Medicare Advantage covers more than half of seniors, and the debate over what it really costs is stuck in two camps. Timothy Bulat, a senior consulting actuary who spent nearly two decades leading Medicare Advantage analytics inside a major insurer, raises a question the loudest voices keep avoiding: who actually pays for the program, and is the value being shared fairly? This episode is based on his article "The truth about Medicare Advantage funding and costs," published on KevinMD. You will hear how plans simultaneously manage costs well and extract excess taxpayer dollars through coding intensity and favorable selection, why headlines about Medicare Advantage being "in retreat" are overblown, and which specific policy levers (risk adjustment reform, benchmarking, slowing annual payment growth) the Biden and Trump administrations have already started using. Listen for the distinction Timothy draws between a funding cut and a smaller increase, and why that distinction matters for every clinician dealing with prior auth. Partner with me on the KevinMD platform. With over three million monthly readers and half a million social media followers, I give you direct access to the doctors and patients who matter most. Ian Wilks was a high-ranking leader of the Church of Jesus Christ of Latter-day Saints. He is a former bishop, a former branch president, and a member of the stake presidency in British Columbia, with responsibility for shepherding twelve congregations and thousands of members. Then-apostle Dallin H. Oaks, now the 18th president and living prophet of the LDS Church, personally recruited him, interviewed him, called him, set him apart, and trained him into that role.¹ For 37 years, Ian devoted his entire life to his church. He paid tithing on his gross income. He sealed his marriage in the London Temple. He conducted hundreds of worthiness interviews. He trained bishops. He stood at pulpits and at altars. He made sacred covenants in the temple to give all he had, including his own life if required, for the building up of the kingdom on earth. In a snowstorm, the cost of those 37 years lands all at once. The thing he wanted most to be true, he learned was untrue. I spoke with Ian on my podcast Cults, Culture & Coercion and in a recent livestream. He is the co-host of the Inside Out podcast with Jim Bennett, son of the late US Senator Bob Bennett. What he told me confirms what former members of the LDS Church have been describing to me since my first book was published in 1988. The Mormon Church meets every criterion of an authoritarian cult under my BITE Model of Authoritarian Control™ and sits on the destructive end of my Influence Continuum©. Ian himself took the BITE Model self-test based on his decades of experience and scored the church at roughly 85 to 90 percent across the four dimensions of behavior, information, thought, and emotion. This group is one of the wealthiest, most influential groups in politics and its believers are over represented (compared to all other faith groups) in our FBI, CIA, Homeland security and faithful people will follow the direction of the Prophet over the Constitution (even though they swear an oath to uphold the Constitution) which should make all Americans worry. What really happens inside the world's most dangerous underground economies? And why do the people running them trust a journalist enough to talk? Mariana van Zeller — Emmy- and Peabody Award–winning investigative journalist, host and executive producer of Trafficked on National Geographic, and host of The Hidden Third podcast — has spent years embedded with scammers, smugglers, cartel members, assassins, and black-market power brokers. In this episode of Mayim Bialik's Breakdown, she pulls back the curtain on the shadow systems that quietly shape the global economy. Mariana breaks down the difference between the black market vs. the grey market, why understanding these hidden economies is the only way to create real change, and how she manages to gain access to some of the most secretive and dangerous players in the world. She explains how empathy, not judgment, is often the key to getting people labeled “the worst of the worst” to open up, and why many of them speak with her because of something surprisingly simple: a deep human need to be heard and understood. She also addresses critics who say she shouldn't give criminals a platform, revealing why listening to these voices is essential to exposing the systems behind global crime. We dive into her most harrowing experiences in the field — including the terrifying moment she and her team weren't sure they would make it out alive, and the time they had to escape a country in the middle of a life-threatening military coup while filming. Mariana also unpacks the current climate in Mexico's drug war, explaining why violence often escalates when major cartel kingpins are taken down, and the troubling implications of the fact that many of the weapons used by cartels originate in the United States. We get into one of the fastest-growing criminal industries in the world: scams. She breaks down everything you've ever wanted to know about romance scams and “pig butchering” operations, including: - Psychological tactics scammers use - Who scammers typically target and why - Massive infrastructure behind modern scam compounds - Why many scammers are actually victims of forced labor themselves - Why victims often stay silent due to shame and stigma Mariana explains why advancing technology, crypto, and AI are making financial scams easier than ever, and why anyone can fall victim — no matter how smart or cautious they are, including the shocking case of a bank CEO who was successfully defrauded. We also explore medical black markets, from organ trafficking to international surrogacy scams, and discuss some of her most intense interviews, from conversations with a suspected Russian sex spy to Anna Delvey, the infamous socialite scammer. Despite spending her career face-to-face with some of the darkest corners of humanity, Mariana reveals something surprising: she still believes deeply in people. In fact, she sometimes finds more hope, honesty, and equality in underground markets than in global governments or legal capitalism. What if the roots of modern cults, extremist