In a recent LowCarbUSA®, hosts Doug Reynolds, MHP and Pam Devine, MHP spoke with Dr. Jeremiah Eisenschenk and Dr. Angie Ausban, two prominent practitioners from the Brainerd Lakes area in Minnesota, about the upcoming LowCarbUSA® Metabolic Health Conference in Brainerd Lakes. This one-day event, scheduled for September 21, aims to unite healthcare professionals and the public around the importance of metabolic health. Dr. Eisenschenk, a board-certified Family and Obesity Medicine physician, shared his journey into metabolic health, which began after attending a transformative lecture in 2016. Since then, he has co-founded the Metabolic Health Conference in Brainerd Lakes and Essentia Health – Brainerd Weight Management Department, bringing his expertise to both the hospital and community settings. "I saw some pretty impactful results, even in six months," he noted, reflecting on the positive changes in his patients' health through carbohydrate reduction approaches. Dr. Ausban, who has a background in Internal Medicine and Pediatrics, recounted her own evolution in understanding obesity and metabolic health. After co-founding the CentraCare Medical Weight Management program, she became certified in Obesity Medicine and later joined CRMC as the Medical Director of the Metabolic Health and Medical Weight Management Program. "It's not just obesity and diabetes; it's everything—heart disease, stroke, dementia," she emphasized, underscoring the widespread impact of insulin resistance. The one-day event is open to the public, but technical enough to also offer optional CME opportunities for healthcare professionals, carbohydrate reduction meals, and local vendors, fostering a collaborative environment between the community to address the growing metabolic health crisis.  The symposium will feature a diverse lineup of expert speakers, including: Robert Cywes, MD, PhD Gurpreet Padda, MD, MHP, MBA Mark Cucuzzella, MD, FAAFP, MHP Vera Tarman, MD Jeremiah Eisenschenk, MD Angelina Ausban, MD Shawn Roberts, MD. This event is unique for the Brainerd Lakes area, as four health systems have come together to support this initiative—a rare collaboration in today's competitive healthcare landscape. Dr. Eisenschenk highlighted the importance of this unity, saying, "If we can coalesce for this greater mission... any part of America, any part of the world, can do this." Dr Lily Johnston  Dr. Lily Johnston, MD, MPH, is the associate medical director for Nexus Healthspan, and is board-certified in both vascular and general surgery. Dr. Johnston received her MD from the University of California, San Diego; she completed her residency in general surgery at the University of Virginia in Charlottesville, and then completed a fellowship in vascular surgery at the Mayo Clinic in Rochester, MN. At 55, he was obese, weighing in at almost 300 lbs., and had high blood pressure and sleep apnea. Over the past few decades, Vic had repeatedly attempted to lose weight through calorie restriction, and although he had often lost significant amounts, he had never been able to sustain the weight loss. It finally occurred to him that cutting out refined carbs- such as rice, potato, and pasta- seemed to help with his weight loss efforts. She received her Bachelor of Science degree from UCLA and attended medical school at Loyola University Chicago-Stritch School of Medicine. She finished her Radiation Oncology Residency at the University of Wisconsin, Madison in 2000 and has been treating patients in the Chicagoland area since that time. She realized that something was missing from conventional medicine and did additional studies to improve outcomes with cancer but also saw many other diseases improve. She has done additional studies in the fields of Functional Medicine, Integrative Medicine, and the Terrain-Theory of cancer. She is a Terrain Certified Practitioner trained by Dr. Nasha Winters and is a founding member of the Society of Metabolic Health Practitioners. Dr Laura Buchanan  Dr. Laura Buchanan is a double board-certified physician in family and obesity medicine with a passion for using lifestyle modification as the first-line standard of care. She is also a certified metabolic health practitioner (MHP), and a Board and founding member of the Society of Metabolic Health Practitioners. She completed her family medicine residency at Wake Forest in 2022 and was a valedictorian of her class at the University of Florida College of Medicine. The mission that drives her is to help people age successfully. Dr. Johnston received her BA from Princeton University, and her MD from the University of California, San Diego. After a residency in General Surgery at the University of Virginia, she completed a Vascular Surgery Fellowship at the Mayo Clinic in Rochester, MN. Dr. Johnston also earned a Master's of Public Health at the Johns Hopkins University Bloomberg School of Public Health, and is accredited by the Society of Metabolic Health Practitioners. She is currently pursuing additional training and certification with the Nutrition Network and the Institute for Functional Medicine. Dr. Johnston has published over 25 peer-reviewed scientific articles, and given numerous presentations. She serves on the Quality Measures Committee and the Coding Committee for the Society for Vascular Surgery. Host Doug Reynolds welcomes Shashikant Iyengar and Anup Singh to the podcast to discuss their contributions to metabolic health in India, and the upcoming Metabolic Health India Conference slated to take place October 27-29, 2023.  