In this week's episode of "Pushing the Limits I had the privilege of interview world renown cancer researcher Professor Thomas Seyfried the author of over 150 peer reviewed publications and the author of the book "Cancer as a Metabolic Disease" For 30 years Professor Seyfried has been in the trenches as a geneticist and biochemist studying cancer cells and working out the intricacies of how to stop them proliferating. Episode Highlights [03:17] Travis' Background Travis has an undergraduate degree in biochemistry. He eventually took over the family business and got married. He then finished his master's degree, focusing on cancer metabolism for his independent study. The prevailing teaching during that time was that somatic mutations cause cancer. Travis was inspired by the book, Cancer as a Metabolic Disease, to do a thesis on metabolic theory. Otto Warburg proposed the metabolic theory many years ago, but the medical community dismissed it as simplistic. New evidence reignited interest in the approach. [08:10] The State of Healthcare Public and health institutions have long accepted the somatic approach to cancer, with billions of dollars backing it. The United States spends the most per capita on health care expenditure among developed countries yet has the lowest life expectancy. The issue with the healthcare system lies in variations of treatment, over-treatment, and drug development process. The healthcare industry is focused more on treating diseases rather than preventing them. Although technology has advanced to assist people in tracking their health, people's health is deteriorating. Even if people live longer, they often experience illness and disability in their final years. [12:48] Changing the Healthcare Industry The healthcare incentive structure must change. Doctors get paid for every drug and procedure they prescribe, leading to overtreatment. There are emerging practices where doctors get a big bonus per patient who doesn't need procedures, encouraging them to keep patients in good health. When blood sugar levels rise, doctors usually diagnose diabetes. However, we can test for fasting insulin regularly to intervene. This test can also help people prevent other diseases and become more health-conscious. Change takes time with systems and industries, but we can take simple lifestyle interventions now. [16:01] Why People Are Getting Sicker Maintaining our health can be challenging. We've been conditioned that taking a magic pill is preferable to putting in the effort. Remember that metabolic dysregulation causes aging, not the other way around. In the last 10,000 years, humanity has drastically shifted to consuming processed foods, carbohydrates, and oils, resulting in noticeable consequences. People have also become sedentary. A hunter-gatherer society has no obesity, insulin resistance, or chronic systemic inflammation. However, we frequently see this in the Western world. [20:32] The Root of Diseases Understanding cellular health and inflammatory processes are critical in keeping yourself healthy. Periods of food scarcity have happened throughout humanity's evolution. Our bodies enter ketosis due to the caloric deficit but most never reach this state. Studies have shown that ketosis can help prevent diseases ranging from Alzheimer's to Parkinson's to cancer and diabetes. Constantly eating keeps our immune system active and prevents it from resting, which has implications for chronic inflammatory diseases. [24:53] What Happens During Ketosis? Ketosis happens in a fasted state, and a ketogenic diet is nutritional maintenance. When we fast, our bodies will mobilise the fat we have in our bodies and begin a process of beta-oxidation to burn off fat. A caloric deficit makes this process faster, converting fats into ketone bodies, which then take the place of glucose. Ketones have more thermodynamic energy and ATP per carbon unit compared to glucose. Ketosis also boosts antioxidant levels. Since diseases are oxidative, ketosis can help to combat illnesses. [32:01] Growing Healthcare Trends Research has shown that fasting before chemotherapy can reduce some radiation side effects Sugar is fuel for cancer cells, but doctors continue giving and allowing patients to consume it. There's more awareness of the latest research. The annual Metabolic Therapeutic Conference has grown to around 1600 attendees. When we approach doctors, we will receive