When we think weight loss, we think of restriction and cutting out the calories. Then begins the game of willpower; some people might also think of gaining weight as a lack of discipline or just a simple culmination of bad choices. But if you think about it, how free is our free will? Aren't our choices a by-product of the options we have to choose from and in the case of food, our natural instinct to satisfy hunger governed by various chemical reactions happening inside our body? When it comes to fitness, isn't willpower overrated? Talking of our internal environment, stabilizing our hunger hormones is the key to weight loss. In my definition, fattening foods are the ones that don't just come with empty calories but also never satisfy our hunger. Think of a box of cookies or a packet of chips; it might have the same or fewer calories as compared to a paneer parantha, but the fact that wholesome foods stabilize hunger as opposed to junk foods that make you even more hungry is what makes chips and biscuits ‘fattening'. Buttermilk is an appetite stabilizer; try having a glass of buttermilk around your peak hunger hours and see the magic. You will feel energized and not driven towards high-salt-high-sugar foods. There is no magic bullet that will reduce the mortality rate of the ARDS patients. It is the early and consistent applications of evidence-based supportive therapies that results in improve short and long-term outcomes. Understanding the ARDS Berlin definition that differentiates the syndrome based on severity of hypoxemia to be mild, moderate or severe, helps the experienced nurse target the correct supportive care. Our knowledge of the diffuse injury to the alveoli and capillary membranes caused by inflammatory mediators and ventilator-induced injury helps for greater understanding of the clinical signs/symptoms and why certain therapies support the ARDS patient better. Due to the complexity of the patient, it is critical that the skilled frontline nurse have a user-friendly structure (8 P’s) to trigger implementation of evidence based care practices. The goal is to PREVENT further injury of the lung, reduce ventilator-associated events and physical and cognitive decline. The team must utilize evidence-based ventilator strategies (PEEP) while ensuring adequate volume for the heart to work at maximum efficiency (PUMPS & PIPES) to help deliver oxygen to the tissue. The critical care nurse plays a key role in minimizing oxygen demand and balancing brain recovery through appropriate use of sedation and paralytic agents (PARALYSIS). The timing and implementation of the prone position (POSITIONING) can determine its effectiveness on oxygenation and prevention of lung injury. Once prone position is no longer useful, early progressive is critical to improve long-term cognitive and functional ability. Appropriate nutrition (PROTIEN) to promote healing and reduce inflammation is an essential component of the care plan. Incorporating evidence based (Protocols) such as the ABCDEF can impact overall outcomes in the ICU patient. It takes all our knowledge and skill to ensure the ARDS patient not only survives but is able to return to a meaningful life as soon as possible.

Our interview this week is with Board Certified Specialist in Sports Dietetics (CSSD) and Registered Dietitian Nutritionist (RDN), Heidi Strickler. Food is on my mind a lot these days and where I get my macro nutrients is something I consider carefully. Like many of you, I'm trying to get the best quality carbohydrate, fat and protein. I'm a meat eater, but have been experimenting more and more with plant based protein. Heidi is going to help all of us better understand how to do that successfully. Heidi is a Board Certified Specialist in Sports Dietetics (CSSD) and Registered Dietitian Nutritionist (RDN) who comes to eNRG Performance from Seattle, Washington. Heidi graduated with a triple degree in Dietetics, Nutrition in Sports & Exercise, and Exercise Science, from Seattle Pacific University in Seattle, Washington. She completed her Dietetic Internship in Illinois, and moved back to Seattle in early 2014 to begin her career as a Registered Dietitian. Heidi has spent most of her professional career at Experience Momentum, a functional wellness clinic in Lynnwood, Washington, where she worked with clients in both nutrition and fitness coaching. Heidi specializes in sports and performance nutrition, oncology nutrition, sustainability, and plant-based eating. She works with high school, collegiate, and adult team sports, CrossFit athletes, and endurance athletes, including runners, triathletes, cyclists, swimmers, and obstacle course racers. As an endurance athlete herself, Heidi's primary focus is with endurance athletes. It was Heidi's experience as an athlete that led her to nutrition. An interest in the human body and physical performance inspired her initial plan to pursue a career in physical therapy. However, once she began competing in college, she became fascinated by the connection between nutrition and performance, and changed the direction of her studies. Heidi is an avid, passionate and competitive athlete and outdoors enthusiast. She began playing soccer at the age of three, which carried her into college on a scholarship at Seattle Pacific University. She transitioned to running collegiate track and cross country during her junior year after discovering the world of long distance running. After college, Heidi took her love for running and passion for the outdoors to the trails of her hometown in Salt Lake City, where she picked up ultra-running and trail racing, and remains highly competitive on the trails today.
