Bob Enyart Live

Listen in as Real Science Radio host Fred Williams and co-host Doug McBurney review and update some of Bob Enyart's legendary list of not so old things! From Darwin's Finches to opals forming in months to man's genetic diversity in 200 generations, to carbon 14 everywhere it's not supposed to be (including in diamonds and dinosaur bones!), scientific observations simply defy the claim that the earth is billions of years old. Real science demands the dismissal of the alleged million and billion year ages asserted by the ungodly and the foolish.     * Finches Adapt in 17 Years, Not 2.3 Million: Charles Darwin's finches are claimed to have taken 2,300,000 years to diversify from an initial species blown onto the Galapagos Islands. Yet individuals from a single finch species on a U.S. Bird Reservation in the Pacific were introduced to a group of small islands 300 miles away and in at most 17 years, like Darwin's finches, they had diversified their beaks, related muscles, and behavior to fill various ecological niches. Hear about this also at rsr.org/spetner.  * Finches Speciate in Two Generations vs Two Million Years for Darwin's Birds?  Darwin's finches on the Galapagos Islands are said to have diversified into 14 species over a period of two million years. But in 2017 the journal Science reported a newcomer to the Island which within two generations spawned a reproductively isolated new species. In another instance as documented by Lee Spetner, a hundred birds of the same finch species introduced to an island cluster a 1,000 kilometers from Galapagos diversified into species with the typical variations in beak sizes, etc. "If this diversification occurred in less than seventeen years," Dr. Spetner asks, "why did Darwin's Galapagos finches [as claimed by evolutionists] have to take two million years?" * Opals Can Form in "A Few Months" And Don't Need 100,000 Years: A leading authority on opals, Allan W. Eckert, observed that, "scientific papers and textbooks have told that the process of opal formation requires tens of thousands of years, perhaps hundreds of thousands... Not true." A 2011 peer-reviewed paper in a geology journal from Australia, where almost all the world's opal is found, reported on the: "new timetable for opal formation involving weeks to a few months and not the hundreds of thousands of years envisaged by the conventional weathering model." (And apparently, per a 2019 report from Entomology Today, opals can even form around insects!) More knowledgeable scientists resist the uncritical, group-think insistence on false super-slow formation rates (as also for manganese nodules, gold veins, stone, petroleum, canyons and gullies, and even guts, all below). Regarding opals, Darwinian bias led geologists to long ignore possible quick action, as from microbes, as a possible explanation for these mineraloids. For both in nature and in the lab, opals form rapidly, not even in 10,000 years, but in weeks. See this also from creationists by a geologist, a paleobiochemist, and a nuclear chemist. * Blue Eyes Originated Not So Long Ago: Not a million years ago, nor a hundred thousand years ago, but based on a peer-reviewed paper in Human Genetics, a press release at Science Daily reports that, "research shows that people with blue eyes have a single, common ancestor. A team at the University of Copenhagen have tracked down a genetic mutation which took place 6-10,000 years ago and is the cause of the eye color of all blue-eyed humans alive on the planet today." * Adding the Entire Universe to our List of Not So Old Things? Based on March 2019 findings from Hubble, Nobel laureate Adam Riess of the Space Telescope Science Institute and his co-authors in the Astrophysical Journal estimate that the universe is about a billion years younger than previously thought! Then in September 2019 in the journal Science, the age dropped precipitously to as low as 11.4 billion years! Of course, these measurements also further squeeze the canonical story of the big bang chronology with its many already existing problems including the insufficient time to "evolve" distant mature galaxies, galaxy clusters, superclusters, enormous black holes, filaments, bubbles, walls, and other superstructures. So, even though the latest estimates are still absurdly too old (Google: big bang predictions, and click on the #1 ranked article, or just go on over there to rsr.org/bb), regardless, we thought we'd plop the whole universe down on our List of Not So Old Things!   * After the Soft Tissue Discoveries, NOW Dino DNA: When a North Carolina State University paleontologist took the Tyrannosaurus Rex photos to the right of original biological material, that led to the 2016 discovery of dinosaur DNA, So far researchers have also recovered dinosaur blood vessels, collagen, osteocytes, hemoglobin, red blood cells, and various proteins. As of May 2018, twenty-six scientific journals, including Nature, Science, PNAS, PLoS One, Bone, and Journal of Vertebrate Paleontology, have confirmed the discovery of biomaterial fossils from many dinosaurs! Organisms including T. Rex, hadrosaur, titanosaur, triceratops, Lufengosaur, mosasaur, and Archaeopteryx, and many others dated, allegedly, even hundreds of millions of years old, have yielded their endogenous, still-soft biological material. See the web's most complete listing of 100+ journal papers (screenshot, left) announcing these discoveries at bflist.rsr.org and see it in layman's terms at rsr.org/soft. * Rapid Stalactites, Stalagmites, Etc.: A construction worker in 1954 left a lemonade bottle in one of Australia's famous Jenolan Caves. By 2011 it had been naturally transformed into a stalagmite (below, right). Increasing scientific knowledge is arguing for rapid cave formation (see below, Nat'l Park Service shrinks Carlsbad Caverns formation estimates from 260M years, to 10M, to 2M, to it "depends"). Likewise, examples are growing of rapid formations with typical chemical make-up (see bottle, left) of classic stalactites and stalagmites including: - in Nat'l Geo the Carlsbad Caverns stalagmite that rapidly covered a bat - the tunnel stalagmites at Tennessee's Raccoon Mountain - hundreds of stalactites beneath the Lincoln Memorial - those near Gladfelter Hall at Philadelphia's Temple University (send photos to Bob@rsr.org) - hundreds of stalactites at Australia's zinc mine at Mt. Isa.   - and those beneath Melbourne's Shrine of Remembrance. * Most Human Mutations Arose in 200 Generations: From Adam until Real Science Radio, in only 200 generations! The journal Nature reports The Recent Origin of Most Human Protein-coding Variants. As summarized by geneticist co-author Joshua Akey, "Most of the mutations that we found arose in the last 200 generations or so" (the same number previously published by biblical creationists). Another 2012 paper, in the American Journal of Physical Anthropology (Eugenie Scott's own field) on High mitochondrial mutation rates, shows that one mitochondrial DNA mutation occurs every other generation, which, as creationists point out, indicates that mtEve would have lived about 200 generations ago. That's not so old! * National Geographic's Not-So-Old Hard-Rock Canyon at Mount St. Helens: As our List of Not So Old Things (this web page) reveals, by a kneejerk reaction evolutionary scientists assign ages of tens or hundreds of thousands of years (or at least just long enough to contradict Moses' chronology in Genesis.) However, with closer study, routinely, more and more old ages get revised downward to fit the world's growing scientific knowledge. So the trend is not that more information lengthens ages, but rather, as data replaces guesswork, ages tend to shrink until they are consistent with the young-earth biblical timeframe. Consistent with this observation, the May 2000 issue of National Geographic quotes the U.S. Forest Service's scientist at Mount St. Helens, Peter Frenzen, describing the canyon on the north side of the volcano. "You'd expect a hard-rock canyon to be thousands, even hundreds of thousands of years old. But this was cut in less than a decade." And as for the volcano itself, while again, the kneejerk reaction of old-earthers would be to claim that most geologic features are hundreds of thousands or millions of years old, the atheistic National Geographic magazine acknowledges from the evidence that Mount St. Helens, the volcanic mount, is only about 4,000 years old! See below and more at rsr.org/mount-st-helens. * Mount St. Helens Dome Ten Years Old not 1.7 Million: Geochron Laboratories of Cambridge, Mass., using potassium-argon and other radiometric techniques claims the rock sample they dated, from the volcano's dome, solidified somewhere between 340,000 and 2.8 million years ago. However photographic evidence and historical reports document the dome's formation during the 1980s, just ten years prior to the samples being collected. With the age of this rock known, radiometric dating therefore gets the age 99.99999% wrong. * Devils Hole Pupfish Isolated Not for 13,000 Years But for 100: Secular scientists default to knee-jerk, older-than-Bible-age dates. However, a tiny Mojave desert fish is having none of it. Rather than having been genetically isolated from other fish for 13,000 years (which would make this small school of fish older than the Earth itself), according to a paper in the journal Nature, actual measurements of mutation rates indicate that the genetic diversity of these Pupfish could have been generated in about 100 years, give or take a few. * Polystrates like Spines and Rare Schools of Fossilized Jellyfish: Previously, seven sedimentary layers in Wisconsin had been described as taking a million years to form. And because jellyfish have no skeleton, as Charles Darwin pointed out, it is rare to find them among fossils. But now, reported in the journal Geology, a school of jellyfish fossils have been found throughout those same seven layers. So, polystrate fossils that condense the time of strata deposition from eons to hours or months, include: - Jellyfish in central Wisconsin were not deposited and fossilized over a million years but during a single event quick enough to trap a whole school. (This fossil school, therefore, taken as a unit forms a polystrate fossil.) Examples are everywhere that falsify the claims of strata deposition over millions of years. - Countless trilobites buried in astounding three dimensionality around the world are meticulously recovered from limestone, much of which is claimed to have been deposited very slowly. Contrariwise, because these specimens were buried rapidly in quickly laid down sediments, they show no evidence of greater erosion on their upper parts as compared to their lower parts. - The delicacy of radiating spine polystrates, like tadpole and jellyfish fossils, especially clearly demonstrate the rapidity of such strata deposition. - A second school of jellyfish, even though they rarely fossilized, exists in another locale with jellyfish fossils in multiple layers, in Australia's Brockman Iron Formation, constraining there too the rate of strata deposition. By the way, jellyfish are an example of evolution's big squeeze. Like galaxies evolving too quickly,

