The Vuelta has turned completely on its head. Stage 8 was supposed to be a relatively straightforward 171km run from Puçol to Xeraco, but with 32km still to race, everything changed when race leader Tadej Pogačar went down hard in the middle of the peloton. Pogačar, who had held the red jersey since winning the opening time trial in Monaco, was left sitting on the road as the race doctors and UAE team staff attended to him. With deep cuts to his left shoulder and face, he eventually left the race in an ambulance with his arm in a sling, bringing his Vuelta to a sudden and dramatic end. It's Pogačar's first abandonment from a Grand Tour and his first DNF since 2023, and it completely changes the shape of the race. Enric Mas takes over as the new race leader, although the Spaniard chooses not to wear the red jersey on the podium out of respect for Pogačar. Pharmacists and Advocacy for Patients in Pain  Friday, August 21, 2026 What happens when patients living with chronic pain, their advocates, and the pharmacists responsible for dispensing opioid medications view the same healthcare system from very different perspectives? This special episode of This Week in Pharmacy brings together the Doctor Patient Forum's founders, Claudia A. Merandi and Bev C. Schechtman, with pharmacist and nationally recognized pharmacy advocate Dr. Shane Jerominski, PharmD—better known as “The Accidental Pharmacist.” The discussion follows a wave of social-media debate surrounding Claudia's use of the term “Harmacists,” which prompted responses from hundreds of pharmacists who felt the label misrepresented their professional responsibilities and the legal restrictions under which they practice. Rather than allow that controversy to deepen the divide, TWIRx created an opportunity for direct, respectful conversation. The purpose of this episode is to help pain-patient advocates better understand the limits pharmacists face when dispensing opioids—and to identify opportunities for pharmacists and patient organizations to advocate together for safe, compassionate and uninterrupted pain care. Five Legal and Supply-Chain Realities Affecting Opioid Dispensing Pharmacists have a federal “corresponding responsibility.” Under 21 CFR §1306.04, a controlled-substance prescription must be issued for a legitimate medical purpose. Pharmacists share legal responsibility with prescribers for determining whether a prescription is valid. Knowingly filling an illegitimate prescription may expose the pharmacist to federal penalties and professional discipline. Review the federal regulation. Schedule II prescriptions must satisfy strict requirements. Federal law generally requires a valid written or compliant electronic prescription before a Schedule II opioid may be dispensed. Emergency oral prescriptions are permitted only under limited conditions and require prescriber follow-up documentation. Schedule II opioid prescriptions cannot be refilled. Under federal law, every additional dispensing generally requires a new prescription. Partial fills are allowed only under specific circumstances, documentation requirements and time limits. Pharmacists cannot simply extend or refill opioid therapy when a patient or prescriber requests it. State laws add another layer of requirements. Depending on the state, pharmacists may have to verify prescriber authority, review prescription-monitoring information, validate electronic or security-form requirements, confirm identification, document partial fills, or comply with state-specific dispensing limitations. For example, California requires controlled-substance prescriptions to meet defined security and validity standards. California Board of Pharmacy guidance. Wholesaler monitoring can restrict a pharmacy's opioid supply. DEA-registered distributors must identify and report suspicious controlled-substance orders under 21 CFR §1301.74. National wholesalers therefore operate monitoring programs that may delay, limit or suspend shipments based on ordering patterns, geography, drug mix and internal thresholds. These controls are imposed upstream and can leave a pharmacy unable to obtain medication—even when the pharmacist believes a prescription is clinically and legally appropriate. Review the federal distributor requirements. These examples are not exhaustive. Pharmacists must navigate overlapping federal law, DEA requirements, state pharmacy and controlled-substance laws, prescription-monitoring systems, employer policies, payer rules, wholesaler restrictions and professional standards. Requirements also vary by state and by the circumstances surrounding an individual prescription. The Central Question How can pain-patient advocates and pharmacists move beyond blame and work together to address the policies, supply restrictions, staffing conditions and regulatory pressures that can interfere with appropriate care? This conversation explores: Why patients may interpret a refusal or delay as judgment, discrimination or abandonment Why pharmacists cannot treat a valid prescriber signature as automatic authorization to dispense How PDMP and risk-scoring information can influence clinical decisions The effect of corporate policies and wholesaler ordering controls The difference between professional judgment and stigma The communication failures that damage trust between patients, prescribers and pharmacists How advocacy organizations and pharmacists can pursue policy reform together Why patients deserve clear explanations, respectful