In this episode we delve into statements by past and current prophets that help us understand the motivations behind removing this pivitoal concept from the gospel. Mitte der 1990er-Jahre ging de Angelo mit Peter Maffay und dem Musical Tabaluga und Lilli auf Tournee, doch dann musste er mehrere Monate Pause einlegen wegen Lymphknotenkrebs. 1997 meldete er sich gesund wieder zurück mit der englischsprachigen Single I Can See the Light zurück, die er in Los Angeles mit dem Team und den Musikern von Michael Jackson aufgenommen hatte. 2005 zog er sich mehr oder weniger aus dem Musikgeschäft zurück, weil er infolge hoher Schulden (1,5 Millionen Euro) Privatinsolvenz anmelden musste. Danach war er in 25 Folgen der Fernsehserie „Unter uns“ als Michael Winter zu sehen. 2008 sowie 2009 stand Nino de Angelo wieder für Unter uns vor der Kamera, diesmal gemeinsam mit Tochter Louisa Gorgoglione. 2009 erkrankte er an der Immunschwächekrankheit ITP, bei der ihm die Milz entfernt wurde, und er sich einer Chemotherapie unterziehen musste. Im März 2016 musste er sich einer Notoperation am Herzen unterziehen und bekam zwei Bypäse gelegt.

House Warming Podcast, Episode 012: Building the Foundation for the Future City that We Need to Be with Maria Hadden, Alderwoman of the 49th Ward    In this episode, Sarah Bury talks with Alderwoman Maria Hadden about the environmental legislation she has supported, including an order to assess the cost of bringing back the Chicago Department of Environment.  Alderwoman Maria Hadden was born in Columbus, Ohio, and her parents Skip and Toni taught Maria  two foundational lessons. 1) Be curious and ask questions. It's important to understand how things work and why. 2) Problems that affect any of us, affect all of us. And that if you see something wrong and you're able to help, you should do what you can to make it right.  After graduating from The Ohio State University with a BA in International Peace and Conflict Studies, she moved to Chicago. She chose Rogers Park because of its affordability, welcoming community culture and proximity to the lake. A weekly regular at The Heartland Cafe, in Rogers Park, Maria found community.Rogers Park was the first place Maria lived in Chicago, where she purchased her first home and where her activism blossomed. When the housing bubble burst in 2007, and the developer of her building fled the country, Maria helped organize her neighbors to save their homes. Maria has been organizing with the community ever since.Maria has been a leader in the 49th Ward through her advocacy, her art, and her work with Participatory Budgeting in Chicago. She lives with her partner, Natalia, and their rescue dog. Maria is a photographer and artist who leads screenprinting workshops for young people. She took over The Art Patch Project from longtime Rogers Park activist Chris Drew and believes art is an important medium for activism and personal expression.Before becoming Alderwoman, Maria Hadden was the Executive Director of Our City Our Voice, a national nonprofit organization she founded to enable communities and government across the country to redesign democracy for more empowered and equitable participation. Her expertise in public participation is grounded in grassroots organizing efforts for social change.Before launching her own organization, Maria was a founding board member of The Participatory Budgeting Project, where she worked for 8 years with communities and government officials in the Midwest and South to create and facilitate democratic processes around public budgets. In this role, Maria has also worked with Aldermen throughout Chicago to design participatory budgeting processes around Aldermanic menu money, school budgets and TIF funds.​Maria currently serves on the Board of Directors of the Black Youth Project (BYP) 100, and Voqal. BYP is an organization of activists aiming to create justice and freedom for all Black people through the principles of leadership development and inclusivity using  Black, queer, feminist lens. Voqal is a nonprofit that uses technology and media to build an educated, empowered and engaged public. The Backyard Players & Friends are living proof that the kindness, generosity and support of a community can change and challenge the trajectory of a local non-profit organization.Ellen White is co-founder and Executive Director of Backyard Players and Friends. Ellen serves as an integral resource for children with developmental disabilities on the South Shore of Long Island.Typically, students with developmental disabilities attend school until 21 and then “age out.” Ellen and her sisters, Cristine Daly and Joan Kelly, along with Ryan Delaney, created BackYard Players and Friends as a resource to address the needs of these individuals.About Backyard Players & Friends:Backyard Players & Friends is a local, inclusive arts-based community program for young adults with intellectual and developmental disabilities. Many of our participants attended High School until the age of 21 when they