Fresh off their presentation at the 2026 Voices of Dentistry conference, the Eric and Alex (a married couple of dentists) dive into the logistics of optimizing a practice for sale, emphasizing that maximum value is built in the 18 to 24 months before a transition by tightening systems and improving hygiene "show rates." The conversation balances technical advice on EBITDA and overhead reduction with a candid—and occasionally spicy—critique of the post-COVID hygiene labor market, ultimately highlighting how dentists can prepare their businesses for a successful handoff to either a private buyer or a DSO. In their session and conversation with Alan, Dr. Alex Cantor and Dr. Eric Applesies highlighted several key levers to pull when prepping a practice for sale. Their core philosophy is that "a dollar saved is $4 to $6 earned" during a valuation. Key Optimization Strategies The 12-to-24-Month Runway: Valuations are typically based on the last 12 months of profit (EBITDA). To ensure your systems are fully "flushed out" and consistent, start implementing changes 18 to 24 months before you intend to sell. Hygiene "Show Rates": Increasing your show rate from 70% to 90% is considered "low-hanging fruit." Because hygiene has low variable costs (mostly just prophy paste and floss), nearly every dollar of a kept hygiene appointment goes directly to the bottom line, which is then multiplied during a sale. Aggressive Appointment Confirmation: Use automated texts to keep appointments top-of-mind. The "48-Hour Rule": If a patient hasn't confirmed 48 hours out, inform them they will be removed from the schedule. Follow Through: Actually remove unconfirmed patients to give your team two days to fill the spot with someone who wants to be there. Overhead Reduction (The "Spend" Side): Supplies: Move away from the "old school" model of buying through a single rep who brings donuts. Use platforms like Crazy Dental or Net 32 to treat supplies as a commodity and secure the lowest price. Labs: Look for efficiency gains through model-less workflows and digital scanning, which can reduce lab fees and turnaround times. Clinical Extraction: If you want to retire immediately after the sale, stop working clinically one year prior. This proves to a buyer (especially a DSO) that the practice's revenue is sustained by systems and associates, not just your individual production. DSO Buyer: Better for high-revenue practices ($5M+ range), but usually requires you to stay on for several years as an associate.     Real Life Pharmacology - Pharmacology Education for Health Care Professionals

    Fibrate derivatives are lipid-lowering medications that primarily target triglycerides rather than LDL cholesterol. Common agents include gemfibrozil, fenofibrate, and fenofibric acid. While their use has declined with the widespread adoption of statins, fibrates remain an important option for patients with severe hypertriglyceridemia, particularly to reduce the risk of acute pancreatitis rather than for routine cardiovascular risk reduction. Fibrates work by activating PPAR-alpha, which increases lipoprotein lipase activity and enhances the clearance of triglyceride-rich lipoproteins. This leads to significant reductions in triglycerides, modest increases in HDL cholesterol, and variable effects on LDL cholesterol. Because they are not strong LDL-lowering agents, fibrates should not replace statins in patients who require LDL reduction, but they can be effective in select clinical scenarios when triglycerides are the primary concern. From a safety standpoint, fibrates are generally well tolerated but require careful monitoring. Common concerns include gastrointestinal side effects, liver enzyme elevations, gallstone risk, and muscle toxicity, especially when combined with statins. Gemfibrozil carries a higher risk of drug interactions, while fenofibrate is usually preferred if combination therapy is necessary. Appropriate patient selection, lab monitoring, and lifestyle counseling are essential to maximize benefit and minimize harm when using fibrate derivatives.     