Remember this shocking story from the 2016 election? The Trump campaign gutted Ukraine aid in the official RNC platform. Today this is mainstream for the GOP, with McConnell folding on Ukraine aid. This is catastrophic for the US and the world. The recent transformation of the GOP into the Party of Putin began gradually, with only a few in Congress, like former Congressman Dana Rohrabacher, who the Kremlin considered an asset, according to the FBI. In 2018, current House Speaker Mike Johnson took tens of thousands of dollars in dark money campaign donations linked to a Russian oligarch close to Putin who was part of the larger Kremlin operation exposed in the Mueller Report. The oligarch is Konstantin Nikolaev, who bankrolled Kremlin spy/GOP honeypot Maria Butina, who was arrested as part of the Mueller investigation, after pretending to be in love with NRA/GOP operative Paul Erickson. After Mike Johnson got caught taking donations linked to Nikolaev and other Russians (Oops!), he gave it back. But it was part of a wider operation used by the Russians in countries around the world to entrench their power in, known as “the golden handcuffs.” Johnson blames other extremists in his party, like concentration camp guard Marjorie Taylor Greene, for "slowing down" Ukraine aid. They're not slowing down aid; they're blocking it, to help their financial backers in the Kremlin win. Johnson and Trump are part of the Kremlin's long game. They develop their assets, then spread their influence deeper and deeper. This began with a few fringe people, like Rohrabacher. Now it's mainstream. This is state capture. What happens if we lose our country? No place is safe. If you think things are bad now, the entire world will be forced to live under the Kremlin's mafia state: violence and terror win, deciding everything. No more laws and elections. Many Republicans watched in shock as the Kremlin hijacked their party. But it was Republican operations, like Citizens United, that helped Russian dark money entrench the Kremlin's influence in the United States. More Americans must wake up that the loss of Ukraine is also the loss of our democracy. What's happening in Ukraine and the U,S. is an ongoing crime of state capture. A mobilização é geral, os voluntários procuram o seu lugar em todas as formas desta guerra, seja fornecendo pequenos materiais para o exército, quer no caso dos homens em idade de combate, para a chamadas tropas de defesa territorial. Eles próprios tomam armas para fazer patrulhamento em zonas, ajudando os militares nesta matéria. Os ucranianos estão convencidos de que vão vencer esta guerra contra a Ucrânia, que estão imbuídos de um forte espírito nacional e essa confiança vai arpar também de um conjunto de pedidos feitos ao longo das últimas semanas, no sentido de um reforço militar ocidental face à Ucrânia. Esse reforço tem surgido e permite à Ucrânia defender-se cada vez melhor das ofensivas russas e contra-atacar. É uma guerra e portanto a Ucrânia também contra-ataca posições russas. Os russos estão sobretudo no sul, no sudeste e no leste da Ucrânia. As principais cidades estão a salvo da presença terrestre russa, mas têm sido alvo de bombardeamentos com mísseis, lançados a alguma distância por posições russas. A situação de alguma carência humanitária tem vindo a ser suprida pelas Nações Unidas e também por uma rede bastante efectiva do ponto de vista local com organizações não governamentais a fazerem esse trabalho. Há várias questões que se levantam, uma delas a Ucrânia pode vencer esta guerra, uma guerra muito patriótica do lado da dos ucranianos. Qual é o ponto de ruptura a teu ver entre os dois campos? O material é importante, claro, mas a questão psicológica e a moral, como descrevias, ainda mais. Sim. As notícias têm que ser lidas com algum cuidado, de um lado e do outro. Quer do ponto de vista ucraniano, quer do ponto de vista russo. Há muita informação contra informação e a natural propaganda, que sempre existe nas guerras. Do lado ucraniano, por exemplo, descreve-se que o moral das tropas russas é baixa, do lado russo também se descreve que os ucranianos se recusam a ir para zonas de combate. Seja como for, do ponto de vista estritamente militar, o poder aéreo russo faz bastante diferença. Os bombardeamentos através de mísseis lançados longa distância vão sobressaltando nas principais cidades e a população civil, nalguns casos, apenas só durante a noite, o que implica noites em branco para muitos cidadãos da Ucrânia. A maior parte dos alvos, na primeira fase da guerra, foram sobretudo estratégicos, militares e públicos, mas há também nas últimas semanas mais edifícios residenciais de população civil a serem atingidas. Isso acaba por dar também mais força aos ucranianos neste combate. Há uma questão muito relevante, tem a ver com a retirada de civis das zonas de combate. Há civis ucranianos que estão a ser retirados para o lado russo. Na Ucrânia diz-se que