I received this question about Protein Supplements for muscle building and want to share the question and the response with you. Lucy's Question: I am learning so much. I have a question about protein supplements. I have heard collagen, protein powders and creatine mentioned. I take a glutamine supplement for my gut. Any comments on how that might be related to muscle building if at all? GREAT QUESTION! Protein Supplements for Muscle Building If you listened to Dr Vincent Pedre... Day 1... if your gut is not absorbing micronutrients and able to break down protein in foods.. you're unable to build muscle NO MATTER WHAT YOU CONSUME. L-Glutamine for Gut Health The L-glutamine helps with healing the gut (IF you've identified and REMOVED the foods that may have contributed to a leaky gut situation). So first, the removal, the testing, the L-glutamine then supports the sealing of the gut (that can't happen if the culprits are still present). THEN... dietary protein or supplemented with protein powder with high quality EAAs or a EAA supplement support muscle building and repair. Collagen for Muscle Collagen supports hair, gut, skin and bone resilience (but is not alone adequate enough for muscle). So, if you count collagen that you've added to coffee as your daily protein for instance, and your highest priority is muscle building, that's a little deceptive. It hasn't actually helped. Though it may help you with blood sugar stabilization, it isn't muscle-specific. You heard Erin Carson credit her collagen habit for rapid repair and recovery of a fluke ankle break. No stranger to supplements in 40 years of strength & conditioning, it was collagen that she mentioned. Creatine for Muscle Building Then creatine - you didn't mention, but I'm adding here because it's a bit like "the next layer" for those that aren't making adequate progress, may be something you want to consider to help. Isn't it funny how we are quick to diagnose and correct food allergies in our pets but many people don't know what their own food allergies, intolerances, and sensitivities are. What if we could run more optimally if we addressed this issue for our health? We discuss food allergies in the US. The gold standard to figure out if you have a food sensitivity is still the elimination diet. With blood tests, it's not uncommon to get false positives. We discuss some of the common symptoms could be allergies and intolerances. We talk about what a food intolerance is and how to address it. Typically this is going to involve leaky gut. Since the advent and spread of the SARS-CoV-2 acute respiratory syndrome in 2019, classified as Coronary Virus Disease-19 (Covid-19) in February 2020, then transcending to pandemic status in Match 2020 by the World Health Organization, scientists have been searching for efficacious medical treatments and environmental guidelines to halt the spread, treat the symptoms, and reduce the risk to contraction. As of this writing, there is no cure. However, with three vaccines in the inoculation phase, it appears that the spread and death rate worldwide is going down, as more people receive the vaccinees – pending the Covid-19 disease variant impact. With each day that passes we learn more about the importance of our gut health in relation to our overall health and well being. Our GI system is directly correlated to many systems in our body. When this is off it throws everything off leading to hormone disturbances, anxiety, depression, brain fog, skin conditions, food allergies and obviously lots of GI upset. Our gut has influence on nearly every system in our body to the list of symptoms goes on and on. In Functional medicine we use the 5 R system to heal and repair the gut. Remove, Replace , Reinoculate, Repair and Rebalance. Remove :Includes removing foods and medications that may be irritating to the GI tract. Chris is 33 years old, originally from New Jersey and right now lives in Savannah, GA. His career began in finance, as he quit drinking, he transitioned to being a personal trainer. He also began a blog, writing the articles he wished he could have read when questioning his own drinking. This became his website and he now is a alcohol recovery coach and has a podcast related to sobriety. He likes to be physically active. Chris also has two dogs he rescued in 2020. [16:57] Can you give listeners some background on your story? Chris started drinking in high school but wasn’t really a partier. He was mostly into swimming and studying. However, when he did go out, he realized he could out drink all his friends. As he moved into an unhealthy relationship with alcohol, he felt like he was living a double life. Alcohol was the opposite of everything he stood for. Looking back he can see that he would have withdrawal symptoms at the age of 20 when he didn’t drink. When he left college and began working, the drinking was now just martinis and more expensive. When he did finally quit, he had to go to detox. Through his research he learned that he had been out of balance with his nutrients which alcohol only exacerbated. [25:21] How aware were you that alcohol was the problem? Chris said he was in deep denial, with outbursts of honesty. A story he tells is standing outside a liquor store one morning waiting for it to open while drinking from a bottle of water he had filled with vodka. He thought to himself “this isn’t normal.” Chris felt he was special because of that he needed to drink to deal with people and jobs. Alcohol to him was a performance enhancing drug. [30:47] Tell me about those first couple months. Chris said once he admitted to others that alcohol was a problem, he felt some inner peace and relief. He also felt the tug of war in his brain, would this be purgatory and him having to be a saint the rest of his life? Chris took the leap of faith that he would figure out what needed to be done. In rehab he began doing meditation and that opened his eyes to the fact that