Visit your primary care provider for additional medical advice.Di: Hi everyone, this is Di Tran, 4th year medical student from Ross university.  It's a pleasure to be back.  To be honest, this project is a part of teamwork of two medical students, myself and another 4th year, her name is XiYuan.  She came from the AUC. Unfortunately, due to personal matters she was unable to make it to the recording today which makes me feel really sad. Jessica: My name is Jessica Avila, MSIV, American University of the Caribbean.Di: The topic we will present today is Multiple Myeloma. Multiple myeloma is typically a rare disease and it's actually a type of blood cancer that affects plasma cells in the bone marrow.Jessica: Let's start with a case: A 66-year-old male comes to his family doctor for an annual health checkup. He is not in any acute distress but he reports that he has been feeling tired and weaker than usual for the last 3 months. He also noticed that he tends to bruise easily. He has a history of arthritis and chronic joint pain, but he thinks his back pain has gotten worse in the last couple of months. Upon checking his lab values, his family doctor found that he has a calcium level of 10.8 and a creatinine level of 1.2, which has increased from his baseline. Given all that information, what do you think his family doctor is suspecting? And what kind of tests she can order for further evaluation?Di: Those symptoms sound awfully familiar – are we talking about the CRAB? You know, the diagnostic criteria for Multiple Myeloma.Jessica: Exactly! Those are called “myeloma-defining events.” Do you remember what those are?Di: CRAB criteria comes in 4 flavors.  It's HYPERCALCEMIA with >1mg/dL, RENAL INSUFFICIENCY with serum creatinine >2mg/dL, ANEMIA with hemoglobin value 10% plasma cells, PLUS any one or more of the CRAB features, we can make the official diagnosis of multiple myeloma. Di:  Before we go deeper, let's back up a little bit and do a little background.  So, what do we know about the immunoglobulins, also known as antibodies? Back from years of studying from medical school, we know that the plasma cells are the ones that producing the antibodies that help fight infections.  There  are various kinds that come with various functions.  Each antibody is made up of 2 heavy chains and 2 light chains.  For heavy chains, we have A, D, E, G, M and for light chains we have Kappa and Lambda.Jessica: Usually, the 5 possible types of immunoglobulins for heavy chains would be written as IgG, IgA, IgD, IgE, and IgM.  And the most common type in the bloodstream is nonetheless the IgG. Di: What is multiple myeloma? In myeloma, all the abnormal plasma cells make the same type of antibody, the monoclonal antibody.  The cause of myeloma is unknown, but there are lots of studies and evidence that show a number of potential etiologies, including viral, genetic, and exposure to toxic chemicals, especially the Agent Orange, which is a chemical used as herbicide and defoliant. It was used as a chemical warfare by the U.S. military during the Vietnam War from 1961 to 1971.Jessica: We need to order some specific blood tests to see if there is elevated monoclonal proteins in the blood or urine. So, to begin with we'll need to take a very thorough history and physical exam. Next, we'll do labs, such as CBC, basic metabolic panel, calcium, serum beta-2 microglobulin, LDH, total protein, and some not so common tests: serum protein electrophoresis (SPEP), immunofixation of blood or urine (IFE), quantitative immunoglobulins (QIg), serum free light chain assay, and serum heavy/light chain ratio assay.If any of the results is abnormal, we should consider referring our patient to an oncologist.Di: Interesting! I read that Multiple Myeloma symptoms vary in different patients.  In fact, about 10-20% of patients with newly diagnosed myeloma do not have any symptoms at all.   Otherwise, classic symptomatic presentations are weakness, fatigue, increased bruising under the skin, reduced urine output, weakened bones that is likely prone to fractures, etc. And if multiple myeloma is highly suspected, a Bone Marrow biopsy should be done with testing for flow cytometry and fluorescent in situ hybridization (FISH). Actually, if any of the “Biomarkers of malignancy (SLIM)” is met we can also diagnose multiple myeloma even without the CRAB criteria. Jessica: The diagnosis is made if one or more of the following is found: >= 60% of clonal plasma cells on bone marrow biopsy, > 1 lytic bone lesion on MRI that is at least 5mm in size, or a biopsy confirmed plasmacytoma. Di: Imaging comes in at the final