About My Guest: My guest for this episode is Dr. Derik Anderson.  Derik Anderson, DC, CPT, CSN helps his patients be healthier by utilizing his education and personal and professional backgrounds.  Dr. Anderson grew up wanting to help people and initially planned to go to business school to work in energy conservation.  After a good friend's life was positively changed by chiropractic care, he had a change of heart. His friend spent two years with horrific intestinal problems and saw over a dozen providers, but no one could help her – until she saw a chiropractor.  The chiropractor knew that the vertebra and muscles around them affected the nerves that controlled the intestines. After treatment, she was no longer bed-ridden and went back to teaching, traveling and leading an active life.  Dr. Anderson realized that health care was his calling and his passion for helping people was channeled into chiropractic.  Dr. Anderson lives what he practices.  He is an avid runner, nationally competitive sailor, surfs, and is a youth coach.  Dr. Anderson eats a diet high in vegetables, low in starch, and drinks several quarts of water each day. Today, we have the honor of speaking once again with Sheila Ames, a registered nurse in Northern California who has been diagnosed with a rare type of a primary immunodeficiency known as common variable immunodeficiency or CVID for short. How did she fare during the recent Pandemic?  Tune in now to find out! At the time of her diagnosis she was working as an ICU nurse and her first doctors order was: no more exposure to infectious patients.  This diagnosis not only changed her career dramatically, it led her to following her life's purpose in opening her own health & wellness online coaching business to help others continue to find and work towards their life's purpose despite the hurdles that life gives us. ​Common variable immunodeficiency (CVID) is a primary immune deficiency disease characterized by low levels of protective antibodies and an increased risk of infections. Although the disease usually is diagnosed in adults, it also can occur in children. CVID also is known as hypogammaglobulinemia, adult-onset agammaglobulinemia, late-onset hypogammaglobulinemia, and acquired agammaglobulinemia. NIAID supports research to determine genetic causes of CVID that may lead to therapeutic approaches to address the disease. Researchers also are exploring how antibody-based drugs may lessen the severity of the condition. ​Causes CVID is caused by a variety of different genetic abnormalities that result in a defect in the capability of immune cells to produce normal amounts of all types of antibodies. Only a few of these defects have been identified, and the cause of most cases of CVID is unknown. Many people with CVID carry a DNA variation called a polymorphism in a gene known as TACI. However, while this genetic abnormality confers increased risk of developing CVID, it alone is not capable of causing CVID. CVID is also linked to IgA deficiency, a related condition in which only the level of the antibody immunoglobulin A (IgA) is low, while levels of other antibody types are usually normal or near normal. IgA deficiency typically occurs alone, but in some cases it may precede the development of CVID or occur in family members of CVID patients. ​Symptoms & Diagnosis People with CVID may experience frequent bacterial and viral infections of the upper airway, sinuses, and lungs. Acute lung infections can cause pneumonia, and long-term lung infections may cause a chronic form of bronchitis known as bronchiectasis, which is characterized by thickened airway walls colonized by bacteria.   People with CVID also may have diarrhea, problems absorbing food nutrients, reduced liver function, and impaired blood flow to the liver. Autoimmune problems that cause reduced levels of blood cells or platelets also may occur. People with CVID may develop an enlarged spleen and swollen glands or lymph nodes, as well as painful swollen joints in the knee, ankle, elbow, or wrist. In addition, people with CVID may have an increased risk of developing some cancers. Doctors can diagnose CVID by weighing factors including infection history, digestive symptoms, lab tests showing very low immunoglobulin levels, and low antibody responses to immunization. ​Treatment CVID is treated with intravenous immunoglobulin infusions or subcutaneous (under the skin) immunoglobulin injection to partially restore immunoglobulin levels. The immunoglobulin given by either method provides antibodies from the blood of healthy donors. The frequent bacterial infections experienced by people with CVID are treated with antibiotics. Other problems caused by CVID may require additional, tailored treatments. PheNet este un algoritm de învățare automată care poate identifica pacienții cu imunodeficiențe comune variabile (CVID), pe baza informațiilor provenite din dosarele medicale electronice ale acestora. Din cauza eterogenității fenotipurilor CVID, pot trece oriunde între 5 și 15 ani de la apariția primelor simptome și până la diagnosticarea corectă a unei boli din această categorie. PheNet încearcă să adreseze acest dezavantaj, putând scădea timpul de diagnostic cu cel puțin un an. Algoritmul de inteligență artificială filtrează datele medicale din dosarele electronice ale pacienților, identificând anumite tipare fenotipice corespunzătoare unui diagnostic de CVID, și clasifică ulterior persoanele în funcție de riscul de a prezenta o boală rară. Testele efectuate au demonstrat că PheNet poate depista corect mai mult de jumătate din pacienții cu CVID, cu cel puțin un an înainte de diagnosticul oficial. Our guest this week is Eric Nixon of Lansdale, PA who works for a blood plasma company and is the father of three all with Common Variable Immune Deficiency (CVID), a rare autoimmune disease. Eric and his wife, Candy, have been married for 33 years and are the proud parents of three: Benjamin (30), Hannah (27), and Daniel (24) all who have CVID, or Common Variable Immune Deficiency, a rare autoimmune disease. Eric also has Multiple Sclerosis and has been on disability for 17 years. Despite all the family health challenges, Eric has been leading small groups of fathers for years and years. Moderna's Kate Cronin on turning a pandemic hero into an everyday hero

