Thanks, Andrea   Mysterious BP Changes Christin writes:  Hi Robb and Nicki, I was looking for resources for a blood pressure question for my husband and while I found a few older show transcripts, nothing quite gave me a clear answer. My husband has seen a steady rise in his blood pressure over the last 2 years. In 2022 at his company health fair his BP was 108/72.  In 2023 it was 134/84. I just took it this evening (manually) and it was 152/100.  I know that consistent time of day readings can matter and for context his health fair readings were in the morning and the one I just took was about 8:45 pm, however he'd been sitting resting for a good 20 minutes on the couch watching tv. This is all confusing because his a1c and general blood sugar readings are all good/normal (I've done both some fasted and response readings to get an idea of how high he spikes with some foods). He's 5'10" about 175lbs so not really overweight.  He lifts weights 2x a week and we typically go hiking and/or walking 2x/week together so he's not getting much vigorous aerobic work over the light-moderate category.  HOwever, especially now that summer is here he's very active with yard work throughout the week, carrying heavy stuff and doing manual labor in the garden and stuff often for 1-2 hours at a time. He asked me if I thought he should reduce his sodium intake (he is a heavy salter of food) and after reviewing some of what you guys have talked about with past posts and some of Hubermans stuff I don't think that's the answer. I did however talk to him about his general junk food intake - he likes to snack on chips and does like his sweets - which he agreed and acknowledged that maybe he needs to reduce that.  He's had a stressful previous year at his job that was messing with his regular exercise regimen and forcing him to frequently stay late/work extra hours and so I do wonder how much this could be playing into things also.  Thankfully that has improved in the last 3ish months letting him get back to a more consistent schedule and workouts. Outside of these few things the only other thing I can think of to tell him is to try and add in more aerobic work to his week at least at a zone 2 level.  Other than that I'm stumped.  Is there anything you would suggest?  Any resources I should look in to?  What else can I tell him? He has no other health issues, we don't take any medication at all and despite his lack of formal aerobic work he can do a tough hike (6 miles with 2000' of elevation for example) without major fatigue or being totally dogged; that said I know that it's important to get regular aerobic work. Sometimes you're fortunate enough to meet someone by chance and your interaction — even if short, has a significant impact on your life. When I happened upon Tom Scannell in May of 2020 during a family trip to Pine Island, Florida, I knew I'd found a fellow freedom fighter in this humble yet extremely driven lifelong, extremely fit farmer. He gave us a tour of his Pineshine Farms, patiently answered my barrage of questions and very generously gave me a book that is now one of my favorite reads: Everything I Want to do is Illegal by Joel Salatin, owner of Polyface Farms.Tom is a rebel in the agricultural space. He had to unlearn most of what he was taught to grow “beyond organic” food in a regenerative fashion, and I admire his focuses on appreciating beauty in Nature and harmonizing with our natural resources. I also love that he's not afraid to “fail forward” nor speak the truth, whether it means exposing the ties between big agriculture and the chemical and pharmaceutical industries, discussing the war on real milk or pointing out the scary fact that just four meat processors control ninety percent of the United States industry. Salatin's book goes into excellent detail on this, as well as the ridiculous hurdles our government (funded by big ag) makes small farmers jump in order to run their operations. It's hair-pull-out maddening!Tom rightfully comments that in today's terribly busy and off track society that Americans “take better care of their cars than their bodies… and spend more on lawn care than growing food.” Do not despair as there is plenty of hope within his message! Tom also explains how the environment in which we live and the foods we consume can help us greatly in creating or regaining our health. Diet for Cystic Fibrosis [16:45] Leah says: Hi Rob and Nikkie, I am a listener of three years now. I am a current college student who just got married to my high school sweetheart of 6 years. I love listening to your podcast and I have learned so much, that it has enabled me to make my health an act of rebellion. Once I am not a broke college student, I will become a member of the Healthy Rebellion. Now on to the question. My husband, who is 23 years old, has cystic fibrosis and is interested in changing his diet. However, I think what is a healthy diet for