Metabolic Health India will be available for live streaming across the globe.  This resource is being made available at no charge. Shashikant, who is the creator of the Metabolic Health India, was the first Metabolic Health Practitioner accredited by the Society of Metabolic Health Practitioners in India. Singh is the founder and CTO of the dLife.in Indian Diabetes Community, and he is working with Iyengar on the organization of this conference. Iyengar talks about his long career in the pharmaceutical industry before devoting his primary focus to metabolic health and therapeutic carbohydrate reduction. After being diagnosed with Type 2 diabetes in 2015, he went in search of alternative ways to manage the condition. That led him to low-carb, high-fat diets. With guidance from Singh, he saw his HbA1c levels drop from 7.2 to 5.2 in a month. Impressed with the results and armed with multiple certifications, he transitioned into health coaching in November 2022. Anup Singh, also a Type 2 diabetic, went a different route, adopting a 100 grams per day carb limit. Though he faced skepticism from the medical community, his HbA1c dropped to 5.0 in May 2011, underscoring the effectiveness of his approach. He emphasized that the challenges of adopting a low-carb diet in India are cultural, given the heavy reliance on carbohydrates in the traditional diet. Both guests discussed the cultural difficulties of implementing low-carb or keto diets in India. Iyengar pointed out that different Indian regions have unique dietary patterns, making it complicated to offer a one-size-fits-all solution. Anup stressed that while the western model of low-carb is difficult to implement in India, he has found success in adapting the approach to suit Indian palates. The podcast ended with a discussion of the upcoming Metabolic Health India Conference, which aims to bring more awareness and resources to the low-carb community in India. Both guests are enthusiastic about their roles in helping people navigate metabolic health challenges in India and look forward to sharing their experiences at the upcoming conference.

Vic Basmadjian  Vic Basmadjian is an advocate of low carbohydrate and ketogenic diets after recovering his own poor health. At 55, he was obese, weighing in at almost 300 lbs., and had high blood pressure and sleep apnea.  Over the past few decades, Vic had repeatedly attempted to lose weight through calorie restriction, and although he had often lost significant amounts, he had never been able to sustain the weight loss.  It finally occurred to him that cutting out refined carbs- such as rice, potato, and pasta- seemed to help with his weight loss efforts. She trained at Box Hill Hospital as a junior doctor and has been a General Practitioner since 2003. Originally she was born in Hong Kong and speaks fluent Cantonese. She has been a doctor for over 20 years, 15 of those practicing in Wantirna Mall Clinic. She has a special interest in obstetrics, women's health, paediatrics and recently nutrition and metabolic health.Her children are now 13 and 10 years old. A few years ago, after her kids stopped being babies, she wanted to lose some weight. She joined the gym, going 3 times a week. After 3 months, no weight was lost. Subsequently, the gym did an 8-week transformation challenge and she successfully achieved her goal of losing 4kg in weight and 4% body fat. She also took up running, doing the C25K run app. Parkrun changed her life. She fell in love with running and was introduced to lots of running friends and a supportive community.She progressed to endurance running and incorporating strength training. In the last few years, she progressed to completing two full marathons using the low carbohydrate approach.In the last 3-4 years, she has taken up special interests in nutrition and the treatment of obesity and metabolic syndrome. She has done extensive education with Low Carb Down Under and Nutrition Network. She is also affiliated with Diet Doctor, as a Diet Doctor Pro practitioner. She has done additional training with The Australasian College of Nutrition and Environmental Medicine (ACNEM). Currently, she is completing accreditation with The Society of Metabolic Health Practitioners. At 55, he was obese, weighing in at almost 300 lbs., and had high blood pressure and sleep apnea. Over the past few decades, Vic had repeatedly attempted to lose weight through calorie restriction, and although he had often lost significant amounts, he had never been able to sustain the weight loss. It finally occurred to him that cutting out refined carbs- such as rice, potato, and pasta- seemed to help with his weight loss efforts. Soon after, his son asked Vic if he knew anything about the ketogenic diet, and that led Vic to start doing his own research on the internet. Dr Philip Ovadia  After growing up in New York, Dr. Philip Ovadia graduated from the accelerated Pre-Med/Med program at the Pennsylvania State University and Jefferson Medical College (now Sidney Kimmel School of Medicine at Thomas Jefferson University). He then went on to complete a Residency in General