Tamara Inoue, UC Irvine's WBB head coach joins this week's episode of @Notevend2 . Inoue's run at UC Irvine is one of the most impressive turnarounds in college basketball. Taking over a program that finished 5–23 in her first season, Inoue has completely redefined the ceiling of UC Irvine women's basketball. She's now on pace to lead UCI to five straight 20+ win seasons, is already the all-time winningest coach in program history, and guided the program to a Big West Championship in 2023–24. This season, her team sits at 17–3, continuing a stretch of sustained success that few programs can match. A Big West Co-Coach of the Year (2020–21), she's steadily transformed the program from rebuild mode into a consistent conference contender. What started as a challenging rebuild has evolved into one of the most stable and respected programs in the Big West. UC Irvine is positioned to remain a force in the conference and beyond. The main goal remains to get back to the NCAA tournament for the second time in three years. Will they win the Big West?

Otro tren descarrila sin heridos entre Blanes y Masanet, ambos incidentes atribuidos al temporal. En Adamuz, Córdoba, el accidente eleva a 42 las víctimas mortales. He won an Australian Criterium National Championship in 2019 and another 4 podium finishes in the event over his career. He also was able to line up at the start of the of Paris-Roubaix in 2018 and 2019.Brenton now works for AusCycling as the Development Academy Manager, where he aims to strengthen the pathway for emerging talents on both the road and track and helps to facilitate the AusCycling Development Academy programs around the country. Lidia Cusack is an emerging professional cyclist who has made her mark in both road racing and cyclocross. She began racing at a young age on her local club team in Chevy Chase, Maryland, and quickly moved into the national and international scene. After graduating high school, Lidia took the bold step to move to Europe to pursue her cycling dreams.  Her dedication paid off with impressive results in UCI events, World Cup races, and World Championships. Notable achievements during her junior years include victory at the prestigious Druivenkoers - Overijse road race and a commanding cyclocross World Cup win in Dublin.  Her 5th place finish at the 2024 Cyclocross World Championship further solidified her status as a rising star in the sport.   As a U23 rider, Lidia is staying patient while challenging herself among elite competitors - knowing that results will come with experience. In her future Lidia dreams of having a long and healthy career, winning a monument on the road, and winning an elite cyclocross world title. She also aspires to be a role model for future American female cyclists, showing them that it is possible to succeed at the highest levels despite barriers that come with being an American in a very Euro-focussed sport. Aside from racing her bicycle, Lidia loves spending time outdoors - mountain biking, backpacking, skiing, sailing, and anything else that brings her to nature. This week on the show Kathryn and Kristi are chatting with French cyclist, Cécile Lejeune. Cécile grew up in the suburbs of Paris, and got her start in endurance sport in triathlon at age 12. She had quick success in the sport winning national titles in both triathlon and running. This success earned her a running scholarship at Arizona State University. But behind the scenes Cécile was struggling with an eating disorder and battling RED-S, and just one month into her collegiate running career she suffered a stress fracture that ultimately led to her being medically retired from running by the team doctors as ASU. Cécile speaks openly about her struggles with her eating disorder and RED-S, and how she has worked her way back to a more healthy view of fueling and training. She also tells us how leaving running allowed her to find her way into cycling and pursue the sport professionally. After a stint on some UCI road teams in Europe, Cécile moved back to the US and is now part of team CCB. Fisiopatología en pocas palabras: Trombo grande en circulación pulmonar → aumento de poscarga del ventrículo derecho → falla del VD → colapso hemodinámico. Disnea súbita, dolor torácico, síncope, hipotensión, antecedentes de riesgo trombótico. Resumen del estudio de Harjola et al. Objetivo principal del estudio Explorar supervivencia y complicaciones hemorrágicas del uso de trombolíticos prehospitalarios para embolia pulmonar de alto riesgo. Diseño Datos de EMS del área metropolitana de Helsinki + hospital universitario. Periodo aproximado: 2007–2019. Inclusión: Pacientes con EP de alto riesgo sospechada clínicamente. Tratados con fibrinolisis intravenosa prehospitalaria. Diagnóstico de EP confirmado posteriormente por imagen o autopsia. Grupo comparador: Pacientes con EP de alto riesgo que no recibieron fibrinólisis prehospitalaria. Resultados clave Total de pacientes con EP de alto riesgo: 60. Grupo con trombolíticos prehospitalarios para embolia pulmonar: n = 23. 44% mujeres. Edad media: alrededor de 57 años. 74% se presentaron en paro cardiaco. 