Dividieron a los sujetos en dos grupos: los del grupo A eran de carácter rígido e impaciente; los del grupo B eran más tranquilos y pacientes. Tras esa clasificación preliminar, se tomó nota de los hábitos de salud general de los sujetos: si fumaban o no, cuánto ejercicio hacían, cuáles eran sus niveles de colesterol en sangre, su tipo de alimentación, etcétera. Diez años después, más de 250 de aquellos hombres físicamente sanos habían sufrido un ataque al corazón. Al analizar los diversos valores mostrados en todas las categorías analizadas en el seguimiento de diez años, se descubrió que el único factor predictor de riesgos de ataque cardíaco había sido la disposición mental. Los hombres que pertenecían al grupo A, de mentalidad más rígida e impaciente, habían tenido una incidencia de ataques cardíacos tres veces mayor que los del B, independientemente de si fumaban, hacían menos ejercicio o llevaban peor alimentación.No quiero decir con esto que los hábitos de salud física no importan; claro que importan, y mucho; de hecho, por eso se tomaron como factor en el estudio, porque se sabe que importan. Pero en todos aquellos varones sanos, sin problemas de salud, lo determinante a la hora de sufrir algo tan grave como un ataque cardíaco fue su actitud mental ante la vida. ¡Más determinante aún que fumar o alimentarse mal! Cuando pensamos en hábitos, solemos pensar solo en las cosas que hacemos rutinariamente; pero hábitos también son nuestras maneras de pensar, nuestros esquemas mentales, nuestros errores de pensamiento y análisis, nuestras actitudes ante lo que nos sucede, nuestros parámetros de conducta, las manifestaciones de nuestro carácter.Enfermedades como las cardíacas tienen relación con nuestros hábitos de pensamiento, y con nuestra manera de gestionar mentalmente el estrés y las emociones; por eso merece la pena trabajar en mejorar nuestras actitudes. La salud, mental y física, tiene mucho que ver con cómo interpretamos la realidad y cómo respondemos a esa interpretación. Teams and teamwork were the themes for our November Journal Club. Jared, Oriana and Ned talk about Ned's choice of topic: Ralph Bakshi's animated film adaptation of The Lord of the Rings. A heavily promoted effort at the time due it being both the first ever version of Tolkien's work done for film as well as due to Bakshi's notable reputation as the most well known American animator since Walt Disney and Chuck Jones, the 1978 movie was a reasonable success but not a smash, and the fact that it was only the first half of a planned two-film sequence – but not marketed as such – led to confused audiences and an upset Tolkien reader fanbase. At once a surprisingly faithful if still heavily redacted version of the book and a very unusual mix of animation styles that grew out of Bakshi's earlier movies, the film has retained a cult following since, and the resulting contrast with the later Peter Jackson movies has proven illuminating to both. What makes the sequences like the Flight to the Ford both compelling viewing and something of a slog? Which voice actors bring a notable depth to their characters along with the script and animation efforts – and which ones end up undercutting their characters completely? Episode 11 Chlamydia with Clau The sun rises over the San Joaquin Valley, California,today is May 8, 2020. On April 28, 2020, the USPSTF released a final recommendation about prevention of tobacco use in children and adolescents. It is recommended that primary care clinicians provide interventions, including education or brief counseling, to prevent initiation of tobacco use among children and adolescents. Note that it doesn’t say prevention of “smoking”, it says prevention of “tobacco use” because we know that vaping is “a thing” among youth nowadays. This is a grade B recommendation, which means there is moderate to substantial benefit for this service. Now, an update about COVID-19. As of May 4, 2020, the CDC reports a total of 1,160,000 cases and 68,000 deaths due to COVID-19 in the USA. It has been a rough year so far for humanity! On May 1st, 2020, the FDA issued an Emergency Use Authorization to remdesivir for the treatment of COVID-19. Remdesivir can be used in hospitalized patients with severe disease. Remdesivir may shorten the time it takes to recover from the infection. It is given intravenously only. The issuance of an Emergency Use Authorization is different than FDA approval. As you know, we ask 5 questions in this podcast, and