me may not be a healthy diet for him. His entire life he has been encouraged to drink sodas and eat absolute junk food just to meet his daily caloric intake. CF patients on average have been told they need almost double the amount of calories a normal person needs a day (between 2,900-4,500). Because of this many CF patients develop diabetes in their later lives, which we are trying to avoid. (It runs in both our families) Two years ago he stopped drinking soda/coffee or anything with lots of sugar or caffeine, he also does not drink alcohol. He drinks about a gallon of water a day, and since getting married in July he has tried to significantly cut down on his processed carb intake. We have been eating mostly proteins and vegetables with rice, seasonal squash and potatoes. We live in North Carolina and we have some really great farmer's markets that I try to source most of our food from. He lifts weights 3-5 days a week, and we are both service industry workers, so we are on our feet for 6-10 hours a day. Since cutting most of the processed carbs out of his diet, he has lost about 10 pounds that he did not really need to lose. He is 5ft 6inch and weighs about 135lbs. He has since found it difficult to put on muscle, wake up in the morning, and maintain energy through out the day. I am wondering if all of this is because he is not taking in as many calories as he needs? Or maybe it's a lack of protein? With our current diet, he eats about 2,000 calories a day and averaging about 40-60grams of protein a day. He has two high fat snacks of either yogurt or cheese a day in order to take specific medicine he needs. This is in addition to his three meals per day and has often found he will get too full and feels sick if he eats more. How could he increase his caloric or protein intake with out feeling full too fast or eating absolute junk food? Any thoughts are appreciated, since most diet advice we have been given for people with cystic fibrosis, is to just eat high fat, high carb, and processed foods, and that is a very s.a.d. diet. ;) Thank you for all that you do to encourage a healthy diet and lifestyle. You do reach a younger audience! :)   2. Gaining Lean Mass And Inflammation [26:48] Alan says: Hello Robb, I have been resistance training consistently since I was 13 years old and I am 29 now. I do not struggle gaining weight when I dedicate myself to it, but I have noticed that when I bulk up, I also increase my symptoms of inflammation, mainly, eczema, skin redness, bloating, fatigue, and difficulty sleeping. I am 6'2", currently weigh 185 lbs, and weight train 3-5 days per week. I haven't measured my body fat in awhile, but I am assuming it is very low judging by what I see in the mirror. The last 2 and a half years I have been following the auto-immune protocol and have tinkered with carnivore as well. I feel the best I have felt in my life, but I have also lost weight, including both fat and lean mass. For the majority of my adult life I hovered around 200 lbs. which is 15 pounds heavier than now. I have tried to increase my calories to gain lean mass while still sticking to the auto-immune protocol, which works in increasing my lean mass, but even though the foods are the same, just the fact of eating more calories in general will almost always increase my symptoms. I tried bulking up while on a carnivore-ish diet, with similar inflammatory results although maybe not quite as bad as when I ate more plants while on AIP. I seem to have a limit of how much food I can eat before symptoms come up, which is at the maintenance level for my 185 lb body. I have also tried different styles of hypertrophy training with high volume and lower intensity, or lower volume and high intensity, both of which don't seem to make a difference in gaining muscle mass as much as dietary or caloric changes. My question is - is there any way for me to increase my calories or gain lean mass without having to suffer the consequences of increased inflammation in the process? Slow Metabolism [33:23] Beth says: Hi Robb and Nicki, Everywhere I look, there's talk about how to increase your metabolism, how to burn more fat etc. but no one that I can find is asking if this actually makes sense. We wouldn't want a car that's an inefficent gas guzzler, why would we want to put more fuel in our bodies than we have to? Even from an evolutionary perspective, I would think having a slow metabolism, a lower basal metabolic rate, would be advantageous. I get that the majority of the population is overweight and looking to lose it, but even in health and fitness communities I can't find any information on this. I personally would absolutely love to be able to cook and eat less without losing weight! So my questions are, might it be healthy to have a slower metabolism, and if so, are there any strategies to facilitate that? Baby's poor digestion [17:18] Taylor says: Hello, Nikki and Robb! I was