The global health landscape faces unprecedented challenges, with diseases proliferating despite advances in medical knowledge and technology. Despite significant investment in research across multiple disciplines, including physics, chemistry, biology, and engineering, some diseases remain elusive to diagnosis and treatment. In contrast, pre-colonial India boasted a healthy populace with a robust understanding of health and effective indigenous medical systems like Ayurveda. However, the dominance of Western medicine during colonization marginalized these traditional practices. Now, there's a resurgence of interest worldwide, including in India, in alternative approaches like Ayurveda, signaling a shift away from solely chemical-based treatments. The systems approach of the Tridosha model of Ayurveda holds a lot of relevant and intriguing aspects that have been found to be effective in diagnosing and treating various physiological conditions. Having said that, in today's world, with the recent Patanjali lawsuit, it is important to delineate what is truly scientific within Ayurveda as well as protect this age-old tradition from those who want to destabilize it, due to ideological reasons. To discuss all this and more, we have with us one of Bharat's most illustrious practitioners and scholars of Ayurveda - Dr. Rama Jayasundar. Dr. Jayasundar heads the Department of NMR, AIIMS, New Delhi. She has had a rather fascinating trajectory, having received her doctorate in Physics from Cambridge University with training in Nuclear Magnetic Resonance. While pioneering biomedical NMR work in Bharat, she went for a medical degree in Ayurveda (BAMS – Bachelor of Ayurvedic Medicine and Surgery), and today is the only one at both the national and international levels to hold dual degrees in Physics and Ayurveda. A version of this essay has been published by Open Magazine at https://openthemagazine.com/essays/the-new-knowledge-war/Generative AI, as exemplified by chatGPT from Microsoft/OpenAI and Bard from Google, is probably the hottest new technology of 2023. Its ability has mesmerised consumers to provide answers to all sorts of questions, as well as to create readable text or poetry and images with universal appeal. These generative AI products purport to model the human brain (‘neural networks') and are ‘trained' on large amounts of text and images from the Internet. Large Language Models or ‘LLMs' are the technical term for the tools underlying generative AI. They use probabilistic statistical models to predict words in a sequence or generate images based on user input. For most practical purposes, this works fine. However, in an earlier column in Open Magazine, “Artificial Intelligence is like Allopathy”, we pointed out that in both cases, statistical correlation is being treated by users as though it were causation. In other words, just because two things happened together, you can't assume one caused the other. This flaw can lead to completely wrong or misleading results in some cases: the so-called ‘AI hallucination'. To test our hypothesis, we asked chatGPT to summarise that column. It substantially covered most points, but surprisingly, though, it completely ignored the term ‘Ayurveda', although we had used it several times in the text to highlight ‘theory of disease'. This is thought-provoking, because it implies that in the vast corpus of data that chatGPT trained on, there is nothing about Ayurveda.The erasure of Indic knowledgeEpistemology is the study of knowledge itself: how we acquire it, and the relationship between knowledge and truth. There is a persistent concern that Indic knowledge systems are severely under-represented or mis-represented in epistemology in the Anglosphere. Indian intellectual property is ‘digested', to use Rajiv Malhotra's evocative term.For that matter, India does not receive credit for innovations such as Indian numerals (misnamed Arabic numerals), vaccination (attributed to the British, though there is evidence of prior knowledge among Bengali vaidyas), or the infinite series for mathematical functions such as pi or sine (ascribed to Europeans, though Madhava of Sangamagrama discovered them centuries earlier).The West (notably, the US) casually captures and repackages it even today. Meditation is rebranded as ‘mindfulness', and the Huberman Lab at Stanford calls Pranayama ‘cyclic sighing'. A few years ago, the attempts of the US to patent basmati rice and turmeric were foiled by the provision of ‘prior art', such as the Hortus Malabaricus, written in 1676 about the medicinal plants of the Western Ghats. Judging by current trends, Wikipedia, and presumably Google, LinkedIn, and other text repositories, are not only bereft of Indian knowledge, but also full of anti-Indian and specifically anti-Hindu disinformation. Any generative AI relying on this ‘poisoned' 'knowledge base' will, predictably, produce grossly inaccurate output. This has potentially severe consequences: considering that Sanskrit, Hindi, Tamil, Bengali (and non-Latin scripts) etc. are underrepresented on the Internet, generative AI models will not learn or generate text from these languages. For all intents and purposes, Indic knowledge will disappear from the discourse. These issues will exacerbate the bias against non-English speakers, who will not think about their identity or culture, reducing diversity and killing innovation.More general problems with epistemology: bias, data poisoning and AI hallucinationsGenerative AI models are trained on massive datasets of text and