Brooklyn-based Alsarah & the Nubatones were formed out of many conversations around modern migration and the cultural exchanges between Sudan and Egypt (plus a common love of the pentatonic scale). Their third full studio album Seasons of the Road has been five years in the making and marked by perilous milestones. “Bye Bye” is one of the first songs they’ve written in English — a break up song written in the tradition of the blues.

One night, Canadian artist Leif Vollebekk dreamt that Jeff Buckley taught him a song in a Manhattan rehearsal space. Vollebekk watched Buckley’s hand on the fretboard of his guitar as he followed along. Vollebekk felt like he was in heaven, as their falsettos rang out. When he woke up, he wondered which Buckley song that might have been. He quickly realized that it was “Rock and Roll,” his own new song.

Setup (Rich): Swimming 101 in college, private in-pool lessons, professional assessments with video and coaches (eg, Swim Labs), reading books, Total Immersion, the pro in the lane next - these are all some of the ways that I tried to improve my swimming technique and form.    In July of 2022, I was invited to Dallas with the TriDot coaches at that time to pilot the TriDot Pool School. This was a program that had been in development for years and the coaches were going to be the students/subjects.   The greatest problems to overcome are drag and balance. At the core of the program is the foundation of the swim position that occurs between strokes. In fact, this program spends 90% of its focus on that position between strokes and maybe 10% on the stroke itself.   We are going to introduce the components of this position which we affectionately call F.A.S.S.T. Fingers flat and forward Arm stretched straight Shoulders and hips at 45 degrees Spine aligned from tail to crown with head facing down Thumb to thigh with palm to sky To really learn the FASST method, it helps to have long blade fins, snorkel and nose clips. Here are some links to what I generally suggest and are common at the pool schools. That's the essence of *Misogi*—an ancient Japanese practice redefined by modern thinkers as a way to push boundaries, build resilience, and redefine what's possible.   The rule? Pick a challenge so big and audacious that there's at least a 50% chance you won't succeed. It should be something far outside your comfort zone, forcing you to confront your limits—and then go beyond them.   For multi-sport athletes, a *Misogi* isn't just about physical endurance. It's about mental toughness, adaptability, and proving to yourself that you're capable of more than you think. Maybe it's your first ultra-distance event, a self-supported adventure, or a challenge that removes the usual metrics and expectations.   For the everyday human? A *Misogi* might mean committing to something that genuinely scares you—signing up for a triathlon when you don't see yourself as an athlete, tackling a project at work that feels beyond your skillset, or even showing up in a space where imposter syndrome tells you you don't belong.   Here's the truth: You don't grow by doing what you already know you can do. Growth happens when you face uncertainty and decide to take the leap anyway.   So, what's *your* Misogi for 2025?     TriDot Workouts of the Week:  To kickoff our February theme on swimming, let's start with a TriDot Swim workout. Since this is an Assessment Week for both April and I, we thought we'd share the Critical Swim Speed assessment. We'll explain what the session is, how to pace it for your best results, how to calculate your Critical Swim Speed and how to track your progress. Warmup 4 x 25 Kicking With or Without Fins (15 sec) 4 x 25 FASST with Breathing (15 sec) 4 x 25 Pinocchio-Glove (15 sec) 4 x 25 FASST-1-FASST (15 sec) 4 x 25 Finger-Tip Drag Drill (15 sec) 4 x 25 Full Stroke Swimming Build to Z4 (15 sec)     Main Set 200 swim starting slow and gradually building pace from Z2 to Z4 for final 50; rest 2 min 1. Perform a 400 time trial effort from a push (not dive). 2. Recover for 10 to 15 minutes with easy swimming and rest. 3. Perform a 200 time trial effort from a push. Be sure to count your laps correctly. Your 400 time should not be faster than twice your 200 time. Balance of time @ Z2 with excellent form Be sure to enter/confirm your assessment results to update your training intensities and race projections.     Cool Down Balance of time @ Z2 and/or repeat warm up drills as time permits. How to calculate your CSS? Critical Swim Speed (CSS) is a measure of a swimmer's aerobic capacity and is calculated using the following formula: CSS (m/sec) = 200 / (time taken for 400m – time taken for 200m). CSS in meters per second can also be calculated using the following formula: CSS (m/sec) = (D2 - D1) ÷ (T2 - T1), where D1 = 50, D2 = 400, T1 = time for 50m swim in seconds and T2 = time for 400m swim in seconds. Swimming fast can be simplified into a physics equation: (S)peed = Stroke (L)ength x Stroke (R)ate. How to track your CSS? Use TriDot and let it do it automatically. Plus keep a SwimDot score of your percentile ranking, normalized and banded to show you against your peers. Keep a spreadsheet and build this formula into it.  We are happy to email you the CSS Logbook which includes a way to track what your 400 and 200 paces were, the ratio between the two and more. Simply go to the G2G Contact Us Page and send with the subject line CSS and we will get that right over to you. Guess what, Rich! It's that special time of the show where we have some fun as I take you through some fun and maybe, “not-so fun dilemmas.” It's time for…!  Today's Fun Segment: Like It, Love It, or Leave It! (Pool Edition) How It Works: You give your guest three pool-related training experiences. They must "Like It" (it's fine), "Love It" (can't get enough), or "Leave It" (never again!) by assigning one to each category. 1. Lane Etiquette Dilemmas Swimming with someone who constantly touches your feet Circle swimming with five strangers in a lane Getting stuck behind a slow swimmer with no passing room 2. Swim Gear Struggles Goggles that fog up mid-set A swim cap that keeps slipping off Wearing a wetsuit in the pool for “race simulation” 3. Pool Training Drills Endless kickboard drills Hypoxic breathing sets (fewer breaths per lap) Sighting practice in the pool (aka, looking like a dolphin) 4. Post-Swim Realities Smelling like chlorine for the rest of the day That intense hunger that hits 10 minutes after getting out Wet hair dripping down your back no matter how well you towel off 5. Annoying Pool Habits The person who sprints past you, then immediately slows down Mid-lane conversations that block the wall when you're trying to turn Someone doing butterfly in a crowded lane 6. Swim Set Preferences 10x100s on a tight interval A long, slow continuous swim IM (individual medley) even though you're only here for freestyle 7. Unexpected Pool Hazards That mystery warm spot in the water Hairballs floating toward you mid-lap The moment your goggles snap right before the main set   Closing: Thanks again for listening this week.  Please be sure to follow us @303endurance and of course go to iTunes and give us a rating and a comment.  We'd really appreciate it! We’re excited by the prospect of a new album by Sharon Van Etten, but this one is a little different. The self-titled LP (due Feb. 7, 2025)  will be the debut of her collaborative work with her band The Attachment Theory — adding a new dimension to the sound and songwriting. “For the first time in my life I asked the band if we could just jam,” Van Etten muses. “Words that have never come out of my mouth, ever! But I loved all the sounds we were getting. I was curious. What would happen?” Magic, apparently. Van Etten went on to share that two tracks from the forthcoming album were written within the span of an hour through this process. We’ll have to wait to hear those, but we can all enjoy “Afterlife” right now. It’s a good preview of what’s to come with its weighty message boosted by a rapturous melody.

