Episode 146: RA vs OA    Future Dr. Magurany explains how to differentiate rheumatoid arthritis from osteoarthritis.  Written by Thomas Magurany, MSIV, Ross University School of Medicine. Comments by Hector Arreaza, MD. You are listening to Rio Bravo qWeek Podcast, your weekly dose of knowledge brought to you by the Rio Bravo Family Medicine Residency Program from Bakersfield, California, a UCLA-affiliated program sponsored by Clinica Sierra Vista, Let Us Be Your Healthcare Home. This podcast was created for educational purposes only. Visit your primary care provider for additional medical advice.1. Etiology: Rheumatoid Arthritis (RA): RA is an autoimmune disease wherein the immune system mistakenly attacks healthy tissues, particularly the synovial joints, usually between the ages of 30-50. Genetic predisposition, environmental factors such as smoking or infections, hormonal imbalances, and lower socioeconomic status have been associated with an increased risk of developing RA(1).Osteoarthritis (OA): OA primarily arises due to mechanical stress on the joints over time. Factors contributing to OA include age, obesity, joint injury or trauma, repetitive joint use or overuse, genetic abnormalities in collagen structure, and metabolic disorders affecting cartilage metabolism (2).The greatest risk factor for the development of OA is age with most patients presenting after 45 years of age. The greatest modifiable risk factor for OA is weight. People with a BMI >30 were found to have a 6.8 times greater risk of developing OA. (3) Primary OA is the most common and is diagnosed in the presence of associated risk factors such as: older age, female gender, obesity, anatomical factors, muscle weakness, and joint injury (occupation/sports activities) in the absence of trauma or disease. Secondary OA occurs alongside a pre-existing joint deformity including trauma or injury, congenital joint disorders, inflammatory arthritis, avascular necrosis, infectious arthritis, Paget disease, osteopetrosis, osteochondritis dissecans, metabolic disorders (hemochromatosis, Wilson's disease), Ehlers-Danlos syndrome, or Marfan syndrome.2. Pathogenesis:Rheumatoid Arthritis (RA):In some patients, RA is triggered by some sort of environmental factor in a genetically predisposed person. The best example is tobacco use in a patient with HLA-DRB1. The immune response in RA starts at sites distant from the synovial joints, such as the lung, gums, and GI tract. In these tissues, modified proteins are produced by biochemical reactions such as citrullination. (4)In RA, an abnormal immune response leads to chronic inflammation within the synovium lining the joints. The inflammatory cytokines released cause synovitis and lead to the destruction of articular cartilage and bone erosion through pannus formation. Immune cells infiltrate the synovium causing further damage. (4) In summary: formation of antibodies to citrullinated proteins, these antibodies begin attacking wrong tissues.Osteoarthritis (OA):The primary pathological feature of OA is the degeneration of articular cartilage that cushions the joints causing surface irregularity, and focal erosions. These changes progress down the bone and eventually involve the entire joint surface. Mechanical stress triggers chondrocyte dysfunction, leading to an imbalance between cartilage synthesis and degradation that cause cartilage outgrowths that ossify and form osteophytes. This results in the release of enzymes that degrade the extracellular matrix, leading to progressive cartilage loss. As more of the collagen matrix is damaged, chondrocytes undergo apoptosis. Improperly mineralized collagen causes subchondral bone thickening; in advanced disease, bone cysts infrequently occur (5). In summary: Osteophytes formation and cartilage loss.3. Clinical Presentation:Rheumatoid Arthritis (RA):The most common and predominant symptoms include joint pain and swelling, usually starting insidiously over a period of weeks to months. RA typically affects multiple joints symmetrically, commonly involving small joints of the hands, wrists, feet and progresses to involve proximal joints if left untreated. Morning stiffness lasting more than an hour is a characteristic feature. The affected joint will be painful if pressure is applied to the joint or on movement with or without joint swelling. Synovial thickening with a "boggy" feel on palpation will be noted. The classical physical findings of ulnar deviation, metacarpophalangeal joint subluxation, swan neck deformity, Boutonniere deformity, and the "bowstring" sign (prominent and tight tendons on the dorsum of the hand) are seen in advanced chronic disease. (4) Around ¼ of patients with RA may present with rheumatoid noduleswhich are well demarcated, flesh-colored subcutaneous lumps. They are usually described as being doughy or firm and are not typically tender unless they are inflamed. They are usually found on areas susceptible to repeated trauma or pressure and include the elbows, fingers and forearms. Osteoarthritis (OA):OA primarily affects weight-bearing joints such as knees, hips, spine, and hands. Symptoms include joint pain aggravated by activity and relieved with rest, morning