The Department of Veterans Affairs is developing a plan to expand its network of healthcare facilities across the country and improve the infrastructure of some of its existing facilities. The VA expects to submit a new infrastructure plan to lawmakers in the first half of the new year, focused on expanding its footprint of facilities as it prepares for a historic expansion of veteran health care and benefits under the PACT Act.

This week we  welcome Dr. Brendon Burley PE, Jennifer Isenbeck PE and Meghan McNulty PE for a show we are calling ASHRAE 62.1 Is Acceptable, Acceptable?  The ANSI/ASHRAE Standard 62.1-2022, Ventilation and Acceptable Indoor Air Quality has been under the microscope with COVID and now RSV, Flu, etc. The standard has been the topic of many comments and critiques. This week we interview three committee members to get their perspective. Brendon Burley, PhD, PE, LEED AP has been in the design industry since 1998 providing freelance CAD services for a number of clients in the Philadelphia area before moving on to work on his degrees and research at The Pennsylvania State University. For the past 14 years he has been focused on designing health care and higher education projects, mostly in the mid-Atlantic region. His projects range from laboratory equipment installations to medical center expansions and replacements, including projects involving advanced imaging equipment, medical and research containment facilities, and clean rooms. In addition to his work experience, he has been a member of ASHRAE since 2011 where he has served as a member of Standard 62.1 Ventilation and Acceptable Indoor Air Quality, Standard 170 Ventilation for Health Care Facilities, and the Environmental Health Committee. He currently serves as the chair for Standard 62.1. Jennifer Isenbeck, PE, LEED AP is currently a staff engineer with Moffitt Cancer Center.  In the Planning, Design and Construction department, she works with various consultants on implementing best practices in healthcare design, campus infrastructure as well as providing design reviews, commissioning oversight and project implementation into operations.  Prior to this role, she was Director of Operations (Facilities) for various corporate and academic institutions.  Jennifer is currently chair of ASHRAE MTG.EBO (Effective Building Operations), past Chair of ANSI/ASHRAE Std 62.1 (2019-2022), DRC, Society Board of Directors (2013-2016).  She currently is a member of ASHRAE Standards and Publications Committees. Meghan K. McNulty, PE, LEED AP is a Senior Project Engineer at Servidyne, in Atlanta, GA. She works with building owners and operators to reduce energy and emissions while improving indoor air quality in existing buildings. She specializes in investigating and modeling energy use, assessing ventilation systems, and compliance with local building energy policies. Meghan is a Registered Professional Engineer (PE) in Georgia and LEED Accredited Professional in Operations and Maintenance (LEED AP O+M).  She joined Servidyne in 2013 with a Master of Engineering from Virginia Tech and a Bachelor of Science in Mechanical Engineering from Princeton University. Meghan volunteers with ASHRAE on the Government Affairs Committee; Standard Project Committee 62.1, Ventilation and Acceptable Indoor Air Quality; and Technical Committee 4.3, Ventilation Requirements and Infiltration, as Chair. In 2019, she received the ASHRAE Government Affairs Award for outstanding effort and achievement in state, provincial, and local government activities in connection with technical issues related to society. Sterilization processes are always important in dental practices to protect your staff and your customers. During the Coronavirus pandemic, everyone is especially concerned about proper procedure. You have to follow Instructions for Use (IFUs) and properly sanitize and sterilize your instruments. In this episode of Talking with the Toothcop, Stephen Kovach—whom I am now referring to as the “Godfather of Infection Control”—joins Andrea and myself to chat about the importance of sterilization processes. Stephen Kovach is Educator Emeritus at Healthmark Industries and has been in the medical field since 1975—now celebrating his 45th year in Healthcare. He is a member of IAHCSMM orthopedic council for loaner instrumentation and a voting member on various AAMI committees. He’s published numerous articles varying in subject matter from perfusion to the importance of cleaning surgical instruments. When I recently heard Stephen speak, he pointed out that “When you sterilize dirty instruments the end result issterile dirt”. Every dental office NEEDS to have a quality management system in place—and most don’t.  