Many of us are aware of our thyroids, but how many of us know what it does or what happens when there's a problem with our thyroid. Mohammad Sidani, M.D., guest for this episode, tells us all about the thyroid, including the origin of its name, and why if you're on thyroid medication you should take it exactly as prescribed. Dr. Sidani is a Texas Tech Physicians surgeon and with the TTP Multispecialty Clinic in the UMC Health and Wellness Hospital. Although Dr. Sidani specializes in endocrine surgery, he explains that there are tailored treatments which don't necessarily involve surgery and if they do, they don't involve removing the entire thyroid. If surgery is necessary, because of the possible complications, he recommends patients talk to a high-volume thyroid surgeon about treatment.

Can certain exercises help your thyroid gland function? Not in a direct way but perhaps in an indirect way! And that's exactly what these neck exercises are designed to do. You can indirectly support thyroid function by exercising your neck which can help in a number of different ways: #1. Neck exercises can help improve blood flow to the area of your neck. Working out the muscles in your neck will increase blood flow to the musculature itself. Your thyroid will benefit due to its proximity to these muscles. #2. Neck exercises help improve venous outflow out of your thyroid gland. Bringing new blood into your neck will mean blood must flow out of it! #3. Neck exercises can help improve lymphatic flow OUT of your neck. The lymphatic system is designed to help carry waste products away from your cells so your body can eliminate them. Exercising muscles is known to improve this system. #4. Neck exercises can bring balance to the musculature of the neck and help with things like tension headaches, trigger points, and neck pain. Thyroid gland massage is something that has gained momentum and attention lately. And when I talk about thyroid gland massage I am not talking about massage therapy for your thyroid gland. I'm talking about actual massage of the thyroid gland itself. Why are some people considering thyroid gland massage as a treatment for hypothyroidism? Well, the reason is as follows: Because the thyroid gland is almost a spongy-like material and because it has some storage capacity of thyroid hormone, massaging the thyroid gland may release thyroid hormone and alleviate or improve low thyroid symptoms. It sounds good in theory but there are several reasons why I do not recommend that you massage your thyroid gland. Here are the biggest reasons: #1. You may cause inflammation and make matters much worse. The number 1 cause of hypothyroidism in the US is Hashimoto's thyroiditis. The hallmark of Hashimoto's is inflammation and immune cell infiltration of the thyroid gland. Do you know what happens when you physically touch or palpate inflamed tissue? It gets worse! Massaging a gland with Hashimoto's may make your condition worse and should be avoided. #2. There's no way to know how much thyroid hormone you are releasing when you massage the thyroid gland. Even if it was a good idea to massage your gland to release thyroid hormone, there's no way to know how much hormone you are releasing. In other words, you are just as likely to make yourself hyperthyroid as you are to feel yourself feel better. Having said all of that, there are still things that you CAN do instead of thyroid gland massage that can be helpful: #1. Massage therapy of the body and especially the head and neck. #2. Neck exercises to balance musculature in the neck area. #3. Chiropractic work or osteopathic therapy. #4. Lymphatic work. #5. Trigger point injection therapy. These options can all be used in place of thyroid gland massage and can help improve thyroid function by improving the muscles and tissues that surround the thyroid gland. Obviously one of the ways to protect the thyroid gland is to limit your exposure to toxins that damage your thyroid. These toxins are found everywhere in our lives. They're in the food we eat (especially if it isn't organic), the air we breathe (car exhaust, chemtrails, second hand smoke, paints etc), and the water we drink (fluoride, benzene, and chloride). (09:09) An outline of Thyroid Function: The thyroid is a butterfly-shaped endocrine gland that sits on the front of the neck just below the Adam's apple. It is made up of two lobes – which lie on either side of the wind-pipe, joined by a small bridge of thyroid tissue called the isthmus. Each lobe is about the size of a plum cut in half The thyroid makes two hormones that it secretes into the blood stream, thyroxine (commonly known as T4) and triiodothyronine (known as T3) The hypothalamus and pituitary gland located in the brain are the master regulators of the whole endocrine system within the body, which controls ALL hormone production Regarding thyroid hormone production, the hypothalamus sends instructions to the pituitary gland by releasing thyrotropin-releasing hormone (TRH). The pituitary gland in turn sends instructions to the thyroid gland via thyroid stimulating hormone (TSH). The thyroid then produces and releases T3 and T4 This cascade is regulated by a negative feedback loop. What that means is: When circulating levels of thyroid hormone are high, the hypothalamus decreases its production of TRH, which in turn decreases the pituitary production of TSH, and thus inhibits production of T3 and T4.  When circulating levels are low, the opposite effect takes place in which the hypothalamus signals the pituitary to further stimulate thyroid hormone production.  The thyroid normally secretes roughly 20% of the body's T3, but ALL of the internally produced T4. T4 is converted to the more metabolically active T3 in tissues throughout the body via a series of different enzymes.  (12:50) Key functions of the Thyroid Gland and its hormones (T4 and T3) associated with running performance: Metabolic rate - Most of us will be aware of their importance in maintaining metabolic rate. T4 and T3 control metabolism (i.e. the way the body uses energy) by regulating the speed with which the body cells work. So, we could think of our bodies as being a car, and the thyroid hormones as the engine. Increased metabolic rate =  increase body heat production Energy expenditure - by regulating the rate at which energy is burned. This can therefore affect weight loss or weight gain. Cardiac function – Thyroid hormones can slow down or speed up the heartbeat, they are also known to increase heart muscle contractibility and cardiac output. They are also known to promote vasodilation (widening of blood vessels), which leads to enhanced blood flow to many organs. Fat metabolism – thyroid hormones enhance utilisation of fatty acids in many tissues and are linked to plasma concentrations of cholesterol and triglycerides. E.g. when thyroid hormones are low, there is an increase in blood cholesterol concentration, in fact increased cholesterol levels...