stiffness lasting less than 30 minutes, joint swelling due to secondary inflammation, and occasionally the formation of bony outgrowths called osteophytes (6). Tenderness may be present at joint lines, and there may be pain upon passive motion. Classic physical exam findings in hand OA include Heberden's nodes (posterolateral swellings of DIP joints), Bouchard's nodes (posterolateral swellings of PIP joints), and “squaring” at the base of the thumb (first Carpal-Metarcapal or CMC joints), bony enlargement, crepitus, effusions (non-inflammatory), and a limited range of motion. Patients may also experience bony swelling, joint deformity, and instability (patients complain that the joint is “giving way” or “buckling,” a sign of muscle weakness). (5)4. Lab findings:Rheumatoid Arthritis: Laboratory testing often reveals anemia of chronic disease (increased ferritin, decreased iron and TIBC) and thrombocytosis. Neutropenia may be present if Felty syndrome is present. RF is present in 80-90% of patients with a sensitivity of 69%. In patients who are asymptomatic or those that have arthralgias, a positive RF and especially CCP predicts the onset of clinical RA. Patients with RA with RF, ACPA, or both are designated as having seropositive RA. About 10% of RA patients are seronegative. ESR and levels of CRP are usually elevated in patients with active disease and can be used to assess disease activity. The synovial fluid in RA will also reveal low C3 and C4 levels despite elevated serum levels.(4) Some non-specific inflammatory markers such as ESR, CRP can help you guide your diagnosis of RA.Osteoarthritis:Lab findings are not significant. Clinical diagnosis if the following are present: 1) pain worse with activity and better with rest, 2) age more than 45 years, 3) morning stiffness lasting less than 30 minutes, 4) bony joint enlargement, and 5) limitation in range of motion. Blood tests such as CBC, ESR, rheumatoid factor, ANA are usually normal but usually ordered to rule out an inflammatory process. Synovial fluid should show a white blood cell count less than 2,000/microL, predominantly mononuclear cells (non-inflammatory). X-rays of the affected joint can show findings consistent with OA, such as marginal osteophytes, joint space narrowing, subchondral sclerosis, and cysts; however, radiographic findings do not correlate to the severity of the disease and may not be present early in the disease. (5)5. Treatment Approaches:Rheumatoid Arthritis (RA):There is no cure for RA.The goal of treatment in RA is inducing remission and optimizing quality of life. This is initially done by beginning DMARDs, include methotrexate, hydroxychloroquine, sulfasalazine, and leflunomide. Methotrexate is the initial DMARD of choice. Anti-TNF-alpha inhibitors include etanercept, infliximab, adalimumab, golimumab, and certolizumab may be used if DMARDs fail. NSAIDs are used to control joint pain and inflammation. Corticosteroids may be used as a bridge therapy to DMARDs in a newly diagnosed patient with a very active disease. (7) Coronary artery disease has a strong association with RA. RA is an independent risk factor for the development of coronary artery disease (CAD) and accelerates the development of CAD in these patients. Accelerated atherosclerosis is the primary cause of morbidity and mortality. There is increased insulin resistance and diabetes mellitus associated with RA and is thought to be due to chronic inflammation. When treated with specific DMARDs such as hydroxychloroquine, methotrexate, and TNF antagonists, there was a marked improvement in glucose control in these patients. (8) RA is not just a disease of the joints, it is able to affect multiple organ systems.Osteoarthritis (OA):OA treatment aims at reducing pain and improving joint function through a combination of non-pharmacological interventions like exercise programs tailored to strengthen muscles around affected joints, weight management strategies, and assistive devices like braces or walking aids if required (9). Medications including analgesics or nonsteroidal anti-inflammatory drugs may be prescribed for pain relief when necessary. Duloxetine has modest activity in relieving pain associated with OA. Intraarticular glucocorticoid joint injections have a variable response but are an option for those wanting to postpone surgical intervention. In severe cases where conservative measures fail, surgical options like joint replacement may be considered (9). Weight loss is a critical intervention in those who have overweight and obesity; each pound of weight loss can decrease the load across the knee 3 to 6-fold. (5) Summary: Medications (NSAIDs, topical, duloxetine), weight loss, PT, intraarticular injections of corticosteroids, and joint replacement.