Stephen points out that dentists need to be referencing the AAMI ST79—The “Comprehensive Guide to Steam Sterilization and Sterility Assurance in Health Care Facilities”. But most aren’t. Why? Stephen and I agree it’s because the high price ($400) is a barrier to entry. While their information is proprietary, it would be wise to have this information widely disseminated for the safety of our patients.  What you may not know about Instructions for Use (IFUs) I’ve talked about IFUs before and have emphasized the importance of acquiring and referencing them for proper instrument sterilization. They outline the exact cleaning and sterilization process step-by-step to ensure proper cleaning of the tools. What you may not know is that device manufacturers have tested this repeatedly, and to get their products to market the FDA HAS to approve the IFUs they’ve submitted. They don’t just slap instructions together willy-nilly. It is a well-researched process that you need to follow. I’ve been surprised how many dental professionals have no idea what I’m talking about when I ask them if they’re using their IFUs. Turns out, IFUs can be hard to obtain. I’ve even asked a device rep for an IFU and was told point-blank it was only provided to clients. Some can be found online—but not all are readily available.  Stephen and I agree you must seek out and obtain IFUs for any tool or instrument you use. Follow at the very least the minimum requirements for sterilization and go above and beyond when possible. Keep listening for our discussion on cleaning hinged instruments.  Document your procedures to protect your practice Stephen has been a part of many depositions where professionals are asked how they documented their process and how they followed their IFUs. If they stray from the directions on the IFUs they must state why they think their cleaning procedure was superior to that of the manufacturer.  If you stray from the proper sterilization procedures and a patient's illness or even death is traced back to you—you’re in serious trouble. You must be able to prove in a court of law that you followed the instructions properly and show documentation.  Some professionals claim they were told something different by the manufacturer—but have no concrete proof. Stephen recommends that if you contact the device manufacturer and they verbally tell you a process that contradicts the IFU—get it in writing. Ask them to outline the company letterhead and mail it to you for records. But if all else fails, following the IFU is the ultimate protection. Why following instructions are SO important during the Coronavirus Pandemic Stephen was quick to point out that he sees some of the best pre-infection control in dental offices. His personal dentist washes his hands in front of him, uses hand-sanitizer, puts on a mask and safety shield, and protective eyewear. Continue to practice these basic means of infection control—but do it correctly.  Stephen has noticed that most people don’t even know how to properly use Clorox wipes. The contact time of the solution on the surface is of the utmost importance. Most people quickly wipe down surfaces with a wipe and call it good. But if you read the instructions (which vary) it states that the surface must remain visibly wet for up to 4 minutes to properly sanitize. Read the instructions people! Contact time is also important for hand-washing. The longer you wash your hands (experts say you can sing the “Happy Birthday” song in your head) the more effective you are removing bacteria, viruses, etc.  Proper sterilization techniques are more important than ever during the pandemic—not because it was less important before—but because it brings you and your patients peace of mind in a time of chaos. Prior to her coming to CDC, she served as an infectious disease epidemiologist for 15 years at the Texas Department of Health (currently known as the Texas Department of State Health Services). Her current area of expertise focuses on environmental infection control, microbial inactivation, and transmission of infectious diseases. She has advised CDC and other federal and state health agencies, healthcare professionals, and the public on issues concerning environmental cleaning, sterilization and disinfection, healthcare laundry issues, prion disease epidemiology, and environmental management of emerging diseases. She is the coordinator of and contributor to the CDC/HICPAC “Guidelines for Environmental Infection Control in Health-Care Facilities” that was released in 2003, and has assisted federal colleagues and other entities in the development of their infection prevention guidelines. She received her PhD and MS in Microbiology from Rutgers, the State University of New Jersey, completed a postdoctoral appointment in viral immunology at Baylor College of Medicine, Houston TX, and has her certificate as a Microbiologist with the American Society of Clinical Pathologists (M[ASCP]). She also serves as an Editorial Board member for the journals Infection Control and Hospital Epidemiology and Applied and Environmental Microbiology.