information their institution has taught and allowed them to give us. Even with COVID-19, people have yet to explore treatments like monoclonal antibodies, hyperbaric, and Vitamin C. [44:00] The Abscopal Effect on Cancer The abscopal effect is when you target a single tumour site, but it affects other tumour sites in the body. Travis shares a case where someone with breast cancer on a checkpoint inhibitor received a much lower radiation dose. In that case, radiation was sufficient to systematically stimulate the immune system to attack cancer, improving the patient's chances of survival. [47:41] Advancements in Immunotherapy Using peptides to manipulate the immune system is still a trial and error process. The immune system's job is to survey for cancer, get rid of it early, and create a checkpoint to avoid attacking itself. [49:44] Emerging Cancer Treatment Research In detection, metastasis is an early stage event rather than a late-stage one. Cancer cells break off, dying when it reaches the bloodstream. This selection pressure creates different genetic mutations in tumours. The somatic mutation theory argues that a series of sequential mutations cause cancer, with each tumour site having a unique set of driving mutations. The epigenetic metabolic theory posits that these mutations are a side effect of the disease. All cancers burn sugar and evade the immune system, but mutations vary too much. Treatments should focus on epigenetic expression rather than mutations. [55:16] Researching Treatment Options The medical field is filled with uncertainty. Cancer treatment options have not been compared. After an in-depth look at clinical trials, we find that what seems to be the best treatment paradigm isn't always the right option. When conducting research, consider the data and evaluate it using a risk-reward framework. You can choose from low-risk alternatives. [59:59] The Soil Approach to Cancer Cancer grows in the presence of a favourable microenvironment and molecular events. It takes a long time for a chronic irritant to develop cancer cells. Cancer cells take on characteristics of our earliest genes, including replication and the appearance of early embryos. You can help your cells "behave" by providing oxygen and avoiding chronic irritants. Nurture the microenvironment through exercising and eating the right foods. Instead of focusing on the seed or cancer, we should concentrate on the soil or microenvironment. [1:04:05] Health and Longevity While we don't know humankind's natural and optimal states, ketosis and fasting provide a glimpse. Cells stop metabolising as we age, causing inflammation. We may be able to extend life spans if we can reduce senescent cells. Travis shares a study in which mice received fisetin, found in strawberries, reducing senescent cells and prolonging their life. There's a lot of new research on substances that kill senescent cells, like senolytics, quercetin, and other compounds. In the world of cancer research, Seyfried's is revolutionary, but it's not getting the attention it deserves. The underlying message is clear, backed by evidence, and surprisingly simple: all cancers—whether brain, breast, colon, or any others—are unable to grow without glucose and glutamine, which means any therapy that disrupts the availability of these molecules to cancer cells will result in the death of those cancer cells. Why are glucose and glutamine necessary components of cancer cell growth? It's because cancers cannot use oxygen to generate energy, which means they cannot use the oxidative phosphorylation pathway used by other cells in the body. Instead, they rely on an ancient metabolic pathway for energy production: fermentation. And what drives fermentation? That's right: glucose and glutamine. Without these molecules, fermentation cannot be carried out, leaving cancer cells without the ability to generate energy, which causes cancer cell death. Taking this to be true, every known major cancer should be potentially manageable at least, and entirely resolvable at most, so long as glucose and glutamine can be targeted and restricted. This is why Siegfried's efforts have been focused on developing a non-toxic diet and drug cocktail that does exactly this. He is skipping the clinical trials because the diet and drug cocktail strategy is not consistent with the double-blind crossover study design used by pharmaceutical companies. Furthermore, no