Fresh off pandemic fame, Moderna wants consumers to know how it's applying mRNA technology to reimagine health and wellness. Among other strategies, the company is leaning into sports and music to reach consumers when they're likely to be most receptive.  Episode TranscriptPlease note, this transcript  may contain minor inconsistencies compared to the episode audio. Damian Fowler: (00:01)I'm Damian Fowler.Ilyse Lieffring: (00:02)And I'm Ilyse Liffreing.Damian Fowler: (00:03)And welcome to this edition of the current podcast.Ilyse Lieffring: (00:10)This week we sit down with Kate Cronin, the chief brand Officer for Moderna.Damian Fowler: (00:15)Moderna is a biotech company based in Cambridge, Massachusetts. The focuses on MRNA therapies and vaccines against a wide range of illnesses including cancer, COVID-19 and the flu.Ilyse Lieffring: (00:26)Although the company was founded in 2010, it became a familiar brand name during the pandemic when it received full approval for its CVID 19 vaccine by the US Food and Drug Administration.Damian Fowler: (00:38)Since then, it's positioned itself to become a more commercial company and it's opening up direct to consumer marketing channels using digital messaging to reach healthcare providers and patients alike.Kate Cronin: (00:53)If you think about the healthcare landscape and where we are, we came out of a pandemic that changed everything. The pandemic turned every company into a healthcare company because companies were focused on the health and wellbeing of either their employees or their constituents or their customers. And that means hotels, airlines, travel, restaurants. It changed everything. And I think what happened is in a post pandemic world, people are laser focused on health and wellness. And looking at Moderna, we are leaning into that new focus on health and wellness and new understanding of health and wellness. And so as a company, we're leaning in on educating the world about our technology and what we believe we're going to do to change the future of healthcare.Damian Fowler: (01:40)And on that point, how has that impacted the way you go to market and the way you have rolled out marketing strategies? I understand it's a pretty big shift for the brand.Kate Cronin: (01:50)For the first 10 years of Moderna's life, we were an r and d company, so we focused on research and development. That means we studied products, the lab, and we did not have any products commercialized, so we never took a product to market. When Covid struck our CEO realized, hey, we have an opportunity with mRNA to see if our technology works against Covid. And the good news is it did. And so we are super proud of the fact that our first commercial product was for Covid, shot in the arms of billions of people around the world, literally prevented serious disease and hospitalization. And so that changed our company. And because the pandemic was the first time when people actually asked for a vaccine or knew their vaccine by name, you know, people would walk around and say, I, you know, I've got Pfizer or I got Moderna, or whatever it was.Kate Cronin: (02:39)And they were proud of the fact that they got the shot to prevent Covid. And so there was instant brand recognition. So Moderna was not a household name until the pandemic and suddenly became a global brand. Everybody knew the name Moderna and what everybody knew was Moderna was a company that manufactured a COVID-19 vaccine. What they didn't know is everything else that we were doing. And so I was hired in the middle of the pandemic to start educating around who is Moderna? What is our brand? What do we stand for? What's our identity and how do we reach consumers about the future of MRA technology and where we're going beyond covid. That's been the the focus. And that's not just for healthcare providers. People were getting it from pharmacists. They were not going to their doctors to get shots. So it was a very much of a consumer driven market. And because it was consumer driven, that's where I was leaning in on my focus on consumers and educating them about mRNA. So many people had it in their arms, did they know exactly what it did and why should they care? We started changing the dynamic in terms of how we reach our audiences.Damian Fowler: (03:45)Would you mind defining what mRNA is? I know it's in the name of the company, Moderna.Kate Cronin: (03:51)Interestingly, the agency that I worked at named Moderna, it's basically about mRNA. And mRNA is known as Messenger ribonucleic acid. And mRNA is a molecule that contains a set of instructions or a recipe, if you will, that direct cells to make a protein using the body's natural machinery to fight disease or prevent disease. And that's really what it is. It's a set of instructions and if you think about Moderna, the name, we came up with Moderna because it's modern, we believe we're gonna transform medicines, but it has RNA at the end. So it was very, it was a very nice play on modern and then RNA, so ModernaDamian Fowler: (04:33)A perfect brand name.Kate Cronin: (04:35)Yeah, yeah, it worked out. It worked out really well.Ilyse Lieffring: (04:37)Do you think Covid and Vaccine Awareness opened up new direct to consumer channels? Do you think it's good timing with immunization season?Kate Cronin: (04:47)So I think COVID definitely opened up new channels and here's why. People started to really care about vaccination in ways they never did before. During the pandemic, you weren't able to go out, you weren't able to see your friends, you weren't able to see your family. Getting the vaccine meant everything and getting the vaccine meant you were able to live your life again. And so you cared about that vaccine and you wanted to know who makes this vaccine. You wanted to know what's in this vaccine and why is it letting me get back to my life? How does it work? How does it all work? I mean, imagine, you know, pre pandemic when you would get your flu shot, people get the flu shot, it's great and you just know you're gonna prevent flu. In the pandemic world, you got your covid shot and it meant everything to you.Kate Cronin: (05:29)And so there became this loyalty to the brand that started. You know, you never knew who manufactured your flu shot, generally speaking, you just generically asked for a flu shot. But when Covid struck and you'd get your booster, you said, well, I got Moderna before, so now I want my Moderna booster. And you were loyal to Moderna, A fandom was created. How do we build on that? People who are fans wanna know like, how do I interact with this company? And you know, what's next? And when we launch a flu vaccine or a combination Covid flu vaccine, we want people to understand, oh, that's the company that brought me the Covid shot. I understand mRNA and I understand what I'm putting in my body. As you know, there was a lot of debate and discussion about these new vaccines and can they be trusted? And the reality is, if you understand the technology and that it's a set of instructions and it's teaching your body to fight, you understand that this new technology is effective and tolerable and there are, it was generated with safety in mind. As we move into new categories, it's the same methodology and it's not a new chemical entity, it's just a new set of instructions. And that's