we’ll start with question number 1.Question Number 1: Who are you?My name is Claudia Carranza, I am a second-year family medicine resident in the wonderful Rio Bravo Family Medicine Residency program in Bakersfield, CA. I grew up in Peru then moved to the States for college, attended a couple of community colleges before transferring to UCSD as a Biology major. Then went to Ross University School of Medicine in the Caribbean where I earned my medical degree. I did 1 year of Internal Medicine residency, and then transferred to Family Medicine and I could not be happier!I am also a wife to a very busy IM resident, I am a Dog mom to Chewie. I don’t have lots of time for hobbies but when there’s time I like to do some strength training, dance, go on walks or runs with Chewie, cook healthy meals, bake and hang out with my hubby and friends. My favorite movie is Love Actually, and my favorite sport is swimming.Question number 2: What did you learn this week?This week I learned about the difference between Chlamydia Test of Cure (TOC) and Retesting. At our clinic, we have quite a few obstetrics patients, and they all get tested for Chlamydia as new OB patients, as part of their prenatal lab panel. When they are positive, they get treatment, and after treatment they undergo a Test Of Cure or TOC, no earlier than 3 weeks after completion of therapy. All patients with documented infection should also undergo retesting; this includes pregnant patients. When we have a pregnant patient who is infected we inquire about their partner and encourage the partner's treatment. Those partners, just like anyone with a documented infection, should have retesting done. Example: Let’s say we get a positive C. trachomatis test on one of our pregnant patients. We have to notify the patient of the results and the need for treatment. The recommended regimen for treatment is 1g oral Azithromycin given as a single dose.If you have a patient who CANNOT tolerate Azithromycin then you may treat with either amoxicillin or erythromycin.Recommended doses: Amoxicillin 500mg orally TID for 7 days, Erythromycin base 500mg QID for 7 days or 250mg QID for 14 days, Erythromycin ethylsuccinate 800mg QID for 7 days or 400mg QID for 14 days.Remember after treating the patient and hopefully also their partner, the pregnant patient will need a TOC. Other patients who require a test of cure are any patients that show persistent symptoms or that were treated using a regimen with inferior cure rates, such as erythromycin or amoxicillin. RetestingRetesting is done to check if a patient has been re-infected. This can be done 3 months after treatment or at their first visit thereafter within 12 months of treatment.Now, think you are at the hospital and you have a pregnant patient that comes to triage in active labor. They brought some of their prenatal records and you know they had a positive C. trachomatis test, and she was treated but she did not have a test of cure, or you don’t have the records to confirm the results. In this case, there is usually not enough time to get a test of cure or retest prior to delivery, so these patients NEED TO BE TREATED upon admission with one of the recommended regimens. Of note, when treating pregnant patients; the antibiotics contraindicated during pregnancy (and lactation) are: Doxycycline, levofloxacin, ofloxacin, and erythromycin estolate.Comment: The pregnancy categories by letters (A, B, C, D, X) were updated on June 30, 2015, by the FDA. Now, all medications are required to include three sections with explanations: Pregnancy, Lactation and Females and Males of Reproductive Potential. Erythromycin should not be used during the first trimester of pregnancy. However, it may be appropriate as an alternative agent for the treatment of chlamydial infections in pregnant women (consult current guidelines)Treatment of the partner(s)There are certain states in which Expedited Partner Therapy (or EPT) is permissible. This means a physician can treat the sex partner of a patient who is being treated for chlamydia; in other words, prescribe their partner medication without having examined them. California is one of the 44 states in which EPT is permissible. Question number 3: Why is that knowledge important for you and your patients? C. trachomatis is the most commonly reported sexually transmitted disease. First of all, the reason why it is important to treat a pregnant patient is to prevent infection transmission during vaginal