Editorial note This report was produced by Rethink Priorities during December 2022 and January 2023. The project was commissioned and supported by Open Philanthropy, which does not necessarily endorse our conclusions. The report focuses on the development story and impact of some global health R&D ‘hits' or ‘success stories' of the last two decades: injectable artesunate, rectal artesunate, Coartem Dispersible, long-lasting insecticidal nets, and dolutegravir. We did not classify these developments as ‘hits' ourselves, or compare them to any other treatments, but we were asked to look into them based on the expectation that they have saved or could save many lives, and could constitute some of the most successful treatments of the past couple of decades. We analyzed the steps that led to their development, with a particular focus on the roles different stakeholders played. Additionally, we estimated the number of deaths these products have averted to date and might avert in the future. We have tried to flag major sources of uncertainty in the report and are open to revising our views as more information becomes available. Executive summary In this report, we attempted to reconstruct the development story behind five global health R&D hits of the last two decades, four types of antimalarial treatments and one antiretroviral used for HIV, and estimate the number of deaths each of them has averted to date and might avert in total. The latter ranges from a few tens of thousands (for rectal artesunate) to a few million (for injectable artesunate and dolutegravir). You can find below two plots (Figures 1 and 2) summarizing our models' estimates for the lives saved by each of these products since their introduction and until obsolescence (you can also find the plots and associated data here). Figure 1: Cumulative number of deaths averted by each product since its introduction Figure 2: Total number of deaths averted by each product until obsolescence In most of the examples studied, the drugs were developed through partnerships involving: (1) a pharmaceutical company that carried out the development of the product, (2) academics who ran the trials to show the product's effects and/or compare it to the standard of care, and (3) philanthropic and public actors that provided funding for various stages. Philanthropic funds were also used in some cases to support local manufacturers and help them get WHO approval. One notable exception is dolutegravir, for which the development seems to have been entirely funded by industry. You can find here a table including the partners involved in the development of each of the products, and the cost and funders for different parts of the process where we were able to find the information. As noted above, philanthropic funds seem to have been pivotal to the development of all products except for dolutegravir. Even though the cost incurred by the pharmaceutical companies is unknown, and likely to be quite large, it seems as though these partnerships are quite frequent and necessary when developing products for diseases mainly affecting low and middle income countries. For all cases in which philanthropic funding played a significant role, the funds needed seem to have been in the range of a few million dollars to the low tens of millions. It should be noted that the total funds these organizations invested toward R&D of products related to these diseases is likely much larger, including funds devoted to these particular drugs through unsuccessful initiatives we were not able to find, and funds devoted to the development of other drugs that were unsuccessful (or just less succe...

In 2008, a teenager was hiking through a forest in California when he decided to stop and set up camp for the night. He walked several hundred feet off the trail into the woods where he found a small clearing in the trees, and set up camp there. On the morning of August 10th, 1991, in a little rural town in Arizona, two women walked across a parking lot toward a Buddhist temple. They were there to prepare a meal for the monks who lived inside. As they walked, the women noticed that none of the monks were outside meditating or gardening, which was strange. And, when they reached the front door, not only was it locked, which was strange, but also, there was also this huge puddle of water forming in front of it which they just couldn't understand. In 1999 there was a rash of disappearances inside of the famous Yosemite National Park in California. When the police couldn't figure out where these people had gone, or who (or what) was behind their disappearances, a rumor started amongst locals that a "monster" must be running loose in the park attacking people. At first, this was just an urban legend, but by the end of the year, police would discover there really was a monster in the park, and they really were behind the disappearances. In 2012, a man living in Omaha, Nebraska met a woman on a dating website and after dinner, he invited her into his apartment. As they started getting intimate, they heard a knock on the door. Annoyed, the man got up and went to look through the peep hole, and he couldn't believe who he saw standing on the other side of the door. One morning in 1983, a truck driver was driving