politics, and even today's “wellness” movements trace back to Nazi occultism, hypnotic manipulation, and one of the most controversial religions in the world? In this explosive episode of Mayim Bialik's Breakdown, Jon Atack — the world's leading expert on authoritarian cults and a former Scientologist who has spent over 40 years investigating its origins — reveals the shocking connections no one talks about. Drawing from his groundbreaking book If Scientology Ruled the World: Nazi Occultists, Sex Magick, Space Aliens, and the Second Coming, Jon uncovers how occult beliefs influenced the Nazi Party… and how those same ideas shaped Scientology. Jon Atack breaks down: - How L. Ron Hubbard repackaged occult practices (including hypnotic techniques, repetition, fixation, and mimicry) and sold them as revolutionary “science” - Outlandish promises that hooked followers: supernatural powers, immunity to illness, genius-level IQ, emotional mastery - How Hubbard's early interest in psychology morphed into the darker techniques behind Dianetics - Hidden factors in Hubbard's personal life (addiction, PTSD, and legal trouble for practicing medicine without a license) that shaped what Scientology would become - Allegations of how followers were treated, including psychological and physical abuse - Frightening recourse Jon himself faced for speaking out Jon also shares his deeply personal story of joining, and ultimately escaping, Scientology. He reveals the major red flags he ignored at first… and the first warning sign you should always look for before joining any movement or ideology. But this conversation goes far beyond Scientology. We also cover: - What actually defines a cult or authoritarian group - How recruitment tactics are now supercharged by the internet and social media - Where dangerous occult practices are still hiding in plain sight - Surprising benefits and dangers of hypnotherapy - How to distinguish modern spirituality from manipulative occult systems - Why today's political climate often mirrors cult dynamics - Why healthy skepticism might be the most important survival skill of the digital age This isn't about throwing away all ideology or spirituality. It's about learning how to extract the good without falling for manipulation. If you care about psychology, cults, spirituality, politics, authoritarian movements, mind control, or the hidden forces shaping modern society…this is a conversation you cannot afford to miss. After seven years of consistent publishing, over 500 episodes, and more than 150 unique guest interviews, Kehla shares the unfiltered truth about what it actually takes to build a top 2% podcast — without crossing 100,000 downloads or going viral. In this episode, she breaks down the myths around podcast growth, global rankings, and download metrics, and explains why longevity, coherence, and ecosystem integration matter more than spikes or frequency. Kehla walks through her real download data from 2019 to 2026, including peak years, drops in listenership, and the non-linear nature of podcast growth. She also unpacks the responsibilities of both podcast hosts and guests, the importance of specificity in interviews, and why saying “I'll talk about whatever your audience needs” is often a sign of authority leakage. Most importantly, she addresses one of the biggest gaps in podcasting today: underleveraging your channel. From transcript analysis and AI-driven content repurposing to threading episodes into email marketing, paid offers, and client transformation, Kehla explains how a podcast can become the spine of a business ecosystem — not just a content stream. If you are an entrepreneur, coach, or thought leader looking to grow a podcast, increase authority, improve podcast interviews, or integrate your show into your business strategy, this episode offers a grounded, no-formula perspective from someone who has stayed in the game for seven years. In the show notes, you'll find the link to the Seven Years In Podcast Compression Diagnostic, a locating tool designed to help you identify where your podcast is coherent, where it's leaking, and how it fits (or doesn't) within your broader business architecture. How do we balance free speech, platform accountability, and democratic integrity when technology moves faster than policy? In this episode, Katie Harbath, the "election whisperer to the tech industry," joins Corey Nathan to discuss the impossible trade-offs facing social media platforms, the evolving landscape of AI and misinformation, and what it means to "panic responsibly" in an era of rapid technological change. Katie spent a decade at Facebook as a policy director managing elections globally, navigating crises from Cambridge Analytica to the 2020 election. Now as CEO of Anchor Change and Chief Global Affairs Officer at Duco, she helps organizations understand how the internet shapes democracy. The conversation explores how to use AI ethically in creative work, the challenges of content moderation at scale, why community notes might be better than fact-checking, and how individuals can reclaim agency over their information diets. Katie also shares her personal evolution on free speech, the difference between distribution and moderation, and why the next four years will require all of us to find new ways to ground ourselves. This episode of Habitual Excellence featuring Jake Raymer will have health care leaders thinking deeply and acting differently. Raymer is both the past education director for the Shingo Model and just did 7 years as the Chief Transformation Officer at Munson Healthcare in Michigan, an operational excellence (or “lean”) leader. Jake is a renowned, powerful and entertaining thought leader across the world of operational excellence, who believes the only research worth its salt is research that holds up in the field, so it's a special opportunity to tap into his key learnings for today's healthcare leaders. Hear Jake point the way to more powerful and practical ways to shape a great “culture,” to manage change much more systematically and specifically, and talk about why more and more healthcare leaders are starting to learn about the Shingo model to take their own operating systems to a more grounded and powerful place, so they can drive more enduring mission-based performance.

