Comfort: Reduces harshness over bumps and prevents excessive bottoming or topping out of the suspension. Control: Maintains balance between the front and rear suspension, providing predictable handling in turns and on uneven surfaces. How Preload Works: More Preload: Compresses the spring further, making the suspension stiffer initially. This reduces sag and raises the ride height. Less Preload: Reduces spring compression, making the suspension softer initially. This increases sag and lowers the ride height. What does a mouse heart have to do with an M&M candy (the regular kind, not the peanut variety)? In our latest episode, Associate Editor Dr. Crystal Ripplinger (University of California, Davis) interviews author Dr. Tim Domeier (University of Missouri) and expert Dr. DeWayne Townsend (University of Minnesota) about the recent study by Haffner et al et al. The authors examined cardiac calcium mishandling and damage in the setting of Duchenne muscular dystrophy (DMD). Using a genetic mouse model of DMD crossed with a mouse that expresses a genetically encoded calcium indicator, Domeier and colleagues investigated changes in intracellular calcium handling in response to increased preload. The unexpected finding was that after the acute preload challenge, many of the cells were missing-in-action. What was going on with these missing cells? Listen as we talk about how individual dystrophic cells blow up if stretched too far, and why pointing microscopes in different directions might be the next step to overcome optical imaging challenges. Everybody's a bit down; we have the January Blues. So I'm privileged today to have invited four experts on their subjects all linked to positivity and taking care of yourself. Your health and wellness. Alison Gross - Physiotherapist The important thing is; Breathing properly and Moving properly. I get a lot of people in January at the clinic. By the beginning of February, we get lots of people with bad strains, muscle strains and things because they've overdone it. They've got this great idea they're going to get fit for the next year. And they will start at it and they forget to stretch and they do it too much all at once and then of course the body says, Hey, what's happening? But you know what? It's just a question of making sure that you do a few simple stretches, you have a few goals and take yourself slowly rather than rushing into it all. It's very important to do that and to set goals. Fitness goals are very important and a little plan about how you're going to do it doesn't have to be a big plan. Alison shows some stretching exercises. Our podcast guest today has an incredible story! Meet Ali Essig. Back in 2019, Ali mother of 6 kids (her twins were 8 months old), was taking a shower when her neighbor banged on the bathroom door. There had been an emergency. No one had been able to get ahold of Ali, so the neighbor rushed into her house to reach her. Ali's husband had had a stroke and had been rushed to the hospital. The neighbor stayed with the kids and Ali hurried to be with her husband. They spent quite a bit of time at the hospital trying to figure out what had caused the stroke. After many tests and days of ruling out different possible reasons for the stroke, nothing major was found. The stroke had left her husband with double vision which made it hard for him to drive, focus, work, it affected everything. Ali's husband was sent home with a note that said he needed to eat a heart healthy diet. Ali and her husband felt very confused and frustrated by the situation. Ali's dad had passed away just a few years earlier from a heart attack at age 55. She was determined to find a solution for her husband. She asked herself, “What's in my power?” They knew how to count macros, how to lose weight after having babies, but what does a heart healthy diet entail? Their main goal was to improve his vision and open the arteries to his brain. Here is what they learned about a heart healthy diet:Cut back on trans fat. Trans fats are found in fried foods, animal products, store bought baked goods, microwave popcorn and frozen dough.Avoid foods with saturated fat. The biggest source of saturated fat in the American diet is cheese.The other thing Ali & her husband learned was that eating plants could act as medicine. This is how they got into plant based eating. This isn't a quick fix, like taking an ibuprofen for a fever. This is a little by little, long haul approach. After 5 months of eating a plant based diet, Ali's husband had his vision come back, his cholesterol and triglycerides numbers had also come down. It was amazing!!One