Our healthcare system is a complex dance of costs and inefficiencies, yet it's one we can't afford to sit out. What results have flowed from shifting the cost of healthcare to employers, and how have attempts to change that system sometimes backfired in unexpected ways?Vivian Lee is a healthcare executive, an Executive Fellow at Harvard Business School, and an author. Her latest book is called, The Long Fix: Solving America's Health Care Crisis with Strategies that Work for Everyone. Vivian and Greg discuss the disconnect between healthcare consumers and payers and dissecting the employer-based insurance model. They scrutinize payment models and incentives, discussing the stark consequences of shifting healthcare costs to employees and the resultant avoidance of crucial preventive care. Vivian talks about the financial motivations of health providers versus patient demands, creating a landscape where escalating costs and quality care seem to be in constant conflict.*unSILOed Podcast is produced by University FM.*Episode Quotes:Why we need to know where our dollars are going in healthcare22:19: I think it's very important that everyone really understands a little bit about healthcare and where their dollars are going. When you look at the graphs, all of us would be doing a lot better if our healthcare expenditures were lower. It's just hidden from us because it's taken out before we even get our paychecks, because our employers are using those dollars that should have been going to our pay raises for the last 20–25 years and using it instead to pay for healthcare costs. I think we need to know it for a number of reasons. One, because we need (we need was deleted in the audio-perhaps restore) the public to move forward to be able to improve healthcare payment policies in this country. We have to act through the way in which we vote. We also need it because we need to understand better our own actions. What of our behaviors are most important for our own health outcomes? And so having a more nuanced understanding of what we can do in terms of our daily behaviors that can impact our own health, I think, would be also really, really important. The US lacking universal healthcare for everybody is a key factor in its healthcare economy05:28: In health care, we are responsible for caring for people when they show up to us, and we can't decline people just because they don't have health insurance. So, as a result, we also have this strange subsidization model within health care where we have to overcharge some people in order to cover the costs of care for those who aren't covered.Money affects change in healthcare16:49: Any time we want to change the system, it's going to be hard. It is hard because there's already so many dollars in healthcare and there's so many vested interests who, naturally, if they're doing well, want to maintain the status quo. So change is always hard, especially in healthcare, because of how much money is already involved. How can we maximize the data from digital health systems37:11: Increasingly, we have the opportunity to use the data from all the operating rooms that are going on in the country and in the world every single day, where we have information about how surgeons are doing things in particular ways with different kinds of patients. We know about the different patients, we know about what the surgeons are doing, and we also know about the outcomes that happen six months, a year later, two years later. We've been collecting these data with our digital health systems for a long time now. What we haven't been doing is using the data to create more evidence to say, "Oh, actually, we really do know that doing it exactly this way with this artificial hip provides really good outcomes," and that would be the evidence that you would need to talk to the surgeon or convince everybody to go to that standard approach.Show Links:Recommended Resources:RAND Study: Skin in the GameKFF Study: Health Care Debt In The U.S.: The Broad Consequences Of Medical And Dental BillsGuest Profile:VivianLeeMD.comLinkedIn ProfileWikipedia ProfileHer Work:The Long Fix: Solving America's Health Care Crisis with Strategies that Work for EveryoneCardiovascular MR Imaging: Physical Principles to Practical ProtocolsGenitourinary MRI, An Issue of Magnetic Resonance Imaging ClinicsSelect Topics in MR Imaging, An Issue of Magnetic Resonance Imaging Clinics

