Are plants trying to kill you? According to Dr. Anthony Chaffee, the answer is YES! Dr. Chaffee lives what he believes, and for the past 20 years has been living an almost exclusively plant free carnivore diet. It is his assertion that most of the so-called chronic diseases we treat as doctors are caused by the food we eat, or don't eat, and can be reversed with dietary changes to a species specific diet. We dive into the history of the human diet, how the balance of protein and fat from a carnivore diet can fuel even an athlete, and the nuances of how plants are toxic and possibly harmful to humans. It is his assertion that micronutrient needs can be filled completely by eating a carefully honed Carnivore Diet. Dr Chaffee is an American medical doctor and Neurosurgical resident who, over a span of 20+ years, has researched the optimal nutrition for human performance and health. He began his University education studying Molecular & Cellular Biology with a Minor in Chemistry at the University of Washington in Seattle at the age of 15, and went on to obtain his MD . He is also an All-American rugby player and a former professional athlete in both the USA and Europe. He took several years off his education for the pursuit of his rugby career and that is when he experienced first hand the incredible difference that diet makes to performance and recovery. He is host of the podcast, The Plant Free MD and guested on numerous podcasts and shows as well. He has an active presence on Instagram , where he shares his insights and latest collaborations. We get a primary care physician's perspective on healthcare from Tim Hindmarsh on this episode of the Perspectives on Healthcare Podcast with Rob Oliver. Originally from Canada, Tim and his wife now practice in the state of Oregon. He straddles the line between the baby Boomer generation and Generation X. In addition to practicing medicine, Tim and his wife also host a podcast. Here are 3 things that stood out as Tim Hindmarsh gave us a primary care physician's perspective on healthcare: · There are only slight differences between the people that you see in an urban setting versus a rural setting. Rural physicians are called on to have a much larger scope of practice regarding the services they provide. · The pillars of quality healthcare are access and competency. · Sometimes, the patient and their needs get lost in the adherence to rules and regulations. Today podcasters May and Tim Hindmarsh describe their clinical work, forays into nonclinical, and what they have learned as podcast interviewers. They have worked primarily in clinical positions. Dr. Tim Hindmarsh has tried several nonclinical jobs, but now seems to have found an enjoyable and fulfilling career in urgent care. Dr. May Hindmarsh has been exploring nonclinical options more aggressively. They are both board-certified family physicians. They have both worked in employed family practice, inpatient care and urgent care, and other settings. They have a thriving marriage and have been involved with marriage counseling and medical missions. Originally from Canada, they live in the rural Pacific Northwest. And they both host the BS Free MD Podcast, so we get into that quite a bit. May opened up about some of her struggles. And Tim provided great insights and advice for finding clinical work that's fulfilling and fun. This episode is a bit longer than my usual, but I think you will thoroughly enjoy it. Welcome back for Part 2 of our interview with Dr. Peter Breggin , the “Conscience of Psychiatry”. We pick up from last week's show in our discussion on the history of psychiatric treatment reform, to the parallels with public and social control during Covid. Today we dive into the basis of his new book- Covid 19 and the Global Predators We discuss the big picture with the events before and during the Covid 19 worldwide pandemic and the international intertwining of big power. Is the response to Covid 19 based on science and acts of goodwill towards mankind? Or is it really a responsive action by predatory capitalists not letting a black swan event go to waste? Listen to his summary and decide for yourself! Peter R. Breggin MD is a Harvard-trained psychiatrist and former Consultant at NIMH who has been called “The Conscience of Psychiatry” for his many decades of successful efforts to reform the mental health field. His work provides the foundation for modern criticism of psychiatric diagnoses and drugs, and leads the way in promoting more caring and effective therapies. His research and educational projects have brought about major changes in the FDA-approved Full Prescribing Information or labels for dozens of antipsychotic and antidepressant drugs. He continues to educate the public and professions about the tragic psychiatric drugging of America's children. As a medical-legal expert, Dr. Breggin has unprecedented and unique knowledge about how the pharmaceutical industry too