you can have a fulfilled life away from alcohol. Fueling his body helped him see the world in full color. [39:35] What are simple tips help you restore your body balance? Chris said there are two main factors: Hypoglycemia (low blood sugar) and deficiencies in neurotransmitters. Start with a daily multivitamin and look into amino acid therapy. L Glutamine can be helpful in repairing muscles and gut health. It also turned into glucose in the brain without a spike in blood sugar. * always speak to your doctor before beginning any regiment. I would like to report on the new product GI Meal Select from Moss Nutrition. The Case: Jane has been experiencing dry, irritated eyes for the past 2 years She’s treated them with steroid drops and antihistamines but the problem persists After consulting many doctors and taking tests, there seem to be no answers. Eye irritations can range from dry, itchy eyes to puffiness. Most people reach for over-the-counter eye drops or antihistamines for temporary relief. If it’s caused by the environment or allergies, this might solve the issue but when it persists - there may be a bigger issue at play. The Investigation My sense was that another organ was driving her eye issues. I suspected that it actually wasn’t an eye issue, so to solve this health mystery, we had to look outside the box. And, I knew just who to contact, Dr. Rudrani Banik (Dr. Rani). She’s a board-certified and fellowship-trained Neuro-Ophthalmologist with a functional medicine approach. You may remember her, she was on episode 50. Dr. Rani confirmed that eye issues aren’t always just about the eyes. The Eye Thyroid Connection The thyroid is related to the health of your eyes. The receptors on the thyroid look very similar to the receptors that are found on the connective tissue in the eye socket. When there are issues with the thyroid, specifically autoimmune issues, there is the possibility that the eyes can be affected. There are a lot of different eye issues that are related to either Graves Disease or Hashimoto’s (autoimmune issues related to the thyroid). However, sometimes the eye issues present before the autoimmune issue is detected or diagnosed making the connection difficult to make. Symptoms of Thyroid Eye Disease The symptoms of thyroid eye issues can start out like many eye issues. For example, you might have dryness, irritation, redness, puffiness or swollen lids. The usual course of action for these symptoms might be to use drops (including over the counter and steroid gel drops), antihistamines or both but if these don’t work, these may be symptoms of thyroid eye disease. Additional characteristics of thyroid eye disease may include the swelling or puffiness of the upper eyelids especially in the morning with the swelling decreasing through the day. Other symptoms may include chronic redness or a shift in the width of the eye socket. The latter may result in the eyes appearing bigger or wider. There may even be some inflammation causing the eyes to protrude or bulge (proptosis). If left undiagnosed and untreated, the muscles behind the eye socket can get involved and cause double vision and even vision loss. Note that this does not happen overnight - it’s a severe advancement of the condition but it’s worth noting what can happen if the situation is not dealt with. Diagnosing Thyroid Eye Disease The most important part of tying an eye issue to the thyroid is to get blood tests. But, it’s not enough to just test the TSH or T3 as many doctors will do as a thyroid test. Dr. Rani says she likes to get the full antibody panel including TPO (thyroid peroxidase), Tg (thyroglobulin antibodies), and TSH (thyroid-stimulating hormone) receptor (which tends to be elevated in thyroid eye disease patients). Dr. Rani also says an eye exam is very important. She measures the lids very carefully to see if they are more open. She also tracks this over time to watch for changes. She’s also looking for congestion in the eye socket, asks about double vision, and checks the ocular motility. Finally she uses an exophthalmometer to determine how the eye rests within the socket. Sometimes a CAT scan or MRI are required. Treatment of Thyroid Eye Disease Once the issue has been properly diagnosed, there are several things that can be done to help treat thyroid eye disease including: Eye Lubrication Treating eye redness, dryness, and irritation can be done with topical medications (typically drops or ointments). Sometimes patients require medications (antihistamines or topical steroids but Dr. Rani only uses steroids in extreme cases (when vision is at risk) because there is a risk of side effects like developing glaucoma or herpetic infections. Diet Changes Certain foods may be triggering thyroid issues (for example, gluten, dairy, and goitrogens like cruciferous vegetables). Avoiding these foods may help. Avoid Smoke Dr. Rani says it's really really important to avoid any kind of smoke or fumes. Studies show that people who smoke have a much worse prognosis when it comes to thyroid eye disease, so it’s suggested that they quit smoking and even avoid secondhand smoke. Cooking fumes can even create a problem. Reduce Stress When it comes to managing the thyroid, stress is a major factor. Stressful periods can trigger thyroid issues so ongoing stress-management is important. Selenium A European study found that selenium (a mineral) can reduce the symptoms of thyroid eye disease. In that study, they used a supplement (100 mcg twice per day) but selenium can also be found in Brazil nuts, eggs, beef, chicken, and pork. Selenium can also be helpful for the conversion of T4 to T3. Zinc Zinc is really important for healthy thyroid function and producing hormones. You can take a zinc supplement, eat foods rich in zinc or do zinc lozenges. Treatment Cautions There are a lot of eye drops for reducing dryness, redness and irritation but Dr. Rani encourages people to read the label. Look