Killers of the Flower Moon has brought the world's attention to one of the most vile murder cases in American history, and at the center of it all was Mollie Kyle-Burkhart. Mollie would have grown up in the traditional way of her Osage ancestors, but by the time she was an adult, her family was wealthy, and she lived in a big home with white servants and with a white husband. Mollie lost all of her family members during the reign of terror, and I can't imagine what it must have been like to go through something that horrible. Visit your primary care provider for additional medical advice.What is vascular dementia?Vascular dementia is a condition that arises due to damage to blood vessels that reduce or block blood flow to the brain. A stroke can block an artery and result in various symptoms, such as changes in memory, thinking, or movement. Other conditions like high blood pressure or diabetes can also damage blood vessels and lead to memory or thinking problems over time.Vascular dementia (VaD) is a type of dementia that slowly worsens cognitive functions and is thought to be caused by vascular disease within the brain. Patients with VaD often exhibit symptoms similar to Alzheimer's disease (AD) patients. However, the changes in the brain are not due to Alzheimer's disease pathology (amyloid plaques and neurofibrillary tangles) but due to a chronic reduction in blood flow to the brain, eventually leading to dementia. Alzheimer's disease pathophysiology is very complex, and studies have shown that patients with AD can experience simultaneously several vascular issues that can affect cognitive function. For example, patients with AD may experience mini-strokes and have a reduction of the flow of oxygen and nutrients to the brain tissue. So, AD can be worsened by vascular factors as well, but the vascular factors are not the main problem in AD.Clinically, patients with VaD can appear very similar to those with AD, which makes it difficult to distinguish between the two diseases. Nevertheless, some clinical symptoms and brain imaging findings suggest that vascular disease is contributing to, if not entirely explaining, a patient's cognitive impairment.Epidemiology.In the US, VaD is the 2nd most common type of dementia (15-20% of cases). Prevalence increases with age (∼ 1–4% in patients ≥ 65 years.) People affected by vascular dementia typically start experiencing symptoms after age 65, although the risk is significantly higher for people in their 80s and 90s.EtiologyVaD may occur as a result of prolonged and severe cerebral ischemia of any etiology, primarily:Large artery occlusion (usually cortical ischemia) *Acute*Lacunar stroke (small vessel occlusion resulting in subcortical ischemia) *Acute/Subacute**Chronic* subcortical ischemiaRisk factors:Advanced ageHistory of strokeUnderlying conditions associated with cardiovascular disease:Chronic hypertensionDiabetesDyslipidemiaObesitySmokingClinical Features:Symptoms depend on the location of ischemic events and, therefore, vary widely amongst individuals, but a progressive impairment of daily life is common. Because of the diverse clinical picture, the term "vascular cognitive impairment" is gaining popularity over Vascular Dementia.Dementia due to small vessel disease:Symptoms tend to progress gradually or in a stepwise fashion and comparatively slower than in multi-infarct dementia.Generally associated with signs of subcortical pathology:Dementia due to large vessel disease Usually, sudden onsetMulti-infarct dementia: typically, stepwise deterioration Generally associated with signs of cortical pathology:Early symptomsReduced executive functioningLoss of visuospatial abilitiesConfusion ApathyMotor disorders (e.g., gait disturbance, urinary incontinence)Later symptomsImpaired memoryFurther cognitive decline: loss of judgment, disorientationMood disorders (e.g., euphoria, depression)Behavioral changes (e.g., aggressiveness)Advanced stages: further motor deterioration: dysphagia, dysarthriaDementia due to large vessel disease Usually, sudden onsetMulti-infarct dementia: typically, stepwise deterioration Generally associated with signs of cortical pathology:Cognitive impairment in combination with asymmetric or focal deficits (e.g., unilateral visual field defects, hemiparesis, Babinski reflex present)Overall, the symptoms vary depending on which areas of the brain are affected.Management and TreatmentThere is hope when it comes to managing the symptoms of vascular dementia. Although there is no cure for the condition, there are medications available that can help make life easier for those living with it. Additionally, there are drugs commonly used to treat memory issues in Alzheimer's disease that may be effective for individuals with