step especially if we able to find one or more sites of osteolytic bone destruction > 5mm on an MRI scan.Jessica: What if the bone marrow biopsy returns > 10% of monoclonal plasma cells, but our patient doesn't have either the CRAB or the Biomarker criteria? Di: That's actually a very good question, since Multiple Myeloma is part of a spectrum of plasma cell disorders. That's when smoldering myeloma comes into play. It is a precursor of active multiple myeloma. Smoldering myeloma is further categorized as high-risk or low-risk based on specific criteria.A less severe form is called Monoclonal Gammopathy of Undetermined Significance, or simply MGUS, with < 10% bone marrow involvement. Those are diagnoses we give once we rule out actual multiple myeloma, which are defined by the amount of M-protein in the serum.Jessica:  When to get started on treatment? Multiple Myeloma is on a spectrum of plasma cells proliferative disorders, starting from MGUS to Smoldering Myeloma, to Multiple Myeloma and to  Plasma Cell Leukemia.  Close supervision/active watching is enough for MGUS and low risk Smoldering Myeloma. But once it has progressed to high-risk smoldering myeloma or to active Multiple Myeloma, chemotherapy is usually required. Some situations may require emergent treatment to improve renal function, reduce hypercalcemia, and to prevent potential infections.Di: As of 2024, treatment of Multiple Myeloma comprises the Standard-of-Care approved by the FDA. In fact, the quadruple therapy is a combination of 4 different class of drugs that include a monoclonal antibody, a proteasome inhibitor, an immunomodulatory drug, and a steroid. Jessica: They are Darzalex (daratumumab), Velcade (bortezomib), Revlimid (lenalidomide) and dexamethasone.  Other treatment plans for Multiple Myeloma include chemotherapy, immunotherapy, radiation therapy (for plasmacytomas) and stem cell transplants. The patient will also be on prophylaxis acyclovir and Bactrim while on chemotherapy. Sometimes anticoagulants are also considered because the chemo increases the risk of venous thromboembolic events.Di: Although the disease is incurable, but with the advancing of novel therapies and clinical trials patients with multiple myeloma are able to live longer.  Problem is the majority of patients diagnosed with Multiple Myeloma are older adults (>65), the risk of falling is adding to multiple complications of the disease itself, such as bone density loss, pain, neurological compromises, distress and weakness.  Palliative care may come in help at any point in time throughout the course of treatment but is most often needed at the very end of the course. Jessica, can you give us a conclusion for this episode?Jessica: Multiple Myeloma may not be the most common cancer, but we have to be aware of the symptoms and keep it in our differential diagnosis for patients with bone pain, easy bruising, persistent severe headaches, unexplained renal dysfunction, and remember the CRAB: HyperCalcemia, Renal impairment, Anemia and Bone lesions.Even without trying, every night you go to bed a little wiser. Thanks for listening to Rio Bravo qWeek Podcast. Their firm specializes in brand development and brand design and does amazing work. We explored their confidence in easing into ownership, the challenges they've faced and how they've carved out success, and of course, we addressed the challenges in addressing the gender gap in agency ownership. Bill was a Quincy fire fighter for 32 years.  He once when he responded to a fire at Home Depot and learned that what was burning was a mixture of pool chemicals, fertilizers, defoliants and insecticides that were all stored together for the upcoming Spring and Summer season.  The fire consisted of a toxic compound to which all the firefighters who were in the area of the fire were exposed. When fire was over, all who responded were sent to area hospitals for blood draws to provide the city with a medical baseline from that time.  When all the draws were completed, an emergency department doctor came into the conference room where Bill and other firefighters were being held and gave them some somber news.  His words still ring in Bill's ears; “With what all of you were exposed to tonight, I would recommend that you get your blood tested regularly, because I can't say when, but I'm pretty confident that many of you are going to come down with some pretty nasty diseases.”   Fast forward to April 2016, after taking a photography class, Bill was walking and felt a strange electrical shock across his chest.  Having been an Emergency Medical Technician for over 20 years, he knew the symptoms were not that of a heart attack.  