code. This means they are susceptible to inherent biases. A case in point: if a dataset is biased against non-white females, then the generative AI model will be more likely to generate text that is also biased against non-white women. Additionally, malicious actors can poison generative AI models by injecting false or misleading data into the training dataset. For example, a coordinated effort to introduce anti-India biases into Wikipedia articles (in fact this is the case today) will produce output that is notably biased. An example of this is a query about Indian democracy to Google Bard: it produced a result that suggested this is a Potemkin construct (i.e., one that is merely a facade); Hindu nationalism and tight control of the media “which has become increasingly partisan and subservient to the government” were highlighted as concerns. This is straight from ‘toolkits', which have poisoned the dataset and are helped, in part, by US hegemonic economic dominance. More subtly, generative AI models are biased towards Western norms and values (or have a US-centric point of view). For example, the Body Mass Index (BMI), a measure of body fat, has been used in Western countries to determine obesity, but is a poor measure for the Indian population, as we tend to have a higher percentage of body fat than our Western counterparts. An illustration of AI hallucination came to the fore from an India Today story entitled "Lawyer faces problems after using ChatGPT for research. AI tool comes up with fake cases that never existed." It reported how a lawyer who used ChatGPT-generated precedents had his case dismissed because the court found the references were fabricated by AI. Similar risks in the medical field for patient treatment will be exacerbated if algorithms are trained on non-curated datasets. While these technologies promise access to communication, language itself becomes a barrier. For instance, due to the dominant prevalence of English literature, a multilingual model might link the word dove with peace, but the Basque word for dove (‘uso') is used as a slur. Many researchers have encountered the limitations of these LLMs, for other languages like Spanish or Japanese. ChatGPT struggles to mix languages fluently in the same utterance, such as English and Tamil, despite claims of 'superhuman' performance. The death of Intellectual Property RightsIntellectual property rights are a common concern. Already, generative AIs can produce exact copies (tone and tenor) of creative works by certain authors (for example, J K Rowling's Harry Potter series). This is also true of works of art. Two things are happening in the background: any copyright inherent in these works has been lost, and creators will cease to create original works for lack of incentives (at least according to current intellectual property theory). A recent Japanese decision to ignore copyrights in datasets used for AI training (from the blog technomancers.ai, “Japan Goes All In: Copyright Doesn't Apply to AI Training”) is surprisingly bold for that nation, which moves cautiously by consensus. The new Japanese law allows AI to use any data “regardless of whether it is for non-profit or commercial purposes, whether it is an act other than reproduction, or whether it is content obtained from illegal sites or otherwise.” Other governments will probably follow suit. This is a land-grab or a gold rush: India cannot afford to sit on the sidelines.India has dithered on a strict Data Protection Bill, which would mandate Indian data to be held locally; indirectly, it would stem the cavalier capture and use of Indian copyright. The Implications are chilling; in the absence of economic incentives, nobody will bother to create new works of fiction, poetry, non-fiction, music, film, or art. New fiction and art produced by generative AI will be Big Brother-like. All that we would be left with as a civilisation will be increasingly perfect copies of extant works: Perfect but soulless. The end of creativity may mean the end of human civilisation.With AIs doing ‘creation', will people even bother? Maybe individual acts of creation, but then they still need the distribution channels so that they reach the public. In the past in India, kings or temples supported creative geniuses while they laboured over their manuscripts, and perhaps this will be the solution: State sponsorship for creators.Indian Large Language Models: too few yet, while others are moving aheadDiverse datasets will reduce bias and ensure equitable Indic representation to address the concerns about generative AI. Another way is to use more rigorous training methods to reduce the risk of data poisoning and AI hallucinations.Progressive policy formulations, without hampering technological developments, are needed for safe and responsible use to govern the use of LLM's across disciplines, while addressing issues of copyright infringement and epistemological biases. Of course, there is the question of creating ‘guardrails': some experts call for a moratorium, or strict controls, on the growth of generative AI systems. We must be alive to its geopolitical connotations, as well. The Chinese approach to comprehensive data-collection is what cardiologists refer to as a ‘coronary steal phenomenon': one segment of an already well-perfused heart ‘steals' from another segment to its detriment. The Chinese, for lack of better word, plunder (and leech) data while actively denying market access to foreign companies. Google attempted to stay on in China with Project Dragonfly, while