treatment and continuity-of-care planning How pharmacists can remain vigilant without losing empathy for people living with pain About the Doctor Patient Forum The Doctor Patient Forum, founded by Claudia A. Risk is unavoidable in investing—and in life. But not all risks deserve equal attention. It is easy to focus primarily on the probability that something will happen. If an investment, career move, or financial strategy has a high likelihood of succeeding, we may assume it is a good decision. But Mark Biller, Executive Editor at Sound Mind Investing, suggests another question may be even more important: If things go wrong, how wrong could they go? That shift—from focusing on probabilities to considering consequences—can help us make wiser financial decisions and protect ourselves from risks that could permanently derail our plans. A Small Probability Can Carry a Huge Consequence Suppose someone told you there was a 99% chance an opportunity would succeed. Those odds sound compelling. But what if the remaining 1% chance of failure meant complete financial ruin? Suddenly, the decision looks very different. A simple illustration is crossing a busy street. The probability of being hit by a vehicle may be relatively small, but we still look both ways because the potential consequence is catastrophic. A low probability does not make a severe consequence irrelevant. The same principle applies to investing. An outcome may be statistically unlikely, but if it could wipe out your savings, destroy your retirement plan, or leave you unable to meet your obligations, it deserves serious consideration. Financial thinker Peter Bernstein summarized the principle well: the consequences of being wrong can matter more than the probabilities of being right. That leads to two important questions: If this goes wrong, how wrong could it go?  And how much would it matter? Why Humility Matters in Investing Financial history offers plenty of reminders that even highly intelligent investors cannot anticipate every outcome. One famous example is the collapse of Long-Term Capital Management in 1998. The hedge fund was run by some of the brightest minds in finance and relied on sophisticated mathematical models. Those models worked under most circumstances—but a combination of leverage and extraordinary market conditions caused enormous losses. The lesson is not that investors should avoid risk altogether. Risk is part of investing. Rather, wise investors recognize the limits of their knowledge. We cannot predict every market decline, economic shock, or unexpected life event. That reality should lead us toward humility and encourage us to build financial plans with room for error. Build a Margin of Safety One practical way to prepare for uncertainty is to maintain a margin of safety. That begins before investing. A strong financial foundation includes reducing burdensome debt and establishing adequate emergency savings. Then, as you invest, diversification can help reduce the danger of concentrated bets, while avoiding excessive leverage can protect against losses that permanently impair your financial position. The goal is not to eliminate every possible risk. That would be impossible. Instead, margin allows your plan to survive when circumstances do not unfold as expected. Biblical wisdom encourages this kind of prudence. Proverbs 22:3 says: “The prudent sees danger and hides himself, but the simple go on and suffer for it.” Wise stewardship does not require us to live fearfully. But it does call us to recognize potential danger and prepare appropriately. Your Emergency Fund Protects More Than Emergencies An emergency fund may seem separate from an investment portfolio, but the two are closely connected. Think of investing like climbing a ladder. Before climbing higher, you want to make sure the ladder is resting on firm ground. Emergency savings provide that foundation. Unexpected expenses are inevitable. A furnace fails. A vehicle needs replacing sooner than expected. A major repair suddenly becomes necessary. Without adequate savings, those expenses may force you to sell investments at exactly the wrong time—perhaps when the market is down significantly. What began as an ordinary household expense can then cause lasting damage to a long-term investment plan. An emergency fund creates financial breathing room so temporary problems do not become permanent setbacks. Protecting Retirees From Sequence-of-Returns Risk Consequences become especially important as retirement approaches. One risk retirees face is known as sequence-of-returns risk. This occurs when significant investment losses happen early in retirement while a retiree is simultaneously withdrawing money from the portfolio. Two retirees could experience similar average investment returns over several decades but have very different outcomes depending on when the losses occur. A steep market decline early in retirement can be particularly damaging because withdrawals compound those losses. Even strong returns later may not fully repair the damage. Diversification can help manage this risk. Some retirees also choose to keep several years of anticipated spending in cash or relatively low-risk investments so they are less likely to sell stocks during a severe market downturn. The appropriate strategy will vary by household, but the principle remains the same: consider not only what is likely to happen, but what would happen to your plan if difficult circumstances arrived at an inconvenient time. How