New research which has just been published in the British Medical Journal, suggests that testing menstrual blood for signs of cervical cancer could be an accurate way of screening for the disease. It's a creative adaptation of an acclaimed memoir by American writer Lidia Yuknavitch which centres on her coming to terms with being abused as a child, battling pain and loss, and her ongoing healing journey. Imogen Poots joins Anita in the studio.The Kurdish-led self-administration in the north east of Syria is a territory where for years women have sat at the centre of political life, security and decision-making. But many are worried that the system is now under pressure following a new agreement between Kurdish authorities and the Syrian government, which will integrate the region into the Syrian state being rebuilt after the toppling of Bashar al-Assad in 2024. Anita is joined by Lina Shaikhouni, journalist at the BBC World Service and Dilar Dirik, Kurdish writer and author of The Kurdish Women's Movement: History, Theory, Practice.Paula Varjack talks to Anita about her show Nine Sixteenths. Despite the country's instability, Deux Mains continues to thrive. “The business is not only growing but flourishing under the leadership of my Haitian colleagues,” Julie said.Beyond exporting chic handbags, Deux Mains has a significant local impact. Last year, they manufactured and distributed 10,000 pairs of leather school shoes for Haitian children. The factory, powered entirely by solar energy, employs 45 full-time staff and serves as a model for sustainable development in Haiti.Julie's story is one of transformation—both personal and collective.     https://teachhoops.com/ Practice expectations are the "unwritten rules" that dictate the ceiling of your program's success. As a coach, you must realize that you are not just teaching basketball; you are teaching a standard of excellence. This begins the moment a player walks into the gym. Whether it's the "shoes on, phones away" rule ten minutes before the whistle or the requirement that every player sprints to the center circle for a huddle, these rituals establish that practice time is sacred. In the mid-season grind of January, it's easy for these standards to slip. However, elite programs understand that "how you do anything is how you do everything." If you allow a player to cut a corner on a sideline sprint, you are inadvertently teaching them to cut a corner on a defensive rotation in a one-point game. The second pillar of practice expectations is vocal engagement and communication. A quiet gym is a losing gym. You must set the expectation that players are "talking to the ball" and calling out screens on every single repetition. This isn't just about noise; it's about "Basketball IQ" and shared accountability. When your veterans are the loudest players on the floor, it creates a culture where the younger athletes have no choice but to follow suit. Use "The Three-Second Rule"—if a coach has to wait more than three seconds for a player to respond or get to their spot, the energy is too low. By keeping the pace high and the communication constant, you create a "flow state" where the focus shifts from individual fatigue to collective execution. Finally, expectations must be rooted in measurable effort. Instead of simply asking your players to "play hard," define what "hard" looks like: every loose ball is a dive, every shot is boxed out, and every transition is a full-field sprint. Use a "Culture Scorecard" during practice to reward these "zero-talent" traits. When players know that their effort is being tracked as closely as their shooting percentage, their focus naturally sharpens. By the time you reach the postseason, these expectations should be so deeply ingrained that the players are holding each other accountable. This transition from "coach-led" to "player-led" standards is the hallmark of a championship-caliber team that is ready to win when the pressure is at its highest.     (Spirit Rock Meditation Center) We first hear from a member of the community about how he is experiencing and responding to what's happening in the larger society and world in our times. We then fairly briefly review last week's session, first identifying the three traditional areas of training--in wisdom, meditation, and ethics--and how each can be important resources for responding to what's happening in our own experience and in our society and world. We focus especially on reviewing our exploration of "ethical practice," responding in our everyday lives and in the larger society and world in caring and compassionate ways. We then explore the traditional figure of the bodhisattva as one who brings together deep commitments both to awakening and to helping others--helping others both in awakening and in terms of their life needs. We look at examples of bodhisattva vows from Theravada, Japanese Zen, and Vietnamese traditions, as well as from passages from Shantideva's "Guide to the Bodhisattva's Way of Life." We show images of archetypal bodhisattvas such as Avalokiteshvara, Tara, Kwan Yin, and Manjushri and discuss the ten ways of training of the Mahayana bodhisattva. We suggest a number of contemporary exemplars of the bodhisattva vocation, and invite participants to develop their own personalized bodhisattva vows. The talk is followed by discussion.