pharmaceutical company is going to jump on the opportunity to fund a study that tests the efficacy of diet and non-patentable drugs, because there is simply no financial incentive to do so. Instead, Seyfried treats individual patients with these diet and drug therapies and publishes his results in the existing scientific literature. He is currently working on a paper on longitudinal studies showing the impact of diet and drug therapies on tumors, backed by MRI and PET scan images. According to the American Cancer Society, over 1,600 people in the United States die from cancer every day, many of whom were receiving the current standard of care, which is radiation, chemotherapy, or a combination of both. Thomas Seyfried is a professor in the biology department at Boston College and strong proponent of using metabolic therapy as a form of treatment for every type of cancer, arguing that it's more effective and less toxic than radiation and chemotherapy. So, what exactly does metabolic therapy entail? It's based on a simple concept: growth requires fuel, so in order to stop the growth of cancer cells, their sources of fuel--namely glucose and glutamine--must be reduced or eliminated. Reducing the availability of glucose and glutamine results in an increase in ketones, which can be effectively used by the body's normal cells but not by cancer cells. Seyfried explains how the primary difference between cancer cells and normal cells makes this an effective form of treatment for cancer, discusses how to achieve a state of therapeutic ketosis, and describes what to do once a state of therapeutic ketosis has been achieved. He also explains what's standing in the way of widespread implementation of metabolic therapy in modern medicine, and the main problem with the gene theory of cancer and personalized targeted therapy for cancer. “The whole paradigm has to change, the whole concept of what people view cancer as has to change before we understand how much more effectively we can build therapies that don't harm anybody,” says Seyfried. How genetic mutations and negative epigenetic changes in gene expression like oncogenes turning on and tumor suppressor genes turning off are not the main drivers of cancer but really the byproducts of damaged cellular respiration along with defective mitochondria, and the many experiments and studies Professor Seyfried has gathered that have been done over the years to prove this. From an implementation perspective Dr. Seyfried got into how lowering blood glucose and raising ketones along with other metabolic therapies will kill many cancer cells outright and leave the rest much more vulnerable to other therapies and how one would get started on this approach through fasting and the ketogenic diet. Psychologischer Aspekt wirkt wie ein Reset des Körpers. Gerade nach Phasen wo es eher weniger optimal gelaufen ist, kann ein Fastentag sehr “reinigend” wirken. So zu sagen als klarer Schnitt - ab jetzt ehts wieder ander weiter.. Die Müllabfuhr wird aktiviert und das Recycle-Program gestartet Fasten aktiviert das recycle-Program in der Zelle. Strukturen, die einfach nicht mehr so gut funktionieren, werden abgebaut. Gehirngesundheit Es wurde wissenschaftlich nachgewiesen, dass Fasten die Produktion von sogenannten Brain-derived neurotrophic factor (BDNF) erhöht, ein Protein im Gehirn, welches instrumental ist für die Neurogenese (Entwicklung von neuen Neuronen) und die Schaffung von Synapsen (Verbindungen zwischen Nervenzellen). BDNF ist also eine Substanz, die das Wachstum von Nervenzellen im Gehirn erhöht, aber es ist viel mehr als das. BDNF ist neuroprotektiv gegen Stress und Toxine des Gehirns und ist auch an der Insulin-Empfindlichkeit beteiligt. Menschen mit einem höheren Niveau von BDNF haben geringere Depressionen. Wird BDNF depressiven Menschen gegeben reduziert dies deren Depression. Ein erhöhtes BDNF verbessert zusätzlich die kognitiven Fähigkeiten. mTOR Was ist mTOR? Ist eine Abkürzung und steht mammalian target of rapamycin. mTOR ist an der regulation von Zellwachstum und Zellteilung beteiligt. Eine verringerte mTOR Aktivität ist mit Langlebigkeit. Was gibt es für unterschiedliche Formen und Protokolle? Langzeitfasten - mehrere Tage Intermittierendes Fasten - mehrere Stunden (12 - 18h) Intemittierendes Fasten Kein Frühstück, Fasten, Ruhe für Verdauung IF oder intermittierendes Fasten ist eine gute Strategie; während