________________________________Conclusion: Now we conclude episode number 146, “RA vs. OA.” Future Dr. Magurany explained that rheumatoid arthritis is an autoimmune disease that presents with joint pain and inflammation, mostly on hands and small joints, accompanied by morning stiffness longer than 1 hour. The rheumatoid factor and ACPA may be positive in a percentage of patients but not always. The base of treatment is early treatment with disease-modifying antirheumatic drugs to induce remission of the disease. OA affects weight-bearing joints with little to no inflammation, treatment is mainly lifestyle modifications, analgesics, intraarticular injections, and joint replacement. This week we thank Hector Arreaza and Thomas Magurany. Episode Notes James Madison slowly sat up in bed. He felt very weak that morning. The sun had just risen, and soon the enslaved maid Sukey would bring him his breakfast from the basement kitchen. But it irritated him that he couldn't eat it at the dining table. The eighty-five year old “Father of the Constitution” had been confined to his bed for six months. Rheumatism and liver dysfunction crippled him. Madison still had animated conversations with his family and visitors. But he knew he wouldn't last much longer. And he was the last of the Founding Fathers.

George Whitefield Chadwick - 'Aphrodite'

- In this episode we will look at George Whitefield Chadwick, a member of the so called Boston 6, and dubbed by John Phillip Sousa as the “Pride of New England”. We will hear parts of his single movement work _Aphrodite_ , and find out how closely the music ties to the mythical figure herself. Also, we will explore Chadwick's time as student, teacher, then president of the New England Conservatory of music.###Composer- George Whitefield Chadwick's life spans an interesting series of world historical events and societal shifting inventions. I'm quoting from Bill Faucett's biography on Chadwick, as he puts it very articulately: “Born in 1854, just a few years before the first volley of the Civil War, he lived to see the devastation of the Great War and the turmoil wrought by the onset of the Great Depression.” “Chadwick also watched as technology improved—and sometimes invaded—his life via electricity, the phonograph and the gramophone, the telephone, and the motion picture. He traveled widely—first by horse cart and train, then by steamship and automobile, and eventually by air—in the United States and abroad. Chadwick's travels enabled his presence at many of the age's most consequential musical events…” Can you even imagine? Living and being so highly productive before electricity, the car, and the telephone? I know there are many others that did this, but it is a heavy concept to take in.- So Chadwick, born in Massachusetts, studied piano for a little bit at the New England Conservatory in Boston, then with an eye toward studying in Europe, as did most musicians at the time, he took a teaching position at the Michigan Conservatory of Olivet College to earn and save money. After 3 years, he went to Germany and studied piano and organ at the Munich Conservatory. After 2 years there, in 1879, seemingly ready to come back to the US anyway, Chadwick was invited to conduct his own _Rip Van Winkle_ Overture at Boston's Handel and Haydn Society Triennial Festival. So, he packed up and came back to Boston for good. Then, over nearly a decade, Chadwick held various church organ positions, conducted orchestras, managed festivals, and composed more and larger pieces. In 1897, though reticent at first to accept the position, Chadwick became director of the New England Conservatory of Music. (as a side note, this is very similar to how William Schuman became director of Juilliard, after declining the position multiple times) Though the position took up much of his time, Chadwick continued to teach composition and compose a great deal himself. He was even commissioned to write for Connecticut's Norfolk Music Festival, for which among other pieces he composed _Aphrodite_, the piece we will discuss shortly. - Chadwick's composition portfolio is large and eclectic. Piano and organ works, chamber music, orchestral, concerti, choral and stage pieces. He worked a great deal between that and his position at NEC. The ONLY thing that slowed him down was his health. First, it was Rheumatism, which made him lose some teeth and degraded his eyesight. That led to gout, which gave him a lot of chronic pain. But what did him in was a heart condition. Because he consistently wrote in his diary throughout his life, we have a clear though sobering account of the days leading up to his death. For example, on December 27, 1930 he writes “Paderewski dinner and concert,” “Had a heart attack and could not go.” Just like that! As if it was a minor inconvenience!###Culture- Our piece today is _Aphrodite_, written in 1911, Chadwick actually wrote this long, one movement work in a single month! The piece itself is quite hefty orchestrationally as well, notably calling for triple woodwinds, 4 horns, 4 trumpets, off-stage trumpets, both harp and celesta, and field drums.- As I mentioned, Chadwick wrote this piece for the Norfolk Festival. To put the Norfolk Festival performance into perspective, June of 1912, this was only 6 weeks after the sinking of the Titanic! Even Chadwick remarked “[it] has so overshadowed all other affairs that I could not get into any mood to write.”