On the podcast today we speak with Dr. Brett Ferstl, PT at Kinetic Sports Medicine and performance on how to treat tendon pain. Tendon pain has been called many things over the years, tendonitis, tendonosis and more recently tendinopathy. For all athletes, from the competitive team sport athlete, to the recreational runner to the weekend warriors, tendon pain is very common and can be a nuisance that affects your performance for an extended period of time. According to the Centers for Disease Control and Prevention, chronic pain—pain lasting at or beyond three months—affected over 20% of U.S. adults, or 51.6 million people, in 2021. Symptoms were severe enough to substantially restrict daily activity for 6.9% of Americans that same year. And with chronic pain comes soaring medical costs, pharmaceutical over-reliance, and addiction. Mounting multidisciplinary research suggests that most chronic pain is not of structural origin. In other words, most chronic pain can not be directly attributed to injury or physical abnormality. Neuroplastic pain results from the brain misinterpreting signals from the body as if they were dangerous. We habituate to pain, creating behaviors that either avoid pain or alleviate symptoms. Encouragingly, those undergoing a psychological treatment known as Pain Reprocessing Therapy (PRT) are showing vast improvements in pain management without pharmaceutical or other medical interventions. One major study found that two-thirds of chronic back pain patients were pain-free or nearly pain-free after four weeks of PRT interventions. In addition, patients showed visible changes in the prefrontal brain regions associated with pain after therapy. While psychological treatments are effective in managing chronic pain, this does not imply that the pain is imaginary. My guest today, Miriam Gauci Bongiovanni, suffered needlessly until she discovered the concept of neuroplastic pain. Today, now pain-free, she works from her home in Malta as a Certified MindBody Practitioner and Trauma-Informed Coach. But beyond her skills as a wonderful teacher and educator on chronic pain, I found her story of embracing a nontraditional career fascinating. Today we dive in on everything from how our personalities and fears inform our pain cycles to living a good life. Dr. Mitchell Yass is the creator of the Yass method which has been utilized to resolve the pain of thousands, including those who had surgery to resolve their pain with no improvement afterwards. He has been treating patients for almost 3 decades using his unique approach of interpreting the body's presentation of symptoms to identify the tissue eliciting the symptoms versus the false premise that the cause of pain can be identified by diagnostic tests like the MRI. There are many synonyms for pain. You can call it anxiety, stress, irritation, struggles, overwhelm… the list goes on and on. No matter what you call it, the same parts of the brain are being activated. Emotional pain can feel like physical pain, and there is no way to completely avoid it. In today's solo episode, I answer a common question asked by several of my show's listeners. I am addressing knee pain after a 20-mile run and how to proceed with marathon training.I answer the below question during today's episode: Hello! I'm currently training for my second-ever marathon. I did a long run this weekend (20 miles) and since have had an odd pain in the back of my knee. Immediately after the run, I was limping heavily, finding I couldn't straighten my right leg. The pain has eased now, and most of the time, I can walk normally but now and then, it sort of spasms, and I find I have to lift my leg and ease it down again while supporting the base of my hamstring. I think I will try a light, slow five-mile run on Friday to test it. I'm due to run 12 miles on Sunday, but I'm wary of injuring myself further, as it's still not right. Do you have any experience of this? I'm worried that if I don't run at all this week, it could drastically affect my marathon fitness. I don't care what time I get; I want to finish. When recovering from a low back injury you need to take the appropriate steps in order to recover effectively and build tolerance in the long term. Far too often people rely on passive treatment alone to support their recovery, without understanding that the simple things they’re doing every day are often contributing to the problem!!