estão a ser forçados a sair para a Rússia, para autênticos campos de concentração. Não é possível confirmar de forma independente essa informação. Temos ouvido as difíceis condições dos corredores humanitários a partir de zonas do leste, com muita dificuldade para retirar pessoas. Eu falei com algumas, algumas saem destas zonas sob tiros e não é nada seguro sair nestes corredores humanitários. Estamos a falar sobretudo de mulheres, de pessoas idosas e de crianças porque os homens não não estão a sair. Nalgumas cidades, como no sul, um dos testemunhos que recolhia nos últimos dias, os homens estão a ser obrigados a listarem-se pelo lado russo e a combaterem os seus compatriotas ucranianos. Alguns ucranianos fogem para o ocidente outros, como descrevias, forçados a ir para a Rússia. Os civis não têm para onde ir? Os ucranianos que estão a sair da zona do leste distribuem-se um pouco por toda a Ucrânia sobretudo no centro e no oeste. Ao longo deste processo, muitos foram para cidades como Kiev, mas depois sobretudo acabaram por se localizar mais em cidades do ocidente como Lviv ou Ivano-Frankivsk. Como sabemos desde o início desta guerra, existe uma fracção, quetem procurado outros países que se imitam, nomeadamente, à Polónia. Há uma fracção bastante significativa, estamos a falar de quatro a cinco milhões de pessoas, que depois acabam por ou ficar na Polónia ou seguir para outros países da Europa Central. As redes de ucranianos, por exemplo, na Polónia existem e também na Hungria, devido a migrações anteriores e portanto essas pessoas, na maior parte dos casos, começaram por ficar em casas de familiares. Nalgumas cidades começam a surgir uma espécie de aldeias de contentores para receber esses ucranianos. O mesmo acontece na Ucrânia ocidental, inicialmente todos foram muito para a cidade de Lviv, mas depois todas as autarquias acabaram por se organizar de forma a receber em vários pontos estes ucranianos deslocados dentro do próprio país, com a esperança de poderem regressar; Alguns já começam a regressar a normalidade, já começa a surgir em cidades como Kievpor, por exemplo ou também Odessa, no sul do país. Já estiveste em Odessa. Agora estás nos arredores de Kiev. Como é que descreves o dia-a-dia aí? Eu percorri a Ucrânia toda. Na verdade, fiz duas viagens à Ucrânia. Uma primeira onde passei mais tempo em Lviv, na Ucrânia Ocidental, e agora numa segunda fase percorri toda a zona leste da Ucrânia, em Kharkivem, em Dnipro, em Nikolaev e Odessa. São situações muito diferentes. Enquanto em Kharkivem, como tenho descrito para a RFI, está tudo pronto para um combate terrestre com barricadas colocadas um pouco em todos os pontos estratégicos da cidade, em todos os cruzamentos. Há pneus amontoados para poderem fazer as posições de defesa. Em Odessa, há uma situação perfeitamente normal. Uma cidade mais normalizada, onde as pessoas aparentemente estão mais despreocupadas. Sendo que no último fim-de-semana, a queda de um míssil sobre uma zona residencial assustou muito as pessoas de Odessa. O mesmo acontece frequentemente à cidade de Lviv, no início estava aparentemente segura, mas também acaba por sofrer um impacto, psicológico com a queda de alguns mísseis, sobretudo em alvos relacionados com zonas industriais e nalgumas instalações ferroviárias de mercadorias. Nas restantes cidades como Dnipro, depois de uma grande pressão inicial, está um pouco mais estabilizada. A situação mais complicada que vivi e observei foi em Kharkivem, a segunda cidade na Ucrânia, extremamente pressionada com bombardeamentos constantes. A diplomacia russa fala de perigo real de uma nova guerra mundial. António Guterres, o secretário-geral das Nações Unidas, vai encontrar-se com o Putin. O que é que procura a Rússia? E o que é que procura Vladimir Putin? Procuram assustar? A Terceira Guerra Mundial tem que ser cruzada com um outro perigo que se fala menos hoje, que é o dia do aniversário do acidente de Chernobyl. Hoje há um conjunto de ataques com mísseis a baixa altitude na zona de Zaporizhzhia, onde há uma central nuclear que é, de resto, a maior central nuclear ucraniana. Esse perigo, de utilização de armas químicas e nucleares têm vindo a ser avançado pelo lado ocidental e também pela Ucrânia. Até agora não há nota de que tenha acontecido, apesar das preparações que existem aqui do lado ucraniano. Do ponto de vista diplomático, há duas ou três linhas que têm que ser aqui clarificadas. A primeira tem que ver com a mediação em relação à paz. Claramente que a Turquia está na dianteira e, por isso, o secretário-geral das Nações Unidas esteve na Turquia para encontros com Erdogan. As decisões têm estado paradas ao longo das últimas três semanas, depois de reuniões que ocorreram do ponto de vista físico. Há algumas conversações virtuais. Há contactos entre ambas as partes em relação a duas áreas. Uma tem a ver com a troca de prisioneiros. Tem acontecido quatro ou cinco trocas de prisioneiros entre