"How do I extend positive mood and high energy over my lifetime?" Using distributed neuroscience technology and the Six app (measuring brain activity continuously at one-second frequency), Dr. Zak's research team discovered something revolutionary: People who have 6 or more "key moments" daily are truly thriving—engaged in life, resilient to stress, and sustaining high performance. What Are Key Moments and Why Do They Matter? "Key moments are high-value experiences that help us grow as human beings and thrive," Dr. Zak explains. "What we found is that the systems in the brain that give us these high-value moments are deep in the brainstem, hidden from our conscious awareness." Dr. Paul Zak This explains why traditional self-assessment wellbeing surveys fail: Most people cannot accurately identify what truly makes them happy. "When we ask people, 'What was your most important moment yesterday?' they don't know," Dr. Zak reveals. "Because it's hidden from conscious awareness. Many times, people will do something they think is really fun that doesn't give their brain a lot of value." The Neuroscience: Why Social Connection Drives Happiness Recent research from Oxford University confirms what Dr. Zak's neuroscience proves: About 50% of our happiness is due to the quality of our social relationships. But here's the critical finding for women leaders: 80% of key moments are social experiences. "It's the people that give me that ability to be present and emotionally open," Dr. Zak emphasizes. "Sometimes I'll get a key moment when I'm really in a great writing project, but mostly, it's when I'm out at a conference, having dinner with people, giving talks." "No. If I say 'I'm having a tough day' and you're like 'oh, that's terrible' with no emotion—that's not a friend, that's a robot." Emotional experiences are saved in memory in a particular way that makes them more easily accessible. When you share authentic emotions, you activate neural pathways that build trust, create connection, and generate the key moments that sustain thriving. Critical for Women Leaders: This isn't about oversharing or being "too emotional" (a bias women already face). It's about strategic vulnerability that makes you relatable, trustworthy, and capable of building the deep connections that drive both happiness and high performance. The Love Plus Algorithm: A Neuroscience Framework for Daily Happiness When Time Magazine asked Dr. Zak to write three sentences on New Year's resolutions, he created what he calls his "algorithm for living a happy and fulfilled life": Love Plus. The Love Plus Framework: L - Love and be loved Invest deeply in relationships. Research shows 50% of happiness comes from social connection quality. For women leaders, this means prioritizing meaningful relationships with family, friends, and trusted colleagues—not just networking transactions. O - Openness to new experiences Travel, try new activities, engage with different perspectives. Novel experiences create neurological growth and generate key moments that sustain thriving. V - Volunteering and giving back "The evidence is so overwhelming that helping others makes you happy," Dr. Zak notes. Even small acts of generosity—buying a colleague coffee, mentoring a junior team member—create reciprocal happiness loops. E - Exercise Physical movement isn't just wellness theater. It oxygenates the brain, reduces stress hormones, and creates conditions for key moments to emerge. PLUS: • Purpose: Connect daily work to larger meaning and impact • Learning: Continuous growth through reading, courses, new skills • Unique experiences: Prioritize extraordinary moments that create lasting memories • Silence: Create space for reflection, creativity, and strategic thinking How Women Leaders Apply Love Plus Daily Dr. Zak's framework isn't theoretical—it's immediately actionable: Morning: 10 minutes of silence before checking devices (builds presence, reduces cortisol) Workday: 2-3 "connection moments" with team members beyond task management (builds trust, creates key moments) Lunch: Walk outside without phone (exercise + silence + openness to new observations) Afternoon: Learn something new—read an article, take a short course, explore a topic (continuous learning) Evening: Invest in deep relationships—quality time with family/friends, not just logistics (love and be loved) Weekly: Volunteer or mentor (giving back creates sustained happiness) The Do-Not-Do List: Creating Bandwidth for Thriving "Many executives tell me they don't have time for key moments," Dr. Zak acknowledges. His solution? The do-not-do list. "I realized I was doing a lot of things on my to-do list that weren't actually that valuable. So I made a second list called my do-not-do list. And it's way longer than my to-do list." Examples from Dr. Zak's Do-Not-Do List: • Do not attend meetings without clear agendas and time boundaries • Do not respond to every email within 2 hours (batch processing instead) • Do not say yes to every speaking invitation (protect creative bandwidth) • Do not schedule back-to-back meetings all day (protect key moment opportunities) • Do not work weekends as default (protect relationship investment time) For Women Leaders: What activities drain energy without creating value? It was a showcase under unfamiliar circumstances, with coaches installing concepts on the fly and players trying to absorb it all. Practice winners with a Detroit lens One offensive winner stood out. Wyoming tight end John Michael Gillenborg flashed real juice. He is a former basketball player who played only three high school football games after COVID wiped out his senior year. Athlete first, growing football player second. In one on one drills he was a problem. He separated cleanly. He was uncoverable for stretches.