vascular dementia. Sometimes, people with vascular dementia may experience mood changes, such as depression or irritability. These changes can be managed with medications used for depression or anxiety.Vascular risk modification: If your patient is experiencing cognitive impairment and has clinical or radiologic evidence of cerebrovascular pathology, getting screened for vascular risk factors, especially hypertension, is essential. Treatment can help prevent dementia, but it may not be as effective in reversing it. Statins are given after a stroke regardless of lipid levels.Antithrombotic therapy: For patients with vascular dementia who have had a clinical ischemic stroke or transient ischemic attack, they must receive the appropriate antithrombotic therapy based on the specific stroke subtype to help prevent any future ischemic strokes.When considering antiplatelet therapy for patients with vascular dementia who have not had a clinical ischemic stroke or TIA, it is important to make an individualized decision. For instance, we may prescribe aspirin at a dosage of 50-100 mg daily for patients with an infarction seen on brain imaging but not for those with only white matter lesions.Cholinesterase inhibitor therapy: It is recommended to start cholinesterase inhibitor therapy, such as donepezil or galantamine, for patients with vascular dementia who have a gradual cognitive decline that is not a direct result of a stroke. The evidence suggests that this treatment may offer a small cognitive benefit, but the clinical significance is unclear. Experts do not recommend cholinesterase inhibitors for patients with dementia diagnosed after a stroke if there is no gradual cognitive decline.Antipsychotics: We can briefly mention antipsychotics. They may be used but we have to remember they may increase mortality in the elderly, and the patient and family must be aware of this risk. Some examples are risperidone, quetiapine, and olanzapine, use them cautiously. Let's talk beyond medications, what other treatments can we offer? Non-pharmacologic options: In addition to medications, there are various ways to help a person with vascular dementia. Research has shown that physical exercise, sleep hygiene, and maintaining a healthy weight can not only enhance brain health but also reduce the risk of heart problems, stroke, and other diseases that affect blood vessels. Patients must be encouraged to eat a balanced diet, get enough sleep,limit alcohol intake, and encouraged to quit smoking, as these are other crucial ways to promote good brain health and reduce the risk of heart disease. Additionally, comorbid conditions such as diabetes, high blood pressure, or high cholesterol, must be treated, because they affect brain function and quality of life overall.It is essential to understand that emotional outbursts and personality changes can be caused by underlying brain disease and are not always intentional responses or reactions. When behavior problems overwhelm an individual, their family members, or friends, it is critical to seek support. Patient and caregiver support groups are helpful, offering a space to vent, grieve, and gain practical advice from others experiencing similar challenges. Exploring other sources of support, such as adult day programs, can also benefit caregivers and individuals affected by vascular dementia. Conclusion: Now we conclude episode number 163, “Vascular dementia basics.” Future Dr. Ruby explained that vascular dementia is mainly caused by an impaired circulation of blood and oxygen to certain areas in the brain. In a conversation with CancerNetwork®, Robert A. Winn, MD, spoke about current obstacles in achieving equitable care for patients with cancer and discussed how initiatives from community health centers and federal bodies alike may help increase access to anti-cancer therapy. Winn, director and Lipman Chair in Oncology at Virginia Commonwealth University (VCU) Massey Comprehensive Cancer Center, senior associate for Cancer Innovation, and professor of pulmonary disease and critical cancer medicine at VCU School of Medicine, began the discussion by defining health equity as a principle. By eliminating disparities that impact access to treatment for certain populations, it may be possible for all patients to receive the same kind of care and potentially experience the same outcomes as part of an “even playing field.” According to Winn, initiatives that may promote health equity in oncology include the ruling from the Centers for Medicare & Medicaid Services (CMS) allowing for the reimbursing of navigation services to help patients and their families access treatment for cancer and other serious diseases.1 Although Winn said that the field was trending towards the goal of health equity, he stated that more progress was necessary for biomarker testing in lung cancer and other malignancies. Additionally, increasing access to new treatments, technologies, and screening mechanisms across different communities represented another challenge concerning progress.  