why we think it's important for people to understand it. So as we launch new products, they don't have to understand the intricacy, but the general gist of how it works,Ilyse Lieffring: (06:41)How would you say Moderna is going about fusing this education in with its marketing strategy?Kate Cronin: (06:49)So knowing that we want to reach folks and educate them, we lean in on this idea of education and entertainment, which I'm calling edutainment. People retain more when they're being entertained, I think. And so with that in mind, we're reaching people where they are. So instead of lecturing consumers about, you know, you need to get vaccinated, we spend more time engaging with them when they're at sporting events or when they're at concerts, reaching them when they're having fun and they're open to hearing more. So for example, at the US Open, we sponsored it two years in a row and as a sponsor we also have a booth and we're able to engage with consumers at the booth. They can download a QR code, they can sign the lens, which is something that champions do. And then they, we create a boomerang they can use on social media and then they can become part of the Moderna community where people are able to opt in and say, yeah, I wanna hear more from Moderna.Kate Cronin: (07:43)Same thing with the iHeart Music Festival. And we actually had wristbands where they could download a QR code, find out where they can get their vaccines, so go to vaccines.gov. And also Ryan Seacrest did some informational podcast stuff for us and he announced the concert and you know, announced this idea of stay healthy, stay up to date, get your vaccine, bringing this model in. We also looked at habits of consumers and there are people who are religious about getting their flu shot. Those are the people where you say, well if you're religious about getting your flu shot, how do we make sure you also get your covid shot? And the CDC says it's safe to get your flu and your covid shot at the same time in terms of compliance. It's easier to get that message and say while you're at the pharmacy getting your flu shot, get your covid shot and then you don't have to go back for another visit. And that increases compliance as well. The future for Covid is gonna be very similar to flu once a year, get your flu shot, get your covid shot, and then you don't have to get another one until the next year. That's for most people. I think for the highly immune compromise or the elderly, there might be a boost needed in the spring, but the current recommendation is once a year.Damian Fowler: (08:45)It's fascinating to hear you talk about, you know, using sports, using music celebrities, you know, to reach audiences. And I know that Moderna has taken this omnichannel approach using these very tailored messages. And I know your CCO talked about this as a priority. Could you talk a little bit more about that omnichannel approach across different digital channels is an effective way of reaching, I mean an audience of potential patients, I suppose is the way of putting it.Kate Cronin: (09:10)We're in a world where the channels where people get their information has exploded. 10 years ago we didn't have Instagram, TikTok, pharma traditionally would use DTC advertising. And so we look specifically at our audiences and where they are, where they get their information, including inline broadcast tv. I mean a lot of older people watch, you know, shows like CBS this morning. If you think about when I find myself watching CBS this morning, I've sort of realized I'm I'm, I've hit a certain age, but uh, , we also do media buys based on where we have sponsorships. So we did a whole thing with series of ads with ESPM where we did a changemaker campaign where we celebrated changemakers. So last year we celebrated Billie Jean King. This year we did creative that celebrated Arthur Ashe. Again, Moderna is really associating ourselves with change makers because we believe we are change makers in the healthcare industry, which is part of our platform.Ilyse Lieffring: (10:33)Now Moderna has to work directly with healthcare professionals. Would you say the same channels to reach consumers are also just as beneficial perhaps in reaching those healthcare professionals?Kate Cronin: (10:44)Healthcare professionals, they get a lot of their information through journals and studies. And I like to say the number one journal that physicians read is the Wall Street Journal. So we really push out our data and our clinical trial information through earned media and also specific to physicians. There are medical meetings and associations where they get their information, but we've been incredibly transparent as a company. When we get new data out, we issue a press release, we call top journalists, let them know. We offer up our executives and our doctors as spokespeople to share the information and spend more time with the journalists. This has been like this through the entire pandemic. And I think that's one thing about Moderna is we are incredibly open and transparent with everything in our data. And so I think earned media is the fastest way to get information out. Once you issue a press release, you're able to talk to the media directly and then folks trust the media and they're gonna give an unbiased objective story.Ilyse Lieffring: (11:40)Now I know during covid, like social media was full of disinformation regarding Covid and it was a challenge for any company. Do you feel that disinformation is still a big challenge?Kate Cronin: (11:52)Challenge As a company, we need to focus on what we do well, which is deliver the science and deliver the information and education around the science. We've struggled with disinformation about Moderna and about our executives and we look at that through the lens of what can we change and what should we change and what do we just need to leave alone because we're never gonna be able to change. We then focus on education. We also did some things with kind of debunking myths. We actually hired somebody to do some work with our employees on pre debunking myths versus facts and getting our ed, our employees educated on, here's some things you're gonna be hearing about on the internet or from friends and family and here are a set of facts that you can use to debunk some of the things that you might be hearing so that our employees are better armed with information and can be good ambassadors for the brand.Damian Fowler: (12:43)Now I guess, you know, other biotech companies must also be kind of lockstep with you in the sense of that myth or debunking or myth busting. But at the same time I'm wondering, you know, about the space and your sense of place in it. How do you think about the space?Kate Cronin: (12:58)So we are very different from a pharmaceutical company in that