reason Ali is my hero is that she went on this journey with him. It is not easy to get a family of 8 on a plant based diet. But Ali used her mama bear power to fight for her family, get her husband healthy and improve the quality of life for them all through eating for longevity. Ali patterns her life and eating after the blue zones around the world. Blue zones are areas where the most centenarians (people who live into their 100's) live. She doesn't pattern her eating after body builders, but after longevity of life.I love how Ali uses an abundance mindset. She doesn't look at what she is taking out of her diet as much as she looks at what she is putting in. What are you gaining? Not, what are you losing?Ali taught me about the gut microbiome. Different gut microbes like to eat different foods- and they are picky eaters. Think of a rain forest and all of the life they have. As you're eating variety, you are feeding those microbes their favorite foods. Just like if you were feeding all the life in a rain forest, you would have to have a variety of foods. Ali challenges us to eat 30 different plants every week. This includes: fruit, vegetables, grains, legumes, nuts & seeds.How do we move our families & ourselves more towards plant based eating?Start with recipes they already love (for example- use half ground beef & half beans for tacos)Add more fruit - even if it's not a variety. 5 apples are better than 1 for nutrients and fiber. Increase what they will eat.Get fiber focused! Learn more about what foods have fiber and eat more of thoseEat at least 5 fruits & veggies each day (are you up for the 30 plants per week challenge??)Preload on vegetables and let kids snack on them before or as you are making dinner (prep your veggies first so this can happen)Don't get discouraged! Kids will often need to try a food 15 times before they like it.You don't have to be perfect. While multi-page apps (MPAs) seem like a new fixation for the developer community, they're pretty ancient news. If you were browsing the web in the 90s, you were likely visiting a multi-page app. Lucky for us, web tech improved, and made the entire experience of multi-page apps more appealing. One of the cons of MPAs is that they don't have preload and prefetch out of the box. Single-page apps (SPAs) on the other hand give you the power to decide how your site loads by loading all the resources for your current page and fetching resources for a page you might navigate to after. This might seem small but it makes a huge impact on your site. The good news is, more computing power gave way to better server-rendered technology, a huge win for multi-page apps. Unlike single-page apps (SPAs), multi-page apps parse very little JavaScript. That shaves seconds off your initial load which is no small feat. Shawn O'Neil from Speed Sense is an expert on website speed. In this podcast, we discuss how you know if you have a website speed issue and how to build an engaging website experience Why is site speed important? Website speed is important for three reasons: Conversion rate. Faster sites give a better user experience leading to lower bounce rates and higher conversion rates SEO. Site speed is a ranking factor for Google natural search and is measured by their core web vitals metrics Return on ad spend. Sites which convert better will give a higher return on ad spend. How do you know if you have a site speed problem? You can identify if you have a site speed issue by scanning your site using the popular site speed using a tool such as Google page speed. This will give two categories of metrics Real user metrics (RUM). How users are experiencing your site Synthetic. An estimated speed metric The metrics given on the Google page speed tool will give benchmark for the site speed. If the major pages on the site pass core web vitals then that is the minimum benchmark for judging your site speed is acceptable. Another good measure is to compare your site to your competitors. What are Core Web Vitals Google has chosen the metrics in the core website vitals as they are a good proxy for user experience. They fall into three groups: Is the page loading Is the page responding quickly Is the visual presentation stable Whether you will pass core web vitals will depend on (amongst other things) our technology stack (e.g. Shopify, Magento) and the number of applications which are loaded with the page. A common problem is below the page functionality e.g. tracking is slowing down the site. Choosing a fast theme Though it is an imperfect measure, when you are choosing a new theme, test it using Google page speed. This