The conversation in this episode dives into suicide prevention, stigma, and the changes needed to get to Zero-Suicide. Synopsis: The Usual Place host Natasha Ann Zachariah hunts for new perspectives on issues that matter to young people. In Singapore, suicide remains the leading cause of death for young people aged between 10 and 29.  A 160-page white paper report called Project Hayat (meaning ‘life' in Malay) was launched in conjunction with World Suicide Prevention Day on Sept 10 by advocacy group SG Mental Health Matters. Project Hayat outlines a national suicide prevention strategy for Singapore, and laid out its own research findings and 23 recommendations for addressing the gaps in suicide prevention here.In this episode, host Natasha Ann Zachariah finds out how we could talk more openly about suicide matters among young people, and what more can be done to address the gaps in support. Le Bonheur Children's Hospital has treated patients as young as 5 years old for self-injury, suicidal thoughts, and suicide attempt. Those who attempt or die by suicide do so because they cannot imagine a better day. For more than 71 years, the journey to a healthier childhood has led thousands of children and their families to Le Bonheur. That's why Le Bonheur was one of only 16 hospitals in the country selected to develop a pediatric-specific approach to the youth mental health crisis, through a grant made possible by Children's Hospital Association, Cardinal Health, and Zero Suicide. Dr. Goldstein Grumet is Vice President for Suicide Prevention Strategy and Director of the Zero Suicide Institute at the Education Development Center. Her role with the organization has her integrating comprehensive suicide prevention approaches across sectors, settings, and ages and delivering solutions that address shared risk factors to improve long-term mental health and well-being. She has provided strategic guidance for the establishment and evolution of the Zero Suicide framework since its launch in 2012 and leads development of the resources that enable the effective implementation and dissemination of this framework. Dr. Goldstein Grumet has expertise in behavioral health transformation, state and local community suicide prevention, quality improvement, school mental health, and the use of evidence-based practices for suicide care in clinical settings. She has collaborated on numerous grants and publications about systems-based approaches to suicide prevention. Julie has a Ph.D. in Clinical Psychology from The George Washington University and completed a Postdoctoral Fellowship at The University of Maryland in school mental health. Dr. Goldstein Grumet lives in Silver Spring, Maryland, United States. TRIGGER WARNING: Please note that this episode contains a discussion of suicide, self-injurious behavior, depression and/or reference of other mental health disorders that may act as triggers.    A national project called Zero Suicide aims to address the issue of suicide in the medical and mental health systems. The effort is founded on the idea that suicide can be stopped and that systems of medical and mental health care can do more to do so. Seven elements make up the entire Zero Suicide prevention strategy: LEAD, TRAIN, IDENTIFY, ENGAGE, TREAT, TRANSITION and IMPROVE. These elements insure a practical comprehensive approach to suicide care. In this episode, Dr. Gaggino interviews Dr. Julie Goldstein Grumet. Dr. Grumet is a clinical psychologist and public health expert. Dr. Grumet is the director of the Zero Suicide Institute at EDC, Inc. and she developed the Zero Suicide Toolkit to assist health and behavioral care leaders and leadership teams in implementing the seven elements (mentioned above) of the initiative.   Her goal has been to develop the framework, tools, and work schedules required for the Zero Suicide effort to be implemented effectively.     [00:30 - 06:29] Opening Segment Dr. Gaggino introduces us to Dr. Julie Goldstein Grumet Dr. Grumet tells us about her background and how she came into the field Julie recognized that the healthcare system had been left out of the first national strategy for Suicide Prevention. Zero Suicide is an aspirational goal and a methodology for suicide prevention.  In 2012, The Action Alliance included ZS as part of the National Strategy to transform healthcare systems.  ZS is a bundle of strategies that include both clinical components and implementation components.  Strap on your seat belt, this is your roadmap to better care. LEAD, TRAIN, IDENTIFY, ENGAGE, TREAT, TRANSITION, IMPROVE.  You can start at any spot though would HIGHLY recommend LEAD then TRAIN!!!   