we are a platform technology company. So when you look at competitors, we don't really see pharma as competitors. We see ourselves as a unicorn because we are a platform technology company. An analogy would be Apple with the iOS system and Apple's iOS system is basically everything runs off of iOS and when you have a new app, it's off of the iOS. So it's updates to the iOS. We look at our mRNA platform the same way we have mRNA platform technology and whenever we have a new product, new vaccine, new therapy, it's basically like an updated app to the iOS system. So we take that iOS, we recode for a new product using mRNA, and then we tweak it based on the needs. So for example, we have a flu vaccine that we're studying. We had to tweak it because we're working off of one system and we're basically recoding the product. We were able to update it very quickly and study a newer version of it. And then that's the one that we're gonna be continuing to study and hopefully, you know, launch, it's very unusual 'cause when a pharma company has a product, it's a new chemical entity, they call it an NCE, they test it. If it fails, it's done, it's over, they have to start over again. We don't start over again. We can tweak and modify very quickly. So we have agility in a platform.Damian Fowler: (14:20)Yeah, that's a fascinating analogy. And at the same time, you know, you're creating these new products and you obviously have to give them brand names too. I know that the Covid vaccine isn't, isn't now Spike vax, right? For instance.Kate Cronin: (14:32)So it's an interesting conundrum. As I said earlier, the pandemic made Moderna a household name. So now we're out of the pandemic and we're an endemic market. An endemic market, you have to have a brand name, I wanted to name it Moderna COVID-19 vaccine. And they don't let you do that. They won't let you do that, you have to call it something. So the team came up with Spike Vax, but we couldn't just call it Spike Vax because no one's gonna know what Spike Vax is. So we call it Spike Vax by Moderna. If you think about consumer branding overall, there's this idea of a branded house or you have a house of brands. We are a combination of a hybrid of a branded house and a house of brands. So hybridizing it is making sure the company name is included because everyone knows our name and also attaching it to a product name. And then there are other ways to connect the dots in terms of the brand. So we have little M that shows up. And then just in terms of the architecture, the visual architecture works when you have different products that you're launching. 'cause we want people to know when they see it, they go, oh, that's a Moderna product. So Bio Moderna is very intentional in terms of how we brandIlyse Lieffring: (10:02)Can you tell us a little bit about what the Spike Vax by Moderna campaign looks like?Kate Cronin: (10:07)Spike Vax by Moderna is our branded campaign and it's about all the things you do with your body. You know, you exercise that body, you provide nutrients to that body, you spike vax, that body. And so just normalizing what you do with your body. It's just like you wanna keep your body healthy and you wanna live your life and do all the things you do with that body. Spike vax, that body. We also say, you know, you flu shot that body spike vax, that bodyIlyse Lieffring: (15:36)Now. What has aligned with kind of the rise of healthcare data and general consumer interest in their healthcare has also been the rise of retail media and data being that Moderna is going direct to consumer. How important is retail data to its vaccine marketing?Kate Cronin: (15:54)We look at data and analytics, that's something that drives everything that we do. If you think about Covid vaccines, we're able to look on a weekly basis and see how many people are getting shots in arms, where are they getting the shots? All the way from when it goes to the wholesaler to when it's shipped to the retail pharmacies. We have a good sense of the penetration of the market in terms of vaccination and we can see regionally attitudes as well. You know, where are people inclined to get a vaccine, where are they on the fence? And so we're able to target our efforts and educational efforts in markets where we think we can move the needle and where we can get people to understand the importance of going and getting their vaccine. We work closely also with the retailers, the CVSs and the Walgreens of the world who are working with us to educate and use their channels as well to make sure that when people are in the store, they are hearing information about staying up to date with their COVID-19 vaccine. So super important to have that information. It's also important to know where are their outbreaks of covid, how many outbreaks there are, what are the hospital, what are the weekly hospitalization rates? Because Covid continues to be quite serious. People are still getting sick unfortunately. And so we're monitoring that as well. And that informs a lot of our marketing efforts also.Damian Fowler: (17:10)Do you think there's still more work to be done in the sort of educating the consumer about the virus? I mean, I've heard people say, well I've had Covid or I've had my covid shot, so I therefore I have antibodies, I don't need to get a shot. I mean, how, how do you think about these different cohorts?Kate Cronin: (17:26)There's the people who, who are gonna go and get vaccinated. There're the folks who are hearing misinformation and saying, you know, I'm just not sure should I, I've already had COID. So those are the people we really need to lean in on because we need them to understand. We just need to do our part and educate people on that. And then there's the, the vaccine fatiguers. That's a new category. This is a new category based on covid. I mean, vaccine fatigue is not anti-Vax vaccine fatigue is someone who did the right thing. They went every time they got all their shots and they're tired of getting shots. They're tired of being told they need to get another vaccine. And those are folks who are, you know, they wanna do the right thing, but they're just, they're tired of it. The market has changed. It's a one shot, not a booster system anymore.Kate Cronin: (18:10)You get one shot just like flu and then you go back the next year. Them understanding that is important too. 