is imperfect as the theme demo will not have any user metrics. Top tips for building a fast engaging experience Priorities things that are the primary purpose of that page. For example, on a product page, this will be the product details. This data should load before services like reviews, social sharing etc. Preload your largest contentful paint (LCP) image. Third-party tags in tag manager should be loaded after the main page content. Image size. Compress your images properly. Official Website: https://www.lawabidingbiker.com Setting your motorcycle suspension sag by adjusting preload on your rear shocks is very important. It affects the geometry of your motorcycle and how it handles overall. This doesn't need to be a complicated process as many make it seem in other online videos and articles. We're here to break it down in simple terms for the everyday motorcycle enthusiast. (listen to this podcast episode to get all the details!) Setting sag just means that you are adjusting how much your bike lowers (sags) and rides in the rear when a rider, passenger, luggage or a combination is on the bike. The bike should only sag down so far and within a pre-determined range. Whether you're riding street, adventure, dual-sport, or dirt, setting motorcycle suspension sag is a universal process. SUPPORT US AND SHOP IN THE OFFICIAL LAW ABIDING BIKER STORE Most stock Harley-Davidson rear shocks have a pre-load adjuster knob and the owner's manual gives you load weight ratings and where you should set the preload. The air shock system on the Harley works the same by adding or removing air. If you install aftermarket shocks of any kind then you'll need to learn how to properly set motorcycle suspension sag. Finding your sag number: First, you'll need to know how much overall travel your particular motorcycle shock has. Most OEM or aftermarket Harley rear shocks have 3 in. of overall travel or 76 mm. The rule is that you should take that number and your target sag number should be 1/3 or around 30% of that number. So, 3" of travel x 30% equals around 1". So, in this case, you'll want your sag to be 1" or 25 mm. On my 2018 Kawasaki KLR 650, the rear shock is rated for 7.3 inches or 185mm of travel. So, 7.3" x 30% equals 2.19" or 55 mm. So, my target sag number for my KLR 650 should be 2.19" or 55 mm. Tip: I hate math and I'm always in a hurry so I use a Tusk Sliding Sag Scale instead of a standard tape measure. I simply set the sliding sag scale to zero when the weight is off the rear. Then, with the load on the rear of the bike, I can simply look at the sag scale measurement in inches or millimeters. How to measure your motorcycle suspension sag: Note: I'll explain below as if using a standard tape measure on not the easy to use slide sag scale. I like to start with no preload added on the rear suspension. Now, make a mark on your strut or rear fender with a marker or piece of tape. Then, lift the rear wheel off the ground, thus removing all weight from the rear shock. Now, measure from the center of the rear axle to the mark you made and note that measurement. Next, put the weight back on the rear wheel and shock. Put the rider on the bike and have them bounce a few times to settle the rear shock. Measure again from the center of the rear axle to your mark and document that number. Now, subtract the second measurement from the first and that is where your sag is. Adjusting your sag: Note: We'll use the above Harley sag number of 1" for demonstrative purposes here. If your measurement was over 1" then you are sagging too much and you need to add preload. By adding preload you are compressing the spring and raising the rear of the motorcycle. If your measurement fell short of 1" then you have too much preload on the shock and you need to remove preload, thus lowering the rear of the motorcycle. Repeat the second measurement as you add and remove preload until you hit your target sag number. You can't get to your proper target sag number: Note: We'll use the above Harley sag number of 1" for demonstrative purposes here. Let's say you are still at 1-1/4" even after you've added all the preload adjustment you can by compressing the spring and raising the rear of the bike. You simply can't get it to 1" because you can't add any more preload. This means your spring/shock is too soft and you should get a stiffer one. The flip side of that is let's say your measurement is only at 3/4" and you've added no preload. You can't add anymore sag to get to the 1" as adding preload would just raise the bike. You simply can't get the motorcycle to sag enough. This means your spring/shock