specifically for polyvinyl alcohol. It is a lubricant but the pH is not balanced to the eye so it can be very toxic to the surface of the eye and make dry eyes worse. Some brands that Dr. Rani says are free of alcohol are Refresh, Systane, and Genteel. Who to See - Optometrist vs. Opthamologist Many people aren’t sure about the difference between an optometrist and an ophthalmologist - and who to see if they think they may have thyroid eye disease. Both are doctors but the optometrist is a doctor of optometry (OD) and must go to college for four years and then optometry school for four years. Ophthalmologists are MDs (medical doctors) with four years in school, four years in medical school and then an additional internship and residence (another four years). It’s training in both medical and surgical aspects of the eye. For thyroid eye disease, Dr. Rani says you would want to see an ophthalmologist because you may need to get special treatment which may include surgery. Mystery Solved Jane didn’t specifically present with many classic thyroid symptoms, but knowing the connection, I knew I needed to evaluate this further. Comprehensive lab tests revealed that Jane’s TSH, T4 and T3 were completely normal however her thyroid peroxidase antibodies (associated with Hashimoto’s) were 325. Ideally they should be below 35. We didn’t know how long her antibodies had been elevated but the good news was that (unlike many others with Hashimoto’s) her actual thyroid function had not been affected. The eye issues were the first symptom. .With the mystery solved, we knew we needed to address the immune system which was confused and attacking the thyroid and the eyes due to molecular mimicry. Next Steps If you heard episode 32, then you may recall that there are 4 main immune triggers - foods, toxins, infections and stress. For Jane, I needed to find the sources of the immune confusion and so we started to explore which triggers were key for her. Since stress is a trigger for most of us, we addressed it with breathing, mindfulness, and neuro associations that she had formed over the years. I then ran a food sensitivity test and she had a high reaction to both gluten and dairy. So, we removed it from her diet. I also ran a stool test and found a parasite called blastocystis hominis. While she was not having major digestive issues, this is a type of infection that can create an immune trigger even if there are no major IBS symptoms. This parasite is not always the easiest to eradicate and typically takes about 10 weeks of support. I have very good results with a product called Para 1 and Para 2 which I then follow with liposomal artemisinin and then GI MicrobX. This did the trick and her test was clear when we rechecked in 3 months. Happy Ending When we first started treatment, her eye issues actually got a bit worse in the first 3 weeks. This is not uncommon when cleansing and making a lot of changes. By week 4, she started to feel better. Eight weeks in, her eyes were about 50 percent better and after 12 weeks on both the diet and cleansing protocol, her eye issues were completely gone. New blood work revealed that her thyroid peroxidase antibodies were down to 75 (from 325). She will continue her gluten and dairy-free diet while we support healing her gut with probiotics, enterovite and L-Glutamine. Through all this, she has focused on reducing stress through mindfulness breathing and taking time for herself. (0:27) Stay tuned to the end to see Stacy's embarrassing blooper! When Stacy started Paleo 10 years ago, nuts were extremely popular, and then the "paleo police" got ahold of them and they became demonized for their caloric density and use in dreaded baked goods. Sarah has found nut research fascinating and surprising as the health community vacillates between love and hate (9:02) Peanuts account for 2/3 of American nut consumption despite actually being a legume (it grows underground, not on trees!) Peanuts have challenges: they're very pesticide heavy and are susceptible to mold. Most scientific studies can't separate out quality. Are the problems with peanuts problems with peanuts or quality? Is it the HFCS or hydrogenated oils perhaps? A mere 20 grams of tree nuts per day is associated with substantially reduced risk (think 20-70%) of cardiovascular disease, cancer, neurodegenerative disease, kidney disease, diabetes, infections, and mortality from respiratory disease. High fat plant based foods: nuts and seeds, olives and avocados. That's it! Nuts are unique because they have a lot of mineral content. High in polyphenol and phytosterol which lower cholesterol. High in Vitamin E and B vitamins as well. Unique fiber types and fats that are good for gut microbiome. High in L-Argenine and L-Glutamine, which are essential amino acid they need to consume as well. These amino acids alone change the gut microbiome composition. These properties alone might account for why nuts have all these health benefits. Numerous studies show that people who regularly eat nuts tend to have more favorable blood lipid profiles, and one meta-analysis of 25 clinical studies showed that nut consumption had a dose-response cholesterol-lowering effect. Interventional studies consistently show that increasing nut intake has a cholesterol-lowering effect, even in the context of healthy diets. Plenty of research suggests that, despite their energy density, nuts and seeds don’t contribute to weight gain, and they may even protect against obesity and diabetes. This is probably because of their gut microbiome effects. "Our gut microbiomes basically control our biology. They're more influential on our biology than our brains. Everything about our health is intertwined with the gut microbiome." Read Sarah's Ebook, The Gut Health Guidebook! Different nuts have different Omega-3 to Omega-6 ratios, which is tied to cholesterol and cardiovascular health. But the heart benefits of nuts aren't tied to the ones