Check out today's questions: Arianne: Hi Dr. Cabral, First, I want to say thank you for all of your resources! I start every day with your podcast - it is truly the best out there! My question is regarding the medication, Leucovorin, and its use in treating Autism. I have a 3 year old son who is non-verbal and is moderately Autistic. His pediatrician has prescribed Leucovorin on a titration schedule where he will be monitored for its effects. To note my own lab results, I have MTHFR, high levels of mercury and aluminum, low electrolytes/minerals, anemia/low ferritin, and gut issues - likely these were passed along to my son. I plan to have him complete the labs now that he is old enough. What is your advice on Leucovorin and its safety & efficacy, also should I be concerned with potential fillers and aluminum in the drug? Sheena: Hi Dr C! Hope you and your team are well. I was just wondering if all bovine collagen has protein in it, even if it doesn't say it on the label? And if so, does the protein count towards my daily protein intake? Jonny: Hello Dr. Cabral, Recently done some mitochondrial testing and found that my complex 2+3 are operating at 16% activity, and complex 2 at 58%, so there unable to turn substrate into energy efficiently. I'm wondering if you have any advice. Currently dealing with CIRS, Sibo/dysbiosis/chronic stress/Trauma. Non starchy carbs make me jittery, as well as stressors in general, but it's not blood sugar, as I've worn monitors and my levels seem fairly ideal. I think it's the CIRS jitters, shakiness that I've heard you speak of. I just got your new book, so I'm going to read through that for any tips. Already taking adrenal soothe, working with a therapist, doing my best to lower the stressors in my life. I know 90-120 days for red blood cell/mitochondrial turnover, so things can take time. Based on a popular well known Christmas carol this episode reminds us about prescribing thoughtfully, recognising key red flags, and keeping often-missed diagnoses like bile acid diarrhoea, coeliac disease and liver disease on the radar. The episode also reinforces the importance of early-life microbiome influences and structured differential diagnosis for abdominal symptoms in primary care. Prescribing and de-prescribing • Taper PPIs rather than stopping abruptly to avoid rebound acid hypersecretion, driven by upregulated gastrin during PPI therapy. • Always link NSAID use and H. pylori status to ulcer risk, and remember: gastric ulcers typically cause pain with meals, duodenal ulcers 2–3 hours after eating. Diagnosis, tests and red flags • Use three coeliac test “groups”: serology (tTG/EMA, with total IgA checked), genetics (HLA‑DQ2/DQ8) and duodenal biopsies; ensure patients eat gluten for at least six weeks pre‑testing and to endoscopy. • Actively screen for GI red flags: dysphagia and weight loss (upper GI), PR bleeding and unexplained iron‑deficiency anaemia (lower GI), and escalate for urgent investigation. Practical tools and endoscopy indications • Use the Bristol Stool Chart (types 1–7) routinely in consultations to standardise conversations about stool form and avoid ambiguous “food analogies.” • Remember the three main indications for endoscopy: diagnostic (e.g. dyspepsia, chronic diarrhoea), surveillance (Barrett's, polyp follow‑up) and therapeutic (RFA/EMR in Barrett's, polyp removal). Conditions to consider and not miss • Keep bile acid diarrhoea prominent in the differential for IBS‑D: up to ~40% of IBS‑D patients may have it, particularly with ileal disease/resection, Crohn's, or post‑cholecystectomy. • Maintain a broad GI bleeding differential beyond cancer (e.g. gastritis, peptic ulcer, Mallory–Weiss tear, haemorrhoids/fissures, liver disease/coagulopathy, IBD, angiodysplasia, diverticular disease). Liver disease, microbiome and early life • Remember major causes of liver failure in primary care: excess alcohol, paracetamol overdose, DILI, autoimmune hepatitis, Wilson's disease, haemochromatosis, viral hepatitis B/C and progressive MASLD. • Support breastfeeding where possible to promote a healthy infant microbiome (HMOs favouring bifidobacteria) and recognise how birth mode and early microbes shape immune development and later allergy/immune risk. Trying to improve gut health with fiberIncreasing fiber is one of the worst things you can do for your gut, yet it's commonly recommended. If your gut is damaged, fiber can exacerbate the issue, contributing to gas and inflammation. If you have gut inflammation, the carnivore diet can help!The carnivore diet helps increase glutamine, an amino acid that promotes gut healing. Zinc carnosine and butyrate, found in butter, are excellent for a healthy gut. Intermittent fasting and periodic prolonged fasting are crucial for maintaining digestive health.Approximately 40% of the population has SIBO. L. Reuteri can help reduce SIBO and has many other amazing benefits. It increases oxytocin, lowers stres, and even increases social capacity. Ensure you have strong stomach acid for healthy digestion and protection from microbes. 2. Commercial probiotics and yogurt The amount of probiotics in these products is so low that it won't make a difference in gut health. Many commercial yogurts are low-fat and contain added sugar and starch. 3. Colon cleansingThis eliminates beneficial gut bacteria, which can ultimately lead to constipation and other health issues.