Much Risk Can You Afford? Risk tolerance is often discussed in terms of emotion: How comfortable are you when markets fall? That matters, but consequence-based thinking adds another dimension. Ask what would happen if an investment or strategy failed. Would the loss merely be disappointing? Or would it prevent you from retiring, eliminate your emergency reserves, jeopardize your home, or keep you awake at night? If a negative outcome would derail your financial goals, you may be taking more risk than you can afford—even if the probability of success appears high. On the other hand, if you can absorb the downside without seriously damaging your financial plan, then probability can play a larger role in the decision. This framework also guards against becoming too conservative. Avoiding stocks entirely in retirement may reduce short-term market volatility, for example, but it introduces another potential consequence: a portfolio may fail to keep pace with inflation over a retirement that lasts several decades. Wise risk management considers both sides. Stewardship Leaves Room for the Unexpected We cannot know exactly what markets, inflation, interest rates, or the economy will do next. And Scripture never promises that careful planning will remove uncertainty from our lives. Our confidence ultimately rests somewhere deeper. As Christians, we believe God is sovereign and that our ultimate security is found in Christ—not in the performance of our portfolios. That frees us to approach financial decisions with both wisdom and humility. We can plan carefully without pretending we know the future. We can prepare for risk without being ruled by fear. And we can leave margin in our finances because we recognize our own limitations. The goal is not to predict every possible outcome. It is to build a financial life capable of enduring when some of our predictions inevitably prove wrong. Before taking a significant financial risk, don't simply ask, “What are the odds that this will work?” Ask one more question: “If it doesn't, can my financial plan withstand the consequences?” That question may be one of the most valuable safeguards a wise steward can use. TONGAN CRIP THREAT REMAINS SIGNIFICANT IN SALT LAKE CITY.// -----BEGIN TEARLINE------International Events-Middle East: Iran struck two more ships in the Strait of Hormuz last night, continuing the trend of the past couple of weeks. This morning, the UKMTO retroactively admitted to a merchant vessel being struck on August 15th, with the ship sustaining damage and one crew casualty. This afternoon, Houthi forces conducted a ballistic missile strike on the UAE, with missile warnings being issued in Dubai, for the first time since May. No locals reported witnessing any interception, and government officials stated that one missile splashed down in the Gulf, and another landed outside their territory.Analyst Comment: On the rhetoric front, this morning President Trump continued claiming control of the Strait of Hormuz, posting a graphic which claims the Strait is now U.S. territory. Even though waterways traditionally can't be claimed as land territory, and while this may just be a more lighthearted meme, at the other end of the spectrum this could be more serious. If the White House is indeed serious about making a territorial declaration of some kind, this could be an effort to move the goalposts to allow for the use of more substantial weapons. If Hormuz is America, then the US can claim that Iran is attacking the homeland. Of course, this logic is a stretch at best, but it's exactly the kind of reasoning that would make sense for the White House to rely on at this stage in the war.-HomeFront-Utah: One suspect has been arrested in conjunction with yesterday's events in Salt Lake City. Afuhia Masiu Manatau was arrested for his role in yesterday's shooting at the Tongan funeral in Kearns. During the funeral, a fight broke out among the congregation, between his 17-year-old son, and several others. During the fight, Manatau gave a firearm to his son, which resulted in the son attempting to murder the others. Manatau's son (the shooter) was shot in the chest during the engagement, and Manatau himself has been arrested.Analyst Comment: Concerning casualties, so far one fatality was reported as a result of the funeral shooting. The victim has been identified as Vilisoni (Soni) Angilau, a confirmed Tongan Crip who was recently released from prison after murdering a member of a rival gang in 2013. Soni Angilau's immediate relative, Naisa Angilau, was also recently released from prison for a completely different shooting as well, after he attempted to kill a gas station clerk in 2009. Of note, Soni's brother Siale also met a violent end as he was also shot and killed during a high-profile shooting that took place inside the courtroom during his trial. Siale Angilau was shot and killed by federal Marshals after attempting to stab a witness to death during his trial for racketeering back in 2014. All of these family members have an extensive presence on social media, and all self-identify as Tongan Crips.