Winn also spoke about educating others on cancer-related disparities, including his colleagues and other resourceful groups in cancer care. He detailed his experiences with working on the yearly American Association for Cancer Research (AACR) Cancer Disparities Progress Reports since the first was published in 2020, which he described as an element that he hopes will educate people and spread awareness. In addition to collaboration among professional bodies including the Association of American Cancer Institutes and the American Society of Clinical Oncology, Winn highlighted local efforts in spreading awareness and improving progress towards health equity. “By not sitting on our heels and waiting for the community to come to us, but by going out to the community and talking about the importance of screening, talking about the importance of finding a new drug and what that means, and talking about these new technologies, we've actually gotten a very robust group of community members who are actually by our sides and actually making other people aware out in the community,” Winn said regarding Facts & Faith Fridays, an initiative from Massey Comprehensive Cancer Center in collaboration with local faith-based leaders designed to spread awareness of anti-cancer treatment.2 “That has resulted in very positive outcomes for us.” References 1.        CMS finalizes physician payment rule that advances health equity. After the 2024 Gastrointestinal Cancers Symposium, Jun Gong, MD, and Daneng Li, MD, sat down to discuss the most relevant trial data to have come from the conference. They convened for a live X Space hosted by CancerNetwork®. During the discussion, they covered different trials across the gastrointestinal space, which included those evaluating different disease states from hepatocellular carcinoma (HCC) to colorectal cancer (CRC), and those assessing circulating tumor DNA (ctDNA) dynamics. Gong, a hematologic oncologist focusing on gastrointestinal and genitourinary cancers at Cedars-Sinai Medical Center, and Li, an associate professor in the Department of Medical Oncology and Therapeutics Research at City of Hope, each gave their perspective on the clinical trial data and discussed if they had implemented any of these study treatments into clinical practice.  The studies they covered included:  1.        Phase 3 NETTER-2 Trial (NCT03972488)1: - Investigated lutetium Lu 177 dotatate (Lutathera) plus octreotide vs octreotide alone for advanced gastroenteropancreatic neuroendocrine tumors (GEP-NETs). - Lutetium Lu 177 significantly improved progression-free survival (PFS) and overall response rate (ORR) compared with octreotide alone. - The agent may be considered for patients with high-grade GEP-NETs who desire significant tumor shrinkage. 2.        Phase 3 EMERALD-1 Trial (NCT03778957)2: - Studied transarterial chemoembolization (TACE) plus durvalumab (Imfinzi) with or without bevacizumab (Avastin) for unresectable HCC. - Durvalumab/bevacizumab plus TACE improved PFS compared with placebo plus TACE. - TACE may be preferred over transarterial radioembolization (TARE) due to faster patient recovery. 3.        Phase 3 CheckMate-8HW Trial3: - Evaluated nivolumab (Opdivo) plus ipilimumab (Yervoy) vs chemotherapy for first-line treatment of microsatellite instability-high/mismatch repair deficient metastatic CRC. - Nivolumab/ipilimumab demonstrated superior PFS compared with chemotherapy. - Chemotherapy may no longer be the standard first-line treatment for this patient population. 4.        BESPOKE Study (NCT04264702)4: - Assessed the impact of minimal residual disease (MRD) detected by ctDNA on disease recurrence in patients with stage II and III CRC receiving adjuvant chemotherapy. - MRD positivity was associated with worse disease-free survival (DFS). - ctDNA clearance at 12 weeks indicated improved DFS.  5.        GALAXY Trial5: - ctDNA is a promising biomarker that can be used to predict recurrence in patients with CRC. - Patients with ctDNA-positive disease had a worse DFS than patients with ctDNA-negative disease. - This suggests that ctDNA may be useful for making treatment decisions, but more research is needed before it can be used in clinical practice. 6.        