The Derek and Sarah Tiller Family Endowed Football Scholarship will support Georgia high school offensive linemen, reflecting Tiller’s own football roots and honoring Aspinwall’s legacy. Tiller, a Wofford alum, aims to turn tragedy into a positive by giving future athletes life-changing opportunities while celebrating Coach A’s impact. The suspected gunman is in custody and was identified as a 14-year-old student of the school, which is about an hour outside of Atlanta. Here's what we know: Victims The four people killed have been identified as: - Mason Schermerhorn, a 14-year-old student - Christian Angulo, a 14-year-old student - Richard Aspinwall, a teacher - Cristina Irimie, a teacher The adults killed were both math teachers, and Aspinwall was also an assistant football coach, according to the school's website. Nine other people — eight students and one teacher — were wounded and hospitalized, all of whom are expected to survive. Suspected shooter The suspected gunman has been identified as Colt Gray. - Gray will be charged with murder and tried as an adult. - Gray was questioned by law enforcement last year regarding “several anonymous tips about online threats to commit a school shooting at an unidentified location and time,” according to FBI Atlanta and the Jackson County Sheriff's Office. At that time, there was no probable cause for arrest. • Colt Gray, a 14-year-old, has been charged with four counts of felony murder for the shooting at Apalachee High School. • Colt's father, Colin Gray, has been charged with involuntary manslaughter, second-degree murder, and cruelty to children for allegedly providing Colt with a firearm. • Prior to the shooting, authorities received anonymous tips about threats to shoot up a school, leading them to investigate Colt and his father. • The investigation revealed that Colin Gray had been teaching Colt about firearms and safety, claiming he was trying to get his son away from video games. • Colin Gray said he kept his firearms unloaded, but investigators found conflicting evidence regarding Colt's access to the weapons. • The shooting has devastated the community of Winder, Georgia, with residents grappling with the tragedy and remembering the victims. • Colt's aunt claims he struggled with mental health issues and sought help before the shooting, raising concerns about the lack of support he received. PT on ICE Daily Show. This is Dr. Jordan Berry, Lead Faculty for Cervical Spine and Lumbar Spine Management. Today we're chatting about a topic called Low Irritability Equals Function First. Okay, so I hope you're having an awesome Thursday. We're about to break down just a concept that I think matters when you're thinking about the novice versus the expert clinician and how they're efficient during their initial evaluation. This key concept of when you're thinking about going into the objective exam and you know the irritability is low or at least moderately low, we're always gonna test the functional movements first. Okay, so a few concepts that we talk about during our live cervical and lumbar spine management courses, when we're thinking about the objective exam and what the expert clinician does different as opposed to the novice, one of those things is that they have a very long, detailed, subjective exam, and they have a short, clear, and crisp objective exam. and how as you gain more experience and more pattern recognition, typically that will sway even more lopsided towards being a longer subjective while having a shorter and more dialed objective exam. And then another concept we talk about is that when the patient irritability is low, you have to be really aggressive during the physical exam testing in order to recreate the symptoms, right? Because if you under test, then you might not actually recreate those familiar symptoms to know that the treatment that you're about to apply is going to work and that you're moving in the right direction. And so, one way that you can accomplish both of those things, right, with keeping a short, clear and crisp objective exam, and then making sure that you're going to be aggressive during the physical exam testing when the irritability is low, is always thinking about testing the functional movements first. Okay, so let me give you a clinical example with this, and then we'll break it down and talk about why it matters and why it's important. So, Imagine that you're in an initial evaluation and you've done your body chart and you know that the symptoms are somewhere around the area of the lumbar spine, like we'll say low lumbar into the right glute wrapping around towards the right hip, maybe even like anterior lateral right hip as well. But you know there's some vague diffuse symptoms that are somewhere in the lumbar spine and somewhere in the hip as well. And during this objective, you also gather that an aggravating factor is squatting anything over 95 pounds. And so day one, during the initial eval, you know you're gonna be trying to differentially diagnose if the symptoms are coming from the lumbar spine, or if they're coming from the hip, or maybe both. But primarily, again, the initial evaluation, day one, during the objective exam, we're trying to tease out What is the primary symptom generator? We have to nail that down day one. What a novice would do is as they're going into the objective exam, they would likely just hammer through a battery of tests for the lumbar spine and the hip. So they'd probably have that person hop up and you're going through all the basic stuff, right? You're going through active range of motion, your joint exam, your segmental exam, potentially neurodynamics, your test and hit PROM and strength testing and palpation. You're essentially just working down this battery of tests to try to see if