a Rússia e a Ucrânia e depois a negociação dos corredores humanitários. É aqui que entra o secretário-geral da ONU, a Ucrânia pede a Guterres pressão junto da Rússia para que abra corredores humanitários seguros, uma vez que a Ucrânia não acredita que os russos providenciem segurança suficiente para estes corredores humanitários, nomeadamente, a partir de Mariupol. Do lado russo há também uma desconfiança em relação a António Guterres porque consideram que desde o início, o secretário-geral das Nações Unidas não é um mediador equilibrado e está do lado da Ucrânia. Por sua vez, a Ucrânia também não está totalmente satisfeita com António Guterres e digamos que a missão do secretário-geral da ONU é difícil sobretudo num contexto da ONU em que o verdadeiro poder está no Conselho de Segurança, onde a Rússia é um dos países com direito de veto. Os Estados Unidos convocaram para hoje uma reunião com os aliados. A Alemanha recebe cerca de quarenta países os europeus que dão apoio material sobretudo ao Ucrânia para se defender da Rússia. Segundo muito analistas a Rússia está a patinar há dois meses. Podemos dizer, José Pedro, que o exército hierárquico russo é um exército do XX, que luta contra civis ucranianos do século XXI? Existem algumas carências do exército russo. Uma delas diz respeito às comunicações. Tanto quando julgo ter percebido de algumas conversas aqui na Ucrânia e com alguns analistas militares, por exemplo, ao nível das comunicações é possível interceptar muitas comunicações do lado russo porque por parte dos ucranianos. É preciso dizer que na Ucrânia fala-se russo de forma fluente ou seja é uma língua quase equivalente à ucraniana e portanto é muito fácil entender russo. Essas comunicações digamos mais do século XX, ainda persistem nos contingentes militares russos. De resto, os ucranianos foram abastecidos de materiais recentes de intercepção de comunicações por alguns ocidentais. Os aliados ocidentais não foram apenas fornecendo carros de combate, tanques ou defesa anti-aérea, mas também equipamentos sofisticados de comunicação ao mais alto nível e com formação dada. Essa é uma desvantagem, digamos assim, para o lado russo que consegue contrabalançar com o poderio de mísseis, com alcance a longa distância. Na verdade a estratégia militar russa parece ser disparar em várias direcções, em vários pontos da Ucrânia, de forma a que a Ucrânia esteja permanentemente em alerta do ponto de vista antia-éreo, não se podendo concentrar em toda a sua defesa, por exemplo no Donbass, na zona do leste da Ucrânia, onde os principais combates decorrem. Hoje, 26 de Abril, a Ucrânia está destruída sem saber de quando é que esta guerra termina? Sim, as opiniões dividem. Há quem acredite que a guerra vai terminar dentro de duas semanas, mas isso é uma referência ao dia 9 de Maio. O dia 9 de Maio é um dia importante para os russos porque assinala a vitória sobre a Alemanha Nazi. Putin quererá fazer um ponto de situação e apresentar uma vitória ao povo russo, que poderá ser, por exemplo, conquista de Mariupol ou noutras perspectivas, mais gravosas, poderá ser a conquista terrestre de todo o sul até Odessa. Há um outro cenário, por exemplo, eu falei com o presidente da câmara de Nikolaev que nos dizia que achava que a guerra iria durar mais um ano. Seria uma guerra longa e portanto eles estão preparados para todas as eventualidades. O espírito é bastante positivo do lado ucraniano. Devo dizer que é isso que se transparece das conversas quer com militares, quer com responsáveis políticos. Seja como for, há outras consequências do ponto de vista económico: as culturas agrícolas que têm que ser escoadas. Os portos estão fechados e portanto não é fácil, aliás, é praticamente impossível escoar grandes quantidades de, por exemplo, produtos agrícolas ou outras matérias da indústria ucraniana. Por outro lado, a própria indústria ucraniana vai sendo bombardeada. Refinarias, centrais termo-elétricas, infraestruturas de eletricidade, algumas infra-estruturas ferroviárias. Estamos a falar de estruturas que vão demorar o seu tempo a ser reconstruídas. Vai demorar ainda algum tempo até a Ucrânia levantar-se e a própria ONU, através do seu programa alimentar mundial, também nos diz que prepara uma operação de longo prazo na Ucrânia a partir de agora.