Bill later found out that the atrial valve in his heart got stuck in the open position, and he lost consciousness. When he lost consciousness, Bill was running on asphalt and face planted on the pavement.  Bill hit the ground with such force that the impact caused a veritable pre-cordial thump on his chest and closed the valve, allowing his heart to work properly again.  When he came to, he was rushed to a hospital where his atrial valve was replaced.  Bill was then sent home only to be called two weeks later and informed that the pathology of his tissue revealed that he had abnormalities and was immediately referred to Dana-Farber. Having been in the medical field for such a long time, Bill was embarrassed to have no idea of the level of severity and was shocked to learn he had multiple myeloma. According to Bill, after learning from his doctor about his cancer – his response, after taking a deep breath was, “well, I guess I'm going to lose some weight.”  Dr. Laubach looked quizzical at my response.  “You do understand that you have a serious cancer?” he replied.  Bill nodded my head, looked him in the eye and said, “I know, do you want me to cry? Stamp my feet? Punch the door? I'm still going to have it.”  Dr. Laubach shook his head and smiled.  “I've never had anyone respond to news like this in my career.  If you keep this attitude and outlook, you've got a great chance of beating this.”  Bill took his advice to heart, and even through my continual treatments of chemotherapy. Das Stück verteilt sich auf 18 Stationen und anschließenden Schlußakt. Sujets traités :  Coup d’envoi de la campagne de vaccination des plus de 75 ans, hier, en Alsace. 11 centres ont pu ouvrir leurs portes contre 15, initialement prévus. En effet, 4 communes ont dû reporter en raison d’un manque de vaccins. Un report de 10 jours qui concerne les centres de vaccination d'Illkirch, Lingolsheim, Schiltigheim et Wasselonne. Comme pour prouver que le vaccin n’est pas dangereux et indispensable pour sortir de la crise, les trois lauréats strasbourgeois du prix Nobel : Jean-Pierre Sauvage, 76 ans, Jules Hoffmann, 79 ans et Jean-Marie Lehn, 81 ans, ont été vaccinés en même temps, hier matin au CHU de Strasbourg. En Alsace, 1.089 malades sont hospitalisés à cause de la Covid-19, selon les derniers chiffres de Santé Publique France. Parmi eux, 120 sont en réanimation. Pour la seule journée d’hier, 14 décès sont à déplorer dans les hôpitaux alsaciens. Le corps sans vie du skieur de 26 ans, emporté par une avalanche dimanche, au Hohneck a été retrouvé hier. L’important dispositif déployé juste après l’accident avait été suspendu par la tombée de la nuit. L’autre skieur de 28 ans secouru dimanche en fin d’après-midi souffrait d’importants traumatismes faciaux. Ses jours ne sont pas en danger. Rupture de canalisation, hier matin, à Scherwiller. Elle a été provoquée par le froid. Première conséquence pour les habitants : une diminution de la pression au robinet. Puis, les techniciens de SDEA ont dû couper l’eau avant de la rétablir en fin d’après-midi. L’intervention a perturbé le trafic puisqu’une circulation alternée a été mise en place. La ministre de la Transition écologique confirme l’enfouissement définitif des déchets de Stocamine. Barbara Pompili donne son feu vert au confinement du site sans déstockage complémentaire. 42.000 tonnes de déchets qui resteront enfouis par 500 m de fond, à Wittelsheim. En parallèle, la ministre alloue 50 millions d’euros à un plan de protection de la nappe d’Alsace. Plan qui prévoit notamment l’extraction de 750 tonnes de lindane sur le site PCUK à Wintzenheim. “Nous ne voulons pas d’une dette écologique supplémentaire pour les générations futures”, a réagi le président de la Collectivité européenne d’Alsace, Frédéric Bierry, qui propose à la ministre que la CEA soit maître d’oeuvre des travaux de déstockage avec une aide de l’Etat de 456 millions. + de 80 commerces restent ouverts entre midi et 14h à Sélestat, une réponse à l’avancement du couvre feu à 18h, et pour faire face à ce nouveau coup dur pour ces commerçants déjà très éprouvés par la crise sanitaire. Abstract: Recent studies have established that one-trial-back decision policies (Win-Stay/Lose-Shift) and measures of reinforcement learning (RL), e.g. learning rate, can explain how animals perform two-armed bandit tasks. In many published studies, outcomes reverse after one option is selected repeatedly (e.g. 8 selections in a row), and the primary measure of performance is the number of reversals completed. Performance and Win-Stay likelihood are