In today's episode, Gemma shares her experience and impressions of working with clients who have had the unfortunate experience of being 'diagnosed' with "treatment-resistant depression".  Gemma believes this is a dangerous and disempowering label which does nothing to help people get to the bottom of what's driving their depression.   Gemma has been doing therapy with people in a clinical practice setting since 2001. In 2005, she finished  an eleven-year association with a tertiary referral clinic for mood disorders, where she worked in various positions in a clinical research capacity.   During those eleven years, Gemma came in contact with hundreds of people referred for depression. Many of those patients had some type of mood disorder, like major depressive or bipolar disorder, and they were often referred to as treatment-resistant. In this episode, she shares her thoughts centering around people coming to therapy when they have been spending years struggling with a mood disorder like chronic depression or recurrent bouts of a major depressive episodes. She talks mainly about the psychological nature/risk factors and the psycho-social aspects of depression. NOTE: This podcast was recorded toward the beginning of lockdown for COVID (important for the beginning of the podcast!)Katherine Knapke was raised in a small, Mennonite community in Columbus, Ohio.  Taught to be peaceful, quiet and not ask for anything you want, Katherine's upbringing stoked the fire for chronic depression, which ultimately led to a devastating struggle with multiple eating disorders, a panic disorder and crippling anxiety.  After escaping an abusive relationship in college, she relapsed into her eating disorder.  Her story of finding herself in the midst of her recovery and surrounding herself with healthy people who support her recovery is beautiful and inspiring, and reminds us that all things are possible.Katherine is now the Chief Operating Officer at the American Negotiation Institute.  She also hosts the Ask with Confidence podcast, which is designed to empower women by helping them develop the skills and confidence they need to better navigate difficult conversations and achieve the highest level of success. Lorraine Pestell was born in London and has lived and worked as an IT professional all over the world, including the UK, several European capitals, the USA and Singapore, before moving to Melbourne, Australia, for the turn of the millennium.  Having suffered from chronic clinical depression since childhood, and later diagnosed with Post-Traumatic Stress Disorder after a violent marriage and other challenging events, she turned to creative writing as therapy and to escape from the world. A passionate advocate for mental health awareness and rational suicide, Lorraine has two main goals for her writing:  first, to inspire fellow sufferers to rise above their symptoms and make a success of their lives; and second, to encourage non-sufferers to tolerate, support and even love those afflicted with mental illness in their quest to live a "normal" life. The six-part contemporary fiction serial, "A Life Singular", is truly Lorraine's life's work, starting out as an adolescent romance in her early teens.  It has since become six long books exploring the human mind and social justice, wrapped in the universal theme of love and our endless fascination for celebrity.  Parts One to Four are available now, with Part Five due for release in March 2016.

Theory of Mind is a mental process which can be critical to the understanding of the maintenance of psychic disorders and can have implications for their treatment. The idea of a conceptual organisation of the Theory of Mind is supported by current research from Developmental Psychology (see Perner, Mauer & Hildenbrand, 2011). We could show that Theory of Mind seems to develop with the understanding of identity and is based on a common conceptual basis: the understanding of the relationship between reference and representation. Theory of Mind means the ability to ascribe and reflect own mental states and mental states of others. The concept "Theory of Mind" is used very heterogeneousl and is defined very broadly in the research with adults. The aim of this work is to describe the theoretical and conceptual basis of the Theory of Mind and to show its importance for the Posttraumatic Stress Disorder and Chronic Depression with regards to aetiology concept as well as for its implications in the specific therapy methods [Dialectic Trauma Therapy of (Butollo and Karl, 2012) and the Cognitive Behavioral Analysis System for Psychotherapy (McCullough, 2000, 2006)]. I will take into account the attachment theory, the ability of perspective-taking and the integration of paradoxical information as well as the meaning of trauma as a determining factor in the organisation of the Theory of Mind. The differentiation between reference and representation forms the basis of the ability of perspective-taking as well as understanding identity. This work shows related concepts as well as established assumptions about the organisation of the Theory of Mind and Mentalizing and introduces a meta-representational concept to distinguish between reference and representation. The environment is seen as an external source of supply to the formation of mental representation. This work intends a systematic processing of the conceptual fundament of the Theory of Mind as a basis for the use in the clinical-therapeutic area.

Background: Despite limited effectiveness of short-term psychotherapy for chronic depression, there is a lack of trials of long-term psychotherapy. Our study is the first to determine the effectiveness of controlled long-term psychodynamic and cognitive-behavioral (CBT) treatments and to assess the effects of preferential vs. randomized assessment. Methods/design: Patients are assigned to treatment according to their preference or randomized (if they have no clear preference). Up to 80 sessions of psychodynamic or psychoanalytically oriented treatments (PAT) or up to 60 sessions of CBT are offered during the first year in the study. After the first year, PAT can be continued according to the `naturalistic' usual method of treating such patients within the system of German health care (normally from 240 up to 300 sessions over two to three years). CBT therapists may extend their treatment up to 80 sessions, but focus mainly maintenance and relapse prevention. We plan to recruit a total of 240 patients (60 per arm). A total of 11 assessments are conducted throughout treatment and up to three years after initiation of treatment. The primary outcome measures are the Quick Inventory of Depressive Symptoms (QIDS, independent clinician rating) and the Beck Depression Inventory (BDI) after the first year. Discussion: We combine a naturalistic approach with randomized controlled trials(RCTs)to investigate how effectively chronic depression can be treated on an outpatient basis by the two forms of treatment reimbursed in the German healthcare system and we will determine the effects of treatment preference vs. randomization.