often commits fraud in researching and marketing psychiatric drugs. He has testified many times in malpractice, product liability and criminal cases, often in relation to adverse drug effects and more occasionally electroshock and psychosurgery. During the 1970s Dr. Breggin began his reform work by organizing an international campaign to stop the resurgence of lobotomy and other psychosurgery. He was a huge advocate and voice for the ending of psychosurgery which led to the creation of the International Center for the Study of Psychiatry and Psychology. Dr. Breggin has authored dozens of scientific articles and more than twenty books, including medical books and the bestsellers Toxic Psychiatry and Talking Back to Prozac. Two more recent books are Medication Madness: The Role of Psychiatric Drugs in Cases of Violence, Suicide and Crime and Psychiatric Drug Withdrawal: A Guide for Prescribers, Therapists, Patients and their Families. His most recent book is Guilt, Shame and Anxiety: Understanding and Overcoming Negative Emotions. As a medical-legal expert, Dr. Breggin has unprecedented and unique knowledge about how the pharmaceutical industry too often commits fraud in researching and marketing psychiatric drugs. What do you call a psychiatrist with over 50 years in clinical Medicine, who fought for the ending of lobotomies and huge reforms in mental health? Tim has labelled him “The Godfather of BS Free Medicine!! His passion for science and justice has earned him the title of ‘The Conscience of Psychiatry”. His work as patient advocate and expert witness helped take down psychosurgeries ( the lobotomy) and the overprescribing of psychiatric medications. He has a HUGE heart for his patients which is evident in today's interview. Today is Part 1, and In this episode we dive into a bit of the decades of his work in the reformation of Psychiatry. He has testified many times in malpractice, product liability and criminal cases, often in relation to adverse drug effects and more occasionally electroshock and psychosurgery. To vaccinate or not vaccinate, that's the question when it comes to our children and Covid-19. While the controversy has been huge, we discuss the evidence and data on early Covid treatment in everyone and also infections, morbidity, and risk to kids from infections as well as vaccines. At the time of this interview, rates of myocarditis in healthy young adults are now being correlated with the vaccine. The CDC confirmed at least 226 confirmed cases of myocarditis, have been reported in people under 30 who got the Pfizer or Moderna vaccines. How will this impact our children short term and long term? Take a listen to this highly respected researcher whose specialty is interpreting studies, aka “Evidence Based Medicine” Our guest this week is Dr. Paul Alexander. He received his bachelor's degree in epidemiology from Toronto, Ontario, a master's degree from Oxford University, and a PhD from McMaster University's Department of Health Research Methods, Evidence, and Impact. This week we are talking about “drugs”, specifically the world of PBMs with Dr. Lee Strandberg. Pharmacy Benefit Managers(PBM) are the middlemen between your pharmacist and your prescription and your insurance company. Yep.. more middlemen. If you have never heard of the term PBM, well you are not alone. Most people have no clue that they exist, how they work and how the cost or medications is controlled, negotiated or paid for. They are the one paying the pharmacist for your medication! While there are approximately 60 PBS, the 3 largest ones currently control 80% of the market. We discuss both arms of the pharmacy world- how the pharmacy acquires the medications and purchases them. Hang on as we unravel the mysterious chain of events on how the pharmacy gets paid by the PBM and how they negotiate the prices with your insurance company. We do get a little deep in the weeds on cost control and price negotiations, so pay attention. We only WISH that getting a Zpack could be as simple as buying a head of lettuce from your local farmers market. Dr. Lee Strandberg is our guest this week who intimately knows the ins and outs of the complex layers of the pharmacy world. He is a Professor Emeritus, Pharmacy Economics and Public Health Oregon State University and was Director, Managed Care pharmacy for SHS Plans from 2005-2011. He was appointed by Oregon Governor Kitzhaber to the OR Health Policy Board, Medicare-Medicaid Integration of Care and Services work Group which wrote part of the CCO and reporting structure. He was a committee member on the OR Evidence Based Review of Prescription Drugs Preferred Drug List. During the past 40 years he has presented papers, seminars and consulted on the impact of pharmaceutical use on health care costs and quality. He has been a consultant to governmental agencies in Australia and Canada. Welcome to the first Doctor Me First Summer Showcase episode where we