Historically, results of direct ACL repair are poor. As such, it has been viewed as a substandard surgical option in comparison to ACL reconstruction, the modern gold-standard approach. However, ACL reconstruction is not without its flaws, and as technology and techniques have advanced, the idea of ACL repair has reemerged. Coming to us from Tripler Army Medical Center, Drs. Christian Cruz and Craig Bottoni share their results with ACL repair in their military population. Like studies from the past, the failure rate was high at 26%. However, those that succeeded had good to excellent results in an average return to full duty time that beats what is expected with ACL reconstruction. Drs. Cruz and Bottoni share their experience with this active population and why ACL repair may make sense in certain situations as well as when to avoid it.

As COVID infections locally and nationally continue to soar, Hawaii's health centers are discussing the possibility of instituting vaccine mandates for staff members. Hilton Raethel, the president and CEO of the Healthcare Association of Hawaii, said hospitals are having “substantive and serious discussions” about whether vaccines should be required for all health care workers. He added that a climbing positivity rate has many in the industry concerned. On Monday, the U.S. Department of Veterans Affairs announced a vaccine mandate for its medical employees. It's the first federal agency to require health care workers to get COVID vaccines. The mandate applies to Tripler Army Medical Center, where 80% of 1,400 health care workers are vaccinated. The remaining 20% have eight weeks to get the shot.

He received his medical degree from Emory University School of Medicine in Atlanta, Georgia, and did his residency at Tripler Army Medical Center in Honolulu, Hawaii. Dr. Bailey has been in practice for more than 20 years. In this episode… Although Dr. Richard Bailey had suspected his own hearing loss for some time, he wasn't aware of how severe it was. However, the pandemic changed that. The difficulty of understanding patients talking through masks made him realize how much he'd been relying on his other senses to compensate for his hearing impairment.  This experience had a profound effect on Dr. Bailey. It was the final push he needed to seek treatment. Now, his life is full of sounds that he never knew he was missing, and he no longer struggles to understand what's going on around him. Dr. Bailey's firsthand knowledge of hearing loss also gives him a better understanding of what his patients are going through so he can offer them a deeper level of care. In this episode of Listen Up!, Dr. Mark Syms is joined by Dr. Richard Bailey, an ENT Specialist, to discuss how Dr. Bailey's personal journey with hearing loss has impacted his life and work. Dr. Bailey also talks about what inspired him to become a doctor, advancements in hearing aid technology, and the people who have made a difference in his medical career. Stay tuned!

Dr. Beyeler recounts her clinical observations practicing internal medicine in rural community hospitals, the lessons she's learned on patient recovery, longevity, and how patients can partner with their providers to take their health into their own hands. Internal medicine is a specialty where scientific knowledge and clinical expertise is used to prevent, diagnose and treat diseases that affect adults. Internists may continue their education to focus on diseases of the heart (cardiology) or other subspecialties, but every internal medicine physician has spent three of their seven years in medical school or post-graduate school on subjects that affect a broad range of medical problems rather than on one organ system. Dr. Natalie Beyeler is a highly sought after medical internist specialist whose 35+ years of clinical experience and integrative approach to healing make her a one of a kind provider. Bridging her intuition with scientific knowledge Dr. Beyeler uses her clinical expertise to prevent, diagnose, and treat diseases that affect adults in her thriving medical practice. Practicing internal medicine in rural community hospitals, Dr. Beyeler gives each patient her undivided attention while treating their ailments, mending their wounds, and infusing heart-based healing into everything she does. He served as Consultant in Endocrinology to the Army Surgeon General, Program Director for the National Capitol Consortium Endocrinology Fellowship and as an Associate Professor of Medicine at the Uniformed Services University of Health Sciences. Dr. Bernet has received numerous military awards, was awarded the “A” Proficiency Designator for professional excellence by the Army Surgeon General and the Peter Forsham Award for Academic Excellence by the Tri-Service Endocrine Society. Dr. Bernet graduated from the Virginia Military Institute and the University of Virginia School of Medicine. Dr. Bernet completed residency at Tripler Army Medical Center and his endocrinology fellowship at Walter Reed Army Medical Center. Dr. Bernet’s research interests include: improved diagnostics for thyroid cancer, thyroidectomy related hypocalcemia, thyroid hormone content within supplements as well as management of patient’s with thyroid cancer. He is the current Secretary and CEO of the American Thyroid Association. In this episode Dr. Bernet describes that Hashimoto’s thyroiditis is an autoimmune condition that usually progresses slowly and often leads to low thyroid hormone levels — a condition called hypothyroidism. The best therapy for Hashimoto’s thyroiditis is to normalize thyroid hormone levels with medication. A balanced diet and other healthy lifestyle choices may help when you have Hashimoto’s, but a specific diet alone is unlikely to reverse the changes caused by the disease. Hashimoto’s thyroiditis develops when your body’s immune system mistakenly attacks your thyroid. It’s not clear why this happens. Some research seems to indicate that a virus or bacterium might trigger the immune response. It’s possible that a genetic predisposition also may be involved in the development of this autoimmune disorder. A chronic condition that develops over time, Hashimoto’s thyroiditis damages the thyroid and eventually can cause hypothyroidism. That means your thyroid no longer produces enough of the hormones it usually makes. If that happens, it can lead to symptoms such as fatigue, sluggishness, constipation, unexplained weight gain, increased sensitivity to cold, joint pain or stiffness, and muscle weakness. If you have symptoms of hypothyroidism, the most effective way to control them is to take a hormone replacement. That typically involves daily use of a synthetic thyroid hormone called levothyroxine that you take as an oral medication. It is identical to thyroxine, the natural version of a hormone made by your thyroid gland. The medication restores your hormone levels to normal and eliminates hypothyroidism symptoms. You may hear about products that contain a form of thyroid hormones derived from animals. They often are marketed as being natural. Because they are from animals, however, they aren’t natural to the human body, and they potentially can cause health problems. The American Thyroid Association’s hypothyroidism guidelines recommend against using these products as a first-line treatment for hypothyroidism. Although hormone replacement therapy is effective at controlling symptoms of Hashimoto’s thyroiditis, it is not a cure. You need to keep taking the medication to keep symptoms at bay. Treatment is usually lifelong. To make sure you get the right amount of hormone replacement for your body, you must have your hormone levels checked with a blood test once or twice a year. If symptoms linger despite hormone replacement therapy, you may need to have the dose of medication you take each day adjusted. If symptoms persist despite evidence of adequate hormone replacement therapy, it’s possible those symptoms could be a result of something other than Hashimoto’s thyroiditis. Talk to your health care provider about any bothersome symptoms you have while taking hormone replacement therapy. NOTES and REFERENCES Request an Appointment Victor Bernet, M.D.