Amazon, Meta, Twitter were forced to exit the market. Meanwhile, ByteDance, owner of TikTok, is trying to obscure its CCP ties by moving to a 'neutral jurisdiction' in Singapore, while siphoning off huge amounts of user data from Europe and the US (and wherever else it operates) for behavioural targeting and capturing personal level data, including from children and young adults. The societal implications of the mental health 'epidemic' (depression, low self-esteem, and suicide) remain profound and seem like a reversal of the Opium Wars the West had unleashed on China. India can avoid Chinese exclusivism by keeping open access to data flows while insisting on data localisation. The Chinese have upped the ante. Reuters reported that “Chinese organisations have launched 79 AI large language models since 2020”, citing a report from their Ministry of Science and Technology. Many universities, especially in Southeast Asia, are creating new data sets to address the spoken dialects. West Asia, possibly realizing the limitations of “peak-oil”, have thrown their hat in the ring. The United Arab Emirates (UAE) claims to have created the world's “first capable, commercially viable open-source general-purpose LLM, which beats all Big Tech LLMs”. According to the UAE's Technology Innovation Institute, the Falcon 40B is not only royalty free, but also outperforms “Meta's LLaMA and Stability AI's StableLM”. This suggests that different countries recognise the importance of investing resources to create software platforms and ecosystems for technological dominance. This is a matter of national security and industrial policy.“We have no moat” changes everything: welcome to tiny LLMsChiranjivi from IIT Bombay, IndiaBERT from IIT Delhi and Tarang from IIT Madras are a few LLMs from India. India needs to get its act together to bring out many more LLMs: these can focus on, and be trained on, specialised datasets representing specific domains, for instance, that can avoid data poisoning. The Ministries concerned should provide support, guidance, and funding. The obstacle has been the immense hardware and training requirements: GPT-3, the earlier generation LLM, required 16,384 Nvidia chips at a cost of over $100 million. Furthermore, it took almost a year to train the model with 500 billion words, at a cost of hundreds of millions of dollars. There was a natural assumption: the larger the data set, the better the result with ‘emergent' intelligence. This sheer scale of investments was considered beyond Indian purview. A remarkable breakthrough was revealed in a leaked internal Google memo, timed with Bard's release, titled "We have no moat, and neither does OpenAI," a veritable bombshell. It spoke about Meta's open sourcing its algorithmic platform, LLaMA, and implications for generative AI research. Although there is no expert consensus, the evidence suggests smaller datasets can produce results almost as good as the large datasets.This caused a flutter among the cognoscenti. Despite Meta releasing its crown jewels for a wider audience (developers), there was an uptick in its stock value, despite failures in its multiple pivots beyond social media. To understand this better, Geoffrey Hinton, the ‘godfather' of deep learning, explains in detail: All large language model (LLM) copies can learn separately, but share their knowledge instantly. That's how chatbots know more than an average person. The performance trajectory of different LLM's has skyrocketed; for example, consider this: Using LLaMa as a base, researchers were able to quickly (in weeks) and cheaply (a few $100) produce Alpaca and Vicuna that, despite having fewer parameters, compete well with Google's and openAI's models. The graph shows that the answers from their chatbots are comparable in quality (per GPT-4). A fine-tuning technique called LoRA (Low Rank Adoption) is the secret behind this advance.This abruptly levels the playing field. Open-source models can be quickly scaled and run on even laptops and phones! Hardware is no longer a constraint. Let a thousand Indian LLMs bloom! The way forwardGiven the astonishing amounts being invested by venture capitalists and governments in generative AI, there will be an explosion in startup activity. There are already a few in India, such as Gan, Kroopai, Peppertype.ai, Rephrase.ai, TrueFoundry, and Cube. Still, TechCrunch quoted Sanford Bernstein analysts who painted a gloomy picture: “While there are over 1500 AI-based startups in India with over $4 billion of funding, India is still losing the AI innovation battle”. Without exaggeration, it can be argued that this is an existential threat for India, and needs to be addressed on a war-footing. The AIforBharat initiative at IIT Madras is a start, but much more is needed. A sharply focused set of policies and regulations needs to be implemented by the government immediately that will both prevent the plunder of our intellectual property and data, and also encourage the creation of large numbers of models that make good use of Indian ingenuity and Indic knowledge.2245 words, 4 June 2023 This is a public episode. Independently, using either Aloe Vera gel or egg whites is good for a natural conditioner and moisturiser effect but using both of them together has no existing precedence showing they work. He also advised Jhansi to find out the cause of her hair fall. “You need to consult a dermatologist to evaluate the causes of your hair fall. If it is due to any nutritional deficiency, lack of blood or genetic cause needs to be evaluated before you can take any remedial steps for controlling your hair fall,” said Dr John Watts.