is too stiff and you should get a softer one. Just understand that if you set your sag for a rider only and you're at the 3/4" then with a rider or luggage you'll likely be at your 1" sag. You need to take everything into account. You should measure motorcycle suspension sag for rider only, rider with passenger and with luggage. You measure the rear sag with the rider/passenger/luggage on the bike. Then, simply keep notes of where you should set your suspension preload adjuster for each load. This way you can avoid having to measure again every time you have a passenger or luggage. Preload adjusters: Different motorcycle shocks have different styles of preload adjusters. Many aftermarket shocks have a collar you can turn by hand or a system where you use a spanner wrench. We under-treat pain in children. This is mostly from an old culture of misunderstanding or fear of overdose. Four Components to Successful Pain Management and Intranasal Medication Administration Right drug, right dose, right patient, right timing Right Drug – Not every medication is easily amenable to intranasal administration. We can use intranasal drugs for analgesia, for anxiolysis, for seizures – but not all drugs used for those purposes will perform well – or at all – via the IN route. Right Dose – Dosing with IN meds will vary considerably from the IV route. Rule of thumb: the IN dose is 2-3 times the IV dose. Right Patient – Is this patient and family appropriate for “just taking the edge off”? What is the level of anxiety in the room? How is the child relating to the parent, usually it’s the mother there. What else is going on in that clinical snapshot as you walk in? Right Timing – Mostly IV and IN onset times are very similar. Notable exception: intranasal midazolam may take 10-15 minutes to take effect – something to keep in mind when you plan your procedure. Intranasal Medications bypass first-pass metabolism, and a portion of the drug is delivered into the CSF immediately via the nose-brain pathway. Ideal Volume for Intranasal Medication: 0.25 to 0.3 mL per naris Absolute maximum: 1 mL per naris (but expect some run-off) Preload the device with 0.1 mL solution for dead space Administer intranasal medications in the sniffing position. Lie the patient flat with occiput posterior, put patient in the sniffing position, seat the mucosal atomizing device cushion in the naris, aim toward the pinna of the ear, and shoot fast – you have to push the drug as fast as you can to atomize the solution. Intranasal Fentanyl Safe, effective at 2 mcg/kg. Most commonly stocked concentration of fentanyl is 50 mcg/mL. A 40-kg-child will reach the maximum volume possible for administration (40 kg x 2 mcg/kg = 80 mcg; at 50 mcg/mL – that makes 1.6 mL – if we divide the dose, it’s not ideal, but is still under our maximum of under 1 mL per naris.) You graduate from intranasal fentanyl in elementary school. Sufentanil for adults (half the volume of fentanyl) – 0.5 mcg/kg, which can be repeated as needed. Intranasal Midazolam Intranasal Midazolam or versed for anxiolysis is dosed at 0.3 mg/kg (up to 0.5 mg/kg for procedural sedation) Here, another practicality weighs in. The IV preparation for midazolam is 5 mg/5 mL – this a very dilute solution. You need to use the 5 mg/mL concentration to have any success with intransal midazolam because of the volume needed for the right effect. A 20-kg-child will near the maximum volume for intranasal midazolam (0.3 mg/kg is 6 mg, at 5 mg/ml, 1.2 mL, or 036 mL per naris). Kindergarten graduation is when to drop the intranasal midazolam. Intranasal Ketamine The IV dose for ketamine for pain control is 0.15 to 0.3 mg/kg, usually as an infusion over an hour. The intranasal dose of ketamine for pain control is 1 mg/kg.Low-dose ketamine may be used for pain control as an adjunct and opioid-sparing agent. Intranasal Dexmedetomidine Dexmedetomidine is an alpha-2 receptor agonist, a smarter clonidine. Clonidine is also an alpha-2 agonist, and it can cause a marked decrease in blood pressure with some mild sedation. Dexmedetomidine targets receptors in the CNS and spinal cord, and so it provides deep sedation, with very minimal blood pressure effects. It induces a sleep-like state. In fact, EEGs done under dex show the same pattern as seen in stage II sleep. Dex is safe, if titrated, and does not depress airway reflexes or respiration. Dose is 2.5 mcg/kg IN, and can add another 1 mcg/kg if needed. The downside is that it can last 30 minutes or more, but it may be a good choice for an abdominal ultrasound or CT head in unruly toddlers. Before You Go: The “Semmelweiss reflex”.