###Back story and anecdote of piece- Apparently, the impetus for this work was born from Chadwick's fascination with the marble bust of Aphrodite herself at the Museum of Fine Art in Boston. Though the work is entirely instrumental, Chadwick worked out a translation of ancient Greek poetry to accompany the score:In a dim vision of the long agoWandering by a far-off Grecian shoreWhere streaming moonlight shone on golden sandsAnd melting stars dissolved in silver seas,I humbly knelt at Aphrodite's shrineImploring her with many a fervid prayerTo tell the secret of her beauty's powerAnd the depths of the ocean whence she sprang. At last the wave-born goddess raised her handAnd smiling said: “O mortal youth, behold!”Then all these mysteries passed before mine eyes.- Intriguingly, while the vast majority of Chadwick's work is Romantic, Germanic, structured in form, this piece is, obviously, highly programmatic. While it is one movement, he designates titled sections: Moonlight on the sea, Storm, Requiem, The Lovers, Children Playing, Approach of a Great Army and Hymn to Aphrodite, Moonlight scene partly repeated, and Finale. A thematic strain is persistent throughout, instead of a formal rotation.###Analysis of pieceNow let's hear the piece itself. This 2002 NAXOS recording is of the Nashville Symphony conducted by Kenneth Schermerhorn. On a side note, because of the pandemic, the Nashville Symphony was forced to cancel their entire 2020-2021 season. This is of course grave news for the organization, the musicians, and their audience. I truly hope that the situation turns around for the Nashville Symphony and they get back to making music as quickly as is possible for everyone's safety.- A very melancholy beginning, the solo viola plays a variation of the Aphrodite motive that will persist throughout the piece. Accompanied by clarinets, the short phrase ends on a deceptive and un-resolving chord, followed by an ominous statement by the timpani.- After this repeats, building suspense, the texture settles in, an undulating figure in the strings, while the English horn plays the Aphrodite motive.- Though this section expands and builds, it is never harsh, never erupts. Not only does the activity remain relatively calm, the melody itself is rarely presented with a bright instrument, like oboe or flute, and even when it is it's combined with clarinet or English horn to mitigate the brighter timbre of the instrument. This is a keen insight of orchestration on the part of Chadwick- Eventually this transitions to a raucous Storm section, heavy on the low brass and percussion.- Having built up a substantial amount of tension, Chadwick opens up directly into the Requiem. Immediately, the simple theme is accompanied by quick running chromatic scales in the lower register, swelling over and over.- Having released a great deal of the opening tension during the requiem, Chadwick enters “The Lovers” section. This opens with a violin and horn duet that develops an intimate melodic line, likely signifying the lovers themselves.- After this opening, the lyrical line is developed to a fulfilling crest, and in Wagnerian style opens into a lightness and quicker tempo only to arrive at yet another, greater, fuller moment of bliss.- Following a very light, waltz-like section meant to be the “Children Playing”, Chadwick then makes use of offstage drums and trumpets to introduce the extended march of an approaching “Great Army”. The march builds up and as the army arrives, a grand Maestoso gives way, presenting a “Hymn to Aphrodite”.- When the drama finally plays out, Chadwick reprises the “Moonlight” theme from the opening, but this time with a bit more brightness. At last, he brilliantly allows the undulating accompaniment to organically expand, adding broader and larger swelling wind chords over the top, as if breathing in sweet relief to the end.###Outro- Chadwick was often accused, notably by his own colleague Horatio Parker, for being technical, craftsman like. Honestly, I don't hear what they hear. I find my self more engaged in this music every time I hear it. On top of that, Chadwick partly has a lot to do with the fact that in my own conservatory training I was offered and even at times encouraged to enrich myself with traditions other than just that of Europe. Do not get me wrong, I can play Bach, Mozart, Beethoven, Brahms, Mahler, etc. ALL DAY LONG. Though, I am thankful to people like Chadwick who encouraged American teachers, performers, and composers to believe in their own set of traditions, skills, instincts. Now, we have a mix of everything. In this week's episode of the Spine & Nerve podcast Dr. Nicolas Karvelas and Dr. Brian Joves discuss one of the most commonly utilized medications, non-steroidal anti-inflammatory drugs (NSAIDs).   NSAIDs are medications with anti-inflammatory, analgesic, and anti-pyretic properties.  The mechanism of action of NSAIDs is predominantly through inhibition of cylo-oxygenase, but there are multiple other pathways by which NSAIDs exert their therapeutic effect including but not limited to modulation of neutrophil function and cell membrane function.  It is important to understand that there are multiple different classes of NSAIDs, and each NSAID has its own unique properties resulting in differences in regards to safety and efficacy.     