best for that ratio. If that was the mechanism, than no nut should have that benefit because none had the most beneficial Omega-3s like seafood has. Most likely they are heart healthy because of monounsaturated fat. Nuts and seeds are an early reintroduction on AIP because their benefits far outweigh the reasons you would cut them out (essentially only because they have a high instance of intolerance). Stacy found that dose was important with her reaction to nuts and that quality was important. Don't oxidize your nuts by having them sit for months or buying cheap low quality nuts. Benefits of Walnuts Let's talk about the benefits of individual nuts that have been studied (29:44) Sarah is most interested in the mechanisms of provoking health, they "why?" of it. And she keeps seeing it linked back to gut microbiome benefits. Best studied nuts are walnuts, almonds and pistachios. In one randomized, controlled human trial, almost 200 healthy adults were given either eight weeks of a walnut-enriched diet (43 g of walnut per day) followed by a nut free diet, or the same two diets in reversed order. The study found that walnuts significantly increased the abundance of Ruminococcaceae and Bifidobacteria, while opportunistic Clostridium sp. cluster XIVa species species significantly decreased. Overall, walnut consumption appeared to give a major boost to probiotic and butyric acid-producing species in the human gut. Another study tested the effects of eating either 42 g of walnuts or 0 g of walnuts daily for three weeks, and found that walnut consumption resulted in a 49-160% increase in the relative abundance of Faecalibacterium, Clostridium, Dialister, and Roseburia, while also causing a 16-38% lower relative abundance of Ruminococcus, Dorea, Oscillospira, and Bifidobacterium. Likewise, compared to the walnut-free diet, the walnut diet reduced the microbially derived, proinflammatory secondary bile acids deoxycholic acid and lithocholic acid by 25% and 45% respectively, coinciding with a reduction in LDL cholesterol. The researchers concluded that the gut microbiota may play a direct role in some of the health benefits associated with walnuts! In rats, a similar study feeding walnuts versus a macronutrient-matched walnut replacement (corn oil, casein, and cellulose fiber) found that walnuts enriched the microbiota with probiotic bacteria including Lactobacillus, Ruminococcaceae, and Roseburia, while also significantly reducing Bacteroides and Anaerotruncus. Benefits of Almonds Because almonds are one of the highest Omega-6 nuts, they're often considered "the worst nut" but in terms of the gut microbiome, they're fantastic! (38:43) They've compared defatted almonds and skinned almonds, but really the benefit seems to be from the fiber improving species diversity. The more species you have, the more cooperation between species. For example, creating B12 has 30 different processes and it takes many bacteria cooperating to form B12 in the gut. And more species mean less opportunity for the opportunistic bacteria to take over. A study of college freshmen found that eight weeks of almond snacking (vs. graham crackers) (56.7 g per day of almonds) led to a significant increase in microbiota α-diversity and a 48% decrease in the abundance of the pathogen Bacteroides fragilis. In another study, 48 adults were fed a daily dose of 56 g of roasted almonds, 10 g of almond skins, or 8 g of just the fiber as a control. After six weeks, fecal levels of bifidobacteria and lactobacilli were significantly higher from consumption of both the almonds and almond skin, while levels of Clostridum perfringens significantly decreased—suggesting components of the almond skins (present both in whole almonds and the skin only) were responsible for these prebiotic effects. Stacy heard someone redefining how you should think of macronutrients as protein fat and fiber instead of protein fat and carbohydrates, because it's the fiber that will be the health benefits. Sarah agrees the fiber targets are a good idea because fiber is often neglected and is very important. Fiber is carbs we can't digest but our microbiome can! And small changes to fiber molecules mean that some species can break it down and some can't. So variety in fibers is important for species diversity! The Benefits of Pistachios Pistachios are different from other tree nuts. They're in the cashew family (same as poison ivy and mango, actually!) (50:18) They definitely contribute to diversity of gut microbiome species like other nuts. Uniquely, they also reduced the levels of trimethylamine N-oxide (TMAO) measured in the urine. TMAO is the byproduct that is the link between red meat and cardiovascular disease. (though it is unknown whether it's a gut microbiome issue that produces it or if the TMAO causes the bad microbiome). Other Nut Benefits Sarah is going to hit you with her fun facts! (53:19) Every nut and seed that has been evaluated in terms of its impact on the microbiome shows unique benefits. That means variety is best! Hazelnuts and pecans have the highest proanthocyanidin (an awesome class of polyphenols) content Highest contents of ellagic acid (walnuts and pecans) also presented the highest total phenol contents Macadamias and cashews are the highest in monounsaturated fats (also good for microbiome, and heart-healthy) Brazil nuts are super high in selenium, like 1 has 175% of the RDI! And an ounce has 1000% of the RDI! Selenium is important for immune and brain function. And it's why you go crazy from mercury because it permanently binds with selenium. Hatters should have been pounding brazil nuts! And Ounce per Day! An ounce of nuts per day is the best idea. No further benefits after about an ounce. (58:26) Moderation is a "dirty word" in health communities because people use it to rationalize less than ideal choices. But in terms of nuts, this actually is the best option. Going beyond that can increase risk of stroke. So