hoping you may have some advice other than probiotics, since that seems to be the only advice I get on this topic and they don't seem to help. I have a 9 month old baby with a very sensitive digestive system. We did everything "right" when it came to introducing good bacteria: natural home birth, no antibiotics, EBF until introducing solids. I have to cut dairy while breastfeeding because he has a CMPI. Since introducing solids, he is only able to handle meat, eggs, fish, and avacado. Anything else gives him extremely painful gas and he will be up all night screaming. Should I continue to try to introduce new foods or hold off until a certain period of time to let his gut heal/develop? His current daily food intake will look something like this: egg fried in lard, sausage patty, avacado, fish roe, ground beef, chicken thigh, over cooked white potato, and orange slices on the rind so he just gets the juice and no fiber. Thank you for any advice you may have! 2. Beef Liver [21:02 Chris says: Robb, I like to eat beef liver a couple times per week. In the states, I was able to find grass-fed sources, either locally or through US Wellness Meats. However, I recently moved to Okinawa, Japan with the military and haven't found any local sources for grass-Fed or companies that will ship here. The grocery store carries frozen liver slices from Skylark meats, but they're not grass-fed. For that matter, I can't find any information on how Skylark raises their cattle, so presumably it's a large commercial operation. So, am I better off ordering some grass-fed beef liver capsules from a reputable company or do you think the liver from Skylark is okay, albeit suboptimal? 3. Essential Tremor [26:07] Jenny says: Hello, Robb and Nicki - Thank you for providing truthful and unbiased information on health.  I love your podcast and the Healthy Rebellion group. I am a healthy, 50 year old female.  I feel great but I have Essential Tremor.  I also have Celiac Disease.  Interestingly, I only know one other person with Celiac Disease, and he also has Essential Tremor.  Most of the time, my tremor does not bother me too much, and I have learned how to deal with it.  However, there are certain situations I am in once in a while where I would like to be able to reduce the tremor in my hands.  I have to do a pistol qualification course four times a year for my job, and I would love to reduce my tremor before the test.  I am drug tested for work and cannot risk using any CBD products, which is too bad because I have heard they can help with tremors.   I have a few questions for you. 1.) In your research regarding Essential Tremor, have you found a link between people with Celiac Disease also having Essential Tremor? 2.) If a cloth mask captures 70% of particles or 80%; you still have no idea whether asking a child to wear that fabric mask (and accepting that use will be imperfect) has a net health benefit to that child or others around them. Universities' Covid Policies Defy Science and Reason Parents and students should challenge dogma with data. https://bariweiss.substack.com/p/universities-covid-policies-defy Dr. Marty Makary is a professor at the Johns Hopkins School of Medicine. At these institutions of higher learning and thousands more, science is supposedly held in the highest esteem. So where is the scientific support for masking outdoors? Where is the scientific support for constantly testing fully vaccinated young people? Where is the support for the confinement of asymptomatic, young people who test positive for a virus to which they are already immune on a campus of other immune people? The data simply do not justify any of it. For the past two years, this country has imposed extensive, and often unnecessary, restrictions on over 54 million school-age children, even though they are the least likely group to suffer serious consequences of a Covid infection. Instead, we have damaged their education, kept them from seeing human faces, and treated them as vectors without a right to a normal childhood. College students are not the only young people we have harmed. A recent Brown University study found that “children born during the pandemic have significantly reduced verbal, motor, and overall cognitive performance compared to children born pre-pandemic.” The researchers pointed out that families of lower socioeconomic status were most affected. At the same time, adults have been allowed to socialize barefaced at bars, while children outdoors on playgrounds are still masked. Child Constipation [9:36] Katie says: Hi Robb and Nikki! As always, thank you for all you do. My 7 year old daughter started complaining of stomach pain a few months ago. She's always been a good eater and has been gluten free since age 3 (not celiac, but noticed many health benefits for the whole family from cutting it out), eats minimal amounts of processed garbage and her diet is full of high quality