Listen as the doctors discuss important topics including: -drug-drug interactions concerning NSAIDs including NSAIDs interaction with aspirin and SSRI medications.   -research including the PRECISION trial addressing the question of whether or not Celecoxib (a selective COX-2 inhibitor) has increased cardiovascular risk relative to relatively non-selective NSAIDs. -optimal utilization of NSAIDs in the setting of acute musculoskeletal injuries. Background: Consumer surveys provide information on effectiveness and side effects of medical interventions in routine clinical care. A report of fibromyalgia syndrome (FMS) consumers has not been carried out in Europe. Methods: The study was carried out from November 2010 to April 2011. Participants diagnosed with FMS rated the effectiveness and side effects of pharmacological and non-pharmacological FMS interventions on a 0 to 10 scale, with 10 being most efficacious (harmful). The questionnaire was distributed by the German League for people with Arthritis and Rheumatism and the German Fibromyalgia Association to their members and to all consecutive FMS patients of nine clinical centers of different levels of care. Results: 1661 questionnaires (95% women, mean age 54 years, mean duration since FMS diagnosis 6.8 years) were analysed. The most frequently used therapies were self-management strategies, prescription pain medication and aerobic exercise. The highest average effectiveness was attributed to whole body and local warmth therapies, thermal bathes, FMS education and resting. The highest average side effects were attributed to strong opioids, local cold therapy, gamma-amino-butyric acid analogues (pregabalin and gabapentin), tramadol and opioid transdermal systems. Conclusion: The German fibromyalgia consumer reports highlight the importance of non-pharmcological therapies in the long-term management of FMS, and challenges the strong recommendations for drug therapies given by FMS-guidelines.

Edgar Cayce said that if we could control these aspects of the human form, we could extend life to whatever we chose, that's how important these two principles are. Assimilation Assimilation is defined as the measure of the ability of any body to utilize foods we eat. Some people think that having massive doses of food will give them all they need. They could not be more wrong: it is all right shoving nutrition in but it is quite another thing if the body can absorb it and use it in that form. Typical assimilation rates might vary from 20% to 80% and nutrition supplied in some forms will not be absorbed at all. That means out of every 454 grams of food you might only absorb 20-80% of that. What happens to the food after you swallow it depends to a large degree on the other three principles of your body, namely circulation, relaxation and elimination. Circulation is crucial if assimilation is to be maintained at high levels. Circulation is helped greatly by some important factors; exercise, especially cardiovascular. Apparently something like 50 or so times as many blood vessels open up when exercising than just sitting. We have about 60,000 miles of vessels, mostly capillary type, so you can imagine the effect of increased blood flow. Massage, hydrotherapy treatments and osteopathic adjustments help because the blood stream carries the rebuilding forces to the body in these channels. They need to be clear and stimulated so that the blood will flow to feed the cells and tissues. Elimination Every activity your body does produces waste. We get rid of that waste through the skin, the lungs, the kidneys and the intestines. Major problems start to occur when any of these important channels are stopped from doing their job. We start to accumulate waste, bacteria start growing at alarming rates which leads to all sorts of problems as we become toxic. Imagine not cleaning up the kitchen very often -leaving waste- what would happen, your kitchen would become literally alive with all sorts of creepy bacteria. The same with your body. The fact that poor elimination is the most cited cause of disease within the Edgar Cayce research papers totaling over 14,000 gives you some idea just how important this aspect really is. It gives you an idea of the importance of getting rid of toxins, rid of waste from our bodies. Some of the diseases which elimination problems either cause or play a big and major part include: Arthritis, Acne, Anemia, Apoplexy-stroke, Appendicitis, Asthenia, Asthma, Atrophy, Balding-poor circulation, Bladder stricture, Blepharitis, Some cases of blindness, Boils, Brain tumors -poor circulation, Bronchitis, Bursitis, Cancers and tumors, Skin cancer, Canker sores and herpes simplex, Cataracts, Cirrhosis, Colds, Colic-poor assimilation, Colitis, Colon impaction, Complexion problems, Cystitis, Cysts, Some cases of deafness, Dermatitis, Eczema, Edema, Enteritis, Feet swelling, Gall bladder -gall stones, Halitosis-bad breath, Headaches general, Hemorrhoids, Hepatitis, Hernia, Herpes zoster, Unwanted hair, Indigestion, Insomnia, Iritis, Kidney problems, Laryngitis, Leukemia, Lumbago, Lupus, Lymphangitis, Malaria, Measles, Menopause problems, Migraine, Some cases of myopia, Polyps, Nephritis, Neuritis, Oophoritis, Phlebitis, Pneumonia