Bingley-Ellison (or FlyLo) has expanded his palette by co-writing, directing, and scoring for the horror anthology V/H/S 99 as well as other ghoulish flicks.  This Twin Peaks inspired track follows a similarly spooky path, sounding as though it’s coated in a viscous syrup with the bassline dripping into a steady beat. "Why Neuron Counts Shouldn't Be Used as Proxies for Moral Weight" by Adam Shriver

Key Takeaways:Several influential EAs have suggested using neuron counts as rough proxies for animals' relative moral weights. We challenge this suggestion. We take the following ideas to be the strongest reasons in favor of a neuron count proxy:neuron counts are correlated with intelligence and intelligence is correlated with moral weight, additional neurons result in “more consciousness” or “more valenced consciousness,” andincreasing numbers of neurons are required to reach thresholds of minimal information capacity required for morally relevant cognitive abilities. However:in regards to intelligence, we can question both the extent to which more neurons are correlated with intelligence and whether more intelligence in fact predicts greater moral weight; many ways of arguing that more neurons results in more valenced consciousness seem incompatible with our current understanding of how the brain is likely to work; andthere is no straightforward empirical evidence or compelling conceptual arguments indicating that relative differences in neuron counts within or between species reliably predicts welfare relevant functional capacities.Overall, we suggest that neuron counts should not be used as a sole proxy for moral weight, but cannot be dismissed entirely. Rather, neuron counts should be combined with other metrics in an overall weighted score that includes information about whether different species have welfare-relevant capacities. Original article:https://forum.effectivealtruism.org/posts/Mfq7KxQRvkeLnJvoB/why-neuron-counts-shouldn-t-be-used-as-proxies-for-moralThis is a linkpost for https://docs.google.com/document/d/1p50vw84-ry2taYmyOIl4B91j7wkCurlB/edit?rtpof=true&sd=trueNarrated for the Effective Altruism Forum by TYPE III AUDIO.

This week on Every Rom Com we have fun joining Audrey Hepburn and Gregory Peck on their “Roman Holiday.” But we also take this romantic comedy classic very seriously, discussing screenwriter Dalton Trumbo’s time on the Hollywood blacklist, the effects of World War II on Hepburn, and whether or not we ought to feel sorry for royalty in movies. Whether you’re here for the joys of gelato and dancing on barges, a guide to the movie’s stunning locations, or a deep dive into Hollywood history, there’s something for everyone on this week’s show! 0:00-4:10 - Intro, What would you do on your ideal vacation day? 4:10-12:02 - Basic Facts, General Opinion 12:02-33:12 - Cast & Crew In-Depth: William Wyler, Henri Alekan, Franz Planer, Dalton Trumbo, Ian Mclellan Hunter, John Dighton, Audrey Hepburn, Gregory Peck, Eddie Albert https://www.google.com/amp/s/www.latimes.com/archives/la-xpm-1991-08-25-ca-2010-story.html%3F_amp%3Dtrue https://www.telegraph.co.uk/film/trumbo/roman-holiday-writer-dalton-trumbo-credit-restored/ https://en.m.wikipedia.org/wiki/Dalton_Trumbo https://www.britannica.com/topic/Hollywood-Ten https://www.google.com/amp/s/www.hollywoodreporter.com/amp/lists/hollywood-ten-men-who-refused-83976 https://www.brandeis.edu/now/2018/june/blacklist-qa-tom-doherty.html https://www.theguardian.com/culture/2015/jun/19/cult-audrey-hepburn-how-can-anyone-live-up-level-of-chic 33:12-38:41 - Influential Film, Audrey as Fashion Icon, Vespas, Hollywood on the Tiber 38:41-43:24 - Why are so many romantic films set in Italy?, Rome Overview 43:24-58:53 - Rome Filming Locations + Plot Synopsis, Spoiler-Free Zone 58:53 - SPOILERS BEGIN 58:53-1:03:53 - Rome Filming Locations + Plot Synopsis Spoilers Allowed https://www.gpsmycity.com/tours/roman-holiday-movie-walking-tour-5090.html#:~:text=her%20first%20wish.-,G.,but%20a%20trendy%20fashion%20store.

CORONAVIRUS. COVID-19

================================================ PODCAST CORONAVIRUS. Suzi talks to Pablo Abufom and Gilbert Achcar about the ongoing massive protest movements in Chile and Lebanon, where for more than two weeks the mobilization and demonstrations have spread spectacularly in breadth and depth. In Chile 1.2 million took to the streets on Oct 25 and in Lebanon protestors formed a human chain from one end of the country to another, in both places protesting the inequity of the status quo, a generalized protest against neoliberalism, and an unjust order. Protestors have demanded the resignation of their governments in both Chile and Lebanon, and Lebanese prime minister Saad Hariri has now resigned. Lebanon’s October uprising of dignity has shaken its long-resilient sectarian political system to its foundations.