Phase 3 FRESCO-2 Trial (NCT04322539)6: - Fruquintinib (Fruzaqla) improved the quality of life in patients with metastatic CRC when combined with best supportive care and significantly improved quality-adjusted time without symptoms of disease or toxicity compared with placebo and best supportive care. - The study showed positive effects on PFS, response rate, disease control, and duration of response with the fruquintinib combination. - The findings from this trial supported the FDA approval of fruquintinib for metastatic CRC in November 2023.7 References 1.        Singh S, Halperin D, Myrehaug S, et al. On December 15, 1996, Baltimore police discovered the body of twenty-three-year-old Kimberly Spicer buried under a trailer on the property of a pallet factory in the city's southwest side. Later that day, police arrested Joseph Methainy, a man who lived and worked at the factory where Spicer's body was discovered. It didn't take long for police to solidly connect Methainy to the murders of two additional Baltimore women as well. In his wild confessions he claimed massive victim numbers and even cannibalism. In December 1989, Judge Robert Vance was killed in his Alabama home when he opened a package that contained a mail bomb. Two days later Civil rights attorney Robert Robinson was killed in a separate explosion in Georgia. In this first ever CE episode, we discuss the A-Zs of continuous glucose monitors (CGMs). In specific, our learning objective for the CE are: Describe commonly available types of continuous glucose monitors (CGMs) in the US market and the features and capabilities of these devices. Summarize the evidence and guideline recommendations for use of CGMs in the management of diabetes. Identify the role of the pharmacist in the selection of CGMs and provision of education to patients and providers. Interpret the ambulatory glucose profile (CGM data output) and recommend changes in antihyperglycemic regimen for a patient. Key Concepts There are two main types of stand-alone personal CGMs available in the US market – real-time (rtCGM) and intermittently scanning (isCGM). [1] These CGMs vary in their features such as sensor wear time, sensor warm up time, sensor application site, reader availability, approved age for use, fingerstick calibration, non-adjunctive FDA labeling, interconnectability with other technology such as insulin pumps, and drug interactions – these variabilities can be used in decision-making when selecting an appropriate CGM for a patient. [2-7] Based on the evidence for use, both types of CGMs (real-time and intermittently scanning) are recommended in patients with Type 1 and Type 2 diabetes who are on multiple-daily insulin or continuous insulin infusion (pump), patients with Type 2 diabetes on basal insulin therapy, and as adjunct use in patients with diabetes who are pregnant. The strength of recommendations in general is stronger for real-time CGMs than for intermittently scanning CGMs. [1,11] These recommendations are supported by the evidence that CGMs can help improve glucose control, reduce risk of hypoglycemia, diabetes-related hospitalizations, and patient/caregiver satisfaction. Pharmacists play an integral role in education, on-going support, data interpretation, and resulting disease management in patients who qualify for CGM use and providers who care for patients with diabetes. [14] The ambulatory glucose profile is a standardized data output that informs understanding of glucose trends. [15] The recommended goal for most patients is to maintain a glucose range between 70-180 mg/dL with at least 70% of time spent in this range with variability coefficient of no more than 36%. Visit your primary care provider for additional medical advice.The motivation for this episode was a recent publication by the American Academy of Pediatrics, on June 27, 2022, titled Policy Statement: Breastfeeding and the Use of Human Milk. During this episode, we included updated information along with other useful material.Duration of breastfeeding:The American Academy of Pediatrics (AAP), World Health Organization (WHO), and Center of Disease Control (CDC) recommend exclusive breastfeeding at least for the first 6 months, after which one can start to introduce complementary pureed foods. The US Department of Agriculture states that initiating complementary foods earlier than 6 months offers no benefit to the baby and can even be associated with a higher risk of overweight or obesity, especially if introduced before 4 months. Mothers are then encouraged to continue breastfeeding for at least one year and can further continue up to 2 years of age or longer - as long as mutually desired by mother and child. This is an update from previous recommendations regarding the duration of breastfeeding until 1 year of age.Composition of human breastmilk:As the sole source of nutrition for infants in the first 6 months of life, breast milk plays a critical role in development. Human