Description: Type 2 diabetes and obesity are epidemics that can lead to amputation due to a common complication known as peripheral artery disease (P.A.D.), which is plaque build-up in mainly the leg arteries that restricts blood flow. But the right diet can help mitigate risk for limb loss. Hosts Kym McNicholas and Interventional Cardiologist Dr. John Phillips are joined by Nurse Practitioner Kay Smith to chat with special guest Dr. Michael Dansinger, Wellness Director for Boston Heart Diagnostics, about diabetes reversal, eating strategies to for P.A.D. prevention, and the secret to weight loss based on USA Network's The Biggest Loser reality show. Dr. Michael Dansinger is Founding Director of the Diabetes Reversal Program at Tufts Medical Center in Boston . He also serves on the CDC's Expert Panel for Worksite Wellness Programs and on the Council of Directors for the True Health Initiative, a leading international voice for health and wellness. Dr. Dansinger previously served as the Nutrition and Obesity Editor for Medscape Journal of Medicine and was the principal investigator of the Tufts Popular Diet Trial comparing the Atkins, Zone, Weight Watchers and Ornish eating plans for weight loss and heart disease risk-factor reduction (published in JAMA). Dansinger was the nutrition doctor for NBC's The Biggest Loser for 10 years and designer of the Biggest Loser Diet, which won top awards from U.S. News & World Report (including No. 1 Best Diet for Diabetes). Before Dr. Dansinger got into the heart of this episode's feature topic on how diet can reduce a diabetic's risk for vascular complications, the show kicked off with a discussion about obstacles patients face in healthcare. Nurse Practitioner Kay Smith shared her experience in Scotland, spending four hours in pre-operative protocols for a surgery planned for next week, only to have her physician cancel her surgery appointment due to a condition called, atrial fibrillation, that she was never informed of but was written in her medical chart. Kym and Dr. Dansinger weighed in on lessons others can learn from this experience which includes two critical questions all patients should ask following any medical appointment: What are you writing in my chart that I should be aware of so I don't miss any assessment or diagnosis? Can I get a copy of my cases notes for this appointment? This may be available through your facility's medical app or platform. You can also request it through a facility's medical records department. Kym also shared her nightmare experience with her dad falling a second time in two weeks. The first fall was out of their area and resulted in emergency surgery to replace a previous hip replacement and stabalize an elbow fracture with pins and wires. This second fall resulted in an additional hairline fracture in his femur, re-opening of the hip replacement incision, and a displacement of the pins in his elbow. An ambulance trip to the emergency room left Kym and her dad with more questions than answers. The emergency physician and clinicians refused to call the on-call orthopedic surgeon to review his xrays. If they did, they would've learned of his additional hairline fracture in the femur and would've sent him home in a wheelchair. Instead, he was told to get up and walk despite debilitating pain and a continued run of the fracture. Even more, they didn't remove that half pulled off wound dressing, didn't apply temporary steri strips to reseal the wound that opened, and sent him home with only half the wound covered. So, when he arrived home, within 10 minutes the paramedics had to be called as the wound dehiesced further and he had to be taken to a larger hospital facility in Marin County, California with a more experienced trauma team. There, he was scheduled for time in the operating room to properly clean, re-sutre, and dress the incision site. He was also admitted until an orthopedic surgeon could get a physical therapist and occupational therapist to work with him on his new orders for limited weight-bearing. Dr. Dansinger and Dr. Phillips both were horrified by Kym's experience with her dad. They agree that under no circumstances should Kym's dad have been released from the first emergency room without an orthopedic surgeon reviewing the images and without proper wound care. They each expressed the importance of physicians and clinicians taking the time necessary to listen to the patients and fully assess the entire situation and for patients and their advocates to have patience with medical staff. But they were also not surprised due to an overwhelmed healthcare system, especially since the COVID epidemic. Dr. Dansinger says as our population continues to age this will become an even bigger problem of overcrowded hospital facilities and not enough staff to manage them. It's why he shifted from an urgent care doctor to focusing on prevention of chronic illness. That includes diabetes. That's where the hosts transitioned the conversation into focusing more intently on what patients can do to reverse it and mitigate their risk for serious vascular complications such as amputation.