Dr. Brower received scholarships to attend the University of South Dakota after graduating from Washington High School in Sioux Falls South Dakota in 1990. He completed his studies in 3 years while finishing with Cum Laude honors and received a scholarship to attend the Number 1 ranked dental school in the nation which was the University of Minnesota at that time.  He found he had a strong interest and ability in dental implants and orthodontics while at the University of Minnesota.  During his studies there he accepted a Health Professional Scholarship from the United States Army where he received advanced training at Tripler Army Medical Center from 1995-1997 on Oahu over a period of 2 years.  He specifically worked side by side with a gifted team of oral surgeons and an orthodontist that ran the US Army residency training program.  After 4 years he graduated from the University of Minnesota with Honors, and in 1997 accepted an officers commission in the Army where he requested his initial duty station to be in Germany.  He was granted his wish and was again allowed to train with a group or oral surgeons, orthodontist, and a board certified prosthodontist.  He continued his advanced training for 2 years in Germany and then moved back to the US where he worked in a clinic in Louisiana with 26 other dentists. Upon completion of his military tour and honorable discharge Dr. Brower moved to the midwest with his wife Peggy and their family to pursue larger family and settle down into private practice.  Dr. Brower's advanced training allows him to perform many procedures that other general dentists have not received the training to perform in office which gives patients great comfort when considering being "referred out" when something needs to be done.  By allowing one practitioner to see all aspects of the same case and not having each piece looked at by someone different over a wide range of times amazing results can be achieved.  In no other way can multiple specialty procedures be combined into one appointment as you can see from the photos of our wonderful patients on the site.   Dr. Brower began restoring and placing implants in 1997 and continues to be heavily involved in the forefront of implant therapy in the U.S.  He has placed and restored over 8,000 dental implants since 1997 and helped develop two independent dental implant lines for manufacturers.  He teaches live surgery courses in his offices with doctors coming from all over the world to see him perform live surgery multiple times per year.     Dr. Brower believes in being a lifelong learner and strives to also teach others whenever possible.   Dr. Brower has received the highest honors possible to achieve and received his diplomate status with the International Academy of Dental Implants, International Congress of Oral Implantology, and the International Academy of Mini Dental Implants.  He holds Fellowship status in the American Academy of Small Diameter Implants, and is a lifetime member of the American Orthodontic Society where he can also be seen as a featured speaker. He maintains active membership in the the American Dental Association, South Dakota Dental Association and dental licenses in good standing in California, Nebraska, and South Dakota.   Dr. Brower is adjunct faculty at the University of South Dakota.  He can be seen at the University of South Dakota Chemistry department website giving a lecture on organic chemistry with Dr. Griforiy Sereda on the pH of saliva and it's effects. Dr. Brower actively helps their organic chemistry department to develop new kinds of dental products to help prevent and restore cavities.   Dr. Brower has lectured and taught dental implantology internationally and been a keynote speaker at the University of California San Diego where he was filmed for international training of other dentists.  His implant videos can be see on the dentinal tubules network which is the largest depository of dental knowledge on the planet.   Dr. Brower is faculty for the International Academy of Dental Implants and a featured keynote speaker in China and England where implant dentistry is fast becoming something every dental practitioner there performs.   He also teaches orthodontics (braces) and publishes/lectures for the American Orthodontic Society, runs dental forums for orthodontics and dental implants, and moderates the conversations of hundreds of dentists every day.   He is an American Dental Association accredited and approved continuing education lecturer, and has continuing education available through www.dentaltown.com which is the world's largest gathering of dentists on the current state of implant prosthetics in the 21st century.   www.SmilesForSiouxland.com
