Upper Limb Therapy for Infants and Young Children with Unilateral Cerebral Palsy: A Clinical FrameworkSusan Greaves, Brian HoarePMID: 39598017 PMCID: PMC11594546 DOI: 10.3390/jcm13226873AbstractEarly detection and rehabilitation interventions are essential to optimise motor function in infants and young children with unilateral cerebral palsy. In this paper we report a clinical framework aimed at enhancing upper limb therapy for infants and young children with unilateral cerebral palsy during a sensitive period of brain development. We describe two major therapeutic approaches based on motor learning principles and evidence: constraint-induced movement therapy and bimanual therapy. These two therapies have demonstrated efficacy in older children and emerging evidence is available for their application to infants younger than 2 years of age. To provide clinicians with guidance as to when to implement these therapies, we discuss the key consideration when undertaking upper limb therapy programs. In addition, we describe the factors to consider when choosing which approach may be suitable for an individual child and family. Detailed strategies for implementing these therapies in infants and young children of different ability levels are given. Keywords: bimanual therapy; constraint-induced movement therapy; early intervention; unilateral cerebral palsy; upper limb.https://pubmed.ncbi.nlm.nih.gov/39598017/

Part I: Understanding the Serious Consequences of Criminal Charges for Non-Citizens Life is unpredictable. Over the years, our Immigration attorneys have stepped in to help F-1 international students, young professionals, and non-immigrant visa holders (such as H-1B or L-1 holders) who, in a moment of poor judgment, found themselves facing a criminal charge. Depending on the specific offense, it could be classified as a crime involving moral turpitude (CIMT) or even an aggravated felony under U.S. immigration law. These are very technical legal terms with serious implications. Our firm collaborates closely with criminal defense counsel to mitigate potential immigration consequences of charges like DUIs, reckless driving, domestic disputes, larceny/theft, or drug possession. Without coordinated efforts between an experienced immigration lawyer and a criminal lawyer, non-citizens risk deportation or inadmissibility—and even a denied U.S. citizenship application can lead to removal proceedings. When a U.S. citizen faces criminal trouble, the focus is typically on reducing or avoiding jail time. However, for a permanent resident or non-immigrant, a criminal conviction can trigger immigration penalties, including removal from the United States. In the landmark decision Kentucky v. Padilla, the Supreme Court recognized that non-citizens face a “major life-altering” risk of deportation for certain criminal convictions. The Court instructed criminal lawyers to refer their non-citizen clients to immigration attorneys, underscoring the complexity and gravity of these issues for foreign nationals. Part II: Crimes Involving Moral Turpitude—Key Points and Legal Analysis Below is an overview of Crimes Involving Moral Turpitude (CIMTs) and the related legal considerations for non-citizens facing criminal charges. Our strategy is to first determine whether the crime is a CIMT and then assess any aggravated felony issues. Early identification of these factors can significantly impact defense tactics and immigration outcomes. Definition and Significance of CIMTs A Crime Involving Moral Turpitude is generally defined as conduct that is inherently base, vile, or depraved, and contrary to accepted moral standards. Under federal regulations, classification as a CIMT often hinges on intent and willfulness—in other words, whether the action demonstrates moral depravity or a fraudulent intent. If a non-citizen commits a CIMT within five years of obtaining lawful permanent resident status (or if multiple CIMTs occur at any time), they could face increased scrutiny or direct placement in removal proceedings. Examples of Offenses that May Constitute a CIMT Theft and Larceny: In many cases, crimes involving theft or larceny are deemed CIMTs, as they involve intentional taking of property with the intent to permanently deprive. Burglary: Depending on the jurisdiction and the underlying facts, burglary—especially when it involves unlawfully entering a property with intent to commit a crime—may be classified as a CIMT. Fraud and Money Laundering: Crimes that center on deceit, misrepresentation, or an intent to commit financial harm typically fall under CIMT because of the element of fraud. False Statements: Lying or using deception (for instance, to obtain a benefit) can be seen as morally turpitudinous. Falsely Representing a Social Security Number: Misuse of Social Security numbers, especially with fraudulent intent, is also frequently categorized as a CIMT. Why Classification Matters A CIMT can lead to inadmissibility or deportability, placing non-citizens' ability to remain in or return to the United States at risk. Even if jail time is avoided, the immigration consequences—such as denial of admission at a port of entry, revocation of a visa, or inability to adjust status—can be dire. An aggravated felony classification carries even harsher immigration penalties, including near-certain removal with limited relief options. Immediate Steps to Take Seek Qualified Representation: Non-citizens facing criminal charges should promptly consult both a criminal lawyer and an immigration attorney to ensure a coordinated defense strategy. Assess Potential Immigration Consequences: Work with immigration counsel to understand if the specific charge might be considered a CIMT or aggravated felony under immigration law. Explore Mitigating Options: Depending on the jurisdiction, certain plea arrangements or charge reductions can help reduce or eliminate the immigration risks. Maintain Documentation: Keep detailed records of all communications, court dates, and documents related to the criminal charge, as these will be crucial in assessing the potential immigration impacts. Concluding Thoughts Facing a criminal charge in the United States is serious for anyone, but for a non-citizen, the repercussions can be life-altering. Even a relatively minor offense could trigger deportation or inadmissibility. Since Crimes Involving Moral Turpitude and aggravated felonies carry severe immigration penalties, it is critical to involve an experienced immigration lawyer as soon as possible. Our immigration attorneys have extensive experience guiding international students, foreign professionals, and non-immigrant visa holders through these highly technical areas of the law. We work closely with criminal defense counsel to safeguard your future in the United States and help you make informed decisions about your legal options. If you or someone you know is a non-citizen charged with a crime—no matter how minor it may seem—please reach out to us immediately. Early intervention can make a profound difference in protecting your legal status and your long-term goals in the United States.

