Should I Give Up Keto if it Skyrockets my LDL? [18:11] Charlie says: Hi Rob and Nicki, Big fan of the show. After learning about the many health and longevity benefits of keto from folks like yourself, Dr. Peter Attia, and Dom D’Agostino, I gave it a go for 4 months. I followed a clean, dairy-free version with loads of greens and cruciferous veggies, a variety of meat and fish, avocados, olive oil, nuts and coconut, you get the idea. I confirmed ketosis regularly with a blood meter. I felt great during the four months, and by the end even started making gains again with my kettlebells and barbell. The diet wasn’t a huge adjustment for me since I was already essentially paleo. After the four months, I got my bloodwork to compare to my baseline. My triglycerides and HDL remained excellent, but my LDL-C skyrocketed, and my LDL-P is fairly elevated. I am so bummed out and discouraged because I was so excited about all the longevity benefits of ketosis, but my PCP told me to go back to my previous eating habits. Here’s my question: do you have any ideas for ways I can achieve some of the benefits from ketones (BHB) while mitigating my LDL going haywire? Maybe intermittent fasting? Cyclic keto? Exogenous ketones? While the high LDL numbers scare me, I know from listening to your podcast as well as Dave Feldman that LDL isn’t the end-all be-all for cardiovascular risk. I find solace with my HDL and triglyceride levels, also my fasting glucose is very low, and my LP-IR score is below the low range, indicating I'm highly insulin sensitive. I was wondering if you had any good resources, guidance, recommendations or considerations (basically anything) about 5-2 intermittent fasting. It is difficult to sift through all the crap on the internet 3. Avoiding weight regain after losses [34:29] Sarah says: Hey Robb & Nicki! I was wondering if you two could have a nice discussion about the optimal way to avoid gaining back the weight you've lost while dieting. There's a lot of talk about the best way to lose weight, but not enough on what happens after. I know you'd say that to lose weight, to try a low carb diet with adequate protein and electrolytes and exercise, using fat as a lever, and everything will work itself out. However, during the pandemic, I ended up doing a VLCD, and I lost about 42 lbs from mid March to mid May. You'd likely agree that this rapid weight loss is problematic because of lean tissue loss. I'll tell you right now, I wasn't eating adequate protein. The food I ate was primarily protein, but it wasn't much, some days a few meatballs... Also I chose not to exercise during this time. I'm sure I'm not the only one out there who gets impatient and isn't satisfied with losing .5 lbs per week even though it's the most sustainable way to lose. So anyway, I did some research (lost job because of Covid, no kids, lots of time on my hands), and I wrote it all down in a comprehensive plan to avoid gaining the weight back in the summer and moving forward. I was wondering if you might like to consider taking a look? And perhaps bringing some criticism/comments/tips for the the other five listeners on what we can do to stabilize after weight loss. (I'm a woman, 37, currently 208 lbs, besides obesity, all healthy biomarkers, I normally enjoy crossfit style workouts, once a week or less - not hardcore enough to build lots of muscle, and mini-triathlons in the summer) Thanks!! Notes: Conclusion: A diet consisting of high levels of animal-based protein combined with a lifestyle of consistent strength training should go a long way to help the dieter not regain the fat mass that they have lost. A general recommendation is to eat a diet of 35% protein, 15% carbohydrate and 55% fat. The carbohydrate and fat can be adjusted based on how addicted a person is to certain foods, and how they see their body responding during refeeding. An app like MyFitnessPal can calculate the percentage of macronutrients you are consuming. 4. LMNT/Sodium and Breast Milk Production [44:47] Hey Robb and Nicki! My partner Garrett and I have been huge followers of you since the beginning of our relationship, 11 years ago! We just welcomed our second daughter about two months ago. I'm exclusively breastfeeding and I've noticed an increase in milk supply while drinking LMNT. Is it safe to drink it more than once a day or is that not recommended? Thanks, Margo 5. Low T3 Levels after long term keto adaptation [49:24] Diva says: Hello Rob and Nikki, First of all, thank you so much for the wonderful work that you two do! I would like to know what are your thoughts on low T3 levels after long term Keto adaptations. According to Phinney's work, amongst other evidences, it seems that it is physiological normal for Keto adapted people to no longer require high free T3 levels, according to standard recommendations. I am a 44 year old athlete. I have been Keto adapted for over 12 years. I was diagnosed with hashimoto hypothyroidism 25 years ago. My antibodies have been under control since after starting a Keto Paleo lifestyle and I've able to reduce my T4 dose considerably as well. However, my free T3 levels are very low, below the standard recommendations of 2.0 I am COMPLETELY asymptomatic though. My body composition is great, I'm very muscular