anything recreates the familiar symptoms. And so let's say that you go through that 12, 15 minutes of objective exam testing and you figure out that hip passive range of motion, like internal rotation or fader recreates that familiar hip pain. And so now we have an asterisk sign, right? We've got our, um, let's, let's call it internal rotation is what we're going to retest and we've recreated the familiar symptoms. So you've done a good job, right? You haven't done anything wrong, but I would argue that that is not expert level because number one, it took us a fairly long time to get to that answer of what is recreating the symptoms. And honestly, the patient doesn't really care about any of the stuff that you just tested. So, an expert here is going to look at function first. So, we might do some of the same objective testing that we did just a minute ago with the novice, but the first thing that we're going to do if the irritability is low to moderate is look at function. So, if the subjective exam we found out that anything over a 95-pound squat recreates the familiar symptoms, well, I'm going to look at a 95-pound squat. So I get that person out in the gym, maybe we do a warm-up set, and then we load up to 95, and right when they drop down, right when the patient drops down into the bottom of the squat, they get that familiar hip pain. Now, right then, you have one of your asterisk signs, but we could also modify that movement or try to tease out in real time if we can change the symptoms or affect them in any way. So let's say that person drops down into the squat, bottom of the squat, they get their symptoms, and you grab a big mobility band. wrap it around the hip, and give a big lateral distraction, a lateral pull, while they go down into a second rep of the squat, and the symptoms are completely gone. So think about what you've now done. Number one, you have a better asterisk sign, I would argue, because it's something that the patient actually cares about. It's functional, it's very easy to retest, but you've also clued yourself in on your differential diagnosis. Because if I can do something to the hip, right, do a self-mob to the hip or do a lateral distraction for the hip and immediately change the symptoms that we got with squatting, then I know when I go back to the table and I do my more traditional objective exam testing, I'm going straight to the hip. So maybe on day one now, I can leave all of the lumbar spine testing and maybe hold it off until day two. because now I know that I can affect the hip. Now we go back to the table. We do some of the objective testing and I go right towards PROM and I jam that hip up into IR and fader and recreate those familiar symptoms. Boom. Now we've got our two objective asterisk signs. We've got one passive range of motion. We've got one that's functional, the squat. So now when I apply it to some sort of treatment, I've got two ways that I can retest. SUMMARY So number one, why this matters so much of testing function first when irritability is low is differential diagnosis. It's just a fast way to identify oftentimes where the symptoms are coming from or at least cluing you in as to what direction you need to go in instead of just testing all the lumbar spine stuff and all the hip stuff. Now I've clued myself in that I'm probably going to focus on hip day one. So the second thing why it's important is efficiency. We always say during objective exam testing, as little as possible, as much as necessary. So I only want to test the stuff that's absolutely necessary so I'm efficient, but also I don't risk flaring up the patient with doing a bunch of tests and measures that aren't necessary to begin with. And if I can eliminate a few things right off the bat from that functional testing, why not start there? And then lastly, it's way better buy-in. It's way better buy-in. So day one, you're always trying to have the patient walk out thinking, man, I'm finally in the right spot. This person totally gets my issue. And they're definitely going to be walking out saying that if you're first off testing the functional stuff, the stuff that they actually care about that you pick up in the subjective. No patient cares about hip IR, cares about lumbar AROM, cares about palpation. They don't care about that. They care about the thing that they want to get back to that they love. And if you're including that in the physical exam, the buy-in is going to skyrocket. So think about that over the next week or so. About maybe changing the order of your physical exam if this is not typically how you order things. When the irritability is low to moderate and you pick that up during the subjective exam, then when you go into the objective exam, you make sure that you're testing function first. It's gonna help with differential diagnosis, it's gonna help you be efficient, and you're gonna get way better buy-in. All right, so think about that this week. Happy Wednesday and welcome to today's segment of Jerry on Ice. My name is Alex Germano. I'm a member in the older adult division. This weekend I was talking with our students out in Madison, Wisconsin about cognition being a risk factor for falls. And I really wanted to dive into a more complete conversation about our role as rehab providers in screening for cognition. I guess I'll be specifically talking to the physical therapists, but also occupational therapists. And I know speech language pathologists actually do a really great job at cognition and cognitive screening. So I'm kind of talking to more of that PTOT group of humans. Today, we're gonna highlight why we encourage cognitive screening on older patients, and especially older patients without a known diagnosis of cognitive decline, and which cognitive screens will give you the