graduated from the traditional education system. Most families have the opportunity to send their children off to college or the work force after graduation.Parents of students with developmental disabilities don't usually have those options and they are left to find classes & programs for their children to attend...These students are then at risk to lose their network of friends, their social interactions and their community involvement.Young adults with developmental disabilities spend the first two decades of their lives preparing to be full members of our communities. Der preisgekrönte ehemalige Gastwirt Res Hubler (17 Gault Millau-Punkte, 1 Michelin-Stern) ist auch mit über 70 Jahren noch bei bester Gesundheit – bis im Oktober 2021 völlig unerwartet lebensbedrohliche Herzprobleme auftauchen. Eine Untersuchung ergibt eine Herzkranzgefäss-Verkalkung. Die linke Seite des Herzens wird nicht mehr mit Sauerstoff versorgt. Sein Leben hängt an einem seidenen Faden. Und in den Händen der Ärzte, die eine Operation starten und vier Bypässe legen. Der Spitzengastronom überlebt. Doch als er auf der Intensivstation aufwacht, spürt er sich kaum mehr. «Ich war wie losgelöst von meinem Körper und fühlte mich schwach. Dafür spürte ich auf einmal der Friede und die Wärme Gottes umso intensiver!» Seine Beziehung zu Gott sei durch diese Extremerfahrung im Spital nochmals tiefer geworden. Welche Prioritäten Res Hubler im neu geschenkten Leben setzt, erzählt er im Talk. Our minds are so trained to think and talk about the problems of the world that breaking that habit becomes tough. When telling your story always talk and focus on the stuff you want to show up, not the stuff you don't want to show up. There are so many good things in our lives -- way more good things than bad -- but all we talk about and worry about are all the problems. Hemos pasado todo este lunes hablando del tema, elucubrando y sacando conclusiones super absurdas sobre cómo va a terminar esta mierda. Nadie lo sabe y, la verdad, a nadie le importa. Teniendo en cuenta que la mayoría de episodios de Barrabás y Pardillo se escuchaban tan mal que ni se entendía de qué coño iba el capítulo, es muy difícil que nadie sienta el más mínimo interés por saber cómo concluye todo. This episode's guest Charlene A. Carruthers is a Black, queer feminist community organizer and writer with over 10 years of experience in racial justice, feminist and youth leadership development movement work.  As the founding national director of the Black Youth Project 100 (BYP100), she has worked alongside hundreds of young Black activists to build a national base of activist member-led organization of Black 18-35 year olds dedicated to creating justice and freedom for all Black people. Her passion for developing young leaders to build capacity within marginalized communities has led her to work on immigrant rights, economic justice and civil rights campaigns nationwide. Charlene’s forthcoming book Unapologetic.   We talked about the Black Queer Feminist Lens which is an organizing praxis used by the BYP100. It has become the norm that when you turn on the news before the month is over another young black person has been killed at the hands of a police officer somewhere in the country. Recently Chicago has been in the news and something different happened. Out of this a new civil rights movement has emerged and it is organized and its membership is all young people between the ages of 18-35. They have strategically gathered together and collaborated across the country showing support in states where violence has emerged against a community for years. They have been there right under our very noses. The news coming out of major cities have awaken Black Youth Project 100 (BYP 100)  an activist member-based organization of Black 18-35 year olds, dedicated to creating justice and freedom for all Black people. We do this through building a collective focused on transformative leadership development, non-violent direct action organizing, advocacy and education. BYP per their has trained young black activists in direct action grassroots organizing skills, so they can build the power we need to transform our communities. They have mobilized young Black leaders on issues including ending criminalization and dismantling the prison industrial complex, expanding and securing LGBT and women’s rights. They have and are running campaigns using on the ground and digital tactics towards the goal of ending the criminalization of Black youth, racial profiling and police brutality. Joining us will again be Matt Boykin of the Chicago Branch of BYO100. He is  a member of Black Youth Project 100 here in Chicago. Originally from the Suburbs of Atlanta, Maxx has organized for a wide arrange of issue based, political and union organizing over the last few years in Georgia, Virginia and in the last two years in Illinois around the Affordable Care Act, Fight for 15, Movement for Black Lives and fighting for those living with and communities most effected by HIV.