problems, Poison ivy, Prostatitis, Pruritus, Psoriasis, Purpura, Pyorrhea, Rheumatic fever, Sarcoma, Sciatica, Rheumatism, Scolliosis -eliminations were a cause and an effect, Seborrhea, Skin ulcers, Female/ male sterility, Streptococcus infection, Tic douloureux, Tonsillitis, Torticollis, Ulcers, Vaginitis, Varicose veins, Vertigo-some cases, Xeroderma So if you are suffering from any of these complaints it is highly likely that you could be helped by increasing your body's ability to rid itself of toxins and wastes. It is as simple as that. Phenomenal results have been obtained from increasing your eliminations. Peoples lives have literally been turned around. There are several music beds in this podcast. The podcast is NOT over just because you hear music. It is not over until Mrs. bin Laden sings! RSS MANY MUSLIMS TURNING TO MESSIAH OF ISRAEL MUSIC / INTRO MANY MUSLIMS TURNING TO THE MESSIAH OF ISRAEL TODAY DUE TO DISENCHANTMENT WITH ISLAM Many people of the Islamic faith are calling on the Messiah of Israel for salvation. Also, many more Muslims will call upon the Messiah of Israel in the coming days. WHY? Many Muslim people are becoming disenchanted with Islam due to terrorism. They are beginning to think objectively and question WHY the real God Who created the world with such perfection and orderly design would terrorize innocent people, even people of their own faith. Also, many Muslims lately have come to the realization that they are NOT experiencing JOY in their worship. They are sensing something is missing. While Jews and Christians (who alike worship the God of Israel) experience great joy in their public worship, the Muslims do not. Much of the communication in Islam (even in the mosques) foments hatred instead of peace and love, which are characteristics of Messiah. Note: To watch Jews in Israel worshiping Messiah with great JOY, see video at end of Show Notes of this podcast. The only reason Islam is growing in numbers in some countries is because of subjugation, ignorance (lack of education), and poverty. One of the leaders of one of the best known Muslim countries in the Middle East is a subscriber to my news analysis. A member of the third wealthiest family in Egypt has been a Director on one of my Board of Directors, and a close personal friend. On one of my websites, about 80 percent of the healing and miracle testimonies are from Muslims (ex-Muslims) who were healed or received a miracle from Yeshua ha Meschiach. Many had the Messiah appear to them in person, or in a dream or vision. (See Addendum below for several of these testimonies.) In a seminar I conducted in San Diego, California, USA, several who were ex-Muslims attended. Several of them had the SAME EXPERIENCE. They were within two (2) weeks of death (due to disease). Each one of these people was from a different country. Messiah Yeshua appeared to them and asked (the SAME QUESTION): “If I heal you, will you serve me?” They all gave the SAME ANSWER to Messiah: “Yes.” One of those men has already started 32 houses of worship for Messiah. God only knows how many Muslims (and other people) have experienced the love and mercy of Messiah through that person! NOTICE: I have statistics from countries all over North Africa, the Middle East, and Central Asia showing the numbers of Muslims turning away from Islam and to the Messiah of Israel. In the last decade it is almost an asymptotic highly positive curve on the chart. This fact is NOT being covered by the mainstream media. MUSIC THE FOLLOWING PROPHECY OF ISAIAH SHOWS MORE MUSLIMS FROM MIDDLE EAST WILL TURN TO THE LORD  There is an amazing prophecy in the Book of Isaiah showing a great unity between Egypt, Israel, and Assyria. Ancient Assyria made up the confines of present day Syria and Iraq. “And the LORD shall smite Egypt: he shall smite and heal it: and they shall return even to the LORD, and he shall be intreated of them, and shall heal them. In that day shall there be a highway out of Egypt to Assyria, and the Assyrian shall come into Egypt, and the Egyptian into Assyria, and the Egyptians shall serve with the Assyrians. In that day shall Israel be the third with Egypt and with Assyria, even a blessing in the midst of the land: Whom the LORD of hosts shall bless, saying, Blessed be Egypt my people, and Assyria the work of my hands, and Israel mine inheritance.” (Tanakh: Isaiah 19: 22-25) I believe this alliance between Egypt, Syria, and Iraq will happen AFTER the Gog - Magog conflict portrayed in Ezekiel Chapters 38 and 39. MANY MORE MUSLIMS WILL TURN TO THE MESSIAH OF ISRAEL IN THE FUTURE AFTER THE WAR OF RUSSIA, IRAN, TURKEY AND THEIR ALLIES AGAINST ISRAEL WHAT IS EZEKIEL SAYING ABOUT THE FUTURE? (Read Ezekiel Chapters 38 and 39) 1. Russia, Turkey, Iran and other Middle East and North East African nations will form an alliance to attack Israel. This we know for sure.  2. G-d will defeat this alliance of Magog and others on the hills of Israel, and leave only 17 percent of them.  3. It will take seven months to bury the dead bodies, and seven years to burn the implements of war (probably radioactive elements).  