Many of you are already doing IDENTIFY with PHQ screening as we were.  With the new BF guidelines to add suicide screening this is a perfect opportunity to use TRAIN to implement ASQ or CSSR-S screening. Clinical components include screening, risk assessment, safety planning, lethal means safety, care transitions, hand-offs and caring contacts.   Implementation components include training a “competent, caring and confident staff (us too!), continuous quality improvement/data collection, and fidelity to the model. ZS is a culture shift – the system/hospital/practice bears the responsibility to care for patients using evidence-based strategies that work.  These are not home-grown tools - all of the tools/methods are backed by research.  Consider the medical model -asthma, we know

This episode series highlights the work of clinicians of color in the field of integrated healthcare and healthcare innovation. Shadiase Jack LCSW-C, is a staff therapist at an outpatient mental health agency where she provides mental health therapy to children and adults. She willingly accepts challenges and pursues everything possible to bring speedy and workable outcomes. She embraces difficulty with the fist of courage, determination and strong will as failure is not an option. Jerrika Henderson joined the DBH program at CGI after obtaining a Masters in Clinical Mental Health Counseling from the University of Memphis. She currently works on a specialized team as a community therapist catering to underserved youth and their families where she provides in-home intensive intervention treatment as a way to decrease inpatient and residential admissions. Ms. Henderson is devoted to being a change agent, proactively providing support to all through the facilitation and organization of groups, therapy, training, and events all centered around subjects that promote well-rounded health. Ms. Henderson believes that through our commitment to lifelong learning and collaboration with people from all walks of life we are able to share ourselves with the world. She aspires to continue creating and normalizing mental and behavioral health by providing safe spaces for underserved communities. One thing Ms. Henderson wants everyone to always remember is that you may be bruised but you are never broken. Rhea Hill, LPC, DBH-C, provides counseling for adult individuals with depression and anxiety. Her areas of concentration include women's issues, codependency, self-esteem, domestic violence, stress- management, and relationship issues. She is licensed in Texas and has practiced counseling for eight years. CDR Sean Bennett is a Behavioral Health Clinical Consultant with IHSC BHU and works out of the San Diego Field Office. Prior to joining the team, CDR Bennett served as the Behavioral Health Team Lead at Otay Mesa Detention Center (San Diego). His previous assignment was with Indian Health Service (IHS) Headquarters in Rockville, MD where he worked as a Public Health Advisor in the Division of Behavioral Health. At IHS, Sean served as the Domestic Violence Prevention Initiative (DVPI) Program Official; Methamphetamine and Suicide Prevention Initiative (MSPI) Nashville Area Project Officer; and IHS National Lead for Zero Suicide. Jennifer Wims-Madden, MSN, PMHNP-BC is an Adult Psychiatric Mental Health Nurse Practitioner who has served in the mental health field since 2007. She has experience treating patients with various psychiatric disorders across the lifespan and across practice settings and also enjoys her teaching roles. She currently works full time at a Community Mental Health facility as a medical director and provides direct patient care and also provides outpatient services at her own private practice. Jennifer strives to provide a holistic, individualized, personalized, and comprehensive experience for every patient she cares for. Napoleon Harrington MA, LPC, DBH(c) is a Licensed Professional Counselor practicing clinical mental health at Ambassador Counseling & Resource Group in the Metropolitan Detroit area. He also serves as the vice-chair of the State of Michigan Board of Counseling, appointed by Governor Gretchen Whitmer. The episode describes how risk taking gradually creeps up due to suicide being a rare event, rare events cause confirmation bias, what I decided clinically was the right decision...This results in a Creep up of risk taking which is inevitably followed by a Crash down of risk taking when an untoward incident happens, after that risk taking Crawls along for a while. Reflective practice and mindful supervision of one's risk taking is the key grounding one's risk threshold.