Die konventionelle Herzbypassoperation mit Herz-Lungen-Maschine (HLM) ist immer noch der am häufigsten durchgeführte kardiochirurgische Eingriff. Dieser gilt als sicher und effektiv. Die Verwendung der HLM, der kardiopulmonale Bypass, kann jedoch beim Patienten eine komplexe pathophysiologische Reaktion auslösen. Hierbei kann es zu einer generalisierten Entzündungsreaktion mit der Freisetzung proinflammatorischer Mediatoren kommen, die eine massive Verschlechterung der mikrovaskulären Perfusion und Erhöhung der Permeabilität hervorrufen können. Bei einigen Patienten kann der verstärkte Flüssigkeitsaustritt („capillary leakage syndrome”) aus dem intravasalen Raum in das Intersitium zu einer ausgeprägten Ödembildung im Gewebe und in Organen führen. Mögliche Folgen sind kardiozirkulatorische und pulmonale Dysfunktionen, einschließlich einer erhöhten Morbiditäts- und Mortalitätsrate der Patienten. Bei Herzbypassoperationen am schlagenden Herzen, ohne Verwendung der HLM, scheint es zu einer weniger stark ausgeprägten Entzündungsreaktion, einer verminderten Freisetzung von Zytokinen und einer geringeren Expression von Adhäsionsmolekülen zu kommen. Die Folge ist ein besserer klinischer Verlauf und eine niedrigere perioperative Komplikationsrate im Vergleich zu Patienten, bei denen ein Eingriff mit HLM durchgeführt wird. Obwohl bereits bekannt ist, dass Patienten, die mit HLM operiert werden, eine signifikant höhere Flüssigkeitsbilanz aufweisen, konnte bisher noch nicht nachgewiesen werden, dass es bei Herzbypassoperationen mit HLM zu einer stärkeren Flüssigkeitsfiltration kommt als bei Patienten, bei denen der gleiche Eingriff ohne HLM durchgeführt wird. Ziel der Studie war es daher, die Auswirkungen der Herz-Lungen-Maschine auf die mikrovaskuläre Perfusion und Permeabilität zu erfassen, den möglichen mikrovaskulären Schaden mit konsekutivem Flüssigkeitsaustritt ins Interstitium zu quantifizieren und hinsichtlich dessen Korrelation mit Entzündungsparametern zu überprüfen. Zusätzlich sollte in Erfahrung gebracht werden, ob die Entzündungsreaktion, der mikrovaskuläre Schaden und die konsekutive Ödembildung nach koronarer Bypassoperation mit Herz-Lungen-Maschine stärker ausgeprägt ist als bei Patienten, die ohne HLM operiert werden. Die venöse Kompressionsplethysmographie (VKP) ist eine nicht-invasive Untersuchungsmethode, mit der es möglich ist, mikrovaskuläre Parameter zu bestimmten. Mit Hilfe von speziellen Blutdruckmanschetten wird am Oberschenkel der Patienten eine venöse Abflußstauung erzeugt, die zu einer Volumenzunahme der distalen Extremität führt. Diese Volumenänderung wird mit hochsensitiven Sensoren kontinuierlich erfasst und mittels Computer aufgezeichnet. Eine computergestützte „Off-line“-Analyse erlaubt die Berechnung der mikrovaskulären Flüssigkeitsfiltrationskapazität (FFK), die die Permeabilität der Kapillaren im Untersuchungsgebiet wiedergibt. Außerdem kann der isovolumetrische venöse Druck (Pvi), der das Gleichgewicht zwischen den hydrostatischen und den kolloidosmotischen Kräften widerspiegelt, und der arterielle Blutfluss (Qa) in den Extremitäten berechnet werden. In die hier vorliegende prospektive, klinische Studie wurden insgesamt 38 Patienten, die sich einer elektiven Herzbypassoperation unterziehen mussten, eingeschlossen. Von diesen wurden 25 Patienten mit (on-pump) und 13 Patienten ohne Herz-Lungen-Maschine (off-pump), am schlagenden Herzen operiert. Beide Gruppen waren hinsichtlich des Alters und Geschlechts der Patienten, des Ausmaßes der koronaren Herzkrankheit, der linksventrikulären Ejektionsfraktion und der präoperativen Klinik vergleichbar. Die Patienten der On-pump-Gruppe erhielten signifikant mehr koronare Bypässe, das chirurgische