4. The victory for Israel will be so great and miraculous that ALL nations will realize that the G-d of Israel is the LORD. This judgment of God prophesied in Ezekiel is against the enemies of Israel, many of which - if not most - are today Muslim nations. The false religion of Islam was NOT started until the 7th Century A.D. (or C.E.). The name of the false god, non entity “allah,” came from the worship of the moon god by the same name, and was worshiped way before Muhammed started his false religion. At any rate, Muslims all over the world will recognize that JHWH, the LORD G-d of Israel, is THE LORD, and will turn to Him in mass numbers, enjoying the peace and presence of His Spirit: the Ruach ha Chodesh. ADDENDUM: MUSLIMS SEEK MIRACLES One MIRACLE can do more than 25 years of Torah reading or Rabbinical teaching. Yes, the wayfaring children of Israel saw miracles in the wilderness, but you are NOT supposed to be like the majority of them. You are to be like Caleb and Joshua: people of faith! MUSIC I will read a few of the many testimonies of MIRACLES … real miracles … Muslims have received, who then turned away from Islam and turned to the G-d of Israel. I will read just a few, but you may go the the Show Notes of this podcast and read many more. The majority of these testimonies are from Muslims who were healed, and who became followers of Messiah because of the MIRACLES! These are a few of the many wonderful testimonies we have received. Yeshua ha Meshiach is ALIVE to help you today. Ask him for your miracle! As you read, let them remind you that nothing is too hard for the Lord! (Ed. note: Grammar is unedited in the actual written accounts below.) OOOOO On 31st December, I had an accident in which my spinal cord was dislocated. After sometime of fruitless hospital treatment, I asked to be discharged to try native doctor treatment. After two more hospitals the doctors told me that since my spinal cord was badly damaged, my chances of recovery were remote. I came from a staunch Muslim family. My ancestors are all Muslim. A Christian witnessed to me about the saving grace of Jesus Christ. I was convicted of my sins, and confessed them to Jesus and I was saved by his precious blood. Prayers were offered for me. During the prayers, I felt something like an electric shock all over my body and I felt the pain no more. After the prayers, I realized that I have received the baptism of the Holy Spirit and healed by the blood of Jesus Christ. / Elsie Kargbo OOOOO About a year ago, I attended a meeting in which I confessed my sins, and accepted Jesus Christ as my personal savior. At this meeting I was baptized with the Holy Spirit and was slain by the Spirit. When slain, I saw many angels dressed in white around me. A couple of days after my baptism I dreamt my relative by name Mariatu ascending into heaven. I also saw a man Jesus dressed in white robe; his face was bright. Then he said, "You people have been denying - this is what would happen when I come for those washed in my blood." I was a Muslim before, now that my darkness has been turned into light, I go to Muslim prayers no more. / Humu Jalloh OOOOO Yes indeed Jesus Christ is the truth, the way to heaven is to follow Christ. I was a strong Muslim and having heard about the meeting, I went to mock the Christians. The message was on the baptism of the Holy Ghost. After the prayers I saw fire passed before me, and I was slain (knocked down) after a while I saw a great multitude of angels praying around us. I looked at them speechlessly and in amazement. I now tell my people to come to Jesus because He saves. / Minkailu OOOOO I am a retired driver. For four years I was paralyzed and hospitalized, but there was no improvement, so I returned to my home town. Hands were laid on me, and Jesus healed me. Jesus is alive indeed. Jesus showed me that he not only cares for me but for all of my loved ones. My son, Amidu Timbo, who is seven years old and dumb since birth, spoke for the first time after the prayers. There is joy in my family today because Jesus has come into our heart and home. / Abdulai Timbo OOOOO I believed that God has all the jobs. About three days after the open air meeting I was called for an interview and got the job. At night something reminded me that I should give thanks and a testimony. The following week when the group preached at this same place I testified to the public. I was a Muslim, but now I love Jesus Christ and live for him everyday. / Zacharia Jah OOOOO After giving birth to a bouncing baby boy, the child did not walk at one year and six months. This troubled me much that I wondered if he could walk again in life. There was an outreach preaching. I took my son where they prayed for the sick and laid hands on him. Jesus healed my son, the next day he started to walk. / Alice Lamboi MUSIC OOOOO My daughter who did not show any sign of walking since birth, was taken to open air preaching. While the prayers were offered something miraculous happened. I carried my child on hand and felt (like) someone stretched the foot of my daughter Veronica. I gave testimony after the prayer; few days later Veronica walked. / Ransolina King OOOOO A group have been holding open air meetings about 60 yards from my residence. My day of deliverance came