However, he went on to say however, that due to the Russian provocations, the humanitarian corridors were all closed over Ukraine today. This week’s episode includes author Charlotte Andersson and Associate Editor Naveed Sattar as they discuss familial clustering of aortic size, aneurysms, and dissections in the community. TRANSCRIPT: Dr Carolyn Lam: Welcome to Circulation on the Run, your weekly podcast, summary, and backstage pass to the journal and its editors. I'm Dr Carolyn Lam, Associate Editor from the National Heart Center and Duke national University of Singapore. Dr Greg Hundley: And I'm Greg Hundley, director of the Pauley Heart Center at VCU health in Richmond, Virginia. Well, Carolyn, our feature this week has to do with aortic size, aneurysms, and predilection to dissection. But before we get to that, how about if we grab a cup of coffee and go through some of the other articles in the issue? Dr Carolyn Lam: I got my coffee, Greg, and you know what? I'm going to start with quiz for you. Dr Greg Hundley: All right. Dr Carolyn Lam: True or false, in the setting of obesity and/or diabetes, cardiac substrate metabolism shifts towards increased fatty acid oxidation, while lipid accumulates in the heart? True or false? Of course, you're right. Oh, but there's a part two. Can you guess, by increasing fatty acid oxidation, will we induce or prevent obesity-induced lipotoxic cardiomyopathy? Dr Greg Hundley: I'm going to say, because you asked it in the way you asked it, prevent. Dr Carolyn Lam: Wow. All right. Well, the truth is we didn't really know before today's paper. The specific link between cardiac metabolism and lipotoxic cardiomyopathy was elusive and there was no specific therapy available for this condition. And these authors, Dr Rong Tian from University of Washington and colleagues, hypothesized that cardiac pathology-associated obesity would be attributable to the imbalance of fatty acid supply and oxidation. So using a diet-induced obesity model in the current study, they demonstrated that enhancing fatty acid oxidation through deletion of acetyl-CoA carboxylase 2, was sufficient to prevent obesity-induced cardiomyopathy. So, increasing cardiac fatty acid oxidation alone does not cause cardiac dysfunction, but instead protects against cardiomyopathy in chronically obese mice. The cardiac-protective effect of increasing fatty acid oxidation and obese mice is through maintenance of Parkin-mediated mitophagy, and thus preventing mitochondrial dysfunction. These findings indicate that impaired mitophagy contributes to mitochondrial dysfunction in obese mice, and that targeting the Parkin-dependent pathway is a viable therapeutic intervention for obesity-induced cardiomyopathy. Dr Greg Hundley: Very nice. Carolyn. Dr Carolyn Lam: I'm going to be greedy and go on to my next paper. So Greg, do you think cardiac regeneration is possible? Dr Greg Hundley: Well, Carolyn, I would have said, several years ago, no, but that trip that we took to China with Joe Hill and Hesham Sadek, our Associate Editor and our Chief Editor, convinced me otherwise. So I'm going to definitely answer yes on this one. Dr Carolyn Lam: Oh, Greg, you're just too smart. And speaking of China, this next paper is from there, from co-corresponding authors, Dr Nie and Hu, from Fuwai Hospital National Center for Cardiovascular Disease and Chinese Academy of Medical Sciences and Peking Union Medical College. So, using seven genetic mouse lines, they identify that Oncostatin M is the top upregulated cytokine during neonatal heart regeneration. Oncostatin M is a pleiotropic secretory protein that belongs to the interleukin 6 family, and associates with the pathological process of dilated cardiomyopathy. And these authors found that macrophages promote heart regeneration by secreting Oncostatin M, which promotes cardiomyocyte proliferation via a co-receptor, gp130. Employing RNA-seq and functional screening, they further found that Src-mediated gp130 triggered cardiomyocyte proliferation by activating the downstream signaling pathway involving Yap, with Y357 phosphorylation independent of the Hippo pathway. So the last thing that they did was show that gene therapy with adenovirus-associated virus and coding this activated gp130 improved heart regeneration and pumping function, thus serving as a potential therapeutic target. An amazing paper. Dr Greg Hundley: Very nice, Carolyn. What a great summary and so much detail. Well, Carolyn, I'm going to turn our attention to catecholaminergic polymorphic ventricular tachycardia. And this article comes to us from Dr Jason Roberts, from the Western University. Carolyn, genetic variants in calsequestrin 2 can cause an autosomal recessive form of catecholaminergic polymorphic ventricular tachycardia, though isolated reports have identified arrhythmogenic phenotypes among heterozygotes. So in this study, a total of 112 individuals, including 36 catecholaminergic polymorphic ventricular tachycardia probands, 24 were homozygotes for compound heterozygotes, and 12 were pure heterozygotes, against 76 family members possessing at least one presumed pathogenic calsequestrin 2 variant. These were all identified. Dr Carolyn Lam: Wow, a very precious cohort. So what did they find, Greg? Dr Greg Hundley: This international multicenter study of calsequestrin 2 catecholaminergic polymorphic ventricular tachycardia really redefined its heritability and confirmed that pathogenic heterozygous calsequestrin 2 variants may manifest with a catecholaminergic polymorphic ventricular tachycardia phenotype, indicating