Trauma, der operative Zugangsweg zum Herzen, erfolgte aber bei allen Patienten mittels medianer Sternotomie und auch die durchschnittliche Operationsdauer und Anästhesiezeit unterschied sich in beiden Gruppen nicht. Die Messungen der venösen Kompressionsplethysmographie erfolgten am Abend vor der Operation auf der herzchirurgischen Normalstation (T1), eine Stunde nach der Operation auf der herzchirurgischen Intensivstation (T2) und am 5. postoperativen Tag (T3). Gleichzeitig wurde den Patienten Blut entnommen, um die Konzentrationen der Zytokine Interleukin-6 (IL-6) und Tumornekrosefaktor-α (TNF-α) im Serum und die Expression der Adhäsionsmoleküle (Beta-2-Integrine (CD18/CD11b) und L-Selektine (CD62L)) auf der Oberfläche polymorphkerniger neutrophiler Leukozyten (PMNL) zu bestimmen, aus denen sich das Ausmaß der Entzündungsreaktion abschätzen lässt. Es konnte gezeigt werden, dass in der On-pump-Gruppe die Flüssigkeitsfiltrationskapazität (FFK) postoperativ signifikant zunimmt und am 5. postoperativen Tag immer noch signifikant erhöht ist. Die postoperative Zunahme der FFK korrelierte dabei signifikant positiv mit der Aortenklemmzeit und der Dauer des kardiopulmonalen Bypasses. Der isovolumetrische venöse Druck (Pvi) zeigte eine Tendenz zum postoperativen Anstieg. Dieser Anstieg verfehlte zwar knapp das Signifikanzniveau, korrelierte jedoch ebenfalls signifikant positiv mit der Aortenklemmzeit. In der On-pump-Gruppe wurde zusätzlich eine signifikante Zunahme der Konzentrationen von IL-6 und TNF-α im Serum postoperativ beobachtet. Auch die Expression der Adhäsionsmoleküle CD18/CD11b in der On-pump-Gruppe zeigte eine Tendenz zur postoperativen Zunahme. Diese Zunahme verfehlte zwar knapp das Signifikanzniveau, war jedoch verglichen mit der Off-pump-Gruppe signifikant stärker ausgeprägt und korrelierte signifikant positiv mit der Dauer des kardiopulmonalen Bypasses und mit der Erhöhung der FFK. In der Off-pump-Gruppe konnten dagegen keine Veränderungen hinsichtlich der Flüssigkeitsfiltrationskapazität, des isovolumetrischen venösen Druckes, der TNF-a-Serumkonzentration und der Adhäsionsmolekülexpression postoperativ gefunden werden. Nur die IL-6-Serumkonzentration nahm ähnlich wie in der On-pump-Gruppe postoperativ signifikant zu. Zwischen den beiden Gruppen konnten hinsichtlich des arteriellen Blutflusses (Qa), des kolloidosmotischen Druckes (KOD), der IL-6-Serumkonzentration, der Expression des L-Selektins CD62L, der Flüssigkeitsbilanz, der Herzfrequenz und des mittleren arteriellen Blutdruckes keine signifikanten Unterschiede beobachtet werden. Es bestanden ebenfalls keine signifikanten Unterschiede hinsichtlich der Dauer der postoperativen maschinellen Beatmung, des postoperativen Aufenthalts auf der Intensivstation und im Krankenhaus und der perioperativen Komplikationsrate. Die in der vorliegenden Studie beobachtete signifikante Zunahme der Flüssigkeitsfiltrationskapazität (FFK) bei Patienten der On-pump-Gruppe könnte durch verschiedene Ursachen ausgelöst worden sein. Am wahrscheinlichsten scheint hierbei eine generalisierte inflammatorische Reaktion, bedingt durch den Einsatz der Herz-Lungen-Maschine zu sein, da die Erhöhung der FFK signifikant positiv mit der tendenziell erhöhten Expression der Adhäsionsmoleküle CD18/CD11b korreliert. Sowohl TNF-α als auch CD18/CD11b, die beide während des kardiopulmonalen Bypasses verstärkt freigesetzt werden, können direkt bzw. indirekt, über Granulozyten vermittelt, die Permeabilität erhöhen. Für diese Hypothese spricht außerdem, dass die postoperative Erhöhung der FFK signifikant positiv mit der Aortenklemmzeit und der Dauer des kardiopulmonalen Bypasses korreliert. Eine andere mögliche Ursache für die Erhöhung der FFK