. The cancer we get from a lack of sun exposure is much worse than anything you can get from too much sun exposure. [35:30] Hormone imbalances can make you into an entirely different person. Fixing your hormones can have near immediate results where you feel considerably better over the course of a week. Add in vitamins and supplements which work over a long period of time and you have a solid protocol. [37:45] At one point, progesterone was only prescribed to protect against uterine cancer. Menopause and perimenopause shouldn't be something you just power through, yet that's the way that many women deal with their hormone or energy issues. There's a better way, and it doesn't involve being afraid of estrogen and hormone therapy. There is always one next right step and the answer is never “come back next year and I'll check again.” In the case of perimenopause and menopause, there is always a next right step for being healthier next year than you are now. [10:50] Half of Shawn's patients bring labs from other physicians that say everything is normal. You can't just look at the numbers. You have to look at the person and how they personally feel. The narrative part of your patient's history matters. [12:10] Shawn tells the story of a mother run ragged trying to maintain and support her family and how the narrative of the saboteur helped her understand her situation. Women have a tendency to power through painful sex or exhaustion or stress. [15:40] Shawn's mother passed away from ovarian cancer and that started his own journey to learn how hormones influenced the situation. Oncologist's jobs are to help you live longer. They aren't particularly interested in quality of life. [19:10] When it comes to cancer, it's always important to try to identify the root cause whether it was a hormone disruptor or toxic exposure.