Unseen FootageEpisode 7 - Dumb and Dumber, Dumb and Dumber ToApologies for the delay! We've been busy, and some new things are happening with Unseen Footage in the New Year, so we're getting ready for that! In any case, this deluxe episode is here for your enjoyment. We discuss Dumb and Dumber, a favorite among 90s kids, and its sequel, that got released just last month, to not-surprisingly lackluster reviews. Everyone knew it was going to be bad, so let's focus on the good: Jim Carrey's amazing acting ability, Jeff Daniels' making the most of it, some of the most beloved bits and sight gags that stand the test of time, and unintentionally bad characterization that leads to some awesome moments. For the Christmas season, enjoy Lloyd Christmas, and his friend Harry, as they try to make it through two movies alive. Beware the Sea Bass!Download the episode here!

To celebrate, Claire and David retread some familiar ground with director Tim Burton, and his other movie with Michael Keaton: 1988's BEETLEJUICE! What follows is a discussion of the film's overall flaws and shortcomings. Claire wants to give the whole movie an extreme makeover, but David argues for the approach taken, warning about worse things that could have happened, both lighter and darker. Does the movie work on its own, or is it just full of good ideas? Who exactly are we supposed to root for in the proceedings? And find out what Claire would do given eternity in this month's Unseen Footage!Download the episode here!

No one is quite sure how to describe it or explain it, but the ride is enjoyable nonetheless. Claire and David attempt to put everything into words as they talk about the performances of Swayze, Reeves, Petty, and Busey, as they debate the validity of the iconic skydiving scene, Claire puts forward a thesis on action movies, and David puts forth a thesis on bromances. It's September's episode where we discuss either the greatest surfer bank robbery movie ever made, or the greatest surfer bank robbery movie you've never seen, because this is Unseen Footage!Download the episode here!

In subsequent years, especially that weird time called the early 90s, it became somewhat of a cult classic in the vein of Rocky Horror Picture Show. Claire and David dive straight into the problems of "road movies", journeys that teach lessons, and the characterizations of this movie specifically. No one knows exactly how we ended up with Labyrinth, but Jim Henson's puppet wizardry is absolutely fantastic.Download the episode here!

But that's just the surface! Claire and David go into the sharp and witty dialogue, the satirical send-up, and the deceptively complex characters. They also love how beautifully (and permanently) the movie encapsulates the 90s. For those questioning the portrayal of women in the media today, as well as fans of comedy in general, Clueless is a marvelous intersection.Download the episode here!

Unseen FootageEpisode 2- Batman1989's Batman can be argued as the movie that kickstarted the entire comic movie franchise into high gear. Only Superman had made a successful leap to the big screen, and everything else about every other superhero was rather campy and trite. Tim Burton and Michael Keaton set out to change all that, with a darker and grittier adaptation of the dark knight, years before Christopher Nolan ever would. And Burton still manages to balance comedic set pieces with brilliant character nuance. Join Claire and David as they present their respective theses on Batman as a character, as well as how this movie stands up to the rest of the canon.Download the episode here!