are spotlighting the other amazing doctor podcasts that are out there. Over the next eight weeks you will get to listen to an episode from a different doctor podcast and then a solo podcast from me. This episode is from the BS Free MD podcast with Drs. May and Tim Hindmarsh. This week is another Doctales with Cocktails and…..today we call BS on ourselves. We have done some really dumb stuff over the years, and now we are laughing and sharing a FEW of the ridiculousness of our medical past. We have experienced exploding sebaceous cysts causing “something about May”, exploding speculum shrapnel and hydration on the legislature lawn. Tim has even been paid in whiskey for weekend vasectomies! We confess we like our hillbilly medical experience, which involves some vet medicine and NASCAR driving to emergencies. Biggest regrets: May's 9 month stint as Tim's nurse. Tim's frosty faux pas “dicksicle”. We are tasting some Crater Lake goodness today- Cheers to Crater Lake Vodka & Cran for the win. What if you became the patient of the very thing you fought? Hear this amazing story from Dr. Reema Batra, our guest this week in our 3rd installment in our series of “The Doctor as Patient”. Dr. Batra is a Board Certified Medical Oncologist and Hematologist. Her story is not only ironic but provides a unique perspective, having recently been diagnosed and treated for early stage breast cancer. She graciously shares the events that led to fortunate early diagnosis, her unique ability to advocate for her own treatment plan and how she dealt with the emotional struggles. Now having experienced what her patients face, she knows firsthand their uncertainties and fears. Literally walking in their shoes has provided a shared bond and ability to offer unique support for them through their journey. More about Dr Batra: She has been practicing since 2007. Initially she worked in the academic setting at NYU focusing on cancer care and healthcare disparities. She led a project to start a Breast Care Center at Woodhull Medical Center in Brooklyn, NY. In 2012 she moved to San Diego and works with an underserved population. She has worked hard to maintain the NAPBC accreditation at her affiliated hospital, Sharp Grossmont Hospital. She stresses the treatment of the “patient as a whole” and believes that outside of the exam room is where the difference is made in a patient's long term outcome. She is also a wife and mother and loves to run. Thanks to our sponsor this week is Panacea Financial. Don't forget to reach out to them for physician or medical student loans. Hang on to your seats, this is a long one! This week is a continuation from our podcast last week with Dr. Peter McCullough regarding Covid-19! Last week we discussed evidence based treatments for Covid-19 , best practices to avoid hospitalizations and death plus what resources we have. This week is all about the vaccines, safety, adverse events and RISK!. Plus is there a bigger theme in all of this? It feels like we are fighting a war not against Covid, but against each other. With opposing sides in the public and medicine, to vaccinate or not to vaccinate; who , what, when and why IS the question! We used to think asymptomatic spread was frequent and now we are learning otherwise. New evidence from published data available from tracking over 10 million people in China shows there is a less than 1% spread from asymptomatic people. The evidence seems to show we need to “Stop” testing asymptomatic well people! What we are doing in public health now seems wasteful, expensive. Why are we now still having to wear masks if we are vaccinated if it's fully safe and effective as promoted? Next we look at extreme differences between the states handling the pandemic response from Michigan to Texas. Taking it further, look up north to the border to home and native land in Canada. The border is closed, Covid hotels are mandatory for noncitizens entering for ANY reason, with no regard to their health, immunity or current vaccination status. Draconian or Totalitarianism? You won't believe it, but it's TRUE! Johns Hopkins published a booklet in 2017 referencing THE SPARS PROJECT. It was a “response” document and plan to deal with the next pandemic , predicting even the exact genus and species that would hit the world in the near future. You can read how they had already mapped out how to handle, shut down and analyze a global crisis. Next we get into how the response to Covid-19 feels to be weaponized. Schools, businesses, churches, and countries shut down and often with massive fines for not adhering to authority. Is this really The GREAT RESET? We move on to discuss how the vaccine initially was promoted as a “volunteer” basis to protect those at risk. Now we are seeing the implementation of financial coercement, Universities mandating it, talk of vaccine passports, business threatening to fire employees who are unvaccinated. What!! While Dr. McCullough