protein, healthy fats and real, whole food. Her stomach pain increased in both severity and frequency over the summer. We cut back on dairy, with moderate success. We had blood tests done—all normal with no apparent food sensitivities or any other blood marker abnormalities. Finally her doctor did imaging that showed significant constipation. I have tended towards constipation my whole life, and my mom has IBS-C, so there's definitely some familial tendencies. Her pediatrician suggested miralax to get things going, which we have been doing for the past several days, and she's certainly pooping now, but still has daily tummy aches. Her pediatrician said daily miralax wouldn't become habit forming and was perfectly safe for long term use, but this obviously doesn't seem like the ideal answer. Do you have any suggestions on what I should try to bring her some relief, both in the short term but also for her digestive system as a whole? Why would a 7 year old who eats a healthy diet develop such severe constipation? How concerned should I be about using something like Miralax and are there better alternatives for keeping things moving? All my best, Katie 2. Anaerobic Ketolysis? [14:57] Howdy, Wolfs! Question... From what I understand, the body can only metabolize fatty acids aerobically, but it can metabolize glucose both aerobically and anaerobically. What about ketone bodies? Can the body anaerobically metabolize ketone bodies? I've scoured the internet periodically for probably a couple of years looking for a definitive answer to this nagging question, but I always come up empty handed. I have no functional reason to ask this question - I'm just a nerdy personal trainer with a penchant for delving deep and an evolutionary biology/ancestral health bent. 3. Fasting and Fast Twitch Muscle [16:37] Bonni says: From https://body.io/intermittent-fasting-part-3-go-eat-yourself/ "macroautophagy is transient in nearly all tissues except fast twitch muscle[7, 11] ..... Prolonged fasting triggers autophagy to deteriorate muscle tissue faster than severing the nerve to the muscle[29, 30]. .....  The only thing IF does for autophagy is to drive it into destructive ranges for fast-twitch muscle." This is from an old post and the references don't work anymore - do you have any info to back up or refute the fasting /fast-twitch muscle claim? Would eating fat prohibit the muscle catabolism or would protein be needed?  thanks! 4. Finasteride for baldness? [22:43] Steven says: Hi Robb and Nikki, I recently visited the doctor for a checkup and she noticed that my hair is starting to thin up top. Sad day :( She recommended that I get on a medication called Finasteride, and said that if I start taking it now I can likely preserve my luscious locks. I did some digging online, and the testimonials I have found regarding side effects scare the bejesus out of me. Do you have any thoughts on this medication? And do you have any recommendations on the diet/lifestyle side of things to help keep my hair? For context, I am a 32-year-old male athlete (rock climber), and in excellent health. I follow a mostly paleo diet, with a few exceptions here and there. I eat plenty of protein (1 g per lb bodyweight) mostly from grass-fed beef, about 200-300 g of carbs per day to fuel my training and climbing, and plan to start taking more collagen supplements to support tendons, skin, and hopefully hair. I've been making beef bone broth in the instant pot (GAME CHANGER) and it's delish! I usually break my fasts with a cup, and I've saved the fat for cooking. I accidentally bought lamb bones once, and turns out they made amazing bone broth too. So now I wonder- is bone broth ACTUALLY THAT good for me? Which animal is best? [19:10] Kat says: Hi Robb and Nicki, Your podcast is still one of my favourite podcasts :) I did post my question on the Healthy Rebellion and had some helpful thoughts from other members but I wondered if it might be an interesting one to discuss on the podcast: Context: I eat a reasonably high fat meat based diet due to Sjogrens Syndrome having majorly affected my gut. Before I figured out what I could tolerate I lost a lot of weight and was actually warned my heart was under strain and beating too slowly, and I could die if I caught the normal flu. Now I have the weight back on and my heart is back to normal. So when I read things like this I sort of worry, and then talk myself out of worrying after remembering that mouse models aren't always translatable and so many studies have limitations in other ways. Any way, here's a discussion of the study. Thoughts and Feedback on IF [21:58] John says: I'm a long time listener and just wanted to give some feed back and thoughts on my own experiences. Recently you mentioned the mistakes many are making with doing intermittent fasting or one meal a day with muscle loss etc. I can say from my own experiences as a 49 year old man I tried it and it