don't go nuts on nuts! Stacy thinks of it in terms of how our ancestors would have eaten them. They're time consuming to get out of the shell and last longer than fruits and vegetables, so moderation seems like what would have been our relationship to them. While there are some primates that do eat nuts as a primary food source. Sarah is curious how these species have adapted to high nut lifestyles. Accountability: This can't be your friends. Your friends are just as lazy and unmotivated as you are. Let's face it. We are who we hang around. If your friends were strong enough to motivate, inspire, or lead you, they would have already done it, and you wouldn't be where you are. 3. The Decision: Decided you're with it! 4. Action: You must take simple and consistent actions that build positive momentum. When you defer action, the likelihood of you actually doing what you said you were going to do drops by 87.36% because you stay on the someday hamster wheel. Booby-Trapped In the week's episode, I discuss why the path of least resistance is always booby-trapped. This is true in areas of war and all areas of life. The wish is that that path to success is a straight line. The reality is that the line is rarely straight, and it requires grit and determination and advanced navigating skills. Eco-Challenge ~ Adventure Racing ~ Military ~ YOU Weight Loss: The hack, the quick fix, the fad, the magic pill, or potion ~ the bright shiny object. We know that the only real formula for weight loss or overall quality health is committed consistency to the basics. The basics are boring to most people. And when humans get bored and lazy, they get complacent, and complacency leads to looks for a shortcut or the path of least resistance. In war, this is when you end up dead. Money: The get rich quick scam, the pyramid, the MLM, the gold mine in Arizona that sounds too good to be true. All of the above will leave you broke and jobless. I’ve spent since April of this year healing my body, first with the 3 week detox followed by the CBO protocol as I tested positive for candida. During the last week of the CBO I found out I was pregnant! Wonderful news, but very surprising. I’ve been off contraception for 2 years managing my cycle naturally through tracking, very successfully. I have a relatively normal cycle give or take a day or 2. Now the mystery- my conception date happened on the last day of my period. Normally the a safe zone I would have thought? I know the CBO can throw out your cycle but can it throw your cycle out that much? Clearly all of my healing turned me into a fertility machine which I feel very grateful for but would love to know how. :D Thanks again for all you do!! A few questions: 1) What do you think of the product Advanced TRS (toxin removal system) to remove heavy metals and of zeolites in general? TRS is a nanoparticle zeolite that crosses the BBB. Any safety concerns for adults and children? I have read some regarding aluminum content and I believe that since metals are estrogenic, using the full dose for 6 months caused me to have estrogen dominance. I took a heavy metals test after that and there was nothing concerning reported. Do I even need zeolites anymore in this case? Check out today's questions: Nikki: My husband has had dry scalp since his teen years and continues to do so into his thirties. He has been treated by conventional doctors for this with antibiotics/steroids with little effect. For the past 11 years it’s only been him who has struggled with this. As of last year, I have started to have dry scalp as well as our children. We try to eat healthy. We do eat out every other month and consume some white flour. We live on a small farm but are surrounded by conventional fields/farmers. Where should I start in healing my family? We recently ran the big five lab tests on my daughter and hope to get the rest of the family tested as well. Thank you! Anonymous: Hello Cabral Team, I am an undergraduate student looking to pursue a naturopathic or functional medicine health career. I have always had a passion for health and wellness, but last summer, through exposure to your podcast and products, I turned my attention towards natural remedies including detoxification, nutrition, and lifestyle changes to address chronic illness rather than through the use of pharmaceuticals. I am very interested in helping others discover this lifestyle and change their lives as I did; however, I am confused about how to pursue this path as many of my professors and advisors at school are unaware of functional medicine and the jobs it includes. Could you talk a little bit about your path in pursuing your degree, how you chose a medical school, any internships, opportunities, or jobs that lead you there and solidified your decision to attend medical school? Is it better to pursue another degree such as nursing or physician's assistant studies and build a job as a holistic practitioner around that or is it recommended to go to naturopathic/functional medical school? Ingrid: Hi Dr. Cabral, We made a video version of this email that I'm including the link to here which is better to see his skin clearly: https://youtu.be/h4PeKh-O0SQ My husband and I live in Ann Arbor, Michigan. He is a very healthy man of 34 who has never had alcohol, smoked cigarettes, has eaten a vegan diet for the last 3 years, and exercises regularly. Both he and I got a lot of mosquito and bug bites in the summer last year. In October, he realized that a small bump (he figured an insect bite) on his arm had not gone away. Thinking then that it was a wart instead, we put a salicylic acid patch on it. This burned a hole in his skin, which turned into a dime-sized open sore, oozing yellow watery liquid. Dermatologists nearby weren't available until January, so we went to the PCP office instead. The nurse practitioner determined it was impetigo (no tests, just visually) and gave him a topical antibiotic (Miuprocin) to