längeres Fasten negativ sein kann weil Muskelmasseverlust, stressig, nicht leicht umzusetzen, IF = leicht machbar. Angefetteter Kaffee, Butterkaffee oder auch Dave Asprey’s Bulletproof Coffee mit MCT Was ist Intermittierendes Fasten? Intermittierendes Fasten (IF) gepaart mit der Paleo Ernährung könnte die beste Möglichkeit darstellen abzunehmen und zugleich auch Ihre Gehirnleistung zu optimieren. Gesunde Lebensmittel während eines gewissen Zeitfensters essen soviel Sie wollen. Das Zeitfenster zum essen beträgt beispielsweise 8 Stunden in Kombination mit einer Fastenphase von beispielsweise 16 Stunden. Dies sollte für die meisten Menschen möglich sein und ist einfach umzusetzen. Somit haben Sie eine mit großer Wahrscheinlichkeit reduzierte Kalorienaufnahme mit gesunden Lebensmitteln, da Sie nur 8 Stunden zum essen zur Verfügung haben und 16 Stunden Fastenzeit um in einen ketogenen Zustand zu gelangen und Ketonkörper als Energiequelle zu verwenden. Somit haben Sie einen flexiblen Stoffwechsel und eine optimale Möglichkeit Ihre Gehirnleistung zu verbessern und nachhaltig abzunehmen. Es gibt verschiedene Methoden: 36/12 Variante * Im Zyklus von 48 Stunden soll 12 Stunden gefastet werden. Jeder Mensch hat unterschiedliche Gewohnheiten. * Wer das Frühstück gerne ausfallen lässt, für den ist der Weg zu einer 12 stündigen Fastenphase auch nicht mehr weit. Der Körper gewöhnt sich ebenfalls sehr schnell an dieses Modell. Eat-Stop-Eat * Bei Eat-Stop-Eat wird pro Woche eine Fastenphase von ca. 2 Tagen eingelegt in der nichts gegessen wird. * Eine Phase könnte von 12 Uhr mittags bis zum nächsten Mittagessen erfolgen und ermöglicht es jeden Tag zu essen und die Fastenphase trotzdem durchzuführen. 19/5 oder Warrior Diät * Bei beiden Varianten wird ein einzelner Tag in eine Fastenphase und in eine Essensphase unterteilt. Es wird beispielsweise von 0 Uhr bis 19 Uhr gefastet und dann wieder für 5 Stunden gegessen. Lean Gains – 16/8 Diese Methode verwende ich und deshalb werde ich diese im Detail vorstellen. Sie besteht darin, täglich eine 16 Stunden Fastenphase einzuhalten und einer großzügigen Essensphase während der weiteren 8 Stunden. Dadurch entfällt es, Mahlzeiten vorzukochen, oder verzweifelt herauszufinden, welches Essen aus der Kantine geeignet ist. Wenn Sie gerne sehr große Portionen essen, ist diese Methode optimal geeignet. Ich habe es mit dem Ziel der Erreichung schnellen Gewichtsverlust, besserer Gesundheit und Fitness begonnen. Aber etwas noch verrückteres ist passiert. Intermittierende Fasten verbessert die Gehirnleistung. Wie startet man am Besten? Häufige Fehler Leichter wenn man schon mal in Ketose ist. Cancer is like a computer program gone wrong, the kernel panic of the human body. Unfortunately, we have no reboot button, and so cancer is the most serious of diseases. This year, almost six hundred thousand Americans will die from cancer. Despite the fact that cancer receives more NIH funding than any other illness, the death rates remain the same as in the 1950s. Tripping Over the Truth: The Return of the Metabolic Theory of Cancer tells the story of molecular biologist and science writer Travis Christofferson’s journey to discover why cancer has remained so elusive. The central theme of the book is that we’ve fundamentally mischaracterized the origin of cancer. Most scientists hold to the belief that cancer is a genetic disease, Travis explores the idea that cancer is, in fact, a disease of damaged metabolism. The metabolic theory of cancer stems from Nobel laureate Otto Warburg’s observation in 1924 that cancer cells lack the ability to produce energy using anything other than an ancient and inefficient process of fermentation. Warburg’s idea failed to gain critical mass, and by the 1960s had largely faded into oblivion. In 2012, Thomas Siegfried, Ph.D., released his book Cancer as a Metabolic Disease where he expanded on Warburg’s original hypothesis. In his book, Siegfried described how all cancer cells have damaged mitochondria, the organelle responsible for creating energy with oxygen. Inspired by The Emperor of All Maladies by Siddhartha Mukherjee, Travis has done a excellent job of telling the rich story behind the metabolic theory of cancer. In this interview, Travis and I discuss some of the potential benefits and mechanisms of a ketogenic diet, hyperbaric oxygen, gluconeogenesis inhibitors and ketone supplements as possible cancer therapy.