best information. All right, so preface the conversation. This is going to be really talking about those who do not have a known diagnosis that could cause the symptom of dementia. If our patient has a known diagnosis of Alzheimer's disease, I don't really need to be doing a cognitive screen telling me that there is a problem. We know that they have a known diagnosis. It would be more beneficial for me to do something like a global deterioration scale to kind of categorize their level of dementia. and deterioration and then track that over time. So remember, if they have a known diagnosis already, probably no really need to screen. HOW AND WHEN TO PERFORM A COGNITIVE SCREEN So we're talking about people who are telling you that they are cognitively normal, that they do not have a diagnosis that affects their cognition. So why would we recommend this for these patients? Can't you just tell that they're cognitively normal and won't people be offended if I just start screening their cognition and they believe that they're normal? This is probably a number, these are probably a number of the questions that you're wondering right now. So let's make sure we reach out to all of those. ESTABLISH A BASELINE So first we are recommending screening or labeling our patients with a known cause of dementia because we need to establish a baseline. It's also very important that we are identifying problems on the earlier end of the spectrum. Let's think about another problem where we encouraged to do, we encourage like an early screen. That would be for frailty. It's very important to identify people as early as possible because it is so much easier to treat and change the trajectory of a patient experiencing pre-frailty than it is to change the trajectory of the person who's already in the depths of frailty. The same thing goes for cognition. It's much easier to change the trajectory of the human with mild cognitive impairment than it is once they are already in the depths of Alzheimer's disease and dementia. Pre-frailty, just like mild cognitive impairment, are really hard to see, right? It can be really hard to see. There can be a community dwelling older adult who is going to the senior center and seems really active, but they could be experiencing clinical levels of pre-frailty. And it's just, it's hard to see based on maybe the activities they're telling you that they do, maybe their physical function. They walked in without a walker, so they're probably, you know, not pre-frail. But this stuff is really hard to see, and it's why it warrants us screening for it. People do a really good job masking, especially those early signs of cognitive impairment. And from my clinical experience, when if you're not working in the home with them, sometimes the home is where you see this person a little bit more raw, and you see a little bit more of their habits, and you can kind of pick up on some issues. But if you're not with them for some of those critical moments, and they're coming to you in a clinic, you're probably thinking, I just work with super high level outpatient older adults. This is silly, they're not cognitively impaired. Well, let's share some statistics. So the current prevalence for older adults experiencing mild cognitive impairment from age just 60 to 64 is already about 7%. That's a pretty significant chunk. And that increases progressively with each age range up to the ages of 80 to 84, where those 80 to 84-year-olds, 25% of that population, mild cognitive impairment has a prevalence of 25% within that age group, 80 to 84, a one out of four. That's a huge chunk of the population. And actually there was a study done that one out of five older adults that use outpatient physical therapy services are estimated to have a cognitive impairment. So to use a phrase from the spine division, You may not think you're seeing patients with cognitive impairment, but they are seeing you. It's critical to establish this cognitive baseline for our patients like we do their physical abilities. We have no problem taking a grip strength. We're not weird about it. We just say, Hey, let's see your grip strength. We say, Hey, let's do that five times sit to stand. And even if we know they're going to blow it out of the water, we still do it, right? We're establishing a baseline. It gives us a complete and holistic view of their health status. If somebody has a cognitive impairment, they're going to be less likely to stick, follow through and progress with physical therapy or occupational therapy. And there's so many conditions within older adults that cause acute changes to cognition or acute on chronic changes to cognition, including a urinary tract infection or normal pressure hydrocephalus. So having the baseline assessment of cognition could be really powerful in making us feel confident to make the call to the physician, to make the call to one of the medical providers to say, hey, something's going on and something's wrong with this person. We are uniquely positioned as therapists and rehab providers because we have so many touch points with patients, way more than the PCP does. So we are providers who are going to be able to catch something very, you know, something sinister very early. And we can prevent these downstream hospitalizations from sepsis or injurious falls. These things occur as a result of having something that seems simple, a urinary tract infection that often presents as a cognitive change first. So, We can save lives by screening cognition and being aware of the cognitive changes that our patients are experiencing. Now, just like we continue to test our one rat maxes and stay informed on the status of our blood markers and other physical health markers, why are we not comfortable doing the same for cognition? Wouldn't it