is pro- vaccine, he definitely believes that Covid-19 vaccination may not be appropriate for everybody. Covid survivors, potential childbearing women and pregnant women were ALL excluded from the trials, why would we expect them to blindly get the vaccine without any data? This is a risk too big to ask for many people and one that is naively endorsed by many. The CDC is very clear on their statement regarding who should get the vaccine : ask your doctor if the vaccine is right for you. They DON'T GET TO DECIDE! It is a discussion and decision between the physician and patient. No other vaccine( flu, pneumococcal, shingles, etc) has been pushed this hard to the masses in the history of vaccines. To date there are 3514 deaths associated with the vaccine: anaphylaxis , reactogenic deaths (from blood clots, high fevers, etc) and then thrombotic events that occur farther out in time. The numbers for vaccine related adverse events is over 40,000 per 100 million. Something to consider: Is Johnson and Johnson the safest vaccine because it is only ONE vaccine? Finally: The proposition of vaccinating healthy low risk people to protect the population at large is fraught with moral implications. There WILL be college kids who die due to the rare event from vaccine induced causes .. from being coerced into the vaccine because of pressure from the masses. How do we assess risk, benefit and sort through the data………….. What an honor to have this intelligent discussion and input from a world renowned and highly published physician. Dr. McCullough is a practicing internist and cardiologist and Professor of Medicine at Texas A & M College of Medicine. He is board certified in internal medicine, cardiology, and clinical lipidology. He also has a Master of Public Health from University of Michigan School of Public Health. Dr. McCullough has broadly published on a range of topics in medicine with > 1000 publications and > 600 citations in the National Library of Medicine. His works have appeared in the New England Journal of Medicine, Journal of the American Medical Association, Lancet, British Medical Journal and other top-tier journals worldwide. This week is just the beginning of some AWESOME discussions around Covid-19! Get ready since we have an amazing guest and world renowned expert who is a leading expert on the subject of Covid-19 and has the most citations in the National Library of Medicine. Not only is he humble and gracious but very wise and passionate about helping and treating our PATIENTS first! Aside from being an expert, he TREATS Covid patients and is also a Covid survivor. Dr. McCullough is a practicing internist and cardiologist and Professor of Medicine at Texas A & M College of Medicine. He is board certified in internal medicine, cardiology, and clinical lipidology. He also has a Master of Public Health from University of Michigan School of Public Health. He cares for advanced patients with common medical problems including heart and kidney disease, lipid disorders, and diabetes. He is the editor-in-chief of Cardiorenal Medicine, Reviews in Cardiovascular Medicine, and senior associate editor of the American Journal of Cardiology. On November 19, 2020, Dr. McCullough testified in the US Senate Committee on Homeland Security and Governmental Affairs concerning early ambulatory treatment of high-risk patients with COVID-19. He has appeared on news outlets and is passionate about spreading the word about treatment. Today we discuss evidence based treatments of Covid-19 , best practices to avoid hospitalizations and death plus what resources we have. He believes it is immoral, unethical and bordering on illegal to advise NO TREATMENT of outpatients who have Covid-19. The focus has been on PPE, lockdown and isolation as well as treating only hospitalized patients. Forgotten are the patients caught in the middle. We can't just be passive doing nothing to prevent hospitalizations! It is therapeutic nihilism! While the FDA is a regulatory body, NIH is a research organization and the CDC is a public health organization- none of these organizations give guidelines and put the onus on physicians to figure out how to treat their patients. Remember physicians, we work for the patient! Today's episode is the first in our series on “The Doctor as Patient”. So many times at work I have been shocked when patients find out I had the flu, had surgery or experienced a “real medical” problem. No we aren't superheroes, and we DO share your pain. When the doc becomes the patient, they often will experience the same struggles ,vulnerabilities and fears. This can be a very eye opening experience and impact how we see and treat our patients. This week we have the privilege to hear from Dr. Manisha Rayavarapu. She is a Family Medicine physician, diagnosed with Lupus during her 2nd year of residency training. She describes how her faith along with her husband, have been the most important support