This week we are continuing our detour from your regularly scheduled programming because there has been such chaos in the world and I know a lot of parents are struggling to figure out how to talk to their kids about what is going on and I wanted to try to help. So I reached out to a new friend of mine who is another podcasting pediatrician who had a great episode on this topic when those of us in Michigan were shaken by a school shooting in Oxford, MI that you may have seen on the national news.  I'm very grateful to Dr. Lia Gaggino for making time last minute to get together to record with me so we can get this out to you as soon as possible and try to help in some small way.Dr. Lia Gaggino (IG: @pediatricmeltdown, Twitter: @gagginol) is a primary care pediatrician practicing on the west side of Michigan for over 30 years. We need to find a balance between our need for new information and our ability to regulate our emotions. You will likely need some time with the tv off and the phones away to reconnect with loved ones to help you maintain your sanity. For younger kids, you have a little more control over what they consume, so try to limit their exposure to some of the scarier images if you can. Their main priority is going to be their safety, so you may have to offer them more reassurance than you are used to, to help them understand that they are out of harm's way or that you are going to be with them to help keep them safe.School aged kids may or may not want to talk about these scary images, but the best way to know is to ask them about it. Even if they don't take you up on the offer, they will know that you are available when they do have questions, whether it's about this issue or something else in the future. They may also manifest their fear with misbehavior or sleep challenges. So have some grace when those attention-seeking behaviors start getting worse.Teenagers may be looking for ways to give back, so channeling that energy into your local community or finding ways for them to contribute in some small way These conversations are never easy, but your kids will probably have questions, so knowing where reliable sources of information are when it comes to child health, like healthychildren.org from the American Academy of Pediatrics to know where to start. It's important to be truthful in your interactions with your kids at every age. Ohana Zero Suicide are a group of individuals from all over Ireland who have come together with a goal to reduce the suicide rate by promoting, protecting and preserving life. Their focus is primarily on changing the conversation around suicide in this country and beyond. They provide education that will enable anyone to identify the signs that someone may be suicidal, to employ the correct language in asking the appropriate questions and to ultimately direct that person to the appropriate help. Ohana Zero Suicide Awareness is a voluntary organisation which intends to help the general population potentially save a life - with online training to identify the signs that someone may be suicidal and to direct them to the best place for help. A person who is involved as a Director in this is Su Carty - a psychiatric nurse who is also a member of the World Rugby Council, an IRFU Committee member and a former player and referee. Su joined John Duggan for a chat around the training, their sports clubs intiative, and dealing with your mental health during the pandemic. Mercado Discusses Objective Zero Suicide Prevention App on Credit HourVERMILLION, S.D. – Major Chris Mercado, ’04, founder of Objective Zero, discussed his military service and suicide prevention efforts on the podcast Credit Hour. “What we’re doing with Objective Zero is crowdsourcing peer support,” said Mercado. “The idea is that we’re trying to get ahead of those downward spirals into suicide by providing peer support as a pre-crisis service.”Objective Zero was established in 2015. It released a mobile app in December of 2017 that instantly connects veterans-in-need to a community of fellow veterans, current service members and concerned citizens.Mercado mentioned the recent spike in suicide rates as an alarming trend amongst the military. “Early indications are that in 2020, suicide rates in the military are up over 2019 by 20 percent. In the U.S. army, suicide rates are up over 30 percent,” said Mercado. “It’s very concerning.”In 2017, Mercado was named the Military Times Service Member of the Year for the U.S. Army in recognition for founding Objective Zero, a suicide prevention platform. Recent work focuses on collaborating with gun owner groups on suicide prevention projects. In this area, she wrote a suicide prevention module used by over 1,000 firearm instructors nationwide, was one of the founders of the first Gun Shop Project, co-authored the original CALM-Online training, and co-PI'd a large clinical trial of lethal means counseling in the emergency department. Because information changes so rapidly, we are happy to report that by the time this show airs, the response rate is better (not enough) and the reporting of issues is also expanding.Dr. Indelicato (“Dr. I”) is a Licensed Clinical Psychologist and Suicidologist. She is the program coordinator for a Zero Suicide Initiative grant at a rural hospital on the Navajo Reservation. She instructs medical providers and healthcare professionals in safety planning and risk assessment; crisis debriefing; and, she is an expert consultant to inpatient and outpatient units, mobile health, and the Emergency Department. Dr. I coordinates postvention efforts and outreach services to the Navajo community. Dr. I has also appeared in a film by nDigiDreams, a Native women-owned film production that shares the stories of hope and healing of Indigenous people all over the world. The film is part of the Indian Health Service toolkit: Best and Promising Practices for the Implementation of Zero Suicide in Native America.The intersection of behavioral health initiatives and public health issues is a primary focus and passion of