Surgery at the University of Medicine and Dentistry in New Jersey - Robert Wood Johnson Medical School and a Fellowship in Cardiothoracic Surgery at Tufts- New England Medical School. Dr. Ovadia has practised Cardiothoracic Surgery in Beaver, PA and Clearwater, Florida. In 2020 he established Ovadia Cardiothoracic Surgery and now works as an independent contractor Cardiothoracic Surgeon in various locations throughout the United States. In an effort to overcome his lifelong struggle with obesity, Dr. Ovadia adopted a low- carbohydrate focused way of eating in 2015. He has maintained a weight loss of approximately 100 pounds and since March, 2019 has adopted a carnivorous way of eating. He has extensively researched the health benefits of low-carb with a focus on heart health through many hours of reading the medical literature, books and listening to podcasts, as well as personal discussions with many of the physician leaders and citizen scientists involved in the low-carb movement. After growing up in New York, Dr Ovadia graduated from the accelerated Pre-Med/Med program at the Pennsylvania State University and Jefferson Medical College (now Sidney Kimmel School of Medicine at Thomas Jefferson University). He then went on to complete a Residency on General Surgery at the University of Medicine and Dentistry in New Jersey - Robert Wood Johnson Medical School and a Fellowship in Cardiothoracic Surgery at Tufts-New England Medical School. Dr. Ovadia has practiced Cardiothoracic Surgery in Beaver, PA and Clearwater, Florida. In 2020 he established Ovadia Cardiothoracic Surgery and now works as an independent contractor Cardiothoracic Surgeon in various locations throughout the United States. In an effort to overcome his lifelong struggle with obesity, Dr. Ovadia adopted a low-carbohydrate focused way of eating in 2015. He has maintained a weight loss of approximately 100 pounds and since March, 2019, has adopted a carnivorous way of eating. He has extensively researched the health benefits of low-carb with a focus on heart health through many hours of reading the medical literature, books, and listening to podcasts, as well as personal discussions with many of the physician leaders, citizens, and scientists involved in the low-carb movement. By the way, I’m thinking about all of you as you are headed back to school. This morning was my son’s first day of High School and also his 15th birthday. He’s so hilarious. I have three very funny kids. It’s weird to call them kids since they are 20, 17 and 15. I hope your school year gets off to a great start. I’m sending lots of positive vibes out into the world to you. Also, I’m thinking about everyone and COVID as we are in another very serious wave of the pandemic. Health systems are completely stressed, ICU’s are full across the country, and it’s very serious. I want to encourage you to get the vaccine if you are able to. My thoughts and prayers are with everyone. This is so very tough. Last week was Week 5, “I Gotta Feeling” where I taught you to give yourself permission to feel your emotions instead of buffering with food. What an important skill. I love “More than a feeling” by Boston. Each week builds on the next, but you can start this week and get going on slaying your goals and learning the truth about your body’s natural weight set point. I wasn’t planning on addressing this, but there’s so much confusion, especially on TikTok, where creators are saying you are stuck at your current weight. Nope. Nope. Nope. Now, there’s absolutely nothing wrong with staying at your current weight. Let’s drown out our society’s ridiculous noise that perpetuates diet culture and isn’t healthy mentally or physically for any of us. So, if you don’t want to make changes. No problem. You get to decide. If you do want to create a healthier life, you can start wherever you want. Fueling our bodies with healthful balanced food that serves us is possible and there’s some great data about that. I recently saw an article in the NYT about a recent study out that looked at metabolism and is disrupting what we thought about metabolism as we age. So cool! There are four stages of metabolism in our lifetimes. This study is definitely disrupting the status quo thinking about metabolism. There’s only a handful of us doctors across the country and actually, globally, who are disrupting what doesn’t work. We don’t take sponsorships or ad money. There’s some amazing data. There’s so much frickin’ confusion out there, it makes me want to pull my hair out. Listen to experts, especially those of us physicians who are willing to speak out, say where healthcare has gone wrong, and are helping as many as people as possible to create a healthier path. What’s also powerful, is that there are some of us physicians who have also gone through our own weight loss journey. The importance is the focus on metabolic health and reversing insulin resistance. You can check out the Society of Metabolic Health Practitioners website at smhp.org. I’m a founding member and proud of it. Our bodies are status quo homeostasis machines. Our bodies adapt to what we are fed. Sure, it’s more complex than that. Making the change to cut back on processed carbs and then being consistent over time is going to show some major improvements in your metabolic health and help you to