Have you ever wondered how much impact beer, wine, and spirits have on your health and longevity? The US Dietary Guidelines recommend limiting the number of alcoholic beverages we consume, but that can be pretty confusing. How do we even know how much alcohol is considered moderate and how much is too much? Or if we're offered a cocktail at a party, how do we know how much alcohol is in it? My friend, and fellow dietician, Kathleen Zelman is joining us today to answer all those questions and tell us how moderate alcohol consumption can affect our health. She will also explain how alcohol can sabotage our well-being and longevity. Kathleen Zelman is a food, nutrition, and media communications consultant, co-host of the new podcast, True Health Revealed, and owner of No Nonsense Nutrition. Kathleen helped launch WebMD and spent seventeen years as their Director of Nutrition. Kathleen has received many awards throughout her career, including the prestigious Lenna Frances Cooper Memorial Award from the Academy of Nutrition and Dietetics in recognition of her distinguished career and remarkable contributions to the dietetics profession.  Kathleen is also a consultant for the Distilled Spirits Council (DISCUS), so she is savvy on the subject of alcohol. She is here to give us the scoop on alcohol and talk about a new online calculator designed to help us figure out how much alcohol we are drinking. She and her co-host, Dr. Tom Rifai, are doing it on behalf of the True Health Initiative, a non-profit organization that wants to ensure that they get information to consumers, translate science, and give consumers evidence-based facts.  Dr. Eric Rimm, a Harvard professor and true expert on alcohol, was on Kathleen's podcast. He said it's okay to have a glass of wine now and then.  What does moderation mean for men and women, and how does that translate into beer, wine, and spirits? To find out what up to one drink per day looks like, go to www.standarddrinks.org to find a calculator. One standard drink is one and a half ounces of 80-proof spirits, a 12-ounce bottle of regular 5% alcohol beer, or five ounces of 12% volume wine. An exercise to do at home is to measure five ounces of wine and pour it into different glasses to see what a standard drink looks like. Become aware of what you are drinking and what the alcohol content is. Most wines have 12% alcohol, but some have much higher alcohol content- 15 or 16%. Most ready-to-drink canned cocktails are 12 ounces and have 5% alcohol content. However, some brands could contain up to 8 or 10% alcohol, so make sure you know how concentrated they are before drinking them. The information is on the label. Get to know how much you are drinking using the standard drinks calculator. Kathleen explains how it works. Having too many drinks in a row or drinking too quickly is considered binge drinking. It is always best to moderate your drinking or have your drinks with food. Why does one type of alcohol impact some people more than another?  If your face becomes flushed after drinking, it is your body telling you to slow down. It means you are not tolerating the alcohol. Kathleen discusses the potential health benefits of drinking in moderation, whether or not there is any truth to the French paradox, and how drinking too much alcohol can sabotage your health. Kathleen lists the people who should not consume alcohol at all.  Some tips to help you enjoy alcohol, reap the health benefits and make your drink last longer.  A standard drink contains roughly 150 calories. Calories add up, so try to use low-calorie mixers, like club soda, and DON'T drink with a straw! Kathleen enjoys a tall vodka soda with a splash of cranberry and a wedge of lime. Her latest favorite is the Aperol Spritz- a serving of prosecco over lots of ice with a splash of Aperol and a wedge of lime.  Kathleen and I share some recipes for mocktails. They are a good option for a “dry January” or taking a break from alcohol. Blue zones are hot spots in various places in the world where people live particularly long and healthy lives. One of the common denominators in all the blue zones is moderate alcohol consumption. Let it simmer for about 15 minutes, and then allow it to cool.  Pour it into a tall glass filled with ice, and garnish it with some orange slices or a wedge of orange. You can also add a sprig of mint. Or stretch it by adding some sparkling water.