'cause then they know that the vaccination plan is normalized. We've been focused on this idea of stay up to date, stay vaccinated. Our campaign is called No Time for 19, the Unbranded campaign. Get your flu shot, get your covid shot, stay healthy. And that's the focus. So we don't even mention spike vax and that's intentional because we are all about market growth in terms of get people vaccinated and a rising tide floats all boats. That's very different from a very branded campaign where you're leaning in on product attributes versus your competitors. In this particular case, we're more about getting people into the pharmacy and getting their vaccine. So that's our focus.Damian Fowler: (19:05)And that's it for this edition of the current podcast. We have the honor of speaking once again with Sheila Ames, a registered nurse in Northern California who has been diagnosed with a rare type of a primary immunodeficiency known as common variable immunodeficiency or CVID for short.  At the time of her diagnosis she was working as an ICU nurse and her first doctors order was: no more exposure to infectious patients.  This diagnosis not only changed her career dramatically, it led her to following her life's purpose in opening her own health & wellness online coaching business to help others continue to find and work towards their life's purpose despite the hurdles that life gives us. ​Common variable immunodeficiency (CVID) is a primary immune deficiency disease characterized by low levels of protective antibodies and an increased risk of infections. Although the disease usually is diagnosed in adults, it also can occur in children. CVID also is known as hypogammaglobulinemia, adult-onset agammaglobulinemia, late-onset hypogammaglobulinemia, and acquired agammaglobulinemia. NIAID supports research to determine genetic causes of CVID that may lead to therapeutic approaches to address the disease. Researchers also are exploring how antibody-based drugs may lessen the severity of the condition. ​Causes CVID is caused by a variety of different genetic abnormalities that result in a defect in the capability of immune cells to produce normal amounts of all types of antibodies. Only a few of these defects have been identified, and the cause of most cases of CVID is unknown. Many people with CVID carry a DNA variation called a polymorphism in a gene known as TACI. However, while this genetic abnormality confers increased risk of developing CVID, it alone is not capable of causing CVID. CVID is also linked to IgA deficiency, a related condition in which only the level of the antibody immunoglobulin A (IgA) is low, while levels of other antibody types are usually normal or near normal. IgA deficiency typically occurs alone, but in some cases it may precede the development of CVID or occur in family members of CVID patients. ​Symptoms & Diagnosis People with CVID may experience frequent bacterial and viral infections of the upper airway, sinuses, and lungs. Acute lung infections can cause pneumonia, and long-term lung infections may cause a chronic form of bronchitis known as bronchiectasis, which is characterized by thickened airway walls colonized by bacteria.   People with CVID also may have diarrhea, problems absorbing food nutrients, reduced liver function, and impaired blood flow to the liver. Autoimmune problems that cause reduced levels of blood cells or platelets also may occur. People with CVID may develop an enlarged spleen and swollen glands or lymph nodes, as well as painful swollen joints in the knee, ankle, elbow, or wrist. In addition, people with CVID may have an increased risk of developing some cancers. Doctors can diagnose CVID by weighing factors including infection history, digestive symptoms, lab tests showing very low immunoglobulin levels, and low antibody responses to immunization. ​Treatment CVID is treated with intravenous immunoglobulin infusions or subcutaneous (under the skin) immunoglobulin injection to partially restore immunoglobulin levels. The immunoglobulin given by either method provides antibodies from the blood of healthy donors. The frequent bacterial infections experienced by people with CVID are treated with antibiotics. Other problems caused by CVID may require additional, tailored treatments. Today, we have with us today, Sheila Ames a registered nurse in Northern California who has been diagnosed with a rare type of a primary immunodeficiency known as common variable immunodeficiency or CVID for short.  At the time of her diagnosis she was working as an ICU nurse and her first doctors order was: no more exposure to infectious patients.  This diagnosis not only changed her career dramatically, it led her to following her life's purpose in opening her own health & wellness online coaching business to help others continue to find and work towards their life's purpose despite the hurdles that life gives us. ​Common variable immunodeficiency (CVID) is a primary immune deficiency disease characterized by low levels of protective antibodies and an increased risk of infections. Although the disease usually is diagnosed in adults, it also can occur in children. CVID also is known as hypogammaglobulinemia, adult-onset agammaglobulinemia, late-onset hypogammaglobulinemia, and acquired agammaglobulinemia. NIAID supports research to determine genetic causes of CVID that may lead to therapeutic approaches to address the disease. Researchers also are exploring how antibody-based drugs may lessen the severity of the condition. Causes CVID is caused by a variety of different genetic abnormalities that result in a defect in the capability of immune cells to produce normal amounts of all types of antibodies. Only a few of these defects have been identified, and the cause of most cases of CVID is unknown. Many people with CVID carry a DNA variation called a polymorphism in a gene known as TACI. However, while this genetic abnormality confers increased risk of developing CVID, it alone is not capable of causing CVID. CVID is also linked to IgA deficiency, a related condition in which only the level of the antibody immunoglobulin A (IgA) is low, while levels of other antibody types are usually normal or near normal. IgA deficiency typically occurs alone, but in some cases it may precede the development of CVID or occur in family members of CVID patients. Symptoms & Diagnosis People with CVID may experience frequent bacterial and viral infections of the upper airway, sinuses, and lungs. Acute lung infections can cause pneumonia, and long-term lung infections may cause a chronic form of bronchitis known as bronchiectasis, which is characterized by thickened airway walls colonized by bacteria.   