put on it for a few days.This made the spot on his arm worse. The sore widened to be a strip of bumpy, oozing skin about 2 inches wide. We went back to the PCP, saw another NP who determined that the antibiotic must not have been strong enough, and prescribed him an oral antibiotic, Bactrim. Within a few days of the treatment with Bactrim his whole body exploded in hives, bumps and rashes. The original area on his forearm did not improve at all. It was very challenging, he couldn't sleep at all due to the itch and was extremely uncomfortable for the next few weeks. Total misery. We finally met the dermatologist in Jan. She said he likely never had a bacterial infection, that the original bump was probably an ingrown hair, but now he had developed severe eczema due to his response to the antibiotics. When asked why this happened, she basically shrugged and said "sometimes a person develops eczema", and informed us that this is now something we need to just "manage" for life. She gave him a tapered dose of oral Prednisone for a few weeks, and advised the use of hydrocortizone and triamcinolone steroid creams for some of the worst areas. This took his symptoms and level of discomfort from 100% to about 20%, but he never got completely better, some areas of the skin like the nipples were still very inflamed, itchy and scaly. After being off the prednisone, some symptoms elsewere re-emerged slightly, and the derm said this was normal after coming off of steroids. For the areas like the nipples that never got better she prescribed Protopic (a non-steroidal immune suppressant). Despite her assurances, this cream has been linked to development of lymphomas and so we never used it. From mid-March to the beginning of April his skin health really took a nose-dive. The sores and rashes came back at full steam and he's as uncomfortable as he was after the antibiotic. We began seeing an Integrative doctor in our area who had him take the OAT and a blood IgE and IgG food sensitivity test. The doc said that according to those tests, he has a number of food sensitivities and an extremely high level of d-Arabinose in his urine, which indicate a Candida overgrowth. She felt this candida overgrowth is the reason for his skin reacting this way after the antibiotic. She has him on Diflucan and Nystatin, as well as Borage Oil, Vitamin C, Quercetin and Nettles, Vitamin D3, Magnesium, Zinc, L-Glutamine, and a B Complex. We have also removed all his trigger foods from the sensitivity test and are following a paired-down anti-Candida diet (basically we've removed fruits and are eating mostly vegetables, some brown rice, mung beans and lots of nuts and seeds). We are now almost at the end of week 4 of the treatment and are concerned that his itch and other symptoms have not significantly improved. He is still incredibly itchy at night and while the scabs of his sores have fallen off, the skin underneath is raw and red and doesn't seem to be healing at a normal pace. Last weekend, some of the spots that we thought were looking somewhat better began oozing clear serum again, but have now dried up again. It's hard to say if his skin is healing, it doesn't look better, it just looks different? My question is, are we pursuing the wrong course here? We really thought the integrative doc had the answer when she said he had a candida overgrowth. But even she seems surprised at the lack of progress. We are feeling like no one really has a handle on what is actually happening to him.Thank you,Ingrid Cherie: Hello. A few questions. Can you have cheat meals on the CBO protocol, and if so, when can they begin? I am asking because foods are slowly re-introduced and I did not know if a cheat meal with foods not on the Gut guide list would negate all the benefits. Also can you have CBD oil on the protocol? And lastly, when should I take vitamin c and zinc during the protocol? Can I take them with the CBO pills? Cassandra: Hi Dr Cabral I've completed the CBO protocol and stuck to it 100% however I've ran an organic acid test and have high bacterial and yest markers. I'm confused why you approve of legumes and beans on the CBO when they are high in fructans and they feed yest and bacteria. I understand they are easy to digest foods but still all naturopaths I have spoken to say to stay clear of them. I'm really disappointed that I'm going to need to do another round. I'd love to hear your thoughts on this. Many thanks Yvonne: I am just listening to your podcast with Dr Gerry Curatola and think I probably have several issues related to what you're talking about - teeth removed, root canals and mercury fillings and have health issues. My concern is that the detailed information about what is required to return health sounds extremely expensive and only available for the rich. I live in Canada and am on a retirement budget so I either wouldn't have the funds required or if I did I would then be destitute. So does that mean for someone on a retirement budget good health will not be available? Ashlee: Hi Dr Cabral. My husband and I have been trying to conceive for almost a year and would love some guidance. We have completed the 21 Day detox, have both been doing the daily protocol level 2 for 5 months and I have been taking the prenatal package as well. We both just competed the Big 5 labs and are now doing the heavy metal protocol and will do the parasite cleanse and CBO by the time you read this. I am also seed cycling and we are taking vitamin d, adrenal soothe, cal-mag, zinc, DHEA, digestive enzymes and vitamin c as recommended by our health coach. Most of these were recommend for up to 12 weeks. We eat organic as best we can and follow a similar diet to the DCD with the inclusion of carbs. We have also detoxed the house from everything we have control over too. We are hoping that by the time you read this I will be pregnant but if not I am asking where can we go from here? We want to have a natural