26% en shock obstructivo. Mortalidad: Mortalidad prehospitalaria aproximada: 35%. Mortalidad intrahospitalaria: alrededor de 27% de los que llegaron vivos. Mortalidad total combinada: cerca de 52%. Todas las muertes en este grupo fueron en pacientes que llegaron en paro cardiaco. Complicaciones: 2 pacientes con sangrado mayor. Ningún sangrado fatal. Supervivencia a 12 meses: Los pacientes trombolizados que salieron vivos del hospital seguían vivos a los 12 meses. Grupo sin trombolisis prehospitalaria: n = 37. Más añosos (edad media cercana a 72 años). Mayor proporción de paro cardiaco. Mortalidad a 12 meses más alta (≈ 76%, tendencia, p alrededor de 0.06). Comentario para desarrollar: Es un estudio observacional, con n pequeño, no podemos concluir causalidad, pero sí hay "señales" interesantes de posible beneficio. ¿Qué nos dice realmente este estudio? Mensajes principales La EP de alto riesgo fuera del hospital tiene una mortalidad muy alta aun con intervenciones agresivas. En este contexto crítico, los trombolíticos prehospitalarios para embolia pulmonar: Parecen relativamente seguros (pocas hemorragias mayores, ninguna fatal). Podrían ofrecer un beneficio en supervivencia, especialmente en pacientes seleccionados. Limitaciones para mencionar Serie de casos; no es ensayo aleatorizado. Número pequeño de pacientes trombolizados. Posible sesgo de selección: Pacientes más jóvenes y potencialmente con menos comorbilidades recibieron trombólisis. No responde preguntas como: Detalle exacto del protocolo. Diferencias entre equipos. Tiempos exactos desde el colapso hasta la trombólisis. Idea clave: No es un "permiso" para trombolizar a todo el mundo, pero sí una invitación seria a considerar que, en EP de alto riesgo, la inacción también tiene un costo muy alto. El reto práctico: decidir trombolisis en el campo Barreras en la vida real Diagnóstico presuntivo sin imagen: Dependemos de clínica, antecedentes, ECG, quizás eco focal. Miedo al sangrado: Especialmente hemorragia intracraneal. Falta de protocolos claros: Muchos sistemas de EMS no contemplan todavía trombolíticos prehospitalarios para embolia pulmonar. Falta de entrenamiento específico: No todos se sienten cómodos con indicaciones, contraindicaciones, dosis. Cómo ayuda ACLS aquí ACLS bien aprendido: Te obliga a pensar en H y T, no solo en adrenalina y ciclos. Te muestra dónde se colocan los trombolíticos prehospitalarios para embolia pulmonar dentro del algoritmo. Te entrena para liderar un equipo y tomar decisiones bajo presión. Conectar con los cursos de ECCtrainings: En nuestros ACLS discutimos escenarios de paro por EP masiva. Practicamos cómo tomar la decisión de administrar o no trombolítico. Simulamos la comunicación con el hospital receptor después de trombólisis. Caso clínico narrado Propuesta de caso Varón de 48 años. Disnea súbita, dolor torácico, antecedente de inmovilidad o TVP reciente. Hipotenso, taquicárdico, saturación baja, signos de shock. En la ambulancia entra en PEA. El equipo evalúa H y T → EP masiva muy probable. Protocolo local permite trombolíticos prehospitalarios para embolia pulmonar: Se administra el medicamento durante la RCP. Después de varios ciclos recupera pulso. Llega vivo al hospital, se confirma EP por imagen y sobrevive. Puntos a resaltar Valor de: reconocer el patrón clínico, tener protocolos, estar entrenado en ACLS. Conectar con la serie de Helsinki: "Son justamente este tipo de pacientes los que aparecen en la serie: altísimo riesgo, pero con posibilidad real de supervivencia si somos agresivos." Cómo prepararte tú y tu sistema Pasos sugeridos para líderes, educadores y clínicos de EMS Revisar la evidencia Usar este estudio como punto de partida para la discusión sobre trombolíticos prehospitalarios para embolia pulmonar. Evaluar la realidad local ¿Disponibilidad del medicamento? ¿Quién puede prescribir y administrar? ¿Qué soporte hospitalario hay (UCI, hemodinamia, ECMO)? Desarrollar protocolos claros Criterios de inclusión y exclusión. Algoritmo que integre ACLS y trombólisis. Entrenamiento formal No basta con escribir el protocolo; hay que entrenarlo en simulación. Cursos ACLS con escenarios específicos de EP. Simulaciones y revisión de casos Simulacros periódicos con roles definidos. Morbimortalidad / debriefing de casos reales o simulados. LA BIBLIOTECA DE LA HISTORIA nos abre uno de sus archivos, que nos va a acercar a: "El Uno. El primer asesino en serie de la historia de España". Hoy en LA BIBLIOTECA DE LA HISTORIA tenemos el placer de contar como invitado a Jorge Jesús García Bermúdez, un Policía Nacional, que además de presentarnos su novela, ya que él es escritor, nos va a hablar del que posiblemente es el primer caso de un asesino en serie en España. Sin más preámbulos os dejo que disfrutéis del programa. Jorge Jesús García Bermúdez, es policía nacional con casi 20 años ya de carrera profesional, es abogado, máster en práctica jurídica, y como he dicho anteriormente es escritor y autor de varios libros tanto de narrativa como de poesía. Su obra "Nochebuena en la UCI" fue finalista al Premio Nacional de Relatos "Alto Policía, esto es un relato", que es