- I am not a parent, but I am a DONA International certified Birth Doula working in North Carolina - I did not learn about what a doula was until the age of 26 years old in graduate school. Once I learned about the field, it instantly clicked for me. I've always been fascinated with birth but never wanted to be a nurse or doctor. - I had some fears around becoming a labor support doula and not having gone through childbirth but my training reassured me that I was able to do the job - I learned of the Alliance of Black Doulas for Black Mamas (which was rebranded to LEADDoula) through an email listserv that a friend sent me, because she knew I wanted to be doula - ABDBM or LEAD was strictly for Black women who wanted to become doulas, and was focused on serving black birthing people and their families - It was a 6 week program with culturally tailored education, included all required DONA International training to become certified, and afterwards, we were required to serve as doula for at least 3 births to be eligible for DONA certification - Between the program and my own clients, I've had 13 births - Fun fact: My first birth was a cesarean section - I love serving as a birth doula because of the relationships I have built with the moms and families I've served. Nicole and Amir discuss their journey trying to conceive for over a year, which included multiple miscarriages. We've all heard stories about birth. Maybe you've heard your own birth story from your parents. Lisa Greaves Taylor is the founder of Birth Matters NYC and has been working with NYC-area birthing families since 2010 as a childbirth educator, birth doula and lactation counselor. What is a doula? A birth support person. Identifying the difference between a doula, midwife and doctor is also important. A doula does not have any medical responsibilities. A doula is there for physical and mental support. You engage a doula when you are pregnant if they are there for labor support and then there are also postpartum doulas that can help with mom and baby after delivery. Both roles provide the exact support that the mother needs for herself during the pregnancy, delivery and postpartum. There can be some stigma with being a doula, and it's assumed that you only need one if your desire is to give birth without medication. That is not the case, a doula can be helpful regardless or the type of delivery. Marcela has been a part of every type of birth – home birth, birth center, hospital, medicated, unmedicated, cesarean. It's not an additional medical, it's a knowledgeable advocate for the mom. Marcela became a doula because she was volunteering with a non-profit that worked with first time moms in under serviced communities. She was there until the baby was born for the mom and then someone would come help the baby. Marcela hired a doula for her delivery because she wanted to support and wanted her partner to have support and knowing when to jump in. She needed to be pushed into certain situations to progress labor along. How to approach a doctor about hiring a doula, especially if there are hesitancies. A good doula is silent when they need to be silent and vocal when they need to be vocal. They may be perceived by doctors as anti-medical but you need to find the right person. She helps to normalize someone's experience and has a handle for what the couple may be most nervous about during the day. There is no right or wrong way to have a baby and it's all about helping make it more comfortable and less stressful for the mom – physical and emotional support. Regarding the cost and experience of doulas – how do you decide? What are your needs? Maybe the first time it's during labor and the second time it's during the postpartum period because that is what is giving more stress. A lot of FSA accounts will reimburse you for the cost now. DONA International is the best place to find a certified doula. Where do birthwork, the law and DEI (diversity, equity & inclusion) intersect? With our guest, Ravae Sinclair. Ravae is a certified birth doula with DONA International, certified childbirth educator with Lamaze International, lactation counselor, and postpartum doula in the south of Florida (U.S.) area. Since she began her doula career in 2002, and has supported over 700 families through a wide variety of birth and postpartum experiences. After 6 years on the DONA Board working in the roles of Intercultural Director, Ethics Chair, President, Nominations Chair, and Immediate Past President, Ravae is focused on supporting DONA International in the process of anti-racism work, exploring truth & reconciliation and shifting internal infrastructure to dismantle white supremacist policies. DONA International’s influence on the birth world is significant, so being a change agent within the organization is a priority for Ravae. Dominique Wallace lives in Spokane, WA, and has been practicing birth work since 2013. She is currently gaining certification with Birthing Advocacy Doula Training and hopes to further her career as a lactation consultant and then into midwifery. She graduated with a biology degree from Whitworth University in hopes of working in obstetrics and gynecology but realized a specific need within communities of color and changed her course of education after attending her first doula training program with DONA International. Now she hopes to reduce the infant and maternal mortality and morbidity crisis in America through reproductive justice and advocacy as a birth worker. She dug in to understand not only the role of the pelvic bones in labor but also the soft tissues that can either create a good pathway or block baby’s progress through them. Cindy is a mother of three who was using opioids during each of her pregnancies. She knew it wasn’t healthy for the babies, but she didn’t know how to safely stop. Fearing the doctors would notify authorities, Cindy was afraid to ask for help. Then one day, she made a call to a hotline. This led her to a doctor who gave her the support she needed, and now Cindy is in recovery and wants to share her story. Nzinga goes over some of the options available to pregnant women who are using and gives Cindy a call to check-in. In this episode, I