Sadhguru gives a Yogic perspective on health and explains how Yogic practices are focused on bringing balance to the energy body, which naturally brings physical and mental health. When it comes to a chronic ailment, whatever may be the ailment, the root cause for it is always in the energy body. Your energy body functions the way it functions for various reasons. It may be because of the kinds of atmospheres you live in, the kind of food that you eat, the kinds of relationships that you hold or because of your emotions, attitudes, thoughts and opinions. It may also be because of certain external energy situations that disturb your internal energy. In some way, your energy body is disturbed which will naturally manifest both physiologically and psychologically. Once this one layer of the body—the energy body—is disturbed, the mental body and the physical body are bound to be disturbed. Actually it becomes a doctor's concern only after it becomes a medical problem. Till then it is not a doctor's concern because there is no physiological manifestation. Unfortunately, medical science understands only disease. It does not understand the roots of health, as to where it comes from and what the basis of health is. If you are a diabetic, sugar is not the problem with you. It is just that your pancreas is not functioning properly. As an emergency measure you reduce the sugar intake because in the Allopathic system of medicine, they have no other way to activate your pancreas, so this is the only measure they have taught you—“check your sugar every day and take a little more insulin”. The Allopathic system is symptomatic in treatment—the doctor looks at your symptoms and gives you the treatment. Allopathic system is a fantastic system for any infections. Allopathy is the best thing as I see for anything that comes to your body from outside. But it does not offer any solution for the simple ailments which man is creating for himself from within, like diabetes, blood pressure, or migraine headaches. Modern medical science only offers to manage the diseases and never really talks about freeing you from those diseases. There are whole systems of medicine and so many kinds of specialists to manage diseases within certain limitations. A lot of money and time is being spent on just managing those diseases. It is just like people talking about stress management. People want to manage their stress, people want to manage their diabetes, people want to manage their blood pressure. It is ridiculous. This kind of foolishness has entered, simply because they have not understood the fundamentals of how their own life energies function. In yoga, if there is diabetes, we see it as a very fundamental disturbance—diabetes is not taken lightly as a disease. We see that the fundamental structure of the basic body is being disturbed. That is why diabetes happens. And it is different in each individual. For each person, the level of disturbance and the type of disturbance that happens in each system is very different. So it needs to be handled individually. However, irrespective of what the disease is, yoga only aims at balancing and energising or activating the energy body. The basic premise in yoga is that if your energy body is in full flow and properly balanced, there can be no chronic disease either in your physical body or your mental body. Diabetes will go because we are not treating the disease as such, we are seeing disease only as a manifestation of the disturbance that could be existing in the energy body. So if people are willing to do a certain amount of sadhana to balance and activate their energy body, they can definitely be free of all chronic ailments. While excellent newsletters on specific themes within public policy already exist, this thought letter is about frameworks, mental models, and key ideas that will hopefully help you think about any public policy problem in imaginative ways.Audio narration by Ad-Auris. India Policy Watch: The Demarcation Between Science And Ramdev Insights on burning policy issues in India- RSJEvent 1: May 23, 2021. Yoga guru Ramdev:"Everyone should progress through self-evaluation. Some allopathic doctors too consider Indian medical science, Ayurveda, yoga as pseudo-science, (emphasis ours) which hurts crores of people.” Event 2: May 29, 1919. New York Times (reporting on the event on Nov 7, 1919):I will connect the two events (hopefully) in a bit.Take Event 1. This was Ramdev in his response to the letter he received from Dr Harsh Vardhan, the Union Minister for Health. The minister had written to Ramdev about his recent statements about allopathy and doctors. You can read more about it here. Dr. Vardhan was forced to write to Ramdev because of the campaign by the IMA (Indian Medical Association) against the damaging statements made Ramdev about modern medicine. Over the past year, Patanjali Ayurved, a company promoted by Ramdev, has made several claims of curing Covid-19 in seven days through its product, Coronil. He has also, at different times, called Allopathy a ‘stupid science’, falsely claimed over 10,000 allopathic doctors died after taking both doses of vaccines and blamed the global drug ‘mafia’ for conspiring against him, Ayurveda and India in that order. Now you might dismiss the whole thing as a sideshow of which there are plenty at anytime in India. But this isn’t one. Ramdev is a hugely influential figure who has a daily show on the