könnte aber auch eine allgemeine Entzündungsreaktion auf ein chirurgisches Trauma, eine so genannte „postoperative stress response“ sein. Gegen diese Vermutung spricht aber, dass in der Off-pump-Gruppe kein FFK-Anstieg, keine vermehrte Freisetzung von TNF-α und keine gesteigerte Expression von CD18/CD11b beobachtet wurde, obwohl auch bei dieser Patientengruppe der operative Zugangsweg mittels medianer Sternotomie erfolgte und das Ausmaß des chirurgischen Traumas vergleichbar erscheint. Außerdem könnten Veränderungen der einzelnen Starlingkräfte, wie beispielsweise ein Abfall des kolloidosmotischen Druckes (KOD), für eine Erhöhung der FFK verantwortlich sein. Da der KOD aber in beiden Gruppen postoperativ signifikant vergleichbar abfällt und sich keine Korrelation zwischen den Veränderungen des KOD und der FFK findet, ist zu schließen, dass der Abfall des KOD nicht für den beobachteten Anstieg der FFK in der On-pump-Gruppe verantwortlich ist. Zusammenfassend kann gesagt werden, dass die Herz-Lungen-Maschine wohl im wesentlichen die Erhöhung der mikrovaskulären Permeabilität bedingt, indem sie eine Aktivierung des Immunsystems auslöst, in deren Verlauf Mediatoren freigesetzt werden, die einen mikrovaskulären Schaden verursachen. Die venöse Kompressionsplethysmographie (VKP) ist eine nicht-invasive Untersuchungsmethode zur Beurteilung der Mikrozirkulation, die eine frühzeitigere Diagnose einer gesteigerten mikrovaskulären Permeabilität ermöglicht, was besonders bei kritisch kranken Patienten hilfreich ist. Außerdem könnten Patienten, die ein erhöhtes Risiko haben, nach kardiopulmonalem Bypass ein „capillary leakage syndrome“ zu entwickeln, mit der VKP identifiziert und überwacht werden.

In dieser Fall-Kontroll-Studie wurden präoperative, intra- und postoperative Risikofaktoren sowie Langzeitergebnisse bei Patienten, die sich nach fehlgeschlagener PTCA einer notfallmäßigen aorto-koronaren Bypassoperation (ACB) unterziehen mussten, untersucht. Zur Beurteilung des Mortalitätsrisikos und des Langzeitverlaufs nach notfallmäßiger Bypassoperation diente ein im selben Zeitraum operiertes elektives Vergleichskollektiv mit einem vergleichbaren Risikoprofil. Ein weiterer Aspekt dieser Untersuchung galt der Einschätzung einer erhöhten perioperativen Myokardinfarktinzidenz in der Notfallgruppe. Darüber hinaus wurde untersucht, ob die Verwendung der A. mammaria interna (IMA) die operativen und mittelfristigen Ergebnisse bei den notfallmäßigen ACB-Operationen nach fehlgeschlagener PTCA beeinflusst. Der Gebrauch der IMA wird in der elektiven aorto-koronaren Bypassoperation wegen der vorteilhaften Langzeitergebnisse empfohlen. Bei notfallmäßiger ACB bevorzugen jedoch viele Operateure die Revaskularisation vor allem mit venösen Grafts, da die Operationszeiten kürzer und die initialen Flussraten höher sind. Von Januar 1990 bis März 1993 wurde bei 56 Patienten aus 7 kardiologischen Zentren 1h bis 114 h (durchschnittlich 7,5h) nach fehlgeschlagener PTCA eine notfallmäßige ACB durchgeführt. Diese Patienten wurden auf bestimmte Risikofaktoren und auf Kriterien einer anatomisch ungünstigen Stenose für eine PTCA hin untersucht. Als Kriterien für die Beurteilung einer ungünstigen Stenose galten: langstreckige Stenosen, Verschluss eines anderen Hauptgefäßes, Tandemstenosen, exzentrische Stenosen, Stenosen in oder an einer Gefäßkrümmung/Gefäßaufzweigung und distal gelegene Stenosen. 23,2% der Patienten wiesen ein Kriterium, 41,1% zwei, 8,9% drei und 3,6% vier Kriterien auf. Bei nur 13 Patienten (23,2%) wurden die Stenosen als ideal für eine PTCA eingestuft. Alle Patienten unterzogen sich 14,6 +/- 8,2 Monate post operationem einer echokardiographischen und anamnestischen Nachuntersuchung.