birth and feel like we have done everything possible to help the process. Is there anything else we can do? Or what do you suggest our next step be? I thought maybe it was something I ate but when I eliminated certain things from my diet, it is still happening. I get tingly itchiness and anything that rubs against my body, I get welts on my skin. I am not sure if it is a delayed response to food or some other allergen.I have been highly stressed for over 6 months. I am taking 2 capsules of adrenal soothe every night and I plan to take your TAH test again next month. I have also experienced eye floaters recently and my acupuncturist says I have a chi and blood deficiency (my last cycle ran for 25 days which isn’t normal for me). Also my tongue swells and my skin is tender to touch when I have cheat meals which sadly are happening more than Once a week. What can I do to get well again? My friend recently took the MRT food sensitivity test and it has a lot more foods on it than the one you are currently using. I prefer to go with your coaches and health plan but I wonder what your thoughts are on a more comprehensive list of foods from this competitor’s test. Sorry for the long questions but I did want to say that although I am dealing with these issues, I have come so far in my health journey based on your recommendations and health coaches and look forward to continued improvement. We reached out to two of our favorite health and wellness experts, JJ Flizanes and Evita Ramparte, to talk all things digestion! Digestion is everything. It’s your fuel source. Everything you put in your mouth gets broken down in your body and either gets used up as energy, gets turned into cells, or gets eliminated. The quality and variety of what you choose to eat determines how well your body functions. But modern diets are packed with allergens (GMOs, glyphosate-sprayed wheat, chemical additives, gluten, dairy, pesticides, etc.) that the gut cannot process. The result? Inflammation, depression, chronic disease. One of the most common digestive problems is leaky gut. Your gut wall is lined with villi—these are tiny finger-like structures that filter out food particles, absorbing the good stuff and keeping other stuff out. But when you eat foods that your body struggles to break down, it becomes wedged in your gut lining. Over time this creates micro tears in your intestine, which means undigested food particles, that are supposed to be contained within the gut wall, escape into your bloodstream. This triggers an immune response: your body attempts to protect itself from what it considers a foreign invader by sending white blood cells to attack. The result? Inflammation. And the more you keep eating foods you can’t digest, the more tears appear, the more inflammation you experience, and the more your health deteriorates. Recommendations? Today's Topic: FODMAPs! Wendy asks: "I have been diagnosed with hypothyroidism, fibromyalgia & fructose malabsorption. I was eating a FODMAP diet for a few years before starting Paleo 14 months ago. I combined AIP & FODMAP 8 months ago, and while I have felt much better, I'm not symptom free and still often have stomach pain in the lower left abdomen.This pain seems to be more frequent lately. Have I missed something?" (11:09) FODMAPs are poorly absorbed short chain carbohydrates. The acronym stands for Fermentable Oligo-, Di-, and Monosaccharides, and Polyols. These are fructose carbohydrates and sugar alcohols. They are loved by bacteria, but not absorbed in the small intestines Beans are a typical FODMAP rich food, which is where the song comes from. Finian recited the rhyme. He didn't know it before now, but now won't stop saying it! FODMAPs feed the bacteria in your gut, leading to a bacteria bloom. That's why you get bloating, gas, and other GI problems. People with FODMAP issues typically have poor gut health, gut dysbiosis, or a genetic predisposition. Problems with FODMAPs can lead to small intestine bacterial overgrowth (SIBO). Low FODMAP diets have been used to treat IBS and SIBO successfully. But the science seems to indicate that this is just symptom management. The real issue is that FODMAP loving bacteria are actually good bacteria that we need. Just not too many! A low FODMAP diet could eventually lead to a poor gut microbiome diversity. The new recommendation is to follow the low FODMAP diet for 2-4 weeks. Afterwards, go with gut healing like veggies, low-inflammatory diets, broth, L-Glutamine, and seafood. Then, add back in FODMAPs to tolerance. It help grow back these great beneficial bacteria. If it's not going well, there could be further issues like infection or other disorders. See a doctor! Current knowledge indicates that no diet can cure SIBO and only medical intervention can cure it. See a doctor! If you need guidance, see Paleo Mom Consulting! Why are some veggies like garlic, onions, artichokes, all the brassicas, etc. on the FODMAP list? Well, they do contain a lot of fructans, which are fructose rich fibers like inulin Sugar alcohols or polyols are in apples, watermelon, cherries, mushrooms, stone fruits, avocados, etc. Katherine asks: "What are the best resources for following Paleo AND low FODMAP? I'm struggling big time on what the heck to eat and when I look for answers, I can't seem to find a combo of the two 'diets'. Only one or the other. Very frustrating. I'm hungry!" (27:38) In The Paleo Approach and The Paleo Approach Cookbook, everything is labeled with whether it is low FODMAP or substitutions or if it can't be low FODMAP because many autoimmune people have GI issues. Aglaee Jacob of Radicata Nutrition is the expert on this. Get her book Digestive Health with Real Food because she really knows how to navigate this issue! When you're paleo, many of the high FODMAP foods are already gone! Avoid the onion family, the cruciferous vegetables and things like squash. Further, you just have to play with dose! If you