Two wide open cases from the latest issue of the journal. Starting with a case from Italy (1:18), of a 63-yo man having a history of behavioural and cognitive problems since retiring. His many changes included low mood, significant weight loss, and problems with sleep and temperature regulation. He had a background of type 2 diabetes. Initial treatment was on a suspicion of Alzheimer's, but there was no clinical improvement. https://pn.bmj.com/content/25/2/159 A case from the United States is next (17:36), featuring a 66-yo lady experiencing 10 days of generalised weakness, with episodes of forgetfulness and a series of falls. She had previously been treated for left-sided ophthalmic herpes zoster. Neurological examination showed mild right arm and leg weakness. https://pn.bmj.com/content/25/2/164 The case reports discussion is hosted by Prof. Martin Turner¹, who is joined by Dr. Ruth Wood² and Dr. Babak Soleimani³ for a group examination of the features of each presentation, followed by a step-by-step walkthrough of how the diagnosis was made. These case reports and many others can be found in the February 2025 issue of the journal. A puzzling pair of Case Reports from the most recent issue of the journal. First up (1:35) is a man in his mid-fifties, presenting with lumbar spine fractures, which then developed into confusion, vomiting, and abdominal pain. An x-ray showed dilated intestinal loops and his blood sodium levels were low. https://pn.bmj.com/content/25/1/87 The second case (23:06) involves a 21-yo woman, who presented at 18 weeks pregnant with multiple episodes of right upper limb tonic extension, and subsequently developed new-onset refractory status epilepticus (NORSE). https://pn.bmj.com/content/25/1/56 The case reports discussion is hosted by Prof. Martin Turner¹, who is joined by Dr. Ruth Wood² and Dr. Xin You Tai³ for a group examination of the features of each presentation, followed by a step-by-step walkthrough of how the diagnosis was made. Two more fascinating Case Reports from the latest issue of the journal. The first case (1:15) is of a 57-yo woman, with an intermittent posterior headache, which had an associated bilateral pressure-like sensation. The immune system is a recurring feature in the cases discussed in this edition of the Case Reports podcast. The first paper details the cases of two young women, sisters, presenting with overlapping conditions but resulting in tragically different outcomes (1:16). Both were in their twenties, had given birth recently, and developed limb weakness along with several other neurological symptoms. https://pn.bmj.com/content/24/5/422 Our second case is a 72-yo woman with a range of symptoms including diplopia, ptosis, myalgia, and worsening shortness of breath (21:27). She had had surgical resection of a malignant melanoma, and was receiving immunotherapy treatment. Her presentation resembled myasthenia gravis, but initial treatment did not yield a response. https://pn.bmj.com/content/24/5/428 The case reports discussion is hosted by Prof. Martin Turner¹, who is joined by Dr. Ruth Wood² and Dr. Xin You Tai³ for a group examination of the features of each presentation, followed by a step-by-step walkthrough of how the diagnosis was made. This month's cases both feature sudden onset neurological syndromes. The first case (1:23) is that of a 26-yo Brazilian man who awoke from sleep with weakness in all four limbs. The signs suggest a possibility of Guillain-Barré syndrome or polio. A normal cranial nerve examination follows, with no unusual findings - https://pn.bmj.com/content/24/4/342 A 69-yo woman is the subject of the second case (14:33), after she presents with sudden onset unsteadiness and slurred speech when getting out of bed. A stroke was initially examined for by CT head scan, proving unremarkable, but a subsequent MRI scan showed an intense midbrain lesion. The patient subsequently improved, but then returned three months later with occurrences of the same symptoms multiple times throughout the day - https://pn.bmj.com/content/24/4/310 The case reports discussion is hosted by Prof. Martin Turner (1), who is joined by Dr. Ruth Wood (2) and Dr. Xin You Tai (3) for a group examination of the features of each presentation, followed by a step-by-step walkthrough of how the diagnosis was made. Two highly unusual cases in this edition of Case Reports. The first case describes a 64-yo woman presenting to the emergency department with a five day history of bizarre behaviours (1:45). A BBC radio show prompts her, out of character, to reflect aloud about her childhood, and she experiences recurrent periods of unresponsiveness followed by intense agitation. She was kept in hospital for scans and discharged after two weeks, but returned soon after with a similar presentation - (link) A lifetime bodybuilder is the patient in the second case (21:12), with a practice of anabolic steroid injection over several decades. He presents with a three year history of unsteadiness when walking and tingling in his feet, as well as reduced dexterity. His symptoms are found to be brought on by a toxicity from an unexpected source - (link) The case reports discussion is hosted by Prof. Martin Turner (1), who is joined by Dr. Ruth Wood (2) and Dr. Xin You Tai (3) for a group examination of the features of each presentation, followed by a step-by-step walkthrough of how the diagnosis was made. Kaj lahko o njej razberemo iz krvi? Zakaj so geni le del odgovorov?