Since there's a lot of noise in the Epstein space lately, I thought it was time to reintroduce myself to people who might have forgotten—or never knew—why I've been in this fight from the beginning. I've spent the last six years doing the work most people run from: pulling sealed court records, tracking financial infrastructure across jurisdictions, interviewing survivors without exploiting their pain, and naming names that never trend because they still hold power. I didn't parachute in when the headlines were hot. I was already deep in the files when nobody cared, when speaking about this got you labeled, flagged, or worse—ignored. While others turned the story into performance art or monetized it through half-baked speculation, I was chasing evidence through federal court systems, examining asset holdings, and exposing the institutional rot that let Epstein operate for decades in plain sight. This hasn't been about building a brand. It's been about building a case. And if you're just showing up now—confused by the noise, misled by opportunists, or wondering who to trust—know this: I'm not here to entertain you. I'm here to remind you that the truth never stopped being available. It just stopped being convenient.I don't have all the answers, and I've never claimed to. But I've done the work the best way I know how—quietly, persistently, and with a focus on what can be proven. I've made mistakes, corrected course, and kept digging because I believe the truth still matters, even when it's hard, even when it's buried, even when it's inconvenient for everyone. If my work helps cut through the noise for even a few people trying to make sense of all this, then it's worth it. That's all this has ever been about—and that's why I'm still here.to contact me:bobbycapucci@protonmail.com

On today's episode, Vince sits down with Luis Navia, a former Cuban-American drug trafficker who played a central role in the international cocaine trade for over 25 years, and Jesse Fink, the author who chronicled Luis's story. Born in Cuba, Luis fled to Miami with his family after Castro's takeover in 1959. Raised in privilege, he attended private schools and Georgetown University before becoming involved in drug trafficking in the 1970s. Known in South American cocaine circles as “El Senador,” Luis worked with powerful cartels, including Escobar's Medellín cartel, the Cali cartel, and Venezuela's Cartel de los Soles, building extensive smuggling networks across Latin America, the Caribbean, and the U.S. In 2000, Luis was arrested in a global operation that involved the DEA, FBI, and Colombian authorities. After being held accountable for over 10 tons of cocaine, he cooperated with U.S. authorities, providing critical information on cartel operations and serving five years in prison. Together, Luis and Jesse share insights into the dangerous world of drug trafficking, the decisions that led to Luis's rise and fall, and the lessons learned along the way. Steve Stoliar is an author and writer best known for his memoir 'Raised Eyebrows: My Years Inside Groucho's House', in which he recounts his fascinating experience living with the legendary comedian Groucho Marx. Stoliar worked as Groucho's personal secretary during the final years of the icon's life, spending three years at the Marx family home. His book offers a unique, behind-the-scenes look at the aging Groucho, blending humor, affection, and poignancy as Stoliar reflects on the quirks and complexities of living with one of comedy's greatest minds. Through his stories, Stoliar captures Groucho's sharp wit, his eccentricities, and the bittersweet reality of watching a cultural giant fade away. Raised Eyebrows provides fans with an intimate and very funny glimpse into the private world of a man whose public persona was as brash and irreverent as it was endearing.