This is Scott Selinger and welcome to another edge-of-your-seat-amazing talking blogpost - ABC’s for ECP’s, the podcast on behalf of the Northern California’s chapter of the American College of Physicians Council of Early Career Physicians.  Today I’d like to talk a little bit about this new world of patient satisfaction scores.I really like my new car.  I got it a few months ago and it’s been working great.  But then a few weeks ago I folded the back seat down and now it won’t come back up.  So like most men, I hit it a few times and then decided to take it to the dealership.  It was a pretty easy, seamless process of getting in and out but there were a few snags.  For instance, they told me I needed a new part and they’d call when they got it in - but that didn’t happen and when I called 2 weeks later I found out that the part I needed had been there for 10 days.  And then there was the little issue of the fact that when I got the car home, the seat was still broken.  I brought it back, saw a different service advisor, who apologized profusely, and they got it fixed the same day.  The kicker was, every time I was there, I was reminded before I left that “by the way you might be getting a survey …” at which point I cut them off and just gave them a thumbs up and said “all 5’s guys!” because I really wasn’t interested in hearing the spiel – the surveys they send out are based on top box scoring, so if they get anything except a 5 out of 5, they get dinged, even if they had nothing to do with the dissatisfying issue. This is getting to my main point about patient satisfaction surveys and its effect on us as physicians.  This is something I slowly started hearing more about towards the end of residency, but at the time the satisfaction scores at the resident clinic were abysmal and we felt sort of resigned to that because a lot of the dissatisfying factors were systems issues beyond our control.  That’s if you can imagine patients being dissatisfied by frequently seeing random doctors, long wait times, bedside manners that were still under construction, and frequently being told that they didn’t need antibiotics for their cold or opioid pain medications for their headache. But now that I’m out practicing, I’m realizing how huge an issue this is.  I’m still trying to figure out when someone needs steroids and when they need antibiotics or the best way to convince them they need to quit smoking or lose weight and honestly, sometimes this weighs on my mind especially when I have someone demanding something I know is unreasonable.  With all the money that is tied to patient satisfaction scores between Medicare reimbursements based on it as well as organizations and practitioners trying to maintain patient loyalty and the insurance money that comes along with it, it’s no wonder it frequently feels like our profession is starting to more resemble that of the service industry, but saddled with the complexities of human health.  To tie back to the car problem I had, a colleague of a colleague now routinely, in her follow up emails to her patients, has a little tagline at the bottom talking about what they should do if they get a survey!  Patient satisfaction scores are now commonly being tied to physician pay and advancement or retention at their current job, the theory being to incentivize us to make that extra effort to make sure our patients are satisfied by their medical care. And in theory this sounds great because why shouldn’t patients have an exceptional experience every time?  Why should they expect any different of us than they do of their mechanics (and I don’t mean to pick on mechanics - I’m just still a little miffed at mine).  Well, there may be a few reasons, just because our profession is a little different than many others in the service industry.A study[1] just published a few weeks ago in the Journal of Patient Preferences and Adherence (which I, like I’m assuming many of you, didn’t realize existed until today) surveyed about 4000 doctors in a state medical society about exactly this which came back with 155 responses.  Granted, that’s a small number, but there are some disturbing statistics from it.  First, 78% of respondents said that these patient satisfaction surveys affected their job satisfaction moderately to severely, with 28% of people saying they had thought about leaving the medical profession because of them.  Second, just over half of respondents said that they had inappropriately prescribed an antibiotic or narcotic or ordered an unnecessary lab test because of the patient satisfaction surveys. Finally, while there were 6 neutral or positive comments given on patient satisfaction rankings, there were 47 negative comments and I do appreciate the fact that they published them.  They are actually divided up into 5 themes including the two I just mentioned as well as:- that they’re a poor way to evaluate the quality of medical care,- that there’s too much weight on them and not enough administrative effort to improve medical outcomes- that they’re perceived as a conflict of interest        So that’s one side of the coin, and it is a very striking viewpoint from the healthcare providers, but that study of course needs to be taken with a grain of salt as it is a small number and certainly could select out for the most dissatisfied people to respond.        But what about from the patient side?  Is there validity to these viewpoints?  Well there was another larger study about two years ago[2] published in JAMA that looked at patient satisfaction surveys from 52,000 respondents over 7 years.  While high patient satisfaction was associated with less frequent ER visits, it was also associated with greater inpatient use, higher overall healthcare and prescription drug expenditures, and increased mortality.  There was commentary both ways when this article came out and to date as best I could see, there hasn’t been another large study published regarding this, but if you could tie it to the study looking at the physicians perspectives on these surveys, it’s not hard to believe this data might be real.        So what are we supposed to do with all this information?  There’s been so much change in what the role of a physician is supposed to be.  We’ve gone from a paternalistic approach more to one of shared decision making.  But is the heavy weight of patient satisfaction surveys unbalancing the power of shared decision making?  And aren’t we now being asked to be more paternalistic to society by reducing health care expenses, antibiotic and opioid prescriptions, and be more dogged about preventative cancer screenings and weight loss?        I think the missing part of the equation is time and communication and we definitely do need to be able to clearly communicate our thoughts with patients.  So far, I’ve seen great feedback from people when I’ve actually stopped and instead of auto-refilling medications I don’t think are appropriate or ordering easy labs I know aren’t indicated, I explain to them why doing so would give me that gnawing pit in my stomach that knows when I do something wrong.  We’re all so rushed these days, I think this gets lost and I’ve certainly been guilty of it on occasion, but this has to be a point where we hold the line.  This is why most of us went into medicine in the first place - to help people and to be educators, not to be vending machines.          I haven’t gotten a call for a survey for the car dealership yet.  I’m conflicted though and I actually feel differently now than when I started writing this.  