un premio creado para todos esos policías (independientemente del cuerpo policial al que pertenezcan) que tienen esa chispa de creadores de relatos y ganas de contar historias. Su relato corto "Luz tras las rocas", participó en el Certamen de relato Corto "Una mirada desde el corazón de la UCI", organizado por el servicio UCI del Hospital Mancha Centro, relato que fue seleccionado y publicado. "Los Senderos del Destino" es el título de su primer libro de poesía editado en 2019 por la editorial valenciana Loto Azul y presentado en la Feria del Libro de Madrid. El segundo libro de Jorge editado en el año 2020 por la editorial valenciana Olé libros se titula "Envuelta en Azul". Un libro en que habla de la Violencia de Género desde el punto de vista de un policía y tras su experiencia de 8 años en la oficina de denuncias, donde atendió más de 100 casos. Este libro también lo presentó en la Feria del Libro de Madrid en 2021, y además con él participó además en la programación municipal del día 25 de noviembre, que es el día contra la violencia de género. Jorge participó en el libro "Ñ, Manual de Autoestima para Españoles". Editado también por Olé libros, este libro reunió a grandes personalidades de la sociedad española, como Manuel Vicent, Luis María Ansón, Luis Alberto de Cuenca, el Chef con 3 estrellas Michelin Quique Dacosta, el presidente de Cruz Roja Española, la presidenta de la organización española de donantes de órganos y una largo etc., de personalidades de todos los ámbitos de la cultura, la ciencia y la economía española, en el que remarcan las bonanzas de nuestro país y su gran futuro. En este libro Jorge realizó el prólogo de presentación. La Dirección General de la Policía editó su cuento "El portero y los Pelicanos, El sueño de ser Policía" que fue presentado con mucho éxito en la Feria del libro de Madrid. Forhjulslir præsenteres i samarbejde med Continental Dæk Danmark. Husk at skifte til vinterdæk, når temperaturen er under syv grader. Continental har prisvindende vinter- og helårsdæk. Sikkerheden starter med dækket – både på cyklen og bilen. For præcis et år siden, i november 2024, var Alexander Salby med i Forhjulslir. På det tidspunkt stod han et sted i sin cykelkarriere, hvor han havde mistet lidt af cykelglæden og ikke kunne genskrive en professionel kontrakt i Europa. Han tog en chance og skiftede til det kinesiske kontinentalhold Li Ning Star - i jagten på at finde cykelglæden tilbage, vinde cykelløb igen og vende tilbage til det professionelle europæiske cykelfelt.  Det lykkedes i 2025. Han vandt cykelløb. Mange. 9 professionelle UCI-sejre og var blandt de ti mest vindende cykelryttere i 2025. Og vigtigst af alt genfandt han cykelglæden. Ruth Chepngetich got a three year ban, up from two, then down from four, and keeps the marathon world record nobody believes in. We discuss why her case is so frustrating for sport. Imogen Simmonds has been cleared to compete despite an Anti Doping Rule Violation, after she convinced a panel that her positive test was the result of contamination by her partner during intimate contact. And Oier Lazkano has been provisionally suspended by the UCI for athlete biological passport (ABP) abnormalities that date back to 2022. Ross explains the biological passport principles, why a suspension based on the ABP is so rare, and why it might have taken this long to bring the case against Lazkano.In Centre Stage (42:23), two papers on metabolic costs of exercise were published last week, with contradictory findings. We first explore a paper that proposes a metabolic limit of 2.5 our basal metabolic rate, and where that study fits into our understanding of exercise and metabolism. Then we consider another paper that contradicts that understanding by refuting the idea that our bodies constrain certain metabolic functions when we exercise in the equivalent of what Ross calls 'physiological austerity'. We try to explain why these studies contradict one another, the importance of energy balance in metabolism, and why there's a bit of truth in both models on opposite sides of the issue.Our Listener Lens (1:10:12) is inspired by a question from Leon, who asks about using heat as a way to increase cardiovascular stress without overloading his legs. How different is European and North American gravel racing? Velo was at the Gravel World Championships in the Netherlands to inspect the bikes used by Marianne Vos, Tom Pidcock, and many others, with the relatively fast and smooth course necessitating some interesting equipment choices. Integrated air pressure adjustment systems, huge (for gravel) gearing, and wildly different tire combinations were all spotted, as well as more than a few custom paint jobs for the special occasion. Alvin, Josh, and Levy also delve into Suunto's legal action against Garmin and why it differs from the Strava debacle all while Levy attempts to convince the crew that social media is a psy-op while denying being addicted to Strava. We also dig into the latest 3T Racemax2 gravel bike, its background, Josh's early impressions from riding the bike, and ultimately, why the bike is so dang interesting.