delivery. If infection is present during delivery the newborn is at risk for developing conjunctivitis or pneumonia. The most effective therapy if the newborn develops either or both is oral erythromycin; Why can we just treat the newborns? It is not that simple. There are studies that have shown an increased risk of hypertrophic pyloric stenosis (IHPS), especially if the infant is treated before 2 weeks of life. How likely is an infant to get IHPS if treated with erythromycin? There is a study by Rosenman and associates that compared the use of erythromycin prophylaxis with watchful waiting in a hypothetic cohort of neonates exposed to C. trachomatis. For every30infants treated with erythromycin, one additional case of pyloric stenosis would occur. This would be quite a few infants if we let chlamydia go untreated and newborns requiring treatment.Pyloric stenosis in a nutshell is a disorder in which the pylorus or gastric outlet can become very narrow or even obstructed which leads to forceful vomiting and requires surgery to fix it.Another caveat to treating an infant is that Erythromycin is effective in up to 90 percent of cases of conjunctivitis and approximately 80 percent of cases of pneumonia caused by C. trachomatis. Therefore, the infant needs close monitoring and at times a second round of treatment. I want the listeners to think for a minute and Just ask your adult self: How fun is it to take antibiotics? The answer is most likely, NOT at all fun; there are always side effects and your GI system is usually the most affected. Now imagine how a tiny little baby must feel. I would NOT want to put an infant through this treatment unless it is ABSOLUTELY necessary, so if we can prevent it by testing and treating mothers then let’s do that! Question number 4: How did you get that knowledge? I mostly read UpToDate, AAFP, NEJM, and check the CDC website for updates. If I am not quite sure where I will be able to find a specific topic then sometimes I google what my question is and look through the list to see if any of the sources are reputable or if the articles are from a well known journal then I read the contents. Question number 5: Where did that knowledge come from? For this topic, I initially had the help of our host Dr Arreaza and then I read different articles in UpToDate, such as “Treatment of Chlamydia Trachomatis infection” and “Chlamydia trachomatis infection of the newborn”. I also read an AAFP article from American Family Physician “Chlamydia trachomatis exposure in newborn”, the CDC “Legal Status of Expedited Partner Therapy”, “Chlamydial infections”, “STDs Clinical Prevention Guidance” and finally Pubmed “Azithromycin in early infancy and pyloric stenosis.”____________________________OTHER SECTIONSSpeaking Medical: Cataplexyby Terrance McGillThe Medical word of the week is Cataplexy. Cataplexy is emotionally-triggered transient muscle weakness. Most episodes are triggered by strong, generally positive emotions such as laughter, joking, or excitement. Episodes may also be triggered by anger or grief in some individuals. Cataplexy develops within three to five years of the onset of sleepiness in 60 percent of people with narcolepsy. Remember the word of the week: Cataplexy, See you next week!____________________________Espanish Por Favor: Agruras by Yodaisy RodriguezThe Spanish word of the week is agruras. Agruras means heartburn (the medical term is pyrosis), and it is typically described as a burning sensation in the retrosternal area, most commonly experienced in the postprandial period, but can be used as well when trying to describe reflux. The scenario will be a patient saying: “Doctor, tengo agrugras”. Agruras is probably a common complaint among our “chili pepper lovers”. Patients with agruras may require additional evaluation if they have red flags, such as weight loss, hematemesis, loss of appetite, vomiting, or more.Now you know the Spanish word of the day, agruras, all you need to do now is asses your patient’s heartburn.____________________________For your Sanityby Fermin Garmendia and Terrance McGill—Doc, Doc—Who’s there?—Disease—Disease who?