Prior to leading Nazareth Home, Haynes held various positions as a psychiatric nurse and administrator, including a stint at Our Lady of Peace. As a nurse in the facility, she specialized in mental health and often dealt with Alzheimer’s patients. She was approached to be a part of a group that was interested in developing Alzheimer’s centers. There she learned about aging care and its challenges. Mary holds a Master of Science in health management and systems science from the University of Louisville, and a M.S.N., Gerontological Nursing from the University of Evansville. Her hobbies include golf, cooking, the arts, and theatre. Originally from Marrowbone, Ky., Haynes resides in Louisville with her husband and three daughters.

This week on IAQ Radio we welcome Andrew Streifel. Andrew J. Streifel is a Hospital Environment Specialist with the Department of Environmental Health & Safety at the University of Minnesota. Whenever there is a high profile hospital acquired infection one of the first names that comes to mind is Andy Streifel. Mr. Streifel has been involved in the investigation of over 80 clusters of infection related to hospital air quality. He has served as a consultant in over 400 hospitals worldwide on a variety of indoor air quality issues, water microbial contamination and has investigated clusters of bacterial infections due to unsanitary clinical practice. His current research interests involve energy management impact on infection prevention and validation of the air and water safety in healthcare facilities. Andy is a registered Environmental Health Specialist in Minnesota. He currently serves as an invited nonvoting member on the Maintenance Committee for the SSPC 170: ASHRAE Standard for Ventilation of Health Care Facilities 2008 - present. Mr. Streifel is also serving on the revision committee for the 2014 edition of the FGI Guidelines for Design and Construction of Health Care Facilities. He has served on this committee since 1994 and has participated in the development of the 1996, 2001, and 2006, 2010, 2014 & 2018 editions. In addition Mr. Streifel has served on the Special Project committee that authored the HVAC Design Manual for Hospitals and Clinics. He was an author on ASHRAE published articles on Ventilation for the Protection of Immune Compromised Patients-1988, Construction Impact on Indoor Air Quality-1995 and Air Leakage Analysis of Special Ventilation Hospital Rooms-2008. Mr. Streifel has assisted University of Minnesota Institute of Technology & School of Public Health as technical advisor on filtration, patient room airflow studies and particle management projects. He also serves on the National Air Duct Cleaners Association Standards Committee. In addition, he is an author in over 50 articles, published in the academic and professional journals, related to ventilation and water quality in health care.

We are talking about the L word today; Legionella. We are going to get to the bottom of what Legionella is, what we need to know about it, how we explain it to our customers, and how we get everybody working together towards a common goal. Stout, she is the president and director of Special Pathogens Laboratory, and research associate professor at the University of Pittsburgh Swanson School of Engineering in the Department of Civil and Environmental Engineering. A clinical and environmental microbiologist, Dr. Stout is recognized worldwide for more than 30 years of pioneering research in Legionella. The CDC just released “version 2” of their Toolkit & VitalSigns publications that were first released in June of 2016 – the message and advocacy is still very strong and clear that building owners have an ASHRAE 188 compliant “Water Management Plan” (or better) – w/this year’s focus and data particularly on Healthcare Facilities! The VitalSigns document title is: Legionnaires Disease – A Problem for Health Care Facilities!  Here is a copy of CDC's VitalSigns, MMWR and a Link to the CDC Toolkit. AWT quickly responded to the CDC releases w/their own press release of the CDC – note that the CDC (now) specifically recommends contacting a “water treatment professional” for water management plan development – and not a “plumber”.   ASHE also responded w/acknowledgement of the CDC focus on Legionella management in healthcare facilities – see ASHE News. Maybe the most significant event is the CMS update memorandum (effective immediately 6/2/17) that has very specific expectations/requirements for health care facilities receiving CMS funds to have an 188 equivalent water management plan AND “environmental testing for pathogens” (Legionella).