was magical at first but then I did lose muscle and strength and have gone back to a lower carb paleo/ancestral diet and feel much better. Strength, muscle tone, energy, libido, etc.. I think maybe the intermittent fasting thing has become or taken over from the CR or calorie restriction group with many or all of the usual side effects. Obviously fasting and intermittent fasting are great tools especially if some one is over weight or diabetic but we can look at fasting in religions and see that there are probably reasons its only certain times of the year. I don't know just wanted to hear your thoughts and say you as usual were right on. A recent Harvard study show exercise and not fasting is probably the key to health and longevity. LMHR [26:52] Patrick says: I'm a 42 year old male personal trainer in Texas. I'm 5'8", about 170 pounds and stay at around 5-8% body fat. I've been in pretty much the same shape for about a decade. About 3 years ago I learned about Dave Feldman's work after being denied life insurance for having high cholesterol. Initially I was worried since my total cholesterol had always been around 200, LDL 130 and HDL 80 with low triglycerides. But suddenly my total cholesterol had jumped to 380, LDL to 280 and HDL to 98 still with low triglycerides around 70. The only change to my diet had been to completely cut out all processed sugar and other processed carbs. Up to that point I'd been eating a pretty low carb diet but would still occasionally indulge in ice cream or something my wife baked. But after reading Nina Teicholz, Wired to Eat, Gary Taubes, etc, I wanted to see what a year with absolutely no cheating would feel like. About 4 months into this experiment I saw my cholesterol had skyrocketed and learned that I was an LMHR. This of course sent me on a journey down the cholesterol rabbit hole which as you know leads to dozens of other tunnels. Anyway its really interesting that literally the only change I made was cutting junk out completely and this was the result. Btw I've had 2 NMR tests, both had my particle number above 3000. My IR score was 25. They showed that have about 75% large type A LDL which are not associated with atherosclerosis. I've checked my insulin, homocysteine, hsCRP, LPa, and other markers all which have remained optimal. I lift, sprint, row and run. I eat lots of beef and lamb, low carb veggies and fruit. Conventional medicine says I'm at much higher risk for heart disease. Because I cut out sugar? Should we not paying more attention to LDL size or is it possible in an otherwise healthy individual, high LDL may not matter? 4. High Blood Pressure From Salt Consumption? [34:56] Rusty says: My diet, exercise, and the like all need work, but I'm wondering, generally, is there a scientific way of determining if high blood pressure is from salt consumption?  I know you're kind of on the supplemental end of the electrolytes, and it makes total sense for folks out there, in ketosis, and sweating... it's something I intend on doing more of. Is there a way of actually knowing how much salt you're holding onto, so you know you need to supplement or cut back? 5. Electrolytes & Long haul flights [39:04] Karyn says: Hello! I'm lucky enough to be traveling soon and have 2 x 15 hour flights with a 4 hour break between. I plan to stay low carb for the duration of the trip by bringing my own food, but I'm wondering how long haul flights might affect my electrolyte requirements? Using Milk for Calorie Surplus to Gain Weight [19:01] Andy says: Hi Robb and Nicki, I'm a new Healthy Rebellion member and love it. I'm 42, 5'6" and weigh 140. Followed Robb since 5+ years of crossfit in my mid-30s. I have maybe 5 pounds of fat I could lose around my waste. Eat low carb and zero sugar...mostly meat/eggs, fruit and nuts. No "comorbidities". I don't have a big appetite and my body defaults to 135-140 lbs. My parents are also really small. I've been thinking a lot about the importance of strength/muscle mass and aging. My dad has dropped probably 20 lbs and all of his muscle from early 60s to mid 70s. He's not physically active. Over the next few years I would like to get my weight up to a lean 155-160 and maintain at that weight as long as possible. If I starting aging at 135 lbs then I'm going to be in trouble. I've begun a Starting Strength style linear progression with the basic barbell lifts. Lift 3x per week and add weight to the bar each time. I have a real hard time eating enough meat to maintain the calorie surplus it's going to take to gain the weight. I don't want to do it via carbs (rice/pasta/etc). I digest whole milk fine. What are your thoughts on drinking 24-32 oz of milk per day to gain weight? Does pasteurized vs raw matter? Thoughts on long-term consumption? I'll probably have to keep it up to keep the weight on.  Any other suggestions to add 300-500 calories to my diet besides "suck it up" and eat more? Thanks. Andy 3.