Kathleen discusses the potential health benefits of drinking in moderation, whether or not there is any truth to the French paradox, and how drinking too much alcohol can sabotage your health. Kathleen lists the people who should not consume alcohol at all.  Some tips to help you enjoy alcohol, reap the health benefits and make your drink last longer.  A standard drink contains roughly 150 calories. Calories add up, so try to use low-calorie mixers, like club soda, and DON'T drink with a straw! Kathleen enjoys a tall vodka soda with a splash of cranberry and a wedge of lime. Her latest favorite is the Aperol Spritz- a serving of prosecco over lots of ice with a splash of Aperol and a wedge of lime.  Kathleen and I share some recipes for mocktails. They are a good option for a “dry January” or taking a break from alcohol. Blue zones are hot spots in various places in the world where people live particularly long and healthy lives. One of the common denominators in all the blue zones is moderate alcohol consumption. In episode 21 of the Wellness Paradox Podcast we're joined by Dr. Cate Collings, a board certified cardiologist and lifestyle medicine physician. Dr. Collings is  also the president of the American College of Lifestyle Medicine (ACLM). As an organization the ACLM focuses on lifestyle as a true clinical intervention for the treatment of chronic disease (in place of, or in compliment to, traditional medical interventions like medication and surgery). As Dr. Collings will discuss, these pillars of healthful eating, physical activity, stress management, relationships, sleep, and tobacco cessation will truly resonate with many fitness and wellness professionals. While we'll touch on the ACLM's pillars, as well as the overarching concept of lifestyle medicine; this discussion primarily focuses around how fitness and wellness can collaborate with medical professionals (particularly medical professionals that ascribe to the ACLM's philosophy). As Dr. Collings points out, the ACLM and their network of allied health professionals may be the most direct conduit to fitness and wellness professionals becoming part of our healthcare delivery system. Dr. Collings discusses an actionable framework for how fitness and wellness professionals can begin their collaboration with medical professionals as well as several other opportunities for engagement with the ACLM. More than anything else, as a former exercise physiologist, Dr. Collings has a deep appreciation for the role exercise can play in disease treatment and prevention. Indeed, she very much “speaks the same language” as fitness and wellness professionals. As a result, she is able to provide actionable insights to operationalize the exercise is medicine paradigm. Our Guest: Dr. Cate Collings, MD, MS, FACC, Dip ABLMDr. Collings is Founding Director of Lifestyle Medicine for Silicon Valley Medical Development and El Camino Health in the San Francisco Bay Area.  She serves as current President of the American College of Lifestyle Medicine. Dr. Collings has dual board certifications in cardiology and lifestyle medicine.  She holds a graduate degree in exercise physiology, as well as professional culinary and coaching certifications.  She received her education from the University of California, University of Wisconsin, and Stanford University Medical Center and is a member of the Alpha Omega Alpha National Medical Honor Society.  She has more than 30 years of practice in invasive, general, and imaging cardiology and now focuses her time on the Lifestyle Medicine movement at local and national levels.  As a passionate patient advocate, she critically examines how to blend digital and virtual tools with authentic human connection to foster sustainable behavior change and health restoration in diverse socioeconomic groups.  Dr. Collings has served in multiple capacities and leadership positions over the past 25 years including Founding Medical Director of El Camino Hospital Cardiopulmonary Wellness Center. Are you one of those people that think lectins — proteins that are found in many fruits, vegetables, whole grains, nuts, beans, and legumes — are actually bad for you?Sadly, there’s probably one reason you might believe this misinformation, a book titled, The Plant Paradox, by Dr. Steven R. Gundry MD.In it, Gundry makes a wide variety of unsupported claims that many of the plants we consider to be healthy are actually bad for you.In fact, Gundry goes as far as to claim, “I believe lectins are the #1 Biggest Danger in the American Diet.”The #1 biggest danger is making a claim like that, especially when there is a significant lack of science to suggest anything so bold, and very little evidence to even be worried about lectins in the first place. Lectins, as they are consumed in a diet, just aren’t an issue. And, unless you’re eating raw kidney beans (why are you eating raw kidney beans!) the alleged poisonous nature just isn’t realistic. Dr David L. Katz, MD, MPH, FACPM, FACP, FACLM is the founding director (1998) of Yale University's Yale-Griffin Prevention Research Center, Past-President of the American College of Lifestyle Medicine, and Founder/President of the True Health Initiative.  