The World Wide Web launched in the public domain on April 30, 1993, a little over 30 years ago. It was a major technological leap forward for humanity. It was a game changer, full of possibility… and uncertainty.  Experts are reminding us a lot lately that artificial intelligence (AI) has also been around for many decades. Nevertheless, much like the Internet in the 1990s, ChatGPT becoming publicly available in November 2022 represents another paradigm shift for humanity and its relationship with technology. One billion ChatGPT web visits took place following its launch. According to PwC, AI is predicted to contribute $15.7 trillion to the global economy by 2030. Yes, the stakes are high. Yes, it's a game changer. Yes, it's full of possibility… and uncertainty. Last month, the International Monetary Fund (IMF) released a study predicting that AI will affect close to 40 percent of all jobs. For some, it will be beneficial, boosting their productivity. For almost everyone else, their jobs are at risk. This report was published as business and political leaders from around the world prepared to gather in Davos, Switzerland, for the World Economic Forum, where AI took center stage. Highlighting the apprehension around this “disruptive” technology, the response from governments has been surprisingly swift. A number of countries signed a declaration on the safe development of the technology at an AI Safety Summit hosted by the UK late last year. And we're seeing increased regulation around the world, including in the European Union, China, and the U.S., meaning in the world's largest economies.  As businesses across all sectors explore AI's potential, they must also wade through its unknowns and navigate evolving regulation. In other words, they must innovate and use AI responsibly. Our guest today is Jon Iwata. He is an Executive Fellow at the Yale School of Management where he co-leads a program studying the leadership implications of stakeholder capitalism. He also directs the Data & Trust Alliance, a not-for-profit organization established in 2020 by CEOs of major companies including American Express, Johnson & Johnson, Nike, Pfizer, Starbucks, and Walmart. The Alliance develops and promotes the adoption of responsible data and AI practices. Among his various accolades and accomplishments, Mr. Iwata is also the co-inventor of a U.S. patent for a nanotechnology and process for atomic-scale semiconductors. Resources: About Jon Iwata The Data & Trust Alliance AI Will Transform the Global Economy. The World Health Organization has declared climate change as "the greatest threat to global health in the 21st century." As our planet grapples with the accelerating impacts of climate change, it is crucial that we adopt a climate lens in the value transformation of our healthcare industry. The repercussions of climate change extend far beyond environmental shifts; they manifest in various health issues, including challenges with clean water access, increased allergens, respiratory diseases, heat induced illnesses, and the proliferation of infectious diseases. It's essential to acknowledge that while climate change affects everyone, the burden falls disproportionately on historically marginalized populations, highlighting the interconnectedness of climate impacts and social determinants of health in underserved communities. In our pursuit of delivering safe, effective, and efficient care amid the climate crisis, we as healthcare leaders also bear the responsibility to address the substantial greenhouse gas emissions generated by the sector. Accounting for nearly one fifth of the U.S. gross domestic product, the healthcare industry possesses considerable purchasing power that can be harnessed to steer the nation toward cleaner energy and a low carbon supply chain. Beyond mitigating environmental harm, embracing preventive models of care and enhancing care quality that lowers excess utilization naturally aligns with lower carbon footprints. Value-based care, therefore, can become a powerful catalyst in propelling us toward a net zero carbon future that will build a sustainable, resilient future for our planet. This week we are  interviewing Dr. Vivian Lee, a healthcare executive dedicated to the advancement of value-driven transformation in health and tackling climate change. Author of the acclaimed book,The Long Fix: Solving America's Health Care Crisis with Strategies that Work for Everyone, she is an Executive Fellow at Harvard Business School and Sr Lecturer at Harvard Med School. Prior to her Executive Fellowship at Harvard, she was the founding President of Verily Health Platforms, an Alphabet company combining a data-driven, people-first approach to precision health.  Dr. Lee is also a former health system CEO, medical school Dean and member of the National Academy of Medicine. She is regularly listed among Modern Healthcare's Most Influential Clinical Executives as well as Modern Healthcare's Most Influential People in Healthcare.   Episode Bookmarks: 01:30  The World Health Organization has declared climate change as "the greatest threat to global health in the 21st century." 02:00  Climate change burden falls disproportionately on historically marginalized populations. 02:30  The responsibility to address the substantial greenhouse gas emissions generated by the healthcare sector. 03:00  Introduction to Vivian Lee, M.D. 04:30  One in four deaths can be attributed to preventable environmental causes...and climate change is exacerbating these risks. 06:00  Climate change resilience as a lens for value-based transformation. 06:45  The irrefutable scientific evidence about the unprecedented levels of carbon dioxide in our atmosphere. 07:00  What are health care leaders going to do about this?  (The need to manage the crisis through an empowered workforce.) 08:30  Exposed vulnerabilities in our supply chain as a complication to address climate-related crises. 09:00  The healthcare industry is responsible for 8-10% of the overall carbon footprint of the country (more than twice the #2 country!) 10:00  How do we decarbonize healthcare in order to "do no harm"? 11:45  Pollution from health care–associated energy use results in an estimated 405,000 disability-adjusted life years annually (a burden comparable to that of preventable medical errors). 12:30  The strong business case for health care organizations to reduce their carbon footprint. 13:00  Tax credits offered by the Inflation Reduction Act ...

Welcome to the What's Next! Podcast with Tiffani Bova.    This week I'm thrilled to bring back a special episode of What's Next! Podcast where I capture a frank conversation with my friend and fellow Hawaiian, Guy Kawasaki. He's the Chief Evangelist at Canva, a brand ambassador for Mercedes-Benz, and an Executive Fellow at the Haas School of Business. He is also the author of The Art of the Start, The Art of Social Media, Enchantment, and nine other books.     THIS EPISODE IS PERFECT FOR… anyone who wants to up their social media game.    TODAY'S MAIN MESSAGE… even for an expert, there is still room to learn. Guy is dedicated to making a difference via social media every day by pushing the envelope and learning as he goes.    In the world of social media, we both believe that you have to be thoughtful with your posts. The days of simply showing up are over. You can't just tweet once and call it a day. Video has to be better. Graphics have to be better.     Guy, however, takes a more aggressive approach. He's comfortable posting about politics and difficult conversations that seemingly don't relate to his brand. He's less risk-averse and is willing to take chances on social media.     I, on the other hand, will craft a Tweet and not hit send. I won't Tweet something if the content doesn't represent my “brand,” if the intention isn't correct, or if it doesn't add value. I am more cautious.     There's no right or wrong way to approach social, just choices. Guy believes social media is similar to life: error correction and experimentation.    WHAT I LOVE MOST… Guy focuses on making a difference. He doesn't look back – he pushes forward to keep learning, experimenting, and growing. This forward-thinking is something we can all learn from.    Running Time: 37:32   Subscribe on iTunes   Find Tiffani on Social: Facebook Twitter LinkedIn   Find Guy Online Website  Twitter  Facebook

We're so excited to kick off our summer book club with this amazing book: The First, The Few, The Only. When we interviewed Deepa for this episode, we remember being so excited to talk to her about this book because, while we've talked about a lot of books written for White people to learn more about race and racism, it's not often that we come across business books that are written specifically for women of color. In fact, women of color still seem largely invisible in many ways in the workforce - but we're absolutely not.   So get ready to lean in (but maybe not in the way that you're used to hearing that phrase) and reimagine what a truly inclusive workplace could and should look like, from a perspective that you may not be hearing in your own. And, if what you hear is something you'd like to dive deeper into (we hope so!), please pick up the book and read it for yourself. We'd love to hear your thoughts once you do.   What to listen for: How this book is different from one written for white women Defining the term “woman of color” Five archetypes that many women of color take on in their places of work – do you recognize any of your colleagues? The view of solutions and the new rules of power – can we change the current structures of society, or do we need to break the system to start again?