national TV channel watched by millions. He singlehandedly created a packaged goods behemoth in the last five years riding on a deeply held belief among Indians about the ‘purity’ of Ayurveda. There have been many questions on those claims. But that’s another story. Anyway, there are a few real issues to contend with here. First, we are in the middle of a pandemic and our fastest way out of it is to vaccinate people at lightening speed. Any statement or action that stops us from doing this is bad and must be stopped. Second, India has made real progress on key health parameters like infant and maternal mortality rates, malnutrition, life expectancy and disease burden in the past two decades. But the harsh reality is it is still placed in the bottom quartile among nations on these metrics. We have had some hard fought wins in making modern medical treatments acceptable among people. We shouldn’t be frittering away these gains by raising suspicions about it among the masses. Lastly, the frontline workers and doctors have put their lives on line during the pandemic to serve the patients. We were showering flower petals on them a year back. Since everything is political now in India and nationalism can be used to defend the indefensible, the troll armies have turned this episode into a dystopian farce where pharma ‘mafia’ and Christian lobbies are the villains and Ramdev is the one exposing their nefarious plans. Now any doctor speaking against Ramdev risks being labelled a pawn hurting India’s interests and its inevitable rise that these lobbies are hard at work to stop. That we want to alienate doctors, hospitals and pharma companies in middle of a pandemic is beyond my comprehension. This is a self goal like no other. The Demarcation ProblemAnyway, the point about pseudoscience made by Ramdev is of interest to me. Who defines it? If crores of people follow something, does that mean it shouldn’t be called pseudoscience? Palmistry, astrology, homoeopathy, Ayurveda - where do they stand on the spectrum of science and pseudoscience? Since some kind of revivalism is on within the Indian society, it will be useful to explore this further. That brings me to Event 2. Why was the eclipse on May 29, 1919 such an event? Here’s why:A hundred years ago, Albert Einstein wasn’t a household name. He was a professor in Berlin, known to scientists, intellectuals, his divorced wife and the first cousin who would soon become his second wife — but not to the world.His rise to superstardom began on May 29, 1919, when the moon and sun lined up just right for a solar eclipse. Photos of the astronomical event showed something strange: A few of the stars visible during the blackout were in the wrong place.Einstein had foreseen this. Using his theory of general relativity, he made the seemingly crazy bet that the stars’ positions in the sky would shift during an eclipse, and even calculated by how much.As the data came in and the results were confirmed, the general theory of relativity was proven. Newtonian physics was no longer the truth. “Revolution in Science,” the front page of The Times of London proclaimed. “New Theory of the Universe: Newtonian Ideas Overthrown.” The New York Times followed suit with “Men of Science More or Less Agog.” Following this with interest was a precocious 17 year old student in the University of Vienna. His name. Karl Popper. This event would leave a deep impact on him as he thought about the nature of truth and the philosophy of science in his later life. The idea that Einstein could precisely postulate in advance what would happen during a solar eclipse and then have the courage for it to be proven or be ‘falsified’ publicly was in sharp contrast to other ‘sciences’ that were in fashion during those years in Europe, namely, Marxism and Psychoanalysis. For over two centuries, scientists had empirically tested Newtonian laws and it worked in all known cases. New inventions came up based on these laws including the steam engine and the power loom that revolutionised societies. Kepler showed how the laws worked for planets and other celestial objects. Haley predicted a comet would reappear again in 76 years based on it. The scientific method that Bacon had proposed involving observation, hypothesis, test and conclusion was proven over and over again for Newtonian physics. That’s how the universe worked. Yet, when Einstein argued that they didn’t work for the special case of really large objects and it was proven during the eclipse of 1919, the entire community of physicists updated their priors. For Popper this was the ‘demarcation’ between what he called science and non-science (later termed pseudoscience by others ). To him all observation is selective and can be used to prove anything. Psychoanalysis was a prime example. Therefore, he dismissed inductive reasoning as the method of drawing scientific inference. Because every single observation so far has followed Newtonian law, doesn’t mean it is the truth. Because the Sun rises every morning doesn’t necessarily prove it will rise tomorrow. More has to be done.Popper instead offered ‘falsifiability’ as the test to ‘demarcate’ science and non-science. As he put it, you can always cherry-pick evidence to prove any theory. Like psychoanalysts and Marxists of his time were doing or what Ramdev is doing with his claims. That’s not enough. For him what really counted as science was if you could stake your theory on a future prediction that could turn out to be false like Einstein and the physicists did with the solar