confounded by using recent performance to drive reversals. An alternative design reverses outcomes across options over fixed blocks of trials. We used this blocked design and tested rats in a spatial two-armed bandit task. We analyzed performance using Win-Stay/Lose-Shift (WSLS) metrics and a RL algorithm. We found that WSLS policies remain stable with increasing reward uncertainty, while choice accuracy decreases. Within test sessions, learning rates increased as rats adapted their strategies over the first few reversals but inverse temperature remains stable. We found that muscimol inactivation of medial orbital cortex (mOFC) mediates task performance and negative feedback sensitivity. Finally, we examined the role of the adrenergic system in bandit performance, and found yohimbine (2 mg/kg) dramatically decreased sensitivity to positive feedback, leading to decreases in accuracy and inverse temperature. These effects are partially dependent on a2 adrenergic receptors in OFC. Our findings demonstrate a correspondence between reward schedule, WSLS policies and RL metrics in a task design that is free of the confound between Wins and reversals, and that the noradrenergic influence of mOFC on WSLS policy is dissociable from the regions general role in cognitive flexibility. Abstract: This study examined how the medial frontal (MFC) and orbital frontal (OFC) cortices process reward information to guide behavior. We simultaneously recorded local field potentials in the two areas as rats consumed liquid sucrose rewards and examined how the areas collectively process reward information. Both areas exhibited a 4-8 Hz "theta" rhythm that was phase locked to the lick cycle. The rhythm similarly tracked shifts in sucrose concentrations and fluid volumes, suggesting that it is sensitive to general differences in reward magnitude. Differences between the MFC and OFC were noted, specifically that the rhythm varied with response vigor and absolute reward value in the MFC, but not the OFC. Our findings suggest that the MFC and OFC concurrently process reward information but have distinct roles in the control of consummatory behavior. Copy rights belong to original authors. Abstract: Toxoplasma gondii is widely reported to manipulate the behavior of its non-definitive hosts in ways that promote lethal interactions with the parasite's definitive feline hosts. Nonetheless, there is a lack of data on the association between T. gondii infection and costly behavioral interactions with felids in nature. Here, we report that three decades of field observations reveal T. gondii infected hyena cubs approach lions more closely than uninfected peers and have higher rates of lion mortality. Our findings support the hypothesis that T. gondii's manipulation of host boldness is an extended phenotype that promotes parasite transmission from intermediate hosts to feline predators. While upregulating hyena boldness toward lions might achieve this, it may also reflect a collateral influence of manipulative traits that evolved in other hosts (e.g., rodents). In either case, our findings corroborate the potential impacts of a globally distributed and generalist parasite (T. gondii) on fitness-related interaction with felids in a wild host. In case you haven’t heard, homelessness is booming in California. Some 130,000 Californians are homeless—which is nearly a quarter of the national total. And that rate of homelessness is among the highest in the country. Homelessness jumped 14 percent from 2016 to 2017 in California and city leaders took note. From 2017 to 2018, most of California’s major urban areas saw a slight decrease in homelessness—about 1 percent. But Santa Cruz saw an increase. This Surf City, boardwalk town in Central California now has the second highest per capita rate of homelessness in California. In this beach community of some 60,000 people, roughly 2,200 are homeless in Santa Cruz. And each night, The Salvation Army shelters nearly 10 percent of them. Captain Harold Laubach directs The Salvation Army’s efforts in Santa Cruz—which includes the corps, the church, four winter shelters, one being an encampment space that provides safe shelter outdoors, and a homeless navigation center. In today’s show, Captain Laubach takes us in side a homeless shelter to explain how it operates and how The Salvation Army aims to meet individual needs inside. He details how The Salvation Army partners with its neighbors to mitigate concerns and gain support. Captain Laubach explores the complex and complicated issue of homelessness—and the sometimes surprising reality of those who face it.