milk has a unique composition of proteins, fats, and lactose, as well as vitamins, electrolytes, antimicrobial, anti-inflammatory immunoregulatory agents, and living leukocytes, all of which contribute to the developing immune system of the child. Breast milk is rich in Vitamins B1, B2, and B6, Vitamins C, A, E, Ca, Mg, phosphate, and folate. However, it is low in Vitamins K, D, B12, and iron, therefore supplementation of these nutrients is required. It is important for mothers to consume an adequate and healthy diet for their breastmilk to contain appropriate levels of these nutrients. Water-soluble and Fat-soluble vitamins can be low in breast milk if the mother has a deficiency. Selenium can be low if maternal serum levels are low. Dietary iodine deficiency may also be exacerbated by smoking; iron deficiency; and consumption of large amounts of foods that interfere with the production of thyroid hormones, known as goitrogens, including Brussels sprouts, kale, cabbage, cauliflower, and broccoli. Maternal diet:Mothers should consume iodine-rich foods, such as lean meat, eggs, dairy, beans, and lentils. It is important to choose a variety of whole grains, as well as fruits and vegetables, and continue taking multivitamins. Fun fact: Different foods will change the flavor of your breast milk. This will expose your baby to different tastes, which might help him or her more easily accept solid foods down the road! It is recommended that mothers consume 290 mcg of iodine and 550mg of choline a day. Is there anything that mothers should avoid in their diet?-Limit seafood: Although fish is a good source of protein and lean meat, it contains some mercury, which can be transferred to the baby's diet. High amounts of mercury can have an adverse effect on the baby's brain and nervous system.-Limit caffeine: Also, we know a lot of people love their morning dose of espresso! Low to moderate amounts, equivalent to 2-3 cups of coffee per day, do not adversely affect the infant. However, anything more than around 300 mg of caffeine can cause irritability, poor sleeping patterns, fussiness, and jitteriness. Remember! This also includes sodas, energy drinks, tea, and even chocolate! As a reminder, one cup of coffee can have 95mg of caffeine.Vegan mothers: Vegetarian/vegan mothers may have very limited amounts of vitamin B12 in their bodies, which can result in neurological damage to the baby. Iron levels may also be sparse since plant-based foods only contain non-heme iron, which is less absorbable than heme iron. The American Dietetic Association recommends supplementation of vitamin B12, iron, and other nutrients such as choline, zinc, iodine, or omega-3 fats. Benefits:For the baby: Studies show that exclusively breastfeeding for 6 months decreased rates of neonatal and infant mortality as well as pediatric disorders such as otitis media, diabetes mellitus, obesity, lower respiratory tract disorders, asthma, atopic dermatitis, sudden infant death syndrome (SIDS), severe diarrhea, and inflammatory bowel disease. The longer an infant is breastfed, the greater the protection from certain illnesses and long-term diseases. For the mother: The longer a mother breastfeeds, the greater the benefits to her health as well. Mothers who breastfeed experience a lower risk of hypertension, type 2 diabetes, and breast, ovarian, and endometrial cancers. Contraindications:-Alcohol: Having up to 1 drink per day is not harmful to the baby, especially if the mother waits at least 2 hours before feeding the infant. This allows time for the blood alcohol concentration in the breastmilk to decrease. Consuming more than 2 standard alcoholic drinks daily is highly discouraged.-Tobacco: Cigarette smoking, or the use of nicotine products, is associated with decreased production of milk, shorter lactation time, and an increased risk of SIDS, asthma, and other respiratory illnesses in infants. Therefore, mothers should be strongly encouraged to stop smoking and minimize secondhand exposure. We know it is very difficult for people to quit abruptly. While transitioning to cessation, mothers should be counseled to smoke right after breastfeeding to allow the greatest amount of time for nicotine to exit the body until the next feed. Other cessation alternatives such as the patch or gum can also be used during breastfeeding.Varenicline: No human data is available to assess the risk of infant harm, but it is likely excreted in the milk, no data on the assessment of milk production.-Other substances: Marijuana, opioids, amphetamine, cocaine, and other illicit drugs are contraindicated due to their effects on neurodevelopmental behaviors. If these substances have been used intrapartum or during breastfeeding, it is important to monitor the baby for Neonatal Abstinence Syndrome. Some symptoms include poor weight gain, tremors, high-pitched crying, stuffy nose, poor feeding/sucking, seizures, irritability, poor sleep, vomiting, and diarrhea.