Katz earned his BA degree from Dartmouth College (1984); his MD from the Albert Einstein College of Medicine (1988); and his MPH from the Yale University School of Public Health (1993). The recipient of many awards for his contributions to public health, he has received three honorary doctorates. Show Notes In this episode of Food, Success & Life for the Modern Woman, Dan Pardi discusses how he and his team help people make behavior changes, leading to their ultimate goal. They focus on food, movement, and sleep as fundamental domains of your health practice. They provide lifestyle guidance and help you track key behaviors so you know if you’re living within your ‘Zone of HealthTM’ on a daily-to-weekly basis.   What is the Human Operating System? An application to help people implement ideas about how to affect behavior in a positive way. In order to be able to adopt this knowledge and change behavior, you need to be able to answer these 4 questions: Why you should do it? How do you do it? Are you doing it? Is it working? Many people have all the best intentions of making a change, maybe even have a plan, but never quite seem to follow through. With HumanOS, you gain the ability to understand why we change our behavior, one way or another. With the HumanOS model, you have the ability to pull through, and make the change in behavior. We have to disconvenience ourselvess to make change. Like not eating donuts for breakfast every morning. Change can happen in the moment.   Who can use HumanOS? People that are not trying to solve a specific problem right now. These are better-agers. Trying to age gracefully and keep the performance they currently have. There’s motivation to do well. They are optimizers- these are people that care to perform well in activity or mentally or both. They are trying to optimize their experience with new techniques. And then there’s people that want to make a change, like lose weight. They are not at the weight they want to be and have a motivation to make change. And people who have been diagnosed with something and have an immediate pain. For example someone who has diabetes and wants to find a way to eliminate that pain.   Create Lasting Behavior Change There are great direction followers. People who are like that are very good at following direction and getting something done. They’ll get results, but results will not last because they didn’t’ develop a habit, they followed direction. The other group of people are those who learned along the way. They took a good understanding of the subject they were covering. Those people become independent and knowledgeable. This is what we all want. In Dan’s courses, they try to persuade people to understand the subject and believe that it is the right way. And then give people a clear path to get the results. As well as getting feedback from the users. They also take into account failure, because failure is a part of everything and it needs to be built in.   Food, Movement and Sleep For Sleep: Dan encourages people to aim to get complete sleep. Means you’re waking naturally, because you’re getting enough sleep. That’s a goal. Maintain lights off at night because it determines our circadium rhythm. How much light we get will determine how much sleep we get and how restorative it is. Dimming nights in the evening will help.   For Exercise: The best guidance to determine what level of physical activity we need is from the Department of Health and Human Services which put out a weekly exercise recommendation. They’re created by an advisory committee of experts every 8 years. They analyze all the literature that has come out. The next time the committee gets together there will be a whole slew of new information. We also know that just sitting too much is not good. We need to get up and move, low intensity. If it’s easy and simple, more people will do it. We do need to incorporate intensity to maintain muscle.   For nutrition: This is challenging. But we do know a lot by trying to eat whole, fresh foods, and portion control. You will go a long way by consuming the fewest items out of a package. We want to avoid refined and over processed foods. And to have plant substances, fiber, and phytonutrients, the compounds that come out of these plants.