People with CVID also may have diarrhea, problems absorbing food nutrients, reduced liver function, and impaired blood flow to the liver. Autoimmune problems that cause reduced levels of blood cells or platelets also may occur. People with CVID may develop an enlarged spleen and swollen glands or lymph nodes, as well as painful swollen joints in the knee, ankle, elbow, or wrist. In addition, people with CVID may have an increased risk of developing some cancers. Doctors can diagnose CVID by weighing factors including infection history, digestive symptoms, lab tests showing very low immunoglobulin levels, and low antibody responses to immunization. Treatment CVID is treated with intravenous immunoglobulin infusions or subcutaneous (under the skin) immunoglobulin injection to partially restore immunoglobulin levels. The immunoglobulin given by either method provides antibodies from the blood of healthy donors. The frequent bacterial infections experienced by people with CVID are treated with antibiotics. Other problems caused by CVID may require additional, tailored treatments. Crise sanitária, económica e de segurança, o jornal detalha um "caminho difícil" percorrido em doze meses, num ano atípico para o Presidente francês. No site online do La Croix, o diário católico destaca a tradicional mensagem do Papa Francisco que insistiu na "necessidade de fraternidade" em todos os continentes, em tempos de pandemia provocada pela Cvid-19, o sumo pontífice exortou para que haja vacinas para todos. And that calls for leaders who can address the emotional toll that the isolation is taking on their employees' physical, mental and financial well-being, says Howard Ross, a lifelong social justice advocate and a principle in Bethesda, Maryland-based Udarta Consulting. Mr. Ross is considered one of the world's thought leaders on identifying and addressing unconscious bias. “People see diversity as this sort of ancillary thing that we do when we have the time for it, but if you look that impact of COVID on especially the African-American community and Native American community, we that this COVID experience is bringing to light some of the issues of systemic racism in our culture,” Mr. Ross says. “If anything, more than ever we need to incorporate these conversations into the things we are doing as we try to create a sense of belonging at a time of separation,” he says. Even as economic pressures may be forcing leaders to consider cutting payroll, Mr. Ross says companies need leaders who show genuine compassion to create a sort of “we're in this together” sense. “The kind of situation we're in now creates enormous stress,” he says. “Any sense of difference we have gets multiplied.” Howard Ross, who has spent more than 30 years helping managers become leaders in their organizations offers specific ideas on how companies can create a better sense of teamwork in the times of CVID in this CVBT Audio Interview Podcast. Powered by iReportSource All of the discussions about surgical masks got me thinking; what do we know about viruses (CVID-19 or others), and what are the EXPERTS saying? I wanted to explore this topic and share my thoughts. As of April 1, 2020, the WHO, CDC, OSHA guidance on wearing masks (if you are HEALTHY) is the same; you do NOT need to wear a mask except for specific circumstances. Let's read what they have to say. World Health Organization On the Questions and Answers page: Should I wear a mask to protect myself? Only wear a mask if you are ill with COVID-19 symptoms (especially coughing) or looking after someone who may have COVID-19. A disposable face mask can only be used once. If you are not ill or looking after someone who is sick, then you are wasting a mask. There is a world-wide shortage of masks, so WHO urges people to use masks wisely. WHO advises the rational use of medical masks to avoid unnecessary wastage of precious resources and misuse of masks  (see Advice on the use of masks). The most effective ways to protect yourself and others against COVID-19 are to frequently clean your hands, cover your cough with the bend of elbow or tissue and maintain a distance of at least 1 meter (3 feet) from people who are coughing or sneezing (the rest of us use 6 feet). See basic protective measures against the new coronavirus for more information. CDC What does the CDC say about wearing asks? Wear a facemask if you are sick If you are sick: You should wear a facemask when you are around other people (e.g., sharing a room or vehicle) and before you enter a healthcare provider’s office. If you are not able to wear a facemask (for example, because it causes trouble breathing), then you should do your best to cover your coughs and sneezes, and people who are caring for you should wear a facemask if they enter your room. Learn what to do if you are sick. If you are NOT sick: You do not need to wear a facemask unless you are caring for someone who is sick (and they are not able to wear a facemask). Facemasks may be in short supply, and they should be saved for caregivers.   OSHA Surgical masks are used as a physical barrier to protect the user from hazards, such as splashes of large droplets of blood or body fluids. Surgical masks also protect other people against infection from the person wearing the surgical mask. Such masks trap large particles of body fluids that may contain bacteria or viruses expelled by the wearer. Surgical masks are used for several different purposes, including the following: They are placed on sick people to limit the spread of infectious respiratory secretions to others. They are worn by healthcare providers to prevent accidental contamination of patients' wounds by the organisms normally present in mucus and saliva. Worn by workers to protect themselves from splashes or sprays of blood or bodily fluids, they may also keep contaminated fingers/hands away from the mouth and nose. Surgical masks are not designed or certified to prevent the inhalation of small airborne contaminants. These particles are not visible to the naked eye but may still be capable of causing infection. Surgical masks are not designed to seal tightly against the user's face. During inhalation, much of the potentially contaminated air can pass through gaps between the face and the surgical mask and not be pulled through the filter material of the mask. Their ability to filter small particles varies significantly based upon the type of material used to make the surgical mask, so they cannot be relied upon to protect workers against airborne infectious agents. Only surgical masks that are cleared by the U.S. Food and Drug Administration to be legally marketed in the United States have been tested for their ability to resist blood and body fluids. She is obese and has OSA. She is on flonase. She had a low grade fever. Exam doesn’t reveal much besides a serous OM and some mild anterior cervical lymphadenopathy. Conservative care was advised, as well as follow-up in the next couple weeks if not improving. Dr. Peiterson now will tell us the chain of events in this peculiar case. Joining Dr. John Peitersen in the discussion today are: Dr. Barrett Larson from Ridgeview pulmonary medicine, Dr. James Currie, Lakeview Clinic infectious disease, Dr. Matthew Herold, Ridgeview emergency medicine, Dr. David Gross, Radiologist with Consulting Radiologists, Ltd, Dr. Susan Bowers, Pathology, Dr. Kevin White, Ridgeview hospitalist, and various others from the provider and allied health audience.      