still have issues with FODMAPs after a 2-4 week intervention, it's time to call in the experts! One of the cool things about FODMAP issues, is that it's one of the conditions with GI health you can actually cure it! Bill from Primal Palate shared his story with FODMAPs here and here. There are components to the gut health issue: getting the digestive enzymes right, getting the bacteria right, and getting the healthy cells right. All of those things are helped with a nutrient dense, low inflammatory diet, reducing stress and getting sleep! When Sarah did her nutrient dense diet, she never even needed to limit her FODMAPs. Soluble fiber – I like mixing acacia fiber in a glass of water (you can also add to a smoothie) - here's why. Note: don't overdo it or it will clog you up! 4. Cocoa - go for unsweetened cacao powder and make a hot chocolate (just mix with warmed almond milk) or a delicious, super simple dessert like avocado pudding or any of these suggestions inspired by the Paleo community. Dark chocolate – I like 85% and higher, but make sure the cacao content is at least over 70%. 6. Fizzy water - I like seltzer/mineral water with lemon or lime added. Drink this first, then decide if you need the thing you were craving. 7. Replacement treats - sometimes these will work to shut down those cravings. Pay attention to what works for you: • Decaf coffee (or tea) after a meal (note: the nutritional police are still debating this one, but it works for me) • 1 glass (or 1/2 glass) of red wine after a meal • Have whatever you're craving PLUS a healthy treat. For example, chocolate with an apple; half that cupcake and a pear, etc. • Paleo desserts - as mentioned above, make something yummy without a load of sugar in it. Just google "Paleo desserts" or scroll through my Facebook page for ideas. There are so many super easy things you can make in 5 minutes that are utterly satisfying and don't sabotage your wellness goals. What to DO instead: INTERRUPT THE PATTERN! • Brush your teeth and drink a tall glass of water • Chew 100% Xylitol Gum or a super strong mint • Get visual – keep a picture of you looking great or of something that makes you happy and reminds you of you at your best! • Get outside: breathe deeply (all the way down in your belly) and/or walk for 10-20 mins. • Do a 1-minute cardio blitz. For example, jump on a rebounder (mini-trampoline) or do 25 jumping jacks and 10 burpees right there in the kitchen - better yet, turn on some music and DANCE! • Get naked. Or, just change clothes into something you love. Anything to be reminded of being in your body. • Or, just EAT IT, but don’t eat AROUND IT. For Pete's sake, it's just FOOD. Be the boss of it. Eat it, enjoy it and pat yourself on the back for not freaking out. Don't eat everything else in sight in order to avoid that thing you're craving. Also, am I "wasting" the L-glutamine by taking it on weight training days and not running days. James says: The dreaded cramping question! I'm a competitive age group triathlete and I've been having cramping issues in races. I know the usual answer is your not training at the same intensities as I race at and/or you're not taking in enough nutrition/electrolytes. In my case, neither are the issue (I think). I train 15-18 hrs/week which includes a lot of intensity and I am spot on with my diet, training nutrition and race day nutrition. I take all of it very seriously, so when it comes to cramps I am stumped. The cramps can come on as early as the swim. Often I get them near the end of the bike though and almost guaranteed in the run. I never get them in 10k's or 5k's, just the longer stuff half marathons, olympic triathlons and especially half ironmans. Do I need to just load up on the salt tabs? Tweak in the diet? I've never taken a product like either of these and was wondering if you knew about them? I want to cut the fat, without losing muscle and am not sure of the best way to do this. Should I stop using creatine, the zmk, and the shock therapy if I use the thermogenic? Are any of these contradictory products that should not be taken together? Many people cannot utilize their dietary protein, instead it is converted to sugars for fuel, or to fat for storage. This is due to lack of nutrition, malabsorption and exposure to toxic metals (silver amalgam dental fillings, vaccinations, lead paint, and a wide variety of industrial products). Inhibiting protein breakdown causes inadequate nutrient absorption & toxin elimination. This wreaks havoc on your immune system, resulting in pain and many chronic & degenerative diseases. Amino acids have to be first digested before they can be assimilated. Amino Acids are broken down in the stomach by pepsin, which is converted from pepsinogen by lowering the pH of the stomach with hydrochloric acid. If the pH of the stomach doesn't lower to around 2.5, the first stage of protein breakdown fails. The semi-digested food moves to the small intestine where a pancreatic enzyme named trypsin, along with other proteases (protein-breaking enzymes) breaks down the protein further. The above are all the stages of digestion. The next important phase is assimilation. Intestinal villi (the little finger-like projections in the small intestine that are responsible for assimilation) shrink in response to stress. Stressed people don't assimilate food - of any kind. The only known substance that will rejuvenate the small intestinal villi is the amino acid L-Glutamine, which turns into glutamic acid to cross the blood-brain barrier where it is converted into glutamate (the most stimulatory neurotransmitter in the brain) and eventually breaks down into GABA (the most inhibitory neurotransmitter in the brain). Living in our industrialized society presents a large number of potential problems that inhibit protein digestion, leading to amino acid deficiency: HCl, proteases, L-Glutamine and others, all of which can slow or stop digestion and assimilation.