eclipse. A million instances of something working isn’t enough to prove something is science but a single counter-instance is enough to falsify the claim that it is science. That’s how the claim should be tested. On falsifiability. So, science is always about a provisional truth till someone falsifies it. The moment it is falsified, it is no longer science. Science, to Popper, therefore was a grand pursuit to solve big problems and not about making series of tiny empirical observations and figuring out the cause behind them. The observations were only to be made to serve the grand pursuit of truth.Applying FalsificationNow there were others who debated with Popper on the philosophy of science, most notably, Kuhn who coined the term ‘paradigm shift’ to explain how science evolves and those debates are best left for another day. For now, let’s consider ‘falsifiability’ as explained by Popper and view modern medicine as it is practised today. The entire drug discovery and development process involving identification of likely compounds useful in curing a disease, synthesis, characterisation, validation, optimisation and assays that will make it ready for clinical trials follows the principle of finding a single counter instance that would falsify the results. The clinical trials, multiple reviews by the regulators and then the monitoring of the drug performance after it has been launched in the market are all meant to ensure that any instance of failure is captured and studied to eliminate the root cause. This is a rigorous process to stand the test of ‘falsifiability’ and keep modern medicine as close to true science as possible. I will leave it to you to assess other ‘sciences’ I have mentioned earlier on this scale. For me none pass the muster.To be clear, this isn’t to overstate the primacy of science in our lives. You might find comfort and peace of mind through astrology. Yoga and Ayurveda might help you to stay fit and build your immunity. You might have personal experience of homoeopathy working for you. Some scientist who retired from ISRO (or IISc) or some former Nobel prize winner might believe in past life regression or Vedic chants to cure something. These could all be observable truth for those instances. It won’t make them science. Because they can easily be falsified. And since we are at it, let’s not fall to the usual exaggerated claims that do the rounds on Whatsapp. Yes, China is big on their traditional medicines. But if you dig deeper, most of the traditional medicines sold are for the usual reasons: to make you “strong” (wink), for skin ailments and for common colds and fever. Nobody is claiming it will cure Covid-19. The other argument about modern medicine deriving their compounds from nature like Ayurveda also needs to be understood better. It is true many modern drugs have compounds that are extracted from plants and herbs that we often use in Ayurveda. But the modern medicine process is quite exact about the compound, the amount and how it should be delivered into our system. Eating the same plant or herb as a paste or in food isn’t the same thing though it might occasionally yield the same results. There is a difference. The same as that between Sanjeevani and the Dronagiri mountain. Between specifics and generalisation. And no, searching for Sanjeevani isn’t exactly a scientific pursuit. One of the duties of citizens mentioned as part of the Directive Principles of State Policy in our Constitution is “to develop the scientific temper, humanism and the spirit of inquiry and reform”. This is good goal for every Indian to pursue. You don’t need to jettison any tradition for it. The two can co-exist. Global Policy Watch #1: When Conspiracy Theory Comes True Bringing an Indian perspective to burning global issues- RSJThere was this amazing piece in the Newsweek about how a global ragtag band of gumshoes, data scientists, molecular biologists and conspiracy enthusiasts exposed the many false claims of the Wuhan Institute of Virology about the origins of Covid-19. The mountain of evidence collected and meticulously analysed by the group, who now go by the appropriate sounding name DRASTIC (Decentralized Radical Autonomous Search Team Investigating COVID-19), has been enough for the Biden administration to launch an investigation into the source of the virus. The lab origin of the virus was a theory that few believed in even a month ago. But now it is almost mainstream. Conspiracy theories are often used as a label to delegitimise notions and those believing in it are seen as misfits. Calling something a conspiracy theory is to lower its credibility. They have been the bane of many elections, triggered unrest and have kept social media platforms on their toes. But the very public victory of DRASTIC will have an impact on this discourse. It won’t be easy to wave something off as a conspiracy theory. Pranay writes more about this in the next section below. Interestingly (and fittingly, perhaps for this edition), the first philosopher to write about conspiracy theory was Karl Popper. In his book The Open Society and Its Enemies, Popper moves away from science in the realm of nature to the state and the society. He describes ‘conspiracy theory of society’ as an approach to explain a social phenomenon by finding out those who have planned or conspired for it. But Popper argues most conspiracies that are hatched don’t go as per plan and end up with unintended consequences. The role of social sciences, therefore, is to explain these unintended consequences arising