on her journey. She shares her struggles with navigating the health system ,uncompassionate colleagues and administration, to family who may not understand how her illness affects her life. She has gone on to write a book on her experiences, struggles, fears and strength on coping with her ‘new normal'. Her memoir, “Butterflies, Boards and Blessings” is her way of helping educate and raise awareness about lupus. She lives by her favorite quote : “life is 10% what happens to you and 90% how you react to it”! Our sponsor this week is Physician Financial Services. Reach out to Larry Keller for your disability insurance needs. He's been around for a while in many physician communities helping them with the coverage they need. Well, it finally happened, the COVID-19 vaccines have started to arrive and get administered to health care workers in the United States. The first out of the gate is the Pfizer vaccine which will almost certainly be followed soon by the Moderna vaccine. Both vaccines use a revolutionary new technology for inducing an immune response, messenger RNA (mRNA). As millions of doses start rolling out throughout our hospitals and nursing home care workers we need to reflect on whether the vaccine is effective, safe, and how it should be distributed. My discussion today is with Dr. Tim Hindmarsh who is the co-host of a fellow Doctor Podcast Network show called BS Free MD. Dr. Hindmarsh has extensively studied the vaccine and faces questions every day from his patients about whether or not they should get the vaccine. Is the Vaccine Safe? The preeminent question that everyone asks me in the OR or on social media is whether or not the vaccine is safe. The simple answer is yes. A more nuanced and accurate answer is almost certainly but perhaps time and millions of doses will lead to a more robust feel as to how safe the vaccines really are. Obviously, there is no long term data because there hasn't been enough time passed to gather data. What we know of short term results is that the vaccines seem very safe. The usual vaccine related complications that people really worry about (Guillan Barre, transverse myelitis, etc.) occur within the first six weeks or so of vaccination. Those haven't been seen in the trials so far with tens of thousands of volunteers. But that doesn't answer the broader question of what the potential long term complications of the vaccine might be. I believe by the time the vaccine gets to the public distribution phase we will have a very good idea if there are any side effects that just aren't being picked up right now. Since that is months away we will have more than enough time and doses given to evaluate its long term safety. Does the Vaccine Work? Again, we can answer this questions two different ways. Short answer: yes. Long answer: yes and maybe. The studies clearly show that the vaccines minimize the likelihood of developing severe COVID-19 disease. And functionally, that is all that really matters. Yes, the chance of you getting a severe infection to COVID isn't 50% but it is also much greater than zero if you are older. And even quoting survival numbers is not that helpful if you end up sick and unable to work for months or some complication to the virus which happens even to those who survive. The other question that we don't know is if you can still get infected if you've been vaccinated and transmit the virus to others. The studies weren't designed to find that answer so it will be months before we know the answer. It is possible that the immune response triggered by the vaccine prevents the presentation of symptoms but that you still get infected and spread the virus. A good example of this would be the oral polio vaccine which allows for infection but it prevents the devastating paralysis effects (the US no longer uses the oral vaccine but an inactivated injection now). Should I Get the Vaccine? Here the answer is it depends. If you're elderly or have any of the now well known co-morbidities (heart disease, diabetes, pulmonary disease) then getting the vaccine should be a no-brainer. Contracting COVID-19 will almost assuredly be a worse chance of survival than any theoretical risk from the vaccine. Also, those working in that population or coming in contact with patients should probably get it too. Having that much exposure just puts you at a risk that probably makes the chance of severe COVID too high when compared to theoretical risks. The tricky answer lies in what to do with a healthy 25 year old. I suspect that by the time that individual has the opportunity to get the vaccine we will have more knowledge of any complications and make a better assessment at that time. For now, just remember that even if you get COVID-19 and survive or have mild symptoms of fatigue and fever it's not any fun. Just as I don't believe the flu will kill me I still don't want to get it because I won't be able to work and I will feel miserable.