Can the thyroid gland grow back or is it possible to regrow your thyroid? We will explore that very question in this video. One question I get from many thyroid patients is whether or not the thyroid gland can grow back. They are asking because they have often been told that they will need to be on thyroid medication for the rest of their lives. This proposition doesn't sound appealing, so it's at least worth looking into whether or not it's possible for the thyroid gland to regrow. Unfortunately, it's not possible, at least not in the way that you are probably thinking about it. The thyroid gland, like brain tissue and heart tissue, does NOT grow back once it's dead. But there is a difference between DAMAGED thyroid gland tissue and DESTROYED thyroid gland tissue. Damaged thyroid gland tissue can be salvaged and it may be possible to restore some of this damaged gland tissue back to its former glory. There are, however, some situations and conditions in which it's not possible to see this. Well, you can't regrow thyroid gland tissue which is DEAD but you may be able to heal or accelerate the healing process of DAMAGED thyroid gland tissue. It turns out that there are certain cold lasers, termed low level light therapy, which may have a beneficial effect on treating Hashimoto's thyroiditis. These lasers are COLD lasers which mean that they don't burn your skin. See all the Healthcast at https://www.biobalancehealth.com/healthcast-blog/ This discussion is the second part of a series on the factors that make human age.  My view of the aging process is like the falling of the house of cards, it starts with one thing and then the effects multiply and cause other changes of aging. The first changes are found in the sex hormones and their decline after age 40, which triggers a decrease of growth hormone, which results in defects in our immune system and our metabolism slows, our heart and blood vessels lose their elasticity (we get hypertension and narrowed arteries) and our GI system stops absorbing necessary nutrients and producing  the neurotransmitters produced in the gut that protect us from becoming depressed. There are 10 biological factors that cause us to age: 1 The endocrine system the immune system the metabolic system the cardiovascular system the G I system the Thyroid Gland the nervous system Brain Function The Muscular system The Sensory system For those of you reading or watching our healthcast, we are displaying my algorithm that depicts the sequence of these changes that cause us to age.  From my perspective if we can replace or stimulate the production of the first hormones that decline, testosterone, estradiol and growth hormone, then the rest of the changes will either not occur or occur at a much later date in our lives.  That is the goal of BioBalance Health LLC—stop the progression of aging, early in the process. Aging of the Metabolism: One of the most obvious signs of aging is the outcome of metabolic malfunction which slows our metabolism and makes us gain fat around the middle and trade muscle for fat.  This algorithm shows how the loss of T, E2 and GH trigger the aging of the metabolism by decreasing muscle mass, increasing fat mass, and making the body more efficient with calories so when we eat, more becomes fat.  Leptin a hormone that helps us burn fat decreases with loss of muscle and we gain even more weight.  The onset of Estradiol loss (menopause) makes us insulin resistant, causing us to decrease the amount of blood sugar we can get into the cells to make energy, and divert it to the formation of fat.  This is a known side effect of estrogen loss in women and the increase of estrone and belly fat in men. [1] “When thinking of insulin activity, the changes in the body's changes in. lean versus fat mass decrease strength and physical activity. This alters energy metabolism and impairs resistance to infection. Increasing levels of total and central body fat in advancing age, contribute to the development of metabolic disease and lead to increased likelihood of cardiovascular problems.             “age-related increases in fat mass and waist circumference is greater in women than in men.”             “changes in physiology with age reflect a decline in physical activity-related energy expenditure—an important predictor of total and central body fat.”[2]   Happily, all of these issues can be reversed with replacement of T and E2 to women and T in men when they are replaced with pellets and not oral therapy, and or with daily weight training. Oral therapy continues the low metabolic rate, that slows with age.  My patients complain that they eat the same amount but still gain weight and it is true.  A Calorie is not the same for young people with adequate sex hormones as it is for men and women over 45!  “Doctors generally think that aging is associated with impaired glucose handling. Age-related changes in anthropometric characteristics and environmental factors have been hypothesized as being among the main causes for this association. As far as the anthropometric changes are concerned, a decline in lean body mass and an increasing fat mass are common findings in aged individuals.”   “Researchers report that age related environmental factors—specifically, changes in diet quality and decline in the degree of physical activity—may be common causes that hasten the changes in insulin and insulin-resistance that may develop as people age.”[3]   Aging of the CV System We need both pressure and flow in our cardiovascular system. We want to understand the impact of the aging process on the cardiovascular system and the effects of this process on any diseases or lifestyle changes that are typical in the aging process. Our veins need to be full of blood and the heart muscle needs to pump the blood throughout the system. As our veins or arteries harden or become filled with plaque this becomes more difficult for the heart. We can develop hypertension and have the resting pressure be too high or we can have difficulties with the heart pumping too hard to try to force blood through hardened or stiffened arteries. These problems can lead to damage of the cardiovascular system and to death. These are now thought to be age related changes in the cardiovascular system: Stiffening of the arteries which increase systolic blood pressure Decline in peripheral circulatory factors, including a decrease in muscle mass with age during exercise, a decreased ability to direct blood flow to muscles, and a decreased ability of muscle to utilize oxygen. Decline in aerobic exercise capacity, whether measured as total work performance or maximal oxygen consumption. THIS DECLINE APPEARS TO BE APPROXIMATELY HALF OF THE 10% PER DECADE DECREASE SEEN IN SEDENTARY PEOPLE. Decline in maximal exercise heart rate is a universal age -related occurrence. Age related changes are totally non universal Aging of the cardiovascular system is also triggered by the loss or deficiency of Testosterone.  When people lose testosterone and gain fat and lose muscle the inflammatory process in the body increases which causes plaque formation on the vessels, the amount of Nitric Oxide that is secreted by the arteries diminishes and the blood pressure rises making the heart work harder and harder.  Another action of low testosterone is that there is a reflex that increases cholesterol when free T decreases, in fact when someone has a fast rise in Cholesterol, then I check their cholesterol and most of the time it is elevated.   If we can stop the loss of T or replace it and replace Estrogen to women, we can slow the rate of heart disease and maintain the nitric oxide and lower blood pressure.  [1] Klatz Ronald MD., The AntiAging Revolution: Basic Health Pub.,ISB 978-1-59120-200-4 [2] Ibid. There isn't a clinician in the world, across any modality, who won't agree that unregulated chronic stress complicates every disease or physiological dysfunction known to us at this time. And if it doesn't just exacerbate the symptoms, it can generally be traced to the very root cause that created the dis-ease to begin with. You literally won't find another single determinant capable of achieving so much, and as such, stress must be acknowledged, better understood and considered as a daily component of your life. As a clinician, it's interesting to witness on a daily basis that most people don't identify with 'being stressed', despite the obvious signs of physiological deterioration. As adaptive creatures we literally accept stressors as normal components of life in the modern time, and push through the early warning signs that stress provides us with; the allergies, the fatigue, the brain fog, the menstrual dysfunctions and mood irregularities all provide the early framework for what should be acknowledged as an organism under load. And by the time autoimmunity & other inflammatory disease rears it's ugly head, opportunity has been lost, and the battle to control the acute phase response takes over. Regardless though, stress will & must be considered at some stage, so unless you choose to spend your life living between acute flairs, it's time to take stock.   "In it's primitive form, stress was an acute response for survival. It had a beginning and an end"   From my personal perspective, so many of us fail to acknowledge or accept that we are stressed due to the underlying subconscious aspects of failure. Accepting what is literally forces us to accept that we may not be where we initially expected to be at this time, or that we are actually out of control and not coping. Either way failure and shame is generally attached, & for most this is a hard reality to contemplate. And what do we do with this new enlightened state of acceptance? How big will the changes need to be to render the acute stressors inert? Will we have to face our unhappiness and make any of the massive changes staring us in the face in order to re-set ourselves and begin to live harmoniously once again?   On this episode of The Travelling Wellness Show, Dr Elen and I discuss stress at length, and the how it creates a cascade of neuroendocrine events that ultimately ends in chronic inflammation, thyroid disturbances and a person out of control.