out intentional human actions. That sounds like what we strive to do in this newsletter. There will be lot more written as the origin of the virus story progresses. This might be the most consequential conspiracy theory of all time. Global Policy Watch #2: Networks vs Hierarchies Bringing an Indian perspective to burning global issues— Pranay KotasthaneThis week had its fair share of mind-boggling stuff. To name a few: a court case by a Bollywood celebrity against 5G deployment, a well-known lawyer promoting vaccine hesitancy, and reports documenting how a group of passionate folk forced renowned scientists to eat their words on the virus lab-leak theory.There’s something common to all these events. To borrow a phrase from media analyst Martin Gurri, such instances illustrate ‘the crisis of authority’ in the Information Age. There’s no theory that won’t be questioned by radically connected individuals, irrespective of the evidence backing it. This newsletter has discussed Radically Networked Societies (RNS) many times over in disparate contexts. We have discussed how no issue is local anymore. Even highly-specialised issues are vigorously contested by laymen. Even decisions by authoritative organisations are questioned, debated, and sometimes overturned, in a vastly expanded information sphere. The thesis of Martin Gurri’s 2014 book The Revolt of the Public is along similar lines. He frames the current upheaval as an asymmetrical warfare between hierarchical institutions of the Industrial Age and hyper-networked individuals of the Information Age. In his words:“When judging his government, Homo informaticus can then do so in light of alternative possibilities—different views of the same policy or event, different values invoked for an action or inaction, different performance by other governments, real or imagined. The first step toward skepticism is doubt, and Homo informaticus, exposed to an independent channel, must confront choices and doubts when constructing his story of the world.…Governments of every stripe have had trouble grasping the sudden reversal in the information balance of power. Proud in hierarchy and accreditation, but deprived of feedback channels, the regime is literally blind to much global content. It behaves as if nothing has changed except for attempts by alien ideals—pornography, irreligion, Americanization—to seduce the public. Most significantly, the regime in its blindness fails to adjust its story of legitimacy to make it plausible in a crowded, fiercely competitive environment.…The consequences are predictable and irreversible. The regime accumulates pain points: police brutality, economic mismanagement, foreign policy failures, botched responses to disasters. These problems can no longer be concealed or explained away. Instead, they are seized on by the newly-empowered public, and placed front-and-center in open discussions. In essence, government failure now sets the agenda. As the regime’s story of legitimacy becomes less and less persuasive, Homo informaticus adjusts his story of the world in opposition to that of the regime. He joins the ranks of similarly disaffected members of the public, who are hostile to the status quo, eager to pick fights with authority, and seek the means to broadcast their opinions and turn the tables on their rulers. The means of communication are of course provided by the information sphere.”Gurri, Martin. The Revolt of The Public and the Crisis of Authority in the New Millennium (pp. 89-90). Stripe Press. Kindle Edition.There’s another way to model this crisis of authority. In edition #11, I had discussed the Overton Window. This framework suggests that for any political issue, there's a range of socially acceptable positions that's narrower than the range of all possible positions. These socially acceptable ideas are seen as being inside the Overton Window — they are mainstream and uncontroversial. On the other hand, policy positions outside it are viewed as shocking, upsetting, and electorally harmful. The key insight of this framework is that, with social pressure, the Overton Window can shift over time; today's radicals may become tomorrow's moderates. In the Information Age, something even more striking has happened. The Overton Window on practically every issue has been stretched such that nearly all possible positions on an issue have become socially acceptable. With that happening, the older institutions, which earlier exuded authority, are shredding legitimacy with every decision they make.This is a value-agnostic assessment of the networked public. In the case of Wuhan lab-leak theory, the effect is a positive one as it has at least thrown light on the geopolitical power play that has suppressed this line of inquiry. The curious case of growing QAnon popularity in the US is the other side of the same coin. But if I were forced to make a ‘bane or boon’ type call on our networked existence in the political sphere, it would be as follows. The backlash of the public is a boon in that it pushes governments towards more accountability. It puts the fear in a government that any wrongdoing will be found out, exposed, and mocked. The bane is that with the multiplication of identity-inspired comradeships across the spectrum, forming coalitions of any kind has become difficult. Every sect is splintering and trust between the splinters is a casualty. The cacophony of conflict spreads to every issue, regardless of how important and urgent it is. Governments become more opaque, more indecisive, and less