down a lonely Alaskan highway, when he saw a young girl come bounding out of the forest that butted up against the road. The girl ran out onto the highway, turned toward him, and began sprinting in his direction while screaming and waving her arms in the air. The driver came to a quick stop and as the girl got closer, he saw she had no shoes on and had a handcuff dangling from one of her wrists. When she reached his vehicle she threw herself inside and screamed at him to drive. The driver had no idea who she was or what was wrong, but it was obvious she was in serious trouble, so he didn't ask questions and just hit the accelerator. It would turn out this young girl had been kidnapped, and that morning she was being transported out to an isolated cabin in the middle of the Alaskan wilderness to play a "game." She had managed to escape, but before she did, her captor had told her what this game entailed, believing she would die before she could tell anyone about it. But of course, she lived, and after the truck driver brought her home, she spoke to police and told them all the horrible details of the wicked game in the woods. On March 20, 1995, a crumpled up newspaper was spotted lying on the floor of a Tokyo, Japan train car. In Japan, the subways are always immaculately clean, so this newspaper stood out like a sore thumb. As commuters stared at the trash, wondering who would be so inconsiderate to litter, a wet spot began to form at the center of the newspaper, like it was sitting on top of a liquid that was now starting to seep through. On Christmas Eve in 1992, an 18 year old woman named Danita Gullette stepped outside of her place of work into the freezing Dayton, Ohio night air and began walking toward her mother's house. After a few minutes, Danita decided to call her mother to ask her something that couldn't wait until she arrived. So Danita stopped at a payphone, slipped a few coins into the phone's coin slot, then dialed the number she knew by heart. At her mother's house the phone started to ring, but when Danita's sister picked up the receiver, the phone was silent. In 2012, a young man drove his truck into an Anchorage Alaska parking lot and came to a stop next to a small, bright teal colored shack. This shack was a coffee shop, and this young man worked there as a barista. That morning he had a shift, so he hopped out of his truck, went around to the side of the structure and put his keys into the door's lock. When he turned them, he noticed there was no resistance, the door was already unlocked. Confused, he pushed the door open and saw right away the place had clearly been robbed. He called his boss, who in turn called the police, and then they asked to review the shop's security footage from the night before. Sicily has a long history, and all of it is tied up with the evolution of wine and food in the Sicilian culture. In this show, we look at how this huge Mediterranean island played a major role in every major civilization from indigenous tribes to the current generation of young winemakers who seek to carve out a niche for Sicily and its unique wine culture. Here's a brief timeline of what we talk about:  Sicilian Wine Timeline... 10,000 years ago: Natural grapevines on Etna Indigenous groups – Siculi, Socani, Elymi (Greeks who brought wine to Sicily) Photo: Metropolitan Museum of Art 8th – 3rd century BCE: Greeks arrived, introduced grapes and planted a lot of vineyards. They introduced pruning, varietal selection, bush training, and techniques to make great wine. Wine became an economic essential, as Sicily's strategic position allows Greeks to export wine all over the Mediterranean. Inzolia, Zibibbo, Lucido/Catarratto were brought from Greece. 3rd century BCE: Roman Republic wins control of Sicily over the Greeks. The Roman Empire reigns afterwards. During both eras, the Romans planted more grapes, refined viticulture and winemaking techniques and traded Sicilian wine throughout the Roman empire, enriching wine merchants on Sicily. Mamertino, Julius Caesar's favorite wine was made in Sicily. Wine vessels from Sicily have been found in France and other parts of Europe. Photo: National Gallery Open Access 535 AD –826 AD: After the chaos that ensued after the fall of the Roman Empire, Byzantines conquered Sicily and used it as their base in the Mediterranean to take over other parts of Italy. The church revived viticulture and make wine for religious purposes and for trade around the Mediterranean.   826 AD –1061 AD: Muslim rule -- not great for wine, as it is against the law to consume alcohol. Viticulture did not prosper, but it didn't die. A few people still drank, and Z'bīb, Muscat of Alexandria, thrived as a table grape. The food and spices introduced during this time had a lasting impact on the cuisine of Sicily.   1061 AD –1189 AD: The Normans, Christian descendants from Vikings conquered Sicily and brought wine back to the table in full force. The rulers expanded vineyards and wine became an economic mainstay for the Normans – they traded it and it was part of life for the aristocracy so Sicilian wine had status. Rather than throw out the influence, the Normans incorporated Arab spices and cooking in their food. Vermicelli (pasta) likely was made here in 1154 AD, 100 years before Marco Polo was born.   1189 AD – 1266 AD: Norman rule ends and Henry VI of Swabia claims the throne.   