and carry 9% body fat effortlessly throughout the years. Despite knowing about the T3 efficiency that Keto adapted people acquire, about two years ago I decided to experiment with adding exogenous T3 to my T4 dose. I noticed no difference on energy levels but I did notice that I started losing muscle mass! Which makes sense because one of the reasons that T3 may be lower on Keto is due to muscle sparring. I stopped taking T3 about one year ago and have gone back to T4 only since then. But one trend that I noticed is that my fasting insulin has increased a little. Nothing much. It used to be around 3 and now its 5.3. This started happening after I added T3 to the mix. I have since stopped taking T3 but my insulin has not gone back to levels that it used to be before taking it. I know that artificially raising T3 levels may raise insulin but shouldn't it have gone back down after stopping the med? I wear a CGM and my blood sugar control is stellar! I can handle carbs very well, I've been testing with that, and even after eating tons of carbs, like 200g at one seating, my blood sugar didn't move past 13 points. The maximum increase was 105. And I'm not kicked out of ketosis. Other than that my average blood glucose is about 85. The only times that it goes up is after high intensity exercise, then it goes above 150, 170 sometimes! But I know that this is physiologically normal so it doesn't concern me. My question is: what are your thoughts on the T3 efficiency after being Keto for so long? Should we try to artificially push up FT3 levels despite having no symptoms? And should I worry about a fasting insulin of 5.3 despite obviously being super insulin sensitive?? Dr. Dom D’Agostino is a research scientist and Associate Professor of Molecular Pharmacology and Physiology at the University of South Florida Morsani College of Medicine. Dominic D'Agostino is an associate professor in the department of molecular pharmacology & physiology at the University of South Florida, Morsani College of Medicine, and a senior research scientist at the institute of human and machine cognition. Dr. Dominic D’Agostino returns to STEM-Talk to give Ken and Dawn an update on his research into ketogenic nutrition. Dom was the guest on episode 14 back in 2016 when ketogenic diets didn’t even show up on a list of the top-10 diets that people Googled. Since then, the search term “ketogenic diet” has risen to the top of the list. In today’s episode, Dom talks about his past 10 years of research into ketogenic diets and what he is learning about the physiological benefits of nutritional ketosis. Dom is tenured associate professor in the Department of Molecular Pharmacology and Physiology at the University of South Florida Morsani College of Medicine.He also is a research scientist here at IHMC.Throughout his career, Dom has been a researcher with a diverse background in neuroscience, molecular pharmacology, nutrition and physiology. Show notes: [00:02:55] Dawn begins the interview mentioning that when she and Ken started hosting STEM-Talk, the ketogenic diet wasn’t on the list of the top-10 most Googled diets of 2015. Today, however, Dawn points that ketogenic diet is number one on the list. She asks Dom if he foresaw sudden mass interest in a ketogenic diet coming. [00:04:12] Ken asks Dom for his thoughts on how the ketogenic diet has went from being very obscure to becoming a household term. [00:06:04] Ken comments on the evolutionary component of the ketogenic diet and how our ancestors must have gone in and out of ketosis based on the availability of food. He also comments on the unique aspect of the ketogenic diet, being that it has an objective measurement, and asks Dom to talk about that. [00:06:59] Dawn comments on the cynicism regarding the ketogenic diet, particularly from nutritionists. She asks Dom to address the criticism and pushback that the ketogenic diet receives from so many nutritionists. [00:10:02] Ken mentions that some fields are resistant to change and new science due to the emotion behind established theories. Dom agrees and then talks about how people, even doctors, are resistant to new data and new science. [00:11:13] Dom talks about the most common misconceptions and overrepresentations of the ketogenic diet. [00:12:54] Ken discusses his dissatisfaction with the term “ketogenic diet” since the word diet implies the mandated consumption of certain food items. He goes on to say that if one is in ketosis, then, by definition, they are doing a ketogenic diet, even though they may be in ketosis because they have been fasting and haven’t eaten anything. Ken and Dom discuss how knowledge about ketogenic nutrition has changed over time and that it is certainly possible to eat an unhealthy ketogenic diet. [00:15:35] Dom and Ken talk about the results of a recent Megan Roberts paper, “A Ketogenic Diet Extends Longevity and Healthspan in Adult Mice,”that showed a 13% increase in the lifespan of the mice along with remarkably improved healthspan. [00:20:26] Dom shares his thoughts on the potential of exogenous ketones in the context of blood glucose regulation. [00:27:07] Ken asks if Dom has been tracking Virta Health, which was founded by Dr. Jeff Volek who was interviewed in STEM-Talk episode 43. Virta Health