be great information to know that you're experiencing cognitive decline early? Or do you want to know about it when you're already in the worst stages of it? Wouldn't most people want a fighting chance? Well, there's a ton of stigma around cognitive health and cognitive decline. And that is a big barrier to doing some of these screens. But I'm going to share a quote with you from a participant from a town hall from the Alzheimer's Association. They do these big town halls and they get people's subjective experiences. And this person said, an early diagnosis is vital. It's made all the difference in the world in my life. It gives us a purpose and allows us time to get our house in order. On a recent poll, about 54% of Americans would rather know that they had Alzheimer's disease at the earliest stages of mild cognitive impairment. Simultaneously, many Americans are very worried about getting this diagnosis. They're scared because they're worried they're going to learn something serious is going on. They're going to worry that They're going to be over prescribed pills or given the wrong diagnosis. They worry that the symptoms might just go away if they give it time. So we need to work on combating this stigma, right? First, we need to do cognitive screening on all of our patients, right? The ones who don't have a known neurological diagnosis that causes dementia. NORMALIZING COGNITIVE SCREENING We have to do these cognitive screenings on everyone. We make it a very normal part of the process. It's as normal as asking the patient, have you had a fall in the last three to six months? We need to explain why we're doing the test. There's going to be nuance to this conversation, and I'm sure we have all worked with a patient or two where we felt a little bit uncomfortable maybe doing the cognitive screen, because we could feel some tension. Maybe the patient did have a mild cognitive impairment, and we felt like they would get maybe a little bit aggressive. I don't want to say combative, but maybe a bit in denial, a bit defensive if we brought in some of this cognitive screening. But I think when we try to lie about what we're doing, we're just going to do like this quick little thing. Ha ha. I don't think that that's giving the patient the, um, the true reasoning behind what we're doing. And I think sometimes that that trickery can elicit some of these defensive behaviors and postures. So what we need to do is be very clear and kind about what we're doing. We are going to be doing a cognitive screen. I do this on everyone because your cognitive health is as important as your physical health. A SCREENING IS NOT A CONFIRMED DIAGNOSIS Now, when we get a result that maybe isn't perfect, we need to bring a high level of sensitivity and empathy to this screening process, especially while relaying results. Remember, we're not in the position to diagnose somebody. This is a screening test that maybe can tell us something's going on, but that's all I know, all right? I can't make a formal diagnosis of mild cognitive impairment or dementia with the person sitting in front of me. That's not my job, and that's important that the patient knows that too. So we get the results of the screen. We are very clear about the results that we find. And we say, you know what, this means we just have to connect with our provider. And we're gonna get you connected with them to make sure nothing else is going on or there's nothing else that we can treat here. And we want this whole medical team to come together and rally around the person. We are in the perfect position to provide a ton of hope for this person. to provide a ton of hope. Hey, you're doing exactly what you need to do. You're here with me. We're gonna exercise. We're gonna work on some of this cognition together. We're gonna get you to a medical provider and we're gonna come around you and support you. We can't leave the patient hanging with bad results or good results even, right? We need to educate on what the next steps are. We need to educate on cognitive health and what to do to maximize it. We cannot just leave them there without any of the information and just slap a label on them and move on. CHOOSING THE RIGHT SCREEN Now, choosing the right screen is also very important. Interestingly, only 32% of PTs when asked about performing a cognitive screen as a part of a falls risk assessment, only 32% of PTs were performing that cognitive screen. Only 53% of PTs in the home health setting felt like they actually had the skill set to do cognitive testing. And in a study looking at occupational therapists, only a third of the occupational therapists were, I mean, sorry, a third of the occupational therapists were using only clinical observation or ADL assessment in isolation to report that there was a problem. So we kind of have an issue within our profession when it comes to what we are screening for and these screens that we're actually using. Um, We know that a number of people are only screening very specific aspects of cognition, including alertness and orientation, or choosing measures that are only sensitive for dementia, but not sensitive enough for mild cognitive impairment. Think like mini mental state exam. So we are really trying to specifically capture people in this conversation who are experiencing mild cognitive impairment. We need to choose measures that are more sensitive for that condition. These measurements include, just to name a few, The Mocha, okay? Mini Cog. Mini Cog is super easy, very free. You can get it from the Alzheimer's Association right now. The Short Bless Test, the Slums, the Brief Alzheimer's Screen. These are all sensitive enough for mild cognitive impairment. Many of them you'll see have like overlapping qualities. There's a clock on them. They're