If I give my honest opinion, which was my emotional urge and first reaction, it’s going to bring down someone’s score, it might get him disciplined or he might take a salary cut - it could contribute to his getting fired.  And the errors made were system errors too - probably nothing to do with the guy I dealt with.  Giving anything but 5’s would just seem spiteful.  I guess I’ll wait and see …I know I covered a lot of ground here and I’m hoping you have a few opinions to share (and at this point I feel like I know who all of “you” are as I haven’t seen too many downloads yet) so as always, we’d love to hear your feedback on this, so if you have any burning questions or comments,  you can post them on the blog at canocecp.blogspot.com, our podcast page on itunes, our facebook page or email them to canocecp@gmail.com.  And if you have time, be sure to head on over to our facebook page for the Northern California Chapter of the ACP Council of Early Career Physicians so you can find out more about the events going on in the chapter. We butchered Bond history last time out, so why not give the same treatment to horror films now? Actually, it's not that bad, as Kai has plenty of horror street cred; in fact, it's Dylan's lack thereof that Kai mostly wanted to speak about (though we're both pretty lacking when it comes to vintage horror flicks). What makes a great action film? Especially, what makes a great one in the 25 years since Die Hard, which is widely regarded as the quintessential action film, which spawned countless ripoffs, which has become the one against which most modern action films have been judged? Those were the questions the folks at Vulture attempted to answer a few months back, posting a list of the 25 best actions films since John McTiernan's 1988 classic. It was an eclectic, somewhat controversial list that I not only really dug - even if I didn't agree with all of the choices - but thought would make for a solid jumping-off point for a discussion on...another episode of Film Don't Hurt. It's 2013, and all the biggest films are based on comic books. The top sitcom of the last half-decade features nerdy scientist types as leads...and it's not about them being bullied. A certain convention set in Southern California dominates pop culture for a week every year, and has spawned a million wannabe conventions all around the country. People who have scads of Star Wars action figures aren't mocked, but rather celebrated (and can make a pretty penny off them, if they're old enough and in good enough condition). So the question is: what does it mean to be a geek anymore? Who is a geek? How far does one have to go to be considered a geek amongst the geeks? These are the questions that plague Kai, so what better place to talk about them than with his MILF co-founder on another episode of FILM DON'T HURT? Then Scot welcomes Tom Shields, Peter Carpentier, and Rick Kiernan to discuss the Alpha for Catholics program in the Boston archdiocese. Finally, Scot and George Martell talk about George's photographs and the unique ways the Internet allows him to share them. **1st segment:** Scot read a statement from Jim Wright, president of The Station of the Cross, on Fr. John Corapi's administrative leave and then read a statement from Fr. Corapi from his website. The Station of the Cross has suspended ticket sales for the August 6 conference with Fr. Corapi pending consultation with advisors and resolution of the situation. * [Fr. John Corapi's statement on his website](http://www.fathercorapi.com/default.aspx) **2nd segment:** Scot welcomes Pat Gohn and Rachel Balducci of Faith and Family magazine. Pat said Faith and Family magazine and their website is a ministry of high quality for Catholic families and especially with Catholic moms in mind. Faith and Family Live is a great resource that brings together bloggers and has built a close-knit online community. Mom's Day Away is a day to bring the community of Faith and Family together. Rachel talked about the experience of meeting so many of the women who've been reading her blogs and book. It's an opportunity to give these Internet relationships a soul. There is a challenge to forming relationships online. It can be easy to forget there's a person on the other end of the computer. She was at the Catholic New Media Celebration in Boston last summer and had a great time meeting so many real life connections with people she'd already met online. You need the real-life connections to go along with the online interactions. One of the three keynotes is Jennifer Fulwiler. She grew up never believing in God, raised as an atheist. After getting married, she and her husband starting exploring the lack they felt in their lives. The Internet became a resource for her and through the Catholics she met online she discovered God and joined the Catholic Church. * [Jennifer Fulwiler's "Conversion Diary" blog](http://www.conversiondiary.com) Danielle Bean is the editor of Faith and Family magazine and one of the other speakers. Pat said it is her vision and leadership that has brought the magazine and website to where it is today. She's an author as well as wife and mother of 8. She brings a lot of exuberance and energy to everything she does including this day. * [Danielle Bean's books on Amazon.com](http://www.amazon.com/gp/redirect.html?ie=UTF8&location=http%3A%2F%2Fwww.amazon.com%2Fs%3Fie%3DUTF8%26redirect%3Dtrue%26ref_%3Dsr_tc_2_0%26keywords%3DDanielle%2520Bean%26field-contributor_id%3DB001KHVGS2%26qid%3D1300738838%26sr%3D1-2-ent%26rh%3Di%253Astripbooks%252Ck%253ADanielle%2520Bean&tag=catholicnetrevie&linkCode=ur2&camp=1789&creative=390957) * [Danielle Bean's blog](http://daniellebean.com/) Stoneham was chosen as the location for this first day because it was close to the publisher's home base in Connecticut and in a larger metropolitan area. Fr. Bill Schmidt of St. Patrick, Stoneham, generously offered the use of his very beautiful parish, which is conveniently located on a major roadways. They are keeping it small with just 200 tickets available. Rachel said although it's called Mom's Day Away, it's for all women. The three speakers all have big families and know what it takes to extricate themselves from the home. It's an opportunity to get together with other women, but also to get spiritual refreshment. It can be as intense or as laid back as they want it to be. She hopes women will go home refreshed and energized and able to be the best women they can be. Pat said her hope for the day is that there is a lot of laughter and time to talk over a great luncheon. There will be moments of prayer, adoration, silence, confession and the celebration of the Mass. They are joining together to build community between the women who come. There is a desire to celebrate the vocation of motherhood. Rachel said they're not forgetting about Lent with this celebration, but they're putting aside the sackcloth and ashes for just one day.  