a need to clinically screen these individuals. Among individuals heterozygous for a pathogenic calsequestrin 2 rare variant, medical therapy and exercise restriction are likely not necessary in the absence of the catecholaminergic polymorphic ventricular tachycardia phenotype. Though, you have to be certain over time, an intermittent clinical screening to ensure they remain phenotype-negative should be obtained. Dr Carolyn Lam: Wow, Greg, clinically important study there. Well, I'm going to go back to the basic science world and talk about calcineurin. Now, calcineurin has long been implicated in the induction of pathological cardiac remodeling but has not been therapeutically targetable for the prevention of heart failure because of its pleiotropy and our lack of understanding of its specific protein-protein interactions and compartmentation within the cardiomyocyte. Dr Greg Hundley: Okay. Carolyn, do you want me to give background on calcineurin? Dr Carolyn Lam: No, Greg, you're off the hook. I'm going to give you some background on calcineurin. So, calcineurin is the calcium-calmodulin-dependent phosphatase that exists as a heterodimer, consisting of a catalytic subunit and a regulatory subunit. Now, of the three catalytic subunit isoforms, alpha, beta, gamma, it's the beta isoform that appears to be the most important for the development of cardiac hypertrophy. Binding of calcium to the calcineurin regulatory subunit enables binding of the calcium-calmodulin complex, thereby releasing auto-inhibition and freeing the enzyme to dephosphorylate downstream substrates. That's the background. Now, in today's issue, we have this great paper from co-corresponding authors, Dr Kapiloff from Stanford University, and Dr Nikolaev from University Medical Center Hamburg. And, with their colleagues, they described the discovery of a calcineurin catalytic subunit beta binding protein Cdc42-interacting proteins 4, and I'm going to call that CIP4, which functions as a scaffold to sequester the pool of calcineurin near the sarcolemma of cardiomyocytes, where it regulates pro-hypertrophic signaling. These findings have really important implications for understanding how cardiac calcineurin is selectively activated by stress signals, as opposed to the pleiotropic second messenger, calcium, that really floods the cardiomyocytes during each contractile cycle. Furthermore, the data provide proof of concept for an innovative therapeutic approach, whereby CIP4-anchoring activity is selectively inhibited to block the action of a small pathogenic pool of calcineurin as a means of treating heart failure. How about that? This is really discussed in an elegant editorial by doctors, Woulfe, Travers, and McKinsey. Dr Greg Hundley: Very interesting, Carolyn. Sounds like another possibility for treating and managing heart failure. Well, let me share with you some of the other findings in our mailbag this week. First, I've got, from Professor Lang Li and Stephen Wiviott, they swap research correspondence regarding the prior publication entitled, Effect of Dapagliflozin on Atrial Fibrillation in Patients with Type 2 Diabetes Mellitus, Insights from the DECLARE-TIMI 58 Trial. And then Professor Laszlo Littmann has a nice ECG challenge for us related to a high-risk ECG that exposed some downstream worrisome vital signs. Dr Carolyn Lam: In addition, there's a perspective piece by Dr Nambi discussing the fact that a zero-calcium score is desirable, but isn't enough to defer therapy, given that up to one-third of events will occur in this group. There's also an In Depth paper by Dr Borlaug, entitled, “Altered Hemodynamics and End Organ Damage in Heart Failure, The Impact on the Lung and Kidney,” and oh boy, this one is so beautifully illustrated. Just a must read for the understanding of the hemodynamics in the lung and kidney and heart failure. Next is a research letter by Dr Loeys on enrichment of rare variants in the Loeys-Dietz syndrome genes in spontaneous coronary artery dissection, and not in severe fibromuscular dysplasia. And finally, another research letter by Dr Arora on racial differences in serial NT-proBNP levels in heart failure management with insights from the GUIDE-IT Trial. What a rich issue, but let's move on to our future discussion, shall we? Dr Greg Hundley: You bet, Carolyn. Well, listeners, we're now getting to our feature discussion and it's very interesting this week where we're going to evaluate aortic aneurysms. And we have with us one of the lead authors of this paper, Dr Charlotte Andersson from Boston Medical Center, and our own Associate Editor, Naveed Sattar from Glasgow, Scotland. Charlotte, welcome to our feature discussion. Could you tell us a little bit about the background and the hypothesis that you put forward with this study? Dr Charlotte Andersson: The background for this study was based on clinical work and what we observed in clinic. We had a few