has been publishing impressive results of its trials that show people reversing type-2 diabetes via a well-formulated ketogenic diet. [00:29:13] Ken adds that the reported numbers from Virta show 60% to 70% of their patients going off their insulin medication or greatly reducing their insulin levels. [00:30:55] Dawn asks about Dom’s experience going underwater for 10 days in participation of a NASA NEEMO mission. She asks him to talk about his personal experience as well as his background in hyperbaric physiology. Welcome back everyone, I’m Brian Sanders and I quit my job and have dedicated my life to the investigation of nutrition and lifelong health. I’m creating the feature length documentary Food Lies, this podcast, and a health technology company here in Los Angeles with a doctor and 2 other partners. Today we have Layne Norton who I had a great time talking with and has a lot of good messages… Nobody should get too caught up in one way of doing something, even if it has tons of apparent benefits. I’ve always tried to keep an open mind and have repeatedly talked about how many different dietary strategies can work, especially on the two opposite ends of the spectrum. Just don’t get caught in the middle like most Americans. This wasn’t supposed to be a debate. Previous guest Dr. Dom D’Agostino recently had a quote debate with Layne on Joe Rogan’s podcast which I recommend that everyone check out. I don’t think Dom challenged him enough so I wanted to have him on and ask a few more questions. I believe people can do fine running on fat or glucose, or both, there's just benefits and downsides depending on which. I see almost no downsides to fat, it’s working for me, I see way more lines of evidence pointing to this being our default state, and so I continue to lean on this side as the preferred metabolic state. We talked about a very interesting study that just came out in November that I feel puts a big hit on his position that if you keep protein and overall calories consistent between to groups of people, the fat and carb content doesn't matter. This study which I linked to in the show notes https://www.bmj.com/content/363/bmj.k4583 by Dr. David Ludwig controls for these, lasts 20 weeks - so long enough to actually get the subjects fat adapted (which I wanted to make a bigger point of that almost all other big studies fail to do) - and clearly shows as you drop your carb intake and replace it with fat, you burn more calories. If you’re burning more energy, you're losing weight. I have so many regrets on things I could have brought up and I’m sure I left things out. Many people listening will probably be yelling at their phones that I could have contradicted the old Kevin Hall studies in this way or brought up some point here or there. I’ll just say it’s hard to get everything out in the moment, live on air. One thing I was was going to say was NuSi funding the study doesn't make it biased. They purposely found researchers that don’t agree with the hypothesis to run the experiments with Dr. Ludwig. The subject changed and I forgot to bring this up. I also wanted to point to the blood markers that improved in the low carb group in this study compared to the higher carb group. This is important no matter what, but even more important to me because they weren't in a calorie deficit - they were just maintaining weight. That’s huge. Layne always says all the benefits of low carb can be attributed to being in a deficit. He said he didn’t get a chance to go over the study yet though. I’ll have to ask him about this another time. I posted the images of the graphs from the study on Instagram so you can see them there if you go back a few posts. Just search for @food.lies. Also, I’m not diminishing all the great aspects of keto. I feel like I’m going to piss off the keto community in this episode because they think I’m dismissing it or not bringing up all the studies showing the unique benefits. That’s for another time - I’m talking to Dr. Ryan Lowery about coming on the podcast soon who is the guy Layne is having a little online war with that comes up in this episode. He’ll talk about all the great studies on the benefits of a ketogenic diet. I wanted to focus on other things with Layne. Today we have the mighty Dom D’Agostino who is at the forefront of the science behind why low carb diets work and specifically the more extreme form of a low carb diet you’ve probably heard a lot about lately - the ketogenic diet. It can be used therapeutically with the help of doctors to treat and reverse diseases like type 2 diabetes and epilepsy. It can also help the everyday person lose a lot of weight if done correctly. It’s not a fad diet - although it currently could be considered a fad just because technically it is very popular right now and might get less popular in the future. Are there any studies showing the performance of the ketogenic diet when t comes to muscle gain versus a higher carb approach? Maybe less muscle gain but fat free? 4. [21:33] Keto and true HIIT Greg says: Hi, I eat a Targeted Ketogenic Diet (TKD) and do Crossfit. I primarily consume more significant amounts of carbohydrates (max 20g for 195lb male) before doing classic CrossFit workouts where HIIT is part of the workout. I do this because: 1. I perform better than without the carbs. 