Scot asked whether men should buy tickets for their wives to go? Pat said it would be a great Lenten service to the family if the husband could say to his wife that he will take over with the kids for a day so she can have this day away. Of course for moms with husbands who work on the weekends or are single moms, it would be great for others to step forward for them so they can get away. She also asked for prayers for the day. It will be 8am-5pm, April 2 at St. Patrick's Parish hall, Stoneham. Less than half of the available tickets are left. Tickets must be purchased online in advance, not at the door. * [Faith and Family Live's Mom's Day Away page](http://www.faithandfamilylive.com/blog/register_today_for_moms_day_away/) **3rd segment:** Rachel is the mother of 5 boys (and 1 baby girl) and she chronicles her experiences raising them on her blog. She started blogging 5 years ago when she had 4 boys under the age of 8. When the fourth boy became 3, life became overwhelming with a pack of boys roaming her home. Girls provide a tempering influence on their brothers. As a former newspaper reporter, writing is therapeutic for her and the blog helped her take a step back and look objectively at her life with her kids. Sharing funny stories helped her deal with the difficulties. Other moms write to her about how it helps them deal with raising boys.  She has readers from all over the world. She recalls within the first couple of months getting a link from The National Review magazine's "The Corner" blog and she started to see readers from Australia and Europe and elsewhere. She felt good that her writing was reaching all over the world and encouraging the readers. One reader thanked her for reminding her to find the humor in her life's struggles and stresses. * [National Review's The Corner](http://www.nationalreview.com/corner) Her new book is "How Do You Tuck in a Superhero". Her aim is to help other moms become excited that God sent them boys too. Having boys helps her recognize that her job is to raise the boys to know, love, and serve Jesus, but boys are challenging because boys aren't particularly and innately religious. She wants to raise them to love Jesus and to be civilized human beings too. Scot asked whether Rachel always had a sense of humor or did she get it from raising boys. She said she comes from a very funny family that gave her a great sense of humor. **4th segment:** Pat agreed with Scot that Rachel has a great sense of humor and a great perspective. Scot said it's striking to him how close the pioneers in Catholic new media have become just through online relationships. Pat said using the Internet to make connections is part of the new evangelization. It's a new way of reaching out and touching the world in an evangelistic and missionary way. One of Pat's first new media initiatives was the Among Women podcast. Pat had already been writing online and as she got to know more and more writers, she found many of them starting podcasts. She explained what a podcast is: It is like a radio show that downloads to your computer and you can put it on your iPod or other digital media players to play anywhere. You can find them on individual websites or in iTunes. "Among Women" is a phrase from the Hail Mary and she chose it because when she's with other Christian women, she's happy to find community in Christ in the unique ways they relate. The podcast is part faith-sharing, part teaching, and part cup of coffee together. An episode could be about how to use the Bible as a devotional aid in prayer or recovery from abuse or women's issues. It looks at spiritual issues in women's lives and gives them expression. The first segment of each program is a profile of a saint or mystic and the second is a conversation with a woman sharing something from her own life. It could be an author or speaker or just a woman in the pew. It celebrates the faith and beauty and grace in a well-lived Catholic life. She's reached 100 episodes with her show. She and her husband Bob are very active at St. Michael's in Andover. They've been involved in faith-sharing groups, like Renew and Arise; prayer groups; Adoration of the Blessed Sacrament; and music ministry. Their involvement evolved as their family has grown and evolved. St. Michael's is the largest parish in the Archdiocese and has a very long list of ministries and programs available. Pat has worked on the ministry team at the parish and saw how much work is involved. The downside of such a large parish could be the need to find a ministry to become involved with so one can feel a part of a very large community. That can happen in any parish. It's important to make familiar connections with that parish, to find opportunities for one-to-one sharing and relationships. * [St. Michael Parish, Andover](http://www.saint-michael.org/) **5th segment:** Scot welcomes Tom Shields, Peter Carpentier, and Dick Kiernan. Alpha for Catholics is a 10-week practical introduction to the Christian faith, primarily for non-church goers but also for Catholics refreshing their understanding of the faith. It includes a dinner followed by a 45-minute video and a 45-minute discussion. There's also a weekend away mid-course.                      It was first designed for non-Catholic Christians, but Catholics asked for it as well. Many bishops around the world have recommended Alpha for Catholics as well as papal preacher Fr. Raniero Cantalamessa. Some of the fruits of Alpha included the story of a Springfield diocese priest who was preparing to retire, but when he saw the effects of Alpha in enlivening his parish, he asked the bishop to delay his retirement.  There is also Alpha for Youth, which uses youth presenters and talks and videos intended for them. Many agnostic and atheist-leaning kids come in and are completely turned around.  There have been 16 million globally who have gone through Alpha. In New England, there have been more than 5,000. Why should Catholics do Alpha? More than 80% of Catholics don't attend Mass and many of those who do go, go out of obligation. Alpha helps engender a personal relationship with Christ. Alpha attendees routinely come to full sacramental participation in their parishes. It provides an important point of entry back into the Church for lapsed Catholics as well. It works best with the blessings of the pastor. There is a regional team of Catholic advisors to help train the parish's Alpha team. There are five upcoming luncheons and evening gatherings for pastors, parish staff, and lay volunteers to introduce them to Alpha leading up to training in June, which will equip them to lead Alpha in their parishes. * March 28, St. Mary, Rowley * April 4, St. Malachy, Burlington * April 5, Sacred Heart, Bradford * April 11, St. Joseph, Needham * April 14, St. Joseph, Holbrook One of the good ways to start Alpha is to attend a course at a nearby parish with other key leaders from your parish. A list of Alpha courses is available on the Alpha USA website. * [Alpha USA: New England](http://alphausa.org/Groups/1000045884/New_England.aspx) * [Alpha New England](http://www.facebook.com/group.php?gid=35964711541) * [Alpha for Catholics Boston](http://www.northshorecm.org/AlphaForCatholics.html)  **6th segment:** Scot welcomes George Martell, the archdiocesan photographer, to talk about the Boston Catholic Flickr.com photography site. As he shoots photos at live events, the photos are automatically uploaded so they are available within minutes of being shot. It's especially good for people to follow along with friends and family who are on pilgrimages. George and Dom Bettinelli will be with the World Youth Day pilgrimage to Madrid this summer. George said that working with the Boston Herald for almost 20 years he never had that immediacy of contact with viewers. * [The BostonCatholic Flickr page](http://www.flickr.com/BostonCatholic) * [George's photos of the devastation of Haiti after the earthquake](http://www.flickr.com/photos/bostoncatholic/collections/72157623543912342/)