patients where we had been stricken by the fact that they came in with an acute aortic syndrome and they had a first-degree relative themself with the condition, but they did not look syndromic at all. And we started to wonder, what is the actual risk in the community, in people without obvious syndromic features of suffering from an aortic event itself. And although there are quite a few studies out there that have, to some degree, focused on the familial clustering of aortopathies, there is not a lot of information based on communities and whole entire populations. So we wanted to, frankly, estimate what is the incidence rates of aortic dissections and aortic aneurism in the community if you have a first-degree relative that has suffered from the disease themselves. Dr Greg Hundley: How you organize your study and what was your population and what was your design? Dr Charlotte Andersson: This study was actually based on two independent samples. First, we used the Framingham Heart study population that is very densely phenotypes over many years of spanning three generations of participants, where we looked at people who had at least one parent who had an aortic size in the upper quartile index to body-surface area and adjusted for age and sex. And we saw what's the risk of you, as a child, having an aortic size in the same upper quartile. And second, we looked in the general Danish population, the Danish healthcare system is, as you probably know, governmental funded and we have very good registries of all hospitalizations, all outpatient visits, and so we were able to link more hard clinical events in people with and without a first-degree relative. What we did was we started time when people had an aortic dissection, we identified all the first-degree relatives in these people, and we matched them with up to 10 sex and age match controls from the general population without a first-degree relative with the disease. Dr Greg Hundley: What did you find? Dr Charlotte Andersson: We found that in the Framingham sample, if you had at least one parent who belongs into the upper quartile of aortic size, you had an odds ratio of two to three, adjusted for various clinical risk factors, such as hypertension and smoking yourself. And in Danish population, we found that if you had a first-degree relative with an aortopathy, the hazard rates for you developing the disease yourself was almost a tenfold-increase compared to age and sex match controls. And importantly, seemed like hazard ratios use were, more or less, unchanged when we start adjusting various known risk factors, such as bicuspid aortic valve, Marfan syndrome, and Ehlers-Danlos syndrome, normally those kinds of things. And we also found that the younger your proband were at the time of an acute event, the higher was your relative risk yourself. So among people who were below the age of 50 when they suffered an event, the hazard ratios were up to a 50-fold increase. Dr Greg Hundley: Very nice. Naveed, what attracted you to this article as it was coming through the editorial process? And then second, how do we take the information that Charlotte's just conveyed and will be published here today, how do we take this in the context of what we already know about aortic aneurysms? Dr Naveed Sattar: I think it's a beautiful study, so well done, Charlotte. I think it's a beautiful fusion. As Charlotte said, an in-depth cohort study, which has got very well-measured parameters of systematic points and a fantastic population-based data set from Denmark, which Sweden shares and Scotland shares and relatively small countries like us share. So small countries like Denmark punch above their weight in these kinds of studies, which is fantastic. But there's a rich seam of research that comes from these, and this is one of them. So I think that fusion of two data sets with different strengths and limitations combined giving off same signals is good. I think, as Charlotte said, this is the first major population study to look at this question. So there's been people around the world who have got this sense that the aortic aneurism may well be familial, this provides, probably, some of the best data to suggest, yes, it definitely is. Now the questions going forward is, okay, at what point do you screen everybody's got a family history with a proband, or do you screen those who've got a family history of younger probands? And I think what Charlotte and the team and other people around the world thar are going to look at this say, "Okay, we now think, in addition to screening, for example, in the UK and the US we probably screen just men above 65, where most of the disease is, do we also then implement screening in younger people with family histories? And who do we screen, and when and how? And do we need to develop some kind of risk score?" And then when we do that screening, what do we do about it? Is going to be the questions and