am joined by Leiko Hidaka, an Evidence Based Birth Instructor from the Dominican Republic. Leiko holds a Master’s degree in Human Settlements and Environment. She earned her birth doula certification with DONA International, and her childbirth educator certification with the International Childbirth Education Association. She serves as International Coordinator for ICAN, and also leads the local ICAN chapter of the Dominican Republic, where she is a highly respected and passionate leader for women’s autonomy through evidence based care. Leiko became an EBB Instructor in 2018, and she teaches classes and workshops for parents and birth professionals in the Dominical Republic. She is also working to establish a free standing birth center there. Leiko and I talk about what birth is like in the Dominican Republic, and the challenges all women face in finding the provider who is right for their unique needs. Today's podcast is with Professor Katherine A Mason, the Vartan Gregorian Assistant Professor of Anthropology. Professor Mason is a medical anthropologist who has conducted ethnographic fieldwork in China and the U.S. Her research addresses issues in medical anthropology, population health, bioethics, China studies, reproductive health, mental health, and global health. Her first book, Infectious Change: Reinventing Chinese Public Health after an Epidemic, based on fieldwork she conducted in southeastern China on the professionalization and ethics of public health in China following the 2003 SARS epidemic, was published by Stanford University Press in 2016 and won the Foundation for the Sociology of Health and Illness Book Prize in 2019. Mason is currently working on a multi-sited ethnographic field project that examines family experiences and models of care for perinatal mood and anxiety disorders in the U.S. and China. As part of this project, she became a certified postpartum doula (DONA International, 2018) and earned a certificate in maternal mental health (PSI and 2020 Mom). She is also a core consultant on the AmeRicans’ Conceptions of Health Equity Study (ARCHES), funded by the Robert Wood Johnson Foundation. Mason is affiliated with Brown's Population Studies and Training Center, and the Program in Science and Technology Studies, and is a Faculty Fellow at the Swearer Center (2018-20). Her research has been funded by the Social Science Research Council, Wenner-Gren Foundation, Robert Wood Johnson Foundation, U.S. Fulbright program, and Association for Asian Studies. She has previously held positions as a Robert Wood Johnson Foundation Health and Society Scholar (2013-2015) and a Lecturer in the Health and Societies program at the University of Pennsylvania (2011-2013). Let's welcome our first guest on Word To Your Mothers, Hope Lien of Held by Hope and Breast Feeding Outside the Box. Hope's passion is to support parents to be empowered and find their perfect parenting style as they bring their babies home after birth, adoption, or surrogacy, giving them the care they need to be filled, so that they can therefore pour into their babies. It's her delight to journey with parents of all ages and backgrounds through ups and downs of motherhood and fatherhood. All families are incredibly unique and deserve care as distinguished as their special backgrounds and preferences. For the past 12 years she ahs been honored to walk with families during the fourth trimester. Hope has been a certified postpartum doula through DONA International, and she's currently certified as a Lactation Counselor through Healthy Children; she's also certified as a Breastfeeding Without Birthing instructor. Hope lives in Minneapolis with her husband and she has the honor of mothering her two children that she calls her little "miracles". Her 5-year old daughter came to their family through adoptions and Hope has the beautiful experience of helping to catch her when she entered the world, as well as induce lactation in order to nurse her for two years. Then, she very unexpectedly became pregnant with her second miracle baby, now 2; her son was born at home with loving support of her doulas, midwives and the support of her partner and his big sister. Even though they are bringing a birth worker perspective to this topic, they both had their challenges; and they discuss how the challenges can bring about many different outcomes. Hope: We knew almost everything in the world about birth and postpartum. Knowing can not protect you from the potential of difficulty. (ex: Postpartum depression, anxiety, birth trauma, NICU stay). But it should motivate you to plan well! Hope did this but wishes she would have spent even more time creating specific support. She realized more support was needed than just meals. She emphasizes creating a lactation and postpartum plan. We research so much on the birth that we forget we are brining these babies home and need to have a plan for postpartum. Be willing to be vulnerable and humble accept help when it's offered. Also, keep reaching out until you get the support you need. There is no such thing as a perfect motherhood. We need to learn that's it's ok to have dirty dishes, dirty babies, dirty mommies and dirty houses. It's ok to ask for support when you need to get your needs met and meet the needs of your baby. Try the best to find your personal BALANCE. This "balance" you desire is not always achievable. Remember that other's perceptions are THEIRS. They don't define who you are or how you are performing. Sondra: Sondra wishes she knew that it's ok to have healthy boundaries and say no. As a birth worker she thought she had to try it all-especially with her first-born son; with her second son, born six years later, this mindset got better. She had to learn how to vocalize those boundaries. She wishes she knew that she needed to ask for help more. Sondra assumed others would just see or recognize that she needed help. Plus, she found it challenging to express what she needed and when. Sondra and Hope also share a few tips for postpartum and why-besides hiring a doula and postpartum doula, of course, LOL! Their action step for this episode is to encourage people to learn to esteem postpartum, too. Also, they want to encourage