2. I feel and recover better afterwards. 3. I am afraid of my body going catabolic because it can't get energy fast enough during workouts. A lot of what I've read about Keto and exercise deals with endurance workout and weightlifting. What is the best approach specifically for Keto and HIIT for optimizing performance and/or safety (avoid going catabolic)? Thank you! Greg 5. [27:12] A question about having carb withdrawal symptoms despite very high blood ketones. Rob says: Hi there, I was wondering if you could shed some light on my usual response to being in ketosis. I've been on a very strict ketogenic diet for about 6 months now. I've tested my blood ketones hundreds of times using an Abbot Freestyle meter and I'm almost always in the 1mmol to 3mmol range. I have experienced some of the commonly talked about keto symptoms such as occasional feelings of euphoria, appetite suppression (sometimes to the point of nausea if I've consumed a lot of fat), trouble sleeping due to that buzzing ketosis feeling in the body some speak of. The trouble is I don't seem to do well when my ketones are derived from my own body fat, or at least it appears that way. I say this because I still often feel classic "carb-starved" feelings if I haven't eaten (irritable, trouble concentrating, etc). Eating lunch solves that, but this seems strange given that my ketones could still be at 3 yet I experience those symptoms. Perhaps those very few carbs in the lunch are what my body wants or perhaps my body prefers ketones from other sources than my own body fat. What has pushed me to a breaking point is a water fast I attempted this weekend. I began my fast after lunch on Friday (extremely low carb high fat) and during the fast felt exactly the same as I have on any fast I've ever done pre-keto. It was really difficult. I was irritable, had headaches, trouble concentrating the exact same feeling as a carb withdrawal. I was testing my ketones frequently in the days leading up to the fast so it should have been a breeze, in theory. And yes, I took plenty of sodium, potassium and magnesium - it doesn't change it at all. To thicken the plot further, I tested my ketones regularly throughout the whole fast. On Friday night they were about 3mmol and by Saturday lunchtime they were up around 4. By Sunday lunchtime they were up at 5-6 mmol yet I still felt like I was in serious carb withdrawal! Is it possible that ketosis is just not for me? I've heard Dom D'agostino mention that a minority of people don't thrive in ketosis. Any help would be appreciated! Rob 6. [35:30] Keto and Picking a Primary Care Doctor Kilson says: Hey guys! Huge fan of the show! I just read through "Wired to Eat" and really enjoyed it. I think it is a really good "catalyst" for getting started in the right foot. I've been messing around with Keto for a while now. Almost 3 years ago I was walking around at 357lbs and found out about keto, started boxing and lifting and made it down to as low as 250lbs! After some injuries and some life struggles I flew back up too 300lbs. For the last few months I'm back on the grind and trying to make it back down to a weight I feel good at. Big part is thanks to podcast like yours! So thank you very much! Oh right!... The question... I haven't gotten a physical or seen a doctor in a while now. Maybe close to two years. I'm having some trouble picking a Primary Care Provider. What are some things you look for in a Doc? If you have questions about the Keto diet...go no further. Our guest today is pretty much the Keto science guy. Dr Dominic D'Agostino is a professor in molecular pharmacology and physiology at the University of South Florida Morsani College of Medicine as well as a Visiting Research Scientist at the Florida Institute for Human and Machine Cognition. Dr D'Agostino tells us how he found his way into the filed of ketosis. He combined interests in nutritional and neuro sciences. From there, Dom developed a deep expertise in underwater medicine and oxygen toxicity seizures where he made the connection with ketones and the brain. What is Nutrtional Ketosis Ketones - more specifically ketone bodies- are chemical produced by our livers. Ketone bodies are produced by avoiding most carbohydrates in the diet (eg sugars, breads, fruits, and certain vegetables). A much higher percentage of healthy fats are eaten during a ketogenic diet. When someone follows this diet the body begins to make and utilize ketones more so than on it's typical energy source - glucose. Nutritional ketosis is the sustained elevation of ketones in the blood. To be clear, as long as someone isn't completely lacking insulin (eg Type 1 diabetic), this low-level of ketosis is NOT the dangerous state of ketoacidosis which develops from having a lack of insulin. What are the benefits of Nutritional Ketosis Lower states of brain inflammation. Better insulin sensitization and use of ketones for energy in the brain. Dr. Dominic "Dom" D'Agostino (@DominicDAgosti2) is an assistant professor in the Department of Molecular Pharmacology and Physiology at the University of South Florida Morsani College of Medicine, and a senior research scientist at the Institute for Human and Machine Cognition (IHMC).