I'm sure Charlotte and her colleagues have thought about these things and it'd be interesting to see what her view is on those things. But I think it was a beautiful study in every sense. Dr Greg Hundley: So Charlotte, he's really set you up nicely, what study do we need to perform next in this area? What are you and your group thinking about? Dr Charlotte Andersson: Yeah, I think there are two implications of this study. First, clinical, as Naveed says. They already had a sense that aortic diseases were heritable, and I think these data definitely support that we should probably screen first-degree relatives. And I think, at some extent, this is what the guidelines already encourage us to do. So I'm not sure it would be feasible to randomize people or do a clinical trial where we screen some but not others. I'm not sure that would be ethical. I think the evidence is too strong for familial clustering and that we should probably screen these people. But I think also, our estimates, they are so strong that I suspect that there are likely more genetic variants associated with non-syndromic aortopathies that we are not aware of just yet. So I think the next step would be to try to disentangle the genetics a little bit more. I have seen some preliminary analysis based on the UK Biobank, for instance, and I think there are more genetic variants to come up with also, more common genetic variants, at least, that we are not aware of just yet. So that would be the next step as I see it. Dr Naveed Sattar: And that might particularity in younger probands. Dr Charlotte Andersson: Right. Dr Naveed Sattar: Those with the younger probands, because it looks like, as you said, the hazard ratio, the risks, are so high, it could also potentially be monogenic, but anyway. Dr Charlotte Andersson: I agree. Dr Greg Hundley: Well, Charlotte, Naveed, we really appreciate your time and taking this opportunity to discuss these really interesting findings and helping us understand that, truly, there may be a familial component to understanding this disease process, particularly in patients with aortic aneurysms that may go on to develop aortic dissections. В проекте федерального закона «предлагается определить воспитание как деятельность, направленную на развитие личности, создание условий для самоопределения и социализации обучающегося на основе социокультурных, духовно-нравственных ценностей и принятых в обществе правил и норм поведения в интересах человека, семьи, общества и государства, формирование и обучающихся чувства патриотизма и гражданственности, уважения к памяти защитников Отечества и подвигам героев Отечества, к закону и правопорядку, человеку труда и старшему поколению, взаимного уважения, бережного отношения к культурному наследию и традициям многонационального народа Российской Федерации, к природе и окружающей среде». Вопросов в связи с обсуждением данной инициативы возникает много и не только у родителей, но и у экспертного, профессионального сообществ. Прежде всего, а как ценности семейные будут коррелировать с ценностями обучающих организаций и в принципе конкретных учителей? Ведь учителя, как и любой человек, бывают разными. Так кто же будет воспитывать наших детей и какие ценности будут им прививаться? Где найти консенсус и объективность картины в вопросе ценностных ориентиров? Может ли «ценность» быть одинаковой для всех? «Воспитание» предлагается вернуть не только в школы, но и вузы. Можно ли уже сформировавшиеся личности в возрасте 17 – 18 лет воспитывать? Складывается ощущение, что мир сходит с ума. Но есть положительные тенденции: люди стали больше уделять времени своим родным и близким, ценить свое здоровье и беспокоиться о здоровье родных, дорожить живым общением и в целом жизнью. В конце концов люди научились разговаривать друг с другом без гаджетов. Природа ожила и заиграла совершенно новыми красками. Человечество получило бесценный опыт, но вот как использовать этот опыт и «выйти» в мир с улыбкой, добрыми намерениями, большой вопрос. Так как же изоляция сказалась на эмоциональном и психическом состоянии людей? Как людям справиться с появившимися страхами, накопившейся агрессией и стать вновь социальными? Как родителям правильно общаться и вести себя со своими детьми после изоляции? Victor’s Vignettes … Stories about Life in soviet and post-soviet Ukraine.