The 2024 NFL Draft marked the fewest number of Ohio State players selected in over 10 years. With only FOUR Buckeyes selected throughout this years draft, it's become pretty clear that Ohio State coach Ryan Day has a clear development issue within the program. What is the real reason behind this lack of development in the Woody? Is this a correlation with the losses to Michigan each of the last three seasons? How can Ohio State get back to producing NFL talent like they did so well for years? Mike couldn't stay out of trouble and paid the penalty of prison time. The Death or Prison podcast is produced by Lean On Me USA Inc, a 501(c)(3) non-profit organization. Your support matters, and here is how you can help us! Thanks to our donors and sponsors, we have made a big impact in the lives of at-risk youth, inmates, families of inmates, and returning citizens. We don't plan to stop. Your support allows us to continue on our path. Within the United States, there are currently 2.3 million people in prisons, jails, and correctional facilities. Approximately 10 million men, women, and youth pass through jail or prisons yearly. Death or Prison podcasts are produced to reduce these numbers. The men on the Death or Prison program know what incarceration is and offer their personal experiences in order to tell how a second chance can become a new life. As the episodes unfold, the perspectives of the panel and their guests unlock an undeniable and enlightening path illuminated by their faith. Our listeners are both civilians and incarcerated individuals. The podcasts are available on 1.5 million electronic tablets in prisons nationwide. As well, the podcast can be found on Apple TV, Roku, and many more platforms, serving viewers worldwide! Reentry information includes but is not limited to mentoring, entrepreneurship, employment, housing, and spiritual guidance. To enable the continuous production and distribution of new podcast episodes, the 2022 goal of Death or Prison is to raise $150,000. This equates to $.10 for each prison electronic tablet. If you have two minutes to spare, dear listeners, please use them to fill out this Daily Delivery survey. Introduction: The Vikings are making some necessary and hard choices this offseason to get younger and shed salary with an eye toward the future. But in losing key players like Adam Thielen, Dalvin Tomlinson, Eric Kendricks and Patrick Peterson (so far), it sure looks like we are very much into the "rebuild" part of the "competitive rebuild" philosophy brought in a year ago. Also, does the signing of a blocking tight end Monday signal a shift in the running game? 9:00: Host Michael Rand is joined by Chip Scoggins and Carlos Gonzalez of the Star Tribune for a behind-the-scenes look at how they told the story of Maple Grove's journey to the boys' hockey state tournament. 29:00: A much-needed win for the Wolves and two departures for the Gophers men's basketball team.

Luis Navia worked for nearly 25 years as a cocaine trafficker for the deadliest Colombian and Mexican cartels until the law caught up with him in 2000. He is the co-author (with Jesse Fink) of Pure Narco: One Man's True Story of 25 Years Inside the Cartels. What We Discuss with Luis Navia: Why did Luis Navia -- a Georgetown student and the privileged son of an affluent sugar trader -- get involved in the illicit international cocaine trade with the most ruthless and infamous Mexican and Colombian drug cartels? How did Luis move between 200 to 300 tons of cocaine (roughly worth $10 billion) over the course of 25 years -- and live to tell the tale? Why did Luis insist on carrying out his business without a weapon -- in spite of being regularly surrounded by the perpetrators of unimaginably brutal violence? What are the logistics for smuggling massive amounts of cocaine over international borders with limited space and personnel? How did Operation Journey, an anti-narcotics effort coordinated across 12 nations and led by the United States and Great Britain, finally put an end to Luis' lucrative cocaine trafficking career? And much more... We all want people to like us and obviously a lot of things determine just how likable you are. However, there are a few simple techniques to make you appear more likable instantly. I start this episode with three ways, according to Psychology Today, that will make you more likable to others when you meet them. And they aren’t what you probably think. https://www.psychologytoday.com/us/blog/let-their-words-do-the-talking/201107/get-anyone-you-instantly-guaranteed-1 Evolution is what got us to where we are today. But the evolutionary road we have traveled to get here is a bumpy one. Biology professor Nathan Lents author of the book, Human Errors: A Panorama of our Glitches from Pointless Bones to Broken Genes (https://amzn.to/2zo2u67) takes us on a journey of the human evolution timeline and points out what went right and what went wrong. The point of the earth and its riding on a the rails of time, is mainly to accommodate human beings and God's plan for us, collectively and individually. As "imagers" of God, we each are born of unique DNA, into a certain group of peoples, within a specific place and time, and for specific purpose. And this, not of our own free will. After birth into our individual situation we are given free will, and with it, almost unending choices throughout our respective lifetimes. The question is, how do we learn God's collective and individual purposes for us, and to navigate through them? Prophecy makes clear that God has specific plans for each people group, and individual life. Throughout His Word and its prophecies, He gives all of us clues and cues, and even time-marks - called "holy days", and a multitude of other facilities, as a witness to the things God wills to accomplish through us individually and collectively. Satan has his plans for us too. We must learn how to distinguish God's voice from his, and to understand God's plan for each one of us. Join the class as we endeavor to learn how to interpret, and use God's Word and its prophecies to understand His collective and individual plan for humanity in the 6,000-year timeline on our unique proving ground called Earth.