June kicks off with important health lessons brought to you by Joel Kahn, MD. How about the relationship between higher testosterone levels in men and a lower risk of type 2 diabetes? A healthier CIMT ultrasound also predicts a lower risk of diabetes, as performed at the Kahn Center. Sauna bathing and better mental health is an interesting research report. The main topic is data that women live longer if they adhere to the Mediterranean Diet. Another paper on the main topic reports that obese type 2 diabetics have less kidney damage with the MED diet enriched with olive oil. Finally, Dr. Kahn discusses the version of this diet taught by Valter Longo, Ph.D., called the Longevity Diet. There is a wealth of important information this week.

Dr. Thomas Dayspring joins me again today for the third class in our Lipid Masterclass Series. Dr. Dayspring brings a wealth of expertise as a certified specialist in internal medicine and clinical lipidology, holding fellowships with the American College of Physicians and the National Lipid Association. This hour-long class is devoted to women's health and abnormal lipids. We continue with our conversation about ApoB, discussing ways to look at other lipid abnormalities related to abnormal ApoBs and talking about our ideal lab values and lifestyle modifications, briefly touching on my lipid journey and the medication I have been taking for the past six months. We examine diagnostics and their limitations, getting into CACs, coronary artery calcification scoring, CIMT, the labs we need to look at, the role of Lp(a), and how it affects our heart. We look into the changes women experience from losing estrogen in menopause, clues women can use to predict future cardiovascular risk, how estrogen helps clear LDL from the body, and the changes occurring with menopause, including higher ApoBs. We also dive into risks women face for cardiovascular disease, the impact of the Women's Health Initiative study, and absorption and synthesis challenges with cholesterol. Stay tuned for more! IN THIS EPISODE YOU WILL LEARN: Why ApoB testing for inherited lipoprotein disorders is essential Why hypo beta lipoproteins (HDL) are critical for maintaining good health The benefits of a holistic approach to treating high cholesterol The three stages of prevention for atherosclerosis How Lp(a) impacts heart health The importance of assessing early heart function through Lp(a) testing How your pregnancy history can help in predicting your risk for cardiovascular disease Why should women be more concerned about cardiovascular disease than breast cancer? How the Women's Health Initiative led gynecologists to be more cautious about prescribing HRT How genetically mediated cholesterol abnormalities can impact our ability to absorb and synthesize cholesterol Bio: Thomas Dayspring MD is a Fellow of both the American College of Physicians and the National Lipid Association and is certified in internal medicine and clinical lipidology. This week Dr. Kahn reviews some "mini" topics related to plant protein, whole grains and health, and hydration. The main topic is a new study in athletes of the intensity of exercise and the progression of coronary artery calcium scores and plaque. Surprisingly, the "very vigorous" group of athletes vs. those classified as only "vigorous" had more coronary artery calcium progression over 6 years of follow-up and repeat imaging. Wow. The words "prevent not stent" are the highest goal of care at the Kahn Center and can be confirmed by the "test not guess" philosophy of using CIMT and CT imaging for plaque detection. In many, plaque is already present and needs attempts at reversal. This week Dr. Kahn discusses two supplements he has used for years for disease reversal based on a rich medical science literature. He then describes two new approaches that are novel, natural, and new but supported by amazing science. Thanks to memoryhealth.com and the discount code Kahn15.

They talk about the importance of breathing through the nose in order to make enough nitric oxide for your body, and how one should breathe through their nose all night. “With the inflammatory panel, we're trying to determine if the body's immune system is doing its job correctly and if we're helping it.” —Dr. Gina PritchardReferences:Tests to ask your doctor for:MyeloperoxidaseLPPLA2CRP testADMA test
In this episode, Ask Why hosts Lora Hooper and Dr. Gina Pritchard discuss stigma surrounding food. They talk about the “drama” surrounding one's relationship with food, considering social, emotional, mental, and cultural factors.Lora and Dr. Gina discuss the fact that one diet does not fit everyone, and the importance of genetic testing as a means of understanding what an individual should be eating for optimal health. The two explain the difficulties that everyone faces when they've been told that they can no longer eat foods that they'd previously thought to be harmless. They also talk about the self-love that is involved in choosing what to eat, and understanding that it's okay to sometimes choose to eat outside of your recommended diet.*“You can get a customized approach that is the most ideal of the individual if you know the genetic makeup” - Gina Pritchard, DNP, RN, ACNP*References:APOE2 Test

But, trust me: no matter what population you work with, OTs should be following this research. Today, on the podcast we are looking at a Cochrane review of all the evidence that undergirds this treatment. CIMT is perhaps the most-studied OT intervention—and it has lessons to teach us all about: neuroplasticity, intensity, and the future of OT

View the Show Notes Page for This Episode Become a Member to Receive Exclusive Content Sign Up to Receive Peter's Weekly Newsletter In this “Ask Me Anything” (AMA) episode, Peter dives deep into the topic of atherosclerotic cardiovascular disease (ASCVD)—the number one killer in the developed world. Peter argues for the importance of paying attention to and understanding ASCVD given its ubiquity and inevitability. He goes into great detail about the development of atherosclerosis and how it can take hold at a very early age, the role of cholesterol, and the causal factors of ASCVD that determine prevention strategies. Additionally, he discusses the important metrics and biomarkers found in blood work, as well as diagnostic tests such as coronary artery calcium scores (CAC) and CT angiograms which help to determine the level of arterial damage present. Finally, Peter lays out the keys to understanding and interpreting calcium scores before wrapping up the conversation with his key takeaways regarding prevention.