hers. She has extensive experience working with underserved, marginalized and at-risk populations including homeless, formerly incarcerated adults, adolescents in juvenile justice settings, SMI, LGBTQ+, as well as individuals who have endured severe trauma, neglect and/or abuse. She has also coached interdisciplinary inpatient psychiatric teams to build trauma-informed treatment practices in the milieu.During her career, Dr. I has supervised pre-doctoral psychology candidates and taught psychiatric residents evidence-based clinical interventions to treat suicidality, self-harm ideations and behaviors, severe mood and/or psychotic symptoms, Post-Traumatic Stress Disorder and Borderline Personality Disorder. She completed a Dialectical Behavioral Treatment (DBT) Certification with Behavioral Tech. David kindly talked with us following his Plenary Presentations and Workshops at the conference so that those of you who couldn't be there in person could hear some of his key points and learnings. David Covington, LPC, MBA serves as CEO and President of RI International, is a partner in Behavioral Health Link, co-founder of CrisisTech 360 and leads the international initiatives “Crisis Now,” “Zero Suicide” and “Peer 2.0.” He is a two-time national winner of the Council of State Governments Innovations Award, in 2008 with the Georgia Crisis & Access Line and again in 2012 with Magellan Health. For five consecutive years, he competed as a national finalist in innovations competitions, including Harvard’s Innovations in American Government in 2009, and was featured in Business Week magazine (Innovate Timeline). He is an acclaimed global speaker and blogger having visited 35 countries (Where in the World Is?), with top-ranked TED-style Talks and conference keynotes. Previously he served as Vice-President at Magellan Health responsible for the executive and clinical operations of the $750 million per year contract with Arizona Medicaid. Mr. Covington is a member of the Department of Health and Human Services (HHS) Interdepartmental Serious Mental Illness Coordinating Committee (ISMICC) established in 2017 in accordance with the 21st Century Cures Act to report to Congress on advances in behavioural health. Mr. Covington is the President-Elect of the American Association of Suicidology and has served on its Board of Directors since 2014. He has served on the National Action Alliance for Suicide Prevention Executive Committee since it was created in 2010, co-chairing task forces on clinical care and crisis services (Research Publications). In 2016, he became the Chair of the National Suicide Prevention Lifeline SAMHSA Steering Committee after serving as Vice-Chair from its creation in 2005. He also served on the National Council for Behavioral Health Board of Directors from 2011 to 2014. Mr. Covington’s management history also includes CEO of Behavioral Health Link and Director of Public Sector Quality Management at APS Healthcare. He is a licensed professional counsellor and has an MBA from Kennesaw State and a Master’s of Science from the University of Memphis. This podcast is the beginning of a new occasional series where we interview Masters in Suicide Prevention. Jerry Reed, PhD, MSW, Education Development Center’s Senior Vice President for Practice Leadership and Lead for the U.S. Division’s Suicide, Violence, and Injury Prevention Portfolio, shares his professional journey from the early days of his career experience to his current position of being a well-known leader and expert in the suicide, violence, and injury prevention fields. Dr. Reed shares the background behind development of the National Strategy for Suicide Prevention (NSSP) and the launch of the National Action Alliance for Suicide Prevention that ranged from the 1990s to today. Dr. Reed speaks to the importance of how sharing stories of personal loss coupled with action requests truly moved forward national policy on suicide prevention and how these stories of individuals with personal lived experience with ideation and attempts continue to advance our policy and practice today. Dr. Reed then explores ways to advance the 2012 revised NSSP, and thus reduce suicide, to include practical examples of what governments; businesses and employers; health care systems, injurers, and clinicians; schools, colleges, and universities; non-profit, community- and faith-based organizations; and individuals and families can do. The Zero Suicide initiative is explored as a very specific example for advancing Goal 8: Promote suicide prevention as a core component of health care services. Finally, Dr. Reed provided an overview of the major priorities of the National Action Alliance for Suicide Prevention in effort to advance the NSSP, particularly to reduce the national suicide rate by 20% by 2025. A national coordinated response with additional resources are needed to attain that goal. We should celebrate the progress and continue to intentionally focus on this issue with time and resources such that eventually any person can use any door to access services when in a time of distress.

The Summit has become a regular part of the IIMHL program. Leading experts in healthcare and suicide prevention sought to progress the ‘Zero Suicide’ initiative, targeting suicide prevention and behavioural health care systems where a cooperative system wide engagement seeks to achieve a target of zero suicides in the health care system (inspired by the Henry Ford Health System in Detroit). Suicide Prevention Australia is specifically looking at what Zero Suicide in HealthCare implementation looks like if rolled out on a wider scale in Australia. Seventy delegates representing 15 countries attended the match. Guest speakers include Dr Michael Hogan, David Covington, Kevin Hines, Jan Mokkenstorm, David Jobes and Virna Little. The summit agenda focused on introducing the zero suicide mission and approach and lessons learned thus far, then built the discussion around the following modules - Clinical pathways, Engaging peer supports, Research and evaluation.