persuasive in turn. One thing is for sure: our industrial-era political setup is undergoing a fundamental transformation in the Information Age. Yet on the other hand, a prominent physician clothing company, FIGS, is accused of publishing disparaging advertisements towards DOs and female physicians – which caused quite an uproar on social media. Add to this that medical television shows, like Grey’s Anatomy, always refer to doctors with an “MD” and not a “DO.” So why does there seem to be this tug of war between the two? What is the difference between a DO and an MD? Is one practice better? We get to the bottom of this with the president of American Association of Colleges of Osteopathic Medicine (AACOM), the organization that supports the 37 accredited colleges of osteopathic medicine in the United States. Dr. Sahil Mehta chats with Dr. Robert Cain, a pulmonology specialist. He was also the former dean of clinical education at Ohio University College of Osteopathic Medicine. Currently, he serves as the president and CEO of the American Association of Colleges of Osteopathic Medicine (AACOM). Also joining in today’s episode is Ragav Sharma, a 4th year osteopathic student and MedSchoolCoach. He hosts The Preventive Medicine Podcast. Differences Between Osteopathic and Allopathic Medicine There is no practical difference between osteopathic doctors (DOs) and allopathic doctors (MDs) they are both licensed to practice medicine in the United States. MD and DO students take up the same subjects, with the exception of biomechanics, which is emphasized more in DO education. In addition, osteopathic medicine emphasizes training students to become physician-servants, and orientation that influences the practice of medicine. Medical students should consider what type of physician they’d like to be before deciding on an MD or DO degree. Dr. Cain likes to think of MD and DO doctors as complementary to one another. With the US’s complex healthcare system, cooperation between MD and DO doctors is crucial to improving the lives of more patients. When it comes down to it, both types of doctors are working towards the same goal of progressing healthcare. Why Osteopathy? Osteopathy was introduced to Ragav, a fourth year med student, by his father, who knew that his son had an interest in muscoloskeletal medicine. Ragav proceeded to apply to both DO and MD medical schools. Eventually, he chose to go to Midwestern University Chicago to pursue Osteopathic Medicine. He believes the holistic philosophy and the manual medicine training from a DO education will supplement his skillset as a future practitioner of musculoskeletal medicine. Application of DO’s Biomechanic Training To this day, Dr. Cain still applies his DO training in the field of pulmonology. Not many people may know this, but the respiratory system is biomechanical. He cites exacerbated chronic obstructive pulmonary disease (COPD) as an example. Patients with this condition have an increased work of breathing. As a DO physician, Dr. Cain prescribes medicine to relieve symptoms but he also checks for changes in the chest wall, since removing any obstructions will decrease the likelihood of respiratory failure. Ragav also mentions an instance where he was able to apply of his osteopathy knowledge. He encountered a stroke patient who could not breathe well during his in-patient rehab rotation. He suspected that the patient’s lung muscles have not fully recovered from the stroke, which made breathing more difficult. As a result of his suggestion, treatment changes were made to include more respiratory therapy and to shift bed positioning. Ragav’s osteopathic training led him to make a suggestion which ultimately made a difference in the patient’s outcome. Advice to DO Students Applying for Residency Ragav’s advice to fellow students is to focus on things you can control such as board scores, clinical rotation performance, and capacity for learning. As long as you put in your best efforts you are setting yourself up to succeed. Keep working hard towards the chosen field you have in mind. It’s also helpful to be aware of specializations that have a preference for DO over MD students and vice versa. Study Tips for the USMLE and COMLEX The content in both licensure exams are similar, but the questions are posed differently. To save on time, you may study for the USMLE and the COMLEX with the same resources. The main difference between the two is that the COMLEX also covers osteopathic medicine. DO students should use osteopathy’s holistic philosophy as a lens when answering questions on the test. For additional preparation, look for reviewer questions that mimic the COMLEX’s questions. Future of Osteopathic Medicine With the merger of the MD and DO education programs, AACOM hopes to ease the matching system for students and their chosen residencies, and simplify the licensure exams for DO students. It is also the AACOM’s goal to eliminate the need for DO students take the USMLE because a specialization requires them to do so. Another challenge that DO students face are the restrictive regulations for shadowing experiences. The AACOM recognizes this, and is already looking for alternative ways to increase exposure to osteopathy. Recent developments and events have increased the visibility of osteopathic medicine more than ever. This is an opportunity to solidify osteopathy in US healthcare system. Dr. Cain hopes to see increased interest and acceptance of osteopathic medicine through ongoing education and awareness efforts of the AACOM.