1266 AD: Pope Clement IV puts Charles, Count of Anjou and Provence, on the throne in Sicily but in 1282 a French soldier insults a Sicilian girl on her way into a church for Vesper services. This sparks the uprising called the Sicilian Vespers, ending French rule.   1282: Peter II of Aragón (Spain) took control of Sicily. Wine was an important economic commondity as it was traded to northern winemaking areas to beef up their wines with color, flavor, and alcohol. Photo: Wikipedia 1400s-1500s: Guilds of wine merchants and growers flourished under the Aragón rule. Tomatoes, chocolate, squash, cactus, and other items were brought on Spanish ships from Mexico, revolutionizing the Sicilian cuisine.   1700s: The House of Bourbon, a power family from Spain who ruled in Sicily, invested in local wine again.   1773: John Woodhouse makes Marsala on the western side of the island, ships it out to England and the American colonies. Marsala was the first Italian wine to be exported America. Marsala was a major contributor to the Sicilian economy and to the islands prestige Photo credit: Dedda71, CC-BY-SA-3.0 1816: Naples and Sicily were united under the Aragón crown in the Kingdom of two Sicilies.   1861: Giuseppe Garibaldi claims Sicily as part of the Italian Republic, ending Aragón rule. The Risorgimento, Italian unification, was not beneficial to Sicily. They found it difficult to integrate into continental Italy.  The economy suffered, and the first great emigration out of Sicily, occurred, spreading of the cuisine and wine traditions around the world – to America, Australia, the UK, and other places.   Late 1800s: Mass plantings of vineyards became necessary to supply Europe with wine in the wake of phylloxera. This was a prosperous time for wine in Sicily until phylloxera hit the island. Due to economic restrictions, poverty, and the level of destruction from phylloxera, Sicily took about 60 years to properly recover from the aphid.   1950s: Sicily finally recovers from phylloxera. Vineyards mechanize, but in the post-World War II – global demand dropped for Sicilian wine.   1960s and 1970s: Again, Sicilian wines exported to bulk wines up from northern areas. Sicily's reputation for quality suffered.   1980s –1990s: Some older families on the island planted international grapes to garner international attention from critics, and build a reputation for good wine. There are so many causes or sources of AI risk that it's getting hard to keep them all in mind. I propose we keep a list of the main sources (that we know about), such that we can say that if none of these things happen, then we've mostly eliminated AI risk (as an existential risk) at least as far as we can determine. Here's a list that I spent a couple of hours enumerating and writing down. Did I miss anything important? Insufficient time/resources for AI safety (for example caused by intelligence explosion or AI race) Insufficient global coordination, leading to the above Misspecified or incorrectly learned goals/values Inner optimizers ML differentially accelerating easy to measure goals Paul Christiano's "influence-seeking behavior" (a combination of 3 and 4 above?) AI generally accelerating intellectual progress in a wrong direction (e.g., accelerating unsafe/risky technologies more than knowledge/wisdom about how to safely use those technologies) Metaethical error Metaphilosophical error Other kinds of philosophical errors in AI design (e.g., giving AI a wrong prior or decision theory) Other design/coding errors (e.g., accidentally putting a minus sign in front of utility function, supposedly corrigible AI not actually being corrigible) Doing acausal reasoning in a wrong way (e.g., failing to make good acausal trades, being acausally extorted, failing to acausally influence others who can be so influenced) Human-controlled AIs ending up with wrong values due to insufficient "metaphilosophical paternalism" Human-controlled AIs causing ethical disasters (e.g., large scale suffering that can't be "balanced out" later) prior to reaching moral/philosophical maturity Intentional corruption of human values Unintentional corruption of human values Mind crime (disvalue unintentionally incurred through morally relevant simulations in AIs' minds) Premature value lock-in (i.e., freezing one's current conception of what's good into a utility function) Extortion between AIs leading to vast disvalue Distributional shifts causing apparently safe/aligned AIs to stop being safe/aligned Value drift and other kinds of error as AIs self-modify, or AIs failing to solve value alignment for more advanced AIs Treacherous turn / loss of property rights due to insufficient competitiveness of humans & human-aligned AIs Gradual loss of influence due to insufficient competitiveness of humans & human-aligned AIs Utility maximizers / goal-directed AIs having an economic and/or military competitive advantage due to relative ease of cooperation/coordination, defense against value corruption and other forms of manipulation and attack, leading to one or more of the above In general, the most competitive type of AI being too hard to align or to safely use Computational resources