-by Victor SergeyevMikolayev, UkraineThe Music Of My Childhood — 1966-1975When it came to music listening in soviet Ukraine, we always had choices.We could tune in on radio receivers to hear soundtracks of the two available TV channels, the state-controlled Moscow channel and the pro-Moscow Kiev channel. Or we could create a cultural environment of our own … underground.Of course, the latter choice was by far the most popular.Not that it was easy.In official stores only government-sanctioned goods were available for purchase. So radio receivers came without 19 and 25 meter bands, to block transmissions of Voice of America and BBC channels.But where there is a will, there is always a way.There was the black market, occasional trips abroad, and of course we could always build our own radios. As well, there were old WWII trophy German radios around, or you could buy good Japanese tape recorders with built-in radios.As I said, we had choices.My home town of Nikolaev is a sea port … and for me, it was a window to the world.The seamen always brought home plenty of vinyl disks from their trips abroad. So, for as long as I can remember, The Beatles, Rolling Stones, Deep Purple, Slade and other prominent names in western pop culture have been part of my consciousness.The all-union company “Melodia” in far away Moscow of course tried to control our musical preferences. It was a useless exercise.The selection in the Moscow-controlled store numbered in the mere hundreds of discs… all, naturally, carrying the label “Melodia.”The black market, however, offered thousands upon thousands of discs from all over the world – the United States, Great Britain, Japan, Taiwan, Malaysia, Spain, France. Even socialist countries like Poland, Romania, and the former Yugoslavia.Our local black market had a name … ‘Skhod’ which means “gathering.” Skhod existed according to its own rules, independent of any authorities. It took place once a week, on Sundays, and it was a ‘sacred dream’ for music collectors like me.But contraband is never cheap. The cost of just 3 foreign vinyl disks was equal to the monthly salary of an engineer. Therefore we would copy the disks and share the music. We used ordinary tape recorders, as magenetic tapes were relatively inexpensive and easy to come by.Sometimes we wondered if we were being disloyal to our heritage by embracing foreign culture as we did. But Ukrainian culture was suppressed by Soviet authorities, with only fragments remaining. We knew of some Ukrainian singers and songwriters, but they were loyal to the Soviet regime. And of course, anything or anyone bearing Moscow’s stamp of approval held no interest for us.In those days I was vaguely aware that in Western Ukraine, in places like Lviv and Ivano-Frankivsk, Ukrainian culture remained strong. But, unfortunately, those cities were too far away to have any influence on us.Taras Shevchenko, Mykhailo Kotsiubinsky, Ivan Franko, Ostap Vyshnia, Volodymyr Ivasiuk, Sofia Rotaru and others were of course known names to us. But they were only relics of Ukrainian culture. Ancient history … throwbacks to the past. All meant to be forgotten.Now every day it becomes more clear just how much of our cultural heritage is lost to us forever… due both to soviet oppressors and the purveyors of western pop culture.At least young people today have more choices, and better ones. On 1 December 1934, Leonid Nikolaev, a disgruntled Bolshevik Party member, shot Sergei Kirov in the back of the head as the Leningrad Party boss approached his office in Smolny. The murder sent shockwaves throughout the Soviet leadership, which with Stalin as its helmsman, used it to concoct a wider conspiracy that fingered oppositionists as the true plotters. By 1937, Stalin had used the murder to initiate full blown political terror against his former political enemies, military leaders, intellectuals, former classes, and ordinary people. When the smoke cleared in the summer of 1938, 2.5 million people had been arrested and an estimated 700,000 had been shot, including many of the purgers themselves. Kirov’s murder is considered by most to be the crucial spark that ignited this conflagration of death. But who really killed Kirov? Was Nikolaev a lone gunman? Or did Stalin orchestrate Kirov’s murder to eliminate a potential rival and justify mass murder? Until recently, the “Stalin did it” theory served as the historical consensus despite skepticism from a few. No longer. In his 832 page tome The Kirov Murder and Soviet History (Yale University Press, 2010), Matthew Lenoe rakes a fine toothed comb over the available evidence about the murder to decisively settle the debate and examine its place in Stalinist and post-Stalinist Russia. Moreover, as part of Yale’s Annals of Communism series, the book contains 172 translated documents, most from Soviet archives. Did Stalin plot to kill Kirov? Lenoe convincingly shows that the most plausible answer to this persistent question is no. Abstract: Although impairments of motivational and consummatory aspects of reward behavior are core symptoms of several psychiatric disorders, the underlying neural and molecular mechanisms remain poorly understood. Here we report that blocking either c-Fos or matrix metalloproteinase 9 in the central amygdala disrupts approach motivation and appetitive discrimination learning, but not reward consumption. Further, we show that manipulation of c-Fos-MMP-9 pathway does not affect aversive motivation and learning. These findings reveal molecular mechanism of motivational anhedonia.