Do you like to keep fit? We're always told that regular exercise is good for our body and mind. More and more people are taking up activities that improve their fitness. But is there a risk some of us might get obsessed and overdo it? Well, for some people, fitness has become an obsession as they aim for perfection. And fitness trackers and apps can add to this addiction, especially if someone is driven by achievement and perfectionism. And sharing data on social media means exercising becomes public and competitive, which could cause problems in someone who is vulnerable. Experts say this can lead to a medical condition called orthorexia nervosa, or addiction to ‘healthy' eating and over-exercise. Untreated, it can lead to malnutrition and mental health complications. According to a BBC Worklife article: “Research from scientists at University College London in 2017 found that higher Instagram usage was associated with increased likelihood of developing orthorexia, especially among followers of the ‘healthy eating' influencers.” Too much exercise can also take its toll on someone's physical health as well. Symptoms of over-exercising include injuries such as stress fractures, tendinitis and a low immune system. So how much exercise is too much? Research by the Journal of the American College of Cardiology found the ideal pace to jog was about eight kilometres per hour – and that it was best to jog no more than three times a week or for 2.5 hours in total, showing that moderate jogging is possibly more beneficial than being inactive or undertaking strenuous jogging. If you're more of a couch potato than a sprinter, this might sound like good news. 50 years after its initial release, Wishbone Ash's third studio album is still influencing rock bands the world over. In a special anniversary episode of 'My Classic Album', Mike is joined by Firekind bass player Dan Morris to celebrate the album 'Argus'. Mike is also joined by the creative force behind 'Argus', former bass player, vocalist and founding member of Wishbone Ash Martin Turner as he reflects on 50 years of his magnum opus. Where did Wishbone Ash get the inspiration for their trademark twin guitar sound? Who was the inspiration behind fan favourite 'Blowin' Free'? With both cricketer Ben Stokes and swimmer Adam Peaty taking time off for their mental wellbeing, we're joined by sports psychologist Dr Martin Turner to discuss the methods he uses to help sports stars cope with acute pressure and adversity. We hear from a former teammate of Biles on the unique demands of gymnastics and we bring you a moving interview with British diving medallist Jack Laugher, who reveals the mental anguish that nearly made him quit the sport.
In this episode we bring you a couple newsletter articles from Teddy Gentry and Martin Turner. Staying The Course with Teddy Gentry and Mistakes To Profit From with Martin Turner. Both are great information on management of cattle.

The 2021 Barkley Marathons Had No Finishers This year’s Barkley Marathons looked slightly different than previous editions. There were few international competitors due to pandemic-related travel restrictions, and because of this, a smaller field of runners (usually 40 runners line up), almost entirely Americans, took on the beast. Additionally, runners were required to get COVID-19 tests ahead of arrival, among other safety protocols. A strong contingent of Barkley veterans lined up at the start, including Big’s Backyard champion Maggie Guterl, two-time Barkley Fall Classic winner Liz Canty, the only three-time Barkley finisher Jared Campbell, and multi-time Barkley attempter Jamil Coury. There was also a lot of excitement around Barkley virgins (first-timers) Courtney Dauwalter, Western States and Ultra-Trail du Mont-Blanc champion; and Luke Nelson, Tor des Geants finisher. Guterl, Canty, and Dauwalter were attempting to become the first woman to finish the Barkley. The race started midweek, in order to deter spectators. The Briton is continuing his comeback from serious injuries at this week’s Volta a Catalunya, but despite headlining Israel Start-up Nation’s team, he was dropped on the opening stage and finished 90th on stage two’s time trial, 2-05 down on former Ineos Grenadiers team-mate Rohan Dennis. The upbeat and confident Froome has made no secret in declaring his intention to win a record-equalling fifth Tour de France, but asked by Cycling Weekly if that was still a realistic goal he could fulfil this season, he said: “I can’t really put any time on it. I am just taking it one week at a time at the moment. “It’s just about following the process and I am not putting a timeline on it. Naturally, I would love to be ready for the Tour de France but I have to keep following the process and do what I can.” What's New in the 303: How Triathlon was Introduced to the World The History and Background of Triathlons A “triathlon” is defined as a three-part sports discipline comprising swimming, cycling, and running. The three sports are contested as a continuous event without a rest. The triathlon can be an individual or team event over varying distances. Triathlon history dates back to the early 1970s and originated with the San Diego Track Club. The triathlon was designed to be an alternative to hard track training. The first triathlon event was held on September 25, 1974. Don Shanahan and Jack Johnstone are pioneers in the history of the triathlon. The San Diego Track Club sponsored the event. The triathlon then comprised a 10 km run, 8 km cycle, and 500 m swim. In 1989, the sport was awarded Olympic status and featured for the first time at the 2000 Sydney Olympics in Australia. Since then, the sport has grown in popularity. In fact, no other sport achieved Olympic status in such a short time. Over the next decade, the triathlon grew by leaps and bounds and soon gained recognition worldwide. They will work alongside multi award winning director, Martin Turner, who will head the broadcast team as Executive Producer. The Collins Cup is a new race format modelled after the Ryder Cup, which will see teams of International, European and USA athletes pitted against one another and aims to create and leverage the excitement, rivalry, drama and personalities of the sport of triathlon to attract a wider fan base. Burton destaca en «The Call of Ktulu» y «For Whom the Bell Tolls». So What!, para Cliff. Some Kind of Monster (2004), Metallica aún sigue teniendo a Burton presente. The Thing That Should Not Be.