being too cheap, leading to one or more of the above (With this post I mean to (among other things) re-emphasize the disjunctive nature of AI risk, but this list isn't fully disjunctive (i.e., some of the items are subcategories or causes of others), and I mostly gave a source of AI risk its own number in the list if it seemed important to make that source more salient. Maybe once we have a list of everything that is important, it would make sense to create a graph out of it.) They see two young men approach the rear of a convertible parked outside. One of the men gets into the driver’s side of the vehicle. The other man points a gun through the open window at the man sitting in the passenger’s seat. As the passenger exits the car, a shot is fired. Reporter Tony Rizzo covered the carjacking story and the trial for the Kansas City Star. The Star also did excellent coverage on the escape. Today we take on quite a lot: Who was Karl Marx, why did he criticize capitalism and want to overthrow it for socialism? Why did our strikeleaders take on Marx's philosophy? And anyway, who are the strikeleaders?Following an introduction to Karl Marx, the episode introduces Carl Skoglund, the "Old Swede," and the three Dunne Brothers, Ray, Miles, and Grant. Given Skoglund's and Ray Dunne's own membership in their early years, we also discuss the Industrial Workers of the World (Wobblies) and their political philosophy. We then turn to the Russian Revolution, the formation of the Communist Party, and its later split, and how all of this influenced the political scene of Minneapolis. My Case Study When my first group coaching program called ‘BAM Academy’ was launched (now called ‘Selling Like a Nerd’), I was able to get over 55 paying members for a 6-figure launch. I want to share with you the top places where those 55 members came from because I believe in not reinventing the wheel. If something’s working you just keep doing it. My buyers typically come from the same places, and the same is true for you. It’s supremely important for you to identify where your buyers come from, so you can rinse and repeat for continued results.   Numbers breakdown of the ‘BAM Academy’ Launch: Invested $14,000 in the launch 90 days between conception and completion of the launch 55 paying members $120,000 in revenue Where did these 55 paying members come from? I’m a work smarter, not harder marketing coach, I believe very strongly in focusing intently on highest potential buyers during your launch. Of course I wanted to connect with any potential buyer until I got a decision, but there were three main groups of people where my buyers came from. So with how busy I am, these are the places I needed to put my focus.   3 Main Sources of Buyers: 1st Place: Previous Buyers Before launch, I already had people who purchased from me before whether it was coaching services, the $47 upsell in my tele-summit or services from my VA company. This gave me a list of hundreds of people who I knew I could tap into. And this is key because if people buy from you once, they’re more likely to buy from you again. I invested a lot of time and energy with these prospects during my launch. Results here were 15 repeat buyers who purchased again during the launch. Lesson Learned: When you’re doing a launch, activate those potential buyers with entry level offers first. Create and sell a low cost offer first that will sell well, so you can generate a list of buyers who will invest in higher priced offers.   2nd Place: New Client Referrals I went into this launch without any affiliates, which would push me to take massive action on my own. My expectations were exceeded when 10 of my buyers came from client referrals. When someone joined the program, I asked them if they wanted to be an affiliate. I gave everyone who said “yes” swipe copy and graphics to use. These affiliates really delivered because they were inspired to bring in others. I used contests where the next person who made a sale would win a prize. But this wasn’t even necessary as they were giving me their support mainly because they believed in me. Lesson Learned: Create a superb experience with your tribe from day 1. The moment someone comes into your network treat them like gold.   3rd Place: My Own Facebook Group It’s great to create a tight tribe with your Facebook group, but another benefit is that 90% of my buyers across all my campaigns come from my Facebook group. This is because people can engage with you and get to know the real you there. Of the 55 people who joined my program, 47 of them had engaged with me in the Facebook group beforehand. Lesson Learned: When you can create the “know, like and trust factor,” selling can become so seamless that you don’t even have to get on the phone. Don’t just build a list, create a community instead.   Imagine if you were able to tap into these three places, and if you tapped into the highest potential buyers, how would that change your business? How much more seamless would it feel to make money because you know exactly where to go to find prospects? How much more of an impact would that make if you were able to tap into those places and get significantly more clients?   TODAY’S ACTION STEP: Identify the top three places your buyers are currently coming from.