and I went to the meeting and stood there for 45 minutes. Whilst the preaching was going on I felt something come out of my body; wait a minute, I am healed. Brothers and sisters rejoice with me, I was sick of Rheumatism fifty one years (51 yr) - I could not walk nor stand for long. I am 80 years old now. Since I received this miracle healing from Jesus Christ, I have walked on foot and back, a distance of two and half miles. I thank the Lord for what he has done for me and I would continue to serve him. / Fred Cowan OOOOO For over eight years I had worms troubling my stomach which resulted to poor appetite and blood pressure, and little or no sleep at night. I tried native doctors as well as treatment in hospitals but my condition worsened. My son directed me to fellowship meetings. I was prayed for, and received a miracle. I am completely healed. I could now eat well, sleep soundly, and the worms trouble me no more. Jesus Christ has healed me so I would follow Him where ever he leads me to. / Musu Kargbo OOOOO Since childhood I had a terrible headache, I was taken to several witch doctors for treatment but no cure. For over 30 years I could not plait my hair. I confessed my sins and Jesus saved me at the meeting. Next was prayers for the sick, as the prayers were offered, I felt the headache leaving my body, and I was baptized with the Holy Spirit. I plait my hair every week now, something I could not do long before. / Mabinty Sesay OOOOO I was bleeding for over one year (menstrual). It could not stop, and it was painful and embarrassing. After the prayer meeting, I told a brother about it, he told me about a woman who was sick with the issue of blood for twelve years and how she was healed by faith when she touched the clothes of Jesus. This stirred me up, the brother and sister laid hands upon me and prayed. It was just too wonderful, I received healing, the bleeding stopped, just as the woman they told me Jesus healed. Praise and thanks to Jesus Christ. / Monday Sesay OOOOO My heartiest thanks to our Lord and Savior Jesus Christ. For many years I have been bitterly crying for a child. I spent a good sum of money to native (witch) and English doctors but I couldn't become pregnant. I went to market to but foodstuff. I saw people gathered around preaching. I drew near and I heard about the saving grace of Jesus Christ. I gave my life up (accepted Jesus as my Savior and Lord). I explained my problems. Told me that Jesus loves and cares for me, that I would get a child as long as I have faith in Jesus. I became pregnant about a month later, and I have now got a baby. / Zainah OOOOO Praise the Lord! I have been seeking for employment [since previous year]. I sought for help from witch doctors and false prophets, who gave me juju to wear and medicine to rub. In August I was in my bedroom when I heard some Christians preaching. The Holy Spirit convicted me to go out and listen to the Word of God. After the preaching, I disposed of the jujus and confessed my sins to Jesus Christ. I am now washed in the blood of Jesus Christ. After this experience the Lord has met all my needs. Happy to say that he has given me a business and prospered it. / Abibatu Jalloh MIRACLES Supernatural occurrences, or wonders, that happen as a result of God intervening in the affairs of man are MIRACLES. These are extra-natural happenings, outside the realm of man's natural, earthly, experiences. MIRACLES as when Elisha, the man of G-d, made the metal ax head swim which was lost in the water [2 Kings 6:1-7]. Or when Elijah raised the widow's son from the dead [1 Kings 17:17-23], not to mention the many miracles of healing, deliverance, and provision in both the Old Testament and New Testament of the Holy Bible. Not everything that appears to be a miracle is from G-d. Just as in Moses' day there were magicians who did lying wonders, so there are today. The Holy Bible tells us that in the last days the coming world ruler (anti-Christ, or false-Messiah) will have his false prophet perform seeming miracles. Many times, people are drawn into false religions and into witchcraft because they think they have evidenced or received a miracle. For example, at times Satan may have his demon spirits afflict a person with sickness or disease. If the person goes to a new age healer, or someone in the dark works of the devil, it may APPEAR that they have been healed. What has happened is simply this: the demons withdrew their affliction, and so it appeared the person was healed. However, because the sick person submitted himself to the "channel" or person being used by Satan for prayer (like a false minister, a shaman, practitioner, psychic, or witch) ... they then end up usually in a worse state. That is, the same and/or other demons then take them over. (Brit Chadashah: Matthew 12:45 / Luke 11:26) Real miracles, however ...God's MIRACLES ... always glorify the Lord. The Holy Spirit is the agent of G-d on earth to supply the resurrection POWER of Messiah ... and He will always magnify Yeshua ha Meshiach, who is G-d who came to earth in human flesh! Do you, or does someone you know, need a miracle today? There is nothing too hard for the Lord. Trust his love for you!