-----END TEARLINE-----Analyst Comments: In Utah, additional details are helpful for understanding how serious yesterday's shooting actually was, especially considering the long history of similar murders. Despite assurances to the contrary, Intermountain Medical Center remained locked down throughout the evening last night, as the threats to the hospital remained constant. Many non-essential employees were escorted out of the hospital by SWAT, and patients' medical procedures were canceled due to the security threat. Intermountain Public Relations officials issued statements claiming that everything was fine and that there was no threat to the hospital, however employees within the hospital confirmed that these statements were not true.Intercepted radio traffic from 911 dispatch also confirms that the reason for the overwhelming police response at the hospital, was due to the Tongan Crip Gang attempting to breach the entrance to the Emergency department. At least one employee was assaulted and wounded in the ER, and a large crowd of Tongans also attempted to break through the Emergency Room entrance on the south side of Unit 4. After this breach attempt, local authorities responded in force, with SWAT officers responding immediately to reinforce the hospital and prevent the facility from being overrun. Snipers were deployed to the rooftops, and at least two Bearcat armored vehicles were posted at the Emergency Department to secure a perimeter around the facility.Concerning future threats, the circumstances of yesterday's funeral shooting are still unclear, with much of the eyewitness testimony being verifiably false as gang members seek to cover up the crime. The main question is whether or not this will result in a wider gang war, and right now the answer to that question is not so clear. The Tongan Crip Gang is known to engage in mutual combat within the gang itself, as rival factions settle internal disputes with violence and murder. Funerals are a common way for hostile TCG factions to meet up, and tensions flare. However, the Tongans also have many rivals locally which might also be inclined to stoke up a gang war. The TCG originally had a foothold in Inglewood, California, where the gang achieved it's extremely brutal reputation. After expanding this foothold to Salt Lake City, the TCG became one of the most legendarily violent gangs in the entire nation. Though small in numbers, they make up for their lack of footsoldiers with sheer violence. Even so, many locals prefer to call them "nice" and ignore this brutality, due to the affiliation this gang has within facilities owned by the LDS Church. This gang is known to murder people randomly without a second thought, and despite Tongans being less than one percent of the population of Utah, they make up over 13% of all gang member affiliation in the region. For those who want to make the argument that "not all Tongans" are causing problems, unfortunately in Salt Lake City specifically, this does not appear to be the case based on the case history and preponderance of evidence. During the Tongan Crip funeral murder at an LDS Ward back in January...not a single churchgoer came forward to name the killer. 100% of the congregants at the Ward where the murder took place, refused to speak to the police or provide a statement regarding what they saw. Police set up an anonymous hotline to protect witnesses, but among the hundreds of Tongans that witnessed that shooting, not a single person came forward to testify and the shooters were eventually charged based on evidence of the murder plans being found on their phones.Even the small handful of Tongans who are not documented gang members are extremely loyal to tribal allegiances; quite literally every single Tongan with a social media presence in Salt Lake County has expressed support for their guy who attempted to murder a gas station clerk with a shotgun. No one within this diaspora has condemned the actions of the man who's funeral even resulted in further bloodshed. The Tongan community has not tried to distance themselves from the frequent murders that are conducted within their areas, and most of the Tongan Wards within Salt Lake close ranks around those who commit violent murders.Concerning yesterday's shootings, posts on social media indicate that other family members of yesterday's deceased have been observed flashing Tongan Crip Gang signs and generally confirming their gang affiliation, specifically within the Tongan Wards. On Facebook, most of the Salt Lake Tongans don't wish each other a "Happy B-Day"...they change it to "C-Day" because the letter "B" is too closely affiliated with their arch rivals...the Bloods. This is a callback to their Inglewood origins...many of the TCG's rivals are much the same as in California. The Rose Park Bloods are their main external adversary within Salt Lake, as both gangs have been expanding into each other's turf. In the same area, the Rose Park Taliban (a gang formed from the children of refugees from the Middle East) also maintains some presence, and is allied with the Bloods. Back in March, a Rose Park Taliban member tried to murder a liquor store employee in Taylorsville (near Tongan Crip territory), so gang violence in southwest Salt Lake specifically from these gangs is a constant. As yesterday's "victim" had previously killed a member of the Baby Regulators gang, it's possible that this was a revenge killing that was unrelated to the other Angilau who's funeral was being held at the time.As a result of this complicated web of alliances and relationships between extremely violent gangs, more violence is expected. Despite the grass skirts, barbeques, and disarming smiles, most of these people are killers, and the killing is only likely to escalate. This month a federal watchdog raised the alarm over thousands of staffing cuts at the Federal Emergency Management Agency.
