    "It was pretty apparent to me that something was going on with him," says Kristi Levine, describing the realization that, based on her experience as a Montessori teacher, her infant son, Trey, was missing developmental milestones. Unfortunately, Kristi's hunch turned out to be correct and Trey was later diagnosed with a rare genetic mutation called CACNA1A which is impacting his motor skills, balance, coordination and speech. Kristi and her husband, Eric, join host Michael Carrese on this installment in our Year of the Zebraseries to help us understand the disorder and its implications for Trey and their family, which includes Trey's older sister Stella.  “There's a lot of guilt involved in being a parent of a child who has a disability because you never feel like you're doing enough,” shares Eric, even though they both work full time and have becoming experts at juggling work, caregiving, advocating, and volunteering with the CACNA1A Foundation. In this candid interview, Eric and Kristi discuss the challenges of parenting a child with complex medical needs, the importance of community support, the ongoing search for treatment options, and share some advice for clinicians caring for patients and families living with rare disorders. “We just want medical professionals to respect and understand what we're dealing with on a day-to-day basis and to see our kids holistically, and not just try to fix the problem medically. The chapter on hospice care distinguishes it from assisted living through the author's experiences and misconceptions, then moves to a broader discussion of Medicare spending, and finally a meditation on dying of old age. The author strikes an effective balance between the personal, political, and cultural aspects of aging. Karofsky dedicates the last chapter of the book to a discussion of recent failures to protect long term care patients during the COVID19 pandemic.

    This episode of The Future Built Smarter explores the evolving landscape of healthcare design and engineering with guest Eric Vandenbroucke, Vice President of Healthcare at IMEG. “2025 started with a lot of optimism, but it's definitely been a roller coaster,” he says. Despite uncertainty in the market amid potential federal reductions in Medicare reimbursements, Eric says many healthcare clients remain focused on sustainability initiatives. “Healthcare contributes about 5 percent of global emissions,” he says. “Owners are committed to reducing their carbon.” Resiliency and future proofing are also critical. From extreme weather to human threats, resilience demands a layered approach, including redundant power, onsite water storage, and cybersecurity. Vulnerability assessments set the stage for creating future-proof plans. “The biggest thing we can help clients with is understanding the state of their building or campus and what its vulnerabilities are,” Eric says. “Healthcare facilities need to be operational—period.” Security is a top concern as well, especially in the wake of increasing threats like active shooter incidents. “We take a holistic approach,” says Eric, stressing that security requires much more than just cameras and guards. Key additional components of an all-encompassing security plan include employing the principles of Crime Prevention Through Environmental Design (CPTED) and personnel training, including tabletop exercises. "No one wants to deal with these events, but being prepared makes a huge difference," Eric says. Technology, of course, is the main driver of innovation for healthcare facility operations, clinical operations, and delivery of care—all of which need to communicate with each other and be interoperable. “We've moved beyond siloed systems to a smart system approach,” says Eric. “Alarms, patient monitoring, building automation systems, command centers, medical records, real-time location solutions, robotics—all of those things and more have to work together.” As for artificial intelligence (AI), Eric says, “Where we are at right now in the industry is just the tip of the iceberg. Our clients are now using AI for clinical documentation, predictive analysis, optimizing their revenue cycle, using chatbots for scheduling, triage, and patient education. On the AEC side, AI is going to be a big tool for design assist. At IMEG we've developed our own AI system that helps us increase our design efficiency and accuracy. We've also developed many tools within Revit and Power BI to help with that efficiency and accuracy, and with coordination. And we continue to grow and expand the use of our virtual reality and augmented reality tools. It's all very exciting. “Technology and trends are going to continue to influence healthcare care moving forward. That's one of the cool things about healthcare: It's never the same. It's always evolving.”