you to walk through the shame and guilt that maybe you are feeling around this new role or any stage of parenting. Derived from the Greek term, “servant woman,” more pregnant women opt to have birth doulas during labor to avoid undergoing a cesarean section. I had a great birthing experience with a doula, too. So I’m thrilled to have Kate Dewey on the show today who can expound on her role during birthing. Early Exposure Kate Dewey says her interest in birthing started in childhood when she first watched the movie, “Look Who’s Talking.” She realized that she loved the idea of birth. And because her grandmother was a nurse, it further influenced Kate Dewey’s yearning to become a mom someday. That dream of having kids and being a certified doula turned into a reality years later. Becoming A Doula Kate Dewey shares that she particularly became interested in becoming a doula after she gave birth to her daughter in 2006. Her personal experience in childbirth inspired her to help women give birth naturally. However, it was not until 2008 when she gave birth to her second child that she trained with DONA International. It was a blessing that she underwent training because her second delivery was everything she envisioned it would be. Doula Vs. Midwife The need for a doula indeed arose out of the huge cultural shift that’s taken place over the last 50 years. When I gave birth, my doula played a significant role in ensuring that the delivery of my baby was as smooth as possible. However, many women still confuse doulas and midwives. First of all, midwives are capable of assisting decision-making aspects. These can be anything from medical exams and whether or not a woman needs anesthesia. In most cases, midwives usually have adequate training in clinical nursing training. Doulas on the other hand, primarily provide emotional and physical support. They also help new mothers and their family time to adjust taking care of the new baby. “As a birth doula, I support women and families as they give birth to their babies. That can mean different things,” said Kate Dewey. “We are present for the home birth and birthing centers. And we help women in delivery by giving physical and emotional support.” Hospitals Providing Birth Doulas Apparently, more hospitals are recognizing the need for doulas apart from midwives. Kate Dewey explains that in the hospital, doulas complement the birth team because they’re not a medical care provider. Nor are they equipped to deliver a baby or give medical advice. Essentially, doulas help mothers understand what birthing options are available. Part of their job is also to make sure that the labor continues to progress. Kate Dewey also explains that there is also a particular technique to help new mothers cope with pain. “We can decrease the overall cesarean rate by 50%. And we can shorten the length of labor by almost 25%. There is also a lower epidural request by 50%,” reveals Kate Dewey. Furthermore, hiring a doula significantly increases the success rate of breastfeeding and lower blood pressure during labor. Overall, having them around substantially contributes to happy childbirth. Hiring A Doula Before hiring, it is essential that you are comfortable with your birthing team. Kate Dewey says that she and her partner KC Johnston, have had ample training and adequate experience to ensure that your pre-delivery and postpartum goes as smooth as possible. According to Kate Dewey, their $1,500 birth package includes prenatal visits and full support every step of the way. Most of their clients truly appreciate the extra help in taking care of the new baby. “The midwife primarily focuses on the safety of the mom and baby. Their job is easier if there is a doula. We’re often at the birth before the midwife,” said Kate Dewey. “Women who are birthing need loving support and informational support. The loving support comes from their partners.” Teaching Others Seeing the clamor for women who wish to know more about childbirth, Kate Dewey started teaching classes in 2010. She says being able to educate lots of women over the years has been truly fulfilling. Years later, Kate Dewey added another feather to her cap. She became a certified Doula Trainer after undergoing training at the Birth Arts International in 2015. “This work is emotionally very challenging. You’re walking into somebody’s birth. And you have to be intensely aware of how important that day is for them,” said Kate Dewey. “Plus, you have to focus all your energy on the client during their darkest hour of need.” Kate Dewey explains that while their training is primarily done in person, there are also online training classes. The online training is self-paced. Ideally, it takes six to eight months to complete the course. There is also a requirement of attending a minimum of five births before one can be certified. Let It Be Birth For those who are interested in childbirth and doula training, Kate Dewey’s website is an excellent resource to get all of that information. They likewise provide other services like massage therapy and placenta encapsulation. Kate Dewey is primarily based in Seattle. So if you’re around the area, I strongly suggest you check out her calendar of events and attend the workshops. You can also schedule an appointment with her to discuss the services they offer in detail. She became interested in Doula work after the hospital birth of her daughter in 2006. She is also trained with DONA as a birth and postpartum doula in the summer of 2008 while she was pregnant with her second child and preparing for her birth. In 2010, she decided to continue her work in the field of birth as a Childbirth Educator. She now teaches classes in Mountlake Terrace. In 2011, Kate Dewey completed her DONA certification and also began to work as a placenta encapsulation specialist. While it is not something she does anymore, she believes in the power of placenta medicine and recognizes the placenta as a powerful source of spiritual energy as well as nourishment for the birthing person postpartum. In 2015, Kate studied with the holistic organization, Birth Arts International. She completed Doula certification and immediately began the work to become a Doula Trainer.