-Maternal infections: Breastfeeding is not contraindicated during most maternal infections. Some exceptions include HIV, Human T-cell lymphotropic virus type I or II, untreated brucellosis, Ebola virus, or active Herpetic lesions on the breast. Women with herpetic lesions may breastfeed from the unaffected breast. -Maternal medications: Medications are relatively safe for breastfed babies, but some contraindications include anticancer drugs, oral retinoids, lithium, iodine, and amiodarone. Mothers should go over their medication list with their primary physician.Pregnancy and Lactation Labeling Final Rule (PLLR): Classification of drugs according to their impact on pregnancy and breastfeeding (categories A, B, C, D, X) was started in 1979, but it was stopped in 2015 and replaced by the Pregnancy and Lactation Labeling Final Rule (PLLR). The former categories were replaced with narrative sections and subsections to include: Pregnancy (including labor and delivery), Lactation, and information for Females and Males of Reproductive Potential (pregnancy testing, contraception, infertility).Role of the physician and stigmas:It is well known that breastfeeding can strengthen the bond between the mother and her child. Therefore, when latching becomes a problem, mothers are quick to become discouraged. If this happens, pediatricians should educate the parents that many breastfeeding problems commonly arise between 4-7 days after birth. Sometimes, exclusive or any amount of breastfeeding is not always possible, despite the mother's best intentions. Chelsea and I continue our series about the Christian revivals in American history.Solomon Stoddard, a fiery preacher saw several revivals in his 60 years pastoring the church at Northampton, Massachusetts, but he embraced and expanded the Half-Way Covenant. Stoddard allowed open membership and open communion. The Northampton church grew to over 600 members, but over time the majority were likely unbelievers.Stoddard's grandson Jonathan Edwards would become the new pastor of the church. He would follow in the footsteps of revivalist preaching, with his famous sermon "Sinners in the Hands of an Angry God." Mountain goats spend winter above treeline on windswept ridges, while coastal populations migrate downhill to spend winters in low-elevation forested areas. Cumulative snow depth varies more than recently fallen snow, which could explain the negative effects of cumulative snow depth on movements and winter range size. Reduced movements during snowfalls, possibly related to low visibility, could also explain these results. Mountain goat ranges were smaller in late winter than early winter but increased with day length as winter advanced. This may be because increased day length enables mountain goats to be active for longer periods and thus travel longer distances.     Richard, J.H. et al. Chelsea and I start a new series about the Christian revivals in American history.The controversy over the recent "Asbury revival" drove us to ask the question "What is a revival?"Using a dictionary, Scripture, and a Spurgeon quote, we come up with a working definition of a revival. Then, we dive deep into the setting of the American colonies in the 1600's and early 1700's.As church life was decaying since the first generation of pilgrims covenanted the first Congregational churches in the Massachusetts Bay colony, pastors begin to look for revival.Congregationalist churches practiced infant baptism but only granted church membership to attendees who could evidence regeneration. As the first generation's children became adults but didn't become fully covenanted members, could their children be baptized? Could they partake of the Lord's Supper? Boston pastor Richard Mather argued that the children were proper covenant members in some sense. The question remained whether to baptize children of "unregenerate" members into the third generation.In 1662, Mather collaborated on what became known as the Half-Way Covenant to revive the church with a structural and political solution. The Imperial Japanese Army was born of, and found its conclusion in, violent conflict. Westernization had come to Japan during the mid to late nineteenth century and divided the nation, for a time reversing the unification efforts of the Tokugawa Shogunate. Consisting of warring clans and provinces governed by a militarily ordered form of government, feudal Japan was a nation designed from warfare, and warfare had long been an essential thread in the weave of Japanese society.