Brian Surber, a seasoned attorney and former special agent, is our guest. The courthouse wasn't off-limits for the predators he's faced. Surber, who brings experience from both the prosecution and law enforcement sides of the justice system, shares gripping firsthand accounts of tracking, arresting, and prosecuting some of the most dangerous criminals in America, including a serial attacker whose crimes spanned states and whose victims were often unaware they were being watched, followed, and targeted. Over the course of his career, he's helped dismantle violent drug gangs, secure convictions for high-profile offenders, and navigate complex cases where digital footprints, DNA evidence, and survivor testimony intertwine. Arresting A Serial Criminal That Attacked Women. “One of the most horrifying cases involved a serial rapist who used surveillance of victims,” Surber explains. “He would attack them and systematically erase any sense of control they had left. Statistics support the pattern. According to multiple studies, serial rapists often go undetected for years due to a combination of rape kit backlogs and the tendency for early victims not to report. Research from Case Western Reserve University revealed that 60% of serial rapists had been previously arrested for other sexual assaults, a fact that underscores the importance of early intervention and vigilant policing. Surber's work highlights this point precisely: catching these offenders early is crucial not only to prevent future assaults but also to expose patterns that only emerge over time. “Many of these men had long histories of violence, often rooted in their own trauma. But that doesn't excuse the choices they made,” Surber says. “Their use of threats, drugging, stalking, and predatory behavior wasn't random, it was calculated.” Arresting A Serial Criminal That Attacked Women. The FBI's Behavioral Science Unit, created in 1974, was instrumental in identifying behavioral patterns that would later help in arresting serial predators. Surber often consulted with behavioral experts to map the escalation patterns, from juvenile fetish crimes and voyeurism to full-blown violent assaults. In one study of 41 serial rapists, researchers found over 1,200 attempted and completed rapes, yet only 200 convictions. “That's a terrifying statistic,” Surber adds, “and it proves how much work we still have to do.” Available for free on their website and streaming on Apple Podcasts, Spotify, and other podcast platforms. The conversation takes a sharp turn when Surber critiques the growing push for criminal justice reform, a movement he argues has been clouded by emotion rather than rooted in facts. “We're seeing people who push dangerous policies under the guise of reform,” he says. “But the truth is, these reforms have in many cases made it harder to protect victims and prosecute predators.” His book, "Injustice for All: The (Familiar) Fallacies of Criminal Justice Reform," aims to expose the unintended consequences of poorly conceived reform efforts. “We're not talking about harmless offenders. We're talking about men who assault dozens of women, sometimes in public or inside institutions meant to protect them,” he writes. “And then we're asked to soften our approach? That's insanity.” Surber's experiences, and his words, are raw, honest, and deeply unsettling. But they are also necessary.     Some of the sickest Medicare Advantage patients ran into problems getting end-of-life care. Ultimately many patients switched to traditional Medicare, costing taxpayers billions, according to an investigation by the Wall Street Journal. This is one of many Medicare Advantage practices that is now under government scrutiny. Both Congress and Medicare agency head Dr. Mehmet Oz are pushing for reforms to curb tactics that can boost federal payments to private insurers. The Department of Justice is also investigating major private insurance companies UnitedHealth, Aetna, Elevance Health and Humana.     Medicare Advantage was designed to save the government money. But a Wall Street Journal investigation found that private insurers used the program to generate extra payments through questionable diagnoses. The investigation uncovered instances of potentially deadly illnesses like AIDS, where patients received no follow-up care, as well as diagnoses that were medically impossible. This happened in part when insurers sent nurse practitioners into Medicare Advantage recipients' homes. I'm Terrence Flynn, Morgan Stanley's U.S. Pharma and Biotech Analyst. The market for obesity medicines is at an inflection point, and today I'll focus on what's driving the next stage of global growth.It's Thursday, June 5th at 2pm in New York.GLP-1 medicines have been viewed by many stakeholders as one of the most transformative medications in the market today. They've exploded in popularity over the last few years and become game changers for many people who take them. These drugs have large cap biopharma companies racing to innovate. They've had ripple effects on food, fitness, and fashion. They truly are a major market force. And now we're on the cusp of a significant broadening of use of these medicines.Currently the U.S. is the largest consumer in the world of GLP-1s. But new versions of these medicines suggest that this market will extend beyond the U.S. to significantly larger numbers of patients globally. On our estimate, the Total Addressable Market or TAM for obesity medications should reach $150 billion globally by 2035, with approximately [$]80 billion from the U.S. and [$]70 billion from international markets.Now this marks a meaningful increase from our 2024 forecast of [$]105 billion and reflects a greater appreciation of opportunities outside of the U.S. We think obesity drug adoption will likely accelerate as patients and providers become more familiar with the new products and as manufacturers address hurdles in production, distribution, and access.Current adoption rates of GLP-1 treatments within the eligible obesity population are about 2 to 3 percent. This is in the U.S., and roughly 1 percent in the rest of the world. Now, when we look out further, we anticipate these figures to surge to 20 percent and 10 percent respectively, really driven by five things.First, after a period of shortages, supply constraints have improved, and the drug makers are investing aggressively to increase production. Second new data show that obesity drugs have broader clinical applications. They can be used to treat coronary heart disease, stroke, hypertension, kidney disease, or even sleep apnea. They could also potentially fight Alzheimer's disease, neuropsychiatric conditions, and even cancer.Third, we think coverage will expand as obesity drugs are approved to treat diseases beyond obesity. Public healthcare coverage through Medicare should also broaden based on these expected approvals. Fourth, some drug makers are successfully developing obesity drugs, in pill form instead of injectables. Pills are of course easier to administer and can reach global scale quickly. And finally, drug makers are also developing next gen medications with even higher efficacy, new mechanisms of action, and more convenient, less frequent dosing.All in all, we think that over the next decade, broader GLP-1 adoption will extend well beyond biopharma. We expect significant impacts on medical technology, healthcare services, and consumer sectors like food, beverages, and fashion, where changes in patient diets could reshape market dynamics.Thanks so much for listening.