going to ask to remember some different words. You're going to have to like count backwards, name the months of the year. A lot of them will feel very similar. So just find ones that you think you can fit into your clinical practice and start looking through them and get familiar with some of them to use and pull out with your patients. SUMMARY Okay, let's wrap this thing up. So one, we are uniquely positioned to be screening providers for cognitive decline, especially those who might appear normal for a while, but then have an acute change to their cognition. We can catch those people and get them medical help much earlier than if they were to wait to see their PCP again, right? It is incredibly important for us to find out who is starting their journey on cognitive decline, who's experiencing that mild cognitive impairment, so that we can triage our approach and make sure to get the medical team involved, the family involved, make sure we start talking a lot about cognitive health with these patients and get the patient motivated to fight back against this decline. We also need to work on reducing the stigma of cognitive testing. Do not make it weird. I have no other piece of advice than that. Don't make it weird. Be very clear about your intentions on what you're doing. Report the results in a very meaningful way. Give hope when something comes out badly, right? We have to be choosing the right tests and measures when it comes up to cognitive screening. We've got to be picking measurements sensitive enough for mild cognitive impairment. On Episode 58, Pete is thrilled to speak with friend, former colleague, and comedy writer and actor with The Groundlings, Michael Aspinwall. Mike and Pete talk about Mike's formative years involving comedy, his inspirations, his days with The Groundlings, what he looks for in good comedy writing, his incredible run on WWE as Dr. Shelby, and some of his own writing and writing process. Michael Aspinwall was inspired to move to Los Angeles and become an actor after a very successful performance in a Paris Gibson Middle School play entitled Tied to the Tracks in his home town of Great Falls, Montana. Many of his mom's friends said he was good, and Michael took them seriously. He went on to high school where he really fell in love with theatre, which further solidified his desire to act, direct, and write professionally, while simultaneously solidifying his social position in the caste system that is high school. Michael attended UCLA's School of Theatre, Film, and Television, and while there, he studied performance primarily, in John Hall's Musical Theatre Workshop. He appeared in John's productions of Andrew Lippa's The Wild Party and West Side Story (not by Andrew Lippa). He was the 2004 recipient of the Carol Burnett Award for Outstanding Male Performer. In 2005, Michael earned his Master's degree from UCLA's TFT under the guidance of Dr. Pat Harter. He created a program that paralleled the California Arts Bridge program, using theatre and art to help younger students access the core curricula. After college, Michael began his career as an actor by waiting tables at the Bubba Gump Shrimp Company, where he performed Forrest Gump themed trivia nightly, and acted as though he cared about whether the temperature of the guests' fare was to their liking. He worked a slew of odd jobs, became a certified yoga teacher and a credentialed high school English teacher, and eventually found his home as an actor at The Groundlings' Theater in Hollywood. Michael found The Groundlings at a moment when the LA audition process was taking its toll. That place and the people in it breathed life back into a ten-year-old kid who used to stay up to watch SNL, then perform Mike Myers's bits in front of his fourth grade class. At The Groundlings, Michael met his sketch group, Big Boss Comedy, and a slew of other mind-blowingly talented people he gets to call his friends. Since his turn in the Groundling's Sunday Company, Michael continues to write and produce original work with his writing partner and fellow Sunday Company Alum, Patric Cagle. The book will serve as the catalogue for a presently unscheduled Cunningham retrospective at the Getty Center, the first major retrospective of Cunningham's work in over 35 years. Cunningham had a remarkable 75-year career that touched on seemingly every movement in American art and photography between the first decade of the 20th century and her death in 1976. She is particularly well-known for her address of pictorialism, her address of modernism, street photography, nudes and portraits. Aspinwall discussed her exhibition “Golden Prospects: California Gold Rush Daguerreotypes.” The show argues that the Gold Rush was the first “broadly significant event in American history” to be broadly documented in substantial depth by photography. It included rich images of San Francisco and of the Sierra foothills transformed by miners in pursuit of gold. It debuted in Kansas City last year, and was to have traveled to the Peabody Essex and Yale before the pandemic interceded. The fantastic exhibition catalogue was published by Yale University Press. For many women, there is a deeply rooted fear of aging and everything that comes with it - wrinkles, grey hair, weight gain. Instead of viewing the aging process as something natural, society's obsession with women's youth and beauty creates a mindset of scarcity that makes us acutely aware of our body's ticking time bomb, yet continually afraid to discuss it. Menopause is one such topic that we rarely discuss. The lack of knowledge around the natural evolution of our cycles means that many women are unprepared, both mentally and physically, for the physical change and the ensuing difficulty of adjusting to their body's new normal.