—Disease the worse disaster I’ve ever seenWhat’s the anesthesiologist’s ABC? Don’t forget to treat sexual partner (or partners) to prevent chlamydia reinfection. It is permissible to send a prescription for the partner, even if you have not seen them. Cataplexy is an interesting symptom in narcolepsy, although it’s uncommon, you need to recognize it when you see it. Shree Parthasarathy, CIO & National Leader of Cyber Risk Services for Deloitte India, and Sharon Rosenman, VP Marketing for Cyberbit presented a webinar on the future of Cybersecurity training on March 11, 2019. Parthasarathy set the tone for the webinar with the statistic: there will be 3.5 million global unfulfilled cybersecurity jobs by 2021. In India, the demand for cybersecurity professionals tripled in the past 12 months, with 15,000-30,000 open cybersecurity jobs. One of the key issues in hiring is applicants lack hands on experience. Parthasarathy then discussed the current context of cyber training & awareness in Enterprises. Parthasarathy said, “Skilling, not trainings, is essential to overcome the skills gap.” Parthasarathy detailed the Deloitte Cyber Academy's simulation-based skilling approach, and Rosenman discussed the Cyber Range Platform. These discussions included cyber range examples. “And what we believe is important,” said Parthasarathy, “whether we operate in government, military, academia, industry, is to really focus on the skilling. It's going to be extremely important for all of these particular stakeholders to continue to basically move up in their skilling maturity as we take this forward.” Also covered in the webinar was the benefits of Cyber Range Training & Education. In this webinar from February 2019, Sharon Rosenman, Cyberbit's VP of Marketing was joined by Alon Nachmany, Cyberbit ICS Security Expert, and Cecil Pineda, former CISO of DFW Airport and CISO/Managing Director of Cyber Watch Systems. Nachmany discussed visibility and the OT security challenge. He mentioned several examples of major attacks on ICS / OT systems from 2013-2018. Pineda discussed his prior experience working in cyber security at DFW airport and laid out the numerous systems at risk. “Think of the airport cyber security, not just the cybersecurity side, the IT and the OT side; it's almost like an orchestra, everything has to be in synch with each other," Pineda said. Pineda continued his presentation by presenting the technical challenges of multiple IT, OT and IoT Systems in a typical airport, and how he and his team addressed those challenges. Next, Rosenman and Nachmany covered Securing the Converged IT/OT Environment. Nachmany walked through several key steps, which included: Enhancing IT Security, revisiting network architecture, obtaining full OT visibility, creating a baseline for OT anomaly, and consolidating OT and IT incident management with SOA. The webinar concluded with a Q&A session with all three speakers.

The financial industry has an image problem; capitalism functions by creating both wealth and poverty. But for financiers, the problem of poverty might just be reimagined as an investment opportunity. "I'm looking at how - the way that finance works in cities is to create a lot of inequalities [and this] is starting to be incorporated back into finance; as people who are in control of capital are looking for both new things to invest in and also looking at a lot of bad PR from this [capitalist] process". Dr Emily Rosenman Dr Sophie Webber is talking to Dr Emily Rosenman from Penn State University about how social finance investment works, and whether it is actually possibly to reduce poverty while making money. Social finance claims to solve social problems emerging from impoverishment and inequality; while spinning a profit at the same time. So what is social impact investment? It’s a way for governments to outsource and incentivise the services that are provided to address social disadvantage, such as poverty. Under this model of social service provision, the government draws up a contract with an investor to address a social issue, say, housing poverty, outlines a set of social targets, and if the social program meets these targets, then the investor gets paid. Guest Dr Emily Rosenman is an Assistant Professor in the Department of Geography at Penn State University. She is an urban and economic geographer who researches relationships between finance, impoverishment, and urbanization. Her current work focuses on philanthropy and other financial instruments that work to counteract urban sociospatial inequalities reinforced or created through the financialization of urban space. Sheridon Stokes Interview. Sheridon Stokes was hired as a piccolo player in the Denver Symphony when he was 16 years old and was the youngest contract musician in Hollywood at age 20. At this young age, he