reverse insulin resistance. Let’s get to goal slaying. So, I’m a major goal slayer. My top strength is Activator and I can grind out a goal with diligence and persistence like nobody’s business. I can also make it pretty grueling on myself. I’ve learned the tools of self compassion and I created Make it fun to get it done and learned that goal slaying can be fun and also it’s not being hard on ourselves that works. It’s self-love superpower. When you learn to be kind to yourself no matter what and have your own back, it’s so powerful. You can do anything. All of this is practice. I’ll tell you the secret to any goal I’ve slayed. The key to any goal slaying is staying with your vision. It’s an elevated approach. It’s not about the how. It’s like looking up and taking a 40,000 foot view. You are the butterfly and not the caterpillar. Expect to feel self-doubt, confusion and overwhelm, fear of failure, victim and lots of discomfort along the way. There’s nothing going wrong. Discomfort is the currency of our dreams. Let’s get going with the week! Monday Goal Slaying Motivation: Choose the goal that commands your attention! What’s your biggest pain point? What’s the biggest obstacle in your way? What’s the trade off? Experience temporary discomfort now while achieving your goals and dreams or the long-term discomfort of not achieving your dream? Choose Your Goal Tuesday : Pick a 30-day goal. Choose one goal at a time. I’m sure you’re really excited, but choosing one goal at a time is very important. Otherwise, you will most likely indulge in overwhelm and that will keep you from taking action on any of your goals. Choose a goal that motivates you, so how important is achieving this goal and is there value in achieving this goal? How does this goal fit into the bigger picture of my life? Am I interested in the outcome? Drink more water? Cut back on processed carbs? Crush your sugar cravings? Work on self-trust eating? Get more organized? Increase your fitness? Get better sleep? What’s your goal and why do you want to achieve it? Why will I do it no matter what and who has your back as you are reaching your goal? SMART Goals Wednesday: What’s a measurable goal? You get to decide how you measure your success. What’s SMART stand for when it comes to setting goals? SMART is an acronym used for any type of goal setting. SMART goals help set you up for major goal slaying success! SMART stands for Specific (make your goal clear and figure out exactly where you will end up when you have achieved your goal) /Measurable (It’s important to figure out how you will measure your success so that you know when and can celebrate when you’ve achieved your goal. By this time, I will have completed and created (insert goal). I will know I’m done because of _______ You can also create your own measures of success. Did I show up as my authentic self? Did I coach with compassion? Did I learn something new? Did I hold space? Did I have fun? How? . /Attainable (make sure your goal is possible, If you set a goal that is outside of your realm of possibility, your brain will use that as evidence to criticize and that will cut down on your confidence. /Relevant means that the goal matters in the bigger picture of your life /Time Bound Take Massive Action Thursday: Now that you have your goal and your SMART goals, it’s time to get going. Self-trust is created by taking action. You are learning to show up for a goal you set for yourself. That’s super powerful. Write down everything you need to do to in the next 30 days to get to your goal. Break it down into steps. Make it Fun to Get it Done Friday: Since this is IME Community, I’m going to teach you my entire philosophy which is to Make it Fun to Get it Done. Start by owning your goal, then get yourself all amped up to take action like Dwight from The Office before a sales call when he was jamming out on air guitar in the backseat while Jim waited patiently outside the car for Dwight to do his thing. Superhero Saturday: Self-love is your superpower to achieve your goals and dreams. Some of you may not have even heard of Superman. You should watch the movie, “Superman” with Christopher Reeves, best Superman ever. Some of the special effects in that movie are hilarious. Clark Kent was Superman’s version of a nerdy base level human. When the world needed saving Clark Kent would go into a phone booth and come out as hottie Superman. Well, you are your own superhero. You are able to go into a phone booth and put on your self-love superpower cape and go save the world. No pressure! Sunday Strategizing: What are the obstacles in your way? What’s in between you and your goal? What’s hard about your goal? What do you need help with? Now, turn your list of obstacles into strategies. You can turn your obstacles into strategies which clears your path to achieve your goals. A strategy could be a skill you need to develop. Here’s an example: If your goal is to eat sugar in moderation, one obstacle may be “I have lots of urges and cravings to eat sugar.” Strategy examples are: “Even though my sugar cravings are conditioned, my brain can be conditioned to create a healthier pathway.” “ I will create a pause when I have an urge or craving to eat sugar.” “ I will keep an urge jar and only count my wins.” Now, get it all on the calendar and mark off your wins!