The initial small segment of this discussion had recording difficulty, so our conversation picks up immediately after the initial presentation of the patient.   CHAPTER 1 REVIEW:      So... let’s recap up to this point. So far we have heard input from Dr. Peiterson, Dr. Larson the pulmonologist, Dr. Gross the radiologist and Dr. Bowers the pathologist. So, initially she was seen for what sounds like a viral URI, and was told to f/u if not improving. Well, we all see this kind of case every day, right? She was then treated by phone with Azithromycin; seen by different providers; Reports “crackling in the lungs’, malaise and subjective fever. She has a Son who had strep 9-days ago. Ears look better today. Cryptic tonsils. VSS. Negative strep test. This was felt to be Viral bronchitis.  CXR offered, patient declined due to $.      Five months later, the patient sees a sleep doctor. Continued cough noted. Pulmonary function tests are likely now indicated. Is there mild asthma? PFTs are able to give us a lot of information. Is the FEV1-FVC ratio acceptable.  Yes, it’s above 80 -  in her case. Chance of asthma markedly low. However the diffusion capacity is low at 83. For some reason, she is not absorbing O2. Nothing really going on with her expiratory loop, or any other major issues with this test. Is the patient’s obesity contributing to her poor lung perfusion? Interestingly, her weight has decreased by 15 lbs since her last visit.       Pulmonary physician recommended a CXR, a 4 week post nasal drip protocol. Additionally is a metacholine challenge needed here? Often a pre- and post-neb peak flow will first be done first. Then the metacholine challenge is done if the clinical picture fits. Is it time to rule-in or out asthma and spare someone years of MDI use. Diffusion capacity should be normal in asthma.       Dr. Peitersen reflects on an often asked board question. When to get a chest xray for the complaint of persistent cough. Barring other obvious reasons such as new chest pain, high fever/shaking chills or focal exam findings, The American College of Chest Physicians recommends that if a cough is present for greater than 8 weeks, a CXR is indicated. This patient’s CXR reveals interstitial changes that bring up a broad list of possibilities on the differential. These include CHF, infection, autoimmune disease.      Chest CT non-contrast was now ordered and shows reticulonodular areas and some regions of consolidation that are almost mass like. Other patchy areas noted throughout. No endobronchial findings. Lymphadenopathy is also noted in various areas of the intra- and extra-thoracic regions. CT with contrast is important to see vascular issues, but also to see small hilar lymph nodes. Sometimes contrast can falsely increase the density of a nodule leading you to call it a granuloma. Hi Resolution chest CT is an older term, but current modern CT scans accomplish this . This involves 1 mm cuts vs. 3 mm cuts. Essentially thinner cuts to see nodules better.      The patient is now seeing a new pulmonologist and has normal vital signs, unremarkable lung exam, which is not totally unusual despite a very abnormal looking xray or CT. A PET CT scan is advised and will show hypermetabolic lesions. Essentially it will help find other areas of concerning activity that would be less risky to biopsy. Radiologist generally avoid biopsy of central lesions that are near important organs and structures. Insurance declines the PET CT, but a node was biopsied in the thigh. Dr. Bowers comments that this biopsy could be a low grade lymphoma, although at this point it would need further assessment, but this is a send-out, looking for B and T cell rearrangement. A hematopathologist would also be good to consult with in this case. For now, this is benign specimen.       Another lymph node specimen was obtained, now axillary. This one shows really no other concerning findings. Tiny granulomas are noted. A variety of staining procedures were performed and all were negative. For Dr. Bowers, Toxoplasmosis may need to be considered.   CHAPTER 2:      Toxoplasmosis seems unlikely because this patient is apparently not immunocompromised. The differential dx does include various other infectious etiologies, such as bartonella, brucellosis and Q-fever. Melioidosis as well. Therefore, a travel history such as to SE Asia should be obtained. So, what now? There are about 20 possible infectious etiologies for this presentation...we need to do more tests. But, the patient was lost to follup for some time.       Now it is 16-months later, and she returns to urgent care with cough, fever, increased respiratory rate, O2 sats are marginal and an abnormal lung exam. Mild leukocytosis noted, and anemia which is new. Dr. White interjects with the following questions: 1. Has she ever been treated with a steroid? 2. Did anyone perform laryngoscopy? In the setting of normal chest imaging, these things should be considered. But of course, since her last CT scan was abnormal, a pulmonary etiology is of highest concern. And indeed a repeat CXR shows worsening overall interstitial change along with increase in the density of the azygoesophageal fissure which was noted on previous CT. The UC provider feels this looks like pneumonia. She was treated for pneumonia and a potpourri of other remedies were tried. Unfortunately, she did not follow-up with her medical doctor. She did see her naturopathologist who resumed drops for bartonella and Lyme disease. As Dr. Currie said, though, Lyme Disease does not present with granulomatous lymph lesions.       She now presents to the Emergency department 18 months after the UC visit. She is SOB, coughing, and states she has “chronic lyme disease”. She is 85% on RA. She has SIRS. Leukocytosis, and a respiratory alkalosis is noted. Her CXR shows Left upper lobe infiltrate that is quite dense. This must be followed to ensure resolution. Lactate and influenza were normal. The commentary from Dr. Herold in the audience was that this patient is not quite meeting sepsis criteria, but quite ill all the same. The decision to initiate broad spectrum antibiotics was made. Further history demonstrates that she was diagnosed with Lyme disease at age 10 and has struggled with health issues ever since. The patient had ongoing frustrations about cost of care and so she continued to see her naturopathologist.       Regarding another good exchange between Dr. Gross and Dr. Herold, involved the discussion of using CT to differentiate this very abnormal CXR for infiltrate vs. empyema. Ultrasound can also be employed for thoracentesis if indeed it is empyema.       Dr. Currie also makes the point that "chronic lyme disease" is not a known condition, so that when patients present with this issue or concern, other underlying disease states must be considered.      