Welcome to the 29th CONKERS’ CORNER. In this interview I have the privilege of speaking with award winning Investment Fund Manager @gervais_mam Gervais Williams. He is currently the Managing Director of Miton Group Plc, where he and the team monitor the market trend changes beyond the credit boom, and aim to set up funds with investment strategies harnessing forthcoming investment trends. Gervais grew up in Abergavenny in Monmouthshire; a county in South East Wales. As the son of a Farmer, he had always liked business but did not know how he was going to get into it. He attended the University of Liverpool where he attained an Engineering Degree and went onto become a Chartered Engineer. It was in 1985 that Gervais made the switch into the Investment industry, after successfully applying for a Trainee Fund Manager role at the Throgmorton Investment Trust. He started analysing small companies, did his industry exams and remained with the group for five years. Gervais first “dabbled a bit” in stocks and shares during the Thatcher revolution and the Conservative government’s de-nationalisation policy of utilities and services. He participated in some of the privatisations, however he admits that he has never really invested that much of his own personal account, preferring instead to invest in the funds he worked for. He states that when he started analysing small companies they were in fashion and were central to all portfolios His criteria then was to look for small companies which generated good earnings growth, with lowish PEs and limited downside risk. Gervais says that there were a lot of bankruptcies at that time and you had to be mindful of their accounting policy issues. Gervais became an Investment Manager in UK & Irish Small Cap Stocks at Thornton Investment Management (now part of Dresdner Bank) in July 1990. It was during this time that he worked with Peter Webb, managing the Thornton UK Smaller Companies Unit Trust. In March 1993 Gervais was appointed as a Fund Manager with Gartmore Asset Management. There he gained an extensive experience of investing in quoted stocks in the UK and Ireland, with a particular interest in companies with smaller market capitalisations. He headed up the UK Small Cap Desk for many years running portfolios such as UK & Irish Smaller Companies, Gartmore Growth plc, Gartmore Irish Growth Trust plc and Gartmore Fledgling Trust plc.During this period of success the team collected many investment awards. The humble Gervais once said that he joined Gartmore at “a fortuitous time as it coincided with a major shift in the markets after a prolonged period of recession”. He remained with Gartmore for nearly eighteen years. When Gervais decided to leave Gartmore he did so with the intention of looking at all the opportunities available to him before choosing to the best in join Miton Group in March 2011. To build up the assets under management and was able to offer investors opportunities to invest new strategies and funds. Since 2011 Miton have established four new funds that Gervais has managed along with Martin Turner, and taken on the mandate for The Investment Company plc. These funds have genuinely active investment strategies. The funds are: the Diverse Income Trust Investment Trust, the CF Miton UK Multi Cap Income OEIC, the CF Miton UK Smaller Companies OEIC and the Miton UK Microcap Trust. On a professional level Gervais has remained focused on small stocks throughout his career. So much so that his experience and passion for that niche led to him writing his book “The Future is Small” http://www.thefutureissmall.com/ explaining the unique characteristics of the UK’s AIM market for smaller, growing companies. When Gervais joined Miton they issued their Full Year results in April 2011 with Funds under Management of £1.7 Billion. Last month Miton http://www.mitongroup.com/ published their Interim results which showed they had £2.542 Billion in Assets under Management. Listen n...