was working with the 20th Century Fox Orchestra under Alfred Newman. Star Trek IV and Leonard Rosenman. With a career that spans five decades and includes more than 100 films and television productions, Leonard Rosenman was once regarded as one of the most promising young composers in America—yet he was never a Hollywood favorite. He sometimes publicly chastised producers and directors for what he felt was musical ignorance and a lack of taste, but managed to make his voice heard by using film scoring as a way to experiment with advanced musical ideas. Some of the memorable movies that he worked on include East of Eden, Rebel without a Cause, A Man Called Horse, The Car, 1978's animated The Lord of the Rings, The Jazz Singer, and RoboCop 2. Rosenman made his acting debut in Gus Van Sant's MILK playing David Goodstein (founder of “The Advocate”) opposite the Oscar-winning Sean Penn as “Harvey Milk.” He has since acted in six more movies and is a proud member of SAG-AFTRA. His current film projects in development include: BETSY and THE EMPEROR starring Al Pacino as Napoleon, AMERICAN NEUROTIC at Sony Pictures, CALL ME BY YOUR NAME to be presented by James Ivory of legendary “Merchant–Ivory” fame and directed by Mr. Ivory and Luca Guadagnino. Peter Soears is his partner. Additionally, Rosenman is currently preparing MATTER OF SIZE, a remake of Israel's most successful comedy, released in 2009, at Paramount Pictures with Jon Turtletaub directing. He is also preparing a movie called SHEPHERD: A Tale of a Dog in World War II, based on the best-selling Israeli novel with Lynn Roth writing and directing. He just sold a Mini-Series to David Ellison's Skydance TV, six hours of television based on Michael Oren's book THE SIX DAY WAR with Rob Eshman writing. Rosenman is also putting together a film based on the life of ANITA BRYANT written by Chad Hodge, to be directed by Rob Epstein and Jeffrey Friedman, starring Uma Thurman and Zach Quinto. Darren Star and Jeffrey Schwarz are his partners. He just returned from Israel where he set up his cousin, Ora Ahimeir's best-selling book, BRIDE (a Sapir nominated book, about their great-grandparents, set in Mandatory Safed and Hungary in the Nineteen Forties) at Herzeliya Studios. It will be his first movie in Hebrew and will be filmed in Israel. In addition Rosenman is preparing a Broadway Musical based on a dream he had in 1985 after having seen PHANTOM OF THE OPERA and LES MISERABLES in one day in London, ANNE RICE'S “VOCE.” It is being written by Craig Lucas with music by Lance Horne and Lisbeth Scott. His producing partners are Belinda Casas-Wells, Chuck Martinez and Allan Levey. In January 2009, Rosenman became a Contributing Editor of the Los Angeles Times Magazine. As an educator, Rosenman has lectured at USC's Stark Producing Program, UCLA, Yale, Columbia, NYU, Brandeis and AFI. Rosenman tours the world with his seminar “HOWARD ROSENMAN'S THE HOLLYWOOD SELL,” an inside look at producing for the screen in which he shares his insights and adventures with writers, actors, directors, producers, and anyone who loves film and TV. The seminar has been presented in Memphis, Omaha, Philadelphia, Boston, New York and Los Angeles with upcoming engagements in Seattle, Austin, San Francisco, South Africa and Israel. He is currently at work on his first book also entitled THE HOLLYWOOD SELL, which will be both a memoir and a how-to-produce guide for aspiring filmmakers. Rosenman is Co-Founder of Project Angel Food in LA, which provides meals-on-wheels for patients with terminal illnesses and is now one of the largest charities in Southern California. In 2009 Beth Chayim Chadashim awarded him with the Herman Humanitarian Award. He was honored by film organization Power Up as “one of 2010's ten amazing men in show business.” In 2013 he received the Inspiration Award from ORT America. He currently serves on the advisory board overseeing the construction of the new film school at his alma mater Brooklyn College, and just offered a faculty job there as a Distinguished Professor. Born in Brooklyn, NY and raised on Long Island, Rosenman is the son of seventh generation Israelis from Jerusalem. He graduated Magna Cum Laude from Brooklyn College with a degree in European Literature. In 1967, he took a leave from medical school to serve as an extern medic in the Six Day War as a part of the Israeli Defense Forces where he met his mentor Leonard Bernstein who encouraged him to go into show business.