While CAP is the leading dx, other considerations in the differential still exist. Dr. Curry also states that azithromycin/Ceftriaxone is a reasonable inpatient treatment regimen going forward. She is feeling better on hospital day 2, but her blood cx come back positive in all 4-bottles. Strep pneumonia is the culprit, and is the current, but certainly not chronic reason for her symptoms. TTE was recommended to rule out endocarditis, especially given her chronic issues. Echo showed high right sided pressures, and a CT PE study was done showing no PE. Dr. Gross discusses the CT reading and notes bilateral signifcant hilar and subcarinal lymphadenopathy. Dense alveolar consolidation around the bronchi and layering left sided pleural effusion. Also noted is a large spleen and some prominent retroperitoneal nodes. Hospital day 3 she has left sided chest pain and had an unchanged repeat chest CT.       Dr. Bowers, the pathologist, discussed the blood cell differential and comments that she is anemic and that is the primary issue. All other counts are normal. Mild rouleaux (stacking of cells) is noted on the morphology and prompts you to think about increased proteins, such as monoclonal and fibrinogen. On hospital day 3, the patient was to go home on levaquin. She is supposed to f/u with pulmonary, but then develops another fever and requires O2 once again. Fever after 40-hours of antibiotics is not entirely unexpected in this patient, especially due to her past history and the likelihood of some underlying etiology that has yet to be discovered.   CHAPTER 3:      Okay, so her immunoglobulins are low. What does that mean? Well, this looks like Chronic Variable Immunodefincy disorder. Does she need IVIG? Yes, it is worth a try per the immunologist. Especially since she is having fevers, rigors and need for increased oxygen. Repeat CXR shows some mild improvement in infiltrate, but a bit more of a CHF pattern, perhaps. ID is involved now and they feel that CVID made sense as a diagnosis. Her symptoms improved and no further IVIG is given. In terms of follow-up, the patient has done quite well. No further hospitalizations to date. There were some barriers in her care involving cost and insurance issues. A repeat CT in 2018 was reviewed by Dr. Gross and she still has some reticulonodular infiltrates. No further dense consolidation in the lung. Lymphadenopathy has improved in general. And the spleen is still enlarged. The patient apparently then was referred to another facility and had another node biopsy after she had yet another scan that showed once again some worsenening. IVIG is helpful for these patients and unfortunately is also very expensive. Many of these patients succomb to cancers of various types, as opposed to infection as they once did many years ago.       According to UpToDate, Common variable immunodeficiency is the most common form of severe antibody deficiency in adults and kids. It is somewhat complex, but in general is due to severe antibody deficiency due to impaired B cell differentiation with defective immunoglobulin production. Recurrent infections, chronic lung disease, GI disease and increased susceptibility to lymphoma are common. Besides having very low IgG, IgA and IgM levels, there is also a poor or absent response to vaccinations.      Feel free to comb through the literature on this one, and while it is not ultra common, it is not unreasonable to consider this in your patients who just can’t seem to avoid getting sick on a regular basis, or who happen to have significantly waned immunity to pathogens they were once immunized for.   Thanks to Dr. Peiterson for bringing this baffling diagnosis to our attention, and to everyone else involved in presenting this case.

Back in December 2017, most people would have found it hard to believe that North Korea would be participating in the PyeongChang Winter Olympics, let alone inviting President Moon Jae-in to Pyongyang at "an early date" or expressing their openness to engage in dialogue with the U.S. But in an interview with NK News last year, Dr. Georgy D. Toloraya, a Professor of Oriental Studies at Moscow University of International Relations University and director of the Center of Asian strategy at the Russian Academy of Sciences, said that North Korea's "completion of a state nuclear force" offered a window of opportunity for dialogue. "I believe that this is the perfect chance to start a peace-building process and if it would start from the Olympics, then that would still be highly symbolic," he told NK News. Fast forward to the end of the Olympics, with inter-Korean relations significantly improved, and even the possibility of a North Korea-U.S. dialogue on the table. Toloraya says the North would never succumb to CVID, the complete verifiable irreversible dismantlement, as they hold the nuclear program sacred. What, then, can the U.S. and North Korea talk about, and how are things really progressing in terms of inter-Korean and U.S.-DPRK dialogue? In this episode of the "North Korea News Podcast," host Jacco Zwetsloot sat down with Dr. Toloraya to discuss how the Korean Peninsula is faring post-Olympics, and how Russia (and the Soviet Union) managed to balance relations with both Koreas over the years. They also delved into Dr. Toloraya's extensive experience working in Pyongyang in the 1970s and 80s (when he also briefly met our first podcast guest Andrei Lankov), in contrast to working in Seoul as the Minister-Counselor and Deputy Chief of Mission at the Russian Embassy in the 90s, when Kim Il Sung passed away leaving Kim Jong Un in power, high-ranking North Korean official Hwang Jang Yop defected to South Korea, and two North Korean submarines were caught in South Korean waters. Der variable Immundefekt oder common variable immunodeficiency syndrome (CVID) ist ein primärer Immundefekt mit einer Häufigkeit von ca. 1:25.000 - 50.000 in der westlichen Bevölkerung und ist somit der häufigste symptomatische primäre Immundefekt. CVID wird nach internationalem Konsens definiert als ein signifikanter Mangel an IgG, sowie mindestens einer weiteren Immunglobulinklasse (IgA oder IgM), verbunden mit einer defizienten Bildung spezifischer Antikörper nach Antigenexposition. Das klinische Krankheitsbild ist dominiert von wiederkehrenden bakteriellen Infektionen, Autoimmunerkrankungen, lymphoproliferativen Erkrankungen und Neoplasien. Ziel dieser Studie war die Überprüfung der Diagnosestellung, Einteilung der Patienten in verschiedene Gruppen (primäre CVID-Patienten: early-onset und late-onset, sekundäre CVID- und Agammaglubulinämie-Patientinnen) und der Vergleich dieser Gruppen in Bezug auf klinische Manifestationen, Familienanamnese, Laborparameter und Therapien.
